• With the 1990 season came back surgery for a herniated disc and agonizing back pain. But it didn’t keep Lemieux from his first Stanley Cup and earning MVP of the playoffs. In early 1993 came the unbelievable announcement that he had been diagnosed with Hodgkin’s lymphoma. He immediately began aggressive radiation treatments. After only two months, on the day of his final treatment, he made the trip to Philadelphia to play in the PenguinsFlyers game.

Final • Four months later, Lemieux was once again in surgery, this time for a herniated muscle in his back. At the end of the season, he was forced to take a leave of absence to recover from the fatigue of cancer treatments.

Lemieux in 2001

• Lemieux was back in full force for the beginning of the 1995 season, scoring his 500th career goal in October. During the following season came his 600th.

• Lemieux announced his retirement in 1997 at age 32, his number 66 was retired, and he was immediately inducted into the NHL Hall of Fame without the usual three-year waiting period. In 1998, the Penguins declared bankruptcy, with over $90 million in outstanding debt. Because the team had been in trouble for quite some time, Lemieux had deferred his salary and was owed $30 million. He converted the debt into equity and became the majority owner of the team. In 2000, he returned to the ice, becoming one of three players who were Hall of Fame members while still playing. (The other two were Gordie Howe and Guy Lafleur.) The following letter from a reader demonstrates that it is certainly the chief occupation of those who champion the companionship of a particular canine at any cost. â&#x20AC;&#x153;We recently got an 8-month-old yellow lab from a family with three small children. The lady said the dog could be aggressive and had nipped at her 2-year-old. â&#x20AC;&#x153;She is a very nervous puppy -- jumps at the slightest movement or noise. She also jumps and twitches when she sleeps. We have experienced her aggression now, and it is quite scary. The second morning we had her, something spooked her and she would not let us near our truck. The doors were all open, and she sat in the driverâ&#x20AC;&#x2122;s seat growling and showing her teeth. I got a treat and put it on the ground near us. When I said â&#x20AC;&#x153;release,â&#x20AC;? she got out, snatched the treat and jumped right back in. She calmed down, but was nervous all day after that. â&#x20AC;&#x153;She has showed her teeth a few times, and weâ&#x20AC;&#x2122;re not sure how to handle it. I read that it isnâ&#x20AC;&#x2122;t always good to approach or discipline her when she is showing her teeth and growling because it could initiate an attack. I know that she is scared and confused.

Within one week she stayed in four different houses, with all new people. Weâ&#x20AC;&#x2122;ve had her for a week, and weâ&#x20AC;&#x2122;d love toproduction keep her. We have no children, but we have niecesHRS: and1 nephews around often. Every time we moved, she growled and showed her teeth -- even when we were nowhere near her. She came out of it, but again she was nervous for the rest of the night.â&#x20AC;?

This is akin to buying a car with no brakes because you like the ashtray and donâ&#x20AC;&#x2122;t smoke. With all the great dogs in shelters that need homes, why insist on the one you know is aggressive? It is one thing to root for the underdog and quite another to invite the snarling underdog to share your home.

Osteoarthritis is the most common chronic disease in America, afflicting the back, knees, hips, hands and shoulders. This aggravating condition can cause disability, pain, difficulty with movement and other activities, and a poorer quality of life. In particular, osteoarthritis of the knee affects as many as 20 million Americans.

In a recent report in Nature Reviews Rheumatology, researchers reviewed the role of mechanical and exercise approaches to improve symptoms and reduce the progression of osteoarthritis of the knee. Data clearly show that the way we walk is a major factor leading to the arthritis.

The process of walking -- though generally unappreciated -- is a highly coordinated activity. Maintaining proper, physiological movements of your hip, knee and ankle joints can drastically reduce your risk of disease.

Walking is associated with outward movement of the leg below the knee, referred to as adduction.

The more this occurs, the more pressure is put on the inside of the knee. Over time, increased pressure leads to damage and osteoarthritis develops.

Walking with our toes pointing straight or inward is associated with a higher risk of osteoarthritis than walking with our toes pointing outward. With each step forward, the ankle and foot bends upward, known as eversion, which removes pressure from the knee.

While walking, it turns out that the amount of adduction, ankle and foot movement and the way we point our toes are greatly affected by the shoes we wear. Observations show that wearing highheeled shoes increases adduction by 18 percent to 23 percent and prevents the ankle from moving upward by 90 percent. Not surprisingly, the need for knee replacement surgery is much more common in women than in men.

High heels are not the only culprits for poor walking mechanics. Comfortable thick-soled supportive shoes are not particularly good for the knees, either. Thick, rigid soles prevent movement of the ankle while simultaneously promoting adduction of the lower leg.

So, what is the best shoe to prevent knee osteoarthritis? Amazingly, the best option is no shoe at all. Walking barefoot is most beneficial in improving the mechanics of movement. Consider shoes that mimic barefoot walking. These include thin-soled shoes, such as flat walking shoes or flipflops. Thinner and more flexible soles offer substantial benefits over thick-soled support shoes.

