USAID’S FP/MNH ICP PROJECT—FIVE SOLUTIONS TO REDUCE UNINTENDED TEEN PREGNANCY

BACKGROUND We – teenagers, USAID’s ReachHealth project, and partners – used a Human-Centered Design (HCD) approach to answer the question: How might we reduce unintended teenage pregnancy in the Philippines? This process resulted in a comprehensive review of the current state of adolescent sexual and reproductive health (ASRH) in the Philippines and identified five priority solutions, designed by teens and supportive adults, to reduce teenage pregnancy. It placed teens at the forefront and mandated their inclusion and participation throughout the process, ensuring that the resulting interventions were directly relevant to their needs. Their network- including parents, guardians, teachers, and health workers, to name a few – were also engaged in this process to gain a robust understanding of the current situation and potential solutions.

In August 2019, a team from USAID’s ReachHealth project spoke with over 200 individuals in the cities of Manila, Cavite, Iloilo, and Davao. Following these conversations, the team synthesized and reflected upon the information gathered and identified key quotes and common themes across conversations. This process resulted in a final list of 12 key insights, or statements that mirror what we heard and explain what is driving the issue of teenage pregnancy in the country. These insights were then presented to another group of teenagers and supportive adults during a 2-day interactive Imagine Workshop in October 2019, wherein participants generated 280+ ideas to address the insights and reduce teenage pregnancy. They created simple, low-tech prototypes of the five prioritized solutions, which were then rapidly tested by the team from USAID’s ReachHealth project with over 140 individuals in Tanauan City and Davao City. These five solutions are #TheBestICanBe Movement, Digital Resource, #SoConnected, Teen Terminal, and Teen Fest which emerged as viable interventions with support from teenagers, adults, community leaders, and other key stakeholders.

#THEBESTICANBE MOVEMENT DESCRIPTION (IMAGINE STATEMENT)

Imagine a coordinated, multi-channel movement that empowers teenagers to reach their full potential while uniting key stakeholders and shifting social norms in order to create an enabling environment for teenage pregnancy prevention.

SOLUTION OVERVIEW

From the HCD process, it was clear that societal norms surrounding teenagers and their sexuality play a large role in perpetuating teenage pregnancy in the Philippines. Teenagers stated that adults block access to reproductive health services and avoid conversations related to love, sex and relationship. Teachers explained that prevention of teenage pregnancy should begin in the home.

Parents expressed discomfort and embarrassment (Hiya) and described efforts to discourage sexual activity, sexuality, all while upholding gender norms that dictated how teenage girls and boys should act. Literature has shown that these stifling social norms and inequitable gender norms become embedded during adolescence and influence how teens socialize with each other and what SRHrelated behaviors they practice1,2.

As teenagers transition through these formative years from childhood to adulthood, they must be embedded in social environments that equip them with the right tools and enable them to reach their full potential as adults.

Aligned with the Department of Health (DOH) and the Commission on Population and Development (POPCOM), USAID’s ReachHealth project envisions a Philippines where all adolescents can grow to be productive, contributing and healthy citizens — they are able to achieve their educational and career aspirations; they value their health and make healthy choices; and they are enabled by a system that sees and responds to their needs.

In response, the Imagine Workshop participants envisioned an overarching campaign that would help shift perceptions of teenage pregnancy and expectations on how teenagers and adults can work together to prevent teenage pregnancy. The goal of this solution is to develop a common voice and platform for teenagers and stakeholders. They coined the campaign, #TheBestICanBe due to its aspirational and motivational nature. Teenagers, and adults alike, will be inspired to contribute towards reduction in teenage pregnancy.

AUDIENCE AND OBJECTIVES

The primary audience for this campaign would be adolescents between the ages of 10 to 19 years old (with a focus on those between the ages of 15 to 19 years of age as they are most at risk). All information will be age- and stage-appropriate. Adults, including family, teachers, health professionals 1

and community leaders, and other key stakeholders are also an important audience that will be reached through this campaign. #TheBestICanBe is an aspirational campaign that aims to remove the shame and despondency linked to prevention of teenage pregnancy across the Philippines. Specifically, this initiative aims to: •

Develop an overarching, unifying, multi-channel platform (using social media, etc) that will resonate with the intended audiences at the local and national level

•

Elevate teenagers – ensure their voices and needs are amplified through the campaign

•

Address perceived social norms about love, sex and relationships of teenagers

•

Challenge existing gender norms around love, sex and relationships

•

Encourage communication and collaboration between teenagers and adults (e.g family, community leaders, health workers, stakeholders, etc) about love, sex and relationships

•

Provide an aspirational and participatory approach that brings key stakeholders together in a collaborative manner under a shared umbrella campaign

•

Inspire the audience to take action (e.g POPCOM’s campaign, Stop, Look, Listen) to reduce/ prevent unintended pregnancies

•

Present actions the teenagers, adults, and community leaders and others can take to become their best selves.

