EMPLOYEE BENEFITS HEALTH PLANS

LIFE INSURANCE

Health and Prescription Coverage

Basic Life Insurance

• HMO plans with comprehensive coverage

Employee Only

• Two different network options to choose from through Community Care

• Employee coverage amount is equal to their annual earnings times two, up to a maximum amount of $50,000.

•P rescription coverage through preferred pharmacies and mail order

• Tulsa County contributes 67% of the premium cost for this plan.

Dental Insurance Plan • PPO-Plus Premiere dental plan through Delta Dental • The PPO-Plus Premiere plan allows employees the freedom to visit any in-network dentist, without referrals.

Vision Insurance Plan • VSP provides coverage for annual eye exams, contact lenses, and eyeglasses

Flexible Spending Accounts (FSA) •H ealthcare Account- Allows an employee to save money on healthcare expenses (such as physician co-pays, prescriptions, deductibles, dental, and vision expenses) with pre-tax money (subject to IRS guidelines) •D ependent Care Account- Allows an employee to save money on dependent care expenses (such as child daycare or adult assisted living expenses) with pre-tax money (subject to IRS guidelines)

Optional Life Insurance • Employee can elect coverage in increments of $10,000, up to a maximum of $500,000. • $250,000 is the guaranteed issue amount for employees • E mployee can elect coverage for spouses in increments of $10,000, up to a maximum of $ 250,000 (not to exceed 50% of the employee’s elected amount). • $20,000 is the guaranteed issue amount for spouses • E mployee can elect coverage on their children (up to age 26 years) in increments of $2,000, up to a maximum of $10,000.

Accidental Death and Dismemberment (AD&D) • Employee can elect coverage in increments of $10,000 up to a maximum of $500,000. • E mployee can elect coverage for spouses up to 50% of employee’s elected amount. • E mployee can elect coverage for children (up to the age of 26) up to 15% of employee’s elected amount.

SUPPLEMENTAL & DISABILITY

Employee Assistance Program (EAP)

Cancer Insurance

•C ompletely confidential counseling services offered through Community Care’s licensed counselors

• Coverage for cancer and 29 other specified diseases •P ays for cancer treatments, hospital stays, transportation, hospice care, and more

Short Term Disability (STD)

Employee Only • 50% of base monthly earnings • Elimination period of 14 consecutive days of disability • Coverage for up to 11 weeks of continuous disability

Long Term Disability (LTD) Employee Only

• 50% of base monthly earnings • Two different long term disability plans are available (Base and Buy-Up LTD). • Tulsa County contributes 100% of the cost of the premium on the Base LTD plan. • Benefit duration on the Base LTD plan is two years. • Benefit duration on the Buy-Up LTD plan is up to age 65.


EMPLOYEE BENEFITS Accident Insurance

• Pays for injuries and events resulting from an accident while off the job • Benefit amount depends on the type of injury and care received. • Benefits can be used to offset medical expenses (such as deductibles and co-pays), home healthcare costs, reduction in income due to lost time at work, and everyday expenses such as utility bills and groceries.

Critical Illness

• Pays a lump sum benefit if diagnosed with heart attack, stroke, kidney failure, coronary artery bypass, coma, major organ failure, permanent paralysis

Tulsa County Paid Holidays • New Year’s Day, Martin Luther King, Jr.’s birthday, President’s Day, Good Friday, Memorial Day, Independence Day, Labor Day, Veteran’s Day, Thanksgiving and the Friday after and Christmas Day

Family and Medical Leave of

The regular retirement benefit, paid monthly to retiree, Absence (FMLA) is calculated using the employee's average of the three highest years of base payroll earnings. Follows the rule • E ligible after 12 months of consecutive employment and 1,250 hours worked prior to leave of 90 (age and years of service).

457 (b) and 401 (a) Retirement Savings Plan (Deferred Compensation Program)

•U p to 12 weeks of unpaid leave • L eaves are subject to approval by following DOL guidelines

•P re-tax retirement savings administered through BOK Financial • County will match up to $50.00 each month • Immediately vested (after benefit waiting period is completed)

Jury Duty Leave

Post-Employment Health Plan (PEHP)

Funeral Leave

• Eligible after completion of twelve months of continuous full time employment •C overs healthcare expenses such as insurance premiums, prescriptions, physician co-pays, dental expenses, and vision expenses • Tulsa County funds the cost for this plan at 100%. • There are two accounts within the PEHP that the County will fund monthly. • Tulsa County will contribute $40 per month into the Universal Account (healthcare, dental, and vision

• Employee must provide a copy of the jury summons. •W ill receive full pay while on jury duty without charge to employee’s leave banks • Employee will receive up to three days of full pay if funeral is for an immediate family member as defined by policy.

Military Leave • Employee must provide a copy of the military orders. •S ubject to USERRA guidelines

Employee’s Federal Credit Union

•O ffered after completion of 90 days of continuous full time employment • Reimbursement of 75% tuition and lab fees (per policy guidelines) after successful completion of course(s), must make a “C” or above • $ 1,000/calendar year $5,000/lifetime maximum •R equires an employee to remain in a full time status with Tulsa County for one full year following completion of course(s)

•H oliday Savings Program - If you have a balance of $1,326 by October 15th of each year, then the credit union will deposit $52.00 into your account on or by November 1 each year.

Tulsa County reserves the right to modify, amend or terminate any benefit plan or practice described in this brochure. In all events, the terms of the Plan as set forth in the Plan document govern and, as a result, no statements made outside of the Plan document, whether verbal or written, change or modify the terms of the Plan. The Plan can be amended only in writing and only by the Tulsa County Board of County Commissioners.
