Our Mission Visiting Nurse & Hospice Care provides high quality, comprehensive home health, hospice, and related services necessary to promote the health and wellbeing of all community residents, including those unable to pay. In 2013, VNHC provided $2.6 million in charitable community benefit.

Our People In 2013, we served over 15,000 people living in the communities of Santa Barbara County, with a focus on the South County region (Santa Barbara, Montecito, Goleta and Carpinteria), and a growing service base in the Mid County region (Santa Ynez and Lompoc valleys). VNHC provides services to a culturally and economically diverse population, with the majority of people being aged 60 and over who require assistance to remain living independently, to recover from an illness or surgery, or to transition at end of life with dignity and comfort.

uch a spirit cannot be accidental. It is imparted and expected by all the staff – and personified perhaps most clearly by Holly Gendron, RN. Her service for VNHC began 20 years ago, in the early 1990s, as a home health nurse. Then she became a key player in the creation of Serenity House, VNHC’s 18-bed hospice facility, and became its first director. “I served in many nursing roles before coming to VNHC in 1994,” Holly recalls. “In fact, I never thought of home health nursing. But one of the differences I

immediately noticed was that these were genuinely happy people doing their jobs.”

“It doesn’t have anything to do with us,” Holly emphasizes. “Home health nursing is all about the patients. And one thing I always hear from patients, families, and doctors is that we never go in in a rush. The patient’s care is never portrayed as a burden. It’s done gently, kindly and with great compassion.” Holly has recently retired, and she looks back proudly at this vital organization. “Back in 1908, VNHC started with nurses doing TB clinics and food charities and more. We kept growing and expanding the care we were providing in the home. Then in 1996 and ’97 there was a great struggle about pay and the reimbursement for home health. Thousands of agencies across the country closed, but VNHC persevered. Now it’s one of the best things Santa Barbara has. I feel the history of VNHC is incredible.”

Holly also is proud of her efforts. “I have done almost every job here at the agency. And I feel like another layer of me grew over the years working in home health at VNHC… raising me to a higher level of education and knowledge,” she says. “And I watched the census for hospice care increase over the years. Being a part of caring for more patients and families was great! I have a huge place in my heart for VNHC. It has been my life’s blood for 20 years.” Holly has concluded her work with a profound appreciation of VNHC. “One thing that has always been apparent at VNHC is how people [employees] come back. This speaks to the value of this agency… that people are always drawn back. It is a place where you can do so many things for many people at so many levels, including yourself.” ❧ www.vnhcsb.org

any of us think of hospitals as places of healing, but complete healing usually takes place after we are sent home from the hospital. Still, we face a serious risk if we are unable to properly care for ourselves at home. We may reinjure ourselves and end up back in the hospital. This is the lesson Denise Segura recently learned – and why Visiting Nurse & Hospice Care’s Home Health Service became critical in her life.

Denise is Chair of the Chicana Studies department and a Professor of Sociology at UCSB, and late last summer she began having difficulty standing and walking. She eventually suffered a terrible fall and fractured her pelvis. “It happened suddenly,” she says, “and the doctors couldn’t figure out why I fell.” Only after months and a slew of tests was she diagnosed with a rare condition called Stiff Person Syndrome.

Doctors at Sansum Clinic and Cottage Hospital treated her pelvis, but back home, she fell again and kept stressing her injuries. She was nearly rehospitalized, where they would have fitted her with a pelvic plate – a costly, major operation.

The Caring Faces of Integrative Therapies at VNHC

J

ohn is agitated and in pain, but doesn’t want to take any medicine that might make him feel drowsy. Amy is nauseous but hasn’t been tolerating the medications that have been offered by the nurse. And Robert, at the age of 40, is struggling with the fact that he has recently been admitted to hospice care. All of these patients found relief through Visiting Nurse & Hospice Care’s Integrative Therapies Services. Integrative Therapies compromise a variety of holistic modalities providing comfort and deep relaxation that can ease symptoms and emotional distress.

