Some of our services include mileage reimbursement for going to doctor’s visits, treatment appointments, blood work, and anywhere the patient goes that is for their cancer. Currently we reimburse mileage at 15 cents per mile, up to a limit of $200 per month. This is available as long as the patient is going to treatments, or follow-up doctor’s visits. We reimburse for cancer related medications that the patient must purchase. We will pay their co-pay if they have insurance that pays for part of their medications. We reimburse up to 6
$250 per month for medication. We also have the nutritional supplement Ensure available for patients or will pay for any other supplement that a patient’s doctor requests, up to $67.50 per month. We have wigs, hats, turbans, scarves, etc. for women who have lost their hair as a result of cancer treatments. We also have a support group, “Living with Cancer”, that meets once a month at Wayne Healthcare and is open to anyone with cancer, their family and friends. We also have a lending library with Information regarding cancer and much free information. TRANSPORTATION SERVICES Community Transportation: Provides medical and general transportation to residents of Darke County anywhere within the county with a 24-hour advance reservation. Transportation is also available to out of county locations such as Dayton, Troy, Richmond, Columbus, Cincinnati, and many other locations with a 48-hour advance reservation. SENIOR 60+TRANSPORTATION General Transportation: Available for persons 60 years of age and older for essential trips to groceries, banks, social service agencies, drugstores and post offices. A 24-hour advance reservation must be scheduled by calling 937-547-9129. Suggested donation is $3.00. Senior Medical Transportation: Provides for clients 60 years of age and older who have no assistance from family or friends and cannot book transportation through any other program or agency in Darke county. Suggested donation is $3.00. Reservations should be made 24 hours in advance by calling 937-547-9129.

HOUSING Darke County Metropolitan Housing Authority: Housing Choice Voucher rental subsidy for low-income families, elderly and disabled persons. Community Action Partnership is working in partnership with the Dayton VA with VASH Vouchers to house Veterans. For information, call 937-548-5380. Housing Crisis Response Program (HCRP): Working with individuals and families at risk for eviction and homelessness to maintain housing and stabilize their situation. Applicants must fall within the 30 percent area median income guidelines. For information, call 937-548-8143. TBRA-Tenant Based Rental Assistance: Funding from the Community Housing Improvement Program (CHIP) in partnership with Darke County Commissioners. Preference is given to those who are homeless, veterans, elderly, disabled, and whose income is at or below 30% area median income. Units must pass Housing Quality Standards inspection and rent reasonableness. Leases are required and applicants must be in good standing with HUD regulations. Family Supportive Housing: Available for families that are seeking safe, affordable housing for up to 11 months to be stabilized with intensive case management. Rent is based upon income. For information, call 937-548-8143. Darke County Emergency Homeless Shelter: Houses persons left homeless due to fire, eviction, or other misfortunate circumstances. Maximum stay for one person is fourteen days, and families up to thirty days. Persons must contact the Greenville Police Dept. or Darke County Sheriff after 4:30 and weekends. ENERGY PROGRAMS: Home Weatherization, reduction of air infiltration and heat loss in homes. Service is provided to income eligible renters or owners. Contact 937-341-5000 or 800-617-2673 for an appointment. HEAP: Home Energy Assistance and Summer Crisis: to help low-income families pay their utility bills. Must be income eligible. PIPP (Percentage of Income Payment Plan): A special payment plan that requires eligible customers to pay a portion of their household income each month to maintain utility service. SALVATION ARMY: Provides limited funds to assist transient and local persons in need. BRIDGE THE GAP: Offers limited funds for emergency situations. Clothing Bank: Located in the basement of Community Action Partnership, Darke County facility. Donations of clothing and usable household items are accepted and offered for resale. Qualified persons are eligible to receive free vouchers, quarterly, for clothing. AFTER-SCHOOL PREVENTION PROGRAMS: Provides early intervention to youth ages seven to seventeen and their families, who may be experiencing conflicts at home, school, or in other areas of their lives. These supportive services focus on improving skills in conflict resolution, anger management, self-esteem, decision making, and socialization skills. Services include school visits, office visits, home visits, and groups. The goal is to gain the skills necessary to deal with life’s many challenges. THE CONNECTION YOUTH CENTER: Youth recreation center where youth can unwind after school while building positive relationships with peers. Youth also work to increase communication skills, cooperation, respect, and teamwork while working on special projects and events. ACES—ACHIEVEMENT CENTER FOR EDUCATONAL SUCCESS: The ACES Program is offered in collaboration with the Darke County Educational Service Center (DCESC). The program provides Darke County Schools and parents a safe educational environment as an alternative to out-of-school suspension/expulsion. The program targets students grades 9 -12 who are credit deficient and/or on the verge of expulsion or dropping out. The ACES program offers an alternative to the regular classroom setting. A Certified Teacher employed by the DCESC incorporates the curriculum for the students. All credits earned are transferred to the student’s home school towards graduation. Students receive diplomas from their home school upon completion of their graduation requirements. Mental health and drug/alcohol counseling are offered to all students enrolled. Students must be referred by their home school principal. WORKFORCE INVESTMENT ACT (WIA) YOUTH PROGRAM: Provided in collaboration with the Department of Jobs and Family Services. WIA prepares youth between the ages of 14-21 to successfully enter the workforce. Participants work to increase job skills, build educational skills, explore career options, manage money, and become productive citizens. This program focuses on employability, civic and social involvement, and developing the whole individual in the following areas: physical, intellectual, and mental health. TUTORING: The tutoring program is offered to any school age students to increase their educational skills and improve their overall academic performance. Students receive individual sessions with their tutor and meet on a weekly basis. This program works in conjunction with the local school systems and is based on a sliding scale fee.

Public Health Infrastructure: The Public Health Infrastructure Grant for bioterrorism preparedness provides funding for a wide range of public 19
health initiatives, which are used to combat threats to public health. The grant provides our agency with funds to prepare for public health emergencies, which include acts of bioterrorism. Emphasis is put on improving communication between our agency and key players in Darke County to develop, exercise, and implement Darke County’s response plans and protocols which are integrated into the local, regional and state levels. Under this grant umbrella is the reporting of communicable diseases for our county residents. This service gives us statistical information of reportable diseases encountered by local residents such as TB, STDs, and Hepatitis. The Darke County Health Department assists in advisement and counseling about infections and offers direction in treatment and testing. ENVIRONMENTAL DIVISION SERVICE DESCRIPTION: The Environmental Division aims to improve services that will protect the environment and improve the environmental factors that affect the health of the citizens of the Darke County Community. Programs include: sending quarantine notices for animals such as dogs and cats that bite a person to prevent rabies, licensing and inspection of operations that sell food, licensing and inspection of recreational camps, public swimming pools and bathing beaches. The Environmental Division also handles nuisance complaints. In order to file a complaint, a form must be completed at the Health Department. Incomplete complaints or complaints that are not signed or health related will not be investigated. Permits for sewage and plumbing systems must be obtained before they are installed or altered. Existing septic systems may be inspected for home sales, land splits, or part of the Operation permit program. Also, septic installers, septic tank pumpers, and plumbers must register with the Health Department. Before a well is drilled or altered, a well permit must be obtained to ensure it meets certain distance requirements. Tattoo operations are licensed and inspected by the Health Department.