WHAT WILL IT LOOK LIKE?

#TheBestICanBe will involve a multi-channel communication campaign in order to ensure it reaches the primary audience members, including 15-19 year olds, as well as parents, and key stakeholders (teachers, health workers, government, USAID IPs). By working online and offline at the different socio-ecological levels – individual, interpersonal (friends, family), community (teachers, health workers) the campaign will have the potential for maximum impact. #TheBestICanBe is a norms-based intervention that integrates gender. It requires a range of strong, coordinated, and complementary approaches to create a “surround sound” effect and influence current societal norms. It works on multiple levels to highlight positive behavioral and attitudinal norms (what others actually do/ believe / do) rather than the perceived norms (what we think others believe / do). Adolescent sexual and reproductive health (ASRH) won’t be the focus and this term will not be used, but rather it will be framed as content surrounding love, sex and relationships.

It includes strengthening relationships with yourself, with others (platonic and romantic), and with your community (however it is defined).

There are several components to #TheBestICanBe: •

A unique visual identity, a design/ ad agency will help develop an identifiable and memorable visual brand for #TheBestICanBe. The identity will have a modern, bold style, use unifying iconography, and be gender transformative.

3-5 TV ads will raise awareness of the campaign and drive traffic to the social media campaign.

Social Media, Regular posts on Facebook and Twitter using role models with established and credible accounts, will present quiz questions, periodic challenges related to desired behaviors, tips for personal and professional development, inspirational testimonials related to love, sex, and relationship from teens across the country, and motivational quotes from popular teenage influencers (in the Philippines and globally). Early stage social media posts will pique interest among the desired audience and build on the buzz generated by the TV ads, while also setting the tone for the overall campaign, and providing a link to the other arms of the movement

•

Linkages to the adolescent website and #SoConnected social media campaign which will have similar visual design and also have an online presence.

•

Linkages to Usap Tayo sa Family Planning, U4U Teen Trail and Parent Teen Trail, and other POPCOM community-level activities and FP counseling- Peer educators will be trained to support social media efforts and provide a link to services. Through consistent branding and linked visual elements, the #TheBestICanBe campaign will be linked to community level support tools, training materials for health providers, educational materials developed for teenage audiences, and other demand generation activities such as teen fest, teen terminal, and community-based efforts led by POPCOM.

•

A call to action: The campaign will challenge 10-19 years and adults (parents, teachers, health providers) should work to exemplify the campaign name: #TheBestICanBe. The name itself is intended to encourage teenagers, adults, and partners to join in and align themselves (and their activities) with this campaign.

•

A teen advisory group: USAID’s ReachHealth projects will continue to include teens throughout this intervention. Together with partners, we will create a national Teen Advisory Group that will support the development of activities for this campaign. It will include a maximum of 10 teenagers from different perspectives and backgrounds. Though this will begin at the national level, we encourage regional and local partners to create their own Teen Advisory Group to support the role out of community level activities.

RATIONALE

The campaign is an appealing, engaging, and compelling space that can provide opportunities for teens to move through the adolescent stage of life, while recognizing that they are going through very real changes, that these changes are normal, and they are not alone. High reach and frequency of content will saturate and reinforce the messages that: •

Teens are capable and empowered. They have drive and ambition.

•

Teens are exploring. They are responsible enough to safely push boundaries, to test themselves, and to see what they can achieve.

•

Teens have options. They can choose to safely express themselves. They can choose to abstain or to have sex responsibly and consensually.

•

Teens are grounded and connected – to themselves, to their friends, to others around them. They have healthy and meaningful relationships.

•

Teens are trying. Both boys and girls are kind, sensitive, loving beings who want to be understood and supported. They are good people.

Teenagers face very real challenges and vulnerabilities from the physical, psychological, and emotional growth that comes with transitioning from childhood to adulthood. They must be equipped with the knowledge and skills necessary to manage the rapid changes in their life. They must build their self-worth and strengthen their ties to the people around them. They must have access to a safe and supportive environment, whether that be in school, at home, or online. They also need to have access to accurate information and quality health services 3. But, currently, this isn’t the case. There are social and gender norms and structural barriers that prevent this from happening.

The DOH’s Adolescent Health & Development Program Strategic Framework recognizes the value of socio-cultural determinants to adolescent health. However, none of the strategies directly tackle these norms. They only mention knowledge as a driver for behavior change. But for sustainable behavior change to occur, there is a need to address the social norms and systems that surround that individual and influence their behavior.

