A Personal Note from Kevin Vermeer: Becoming One Team Through Our Values July 20th marked an important day for UnityPoint Health as all 30,000 employees came together as one team under a common set of values and standards for performance.

This milestone was only possible because of the determined efforts of your peers. Over the past year, managers, care coordinators, nurses, doctors, IT professionals, HR leaders, marketing experts, and many others have been working behind the scenes to make the launch of our new values a reality.

For a number of reasons, I’ve been looking forward to finally celebrating our values with all of you, one of which is that we now have a common language that connects us, defines our expectations, drives our behaviors and reinforces the habits that are central to our culture — regardless of your role or where you work.

You probably know that uniting our culture has been, and will continue to be, one of my top priorities. What you may not know, is that bringing us together as one team is deeply personal for me.

Growing up, I was blessed to have a front row seat to an extraordinary mentor — my dad. He led a mid-sized architectural company and was committed to creating a culture that brought out the best in people. He made it a priority to know everyone, as team members and individuals. And they knew him. He knew that when teams connect and build strong relationships, they don’t just work together — they work for each other.

That’s my desire for UnityPoint Health.

Those deep connections don’t just happen; they are the result of each of us intentionally investing in one another. To move from being a good organization to a great one, every day we must show others what it truly means to Foster Unity, Own the Moment, Champion Excellence and Seize Opportunities.

I’m here to assist staff members and to make our work place better. I’m the “go-to guy” for everything technology related at UnityPoint at Home. Shared resources and learnings can be found in other areas as well. UnityPoint Clinic and UnityPoint at Home share Human Resources and Marketing Communications departments, a setup that not only creates efficiencies but also infuses a core group of staff with in-depth knowledge of both organizations.

Fostering Unity between UnityPoint Clinic and UnityPoint at Home One of the tenets of our new Fostering Unity value is “collaborating across departments, business units, facilities and regions.” Together, UnityPoint Clinic and UnityPoint at Home have embodied the Fostering Unity value in recent years by working together to integrate leadership, share resources and, most importantly, improve patient outcomes.

It starts at the top. Clinical leaders from UnityPoint Clinic and UnityPoint at Home meet frequently to discuss ways to enhance quality and the patient experience. For example, Mag Van Oosten, who started in her role as Chief Clinical Officer for UnityPoint at Home in June, has already begun meeting

with Dan Allen, MD, UnityPoint Clinic Chief Medical Officer, and Marcel Devetten, MD, UnityPoint Clinic Chief Quality Officer, to explore clinical alignment opportunities.

“This type of innovative approach to viewing patients holistically and collaborating with colleagues in other settings is partially what led me to UnityPoint Health,” Van Oosten said.

“UnityPoint Health is really progressive in how it is integrating patient care.”

Similar efforts are taking place on a regional level, as regional leaders of UnityPoint Clinic and UnityPoint at Home meet with their local inpatient counterparts regularly, all with the

PCMH is centered on patient treatment coordinated through a primary care physician.

The goal is to ensure that the patient receives the necessary care when and where they need it, in a manner they can understand. This includes the various types of care provided through UnityPoint at Home, other services in the community and more.

To assist in quickly identifying and facilitating home care, palliative care and hospice care services needed for PCMH clinic patients, UnityPoint at Home clinicians have begun to participate in regular office huddles at the clinics. And in some areas where there is a higher need for behavioral health services, a UnityPoint at Home social worker comes to the clinic to connect patients with social services more efficiently.

As these goals are reached, more patients than ever will receive their care in the ambulatory setting, a change that will help UnityPoint Health achieve its overall population health goals. UnityPoint Clinic and UnityPoint at Home will be ready.

“Now more than ever these two organizations are on the same page,” says Dr. Allen. “From a leadership and an operations standpoint, we are focused on the same goal of wrapping the patient in the right level of care at the right time.”

While UnityPoint Clinic and UnityPoint at Home caregivers currently have a number of electronic interfaces to share patient information, a new tool in development will take it a step further. a movie theater with surround sound and a big screen. So his hospital care team set that as his discharge goal, writing it on his board, where he could always see it.

“Every time he would feel down, we would point at the board, go, ‘Hey, we’ve got a couple more days. We're getting close,’” said Tom Norman, Shawn's father.

