It doesn’t seem so long ago that every time an optical event was announced, you would hold your breath to see if it actually went ahead. Thankfully those days appear to be behind us – with home-grown and global optical fairs and conferences forging full-steam ahead.

If you’re not able to visit one of the many fantastic European trade shows this year, then do make a beeline for 100% Optical in London this month where NEG’s preferred suppliers will be out in force with their latest product innovations and practice support tools.

With more than 140 CPD sessions on offer, there’s also a great opportunity to hand-pick relevant education – both within the general programme and via show participants. Eleven different education hubs will support all training needs, so bring the team and make it a practice outing. With its statement designs in high-end materials, the spring/ summer 2023 collection by Ana Hickmann explores elegant constructions and architectural details, imagined through references to the iconic pyramids of Egypt. Elegant crystals, bevelled details and a palette of colour combinations are among the highlights to look for.

The brand’s Duo-Fashion concept returns allowing a rotational movement on the sides for two engaging looks in one single frame. Our photograph shows model AH1504: a round frame front combined with the Duo Fashion sides. The temple pattern is based on a tri-coloured rainbow design with art deco overtones. Colour for this style include a light elegant pink tone or a classical and timeless black.

General Optical Council

From April 2023, there will be a below-inflation 5.56 per cent increase in the main General Optical Council registration fee for optometrists, dispensing opticians and bodies corporates – to £380. Fees for students will remain the same and the discount for low income fees will increase by £20, from £100 to £120, meaning that the low income fee will remain at £260. Registrant fees have remained frozen at £360 for the past three years.

Menicon

The new Rose K2 XL Oblate design from Menicon is designed to give better vision and comfort to patients following grafts, photorefractive keratectomy or laser surgery interventions. The daily wear lens is suited to any oblate cornea that cannot be fitted within the limbus, including extremely flat corneas, when the tear layer thickness is excessively deep over the central cornea.

With an aspheric back optic zone, front surface aberration control, precise edge lift control and significant reverse geometry in all base curves, which increases as the base curve flattens, the lens mimics the shape of the oblate cornea for every stage of the condition. As a corneo-scleral design, Menicon claims it provides less conjunctival pressure with increased tear exchange resulting in potentially less hypoxia and ischaemia. CooperVision

In partnership with eyecare professionals (ECPs), contact lens wearers and Plastic Bank, CooperVision’s plastic neutrality initiative has now prevented the equivalent of more than 100 million plastic bottles from entering the oceans. Of these, more than 23 million have been offset by ECPs and contact lens wearers in the UK and Ireland.

CooperVision funds Plastic Bank’s collection, processing and reuse of general ocean-bound plastic waste that is equal to the weight of the plastic contained in participating products across 27 countries throughout the Americas, Europe and Asia-Pacific. The initiative also benefits community members in more than 350 coastal communities who receive bonus payments through Plastic Bank for necessities such as groceries, cooking fuel, school tuition and health insurance.

CooperVision country manager for the UK and Ireland, Doug Bairner, commented: “We’re extremely proud of the contribution eyecare professionals and contact lens wearers have made to this important initiative in the UK and Ireland. It really goes to show that small actions can make a massive difference when we all work together with a common purpose. ABDO

The Association of British Dispensing Opticians (ABDO) has developed new menopause guidance addressing symptoms, legislation and the responsibilities of employers and employees. It offers guidance for managers’ discussions with employees and suggested adjustments for specific issues. There is also a template for a confidential discussion between manager and employee, and a list of supporting resources.

ABDO clinical lead, Max Halford, said:

“ABDO is committed to ensuring the health, safety and well-being of its members, employees and the patients we care for, ensuring everyone is treated with dignity and respect. With this is mind, the organisation has developed guidance to support members who are affected in any way by the menopause, to support and inform their managers and ensure that people reporting issues are treated fairly and given appropriate support.” Find the guidelines in the ABDO Advice and Guidance section of the ABDO website.

