Welcome to the second issue of Impressions, the newsletter dedicated to our very

own Tufts ASDA chapter!

This publication is designed to keep you informed about issues that relate to ASDA and organized dentistry on a national scale, as well as concerns that pertain exclusively to our local Tufts experience. It functions as a space for students to voice their opinions on the issues that matter to us as dental students.

Our second issue examines optimism both in the personal and professional environment. This newsletter arrives in the month of February, fraught with a bleak sense of the winter doldrums. However, in the midst of the frigid weather, we encourage you to think of the future warmth of spring, “look on the bright side,” and assess how you can infuse this positivity into your experience at Tufts.

Within the pages of this paper we delve into the future of dentistry as experienced by students. We recap some of the innovations seen at the Yankee Dental Congress, and showcase how one exceptional student makes it his mission to educate future generations about oral health in conjunction with ASDA’s Smile, Share and Care program.

We also explore the ethical ramifications of our profession through an overview of the annual ASDA debate and the concept of “volunteer tourism.” Finally, one of our esteemed faculty members shares her wisdom of well-being and how to “look on the bright side” in our personal lives.

As I peruse the articles in this magazine, I’m honored to be a part of a profession that has the ability to constantly adapt, and part of a community of those passionate enough to share their vision of the profession’s future with others. As you read, I hope that you too are inspired and humbled by their opinions and experiences.

“Everyone Smiles”: An Approach to Children’s Dental Education and Awareness

is a universal language. A simple smile can lift spirits and bring people closer together. In order to maintain a healthy and beautiful smile, proper dental care is essential.

This is the message that Tufts dental student, Gary Kersey Jr., conveys in his children’s book “Everyone Smiles,” an educational step-­by­-step guide on how to keep teeth clean.

February is national National Children’s Dental Health Month, and the book was published a year ago on Feb 15th, 2015. The book aims to raise awareness of children’s dental health, and shows the diversity and beauty of smiles.

Since its inception last year, the innovative children’s dental health book, co­-authored

by Tufts dental student Gary Kersey Jr. and Ashley N. Grisham has been making a global impact from book readings and brushing lessons given to the SOS Children’s Orphanage in Jamaica all the way to the classrooms of

elementary schools in San Diego, California and YMCA’s of North Carolina. The book’s popularity and accessibility has travelled across the nation and even internationally, to the UK. Kersey’s vision is to make the book accessible to children of all communities, whether in dental offices to the middle-­ upper class, public libraries, or social programs that target underserved populations.

“Everybody Smiles” hits the literary market at a time when lack of access to affordable dental care is causing residual effects and detrimental impacts on the general health of low-­ income Americans, especially in children. Tooth decay is the most common chronic illness among school­-age children, yet it is almost entirely preventable.

Furthermore, the rate of caries among low-­income children is

Tufts ASDA Impressions more than twice that for children with more income (31% versus 14%). Ironically, programs like Medicaid and CHIP cover comprehensive dental benefits for children, but 30% of children with private health insurance are uninsured for dental care.

The smile is the integral first introduction a child can make with another. The power of the smile speaks to the mouth’s capability of serving as a global symbol of kindness, tolerance, and positive curiosity.

All of which are vital aspects of community building that children can learn, not by being

told, but rather shown. The goal is that the book is read and viewed in an interactive way. The book has been well received so far. The message will hopefully continue to spread across the world and encourage children to focus on the importance of maintaining a healthy smile.

ASDA’s “Smile, Share, and Care” is a Program held at the Wang YMCA on most Monday afternoons. We read dental related books to the kids and teach them proper brushing techniques using stuffed animals that have typodonts in them and large toothbrushes. The energy in the classroom from the students is infectious and they brighten up the day!