Investments should be made on norms-based interventions, creating a social movement that is not just focused on reducing stigma around adolescent sexuality but also encourages teens to be who they are and demonstrate their agency345. It should happen on multiple levels because changing collective behavior and the social environment will affect individual behavior6.

PLANNING DETAILS

Essential Components •

Teen Advisory Group (TAG) - Members of the Teen Advisory Group should 1) be between 15-19 years old, 2) be willing to share their opinion on the development of this campaign and 3) reside in/near Metro Manila or have regular access to the internet to participate in video meetings for this initiative and 4) have an interest in promoting reproductive health among adolescents (though no previous experience is necessary). The members’ parents will sign a consent form for their child to participate. A FB group and group chat will also serve to facilitate discussions and communication. Face-to-face meetings will be set as needed during weekends or after school hours. All content and activity plans will be created or reviewed by the Teen Advisory Group before receiving final approval by DOH and/or POPCOM.

Imagine a consolidated and accurate ASRH online resource for teens, parents, and health providers that draws on the best aspects of existing digital and social media.

SOLUTION OVERVIEW

Our HCD process revealed that adolescents are seeking information on love, sex, and relationships through channels that are convenient and easy for them to access. The most frequently mentioned sources of information on these topics include peers, web searches, and social media - all of which may provide information that is inaccurate or incomplete. With these insights in mind, a design question was formulated to challenge the participants in the 2-day Imagine Workshop to generate possible solutions: How might we create digital safe spaces for teenagers to learn about sex?

To address this question, the participants created a digital resource that provided reliable, accurate, and extensive information on sexual and reproductive health (SRH) presented in a way that appealed to them and their peers. The goal of the resource is to provide a go-to place for adolescents that can help them effectively navigate SRH-related situations, make informed choices about their own SRH, and easily access reliable resources and tools to help them act on those decisions.

AUDIENCE AND OBJECTIVES

The primary audience for the Digital Resource are teenagers 15 to 19 years old, since they are most at risk of pregnancy and in the most need of this information right now. Teenagers 10 to 14 years old will be a secondary audience, as they can access the website to receive relevant information as well. Parents, health providers, and teachers are secondary audiences of the Resource, as information and tools will be included relating to how these groups interact and communicate with teens.

This Digital Resource aims to: ●

Provide a consolidated, comprehensive online platform for all things ASRH by collaborating with key stakeholders and linking to other, existing ASRH resources, events, and activities

Improve adolescents’ ASRH knowledge by providing reliable, accurate, and extensive information on SRH in easy to understand and entertaining ways.

●

Improve adolescents’ self-efficacy to use FP by providing them with the tools to make informed decisions about their SRH, communicate effectively about ASRH with parents and health providers, and know when, where, and how to access contraceptives

●

Improve parents’ and health providers’ self-efficacy to talk with adolescents about sex, love, and relationships by providing readily available, easy-to-use communication tools and resources

●

Increase audiences’ perceived severity of teenage pregnancy by presenting it as an immediate, yet preventable, health and development concern

WHAT WILL IT LOOK LIKE?

This Digital Resource intends to be the go-to resource for teens in the Philippines to receive complete and unbiased information about ASRH, and for parents and health providers to receive tools and resources to effectively communicate with adolescents about ASRH. It will be a comprehensive information hub to present reliable facts and relevant information about love, sex, and relationships in a fun, fresh, and appealing manner.

There are several key components to the Digital Resource: ●

Visually appealing, user-friendly, and interactive platform. A web developer will create a user-friendly platform that is easy to navigate, looks fun and fresh, and is designed from the users’ perspectives. It will include a comments section below every post or article or chat function where users can leave questions, comments, and/or interact with fellow visitors to engage visitors and foster an online community.

Linked to #TheBestICanBe campaign. The platform will have a similar visual identity to the #TheBestICanBe campaign (branding, logo, colors, etc.). #TheBestICanBe social media pages will be linked in the website for users to easily access, and be a part of, the campaign. Posts on these pages and during other activities will also link to the website to build traffic.

Information and Resources for Teens, Parents, Health Providers, and Teachers. The website(s) will provide information for all three audiences. Depending on budget availability, the web developer will create separate platforms for the four audiences, or, more likely, include parent, health provider, and teacher “corners” on the otherwise teen-focused site.

