The baby boom was just that, a surge of births following World War II. But now those babies of yesteryear are called the silver tsunami, a demographic surge of seniors. And bunches of them are Veterans. On July 27 the House Veterans’ Affairs Committee Subcommittee on Health held a hearing to discuss those Veterans’ access to home and community-based services to help them stay out of facility care as they age. “While America’s health-care systems recover from the pandemic, they are also contending with the enormous challenge of one of the largest generations in history — the baby boomers — entering their later years in need of care,” the chairwoman, Rep. Julia Brownley, said in opening remarks. “These seniors will live longer than any generation before them, but with more complicated health needs than any health system has ever had to contend with. “For VA, preparation for this silver tsunami is crucial since (those seniors) will ultimately make up nearly half of its patient population.”

Florida’s problem Florida is especially prone to the problem of providing Veterans with long-term care as they age. About 790,000 of the state’s about 1.5 million Veterans are 65 or older, according to the Florida Department of Veteran Affairs. And more head to the Sunshine State every year. The state is about to open the Ardie R. Copas State Veterans’ Nursing Home, the seventh state Veterans nursing home in Florida. The state will also open a 112-bed state Veterans nursing home in Orlando, likely in 2022. With those eight state Veterans nursing homes and one state Veterans assisted living facility, which offers a lower level of medical care than nursing homes, there’ll

be 1,102 state beds for aging Veterans. That’ll be about 2,950 beds short of the need, according to Department of Veteran Affairs projections. To get the 4,050 beds the VA figures Florida needs, the state would have to construct another 26 homes. The state built seven and the assisted living facility over 32 years. On the lowest end, state Veterans nursing homes are now at least $60 million each to construct. The federal government pays for 65 percent of the construction. The state would be looking at, on the low end, $520 million to undertake such an ambitious project of building 26 homes, which doesn’t include operating and staffing costs once they’re opened. Of course, inflation is driving up the cost of building homes every year. The state Veterans department is counting on Veterans getting aid and attendance benefits from the VA, so they can age in place and don’t have to seek the constricted supply of beds at state Veterans homes. Florida’s problem and proposed solution aren’t unique. “There are not enough beds or staff at institutional set-

tings in the country to meet the projected need,” Brownley, who’s a California Democrat, said at the subcommittee hearing.

Graying America Dr. Scotte Hartronft, the executive director of geriatrics and extended care at the Veterans Health Administration testified at the hearing. “For the first time in U.S. history, adults over the age of 65 are on pace to outnumber children under 18 by 2034,” he read from his written testimony. “As Veterans age, approximately 80 percent will develop the need for long-term services and supports.” Hartronft’s written testimony said the VA is especially prone to the graying of America. “Most of this support in the past has been provided by family members, with women providing most of the care,” Hartronft’s congressional testimony reads. “The aging of the Veteran population has been more rapid and represents a greater proportion of the VA patient population than observed in other health care

systems.” Rashi Romanoff, vice president for program and partnerships at the Elizabeth Dole Foundation, told the subcommittee said the VA should be thankful for, and shouldn’t neglect, the family members and friends who’ve been providing a lot of care to aging Veterans. “According to a 2020 (Government Accountability Office) report, the number of Veterans aged 85 and older is expected to grow to more than five times over the next 20 years,” she said. Romanoff added, “Already each year, our Veteran family caregivers are providing more than $14 billion in uncompensated care to our Veterans.” Romanoff added a warning. Counting on families’ goodwill when many have other responsibilities and need to maintain employment is precarious. “If not for the dedication and sacrifice of these family members, publicly-funded systems like the VA, Medicare and Medicaid would be simply overwhelmed with requests from aging Veterans,” she

On July 15, 2021, the National Medal of Honor Monument Act unanimously passed through the U.S. House Natural Resources Committee, moving the bill to the full U.S. House of Representatives. The

Act unanimously passed (4160) the full U.S. House, showing a tremendous bipartisan effort. This legislation must now be voted on by the U.S. Senate, before being sent on to the President for his signature. Once enacted into law, the bill will authorize the National Medal of Honor Museum Foundation (NMOHMF) to create a national monument celebrating the values of the Medal of Honor, the highest military award for valor in combat. Tiny - but deadly - enemy Army doctor braved death to beat epidemics

Army Maj. James Carroll wasn’t a Medal of Honor recipient, but he courageously exposed himself to the enemy at the risk of his own life on Aug. 27, 1900. By any measure, he went above and beyond the call of duty in keeping with the highest traditions of military service. Carroll almost certainly died seven years later as a direct result of injuries sustained while saving countless thousands. Fellow physician Jesse William Lazear, who was a civilian surgeon assisting the special Army unit at Columbia Barracks — Quemados, Cuba — died in the same incident.

