Celebrating Life & Love

Pride & Purpose

As we celebrate this momentous occasion, we also recognize the importance of looking ahead with the same enthusiasm that has carried us through the past 45 years. The values that have guided us, the lessons we have learned, and the bonds we have formed serve as the cornerstone upon which we will continue to ensure the highest level of compassion at end of life care is available to all.

Physician Fellowship

The Trustbridge Hospice and Palliative Care Fellowship Program is an esteemed opportunity designed to cultivate the next generation of healthcare leaders in hospice and palliative medicine. This program offers a comprehensive curriculum that integrates clinical practice, research, education, and provides fellows with a robust foundation to deliver compassionate, patientcentered care. Fellows work closely with experienced mentors who guide them through the complexities of end of life care, ensuring they acquire the skills needed to address the physical, emotional, and spiritual needs of patients and their families.

Fellows collaborate with a diverse team of healthcare professionals, including nurses, social workers, chaplains, and therapists, to develop holistic care plans. This teamwork not only enhances the quality of care provided but also fosters a deep understanding of the various roles and perspectives involved in hospice and palliative care.

Trustbridge’s Hospice and Palliative Care Fellowship Program is a cornerstone of our commitment to providing exceptional care.

Extraordinary Comfort Care

Trustbridge stands out among the mere 3% of hospices in the US that offer Extraordinary Comfort Care, a medically sophisticated case management program also known as palliative interventions during a hospice patients plan of care.

The Extraordinary Comfort Care program grants patients with life-limiting illnesses unrestricted access to hospice care, eliminating obstacles to their comfort and well-being. It empowers their physicians to optimize symptom management and continue a range of treatments like radiation, chemotherapy, or transfusions during hospice care.

Nicole’s Journey A Pink Warrior

Nicole, a beautiful 41-year-old, bravely faced the challenges of battling breast cancer, enduring unbearable bone pain. Even potent narcotics failed to alleviate her suffering. Determined to find relief, her hospice physician explored alternative medications, leading to a remarkable outcome. Nicole not only found relief but also regained stability, bidding farewell to hospice care. She joyously resumed walks with her faithful canine companion, cherished lunch outings with loved ones, and savored the simple pleasures of everyday life.

For six precious months, Nicole basked in the beauty of her renewed vitality before transitioning back to hospice care at Trustbridge. Despite lacking income or insurance and being ineligible for Medicare due to her young age, Trustbridge wrapped Nicole in compassionate care and support. Through Trustbridge’s Pet Peace of Mind program, her cherished pet received care and provisions, ensuring his well-being as Nicole embarked on her final journey, granting her peace of mind in her final days.  Nicole passed peacefully with him in her arms.

Hospice Nurse Residency Program

The Trustbridge Hospice R.N. Residency Program is the backbone of educational excellence and support for newly graduated nurses, inexperienced R.N.s, or those returning to the nursing practice interested in expanding their knowledge of end of life care. It offers a unique and collaborative learning environment that is meticulously designed to equip participants with the comprehensive skills and knowledge required to excel as primary nurses. This program underscores the importance of a strong educational foundation in nursing, particularly in the specialized fields of hospice and palliative care, where sensitivity, expertise, and compassion are paramount.

Completing the Trustbridge Hospice R.N. Residency Program is a milestone that opens up further opportunities for nurses to deepen their expertise. A year after completing the residency, successful R.N.s become eligible to sit for the Hospice & Palliative Care Certification examination, a credential that highlights their specialized skills and dedication to providing the highest standard of care to patients with life-limiting illnesses.

In 2015, I was selected to train in the Trustbridge Nurse Residency Program. Starting off as a new nurse was both exciting and terrifying.

The Nurse Residency Program set me up for success by giving me the time to train with seasoned professionals and practice my skills well before being on my own. After almost 10 years, I could not see myself anywhere but in service to hospice patient care. It has been quite a journey, and my coworkers are truly the most compassionate, kind, and hard-working people I have had the pleasure to meet. The experience was truly invaluable.

loved ones. I have been fortunate to be a part of many wonderful moments during visits.

One visit was a patient who was being visited by his son who was from out of town. They welcomed me into their room.  The patient had not been eating anything except vanilla ice cream. The patient’s son gave him the option of two different kinds of getting the patient’s review on the two different brands.

