Welcome Trustbridge
In 2024, Empath Health reached a significant milestone with the affiliation of Trustbridge, a leading provider of hospice care in Southeast Florida. This strategic partnership not only expanded our geographic reach but also strengthened our ability to deliver innovative and high-quality care to individuals and families on Florida’s east coast.
By joining forces, Empath Health and Trustbridge have created a more unified and efficient network of hospice services, streamlining operations, sharing best practices, and elevating the overall standard of care. Trustbridge has long been recognized for its compassionate and
comprehensive services through Hospice of Palm Beach County and Hospice by the Sea—organizations with deep roots in their communities and a legacy of excellence in end-of-life care.
This affiliation has significantly increased Empath Health’s capacity to serve, grow, and innovate, ensuring that more patients and families receive the support they need, regardless of financial circumstances. With a shared commitment to compassionate, patient-centered care, this expanded network enhances our ability to provide vital services, including over $17 million in uncompensated care annually.

CAREGIVER EDUCATION
In keeping with our commitment to caregiver education, Trustbridge ensures its physicians and nurses have the specialized training they need to compassionately and confidently care for families navigating the challenges of chronic or terminal illnesses in children.

Pediatric Music Therapy
In 2024, Trustbridge expanded its commitment to pediatric care by introducing full-time pediatric music therapy roles—an initiative that brings comfort and healing to our youngest patients. Carly Graefy and Saralynn Corey, who first joined Trustbridge through the Addison Hines Music Therapy Internship Program, have embraced these roles.
As part of the Pediatric Team, Carly and Saralyn develop personalized therapy plans for every child. The marvelous power of therapeutic music connects with each child and their family as it provides young ones an active role in their treatment and creates beautiful family memories.
Extraordinary Comfort Care: Bridging Gaps in Pediatric Care
Families are deeply grateful for the Foundation’s Extraordinary Comfort Care fund, which provides for extraordinary comfort measures, and care needs.
The Affordable Care Act’s (ACA) Concurrent Care for Children Requirement (CCCR) ensures that children receiving hospice care can also continue life-prolonging treatments. This means families do not have to choose between comfort-focused hospice support and ongoing medical treatments that may extend their child’s life. However, when

TRAINING THE NEXT GENERATION OF CAREGIVERS
Hospice Nurse Residency Program
Amid the national nursing shortage, ensuring quality hospice and palliative care remains a priority. Trustbridge’s Registered Nurse Residency Program directly addresses this challenge by equipping new nurses with the skills, confidence, and hands-on experience needed to provide compassionate, holistic care. By investing in nurse development, Trustbridge is strengthening the future of hospice and palliative care while ensuring our community continues to receive the highest level of support. This innovative program is actively bridging the staffing gap, one skilled and confident nurse at a time.

AMAYAH GABRIEL
“Interning with Trustbridge has had a profound impact as a clinical professional and my personal growth. While developing my skills in supporting patients and families during their most vulnerable moments, I built relationships with patients, families, and team members that highlighted the meaningful and collaborative nature of hospice care and affirmed my passion for this field. This journey has not only shaped my identity and purpose as a music therapist but also helped me grow as a compassionate individual dedicated to making a meaningful difference.”
SARALYN COREY
“During my internship experience, I had the honor of working with many patients and families at endof-life. I was able to see first-hand the impact of music and the therapeutic relationship on the physical and mental wellbeing of our patients and families and receive feedback such as: ‘this is best part of my day’ and ‘the music is so relaxing; I can breathe again.’ “
CARLY GRAEFE
“My time as an intern at Trustbridge was infinitely rewarding. I was able to work alongside other music therapists as well as doctors, nurses, social workers, and chaplains, which provided me with several rich experiences and important insights into hospice as a field. On a broader level, it was a privilege to work with a company that aligns with my strengths, my goals, and most importantly, my values.”
KEVIN TRAN
“My internship experience was invaluable to my growth as a clinician and as a person. I learned and experienced so much from the music therapy team. The best surprise from my time as an intern was working and getting to learn alongside other professionals in the company!”

