WELcomE To THE DEcEmBER ISSUE oF coSmETIc NEWS. For many aesthetic practitioners changing their mentality from simply being a doctor, nurse or dentist to running and managing a business can be a challenge. While practitioners in the USA are used to being in private practice and viewing their customers as clients rather than patients, in the UK a vast majority of people who move into medical aesthetics do so from the NHS or other areas of medicine where having a strong business acumen is not a part of the job description. Aesthetics is a business and a competitive one at that as such we have dedicated this month’s issue to all things business related, from online marketing, insurance and customer service to finding the right location for your business and choosing the right software (pages 18-35).

The business has been acquired for an undisclosed sum by The Hospital medical Group, based in Bromsgrove, through a new vehicle, RAm1001. The deal has saved 37 jobs at Surgicare. mr Ratten, who is head of recovery at RSm Tenon, said, “We are pleased to have secured the successful sale of Surgicare and certain assets, as it means that a significant number of people will remain in work, and that they will be able to fulfil existing obligations to patients currently receiving treatment.”

In its last reported accounts for the year to December 31 2008, Surgicare posted a turnover of £7m and a pretax loss of £1.3m.

BaCD RE-LauNCHES ITS wEBSITE The British Association of Cosmetic Doctors has re-launched its website: www.cosmeticdoctors.co.uk.

The website provides a valuable source of information for the public who are seeking help and advice on medical cosmetic treatments including Botox®, fillers, lasers, peels and liposuction.

The BAcD was set up in 2001 to advance safe and ethical practice for the benefit of medical practitioners and members of the public. There are now approximately 300 members who are all Gmc registered and meet the stringent committee criteria for membership (proof of medical insurance and cover for cosmetic treatments, qualifications, ongoing training etc). This year the BAcD launched the first Post Graduate/ Diploma in cosmetic medicine specifically for doctors with the University of Leicester medical School. The current chairman is Dr mike comins and the dedicated voluntary committee for the association consists of Dr Samantha Gammell, Dr Harryono Judodihardjo and Dr Simon connolly.

SOlTA mEDICAl ACqUIRES ACNE lASER mAkER ClRS Solta Medical Inc has bought CLRS Technology, a California company that makes a device that treats acne using heat and light. Solta agreed to pay off about $1.1 million in CLRS debt and may make additional payments based on CLRS’ sales and operating income through the end of 2011. Privately held CLRS makes the Claro system, an over-the-counter device that uses light pulses to treat acne. Solta Medical reported $56 million in revenue in the first six months of 2010, and expects about $115 million in revenue for the year.

mELaNIN pRODuCTION CONTROLLED BY mOLECuLaR SwITCH, maY aLLOw TRuE SuNLESS TaNNING A molecular switch that turns off the natural process of skin pigmentation has been discovered and may lead to a novel way of protecting the skin by activating the tanning process without exposure to cancercausing UV radiation.

In a report in the journal Genes and Development, researchers from the massachusetts General Hospital (mGH) cutaneous Biology Research center (cBRc) described how blocking the action of the enzyme PDE-4D3 in the skin of mice led to a significant increase in melanin production. David Fisher, mD, PhD, director of the hospital’s Department of Dermatology told medical News Today that the primary goal of inducing melanin production in human skin would be prevention of skin cancer, since all the common forms are known to be associated with UV exposure, The researchers confirmed role of PDE-4D3 in controlling melanin expression by applying several agents that block PDE production to the skin of the transgenic mice with epidermal melanocytes. After five days of treatment, the animals’ skin had darkened significantly, while treament of control mice with no epidemal melanocytes produced no effect. “Although PDE enzymes degrade cAmP within all cells, different members of this enzyme family are active in different types of cells,” Fisher explains. “We showed that PDE-4D3 is particularly important within melanocytes, and while the enzyme may have a role in other cells, a blocking drug that is applied directly to the skin would probably have limited effects in other tissues.” Additional research is needed to

Voice of the Industry I don’t particularly like the saying ‘no pain no gain’ but in most instances it tends to be true. As someone who works in the aesthetics industry, I fully understand that in many cases the slightly more aggressive treatments are often the most effective and, as such, I don’t mind enduring the discomfort of being prodded with a needle or zapped with a laser, if its going to leave me wrinkle or hair free, however, what I do mind is being told a treatment is painless and non-invasive and then

I have fallen foul of this twice, once about four years ago when laser lipo first came out and was touted as ‘lunchtime lipo’ (didn’t feel so lunchtime when I was covered in bruises and swollen a week later) and once the other day when I went to try out a supposedly non-invasive cellulite treatment. Now this treatment had been promoted to me as being a quick and painless solution to getting rid of cellulite and when I met the nurse treating me she didn’t give me any more reason to be alarmed (describing the treatment as feeling like a deep tissue massage). I have had the cAcI Endermologie treatment for cellulite and tried out the Accent machine and both of those do feel like a deep tissue massage, however, unless you have visited a very sadistic masseur that is not a

In reality it felt like a combination between an aggressive laser treatment and being stabbed all over my legs with tiny little knifes. The shock of how much it hurt meant that I actually had to ask the nurse to stop treatment twice and refused to have the whole treatment done because it just wasn’t worth it. Now I think I have a fairly high pain threshold – I am a woman after all! II normally don’t make any fuss, even when I had a blood gas test in hospital (which by the way the doctor did tell me was going to hurt and I didn’t think was that bad). I have had my veins lasered before and been a pin-cushion on a sclerotherapy training course and I didn’t think anything could be as uncomfortable as that, but I was wrong. Had I been prepared for the fact this was going to hurt like hell I could have either decided that my cellulite wasn’t that bad after all and not have gone ahead with treatment or have gone with the ‘no pain no gain’ philosophy and geared myself up for it.

