Our Mission Visiting Nurse & Hospice Care provides high quality, comprehensive home health, hospice, and related services necessary to promote the health and wellbeing of all community residents, including those unable to pay.

Our Care VNHC serves the communities of Santa Barbara County, with a focus on the South County region (Santa Barbara, Montecito, Goleta and Carpinteria), and a growing service base in the Mid County region (Santa Ynez and Lompoc valleys). The majority of people served

by VNHC is aged 60 and over and requires assistance to remain independent, to recover from an illness or surgery, or to transition at end of life with dignity and comfort. VNHC provides services to a culturally and economically diverse population, ensuring that no one is turned away due to an inability to pay.

Music is a language that transcends all others. It has the power to reach people like none other. In any life stage, music celebrates one’s inner beauty, radiant essence…entire being.

V

isiting Nurse & Hospice Care is honored to highlight the Elly Nadel Music Therapy Program, which provides thoughtfully designed music therapy services to enrich the end of life experience for patients, their family members, and loved ones. This music program provides comforting support to soothe the physical, emotional and spiritual needs of patients with their best interests at heart. Music can provide a reassuring connection at a family’s most meaningful moments, such as the time of transition.

The Elly Nadel Music Therapy Program was implemented in 2008 by Jack and Julie Nadel of Montecito, CA, and has been sustained by their very kind and generous donations.

Elly Nadel, who was a former VNHC hospice patient and a musician herself, inspired the program. Elly’s deep love of her favorite music brought her great peace, comfort and joy at the end of her life. Her husband Jack recognized the important role that therapeutic music played in Elly’s hospice care at VNHC. With the insight and encouragement from his current wife Julie, they founded the Elly Nadel Music Therapy Program in honor of Elly.

Music Therapy is an established healthcare profession and VNHC is fortunate to have the exceptionally talented Stefana Dadas, a Board Certified Music Therapist, at the helm of the program. Providing a personal touch to each patient, Stefana’s passion for music, extensive clinical training, and experience in hospice care is an incredible asset to VNHC, and more importantly cherished by patients and families.

with late-stage liver cancer was entering the final stage of transition. Dr. Martin placed her small therapeutic harp at the foot of the patient’s bed. Beginning with “Amazing Grace” accompanied by a pentatonic scale set to the rhythm of the patient’s breathing rate, Dr. Martin then switched to an Ionian style scale often found in hymns and lullabies to help soothe and comfort the patient. Stefana gently held the patient’s hand, feeling the vibration of the harp and the harmony of the patient’s breath in sync. The session ended with an ancient Celtic song appropriate for crossing over.

The Elly Nadel Music Therapy Program includes caring volunteers and staff who sing and/or play instruments at the bedside, such as the Threshold Choir (an a cappella choir whose mission is to sing for and with those at the thresholds of life). In addition, personalized Therapeutic Harpist Jeanne Martin, PhD, iPod systems are offered to interested patients (cover photo) who helps ease the journey of those who are transitioning by playing the harp and families, programmed and delivered by at the bedside of hospice patients. According to Dr. Martin, “each individual has his or her own musical preference, rhythm, resonant tone, and mood. The key is for the musician to be present for the patient and to deliver the music in a caring and compassionate manner, deeply tuning into the needs of the patients and loved ones.” Her repertoire is diverse, ranging from Bach to Broadway to the Beatles. The style of music played also encompasses the individual’s faith and cultural heritage, along with the era of the music they grew up with. The musical genre changes as the person approaches the end of life. While a patient may at first request music from the Big Band era, as they approach their transition they might prefer angelic scales, soothing lullabies, and sacred music that help bridge the passage.

As Stefana puts it, “It’s a beautiful gift and a joyous way to celebrate and memorialize the person’s life.”

“From it’s inception, The Elly Nadel Music Therapy Program continues to evolve and expand, providing over 300 visits to 100 patients and families per quarter. In 2012, the program reached a milestone as over 1,000 visits were provided to patients and families residing at home, in senior care facilities, Serenity House and Sarah House.

