About the Valley Medical Center Foundation The Valley Medical Center Foundation (VMC Foundation) is an independent, community-driven 501c3 nonprofit organization founded to support and protect Silicon Valley’s most vital public healthcare institution – Santa Clara Valley Medical Center Hospital & Clinics. In partnership with SCVMC, the VMC Foundation raises philanthropic dollars to improve existing services, pioneer new models of care, and advance the cause of high quality healthcare for all. Our vision is that one day, with the help of the VMC Foundation, Santa Clara County’s public healthcare system will be fully resourced such that everyone in our community has access to comprehensive, high quality healthcare, regardless of background or ability to pay. Founded in 1988, the VMC Foundation has raised $85 million in support of vital health programs and services since inception. The VMC Foundation also plays an important advocacy role to educate the public and opinion leaders on the role of SCVMC in the community.

About the Valley Medical Center Foundation

In addition to fundraising, the VMC Foundation maintains an event planning and program management arm to assist SCVMC with the execution of special events, conferences and other activities, as well as directly operating programs such as Reach Out and Read, patient wireless internet access, the VMC Farmers’ Market and more.

Lastly, the VMC Foundation performs a vital administrative role within the SCVMC eco-system. This function includes project management for grants, administration of 150 unique funds and the preparation of financial statements, audits and tax filings as required. *Federal Tax ID 77-0187890

About Santa Clara Valley Medical Center Santa Clara Valley Medical Center Hospital and Clinics (SCVMC) is an integral part of the public healthcare delivery system in Santa Clara County, emphasizing quality care, research, teaching, innovation, and most importantly, a focus on a positive patient experience. For decades, the mission has been to provide high quality accessible healthcare and excellent service to all persons in Santa Clara County regardless of their social-economic status and ability to pay. SCVMC includes a tertiary level acute care hospital with 731 licensed beds, providing the highest level of adult and pediatric emergency medical and trauma services, a regional high risk Neonatal Intensive Care Unit, an ACS-verified Burn Trauma Center, a Primary Stroke Center, a CARF-accredited Rehabilitation Center, emergency and acute inpatient psychiatric services, as well as a range of other specialized services – in some cases the only such treatment in the region. The medical center hosts five highly competitive residency training programs and partners with Stanford University Medical Center for the training of residents and fellows in many Stanford-based specialties. SCVMC also offers high quality clinical training experiences for more than 30 non-physician healthcare professions, including exceptional clinical training for more than 1,000 RNs annually. SCVMC provides a range of outpatient primary and specialty care services. Its large primary care network includes eight health centers located throughout the county, four urgent care clinics, comprehensive dental services and numerous community based health services offering care to young adults, individuals experiencing homelessness, and other vulnerable members of our community.

A critical healthcare delivery system for the region, in 2017 SCVMC care teams served more than 275,000 individuals, performed more than 10,000 surgeries, admitted 25,000 patients for acute inpatient hospital stays, delivered 3,200 babies, and provided more than 800,000 medical treatments in the ED and other outpatient settings. SCVMC is part of the County of Santa Clara Health System (COSCHS), the second-largest County-owned health and hospital system in California committed to improving the health of the 1.8 million residents of Santa Clara County. As an integrated health care system, COSCHS comprises the Santa Clara Valley Medical Center (SCVMC), Behavioral Health Department, Public Health, Emergency Medical Services (EMS), and Valley Health Plan.

About Santa Clara Valley Medical Center

SCVMC also offers comprehensive specialty care outpatient services, including the Sobrato Cancer Center at Valley Specialty Center, a Renal Care Center, and a Diabetes Center.

The Best Darn Ship One of my favorite books on leadership is by D. Michael Abrashoff, a retired U.S. Navy Captain known for rewriting the manual (literally and figuratively) on how to lead in the modern military. In It’s Your Ship, he reflects on the best strategies to foster good communication, discipline and a hyper focus on getting results onboard the destroyer USS Benfold. His mantra, to make his boat “the best damn ship in the best damn Navy” struck a cord with me (as my staff can attest – I say it all the time, except I always say darn because I’m a nonprofit CEO and not a Naval Officer). I believe that being excellent for excellences’ sake is a virtue, and we’ve made that notion core to our culture here at the Valley Medical Center Foundation. It’s why we spent considerable time and effort in 2017 doing things that might not sound exciting, but in service to the concept of becoming that “best darn ship.” From building new systems for fundraising and financial management, to adopting a new Code of Ethics and policies to maximize donor privacy, program efficacy, and customer responsiveness, 2017 was a milestone year in our ongoing quest to be the best organization we can be.