Meaningful Volunteer Experiences
You don’t need special skills—just an open heart and a willingness to care.
Volunteers touch the lives of hospice patients and their families each day and in so many beautiful ways.
“Volunteering at Trustbridge this year has been one of the most meaningful experiences. Sitting with patients and caregivers, listening to their stories, or simply been present, has emphasized to me the value of compassion. It reminds me how much small acts of kindness can truly matter. While it can be emotionally challenging, the fulfillment of knowing you’ve brought comfort to someone in their time of need outweighs it all.”
— Gina Afonso, Patient-Visiting Volunteer

Thank You, Mr. President
By Jonathan D. Fleece President and CEO, Empath Health
President Jimmy Carter led a remarkable life, easily regarded as one of the greatest humanitarians of his generation. But among his many accomplishments, perhaps his most courageous act in life was how he bravely and openly surrendered to death.
In February 2023, the Carter Center announced that President Carter “decided to spend his remaining time at home with his family and receive hospice care instead of additional medical intervention.”
Hospice care is often stigmatized as giving up, but Carter’s choice marked the beginning of a profound, twenty-two-month journey towards
acceptance of life and eventual death.
In typical fashion, Carter challenged this notion with the grace and steadfastness characteristic of his sixty years in public life. When his prognosis became terminal, he quietly enrolled in hospice care, inviting a team of nurses, aides, counselors, chaplains, and therapists into his home in Plains, Georgia. Rather than battling death, he chose to spend his final months in the place he loved, surrounded by the people he cherished.
Imagine how different our world would be if we all embraced life’s end with such inspiring bravery; if we allowed hope to exist in the present moment. In the United States, hospice care is available to every person in the six months leading up to their death, yet most patients spend just a few days or even hours in specialized end-of-life care. According to
the Centers for Medicare & Medicaid Services, the number one complaint from family’s postservice is wishing their loved one had started hospice care earlier.
The goal of hospice care is to empower patients to live their fullest lives for the remainder of their time, supporting them in defining what that looks like. Although the Carters have kept much of the former President’s care plan private, his presence at the funeral of his beloved wife, Rosalynn, last year was a profound demonstration of what patient-centered care looks like.
As an admirer of President Carter’s care from a distance, the tens of thousands of hospice patients Empath Health serves each year give me a respectful perspective on the effort and care that it likely took for the hospice team to make his presence at the funeral a reality. It must have required multiple hospice specialists, from personal care and dressing to wheelchair transportation and physical support throughout the service. In hospice care, we start our visits by asking patients and families, “What is important to you today?” For the Carter family, it was clear that honoring Rosalynn, grieving her loss, and celebrating her life was paramount, and hospice care made it possible
EMPOWERS PATIENTS
Perhaps adding to the stigma surrounding hospice care is the belief that hospice care somehow hastens death. Defying perceptions, President Carter surprised many by living twenty-two months enrolled in care. It was
not that much of a surprise to those of us in the industry; several studies have shown that hospice patients live longer than patients with similar diagnoses who are not on hospice care. Evidence shows that when we switch our focus away from fighting illness and toward what is most important to patients, we see better symptom relief, better pain management, and a profoundly increased peace of mind.
Like most hospice patients in the United States, both President Carter and his late wife, Rosalynn, received in-home care. While some assume that hospice is a place; nearly all hospice patients receive care where they reside, versus in-patient care. Whether it’s a single-family home, a nursing home, or an assisted living facility, hospice services are brought to the patient. For President and Mrs. Carter, hospice enabled them to spend their final days in the home they built 64 years ago. This deeply personal setting allowed them to experience the end of life with dignity and comfort, surrounded by loved ones.
President Carter demonstrated that embracing hospice care is not about giving up, but rather choosing to live fully in the time that remains. By opting for hospice, individuals and their families are provided with comprehensive support that prioritizes comfort, dignity, and personal wishes. President and Mrs. Carter exemplified the courage to face death with grace. May we follow their example, embracing the hope that hospice offers and finding the strength to meet life’s end with bravery and serenity.