VNHC extends deep gratitude to the Nadel family who realized that music therapy can bring comfort, grace, and dignity to the end of life journey. ❧ 6

Meeting Daily Needs

M

edical care is not the only help a person needs when they wish to recuperate at home. Food must be cooked, laundry washed, and errands run. For those who need assistance to remain living independently, the same kind of support is needed. This is where Visiting Nurse & Hospice Care’s Personal Care Services play a vital role.

“After the hospital, I was in a wheelchair and I was crippled,” says VNHC client Alice, “then the lady from Visiting Nurse came. She drove me to my appointments, washed my hair, did the laundry. She was fantastic. It was terrific help. I was so happy!”

In the same way, 91-year-old Charlie describes the help he has received as “a godsend. I’m old; I can’t do a lot of things,” Charlie explains.

“My care worker does the bedroom, laundry, vacuums and waters the plants. And she makes me a great breakfast, too!”

Alice and Charlie’s stories are typical for VNHC’s Personal Care Services. The caregivers are well-trained, carefully screened and prepared for homecare work. They handle an array of essential chores: housework, laundry, cooking, bathing, pet care, companionship, gardening, driving, shopping and more. They also make phone call reminders to ensure that medications are taken as prescribed. Their work is non-medical, but it enhances health and safety, prevents falls, and keeps people living independently.

respite care for family caregivers who need time for personal needs or to take wellneeded breaks.

Geriatric Care Management is another service provided through Personal Care Services.

VNHC staff plan and coordinate various types of health and social care services for older adults to meet their long term care needs, improve their quality of life, and help them maintain independence for as long as possible.

VNHC staff are both professional and caring.

Licensed and bonded, they provide care where people live—in their homes, apartments, retirement communities, assisted living facilities, or skilled nursing facilities. Whether needed for just a few hours or 24/7, caregivers are always available. ❧

Telehealth Better Peace of Mind at Home

Rushed to the ER, she ended up having a heart attack in the hospital, where thankfully, she was attended to by a team who was prepared to address her urgent needs. Without telemonitoring, she would have been at home alone when the heart attack hit.

I

Telehealth likely saved her life!

n February of 2013, a woman who had been diagnosed with pneumonia and Chronic Obstructive Pulmonary Disease and had been in the hospital eight times in recent months, was the first patient to be placed in Visiting Nurse & Hospice Care’s Telehealth Program. She was frequently anxious about her condition and was unsure if her shortness of breath was serious or not, until the day the telemonitoring machine alerted her of abnormal readings. After consulting with the VNHC nurse who verified her data over the phone, 911 was called.

VNHC’s Telehealth Program is ideal for patients with heart failure, COPD, diabetes, hypertension, and pneumonia. The userfriendly telemonitoring services offer an easy line of communication that enables VNHC health professionals to identify symptomatic patients and provide early intervention.

Machines monitor the patient’s vitals, including blood pressure, heart rate, oxygen levels, and weight. It asks patients questions about their current symptoms and provides a short daily tutorial based on the patient’s diagnosis, empowering the patient and caregiver to become more independent with their own care.

For example, a patient with a recent heart bypass and valve replacement had just returned home from the hospital to be with his 8

wife and daughter. Having the telemonitoring machine in place made the transition easier, allowing the daughter to return to her family while giving the spouse the security and peace of mind knowing that they were able to monitor his progress daily. The patient’s wife said she felt the machine did just as much good for her as it did for her husband.

According to Chris Grant, PT, MBA, Director of Clinical Integration at VNHC, “Our goal in utilizing telehealth services is to reduce the frequency of hospitalizations and emergency care of individuals with chronic illnesses. Agencies have reported up to a 70% reduction of re-hospitalization rates using telehealth services.”

Visiting Nurse & Hospice Care strives to provide the community with the highest quality and most innovative care in home and hospice services. VNHC is the only non-profit program in Santa Barbara currently providing telehealth services. ❧

Hospice Care Dignity & Comfort When There is No Cure

T

he quality of care provided through the Hospice Program at Visiting Nurse & Hospice Care is illuminated by the experiences of Sharon, a retiree who was recently admitted to VNHC’s Serenity House with stage-4 breast cancer.