Letter from the CEO of the Valley Medical Center Foundation

This work was inspired in part thanks to our partnership with Santa Clara University’s Markkula Center for Applied Ethics. Together, we’ve worked to review and improve our policies and procedures, develop our board and staff, and improve upon all aspects of our operations. The end goal is an organization better focused on our mission, responsive to donor and community concerns, and governed by the highest possible industry standards.

6

If this doesn’t sound exciting, let me remind you: our greatest strength is your trust in us. I am proud to have earned it, and will work every day to ensure we keep it. Again, my deepest appreciation! In Service,

E. Christopher Wilder, CEO Valley Medical Center Foundation

From Board Chair to Patient A critical role for any non-profit Board member, in particular the Board Chair, is to strongly advocate for the organization. For many this is easy if the product is visible. But for a service organization this can be much harder to quantify. When it comes to SCVMC, I no longer have this concern, as less than two months after being elected as Board Chair of the Valley Medical Center Foundation, I found myself spending nine days enjoying everything that SCVMC could offer as a patient.

So what was my experience? Simply put, amazing. If any hospital stay could be considered incredible, this was it. The quality of care was almost beyond belief, driven largely by the staff that I now consider part of my extended family. Why? Because that is how I was treated by everyone, from management, doctors, nurses, nursing assistants, and environmental services staff; they all made me feel at home, with constant smiles and caring. I was made to feel that I was the only person in the world that mattered to them. Trust me, that can be a huge part of overcoming any illness. That we deliver this quality of care to anyone in Santa Clara County, irrespective of ability to pay, is wonderful and something, as a community, we should be incredibly proud of. My experience has now made it even easier for me to aggressively advocate for the VMC Foundation and SCVMC and I am committed to work as hard as possible to continue to strengthen and broaden the services offered to our community. Chris Worrall, Retired Hitachi Executive Chairperson, Board of Directors Valley Medical Center Foundation

Letter from the Board Chairperson of the Valley Medical Center Foundation

It started with the loss of feeling in my limbs, such that I was unable to move. I insisted to the paramedics that I be taken to the Emergency Department at SCVMC, because I knew it was one of the highest rated in the region. There I spent 11 hours undergoing testing and observation before being admitted and transferred to a medical unit on the 6th floor of the new Sobrato Pavilion. Five days later, I was moved to the 3rd floor Acute Rehabilitation Unit, where my care now included many hours with occupational and physical therapy. I am grateful to have made a full recovery.

n a hospital setting, art is not for decorating. It’s for healing. When the Sobrato Pavilion opened in 2017, the VMC Foundation funded the entire art program to bring beauty, delight and tranquility to all parts of the building. Over 40 unique pieces of art now adorn the facility. For staff, visitors and patients, it provides a respite from the difficult day-to-day realities of their work and recovery. It represents an essential component of the healing mission of the building, and the Foundation is grateful to the hundreds of donors who gave to support this initiative.

Screening for Better Vision

Screening for Better Vision

W

ith funding from Sunlight Giving, all SCVMC pediatric clinic locations were outfitted with specialized vision screeners in 2017. The device allows providers to quickly and easily detect vision issues for patients as young as 6 months old. SCVMC pediatric clinics see 125,000 visits from children annually, predominately from low-income areas of this community. As such, these devices will have a significant impact on children at-risk of long-term vision disorders. Vision issues are a particular concern for young children as they enter school. It is common for these disorders to go undiagnosed, resulting in learning and behavior interventions that do not address the root cause of the problem. These devices add a highly effective tool for SCVMC caregivers to ensure that all kids in this community are school ready. Pictured: Alisha Hastings, RN, demonstrating the device on a young patient

Tattoo Removal Laser ince 1994, Santa Clara Valley Medical Center and the City of San Jose have partnered to offer free tattoo removal services to former gang members as part the Clean Slate Program. Tattoos are a visible part of gang culture, and leave former members marked with the threat of violence and suspicion unless they are removed. The process is not fast or easy, but effective. The VMC Foundation has long played a role in providing equipment and supplies thanks to the generous support of program donors. In 2017, the VMC Foundation purchased a new machine that significantly reduces the time for each tattoo removal. With that efficiency, the program can now provide support to victims of human trafficking who have been “branded” by the perpetrators of this crime.