‘Yellow Jack’ A disease strikes that hardly affects some, puts many down for weeks, but causes others excruciating deaths. Governments pull out the checkbooks, create travel restrictions, and mandate quarantines in efforts to quash the outbreaks. Those who can, disinfect everything around them in a bid to stave off the unpredictable killer. Political campaigns are stopped in their tracks by the travel restrictions and mandatory quarantines. And 2020 was 122 years away. The disease was yellow fever, and outbreaks used to terrify Florida and other states. There’d been 35 recorded yellow fever outbreaks in the North American colonies-turned-states from 1700 to 1800. “Yellow Jack” epidemics became an annual event for the U.S. from around 1800 to 1880. It became a matter of who’d be battling the dread disease from year to year. It was the Jacksonville epidemic of 1888 that changed everything for Florida. One of the worst things about Yellow Jack was folks couldn’t figure how it got around,

so unproven preventatives abounded, including gun therapy. Yeah, really, people tried clearing hypothesized miasma causing Yellow Jack with the concussion of gunshots. Germ theory was young, and a lot of folks were still willing to believe bad air caused diseases if it gave them a way to respond to epidemics — even if those attempts repeatedly failed others. But Yellow Jack was impervious to disinfection and wasn’t airborne. Well, hold on about airborne. In 1881 a Cuban doctor, Carlos Juan Finlay, put forward a hypothesis that yellow fever was borrowing mosquitos’ wings to fly from victim to victim. He might have directly or indirectly borrowed and built on the insect vector hypothesis by Josiah Nott, who suggested it in the 1850 paper “Yellow Fever Contrasted with Bilious Fever.” But, Nott was a bit of a kook even for his time. He was arguably best known for using phrenology to give a seemingly scientific justification for chattel slavery. Finlay, like Nott, was roundly laughed off. In fairness, before Finlay suggested mosquitos, he threw out some wild hypotheses about filth combining with the air and telluric currents creating yellow fever that would appear today only on pseudoscientific websites touting various alien conspiracy theories, too.

The Florida infection of 1888 Today the Florida Department of Health responds when there’re outbreaks, or potential outbreaks, of diseases in the Sunshine State. For example, it was the state’s health department that announced the first known case of COVID-19 in the state on March 2 last year. The department has run COVID testing since then. When the federal government gave emergency-use authorization for the first COVID vaccines, it was the Florida Department of Health that immediately got the vaccinations going in the Sunshine State, and still has its hand in it. The health department’s predecessor, the Florida State Board of Public Health, came about because of the Yellow Jack attack of 1888. A fella traveled from Tampa to Jacksonville apparently unaware he had yellow fever. Jacksonville was having a wet year, so the unnoticed method of transmission — various breeds of mosquitos — was in abundance. In a city of about 20,000, about 5,000 were known to be infected with yellow fever. About 450 to 500 died. The panicked city was filled with yellow flags hung outside homes warning others away. Many turned into black flags indicating Yellow Jack deaths.

Residents tried to flee the city on foot, in carts, trains and boats, but others encircled the city to stop them. They were armed and ready to shoot. Jacksonville was quarantined with deadly force. The Jacksonville epidemic stopped Florida’s social and commercial lives as people got the news and holed up the best they could. Francis Fleming and V.J. Shipman all but suspended their gubernatorial campaigns. Fleming — who was from the Jacksonville area — won that election and would go on to be remembered as one of the state’s worst governors, largely due to the former Confederate Soldier’s record on racial policies. But, he did form the Board of Public Health in 1889 to combat Yellow Jack and other communicable diseases. (Oh, and he’s the one who suggested the red saltire on the state flag, so it wouldn’t look like Florida surrendered again on windless days. Before then the flag had the state seal in the center of a lot of white field.)

It “only” took 16 hours for Britain’s Queen Victoria to send American President James Buchanan the first official telegraph message across a transatlantic cable, on this Aug. 5, 1859. Her Majesty’s message was brief, only 99 words long. In it, she said, in part: “The Queen desires to congratulate the President upon the successful completion of this great international work, in which The Queen has taken the deepest interest.” The wily monarch chose her words well. The reason for the “deepest interest” was she knew that communications, that used to take weeks via the fastest clipper ships between the U.S. and Great Britain, were a huge leap forward in securing more riches for her Empire via the former colonies. And President Buchanan, who served as an Army private in the War of 1812, knew that Civil War was just around the corner. His country would need all the money it could earn from the rich former enemy.