With the support and encouragement of his family and care team, Shawn was discharged from UnityPoint Health – Iowa Methodist Medical Center into hospice care with UnityPoint Hospice and was able to accomplish his wish. The seamless transition was the result of his teams working together to own the moment, providing enthusiasm, accountability and compassion.

Owning the Moment: Making the Most of Your Time and Life It’s one of our new values, but what exactly does “Own the Moment” mean? As part of the UnityPoint Health team, our collective actions determine the experience we create for our patients, and Shawn Norman’s story is an example of that. It was truly a team effort of employees owning the moment that helped him realize a dream.

Shawn spent much of his life dealing with hospitals and doctors. When he was a child, 12

he was diagnosed with Noonan Syndrome, a genetic disease that is related to the heart.

It’s a disease with a wide range of symptoms, and is not very well known. There is no known cure and only limited treatments for it at this time.“It can make you tired. It can make you weak. It can make you face death,” said Shawn.

Being a big Star Wars fan, Shawn wanted to experience the new movie, Star Wars: The Force Awakens, released last December, in

A paramedic team transported Shawn by ambulance to a local movie theater for a private screening with about 30 friends and family members. The medics helped get his hospital bed comfortably adjusted in the aisle so he had the best spot to watch the movie. Horstman was by his side to monitor his condition and provide care. Shawn was thrilled to be able to experience Star Wars the way he wanted to.

But Shawn wasn’t quite done with his bucket list. Under hospice care, he was able to reach another milestone, celebrating his 29th birthday in the spring. Friends and family came to support him and help him mark

his special day. Fittingly, he had a Star Wars theme, with guests wearing t-shirts with “Shawn’s Force” on them.

“We believe that living your life on your terms is just as important at the end of life. It means knowing what’s important to the patients and families that we serve, understanding their goals and respecting their wishes,” Horstman adds. “Shawn was a strong, young man, and he was able to make many memories in his remaining time.”

On June 10, 2016, Shawn passed away peacefully at his home with both of his parents by his side. He donated his body to Des Moines University in gratitude to his doctors and medical teams, and towards research and education for his disease, Noonan Syndrome. His family is thankful for the care his UnityPoint Hospice team provided and that Shawn was able to be at home for nearly six months.

Shawn’s care teams, at both UnityPoint Health – Iowa Methodist Medical Center and UnityPoint Hospice, continually “owned the moment”, treating Shawn with compassion, empathy and respect, helping him make moments matter.

When UnityPoint Clinic Family Medicine – Belle Plaine patient, James Hayek, visited the clinic for the first time in 2014, he was suffering from abdominal and back pain. After discovering a cancerous tumor on his kidney, James began seeing Karl Holmes, PA-C, and his care team for ongoing treatment. “James was so appreciative that we found the cancer and we began to develop a strong relationship,” Karl said.

In 2015, James’ kidney continued to be monitored and a reoccurrence of the cancer revealed that it had become metastatic. With Karl’s guidance, James decided to undergo nephrectomy and oral chemotherapy in 2016.

“Although James was nervous about the procedure and the medication, he had a lot of trust in what I said and continued to confide in me as new things happened,” Karl said.

James continued to receive care from his nurses, Kim Pecenka and Vanessa Palmer, as they treated an abscess that James developed in 2016. “Our nursing staff is absolutely exceptional,” Karl said. “The friendship that our

nurses developed with James through simple wound care was very meaningful to him. They went above and beyond what they needed to do for his care.”

James’ care team continued to go the extra mile to schedule regular visits with him, accommodating his request to participate in a highly anticipated turkey hunting trip.

“James was worried that he would be late to his appointment when he returned from his trip,” Kim said. “I assured James that I would wait for him if he was late. His health was our first concern and we would do whatever it takes to keep him in good health.”

During Nurses Week in 2016, James was moved to show his appreciation for the exceptional care he received and decided to present the clinic with an award. Accompanied by his family, friends and the Belle Plaine Mayor, Dave Fish, James expressed his gratitude during a surprise visit. “Everyone from my patient service representative, Daralee Schnor, to the nurses and providers worked as a team to coordinate my care,” James said.

“I’ve never experienced a team that was more cheerful and accepting. They are more than deserving of this award.”

UnityPoint Clinic Family Medicine – Belle Plaine staff and providers were touched by the gesture of appreciation from James. “We are all grateful to be part of the team and to help the people of this community,” Kim Pecenka said.