Visionix has teamed up with BIB Ophthalmic instruments to distribute its new Essential Line product range in the UK. An innovator in ophthalmic diagnostics for more than 20 years, Visionix is said to have more patents in wavefront technology than any other company in the

world – with more than 10,000 wavefront diagnostic instruments installed in the field. The company pioneered a method to miniaturise wavefront technology, permitting its incorporation in a large group of instruments.

Providers of existing GOC-approved qualifications are required to submit detailed plans of how their adapted qualifications will meet the new education and training requirements, with the GOC reviewing and noting these changes. Transforming optometry and ophthalmology with artificial intelligence and exploring how hybrid home care is enabled via technology, are just two of the free to attend lectures on the Optical Suppliers Association (OSA) stand at 100% Optical this month. The OSA presentations will be delivered by some of the most respected authorities in the UK, including Professors James Wolffsohn and Pearse Keane. These treatments can keep people out of busy hospitals, provide very localised care and build up optical practices.”

Sarahjane Cross has taken over as chief executive officer at UltraVision CLPL following the departure of Diane Angell, who served as CEO for 11 years during which time the group was acquired by Japan-based Seed Co. Sarahjane joined UltraVision CLPL as group finance director in 2019, and successfully led key projects relating to governance, operational efficiencies and productivity across all areas of the business.

Eyecare professionals can now take advantage of ‘early bird’ rates and register for this year’s British Contact Lens Association (BCLA) Clinical Conference and Exhibition, taking place at Manchester Central from 9-11 June. The event will feature keynote speakers including Frank Schaeffel and Dr Gregory Sawyer. The BCLA Medal Address will be presented by Professor Eric Papas while Dr Sonia Trave-Huarte will deliver the Irving Fatt Memorial Lecture. There will be dedicated lecture tracks, hands-on workshops and free sessions at the exhibitor pavilion. At least 45 CPD points are expected to be available.

International Opticians Association

The International Opticians Association (IOA) is holding a summit alongside Silmo Singapore from 11-16 April. It will be the first time IOA members have met face-to-face since 2018. General Optical Council

The General Optical Council (GOC) is seeking views on a new policy setting out the process for registrants to update their registered gender. The new policy seeks to make the process for updating gender “clear and easy to apply”, whilst ensuring compliance with the Gender Recognition Act 2004 and the Equality Act 2010. The consultation is live until 20 March. Feedback will then be considered, with a view to implement the policy in mid-2023. The consultation can be accessed via the GOC website.

AN INDEPENDENT VIEW

Building on our strengths

The start of a new year always brings the opportunity to both reflect on the previous year’s events and look forward to what may develop in the coming months.

Last year saw the dust continue to settle following the pandemic as businesses inched back to normality, albeit with some long-term changes. Both clinicians and patients are more acutely aware of infection control in all walks of life. Whilst optometry has always had a clear emphasis on this, our patients certainly have a much stronger expectation to see it in action.

There is also much greater awareness of mask wearing, so hopefully we can say goodbye to catching every cold and flu bug under the sun when patients arrive coughing and spluttering for their appointments. In all seriousness, this small change can potentially save a lot of lost clinic hours.

So what of the year ahead? We seem to have shifted from one global catastrophe to another, as we emerge from the pandemic into the ongoing conflict in Ukraine and the repercussions of that. The cost of living crisis may well cause patients to stop and consider where their disposable income is spent, so may we see a shift towards more budget vision correction solutions and online retailers?

Patient education is key here and we should continue to promote and explain exactly why independent practice is so comprehensive and patient focused. We must also consider the impact on business overheads. Rising energy costs will be felt across the sector, and it is important to remember that help is always at hand through our own Practice Support Network.

But it’s not all doom and gloom. Having seen how the independent sector weathered the storm of Covid, emerging stronger than ever, I feel the same can be said for the current situation. The absolute joy of being an independent practice is the ability to adjust and refine the business to respond to local demand. This is the strength that will continue to see the independent sector grow in the months and years ahead.