Right man at the right time Why did sending transatlantic messages take so long? we might ask, in this day of internet that transmits data so rapidly it happens faster than the blink of an eye. The answer lies in the early understanding — or lack thereof — of magnetism, and electrical conductivity. American merchant Cyrus West Field is considered the driving force behind solving the early riddles — by much trial and error — into what became a revolution on the scale of landing a man on the Moon. Field was a natural businessman and financial backer. He began his career in the paper business, which he helped build into a powerhouse, becoming one of the richest men in New York at only 34. He was keenly interested in expanding his market overseas. The telegraph, invented in 1844 by Samuel F.B. Morse (“What God hath wrought”), had already proven itself a successful boon to business. No more messages by horseback couriers, stagecoaches or even by train. Communications were now nearly instantaneous. Field gathered a consortium of forward-thinking investors together, and eventually founded a network stretching from Maine to the Gulf Coast, rivaling

Western Union in its success. The time was ripe for expanding overseas.

The monumental task Naturally, the cable, once developed, could only be borne by ship in order to lay it in the ocean. It was so fearfully heavy that it had to be transported in two halves, by two ships, that met in the middle of the Atlantic to splice them together — but this was only possible by learning, the hard way, that transporting the cable in one piece resulted in its complete loss, sending scientists and Field and his associates back to the drawing board. The reason the cable was so heavy was its design. In the core were seven strands of copper, surrounded by a coating of tree sap, and further surrounded by hemp, tar and dozens of iron strands. One mile’s worth of completed cable weighed more than a ton. The scientists Field and company employed knew about the conductivity of metals, with copper being the fastest, and iron being a reliable way to prevent signal energy from scattering. What they didn’t know was a proper signal that reached its destination had to do with electromagnetism. “Early on, [scientist Michael] Faraday and others worried that sending a message across a vast distance would take a long time, but they reasoned, incorrectly, that this would be similar to

The House Veterans’ Affairs Committee ranking member — Rep. Mike Bost — dropped by the Subcommittee on Health’s July 27 hearing about long-term care. The Illinois Republican had an off-topic question for Dr. Scotte Hartronft, director of geriatrics and extended care at the Veterans Health Administration. “Doctor, while not directly related to today’s hearing, the impact of the (SARSCoV-2) Delta variant on Veterans is a top priority in all of our minds,” the representative said. “Please, if you could provide an update on the (Department of Veteran Affairs’) efforts to vaccinate Veterans. Particularly those who are elderly or high risk.”

SARS-CoV-2 Delta variant Some biologists count viruses as living, while others don’t. However one falls on the question of living or not, viruses do have genetic information and replicate themselves, but only inside living cells of infected organisms. Because of replication, viruses are subject to accumulating genetic changes. The genetic changes, called mutations, can make a virus more benign, more virulent, or present no differences to infected hosts. The variations that make a virus more transmissible — thus survivable — are the ones that most often stick and spread. In the case of the SARS-CoV-2 virus, the lineage B.1.617.2, Delta variant, is now one of if not the most prevalent, or at least noticed. It was first detected in India in late 2020, although it could have originated elsewhere. The World Health Organization dubbed it the Delta variant on May 31. The Delta variant has a more efficient peplomer, spike protein code, than the original SARS-CoV-2, so it is more transmissible. It may be more virulent, too. The three vaccines available in the U.S. under federal emergency-use authorizations appear to generally be effective against the Delta variant causing COVID-19, the disease the SARS-CoV-2 viruses cause. However, the rise and rapid spread of the Delta variant has eroded apparent gains the country made against the COVID-19 pandemic. That’s due to various factors including vaccine hesitancy. At press time about 50 percent of the qualifying population have gotten fully vaccinated. Others are partially

vaccinated with one dose of the two-dose vaccines.