Yet, the patient was aware that he was declining, and he would continually ask his son how far along in the dying process he was and if he would live until Thanksgiving. The son was very supportive and reassured his father that whatever happened it would all be okay. As I began to play some music that the patient preferred, he began to weep. I gently played my guitar and stopped singing to ask the patient if the The patient responded through tears, “It’s beautiful.” The patient’s son began to cry as well.

Afterwards I spoke to the patient about what a wonderful and supportive son he had, the patient’s son said “I always tell Dad that he needs to hold up a just a reflection of him.” The son began to cry again, and the patient was tearing up as well, but looked so proud of his son.

At the end of the visit, both the patient and the son expressed how thankful they were for the music therapy visit, but I was the one who should have been thanking them for allowing me in to such a precious moment. This is a visit I will never forget and am forever grateful to have been a part of.

Our Origin Story

The origin of Hospice of Palm Beach County & Hospice by the Sea, now Trustbridge, is a testament to the compassion and dedication of individuals who sought to provide quality end of life care for patients and their families.

Founded in 1978, Hospice of Palm Beach County and Hospice by the Sea were founded in service to their communities of Palm Beach & Broward counties, serving a noble mission to deliver comfort, care, and dignity for all at the end of life.

Its roots were deeply influenced by the growing hospice

in Connecticut. The focus was on providing comfort, pain management, and emotional support to terminally ill patients, the care gained recognition as a more humane and holistic approach to end of life care compared to traditional end of life medical interventions.

Hospice is not a place, but a philosophy of care.

One of the pivotal moments in hospice care history came in 1979 when Governor Bob Graham signed the Florida Hospice Act. This legislation helped pave the way for the growth and development of hospice services in the state. This recognized the importance of hospice care and provided a framework for its regulation and support in

It was a real honor to receive such a warm invitation to this special event today. I regret that I cannot be there in person with all of you to celebrate the 45th Anniversary of the signing of the 1978 Florida Hospice Act. The landmark legislation sparked the beginning of one of the most critical healthcare needs in people’s liveshospice care for those approaching the end-of-life and support for their families.

Today we all know how valuable hospice services are to people. We’ve all seen the impact of good palliative care and support services for those we’ve known personally or heard the stories from the dedicated hospice caregivers and volunteers who feel honored to be present at this most sacred time in a person’s life and to be available to their loved ones during a very painful and difficult time.

You, here at Trustbridge Hospice, have first-hand experience in touching over 200,000 families in South Florida since you opened your doors in 1978. Your palliative medicine, caregiving, and bereavement support truly provide the medical needs and compassion for those actively dying and for their loved ones. Palm Beach and Broward Counties are fortunate to have such dependable resources for end-of-life care and support.

I personally want to congratulate you and all of the hospice centers here in South Florida and around the globe who continue to provide hospice care to those in need. Your compassion and dedication to this profession is immeasurable. You should feel extremely proud of the services that you have provided during the most critical time in people’s lives. Congratulations and many thanks again for all you do.

I chose Trustbridge Hospice because I couldn’t think of a better cause to become my beneficiary.

My Jewish European parents met after WW2 and Hitler’s torture. My father lost his parents and 7 out of his 8 siblings. My mother lost her parents and only sibling. I became their one and only child.

As the years went on, they became ill. It was up to me, and only me, to take care of them.

Without the courageous and caring people at hospice I don’t know how I would ever have been able to care for my mother whose dementia had taken over her life.

I couldn’t thank hospice enough for their aid. It is for that reason I am so thankful to hospice.

Our first interaction with Trustbridge Hospice exceeded even our highest expectations.

As a family, we didn’t know what to expect when we made the first call and what came afterward. The first person on the phone was Jason. He was compassionate, caring, and an excellent listener (being heard is magical). When Nurse Susan showed up at our home yesterday afternoon, she was a beacon of light to guide us through our dark tunnel of fear. Nurse Susan asked the hard questions and gave us the answers in a way we could understand.

My 88-year-old mom who lives with my brother who entered hospice care said “She was so caring, empathetic, and informative.” My mom insisted we write this glowing feedback for Nurse Susan and the entire Trustbridge team who we have interacted with so far. Thank you and keep up the excellent support.

Boldness is in our DNA.

We embrace challenges as opportunities. We do whatever it takes, as we see no boundaries that confine our aspirations and strive to break through the ordinary in pursuit of the extraordinary.

We believe by embracing these qualities, we can create genuine connections and lasting positive impact in the lives we touch.

This is Trustbridge; a fusion of passion, compassion, trust, boldness and an unwavering belief in making every day count in service to our community.