Finding your way forwards

When the going gets tough, the tough get planning...

The news never seems to get any better does it? With the ongoing war in Ukraine, the cost-of-living crisis and the growing pressures on the NHS, it looks like 2023 is going to be another difficult year for many people –both personally and in business. With the additional pressures that seem to befall independent practices, you might wonder why you bother at all?

But let’s look past all the doom and gloom and focus on the positives that come with being part of the independent sector – and what actions that can be taken to grow in challenging times. First of all, don’t bury your head in the sand thinking it will all go away. Instead, make an action plan to work around/offset any issues you’re having, or just tackle them head on.

Take, for instance, the increasing cost of gas and electricity: you might think there is little cost-difference between providers but there are savings to be gained. We recently came to the end of our utilities contract, and decided to shop around. Although our costs still tripled, if we hadn’t switched provider they would have been a staggering 25 per cent higher.

So if you are coming to the end of a utilities contract, do your homework and find the best deal. Get your staff involved too: see if they can come up with ways to save power, from fitting energy-saving light bulbs and turning lights off to maybe using remote edging rather than in-practice glazing. Or ask them to find ways to update equipment to be more efficient.

GET YOUR PRICES RIGHT

We know that costs are increasing across the board, so look at the prices you charge patients and bite the bullet. Increasing prices is always contentious, because it can drive patients into the arms of cheaper providers. But there will always

be providers who are cheaper than you –so instead of focusing on cost, focus on offering value and ways to make paying for products and services as easy as possible.

As an independent practice, patients come to you for your excellent service and the wide array of products you offer – so why not make an even bigger point about these attributes. Make sure everyone gets the time they need and that you discuss all the options available. When people are engaged in the process, they respond positively and will remain loyal.

For some of your patients, cost pressures are particularly hard at the moment, so make it as easy as possible for them to pay. If you haven’t already introduced monthly direct debit options, make sure you do – not just for contact lenses but for professional fees and spreading the cost of purchases.

For instance, why not offer three or six-month interest-free options on frame purchases – providing you take a reasonably sized deposit. I recommend charging a 50 per cent deposit so your costs are covered. Enabling your patients to spread payments will make it easier for them to make those crucial purchasing decisions. You’ll be surprised how very few patients, if any, don’t pay or cancel their direct debit with you.

Offering this service also makes it easier to promote additional purchases when the patient comes to the end of their payment. This provides an opportunity to contact them to come in and look at your latest sunglasses collection, for example, or to consider a pair of frames for sports. Once again, allow them to spread their payments. This may affect your cash flow but once up and running, the overall increase in income

These are just a few ways to address specific areas of concern you may be facing at the moment, but by working through these issues you will find a way forwards. And always remember: you might be independent but you’re not on your own. Hoya is on a mission to curb myopia by providing professional education such as CPD and ongoing support with practice team training, raising consumer awareness and increasing holistic advice to parents and guardians, and enhancing the parent and patient journey.

Over recent months Hoya undertook a two-month awareness campaign targeting parents of youngsters across multiple channels such as Facebook, YouTube, Mumsnet and Google. Extending its campaign to millions of people across the UK and Ireland, the aim is to help them better understand what myopia is and

how new-generation spectacle lenses could help their child. All communications encouraged parents to book an eye examination for their child with their local independent MiyoSmart-accredited optician.

Hoya also recognises that there is a large disconnect between the importance of children’s eye health and the school curriculum. Hoya’s ‘Eye Care We Care’ scheme enables independent ECPs to partner with local schools to promote the importance of eye health and their practice in their local schools and community effectively.

MAKING A REAL DIFFERENCE

The Hoya team attended the glamorous black-tie Optician Awards in December at the Hilton Park Lane, London, alongside more than 500 attendees from across the eyecare industry. The team were thrilled to win the 2022 Lens Award, and would like to thank all ECPs who continue to join them on their mission to curb myopia.