VA’s latest numbers On July 28, the VA reported it knows of 273,000 cases of COVID-19 among Veterans since the start of the pandemic. It reported on that day having 4,054 active cases among people associated with its medical centers. The majority were Veterans, 3,650. The VA is aware of 12,700 Veterans who’ve died due to complications related to COVID-19. About 4,250 of those deaths were among Veterans receiving treatment for COVID-19 at VA medical centers. The VA reported — again on July 28 — there’ve been about 34.6 million cases of COVID-19 in the U.S., along with 610,387 deaths. Almost all of those deaths were among adults. There are about 332.5 million people, 259 million adults, in the United States. About 17.5 million are Veterans and around half of them are enrolled in the Veterans Health Administration. At press time the Orlando VA Medical Center has the most known COVID-19 active cases nationally with 280. The Gainesville VA Medical Center takes the

second spot with 203. Tampa is in fifth place among VA medical centers nationally with 129. Florida had 773 known active cases of COVID-19 among Veterans at press time. It’s had 21,300 known cases and 715 known deaths among Veterans due to complications related to COVID-19 since the first cases were detected in the state in March last year. The VA has fully vaccinated 3.1 million Veterans against COVID-19 nationally. It’s likely many more Veterans have received vaccinations through other sources, such as state health departments.

VA’s answer to Bost Hartronft told Bost the VA is monitoring the situation. “Yes, we’re watching it very closely on the Delta variant, and also looking for any clusters of breakthrough infections,” he said. Breakthrough infections are when vaccinated individuals exposed to SARS-CoV-2 develop COVID-19. “Knock on wood, we’ve had a very high rate of our vaccinations — especially in our elderly,” the doctor continued. “We had a very strong push in the very beginning. Especially those living in nursing-home settings.”

After the war the U.S. had about 50,000 servicemembers stationed in Cuba and other parts of the Caribbean. The Army created its Yellow Fever Commission in 1900 to go to all-out war with the terrible disease. The commission’s work was so important and results impressive at the time one only need name the lead physician for modern Americans to get it — Maj. Walter Reed. In a risky move, Carroll and Lazear put Finlay’s mosquito hypothesis to the test by deliberately exposing themselves to the blood-feeding little flies suspected to be carrying the virus that causes yellow fever. The species they exposed themselves to is now known as the yellow fever mosquito. It’s recognized by distinctive white markings. Both got sick. Lazear died about a month after the deliberate infection.

Veteran Directed Care What subcommittee members and panelists all seemed to agree upon is the need to make the VA’s Veteran Directed Care programs more consistent and commonplace. The

Carroll recovered, but was left with a heart valve defect suspected to be a result of his infection. He died from the heart problem on Sept 16, 1907. The American Association for the Advancement of Science advocated for Congress to award his wife, Jennie Carroll, a special pension in recognition of Carroll effectively dying of a service-connected injury. Congress opted to give the Lazear and Carroll widows $125 a year. That’s about $3,500 today. Not a lot, but a recognition the men died in service to their country.

This was no small task. The Spanish had a name for Florida’s eastern coastline — Los Musquitos. The interpretation isn’t necessary. Yes, the name sounds a bit silly perhaps to us now, but the association was made up of doctors and health scientists who pushed for organized mosquito-control action by the state legislature. Indian River County was the first in the state to have a legislature-created mosquito control district. That was in 1925. St. Lucie County had the second in 1926. Mosquito-control efforts have increasingly turned from chemicals to denying the pests places to breed. The most problematic species along Florida’s east coast is the salt marsh mosquito. They don’t lay their eggs in water as most species do. They lay them in mud that gets covered with water later. St. Lucie County, and others, have created “mosquito impoundment” areas, places it keeps underwater, so the salt marsh mosquitos can’t lay their eggs. And today, Florida summer mosquitos are annoying, but they’re no longer deadly, thanks to the courage and sacrifices of Carroll and Lazear.

silk production was also a big money-maker. But neither wool, which was too uncomfortable to wear in hotter American climes, nor silk, which was too expensive, could match the lightness and durability of cotton. British climate was not suited to growing cotton, so it relied on the U.S. (and to a lesser extent Egypt and India) to sell it cotton in abundance, making the island nation a powerhouse in textile manufacturing, and making millionaires out of American plantation owners in the South.

Great good, but at tough price This financial bonanza on both sides was a direct contributor to

the Civil War — and at the same time resulted in a permanent stain on history from the use of slavery to produce the very benefits people began to enjoy. Thus, the invention of the transatlantic telegraph, that helped bring about so many wonders, not the least of which was cheaper goods, leading to the ability of American workers to lift up their station in life, also left a darker legacy. The flip side of progress was greed-borne misery for huge swaths of people. The Civil War, the exploitation of slave labor and later, the exploitation of the worker in general, were direct descendants of this legacy. It’s up to ours, and future, generations to learn lessons from it.
