THE MEDICAL MARKET
DIGITAL X-RAY A PAST AND FUTURE MARKET
R
adiology has always been an important tool. Since the invention by Konrad Roentgen, almost no diagnostic tool has made such improvements. Through the years the classical X-ray images have gained more and more resolution, at the same time as the X-ray exposure and dose reduced dramatically. The ensuing images were also no longer static. Computer Tomography enabled a totally new way of looking into the human body. At this point the images were digitized for the first time, enabling a new work-flow. Images can now be shared and sent across the hospital or indeed the planet. But the development did not stop there. Modern fusion imaging like PET CT, a combination of X-ray and Positron Emission Tomography (Gamma rays) ,brought a new twist. Things that were previously invisible, like density and blood flow, are now visible. Evolution in the field of IT has also helped in this area of Medical Imaging immensely. The rising capacity of hard drives enabled the storage of more images for a lower price. The rising CPU and video performance of the workstation side allowed “2011 a total new way of image processing. Now 3D image processing is possible in real time using standard desktop systems. The next evolutionary step was already on the way. More and more companies were driving PACS, a complete Digital Imaging Archive. No matter what medical image, all images are obtained, and can all now be stored centrally, ranging from images documenting surgery, to endoscopic videos. On the diagnostic side the next evolution was already on the way, as 2015 started to be the year of Breast Tomosynthesis. Breast Tomosynthesis is a 3D imaging process for mammography, enabling a more detailed look at breast tissue and therefore increasing diagnostic confidence. Looking at the future of X-ray imaging is not easy, but several new cloud services are guiding the way for a non central image storage, bringing medical images to the fingertips wherever you need them.

THE YEAR 2017 AHEAD KEEPING UP THE PACE OF INNOVATION
T
he challenge is big looking forward. The market is growing towards multi-modality solutions, combining various applications on one single screen, will set the bar for a new way of image processing. The new CCL550i2 is the first clear result of this development. “Dynamic gamma” and “auto text mode” set the industry benchmark for automatic image processing. But also recently released QA Standards require new testing scenarios. This increases the pace for the development of new software tools. This is especially influenced by the new de-centralized image distribution. With the advent of cloud technology, PM Medivisor Cloud is a leader in this direction. But this is just a start! All of these new directions challenge us and our new developments. Since the beginning, we at TOTOKU have always taken the lead and we will continue to keep the pace high, providing innovative solutions at our customers request. FEATURES OF DIAGNOSTIC DISPLAYS Display Port
LATEST VIDEO INTERFACES All diagnostic displays offer the latest Display Port interface and DVI. This enables the connection of almost any videocard. Using DisplayPort true 10Bit Grayscale is possible, also with the color displays. The integrated power supply saves deskspace and reduces the footprint of the device.
DVI
LATEST SENSOR TECHNOLOGY Îť-SENTINEL All diagnostic displays offer the best sensor technolgy. A system of internal sensors and the advanced sensor in the front, ensures perfect stability along the whole lifetime of the display, and a full set of remote features is also included to support remote calibration and conformance checking. Together with PM Medivisor, it is a perfect toolset for system and asset management across the hospital.
ISD-TECHNOLOGY TOTOKU ISD-Technology is one of a kind. No other technology is able to bring such a quality improvement to the medical imaging sector. Core to ISD is the possibilty to drive subpixels of the Grayscale displays independently resulting in an increased resolution by a factor of three. This enables a totally new resolution benchmark. With ISD for the first time a resolution of up to 15 Million pixels is possible. This sets a new standard in softcopy reading.
INTEGRATED LOOK UP TABLE All displays offer an integrated look up table which stores all calibration information. This ensures perfect calibration accuracy. In addition calibration is not stored on the videocard which gives our customers a total independence from bundled video cards. All TOTOKU Displays work perfectly with almost any after market card, so compatibility with our software tools is totally ensured.
NEW FEATURES CCL550i2 DYNAMIC GAMMA PRO - PIXEL BY PIXEL PERFECTION Together with CCL550i2 we are introducing Dynamic Gamma, as this solves an issue from which all color displays suffer. There are basically two ways of displaying images. For monochrome images DICOM is the standard and ensures perfect quality. For color images, a Gamma of 2.2 is the perfect match. In the case of multimodality displays this is starting to become an issue. Changing between two presets is of course an option but if you display color and monochrome images at the same time each preset would be a compromise. One possible solution could be of course defining areas on the screen and to setup gamma for each area, but this requires setup and this “box” model is not very flexible. To overcome this issue TOTOKU have developed Dynamic Gamma. Dynamic Gamma is a pixel based gamma, so for every pixel, the display selects the correct gamma. The advantage is clear and obvious and all this is done completely inside the display. No setup or configuration is needed. It works straight out of the box. The user merely needs to enable it from the on screen menu.
AUTO TEXT MODE High brightness displays are needed for a high diagnostic quality. However this high brightness can bring some disadvantages. It can be troublesome, especially when reporting or using work lists. This is because the user can be momentarily blinded by the large bright, white areas in the field of view. Ultimately this can affect the diagnosis, as the eye needs to recover, before going back to the diagnostic image. For this reason TOTOKU have developed the “auto text” mode. When this mode is enabled, the display will detect the potentially disturbing white areas and automatically dim the brightness in those areas. The remaining areas are not affected, ensuring “eye-friendly” reporting without any effect on the remaining diagnostic area. Like dynamic gamma this also works out of the box and no client software or configuration is needed.