At VNHC, these services are part of a patient’s plan of care, right from the start, promoting greater patient comfort and well-being throughout their hospice experience. Almost all patients feel some form of stress. “Research implicitly shows that shifting the body away from a stress response into a relaxation response improves not only the emotional state of a person, but also promotes important physiological changes to improve one’s well-being,” states Susan Clark, RN, Coordinator of Integrative Therapies. www.vnhcsb.org

Caring for the Caregiver VNHC’s Bereavement Services – The Faces of Help and Hope 12

any of us know someone who is currently “in the trenches” caring for a loved one. As we notice the fatigue in their facial expression, and their tendency to focus all attention and energy on the person receiving care, we might wonder, “How do they do it?” We may want to help, but not know quite how to go about it. Perhaps we have figured out ways to offer support, from helping with errands to providing some respite time to the caregiver. The caregiving role can last days, months or even years. Fast forward in time and we learn that the caregiving role has

abruptly ended, as the ill or aging person has died. What will help the caregiver now? Hospice always regards the person and their circle of family and friends as the unit of care. Bereavement services quite naturally continue this care by supporting family and friends in their grief and adjustment process. Many of the people that VNHC support in our bereavement programs have been caregivers to their loved ones. In addition to grief, they now struggle to adapt to an entirely different existence without a major focus in their lives.

Here are some ways to understand and be helpful to bereaved caregivers after a death has occurred: Don’t rush them into a new life. Many bereaved caregivers have shared that they feel pushed by well-meaning friends, “You should take a class … get out more … get everything done that you couldn’t do before!” Give them time to adjust. As with other major upheavals in life, they need patience and time to realize the effects of change. Caregivers often come to the end of their role exhausted and depleted. They’ve been running marathons, not sprints. Many caregivers have a profound physical, mental and emotional fatigue, and a significant sleep debt. Help them find ways to rest. It can take months for caregivers to return to normal energy patterns.

Allow them to express mixed emotions. Anyone who has been the caregiver for a dying loved one is entitled to feel, for example, relieved, sad, abandoned, lost, regretful, aimless, grateful, or any other emotion in any combination. Don’t make it harder by telling them they shouldn’t feel one way or another. Grief is a natural and healthy process that unfolds in its own time. Often, one doesn’t have to do or say anything, other than listen and be present. Practical help is often useful. Caregivers who are recovering their energy and focus may appreciate offers of practical help, from gardening to shopping to organizing. But pay attention to their need to pace themselves, and don’t push too hard.

We can offer further advice and support as well as information on our counseling services and support groups, all of which are free to the community. The Santa Barbara County Medical Society (SBCMS) named Dr. Michael Bordofsky, VNHC Medical Director, 2013 Physician of the Year at the society’s Annual Meeting in December. A resolution was presented recognizing him for his commitment to the field of medicine, particularly within the field of palliative and end-of-life care.

Volunteers Extraordinaire 80’s Club More than 1,057 years of life wisdom were represented at the first ever gathering of the 80’s Club on April 8. Fourteen attendees that give of their time to the agency are at least 80 years young and more.

Golden Heart Award Winners Each quarter, one Visting Nurse & Hospice Care employee is selected to receive the Golden Heart award. They are nominated by their peers based on their outstanding commitment to VNHC’s mission and core values of compassion, integrity, teamwork, and excellence.

Ilona Varner, RN

or 10 years, Ilona Varner has been Visiting Nurse & Hospice Care’s anchor of the graveyard shift – midnight to 8:30 a.m. Since VNHC is a 24/7 health care service, someone on staff has to handle whatever comes along during the wee hours of the night – and most of the time, that’s Ilona, as she is the Night On-Call Nurse. Ilona has grown very accustomed to handling the midnight work. Before working with VNHC, she was an Emergency Room nurse for 30 years on the late-night shift. Now at VNHC, her routine work is to answer all manner of late-night calls from patients recovering at home or in residential facilities. When Ilona gets such a call, she doesn’t

just handle it over the phone. She immediately heads to the patient’s home. From bathing and managing medications to simply listening, “I help the family with a variety of issues,” she explains. “Even though they know their loved one is near death,” says Ilona, “it can be very hard for people to say that final good-bye. For some, every extra moment with their loved one is precious and so it’s difficult for them to let go.” “I often feel I can help people,” Ilona says, “because I’ve lost my own parents. I always relate the experience back to my own experience because that helps me understand what they’re feeling.” Families are not the only ones who value Ilona’s support. The VNHC staff overwhelmingly voted her the Golden