“I was touched that James was so happy with the care he received. Words can’t express how much it meant for our clinic to be honored in this way.”

Vanessa Palmer attributes James’ successful outcome to the dedication of the staff and providers to their patients. “Every patient that walks through our front door is our friend, neighbor or family member,” Vanessa said.

“Our clinic is able to champion excellence by supporting and encouraging each other.

One opportunity identified was to create a patient-centric model where patients could be seen early in the morning before work or late in the afternoon following work. The proposed plan would extend the hours of all five pediatric clinics – opening one hour earlier and closing one hour later – with at least one provider offering acute care access during high demand hours.

In order to put the plan into action, Dr. Rinderknecht met with all five clinic site leads to introduce the plan. Site leads then presented the plan to the pediatric department who responded with valuable feedback.

“It was truly a physician-driven process,” said.

Dr. Rinderknecht. When the final version of the plan was presented, the pediatric department participated in an anonymous vote and the plan was passed by 22 to 4 votes.

Central Iowa Pediatric Clinics Seize the Opportunity to Enhance Consumer Access When faced with the strategic priority to enhance consumer access in 2016, Central Iowa pediatric clinics were challenged to improve patient satisfaction by implementing patient-friendly hours of operation. “Patient access has historically been the lowest of all our pediatric clinics’ patient experience 16

supported the initiative by measuring the impact of the change. “The Central Iowa pediatrics department was able to increase extended access hours from 32.5 hours, or 195 appointments, to 70 hours, or 420 appointments - a 115 percent increase across all five sites,” said Paul.

In order to cover the evening hours, providers missed some of the high demand office hours at their own clinics. “We were faced with a dilemma,” Dr. Rinderknecht said. “How do we increase access at all sites and not increase the workload for providers?”

For many pediatric patients, including Paul’s two children, the change to consumer access dramatically increased the availability of acute, same-day appointments. “We had an eventful ear infection and pink eye season at the Rohrer household this year,” Paul said. “I was able to call the clinic at 7:00 am, have an appointment by 7:20 am and still make it to work by 8:00 am. We also had the option to call the clinic in the afternoon and still have an appointment by the end of the day.”

“The plan to enhance consumer access is a great example of providers, operations and strategy working together to seize the opportunity,” Paul said. “The status quo was challenged, hours of operation were transformed in an innovative way and we created a strong, effective partnership between operations and strategy. Creating better opportunities for patients to access UnityPoint Clinic and our team of providers was the right thing to do for everyone.”

4 medium potatoes, peeled and cut into large chunks

1 medium red bell pepper, seeded and chopped

1 1/2 teaspoons paprika

1 teaspoon onion powder

1 teaspoon garlic powder

1/4 teaspoon salt

•

1/4 teaspoon oregano

•

1 clove garlic, sliced

•

1/4 teaspoon thyme

•

1/2 cup dry bread crumbs

•

1/4 teaspoon finely ground black pepper

•

1/4 cup olive oil

•

1/8 teaspoon cayenne pepper

paprika to taste

•

1 1/2 teaspoons brown sugar

•

salt to taste

1 pound salmon fillet

•

2 teaspoons corn oil

ground black pepper to taste

GRAPEFRUIT SALSA: •

2 cups grapefruit sections

•

1/3 cup chopped green onion

•

1/4 cup chopped red bell pepper

1/4 cup chopped fresh cilantro

1 jalapeño pepper, diced

Preheat oven to 375º F.

2. Combine paprika, onion and garlic powder, salt, oregano, thyme, black pepper, cayenne pepper and brown sugar in a small bowl.

Preheat oven to 400º F (200º C).

2. In a medium baking pan, toss together the zucchini, potatoes, red bell pepper, garlic, bread crumbs and olive oil. Season with paprika, salt and pepper.

3. Bake 1 hour in the preheated oven, stirring occasionally, until potatoes are tender and lightly brown.

3. Cut salmon into 4 portions and brush with oil. Distribute the rub over the salmon generously coating all surfaces. Place salmon on a shallow baking pan with sides.

4. Bake for 15 to 20 minutes, until fish flakes easily.

5. Coarsely chop grapefruit into bite size pieces.

Combine all salsa ingredients and serve with salmon. Grapefruit salsa can be made ahead and refrigerated until ready to serve.