Yankee Dental: An Overview

T

he first time I went to the Yankee Dental Convention, my biggest concern was making sure that I had my animal balloon made after I went to the face-painting booth. This was over a decade ago, when the Yankee was hosted at the Hynes Convention Center, and my family and I would stay at the Mariott to peruse the Prudential and children’s booths while my mother attended to her dental duties. As I grew older, I transitioned from playing games and getting cotton candy in the kid’s section, to playing games and getting regular candy from the dental booths. I helped my mom pick out toys for her patients. My brothers and I stared at rainbow retainers on the wall, and we practiced drilling holes in pennies to test out the latest high speed handpieces. Having a parent who practiced dentistry in the state of Massachusetts allowed me to gain multiple perspectives of the Yankee Dental Congress at various stages of my life. This year, I graduated from the “Guest” to “Dental Student,” badge and attended my first dental convention independently as a student of TUSDM.

Continuing education is an important part of maintaining and updating information in the dental field. In the state of Massachusetts, dentists, hygienists, and dental assistants are all required to accrue a certain number of Continuing Education Units in order to maintain their licenses (40, 20, and 4-6 CEU’s required every two years for dentists, hygienists, and assistants respectively). These credits can be obtained by attending courses such as those offered at the Yankee Dental Congress.

This year, Yankee hosted over 450 exhibitors and offered more than 60 courses for students, hygienists, assistants, dentists and guests to attend. Some of the lectures were hosted by our own faculty members such as “ Successful Options for In-Office Whitening: A Practical Approach,” by Dr. Gerard Kugel. Over the weekend, ASDA also hosted their 7th Annual District One Student Debate, where local dental school students, including two Tufts D’18 students, Alec and Tom,

D’19 student Kimyen Nguyen and Graduate student/Operative TA Fulvio Fratipietro encounter each other at the Yankee.

participated in discussions on various ethical and policy issues. Tufts Dental students also participated in volunteering for the Yankee HIV Dental Workforce booth, which promoted awareness of HIV-related oral health issues.

Every year there are many odds and ends that are hosted at the booths: hand painted artwork for dentists to showcase in their offices, thousands of new appliances and dental materials for offices to stock up on for the year, and even massage tools and chairs to improve ergonomics while working. Photos were scattered on social media of free toothbrushes, toothpaste, tooth whitening materials, and even desensitizing gum strips from Crest.

A few highlights of this year included a laser hand piece by Solea of Convergent Dental, and a tooth brushing station by Crest, where attendees could try out new toothpaste on electric Oral-B toothbrushes. D’19 Student Soroush Samimi was able to try the handpiece and stated: “After learning about the recent leaps in laser handpiece technology, it was a wonderful experience to try Solea, isotopic CO2 laser handpieces, on soft and hard tissue models.

The technology allows for an anesthesiafree experience for the patients, and enables dentists to operate on multiple quadrants in one visit.”

Tufts ASDA Impressions are garnering popularity in the mainstream dental market.

CAD/CAM machines and similar types of milling devices were in abundance, featuring live patient demos and devices that were constantly in motion fabricating orthodontic appliances, occlusal guards, and prostheses. Additionally, intraoral cameras seemed popular with local dentists.

These booths attracted many new and experienced dentists looking to expand and improve upon their practices. As a student meandering the floor I wonder which products that seem new and exciting will one day become commonplace in private practices, and which innovative products I will be shopping for in the future.

In general, the Yankee is a great way for anyone involved in the dental community to explore the latest research, dental topics, updates, and policy changes that are happening day to day. As students especially, Yankee offers many opportunities to test our interpersonal networking skills, learn more about topics that we have touched on in class, or to simply pick up the toothpaste or floss that we haven’t had time to grab from the grocery store.

Tufts Take Ten: 1. In the mood for some authentic chinese food? Try dim sum in Chinatown. Some of our readers suggest Winsor Dim Sum Cafe and China Pearl, both located on Tyler Street.

2. When entering clinic, be sure to introduce yourself to the staff in the dispensary.

They can be a huge asset during your appointments. (2nd Floor- Kerline and Sara; 3rd Floor- Brian, Emily, and Allyson; 4th Floor- Julie and Courtney) 3. Dental school is a lot of work, but don’t forget to continue to make time for your favorite hobbies and friends. These two things will keep you sane while juggling so much.

4. Stay around Boston for a few days during breaks to assist 3rd/4th years, learn your way around clinic, and get to know upperclassmen.