Before coming to Serenity House, Sharon had spent six weeks at Cottage Hospital in cancer treatment. Then she was released to a care facility to recuperate, but her condition deteriorated and her medications increased.

The drugs and treatment led to bouts of depression, fear and confusion. It reached a point where she declared to her doctor, “I want it to stop! No more doctors, no more tests, no more medicines—I want it all to stop!” Sharon was then admitted to Serenity House.

Has it made a difference? Sharon answers with a big smile: “It’s fabulous!”

After a week at Serenity House, her mind cleared and her tension and fears melted away.

She no longer receives cancer treatments.

“Now I just have pain medication,” she says with relief.

Hospice is a special way of caring for people who choose to focus on quality of life when there is no cure for their advanced-stage terminal condition. It honors the dignity of the individual, offering quality, compassionate care when medical treatment no longer offers a cure. Hospice seeks to alleviate pain and soothe a person’s distress as they face the final stage of their life.

is wonderfully soothing. She has a beautiful private room with a stunning view of the mountains. “And the staff is so respectful and reassuring. They’re like angels.”

Sharon’s children are comforted, too. One daughter explains, “All the staff, the nurses… everyone is here for mom! You can’t imagine what a relief that is.”

Sharon’s care is provided by a team of specially trained doctors, nurses, social workers, bereavement counselors, chaplains, volunteers and others whose primary goal is to manage her symptoms so that her last days are peaceful.

The Hospice Program at VNHC also supports family members through this experience. They are given ample private time with their loved one to promote discussion, decision making, loving reflection and emotional closure. As well, VNHC bereavement support and counseling help families cope with the loss of their loved one.

The Hospice Program treats the person rather than the disease. It views the end of life not in medical terms but as a human experience that is aided by medical expertise and a spirit of respect, tranquility, and loving kindness.

Often, hospice care is provided in a person’s own home; and much of VNHC’s hospice service is conducted just that way. But when home care is not advisable, VNHC offers a peaceful, homelike retreat on a Santa Barbara hillside—Serenity House.

Home Healthcare Outlook

C

learly, healthcare costs in the U.S. are increasing at an unsustainable rate.

At the same time, the need for healthcare will skyrocket because of the major trend in population growth. Americans aged 80+ are the fastest increasing age group in the nation, with a growth rate four times greater than the overall population. And, seniors have more health issues than younger adults. This means an increase in cardiovascular care and dementia care, as well as greater management of chronic conditions, like arthritis and diabetes.

While facing this greater demand, there is a decline in key healthcare resources—namely, medical professionals and money. Recent studies report that the U.S. is facing a severe shortage of doctors and nurses. In addition, all levels of government are struggling to pay for healthcare.

Although it may seem difficult to predict anything in today’s complex world, the trends for older Americans are so bold that you can get a clear picture of the future—especially regarding healthcare, according to Lynda Tanner, President & CEO of Visiting Nurse & Hospice Care.

The prospect of population growth and shrinking dollars means, “We will have to do more with less,” Ms. Tanner points out. But she is not pessimistic, as she feels that home-based care is well positioned to be part of the answer to our nation’s healthcare challenges.

“People want to remain in their homes as long as possible, and it costs much less to provide home care than facility-based care.”

Modern technology also plays a vital role, as iPhones and other devices help people monitor vital signs such as blood pressure and blood sugar. For example, Visiting Nurse & Hospice 11

Care now uses telehealth, a system that utilizes monitoring devices in clients’ homes. “This allows us to always be in touch with a number of clients from our central office,” Ms. Tanner explains, “and then do a home visit when a person’s vital signs warrant it.” This helps people remain safely in their homes while reducing costs, since the biggest expense in healthcare is staff.

There also is an increasing emphasis on patient education to promote beneficial lifestyle changes and management of chronic conditions. Health education that enhances stress reduction, nutrition, medication management, fall prevention and exercise will undoubtedly help seniors manage their health problems and medical expenses.

