Laboratory informs Medical Officer of Health of positive COVID result

Once GP / Provider confirms referral accepted: Toi Te Ora updates notes and suspends follow up in NCTS GP / Provider manages case and their household contacts

Toi Te Ora Informs Investigates Isolates Referral form is completed for cases and household contacts and emailed to: Primary Care Clinical SPOC CommunitySIQ

Once release email is received: Toi Te Ora completes and closes NCTS and Episurv records and notifies Community SIQ to ensure cleaning of accommodation services used.