5. When you’re learning a new technique for the first time, it is important to remember you’re not being trained to become a dentist in one day. It doesn’t need to be perfect on your first try or your twentieth try. Just seek progress.

6. Often while studying for exam blocks, the only thing that is on a student’s mind is the exam. Remember to take small breaks while studying to let your mind rest and not burn out too quickly.

7. Did you know Tufts Dental is located right next to the Theater District? Go watch a Broadway musical right next to school. Some shows will have a special lottery that occurs an hour or two before curtain where you can get discounted tickets.

8. To stay on top of lab work or patient management, pick one clinic session every week to devote to this work.

9. When taking a final impression for a crown, make sure the prep is as “dry as a desert”. Use cotton rolls, Dri-Angles, the saliva ejector… whatever it takes.

10. Try to study up on a procedure in advance. If it’s your first time, try to review the procedure with a clinical faculty of that department. If you have an idea of who you’re working with, it may be easier to speak with that professor and give them a heads up that you’re hoping to do a certain procedure with them soon.

However, the research cited for developing this exam indicates that other options may be more appropriate. Since ADEA surveys of admissions directors state that they look at academic talent as one of the last determinants for success in their program, we wonder if simply adding another exam is truly the solution. Students vocalized that they would like another exam, but that it should be specialty specific, or back to boards with a validated numeric grading system.

The ADAT pilot will begin in spring of 2016.

It will be interesting to see how students, test administrators, and postgraduate admissions directors view the exam.

In addition to addressing the ADAT pilot exam, Eidelman and Whitmer also discussed the merits and service of a special care dentistry specialty. A special care dentistry specialty would serve populations of those who are geriatric, hospitalized, or possess intellectual developmental disabilities.

The Special Care Dentistry Association has worked to establish this specialty to treat patients that present with difficulties

in care that the general dentist is not prepared to safely address.

This specialty would provide a referral source for pediatric, general, and public health dentists to aid in collaborative, complex treatment.

As outlined by the ADA specialty application, the “distinct and well-defined field� is required to provide care that is not fulfilled by combinations or minimal modifications of the other nine established specialties.

The need for competent dentists is stronger than ever, and the education process must continue to research

and address the needs of all patients through the safest and most efficient means possible.

Learning how to discuss current issues in an informed manner is an important lifelong tool for any professional to develop. The success of the 2016 ASDA District 1 Debate demonstrates how collaboration between students, legislators, sponsors, and professional dentistry organizations can discuss how to responsibly address the oral health needs of the present and future.

Pursuing Volunteer Tourism the “Right” Way

Throughout

the winter of 2014, I watched, with rapt attention, the events unfolding in Ukraine, my birth country.

I first felt elation at the camaraderie of the revolution, then confusion at the appearance of militants in Crimea and the East, and lastly despondency at the bloodshed in the East.

I have family in a part of Ukraine that hasn’t seen any violence during this conflict. They tell me about the deteriorating economic situation, the internally displaced refugees, and the obstacles many people face to obtain medical care.

But how could I help in any meaningful way? As a student I have limited funds. What about skills? I speak rusty Ukrainian, and even worse Russian. I have half a dental education. Are these skills enough? Enough for what?

I started looking into volunteering opportunities and

found little in the dental field.

This was especially true when I ruled out organizations with any kind of agenda to proselytize religion. I wasn’t surprised that Ukraine was not an epicenter of volunteer projects organized from the US. The languages spoken in the country are not ones many Americans know.

The weather (comparable with Boston’s) does not lend itself to the popular model of volunteer tourism: some proportion of work on stated objective and some component of tropical beach vacation.

One of the most viable volunteer options I found required me to organize my own team of dental care providers and equip them with portable versions of standard dental operatory machinery: portable x-ray machines, drills, suction.

The organization (actually a Ukrainian-American family) work as fixers in a remote

village, far from any active fighting, to situate visiting dental squads in a healthcare clinic where they may carry out an array of dental services.

I talked with a few administrative figures here at Tufts. The response was encouraging, but not exuberant.

Sure, we might have some supplies, but portable x-ray machines would not be forthcoming. Perhaps getting a faculty member involved would lend the trip more supervision and credence.