●

Comprehensive, easy-to-read, and entertaining content. A creative consultant/agency will create posts that present evidence-based information about love, sex, and relationships, and practical day-to-day decision-making tips, in lighthearted, humorous, and easy-to-read ways, so that the content is accessible by the broadest audience possible. The website will Content will be approved by USAID, POPCOM, and DOH and will be reviewed/vetted by the Teen Advisory Group to ensure its appeal to teens. The content will be written in English, with the option to translate the page to Tagalog, Bisaya, etc. (the number of languages, and which languages, will be decided based on feasibility, budget, and expected impact). The resource will cover an extensive range of topics when it is first published, so that it will gain credibility among its first visitors, who will then disseminate it throughout their networks.

Follows a sound digital strategy. A digital media consultant will build and implement a digital strategy for the resource, including plans/calendars for posts/updates, building traffic to the site, search engine optimization, link local platforms, etc.

RATIONALE

In a country where 71% of the population is online, a figure that has increased by 13% in just one year, interventions that hope to get the attention of adolescents must begin to transcend traditional

media and venture into the digital space7. Out of the 76 million Filipinos on the Internet, 8.36 million, of these users are between the ages of 13 and 17 years old. The proportion of teens online cannot be overlooked when carrying out interventions that aim to engage adolescents and effectively respond to the rising rates of teenage pregnancy in the Philippines.

Across the country, various government units and civil society organizations (CSOs) working to address ASRH have already established their own websites and social media pages, or have initiated other forms of digital interventions – all aiming to reach adolescent and youth audiences. However, most of these are managed and maintained in silos, resulting in low impact and limited reach and engagement. Some of these digital platforms also vary in terms of accuracy and completeness of information, offer competing recommendations and priorities, and/or serve too technical or scientific for the average Filipino teenager to engage with and understand. It is crucial that teenagers have access to clear and consistent information about ASRH that is presented in appealing and appropriate ways for their age and interests, which is what the Digital Resource will provide.

#SOCONNECTED DESCRIPTION (IMAGINE STATEMENT)

Imagine using humor, modeling, and facilitation to help parents build the capacity of discussing love, sex, relationships & other relevant issues confronting their adolescents that can result in strengthened connection/bond between them, and in the process improve adolescents’ health outcomes.

SOLUTION OVERVIEW

During the HCD process, it was learned that “when adolescents experience a high degree of emotional vulnerability, it can lead them to engage in unhealthy relationships and risky sexual behaviors.” Many times, broken families or absentee parents were cited as the underlying reasons for this vulnerability, causing teenagers to look outside of the home for emotional security.

Additionally, it was also heard from many audiences that “adolescents are expected to first learn about SRH at home, but the concept of hiya (embarrassment) in Filipino culture affects how parents and adolescents are able to talk about it.” Hence, the following design questions were formulated to generate possible solutions: •

How might we leverage adolescent's needs for belonging and meaningful connections to reduce teenage pregnancy?

How might household members communicate better with teenagers on topics of sex, love, and reproduction?

In response, the Imagine Workshop participants created a solution that can provide a safe space and avenues for parents and adolescents to talk about relevant sexual and reproductive health concerns that young people may have to confront each day and possibly address them. The goal of this solution is to build the capacity of parents and adolescents to better communicate with each other and in the long run strengthen the “connection/bond” between them through their participation in online digital campaigns/platforms & on-the-ground activities. The participants coined the intervention #SoConnected.

AUDIENCE AND OBJECTIVES

The primary audience for this initiative would be parents or guardians with adolescents aged 10 to 19 years that are not yet pregnant. Other community members, including teachers, health care providers, and other community leaders will be a secondary audience.

The #SoConnected is a platform that aims to facilitate learning, positively influence the attitude of parents in discussing sexual and reproductive health within the family and provide practical guidance to enhance their communication skills in engaging with their adolescents.

Specifically, this initiative aims to meet the following objectives: ●

WHAT WILL IT LOOK LIKE?

#SoConnected will serve as a platform to reach parents and guardians with adolescents aged 10-19 using online and offline activities. Since the HCD process highlighted that conservative norms have largely limited the parent’s involvement in ASRH, this intervention will introduce behavioral modeling, repetition of skills, and facilitation to help families build the habit of communication. It will also enable a stable environment of online and offline resources for parents and children to practice their new-found communication skills around SRH topics, teenage pregnancy, and child rearing. This will also integrate key concepts on gender and positive parenting. #SoConnected has two components: a) Online Platform; and b) Offline Activities.

Online Platform This will be co-developed at the national level with key national line agencies.

•

Creation and development of the #SoConnected FB Page. Branding and logo will be designed to ensure consistency of image and clear understanding of the set of interventions intended for the target audience. A social media and engagement plan will be developed by a team from POPCOM, DOH, USAID’s ReachHealth project, and a consultant (TBD). Initial social media cards will also be sourced out from existing platforms and digital resources.

•

Development & Sourcing of Social Media Cards and other Digital Resources.