Zero G Helps Patients Walk Again

hen the Sobrato Pavilion opened at Santa Clara Valley Medical Center in 2017, it became the new home to the nationally recognized Rehabilitation Center for spinal cord, traumatic brain injury and stroke patients. As part of the $10 million fundraising campaign that supported the new center, the VMC Foundation provided funding for the purchase of new, cutting-edge therapy equipment, most notably the Zero G Balance and Gait System. This unique system, deployed in only a few centers in the United States, allows patients to attempt more aggressive forms of therapy earlier in their rehabilitation. Pictured: Patient Matt Wetschler. Remarkably, Matt’s day job is emergency medicine physician at SCVMC. After sustaining an injury to his spinal cord while surfing, he became a patient. His spirit and perseverance personifies everything that this institution stands for, and we are humbled that our donors played a (very small) role in his recovery.

The 2018 edition, based on the Halloween inspired theme of “A Wicked Affair,” promises to be just as spectacular. Join us on October 6, 2018, at Villa Ragusa in Campbell, CA. For more information, visit vmcfoundation.org/gala.

Biggest Gala Ever

t the first VMC Foundation Annual Gala and Auction in 2006, 125 attendees participated, raising close to $40,000. It was considered a great success. Since that time, the VMC Foundation has worked hard to grow this event into one of the premiere charity banquets in the Valley, befitting of the stature and importance of the institution it serves. Thanks to the generosity of sponsors, SCVMC staff and hundreds more, 2017 represented a significant step towards that long-term goal. The biggest and most successful Gala and Auction to date, the 2017 edition, a jazz-age themed extravaganza, topped 500 attendees for the first time, bringing in a total revenue of $350,000.

he VMC Foundation is grateful for the 2,114 individuals, companies and organizations that donated in 2017. We are pleased to spotlight five donors in particular for their exceptional support. Each has their own story. A SCVMC occupational therapist inspired to do more, a local martial arts organization that makes charity the core of their mission, a former patient paying it forward, a healthcare technology titan that wants to give back, and a homegrown entrepreneur, proving you can do well and do good at the same time. Their stories are just a few of the many that made 2017 so special.

t is almost always in the back of the mind of any cyclist – a car door opening unexpectedly in your path. But that did not deter David Lieberman from commuting on his bike in Berkeley, where he lives and works as a professor in the School of Law at the University of California. Then one day, on his way home, it happened. A car door opened without warning, and David’s life was forever changed. He was rushed to the nearest trauma center at Highland Hospital in Oakland – like SCVMC, a public facility. His injuries were significant, and it was not immediately clear how much leg and arm movement he would regain. The care team at Highland, along with family and friends, strongly recommended the Rehabilitation Center at Santa Clara Valley Medical Center, known nationally for the care of individuals with spinal cord injuries. Thanks to his care team at SCVMC, the support of his family, and David’s hard work, he has made a profound recovery. Grateful for the care they received, David and his wife Carol made a donation to the VMC Foundation in support of the Rehabilitation Center, and will have a patient room dedicated in their honor.

“I feel hugely indebted for the care I received,” David said. “We were proud to give.”

his Wisconsin-based company is changing healthcare as we know it, providing the underlying technology that is powering electronic health records at most major health systems in Northern California, including at Santa Clara Valley Medical Center. For patients that use an online portal for access to messaging, appointments, test results and prescription refills, it is likely thanks to technology developed by Epic. It often takes years for fast growing companies to incorporate philanthropy into their normal business operations. Epic has done so in a thoughtful and high impact way. In 2017, they made a major, six-figure gift to the VMC Foundation to recognize the vital and important work of Silicon Valley’s most essential health system.

Since 2016, Armature Systems has been one of the largest sponsors of the VMC Foundation Annual Gala & Auction. In 2017, Jacob joined the VMC Foundation Board of Directors, and Armature Systems will be back as a Premier Sponsor of the 2018 Annual Gala and Auction. And in so doing, he is setting a new standard for giving in the Valley, showing that it is never too soon to make a difference in the community.