Heart Award. They also recognize her competence as a nurse. Ilona does point out that she is only one link in a well-oiled chain at VNHC and that she couldn’t do her job unless others have done theirs. Nevertheless, her co-workers admire and appreciate that Ilona handles the long, lonely midnight shift largely on her own. ❧

Enrique Almengor, HHA

thought I wanted to be a mechanic,” says VNHC’s Golden Heart award winner Enrique Almengor. But on the day his first son was born at Cottage Hospital and he witnessed the wonderful way the nurses cared for his wife and child, his life vision turned around. “They impressed me so much,” he explains, “I felt that I’d like to get into that kind of work.” He has never regretted that decision. Enrique arrived in the United States at the age of 18, coming from a small town south of Guatemala City. Like so many new arrivals, he quickly enrolled in Englishlanguage classes and took on a variety of odd jobs to make a living. However, his experience that day at Cottage led him to enroll in the Certified Nursing program at Santa Barbara City College – and the

result, he discovered, was a deep, abiding happiness from his work. “I get a lot of pleasure from being helpful,” Enrique says about his job as a Home Health Aide at Serenity House. “If you do this just for money, it’s a problem… it’s not satisfying. But if you like helping people, you feel that you’re needed.” As with many nurses and health aides, Enrique’s service has spiritual roots. He is especially inspired by the religious precept that ‘It is more blessed to give than to receive’. And the special situation at Serenity House

motivates and enhances his caring spirit. “Working with these patients,” he says, “you see that what people really need and what they respond to is love. Even if they’re dying and suffering, they enjoy the love and care you can give to them. So working here has really developed my compassion for people.” In recently winning the Golden Heart Award, Enrique felt “very proud and happy.” What particularly touched him is that this award came from his peers. “I felt that everything I do is worth it because my co-workers appreciate my efforts.” ❧

he VNHC Board of Directors hosted the 2013 Chairman’s Council Reception on October 23 at the Montecito Country Club to thank the organization’s contributors for their significant financial support. The evening featured remarks by longtime supporters Jack and Julie Nadel, who in 2008 started the Elly Nadel Music Therapy Program, which is now a part of VNHC’s Integrative Therapies for hospice patients and their families. ❧

Perspectives in Healthcare n March 18, Visiting Nurse & Hospice Care hosted the inaugural PHorum: Perspectives in Healthcare, bringing together key leaders from various sectors of the Santa Barbara healthcare community to educate, inform and discuss key issues and their impact on the County. Sansum Hospitalists coordinate the overall medical care, testing, labs and consults for adult patients hospitalized at Santa Barbara Cottage Hospital or Goleta Valley Cottage Hospital. The Hospitalists care for an average of 10 to 20 patients per day. They work on a daily basis with many staff members of VNHC.

Maravilla Maravilla is a retirement community in Santa Barbara that provides the perfect blend of independence, comfort and convenience, offering a range of lifestyle options from independent living to memory care and a full-service campus community. VNHC has worked together with Maravilla for many years, providing home health, personal care and hospice services for their residents and patients.

he one-hundred-six-year legacy of Visiting Nurse & Hospice Care in Santa Barbara is a testament to the organization’s ability to anticipate our community’s health care needs and trends. While there is a lot of uncertainty in the world of health care at this time, VNHC is poised to provide stability, wellness, and independence through innovative services and technologies in order to deliver health care compassionately and competently to you and your family.