Orthotics can offer another important solution to reducing pain and progression of osteoarthritis.

Lateral wedge insoles, which incline the outside of the foot upward by 5 degrees to 15 degrees, have been shown to dramatically relieve symptoms of osteoarthritis of the knee. In addition, knee braces can help improve gait and reduce pressure on the knee. Worn throughout the day and for periods as long as 12 months, these braces can help reduce pain and improve mobility in a substantial number of patients. For greatest results, the patient must compulsively wear the brace. Sadly, for many adults, the bulky nature of the brace and difficulties putting it on make this therapy impractical.

Finally, gait training and muscle strengthening by an expert physical therapist is critically important. Initially, a careful evaluation of gait will provide guidance for the most appropriate footwear and insoles. Gait training can help assure that toes point out and that adduction is minimized. Exercises to strengthen the muscles that support the knee and improve gait assure minimal inappropriate pressure on the knee.

As the disease progresses, mechanical abnormalities and pain can become very severe and surgical replacement is the only option. While most patients sing the praises of this operation, it must be emphasized that surgery should always be a last resort. With appropriate and conservative management of osteoarthritis, you not only reduce the need for surgery, but you also decrease the potential of worsening disability and suffering.

Remember, no surgery is 100 percent safe or “easy.” The way we walk and our footwear affect the severity and level of disability caused by osteoarthritis. The evidence is compelling that good choices and a commitment to prevention are the best health investments any of us can make.

Tips for Finding Information on the Social Security Website Q:

I really love your column and the information you provide. One reason I appreciate your advice is because I find it so hard to find answers to my questions on Social Security’s website. Did they purposely design it to confuse people?

A:

I’ve got to be a bit diplomatic and tactful in my answer. As a former long-time employee of the Social Security Administration, I’ve got to admit that some of the people involved in designing and maintaining Social Security’s website are friends of mine. Although, like me, I think several of those old friends have retired and all of us old fuddy- duddies are convinced the agency has gone downhill since we left. While we were there, we kept the place humming like a well-oiled machine. But now that we’re gone, the agency -- including its website -- is starting to show some cracks around the edges. (Isn’t that what everyone thinks about his or her old place of work after retirement?)

OK, I’ll stop being an old goat and tell you what I really think. I believe the Social Security website (www.socialsecurity.gov) is a good site to use and navigate, especially when you consider the vast amount of information it has to contain. After all, little old ladies get on the site to replace a lost Medicare card. Attorneys use it to find references and precedents for various Social Security laws. A young bride might go to the website to get information about changing her name on her Social Security

card. A rapidly aging baby boomer will use the site to file for Social Security benefits. A guy from England who was sent to this country by his employer might need to find out about a Social Security treaty agreement between Britain and the U.S. The possible uses for Social Security’s website may not be endless, but they certainly are beyond most people’s comprehension.

Most of my readers are looking for answers to simple questions involving their Social Security benefits or future benefits: “Will I get widow’s benefits when my husband dies?” “I plan to retire this year.

How much can I earn and still collect my Social Security?” “How do I apply for disability benefits?” “How does my teacher’s pension impact my Social Security?” And on and on the questions go.

And here is a handy place to find the answers to those and many other questions. It’s the “Publications” section of Social Security Administration website. SSA produces hundreds of publications covering almost every major Social Security topic. Those pamphlets and fact sheets are lumped into a variety of categories, such as “Retirement Benefits” or “Survivor Benefits” or “Medicare” or “Social Security Number.”

I am very familiar with those publications because I used to be chief editor in the SSA office that produced them. (Of course, they are not quite as clear and concise today as they were when I edited them, but they are still very good!) In fact, I will frequently turn to those publications, even today, to verify some little nugget of information that I had forgotten or wasn’t clear about.

To find these fact sheets and pamphlets, click on “Get a publication” in the “Top Services” section of the homepage at www.socialsecurity.gov. That will lead you to hundreds of sources of information in about a dozen different categories. And they are generally written in an easy-to-understand manner.

rately! So, I understand a current SSA employee’s confusion when it comes to the complicated math involved in figuring a Social Security benefit. Still, he or she should have referred you to a source of information.

You’ll find that information at SSA’s website: www.socialsecurity.gov. One of the fact sheets discussed in the prior answer is called “How your retirement benefit is figured.” But I’ve got a hunch that’s not going to be thorough enough for you, so I’ll steer you to the benefit computation “bible.” On the right side of the homepage, you’ll see a section labeled, “About Our Agency.” Pull down the little box that says “More” and then click on “Our Program Rules.” That takes you to a page where you will find all of Social Security’s laws and regulations.

But you’ll also find that “bible” I mentioned, which contains every single rule and instruction for every kind of Social Security benefit. It’s called the “Program Operations Manual System” or “POMS.” Click on it.
