Dear VNHC Family and Friends, During the fall, Thanksgiving offers us a special opportunity to take time to pause and celebrate the blessings we have in our lives. In the healthcare world, November is National Home Care Month and National Hospice & Palliative Care Month, providing us with an important reminder to recognize all of the individuals who have chosen to serve our nation’s aged, disabled, and dying in their homes. Whether a physician, nurse, social worker, home health aide, family caregiver, or healthcare volunteer, these noble individuals provide care for those when they need it most so that their patients experience independence and dignity while facing injury, illness, or end of life. At Visiting Nurse & Hospice Care, we provide compassionate and trusted care for all in our communities, as we have for more than a century. We are blessed to have so many people, businesses, and organizations dedicated to sustaining our charitable care through donations and contributions. I am thankful for our amazing clinicians, caregivers, volunteers, and donors who uplift our core values of compassion, integrity, teamwork, and excellence. The stories we share with you in this issue of our Heart-to-Heart magazine more than exemplify VNHC’s core values. I am so proud to say I work with the people in these stories who go above and beyond the call of duty to meet the needs of the needy, sustain a legacy of exceptional local health care, fulfill a lifelong dream, and even save a life. Our VNHC employees, volunteers, and donors have incredible compassion, transforming the medical care we provide into heartfelt care. As we enter the holidays and the end of 2017, I hope you will join me in taking a moment to pause, breathe deeply, and express admiration and gratitude to the many hearts and hands who care for us and our loved ones.

B

everly Anderson of Lompoc is no stranger to Visiting Nurse & Hospice Care. She received home health services from October 2016 through February 2017 for specialized wound care due to diabetes and lymphedema. However, Beverly’s husband and caregiver, Malcolm Anderson, would not be alive today if not for the astute observation and action of Beverly’s home health nurse, Rebecca Loney, RN. As a result of Beverly’s diabetes and lymphedema, the healing process from a necessary skin graft surgery was long and comprehensive, requiring Rebecca’s visits over the span of four months. With each visit, Rebecca’s knock was eagerly anticipated. Beverly shared, “She is so neat! When you don’t feel well and the healing is so slow, to have someone come who obviously knows her job inside 4 | vnhcsb.org

out, and brings so much life into the house, she truly brightened things up.” It wasn’t just Rebecca’s cheerful demeanor that was, and still is, appreciated by the Andersons. It is Rebecca’s “above and beyond” care. Knowing they loved to cook, but were both diabetic, Rebecca brought a new diabetic-friendly recipe for custard, including the ramekins in which to bake them. “She knew we were diabetic and I hadn’t had a custard in years. She brought this recipe that used artificial sweetener and it made a very good custard. It was just such a nice thing to do.” It was the little extra things that made the difference, illustrating not only Rebecca’s

competence as a nurse, but also that she deeply cared and wanted what was best for her patient and family. Even the way Rebecca spoke to her patient was well received, “She always took the time to explain things to me in a way that I could understand,” said Beverly. In late January 2017, Malcolm had been experiencing more stress than usual and had started his own dialysis treatment the month before. When Rebecca arrived to treat and redress Beverly’s wound, she couldn’t help but notice Malcolm’s gray tint to his face and his change in vitality. “You’re not energetic like you used to be [Malcolm]. Why don’t you just let me listen to your heart?” Rebecca asked. With a calm but

concerned look on her face she responded with, “You have an extremely fast heart beat and I think we should call 911. This could be dangerous.” Beverly recalled, “You know for several days I guess he had been getting slower and slower and more tired and I just didn’t recognize that there was a problem because it was a slow progression. He was always tired and had to take a nap and if she [Rebecca] didn’t see that, then who knows what would have happened.” After calling 911 and getting Malcolm to the hospital, the medical team was able to identify that he was experiencing Ventricular Tachycardia, and unless there had been some intervention, his heart beat would have continued to increase, leading to an eventual heart attack. vnhcsb.org | 5