“I fundamentally believe health care is a right for every human being and our children will judge us on how we treat people who are unable to take care of themselves.”

t first read, it is a classic Silicon Valley success story. A young entrepreneur takes a risk by leaving his job at an established technology firm to build his own company. Starting from his home, through smarts and hard work, he finds success. But for Jacob Elziq, founder and CEO of Armature Systems, that’s not the entire story. His company, a provider of network engineering, security and wireless services, is now headquartered in Sunnyvale, with a customer list full of Fortune 500 firms. You might think that all of his focus and attention would be on future growth. But here’s why Jacob is unique. He has made giving back a priority now. In a region where so many new companies make philanthropy an afterthought, Jacob decided to get involved in causes he cares about right away, including the VMC Foundation.

ne of the leading martial arts centers in the South Bay, San Jose-based World Class Tae Kwon Do has a long tradition of charity giving as part of their youth programs. As a reflection of their philosophy to “positively touch the lives of as many people as we can,” World Class Tae Kwo Do organizes an annual “Board Break-a-thon” whereby participants break wooden boards using Tae Kwon Do as part of the national “Kicking for Miracles” fundraising campaign. Children’s health has long been a focus for World Class Tae Kwon Do. After learning that that the VMC Foundation was working to create a Women and Children’s Center at SCVMC, they selected the VMC Foundation as the beneficiary for the Board Break-a-thon in 2016. The following year, they supported the VMC Foundation again, raising nearly $30,000. Funds will be used to support women and children’s health programs at SCVMC, ensuring access to essential health services to those most in need, regardless of ability to pay.

M

ichelle Tipton-Burton has seen a lot of change since her first day at Santa Clara Valley Medical Center back in 1988. Trained as an Occupational Therapist, Michelle first worked in acute care units before transferring to SCVMC’s nationally recognized Rehabilitation Center, and has stayed ever since. Inspired by the mission of SCVMC to provide high quality care for all, regardless of ability to pay, Michelle found her passion in her patients. Above and beyond her regular work, she looked for ways to do more for the people she served. She helped to co-found two organizations – OPhoenix and Into the Woods – to provide additional resources and services to rehab patients. With the VMC Foundation, she helped launch the Technology Access Program to provide rehab patients with access to assistive technology to enable communication with home, school and work. From organizing fundraisers to volunteering her time to support events, Michelle has been a tireless advocate for her patients. A long-time donor, Michelle and her family made a generous contribution in 2017 to dedicate a room in the new Rehabilitation Center.

“I found by participating in nonprofits like the VMC Foundation, I was able to give back on a more personal level. It’s a very small contribution that not only brings me joy and satisfaction, but I also consider it a privilege.”

The Cory Society is named for Santa Clara County’s first physician, Dr. Benjamin Cory. At just 25 years old, Dr. Cory ventured across the plains by ox cart in search of a community where he could put his skills to use. He ended up in the beautiful Santa Clara Valley, where he rallied the community to establish the Santa Clara Infirmary – the region’s first hospital, built in 1876 on the land where SCVMC still stands today.

INDEPENDENT AUDITORS’ REPORT To the Board of Directors of VMC Foundation, San Jose, California We have audited the accompanying financial statements of VMC Foundation (the “Foundation”, a California nonprofit public benefit corporation), which comprise the statements of financial position as of December 31, 2017 and 2016, and the related statements of activities and changes in net assets, functional expenses and cash flows for the years then ended, and the related notes to the financial statements. Management’s Responsibility for the Financial Statements Management is responsible for the preparation and fair presentation of these financial statements in accordance with accounting principles generally accepted in the United States of America; this includes the design, implementation, and maintenance of internal control relevant to the preparation and fair presentation of financial statements that are free from material misstatement, whether due to fraud or error. Auditors’ Responsibility Our responsibility is to express an opinion on these financial statements based on our audits. We conducted our audits in accordance with auditing standards generally accepted in the United States of America. Those standards require that we plan and perform the audit to obtain reasonable assurance about whether the financial statements are free from material misstatement. An audit involves performing procedures to obtain audit evidence about the amounts and disclosures in the financial statements. The procedures selected depend on the auditors’ judgment, including the assessment of the risks of material misstatement of the financial statements, whether due to fraud or error. In making those risk assessments, the auditor considers internal control relevant to the entity’s preparation and fair presentation of the financial statements in order to design audit procedures that are appropriate in the circumstances, but not for the purpose of expressing an opinion on the effectiveness of the entity’s internal control. Accordingly, we express no such opinion. An audit also includes evaluating the appropriateness of accounting policies used and the reasonableness of significant accounting estimates made by management, as well as evaluating the overall presentation of the financial statements. We believe that the audit evidence we have obtained is sufficient and appropriate to provide a basis for our audit opinion. Opinion

Financial Statements

In our opinion, the financial statements referred to above present fairly, in all material respects, the financial position of VMC Foundation as of December 31, 2017 and 2016, and the changes in its net assets and its cash flows for the years then ended in accordance with accounting principles generally accepted in the United States of America.