Materials for posting based on the social media plan may be developed by an advertising agency (TBD), POPCOM Information officers (national and regional staff), USAID’s ReachHealth project staff. It can also be sourced from contributors from other government agencies and CSOs working on parenting programs.

•

Launching or Go-live of the #SoConnected FB Page. Introducing the page to the online community is very important. Disseminating the information of the existence of the platform to relevant stakeholders, POPCOM regional offices, LGUs and community-based groups of parents will be planned out.

•

FB Lives with medical experts, parents, and adolescents. At least three segments can be considered: a) “Itanong kay Doc”, a space to discuss and provide experts’ opinion on key adolescent health topics; b) “Chikahang Magulang”, a space where group of parents can share and discuss their experiences on child rearing; and c) “Kabataan Patrol”, a space to hear voices of adolescents how they perceive their world and realities. A parent/guardian personality that teens can ask questions can be included.

Monitoring and Analysis of Social Media Analytics. Once the FB page gets some traction in the online community, regular FB insights will be collected and analyzed.

Offline Activities •

Enrollment of Parents as #SoConnected Players. After running the online campaign for two to three months, complementary offline activities will be conducted. This will also provide the program opportunities to monitor the progress or changes in knowledge, attitude and skills of parents exposed to the online messages and offline activities. A process of recruitment to join a group of parents in USG-assisted or POPCOM preferred sites needs to be established.

Community-based parent groups can be invited to join. (See Exhibit 9 in Appendices for #SoConnected Players (Sample Enrollment Form)

•

30-Day House Challenge. The House challenge may be divided into 4 themes: a) On Understanding Parenting Today and Adolescence; b) On Positive Parenting and Building Skills to Communicate with Adolescents; c) On Discussing Love, Sex, and Relationships, and d) On Sustaining a Loving and Caring Relationship inside the Home. Activities in the daily challenges will be framed using the above mentioned themes. The proposed activities will be very practical, funny, entertaining and educational. Parents enrolled can share their completed house challenge in the #SoConnected FB Page. For those who may not be comfortable posting their challenges, they may share them in a closed FB group.

•

“Parent-Teen Connect Day”. This is a time to celebrate family life and parent-adolescent relationships. A new tradition in the community where parents and adolescents meet regularly can be introduced. Business establishments can be engaged to provide discounts for parents who will have a date with their adolescents.

•

Recognition of #SoConnected Completers. A mechanism to recognize efforts of the parents will need to be instituted especially those who complete the 50-day challenge.

•

Monitoring and Evaluation. Tools to monitor the KAP and assess the program initiative will be developed by POPCOM and USAID’s ReachHealth Project.

RATIONALE

The HCD findings suggest that adolescence is a time when parents are expected by the society to pass on values, beliefs and expectations surrounding sexual and reproductive health (SRH) to their adolescents, but hiya (shame) prevents many parents from talking about these sensitive topics with their children. Effective parent-child communication has been shown to protect teens from engaging in risky sexual practices and experiencing adverse health consequences, such as unintended pregnancy. While parent-adolescent SRH communication is an important source of knowledge and emotional security for adolescents, it appears that parents and children in the Philippines are not properly equipped to communicate openly and honestly about SRH. A 2012 POPCOM study8, showed the importance of parent-child communication in promoting ASRH. The findings revealed that parents in the study sites of Bohol and Metro Manila recognized their role in ensuring the overall health, including sexual health, and well-being of their adolescent

children and that they wanted to be at the forefront of the provision of sex education for their children. However, parents admitted that their ability to do so is constrained by their inadequacies both in knowledge of ASRH and communication skills. They expressed discomfort in discussing matters related to sex with their children and received a low level of sex education themselves.

Another study conducted by Save the Children9 found that adolescents aged 10 to 14 years old had minimal knowledge of ways to prevent pregnancy, STIs and HIV. Of the adolescents interviewed, only 63% could correctly name at least one way to prevent teenage pregnancy. In Mindanao, only 24% of the respondents had ever heard of HIV/AIDS and only 15% could name at least one way to prevent it. The study also learned that the preferred source of information among this age group is their mother. Parents, on the other hand, did not necessarily have the correct information to give their children nor have the needed skills to communicate. The above-mentioned studies align with what we heard from parents and adolescents alike during the HCD Study.

The need to design a program for parents, guardians and their adolescents is also supportive of a provision under the Responsible Parenthood and Reproductive Health Act of 2012 which states that, “DepEd shall include RP-RH Education in the Parent-Teacher-Child (PTC) activities with the objective of ensuring that parents and guardians are exposed to RPRH education.” (RA10354, Rule 11.06). #SoConnected will bring both parents and their children together with the intent of not only improving ASRH knowledge, but also combating the embarrassment and awkwardness that both parties feel during these conversations. It will provide parents and guardians with real-time tools and strategies to have meaningful conversations with their children, and vice versa, about ASRH and beyond.