Beverly added, “Rebecca’s background in dealing with heart problems and heart patients was wonderful. Especially when she really needed to recognize that he had a problem. With her experience, she knew what to do, what to ask, and she could listen to his heart and tell there was something wrong.” Following that fateful and fortuitous visit by Rebecca, Malcolm ended up being hospitalized. After being discharged, Malcolm also required several follow-up visits from a home health nurse. It was without hesitation that Malcolm exclaimed, “I want VNHC. I want Rebecca!” Happily, the couple now continues to enjoy visiting with family and just recently returned from taking the entire clan to see Hamilton. They are also able to continue participating in their favorite hobbies and activities. Malcolm is an avid bridge player and often helps coordinate tournaments in the community. Beverly enjoys reading, although “enjoys” is an understatement. As a retired librarian, she still keeps up her own library that fills nearly all of the wall space in a room of their house. With their health under control and both diligent about their diabetes, they look forward to going on more cruises together. Malcolm even mentioned that dialysis is now available on many cruise ships. It was just one routine visit by Registered Nurse Rebecca Loney that exemplifies what it means to go “Above and Beyond,” and fulfills VNHC’s promise to be “there when you need us most.” You can trust that the care and service by the nurses at Visiting Nurse & Hospice Care will continue to go above and beyond for their patients and their families.

I

ndependent, determined, strong, and loving, Erika Jeschke was born in Germany on September 30, 1941. She attended school to become a nurse and achieved high marks. In her mid-twenties, she moved to the United States to work as an au pair for a family in Beverly Hills. For a time, she even hoped to be discovered by one of the big movie producers and become an actress. Vince “Winand” Jeschke met Erika in a German church in Southern California where he lived at the time. After several failed attempts to ask Erika out, Winand finally got her to agree to a “chaperoned” date. They married in May 1973. Their lives brought them to Santa Barbara where they bought land on the Riviera hillside and began to build their dream home. With the help of architect Joe “Woody” Woodruff, who later became their close friend and Durable Power of Attorney (DPOA) or Health Agent, their dream home became a reality. However, Erika’s dreams included something more, something patriotic.

Winand took every chance to share colorful flowers with Erika.

Erika’s mind remained sharp even throughout her last years. She was an avid Chinese checkers player. Winand teased, “It was never a joke, she was very serious. She beat me twice and I lost once.”

Erika and Winand were married on May 12, 1973 in Los Angeles.

Erika often dreamt about becoming an American citizen. Even as a little girl, she recognized the possibility that came with that liberation: the American Dream. As soon as she knew what needed to be done, she started taking the necessary classes. In 2012, the process was delayed when she was diagnosed with Parkinson’s disease, which was later clarified as Progressive Supranuclear Palsy. The disease may have slowed Erika down physically, but certainly not mentally. “She was in a wheelchair and I got her a citizenship test on the computer. As soon as she saw it, she yelled, ‘Get out of my way!’ She started taking the test and was getting all the answers right, and she knew how to do it,” shared Woody.

Erika and Winand shared a deep and intense love with one another, and always showed each other great compassion. In early 2017, as her illness continued to progress, Erika shared with Woody that she no longer had an interest in curative treatment; the medicine she was taking caused pain in her left leg. That is when her doctors made the decision to move her onto hospice care with Visiting Nurse & Hospice Care. Prior to making that decision, Erika asked her husband to speak with her privately, where she expressed her end of life wishes, including her dream to become a U.S. citizen. Erika received unparalleled care from VNHC’s comprehensive hospice team, and each caregiver who came to the house became a part of the family and a part of her special dream. vnhcsb.org | 9

“They all knew her, and knew what she was like, and what she wanted,” said Woody. “She would respond by grinning because in the end that was the only way she could communicate. When they would come, they weren’t strangers.” Erika especially enjoyed visits from Carol De Trana, VNHC hospice chaplain, who often spoke to her in private. “No one was around. Just them two and as religion was very important to Erika, this was a special moment shared by both of them,” accounted Woody. This time together helped give Erika the dignity, self-reliance, independence, and privacy that she so craved. “I would get everyone out of the room and they would be private, which I thought was good girl talk for Erika.” Upon beginning hospice services, Erika’s care team was aware of her citizenship dream. Jennifer Cisneros, Erika’s social worker, recalled studying with her for the exam. They would spend visits going over the questions. “Even though her body was really failing, her mind was still sharp,” recalled Jennifer. “She was a very smart woman and she was constantly reading the Bible and had other reading material on her lap.” A few weeks later Erika traveled to Los Angeles to take the citizenship exam. “I told her ‘Good luck!’ and of course we arranged for help just in case there was an emergency or anything, for someone to care for her while she was in LA,” 10 | vnhcsb.org