DeNoise Algorithm

Improved visualisation of all tomograms comes with the new Artificial Intelligence (AI) DeNoise algorithm, which filters out noise from the tomogram for the highest and smoothest image quality. The AI DeNoise algorithm enhances the visibility of morphological structures by processing the

original image, and provides greater detail within the retinal layers thereby improving clinical diagnosis.

IOL Formulas

Measuring axial length utilising the optional biometry measurement software has until now only been for used for visualisation and measurement, ideal for those involved in myopia management. For clinics that want intraocular lens (IOL) calculations and formulas, Optopol has now made

Optinet FLEX: an update

Optinet FLEX encompasses all aspects of the day-to-day running of an optical business – from maintaining patient records, handling eGOS claims, scheduling appointments (including automated SMS confirmations and reminders), recording of the clinical assessment and examination results and creation of patient dispense and till receipt.

From a back-office perspective, full financial reporting is available; uploading financial transactions to the Xero accounting software is automated and the ability to interrogate business data to send communications to patients by letter, postcard, email or SMS is also invaluable.

Further enhancements are planned for next year’s release – so watch this space.

Whilst we are committed to ongoing software development, we are fiercely proud of the service and support that we offer, starting with the initial software demonstration which, where possible, takes place in the practice – or online if preferred. By visiting the practice, we get a feel for the business, meet the owner and the staff and start to plan delivery of the software and training.

In focus: the Visual Fixation System

The Visual Fixation System uses a mobile phone to display personalised targets for patients who find eye tests and focusing uncomfortable or challenging. Optometrist Simon Berry, who invented the system, explains how it works...

The Visual Fixation System (VFS) allows the clinician to complete clinical measurements more effectively with patients whose engagement may be variable – such as in patients with a disability, young children or patients with dementia.

The system uses a mobile phone to display a specific target that will engage and/or relax the patient. The target is placed in direct line of sight of the clinician, allowing a number of clinical observations to be made.

The VFS consists of a main unit and various different handles. The handles are interchangeable and should be selected depending on the clinical test required. There are currently two different handles for use with the device, with more under development.

USING A PHONE AS A TARGET

The VFS is compatible with most mobile phones. The flexibility of using a mobile phone means that an unlimited number of targets can be displayed and tailored to the particular needs of the patients. The sound from the phone can also be used to engage the patient.

■ For a young child, their favourite TV show can be played through the phone, or photographs of family members can be shown. The parent’s phone can be borrowed and used for this purpose if needed

■ For a person with a learning disability, a target can be chosen that has the best chance of engaging the patient. This can be tailored to the patient’s interests

■ For an older person suffering with dementia, a clip of their favourite film or musical can be played. Music and photos can be displayed together

It should be noted that due to the optics of the device, the target shown to the patient will be a mirror image of that shown on the phone. This should not matter when displaying photos or videos, however, if the clinician wishes to display text then it will need to be adapted to be in the correct orientation for the patient to view.

To fit a mobile phone into the VFS, there is a sliding drawer that the phone fits into. It is advised to increase the brightness of the phone, disable any screensaver and change the orientation of the phone to landscape. The phone can be kept loose within the cartridge. However, this does mean that care needs to be taken when setting the VFS down on a desk.

Alternatively, the cartridge that holds the mobile phone features a velcro pad to keep the phone locked in place. A matching velcro pad can be stuck onto the back of the mobile phone. This will keep the phone more secure if required. Once the mobile phone is secure and displaying a target that will engage the patient, the VFS can be used for various different tests.

MEASURING ACCOMMODATIVE LAG

Dynamic retinoscopy is a technique that allows the clinician to measure the patient’s accommodative response to a target. This is a useful measurement for many paediatric patients. It can help prescribing decisions for low to moderate hypermetropic prescriptions, and is a useful measurement when considering myopia control and the risk of a patient becoming increasingly myopic.