TEEN TERMINAL DESCRIPTION (IMAGINE STATEMENT)

Imagine a revamped community space where teens are able to safely engage in fun, positive activities with peers, regularly access reliable information, and connect with services and support that expand beyond reproductive health.

SOLUTION OVERVIEW

One of the insights gained from the HCD process is that adolescents experience a high degree of emotional vulnerability, which can lead them to engage in unhealthy relationships and risky sexual behaviors. Additionally, we heard from adolescents and healthcare providers alike that the majority of clients at the Teen Health Kiosks (THKs) are teens who are already pregnant or teens who already have children. Ultimately, the HCD process illustrated that it is extremely challenging for adolescents to access contraceptives until after their first pregnancy. From these insights, design questions were formulated to challenge the participants in the 2-day Imagine Workshop to generate possible solutions: • •

How might we support a teenagers' need for belonging to reduce risk of unintended pregnancy?

How might we make it easier for adolescents to access services before their first pregnancy?

The solution that the adolescents from the Imagine Workshop thought of was to create a space dedicated for adolescents 10-19 years old, where they can engage in fun, exciting, and educational activities in a safe and welcoming environment. The design team coined the name “Teen Terminal,” as the term “terminal” refers to a passenger station, and a Teen Terminal serves as a one-stop place for adolescents. They envisioned that it will become teens’ safe space within their community, where they can hang out, participate in various fun activities with their peers, and connect directly with the services and support systems in their communities. This initiative will reduce vulnerability and increase adolescent’s sense of belonging in a safe and productive environment.

AUDIENCE AND OBJECTIVES

The primary audience for Teen Terminal are Filipino teenagers aged 10-19 years old. Adults, including community leaders, parents, teachers, and health providers, will be a secondary audience as their approval and commitment is essential for setting up a successful Teen Terminal.

The Teen Terminal aims to provide a friendly, safe, and non-clinical environment where teens can hang out, while fostering positive peer relationships and connecting with the health services in their community. Specifically, this initiative aims to: ●

Engage LGUs to co-design and for them to lead in providing a consistent, safe, and non-clinical space in the community for adolescents to hang out and connect with services

Foster positive peer relationships by providing fun and engaging activities in a stable and welcoming space

●

Increase adolescents’ desire to seek services by making the space a fun, engaging, and appealing place for all teens to avail information and counseling

●

Improve adolescents’ knowledge and use of ASRH services before the first pregnancy

WHAT WILL IT LOOK LIKE?

The Teen Terminal will be a community space that will cater to the needs of adolescents whether they are SRH and Non-SRH. It will offer a safe space for adolescents to build healthy relationships and express themselves that current adolescent-friendly facilities appear to be lacking. Upon testing the original Teen Terminal idea with teens and their networks, many logistical issues were mentioned related to creating an entirely new space in communities that already lack many essential resources. The respondents were concerned with the safety, security, and upkeep of these spaces, and the burden that it would put on the Local Government Units (LGUs). To address these challenges, this concept note would like to introduce enterprising options where LGUs can sustain operation and maintenance of the teen terminal spaces.

There are several components to a Teen Terminal: ●

A barangay-owned, frequented and accessible physical structure for all adolescents.

The selected space will be located near a school, barangay hall, or other space where teens already frequent. It will be handicapped or person with disability (PWD) accessible and consider gender-related concerns gender transformative, so that all adolescents can use, and feel comfortable using, the space. The physical space while barangay-owned can be put up/supported by partners from the partner sector.

●

A school based frequented and accessible physical structure for all adolescents or learners. This will be an alternative place to consider teen terminal spaces in schools. This can be done from using the 5 percent budget of Gender and Development (GAD), and with annual contributions from all barangay LGUs. Schools might will be considered in this initiative as it is adolescent territory and there are teachers 5/7 days or more each week that can do the monitoring of the facility and activities therein.

Bright, youthful color schemes in the design of the space. The space will incorporate fresh, colorful designs that appeal to adolescents. As an option, designers can be hired/engaged to generate designs for the space. Adolescent groups may also be tapped to generate designs for the space, which will then be reviewed by SKs and/or a team of local adolescents and approved

by LGUs.

Spacious activity area. Spacious activity area for organized activities with the capacity to accommodate at least 20 adolescents. The Teen Terminal will offer a regular schedule of events for the adolescents as part of its routine. Every other month an event can be conducted here.

For instance, Spoken Words Day, Rap Battle, Art session day, etc.