remembered Jennifer. “The next day, I returned to visit her and the first thing I asked her was how it went, and Erika exclaimed ‘I passed!’” After passing the citizenship exam, the next step was to take the Oath of Allegiance at a naturalization ceremony. The family had planned to travel with Erika to Los Angeles the next week. However, that weekend, Erika experienced a seizure, forcing her to become bedridden, and making it impossible for her to complete the three-hour drive to the Los Angeles Convention Center. Her family’s hopes of making her dream a reality began to dwindle, but only for a moment, until the cooperation, determination, and affinity of the VNHC hospice team began to unfold like Old Glory herself. Over the weekend, Erika’s health continued to decline, and Erika’s immobility increased. Woody called the citizenship office in Los Angeles, but never got a clear answer on what was needed to get a judge to travel to Santa Barbara to perform the ceremony before Erika declined further. Finally, Woody spoke with a true advocate who helped him better understand the protocols and what was needed.


Late Wednesday afternoon, Woody spoke with Erika’s Hospice Clinical Manager Jeneffer Villacrucis, and requested that a letter of Erika’s condition be quickly faxed over to the citizenship office. A second letter was then requested with a doctor’s signature. Erika’s attending physician, Dr. Eric Trautwein, was visiting patients at VNHC’s Serenity House, and drove down the hill to aid in his part of the process, signing the letter and exclaiming, “This is such an amazing story!” As the afternoon became evening, Jeneffer stayed close to her phone, emailing paperwork to Woody and coordinating all last minute details from the immigration court. The immigration officer called her directly, trying to understand the legitimacy of the request and attempting to gain a better understanding of the time constraint. Jeneffer knew that Erika only had days or maybe hours remaining. After the arduous efforts of all involved, a judge was confirmed for Thursday, the next morning. Throughout that night, everyone hoped for at least one more day with Erika…praying that she would not pass…just one more day to make her dream come true. Lucy Figueroa, Erika’s home health aide, volunteered to arrive early in the morning to help dress and prepare Erika for the special day. At 9:00 AM, the judge arrived to the Jeschke home. Erika lay in her bed surrounded by the many people who have been at her bedside: Christine Olsen, Erika’s nurse, who gently wet her lips with a sponge; Carol, Jeneffer V., Jennifer C., Lucy, the hospice volunteer, and Woody; her sister and brotherin-law; and most importantly Winand, her loving husband. Even with all of the people in attendance, the room was peaceful. Winand stood next to his wife, holding her hand, and spoke the oath for her. Erika’s eyes gleamed brightly, acknowledging she knew what was going on around her. Everyone began to cry as the certificate was placed in Erika’s hands. VNHC therapeutic harpist, Jeanne Martin, arrived shortly after the ceremony and played songs on the harp for all to enjoy, as they reminisced with Erika during her final breaths. Winand recalled his last moments with his wife, “Everything is perishable on this earth, and it doesn’t last long. But the memories of a loved one, they mean everything for us.” On June 23, 2017, hours after having her dream fulfilled and becoming a citizen of the United States, Erika peacefully passed away in her home.

Following Erika’s death, Winand wrote several notes to help process his grief, as well as in appreciation for the VNHC team: “Thank you for all the help and assistance that you (VNHC) gave us and making this journey easier to manage; we were clueless. My view on hospice has drastically changed and I recommend VNHC to everyone. You are all very knowledgeable in your individual roles. We couldn’t have given Erika a better end without you.” – Winand Jeschke

Gary will take guests on a journey to better understand the healing power of music, and the role that music and the arts play in helping patients, their families, and caregivers. His keynote will also address the qualitative evidence that supports the validity of music therapy programs, especially at end of life. “The role of beauty, and particularly music as the universal language of emotion, can be directed to create a field of compassion, empathy, and mindfulness that is present in all things that many people want, but sometimes have a hard time getting that through, what shall we say, in rational ways or through medical modalities,” shared Gary. PHORUM guests will be provided with the tools to problem solve, to stop and evaluate interactions between people, and to pause to appreciate the beauty in every experience, especially during the vulnerability of end of life. Proud Partnership