It is also known that certain patients are more prone to having problems with their accommodative response. For example, patients with Down syndrome or cerebral palsy are more likely to have an accommodative lag and these patients should be screened for the condition.

The VFS is the only device on the market that allows the clinician to examine directly along the visual axis. This allows accurate

measurement of the accommodative response.

There are two quantitative ways of measuring accommodative lag. These are referred to as the monocular estimation method (MEM) and the Nott method. There are other methods sometimes described but they are modifications from these two basic techniques. The VFS allows measurements by both techniques. The clinician simply has to change the handle depending on their preferred method of measurement.

OTHER CLINICAL OBSERVATIONS MEASUREMENTS

The VFS is useful as a means to grab a patient’s attention. It becomes invaluable in patients where co-operation is difficult, and allows the clinician a method of observation when other methods fail.

It can be used for a variety of different clinical observations and some of those observations are described below. These are not meant to be an exclusive list of possible tests but just descriptions of some of the techniques already used.

It should be noted that whilst some of the techniques described below might initially appear to be a crude measurement, in situations where patients cannot engage with the clinician any other way these methods are better than not being able to take any measurement at all.

GROSS EXAMINATION

The VFS allows a gross examination of the ocular structures. It can be used, for example, to assess the fit of a spectacle frame, to take facial measurements or bifocal lens measurements.

It can be used with a 20D lens to increase the magnification and allow more accurate assessment of the external eye structures. This is useful for evaluating contact lens fits, or corneal assessments. It can be also used as a gross assessment of binocularity by observing the corneal reflections from the mobile target.

CHECKING PRESCRIPTIONS

Once a prescription has been dispensed, if the result has been slightly rough due to the patient’s co-operation, then the VFS can be used to check the result after it has been prescribed. Anisometropia can be checked because with the prescription in place the reflex should be symmetrical between the right and left eye. Cylinder power and axis can be also be checked because with the correction in place a spherical reflex will be seen.

NEAR COVER TEST, PD AND OTHER MEASUREMENTS

The VFS can be used to take a variety of measurements whilst the patient is engaged in watching a target, for example, the near cover test, pupil reactions or 20D prism test. These measurements are crude, but better than not being able to complete the test. They can be made more accurate by keeping the target on the phone small. There is a balance to strike between cooperation of the patient, the size of the target and accuracy of the obtained result.

FUNDUS EXAMINATION

A fundus examination is possible using the VFS and a modified indirect ophthalmoscopy technique. An ophthalmoscope is placed in the instrument handle, set on 8D – depending on the preference of the clinician, the working range, and the field of view preferred when viewing the fundus. Using a 20D lens in front of the VFS and close to the patient, it is possible to obtain a view of the fundus using indirect ophthalmoscopy.

CYCLOPLEGIC REFRACTION

The VFS can be used for

cycloplegic

refraction. Whilst under cycloplegia, the patient would not be able to focus properly on a target, but they are still attracted to the target even if they can’t see it properly. The VFS and retinoscope is held in one hand, and the lens rack held in the other.

USE WITH A TEST CHART APP

The VFS can also be used for more co-operative patients. A test chart can be displayed on the mobile phone and measurements of the accommodative response can be made. This is particularly useful in myopia control when a more detailed accommodative target is required.

TIPS ON USING THE VFS

◼ Keep the room lights low. The more light behind the clinician, the more distraction there is for the patient viewing the target. When it is not possible to lower the room lights, there is an alternative mask provided for the viewing window. Using the VFS with this in place will make the target easier for the patient to view.

◼ Allow the patient a few moments to watch the target and adjust their viewing before starting any clinical examination. When a retinoscope beam is shone through the beam splitter, it is less distracting to the patient if they are already focused on the target.

◼ Be aware that many patients will reach for the target. They are being shown something that interests them.

