A Letter from Our CEO, Christopher Rinn:
Dear Friends of VNA Health Group,
We are delighted to present our Annual Report for 2023, a testament to the extraordinary impact that VNA Health Group (VNAHG) has made on the communities we serve. This was a unique year for the organization, as longtime President and CEO, Dr. Steven Landers departed after an incredible 11 years of leadership in late summer ‘23. I’ve had the privilege and honor of leading this extraordinary organization since his departure, and what a journey it’s been already!
It’s clear that our founding mission remains at the forefront of the VNA today, and we remain committed to providing the highest quality services to those who need it most. Our employees care for individuals and families during life’s most challenging and precious moments, from the earliest stages of life to the final moments of their journey. As we close the chapter on another meaningful year, we are humbled and inspired by their incredible work and steadfast commitment to providing life-changing care to our patients.
Our achievements and growth are made possible by the support provided by our donors, volunteers, and community partners. Their generosity and dedication help sustain our mission and ensure our continued success—and because of them, we are able to expand our reach, enhance our services, and make a meaningful difference in the lives of those we serve.
However, home health, hospice, and community-based care organizations like ours continue to face daunting challenges. Workforce shortages exacerbated by the COVID-19 pandemic, paired with an aging population and a rise in chronic illnesses, threaten providers’ ability to meet demand and maintain quality care standards. Medicare cuts impact our financial resources and jeopardize the availability of essential care for those in need. Each of these challenges calls for innovative solutions and decisive changes.
We have renewed our commitment to workforce development through clinical training initiatives, our Ladders to Success program, and a stronger investment of energy and resources to grow our team. Through these initiatives, we will help train and develop the healthcare workforce of tomorrow while strengthening our most critical and valuable resource: our VNA Health Group family.
As we look to the future, our focus remains on workforce development, patient-centered care, and operational excellence. By investing in our workforce, embracing a customer mindset, and fostering a culture of continuous improvement, we will ensure our patients receive the responsive, reliable, and high-quality care they deserve.

Special Supplemental Nutrition Program for Women, Infants, and Children
• Completed the first year as a participant in Empassion ACO Reach, a patient-centered Medicare program designed to help improve patient experience, quality of care and health outcomes
• With the support of Parker Health, initiated a pilot program to provide personal care/home modifications to selected ACO patients who need assistance in these areas during transitional periods
• VPS added two nurse practitioners to its staff in 2023 as well as an RN for the Nurse Go program in Monmouth and Ocean Counties; added two medical receptionists to our office staff
• Patient Satisfaction: Overall Assessment for VPS Top Box Score increased from 84.41 in 2022 to 85.07 in 2023, exceeding goal of 85.0
• Implemented a new document signing platform for clinicians called “Suture Health” in an effort to improve turnaround and efficiency of signed orders and documentation. SmartContact five item patient experience survey texted to VPS patients after a first visit
In-Home and Community-Based Palliative Care:
Community Palliative Visits: 1,615 Average
Community Palliative Patients served: 958
• Dr. Marianne Holler, VNA Health Group’s Chief Medical Officer, Hospice and Palliative Medicine, was honored with the Physician of Distinction Award by the Community Medical Center Foundation. Dr. Holler was recognized for her dedication, commitment and extraordinary patient care as well as the education of healthcare professionals and the community on end of life care

The number of participants in the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC) increased 9% (33,062 participants).
Public Health Community Activities
2,127 COVID vaccines, 53 MPOX vaccines, and 692 Flu vaccines were administered throughout three counties (Middlesex, Monmouth, and Ocean).
New Grants for Seniors
CFHI was awarded two new grants from Monmouth County Board of County Commissioners to support seniors and caregivers. The Health Screening and Education for seniors program ($78,610) will complement the current Senior Wellness Nurse program providing education sessions; and the Professional In-Home Support and Education for Caregivers ($160,000) will support seniors and their caregivers in the home through evidence-based curriculum and assessment of needs.
Prevention Resource Network
Provided 1,144 Rapid HIV Tests, diagnosing 4 new individuals with HIV who were linked to treatment the same day.
• The Transgender Empowerment Program has assisted over 50 transgender individuals with advocacy, name changes, legal document changes, and referrals for support and gender affirming medical care
• 74 unduplicated clients received pre-exposure prophylaxis (PrEP) counseling services and were referred to a culturally competent healthcare provider
Harm Reduction Center (HRC)
Provided 606 unduplicated clients syringe access and harm reduction services.
• 99,954 syringes were dispensed, 17,271 were returned
• 3,359 Narcan kits were distributed in the community
• The Low Threshold Buprenorphine Program supported 231 unduplicated clients, providing 1,627 encounters
• In September 2023, PRN HRC launched its Mobile HRC Unit, providing syringe access and harm reduction services daily throughout Monmouth and Ocean Counties
Cancer Education & Early Detection Program (CEED)
• 1,537 cancer screenings were provided in Monmouth County and 1,148 screenings were conducted in Ocean county. Through the collaborative effort with the Cristine Meredith Miele Fund, screening and diagnostic services were extended to 124 patients who exceeded the CEED eligibility criteria
• $50,000 of funding from the Calder Foundation earmarked for the development of educational print materials, advertisements, and billboards for CEED produced over 10,000 informative materials focusing on breast cancer education and prevention, particularly in young women
VNACJ Community Health Center

Total Patient Visits 2023: 60,028 a 16% increase from 2022.
Total Unique Patients 2023: 17,694 a 14% increase from 2022.
• Achieved Centering Pregnancy Program Accreditation at Freehold Family Health Center
• Successful triennial HRSA Operational Site Visit with zero conditions
• Completed Red Bank CHC relocation and service expansion, doubling primary medical capacity and adding dental services
• Received congressional funding for expansion projects in Asbury Park and Freehold

• The LGBTQ Center for Health and Wellness (LGBTQHC) is a program located in the Asbury Park Clinic and the first of its kind located in a Federally Qualified Health Center in the state of New Jersey. There was a 21% increase in unduplicated patients from 2022