●

Enterprising Activity Rooms. These rooms will be based on the services needed by adolescents in an area and an income generating opportunity or support to sustain the teen terminal space. For instance, an internet shop, a music or band rehearsal studio, a dance rehearsal studio, or a “cafe”. The youth groups and the LGU may recommend other enterprising services. Some of the physical space and other amenities and services will not be free, but only subsidized or discounted. Here are some options: a) some of the services being offered are paid directly by adolescent clients, b) maintenance costs are subsidized by the private sector, c) space

rental to merchants interested in setting up stalls can be explored, and d) some adolescents can set up a store or a tea stall for instance. ●

Adolescent-led activities. Adolescent-led activities must routinely be scheduled in the teen terminal. Regular menu of activities must be available or can happen daily in the different teen spaces/rooms. Special activities must be scheduled at least once a month or bi-monthly depending on the capacity of & resources available for the youth groups maintaining the teen terminal. The House officials with other volunteer adolescents will be in-charge to brainstorm the line-up activities for the year. (See Exhibit 10 in Appendices for Sample Activities for Teen Terminal)

At least 1 private counseling room/nook. There will also be an area to provide health services and counseling. The rooms will be designed and set up as entry points to deliver services to adolescents. A visiting service provider and/or social worker or counselors will be assigned to the facility.

Monitoring, evaluation and documentation. Tools to monitor and assess the program initiative will be developed by USAID’s ReachHealth Project. Documentation and profiling even the operating cost will be considered for future replication.

RATIONALE

As pointed out by adolescents and their allies, teens’ emotional vulnerability often puts them in situations and relationships that may result in unintended pregnancy. For this reason, Teen Terminal aims to increase adolescents’ self-awareness and make them feel more empowered and emotionally secure. Having this space dedicated to them, adolescents will divert their energy to fun and productive learning and social bonding activities that will help them make healthier choices, especially with regards to their sexual behaviors and reproductive health decision-making.

Currently, there are many existing public facilities that cater to adolescents in the Philippines, including “teen health kiosks”, “teen hubs”, and “teen parents’ clinics.” Teen Health Kiosks (THKs) and teen hubs function as information hubs and counseling nooks in schools and rural health units.

School-based THKs are usually run by school principals, guidance counselors, school clinicians, and other relevant school personnel, while THKs in rural health units are managed by the Department of Health and its providers. Teen parents’ clinics, on the other hand, provide maternal care specific to pregnant adolescents. While these existing facilities offer health-related services for teens, the HCD process illustrated that these spaces are not catering to all adolescents and that teens desire something more. These facilities are only operated by healthcare providers or other well-intentioned adults, but the Teen Terminal will also be managed in partnership with the Sangguniang Kabataan (SK) officials and adolescents in the community to ensure the space is truly adolescent-friendly. It is important that adolescents take some ownership in the spaces that are meant for them, so they feel comfortable seeking services and will feel more responsible for the choices they make there.

Imagine a youth-driven creative festival that focuses on love, sex, relationships, self- and careerdevelopment with linkages to services for the “whole” teen.

SOLUTION OVERVIEW

One of the insights gained from the HCD process is that adolescents trust their friends, especially those who are more experienced, as preferred sources of adolescent sexual and reproductive health (ASRH) advice and information. It is, therefore, important that adolescents are updated with the latest, accurate ASRH-related information, so that when they are asked for advice and information from their peers they are more likely to be ready and informed. This existing network for RH information among teens also provides opportunity for intervention. However, providing ageappropriate and appealing venues for teens to openly discuss and share ASRH-related experiences and knowledge in schools and the broader community has proven challenging due to lasting conservative beliefs surrounding young people engaging in sexual relationships and using family planning. Furthermore, currently in the Philippines, there are laws and policies mandating the implementation of a comprehensive sexuality education (CSE) curriculum in the country’s schools.

There is, therefore, a need to create opportunities to deliver CSE creatively and to reimagine sexuality education in a way that will allow ASRH-related conversations to be more mainstream.

From these insights, design questions were formulated to challenge the participants in the 2-day Imagine Workshop to generate possible solutions: ●

How might we leverage existing friend networks to improve access to quality and accurate information?

●

How might we create opportunities – inside and outside of the classroom – for effective CSE delivery and reinforcement?

In response to these questions, the teenagers attending the Imagine Workshop envisioned a day full of celebration and activities where young people can gather to have fun, be entertained, and reliably access accurate ASRH information and friendly services for the whole teen, including ASRH-related services. Just as the Filipino culture celebrates colorful fiestas, the participants envisioned this event to be a colorful celebration of teens, their health, and their futures that is created by them, for them and with them.