◼ Keep the beam splitter clean to avoid stray reflections. This is particularly important when performing indirect ophthalmoscopy.

in Welsh: the frames are designed on the Welsh coastline and produced in Italy. For Anna, creating eyewear is a natural progression, as she is a jewellery designer and eager eyewear collector. The environment is also Anna’s passion, and her concept initially came together with the launch of Ffin sunglasses. Sustainability is a defining factor in how Anna wants to present and position Ffin.

Anna will be showcasing her designs at 100% Optical later this month. She says: “It’s really very exciting – and although it’s certainly been a learning curve, I do like challenging myself. We want to develop the brand, and would like to participate at Silmo and perhaps Mido in the future, as we see how the collection unfolds.” She is also adding ophthalmic frames to the collection.

With her jewellery background, Anna understands asymmetrical lines and attention to detail. “I’m a fan of monochrome tones, and black and white is my signature. However, for the next collection I’m thinking of design features with a flash of pink, or inky blue or greens.”

WAGING WAR ON PLASTIC AT WATERHAUL

Sea, surf and shorelines are the manifesto behind the launch of Waterhaul Eyewear. Marine scientist and founder, Harry Dennis, discovered how fishing gear and nets –which contain copious amounts of plastic – could be recycled into amazing eyewear that’s strong and durable.

The company was founded in 2019 and moved forward during the pandemic. Head of sales, Kieran Hill, explains: “2022 was quite exciting for us. We are a small team, and have an in-house approach meaning we are totally hands-on. We are growing organically and definitely getting better known.”

Kieran notes that for the moment, the focus is developing the UK market, but Waterhaul has plans to move into the US optical scene later this year. “However, people find us on the internet and we do have orders from around the world. We’ll also be exhibiting at 100% Optical in London.”

WELSH WOW-FACTOR WITH FFIN

Brave, bold and brimming with creative ideas, Anna Lewis launched her eyewear brand Ffin in 2022. “I like to take risks,” says the Welsh designer, “and in difficult times it is good to take risks.”

Kieran explains that abandoned fishing nets and those at “the end of their life” are gathered from Cornwall, and the high-quality plastic in the nets is totally sustainable. The contemporary design elements in the frames are sleek and timeless in both sunnies and opththalmic versions, with the sunglasses featuring Barberini polarised lenses. All packing is sustainable, with cork used for protective cases.

POSITIVE ENERGY AND GOOD KARMA

Director Lisa Laurent launched Good Karma Eyewear at 100% Optical in 2022, and will exhibit at this year’s event with bio-acetate designs plus a new men’s line and designs in metal. The brand is represented in UK outlets, with plans to expand internationally.

Lisa’s eyewear ideas were formulating for a long time, as she explains:. “The idea of an eyewear collection had been in my head for 15 years. I have an optical practice and discovered there was a gap in the market for frames for women with larger faces. Our models are real people who actually wear glasses.”

“We don’t have a fashion background, but we are very interested in nature and the environment,“ continues Ed. “When it came to creating a name for the company, we had a list in the hundreds. We finally decided our last name was probably the best.”

Bird is particularly proud to be the first UK B-Corp eyewear company, endowed upon labels that meet the highest standards of social and environmental impact. The brothers are personally interested in reducing their carbon footprint, and are ardent supports of communities in Malawi and Zambia.

Materials in the collection include bio-acetate and woods with plant-based dyes. Cleaning cloths are made from recycled plastic bottles, with all metals recycled. Castor bean seeds are used in the children’s collection – Birdies – that are adorable designs for little faces in cheerful colours including Ocean Blue, Yellow and Green. The frames are also extremely durable with shatterproof lenses.

Lawrence designs the collections, and frames for men and women include both ophthalmic and sunglasses in a variety of colours and shapes. Sustainable materials are utilised in wonderfully creative imaginings – such as bio-acetate teamed with cork.

“At the moment we are selling to mostly UK practices,” notes Ed. “However, we do have aspirations to be a global brand. Right now though, we are concentrating on the day-to-day.”