Visiting Nurse & Hospice Care is proud to partner with another local, non-profit organization, Easy Lift Transportation. Like VNHC, Easy Lift provides a wide variety of specialized services in the southern Santa Barbara community, including transportation for VNHC patients and their families. Passengers use vans for medical appointments, work, school, grocery shopping, and quality of life activities such as church, recreation, and entertainment. It is through Easy Lift that many of our patients - often homebound or elderly (and no longer able to drive) - are able to complete everyday travel and errands. As our nation’s population ages and those with physical challenges seek to maintain their independence longer, the need for Easy Lift’s services continues to increase. Because transportation provides the independence vital to maintaining quality of life, Visiting Nurse & Hospice Care strongly supports Easy Lift in this shared goal. “The relationship between VNHC and Easy Lift goes back at least 25 years, if not more,” shared Ernesto Paredes, Easy Lift Transportation Executive Director. “They have always been at the forefront of their industry, serving our community with integrity and finesse. When Rick and I spoke about expanding the relationship between Easy Lift and VNHC, it was a no-brainer. Of course we want to partner with organizations that are serving our community’s most vulnerable! I see this new partnership as a reinforcement of our work together and an opportunity to create a better community.” When asked about the inspiration behind VNHC’s contribution, VNHC Foundation Executive Director, Rick Keith said, “This sponsorship is an illustration of the way we all can work together to meet the needs of our community. VNHC Board of Directors Welcomes Chris McNamara, Chair of the Board of Directors Earlier this year, Visiting Nurse & Hospice Care was pleased to elect Chris McNamara as Chair of the Board of Directors. Chris has a natural appreciation for the well-being of others and extensive experience working in healthcare. Her comprehensive career with Sansum Clinic included positions as Director of Medical Records, Finance Department, Business Services, Managed Care, IPA and Physician Services, and ultimately as Senior Vice President of Operations. Chirs joined the Board in 2011 when VNHC acquired Community Home Health, previously an entity of Sansum Clinic. When asked why she took on this leadership role, it was her confidence in VNHC’s ability and longevity that allured her. “I’ve lived in Santa Barbara most of my life and organizations like VNA, which then became VNHC, Sansum Clinic, and Cottage Hospital have been a constant presence in this community. Quite frankly, given the size of our community which is small compared to other places like Los Angeles or San Diego, the quality of medical care that is provided here is phenomenal. Not only the quality but also the breadth and the depth of services that we are able to provide to this community. I mean anyone who works in healthcare is proud, I certainly am. I’ve worked in healthcare for 30 years in this community and it’s a really good feeling. At the end of the day when people ask you what you do, you can very proudly say I’m part of the healthcare community of Santa Barbara and I put VNHC right up there. Not only is the longevity there, but the quality and the patient satisfaction and the outpouring of gratitude from our patients is truly humbling. I’ve felt that way working at the clinic and I feel exactly the same way working with VNHC.” We are pleased to have Chris McNamara’s leadership, as well as her experience on the Board of Directors.

VNHC Golden Heart Award Winner Each quarter, one Visiting Nurse & Hospice Care employee is selected to receive the Golden Heart award. They are nominated by their peers based on their outstanding commitment to VNHC’s mission and core values of compassion, integrity, teamwork, and excellence. Erendira “Eddie” Olmedo continues to make an outstanding contribution to VNHC by constantly being a great team player and someone we can count on. While working hard at her job, her colleagues really appreciate that she is always there when you need her. Not only is she dependable, but Eddie is always looking for the best and most efficient ways to do things. It’s no surprise that compassion and the patients’ needs always come first as well as making sure the integrity of her work is intact. Eddie’s enthusiasm and leadership has garnered admiration from her co-workers. In Eddie’s department, she is described as “the beautiful heart of Centralized Intake.” 16 | vnhcsb.org