AUDIENCE AND OBJECTIVES

The primary audience for Teen Fest will be adolescents between the ages of 10-19 years old (with a particular focus on those between the ages of 15-19 years of age, as they are most at risk of pregnancy). Other community members, including parents, community leaders, teachers, etc., will be a secondary audience.

The Teen Fest is a youth-led creative festival that celebrates youth, engages youth in their own learning about ASRH, and connects them to appropriate health and development services.

Specifically, this initiative aims to: ●

WHAT WILL IT LOOK LIKE?

Teen Fest is an initiative to bring adolescents together in a safe space, whether it be in a school or in the community, to provide each other with age-based and accurate health and development information and services that are creative and fun. It is not a time for adults to tell teens what to do; it is a time for teens' voices to be heard, respected, and leveraged for their own learning. It will be an event led by teens that puts teen experiences at the center of the discussion, with supportive adults simply assisting in organization and supervision.

There are several key components to a Teen Fest: ●

Youth-led, adult-supported, planning and coordination. Each Teen Fest will be planned and coordinated by adolescents, with supervision from supportive adults. Youth will design their own “menu of activities” based on their contexts and the availability of resources. Citizenship, volunteerism and meaningful participation among adolescents will be highlighted in the process.

●

Activities to celebrate teens. All of the event's activities will be entertaining, creative, and showcase the talents of the communities' youth. It will be a safe and supportive venue to have fun, celebrate life, and meaningfully connect with peers.

●

Delivery of positive ASRH and youth development messages. Each Teen Fest will be used as an entry point to deliver pertinent ASRH messages and connect them to relevant selfand career-development resources. It will allow teens to gain a better understanding of adolescent health and development issues, while building a supportive peer network in their community with others who are going through the same challenges.

●

Link to essential ASRH and self- and career-development services. After learning about and discussing the pertinent ASRH and youth development issues to the adolescents in the given school and community, each event will also be a venue for teens to connect to and avail of essential ASRH and self- and career-development services, on site. The Teen Fest will serve as a direct link between information, education, and services.

●

A call to action. At the end of the event, all participants should hold a positive and optimistic outlook on teens and their futures. It will bring everyone together in a commitment to address ASRH concerns locally and/or support national ASRH efforts.

RATIONALE

Adolescence is a special stage in human development and is marked by rapid and formative physical, cognitive, social, emotional, and sexual growth. This distinctive developmental stage, and the numerous challenges that adolescents face during it, are real and demand special attention in the development of national policies and programs.10 Filipino adolescents and youth are recognized to hold a critical and vital role in the country’s nationbuilding. The Philippine government recognizes their leadership capabilities and the importance of their active participation, especially on matters that directly affect them. The Philippine Youth Development Plan (PYDP) 2017-2022 also seeks to enable, promote, and ensure the youth’s active, meaningful and holistic participation in society to promote their rights and welfare.11 This plan calls for “youth attendance and immersion in programs and projects conceptualized and implemented for, by and with them.” Importantly, it also calls on youth development programs to promote “the whole youth,” rather than a siloed focus of discussions and interventions on only issues affecting their physical health. It recognizes that in order to ensure healthy adolescents, we must engage them in ways that promote their social, emotional, and economic well-being as well. Teen Fest responds to this need in ASRH programming by providing information and services for the whole youth, while putting teen voices at the forefront of the event’s planning and implementation.

Exhibit 11. Example Guidelines for the Teen Terminal •

Teen Terminal will be an SK barangay-owned space and will be managed by adolescents and SK in the community.

•

The House Officials oversee the Teen Terminal. The house officials include an SK official, three younger adolescents (10-14 years old), and three older adolescents (15-19 years old). More than half of the house officials should be girls. It is recommended that a new set of officers will be chosen at least every 6 months to lead in managing the teen terminal space.

Each province will establish a League of Teen Terminal House Officials consisting of Head of Household from different barangays/cities/municipalities. The Head of Household refers to the official representative of a teen terminal in meetings and gatherings. The said league will host an annual gathering to share updates, learnings, and experiences on their respective Teen Terminals.

•

Each Teen Terminal in the community will have a Facebook group to be moderated by the house officials. Adolescents from the community will be added to the said group. All schedules and announcements will be posted there.

•

The League of Teen Terminal House Officials will also create a Facebook group to post some updates and announcements. It is important that all Heads of Household are always connected online to learn from each other to strengthen sustainability of the Teen Terminal.

•

Operating hours will be dependent on the LGU approval with concurrence from adolescent groups. Notices, posters, or tarpaulin will be posted in conspicuous places where adolescents normally congregate.

•

No alcoholic drinks inside the premises.

•

Parents and guardians can only visit the Teen Terminal during open house. Open house happens every first Saturday of the month.