PATHOLOGY DISPLAYS Historically, unlike digital X-ray, digital pathology has had a small market share. From 2013 to 2018 the market size is expected to grow by 100%. Since 2012 market growth has risen from 3% up to almost 15% in 2015. In particular the evolution of scanner and video technology is a game changer here. The wide availability of 4K video systems and the dropping prices for data storage has enabled this technology to be used in small clinics and hospitals, making pathology one of the biggest potential medical markets in the up -coming years.
From the display point of view pathology is a big challenge. The requirements are high, as precision and realistic reproduction of images is crucial. In many cases digital and analogue workflows coexist enabling a direct comparison between the two. Any fault or difference in reproduction is then immediately obvious and will not be accepted. That’s why we put a lot of development into this field and have built the most precise display we have ever made. Together with technologies coming straight from
the video broadcast field we have managed the most realistic colors possible. Covering over 99% of Adobe RGB colors, no bit of information is lost. Together with a new 3D look up table color calibration has never been so accurate. But we did not stop here. Very advanced image processing technologies, like contrast and color enhancer make the most of your images. Boosting quality to a level which outperforms any analogue workflow.

NEW FEATURES JD-C240 COLOR ENHANCER - REAL CUSTOM COLORS Color enhancer brings back the full color control to the user. In the past the only possible adjustment was the modification of red, green and blue gain, color enhancer is much more powerful. You can select any color from RGB or CMYK color models and change saturation, hue and brightness. For each channel you can also adjust the tolerance for this applied change.
Original
Blue channel Adjustment
As you can see this change applies to the sky and not to the remaining picture. Also the white balance is not affected which is a big issue using the conventional way, enabling a much more advanced adjustment compared to the past.
Green channel adjustment
CONTRAST ENHANCER Contrast enhancer is a new technology specially developed for pathology imaging.
Original
Enhanced
Usually this technology just increases contrast and dynamics, which result in a non realistic image reproduction. To avoid this we invented a total new approach. In our case contrast enhancer detects borders and structures and improves those, leaving the rest of the image untouched.

ACCESSORIES CALIBRATION KIT CAL-012A As displays age, luminance and color temperature change. TOTOKU’s Medivisor will keep your displays performing to DICOM standards. Medivisor is included in the calibration kits, along with a photo sensor or “puck” for luminance testing and adjustment.
Features New calibration Software – Medivisor Nx The new automatic display detection analyzes display configuration without any wizards or manual inputs. The automatic sensor detection checks for connected and available calibration sensors and selects the right one automatically. A simple user interface makes it easy for you to calibrate the display with just two clicks, no setup or settings necessary. Also one of the new key features is the new tray icon. It shows real-time status of the displays in the Windows task tray. This ensures diagnostic confidence and constant image quality without touching the display. Calibration to conform to DICOM GSDF It goes without saying that maintaining grayscale accuracy is of significant importance for monochrome medical image displays. Periodic calibration by Medivisor Grayscale and/or Medivisor Color provides accuracy control that conforms to the Grayscale Standard Display Function (GSDF) according to DICOM Part 14.
Supported display models Medivisor Nx All i2 models, CCL196,CCL210, CCL242, CCL240, ME195 Medivisor Grayscale ME351i, ME251i, ME315L plus, ME511L Medivisor Color CCL350i, CCL250i, CCL316, CCL212 /r, CCL182 /r Medivisor for ME201L ME181L ME201L /r, ME181L /r