Loan Closet is Moving

Originally, the Loan Closet at Visiting Nurse & Hospice Care started as an informal program and was housed in the carriage shed of its former headquarters (then known as the Visiting Nurse Association). During that time, the carriage house was the “garage” for the horse and buggy that the nurses used to make home visits. Through the years, the automobile replaced the horse and buggy, but there was no replacing the Loan Closet as the community’s needs grew. In 1948, Thomas M. Storke, then publisher and editor of the Santa Barbara News-Press, broke his leg. During his recovery, Mr. Storke needed a wheelchair to independently get around. He was surprised to learn how expensive the cost of a wheelchair was, and wondered how others with limited means could afford the cost of basic medical equipment. Determined and inspired, Mr. Storke launched a charitable fund to formally open the Loan Closet in 1951. Today, thanks to the generosity and compassion of many donors and contributors, VNHC’s Loan Closet is a community pillar serving more than 4,000 Santa Barbara County residents each year. The Loan Closet provides, at no cost to residents, durable medical equipment (such as wheelchairs, walkers, crutches, shower benches, and other bathroom safety equipment) on a short-term loan. VNHC is proud to continue the Loan Closet legacy and service. In November, the Loan Closet will relocate to VNHC’s main campus in Olive Court. During the move, the Loan Closet will be closed on Thursday and Friday, November 16 and 17, due to construction. The Loan Closet will reopen its doors on Monday, November 20 in its new home at 360 Olive Street, Santa Barbara, CA 93101. Together We Prevail! The 13th Annual Santa Barbara Yacht Club Charity Regatta was undoubtedly the most FUN ever. With more than 400 people in attendance, it was great to see so many new faces as well as seasoned veterans join in the festivities for both land lovers and seaworthy sailors alike. Together, we raised nearly $135,000 this year, and cumulatively raised over $1.3 million, to support the charitable healthcare services of Visiting Nurse & Hospice Care. This achievement is a testament to the successful partnership between the Santa Barbara Yacht Club and Visiting Nurse & Hospice Care, which has resulted in bringing renewed health and comfort to thousands of people in Santa Barbara County.

Skills Day Skills Day for both Hospice and Home Health was held in June in collaboration with the Quality, Compliance, and Education Department. Ongoing training is a great way for all clinicians to refresh their skills used in daily patient care. the

Elisabeth Swede is a beloved hospice nurse, and has been for nearly 11 years with Visiting Nurse & Hospice Care. Between her family’s professional medical background and her innate talent of caring for people; she was destined for a career in nursing. For Elisabeth, her work is sacred and she considers it a privilege to provide peace and comfort to patients and their families during a critical chapter of life. Each patient continues to teach her valuable lessons, often expanding her knowledge of humanity and what matters most. Such variation inspires and nurtures innovation in the workplace. She considers herself an advocate for each of her patients, whether it’s to ensure they are given necessities like proper feeding and bathing, or provided simple wishes, like eating their favorite ice cream for breakfast. Elisabeth is devoted to making sure all of her patients pass with the dignity and quality of life they deserve, regardless of their background or situation.

Like Elisabeth, Marian Watts, who founded the Visiting Nurse Association of Santa Barbara in 1908, also believed in being an advocate for those who could not access the quality of care they deserved. After graduating from the first nursing class at Johns Hopkins University, she set out to ensure that patients who physically could not make it to the hospital, or simply could not afford the medical treatment, were provided with the care they needed. Visiting Nurse & Hospice Care continues the legacy of Marian Watts each and every day through the advocacy and devotion to patients, from nurses like Elisabeth Swede.

Organization Achievements in 2016 Continuously achieve a 4-star rating from the Centers for Medicare & Medicaid Services (CMS) for quality of patient care and patient satisfaction in Home Health. Implement a new Senior Assessment Program in collaboration with Santa Barbara County to provide FREE Senior Health & Safety Assessments in the homes of seniors countywide. Only hospice organization in Santa Barbara County to earn the highest level (Level 4) status for the national We Honor Veterans program, which uniquely helps guide veterans to a peaceful end-of-life experience.

The Legacy of Care

Partner with the Lompoc Valley Medical Center to develop The Legacy of Care (TLC) Program, a home based initiative to provide short-term nurse case management for individuals following hospital discharge to optimize their health management of a chronic disease or medical issue.