“The demand that we do more with less will promote more partnerships among healthcare providers,” explains Ms. Tanner. “If we want to save money as we serve more people, we must reduce the fragmentation in today’s healthcare system. Large hospitals, specialty clinics, doctor offices and homecare agencies must breakdown bureaucratic barriers, improve communication, and eliminate duplication. If we succeed, we will reduce costs while maintaining the quality of care our senior clients need and deserve.” ❧ www.vnhcsb.org

Santa Ynez Valley Recovery Residence

L

ocated at Atterdag Village in Solvang, the Santa Ynez Valley Recovery Residence is a continuous care retirement community that serves 135 residents, including 77 Independent Living apartments, 15 Assisted Living apartments and 47 Skilled Nursing beds. For most of their 60 years, they were known as The Solvang Lutheran Home. They provide skilled nursing and rehabilitation services for both short and long-term needs. Patients may be transitioned home to receive home health or personal care services from Visiting Nurse & Hospice Care, or hospice services may be provided to their residents.

Open House VNHC welcomed the Santa Barbara community to its new offices for an Open House Celebration on January 31, 2013. Over 200 friends and supporters attended the event, had the chance to tour the new facilities, and talk to VNHC leaders, volunteers, and Board members about the hospice and home healthcare they provide to Santa Barbara County.

Chairman’s Council VNHC Board of Directors hosted the 2012 Chairman’s Council Reception on October 25 at the Montecito Country Club to thank the organization’s contributors for their significant financial support. Home Health Aide at Serenity House

f the spirit of VNHC’s Golden Heart award could take human form, it very likely would be Yolanda Vera. Remarkably, Yolanda is a living expression of gentle kindness and excellent care. It is no surprise to learn that Yolanda has long felt drawn to hospice care.

“I began as a CNA nearly twenty years ago at St. Francis Hospital,” she says, where she saw people dying all alone. It touched something deep within her. “I used to hold their hand and just talk to them.” That experience drew her to hospice care. “I knew this was my calling.”

them and reassure them that we’re all here for them.” She also responds to the different needs of each resident. “Some people are ready… more accepting and open to death. But some are not, and they’re very afraid.” Yolanda offers her compassionate care to them all.

The simple aim, Yolanda says, “is to make people comfortable at the end of life.” What’s more, she treats each patient as someone special. “There’s always a special bond. You’re always connected with them.”

What she has learned from this work is that life is truly a gift. “Death teaches you a lot,” she Yolanda has been a CNA/ Home Health explains. “It makes you enjoy the moment, to Aide at Serenity House for about eight years. “I pause and see the beauty, and to see what’s give the residents comfort,” she explains. “I feed really special and meaningful in this world.” ❧

Human Resource Administrator

here was a perfect fit between Vanesa Wendler’s life experience and the ethos of Visiting Nurse & Hospice Care when she came to work here in 2009. As a result, Vanesa, Human Resource Administrator, was a handsdown winner of VNHC’s Golden Heart award.

Her colleagues and supervisors were unanimous in their praise of her helpful efforts on the job. Words that punctuated her nomination ballots included: “wonderful,” “compassionate,” “a big friendly smile” and “always willing to lend a hand.”

Vanesa’s gracious spirit took shape early through the loving support of her family, particularly her Colombian mother and also from an exposure to the wider world as a child. Her family traveled every year to many different countries, especially in Europe. Then in college she enrolled in a Quaker-sponsored program called “Friends World.” This program’s aim, Vanesa explains, “was to make us citizens of the world,” promulgated by travel around the world. She visited Costa Rica, Honduras, Eqypt, Israel, Japan, India, Jordan, Kenya and many more countries – all the while nurturing a deeper understanding and appreciation of the human condition.

These fine qualities reflect Vanesa’s remarkably positive attitude about her role on the job.

“My client is the employee,” she explains, “I always want to be approachable and helpful.”

This spirit of service is not a burden or challenge for Vanesa and instead is a vital source of her job satisfaction. “I find joy in helping employees. My work feels especially meaningful when I can actually help them.”

he Loan Closet has basic medical equipment, such as walkers, wheelchairs, crutches, canes and bathroom safety items, available to the community at no charge. More than 3,000 people received equipment in 2012.

Since 1951, the Loan Closet has been completely supported through private contributions, often from people who have benefitted themselves. Equipment donationsare also gratefully accepted.
