Bay of Plenty Health Care Home Model of Care Outline The Bay of Plenty Health Care Home Model of Care is a bespoke model that has been developed at the request of the working group consisting of representatives from BOPDHB, EBPHA, NMOPHO and WBOPPHO. It closely aligns with the national Health Care Home Collaborative’s (HCH) Model of Care so that accreditation through that collective is possible, whilst providing variation that meets specific needs identified in the Bay of Plenty. In order to mirror the Collaborative’s HCHs Model of Care as closely as possible the BOP model will follow the same format. The first three domains focus on desired outcomes for people with different types of health care issues which is the priority for this project. Domain four identifies potential business efficiencies which may act as enablers for implementation of the BOP Health Care Home Model of Care but not patient outcome focused. 1. 2. 3. 4.