As thoughts and plans were fomenting, I was considering more broadly the merits and shortcomings of volunteer tourism. I had taken Dr. Morgan’s class on Global Service Learning projects, which is in essence, what you call volunteer tourism when it’s done right. “Right” means that a project actually garners benefits onto the people or community 15

Tufts ASDA Impressions that is ostensibly being helped, and that it does so in a respectful, inclusive, efficient, and sustainable manner.

I’d also done some outside reading on the matter. An NPR article informed me that, “Volunteer tourism is one of the fastest growing trends in travel today. More than 1.6 million volunteer tourists are spending about $2 billion each year.” And that, “Most volunteer tourists are women. They’re also young adults, between the ages of 20 and 25...But more and more high school students are also traveling and volunteering.” These programs often attract people with a paucity of domestically useful or lucrative skills (“more and more high school students”) seeking to represent themselves as benevolent and experienced via payment for translocation to a photogenic location, in hopes of recognition back home, and perhaps admittance into programs and experiences that might eventually make them useful. This is a very long way of saying: resume padding.

At the Boston Yankee Dental Conference this year I chanced upon the Christian Dental Society’s booth. They had portable dental equipment available for rent! They were exuberantly encouraging, even after I’d mentioned I was a student. Would they supply a student with the means to do dentistry in a foreign country?

Unsupervised, this would be a terrible idea: just ask the harried faculty member I’d needed help from multiple times for that #31 DO amalgam. (It was really deep

and sub-gingival so don’t judge.)

Let’s give the CDS the benefit of the doubt: I’d also mentioned my mother, a licensed, practicing dentist, was interested in the same service trip.

Maybe this is why they seemed forthcoming with an equipment loan. And their website contains this information: they “make available this portable equipment...to current CDS members...All equipment loans must be approved by the Home Office...It is a requirement that all members planning to go on mission trips need to fill out a CDS Mission Application, [and] sign the equipment loan policy”. Presumably herein are contained checks on your dental credentials and ability to use the equipment safely, an essential component of a dental volunteering done right.

NGOs and charities have long been working on ways to measure their effectiveness.

This is more difficult for them to do than for a corporation because non-profits cannot simply measure monetary

Tufts ASDA Impressions return on investment. They must find some way to quantify amorphous concepts like “public good”. It’s difficult, but we should endeavor to apply similar standards to service learning projects and volunteer tourism.

A well-designed and implemented service trip does a maximum amount of good per dollar and manhour. If healthcare is being delivered, then the number of positive healthcare outcomes, compared to negative ones, should be maximized. Often the most effective and efficient use for time and money is to contribute it to someone

already on location, trained and positioned to render the service you would need to travel and training to provide. Ukraine doesn’t lack dentists, and the dental care in the wealthiest, most cosmopolitan centers is on-par with American care. As in the US, the demographics that lack care do so because they lack the funds to attract trained professionals. Paying (or otherwise enabling) a culturally-competent, licensed, experienced Ukrainian dentist to work on the underserved population could reach a large number of people. Funding someone

relatively local could also ensure follow-up possibilities, reducing morbidity from postprocedure complications.

How’s my volunteer trip to Ukraine coming along?

I’m determined to do it right, and increasingly this seems to mean skipping on the plane ticket, and sending money and supplies instead. I will miss out on the personal experience and gratification of hands on work abroad, but I yearn for efficacy over personal glory.

Kitchen Spotlight Preheat the oven to 425 °F. Lightly grease the baking sheet with oil.

Mix the flour, oats, sugar, baking 1.5 cups of flour powder, salt, cherries in a large bowl.

2 cups of rolled oats Beat the egg in another bowl. Stir in 0.25 cups of white sugar the melted butter (or oil) and milk to the 4 tsp of baking powder beaten egg.

0.5 tsp of salt Stir the wet ingredients into the dry 0.5 cups of dried cherries ingredients and mix to create the dough.

1 egg, beaten 0.5 cups of melted butter/oil Form triangular wedges of the dough (about 3in wide and 1in tall) and place 0.75 cups of milk them onto the baking sheet. Bake the dough for 15m or until they have risen and turned golden brown.