I am not a paying customer however if I was I feel I would have been misssold that treatment and would not have been happy with parting with any cash. I actually left feeling a bit teary and embarrassed wondering whether it was just me that found it so awful if other people described it as a ‘deep tissue massage’ – that does not a great client experience make.

The moral of the story is clinics and manufacturers need to be careful about what they claim about treatments. I would rather be told ‘this is going to hurt but it will have a dramatic result’ rather than being duped into thinking I am hopping up on the couch for a nice relaxing deep tissue massage only to find I am actually about to experience one of the most painful treatments I have ever had in my life.

Getting the right insurance is essential for any new business. There will be some policies you may choose purely for peace of mind, such as insurance for equipment and stock, whereas others are a statutory requirement, including employer’s liability if you employ staff.

Here is a summary of the different kinds of insurances typically needed by cosmetic clinics: Contents insurance – This covers not just equipment and machinery, but general fixtures and fittings, and furniture.

Public liability covers the award of any damages given to a claimant, or member of the public, because of an accident that caused injury or damage caused by your business. While this element of cover is not required by law, all businesses with contact with the public should have adequate public liability insurance as claims can be very high indeed. Without cover, a large claim could potentially sink the organisation involved.

Cost of any drugs and vaccines stored on the premises Pharmaceuticals are expensive and inconvenient to replace. In addition to the drugs themselves, the cover should also include loss of specialist medical supplies.

Business interruption insurance – cover is provided for lost income of the cosmetic clinic in the event of an insured claim. In many instances it is not the actual claim that puts financial strain on a business but the loss of income whilst the business recovers from the claim.

Employers’ liability – This is compulsory for anyone who is an employer. This cover is designed to protect you against claims made by your employees for any injury they have suffered as a result of their employment with you.

Buildings insurance – If your cosmetic clinic premises are rented, check the lease to ascertain whether the landlord or the tenant is responsible for insuring the structure.

Medical negligence insurance – covers cosmetic practitioners giving advice or providing cosmetic treatments. Professional indemnity insurance responds to claims arising from bodily injury, which may include its more intangible forms such as mental injury, distress or psychological harm. The injury can arise from treatment or advice or from a combination of the two. In addition, cover is provided for financial loss arising from the injury. As most medical professionals give treatments rather than advice or give a mixture of the two, there must be bodily injury in order to trigger a claim.

Directors and officers insurance – Directors and officers of a limited company can be held personally liable for their actions in certain circumstances. You can purchase insurance to provide cover against this liability.

Finally, when selecting your insurance cover consider the quality of service the provider is able to offer. A good provider will not necessarily be the cheapest but should be able to offer you a quality, specialist service and look after your policy from start to finish, including underwriting and dealing with claims. As part of its professional service, it should fully understand your business and specialist equipment/premises as a cosmetic practice, as well as having expert knowledge of your particular needs. choosing a non-specialist broker who does not have this expert knowledge could lead to you not receiving the best advice or therefore the best protection for your practice.

peter O'Neill on why the reprocessing of surgical instruments within a theatre/ clinical unit is a tricky issue for all healthcare practitioners, especially under new decontamination and sterilisation rules

The new European decontamination guidelines EN13795-20/30 are a major problem for us all and will not go away. Research has shown that sterilisation alone will not kill CJD (Creutzfeldt-Jakob disease) and the only way to ensure your instruments are free from CJD is for all used instruments to go through a washer decontaminator machine which meets the EN13795-20/30 standards and is connected to a reverse osmosis pump.

peter O'Neill has worked within the healthcare industry for more than 20 years, primarily managing large operating theatre departments and advising hospitals in both the NHS and private sector on various aspects of decontamination and infection control. As well as designing and commissioning three new hospital operating theatre divisions, including the new state of the art 400 bedded PFI hospital in kent.

Peter sits on the European commission governing hospital decontamination and was instrumental in the design of the new European standards on decontamination, EN1397520/30. These new guidelines will affect all healthcare providers especially the private sector, as the guild lines make it impossible for clinics to reprocess their instruments unless they have very advanced decontamination systems on site which meet the EN1379520/30 standards.

he main problem for NHS and private healthcare providers is ensuring all re-usable instruments are protein free after cleaning and before sterilisation. Basically this problem has all come about because of the CJD virus. The European standard EN1379520/30 was brought about as the standard decontamination washing machine used in CSSD units in all NHS hospitals and clinics would not kill CJD. This meant new machines have to be put in place to meet the new European guidelines.

It is now illegal to re-process your surgical instruments within your practice unless you have the new decontamination washer machines which meet the EN13795-20/30 guidelines. These machines are very expensive and also require a reverse osmosis pump, which is also very expensive, so in a nutshell another very expensive European guideline enforced to us in the Uk health sector.

There are three solutions to your unit’s decontamination problem: (1) Purchase this very expensive piece of equipment (2) Send all your instruments off site to an accredited hospital or

a private super centre and hope all your instruments and sets come back!!

(3) Use single-use instruments, which removes all legal responsibilities regarding the new decontamination guidelines from you back to the manufacturer.

Now it is impossible for clinics to send their instruments and sets to nearby hospitals to wash and sterilise unless that hospital carries the accredited certificate that meets the EN137520/30 standard, and there are not many hospitals around that have this. complying with the rules requires practices exclusively to use single-use instruments, or send instruments to a fully accredited sterile services department, or a combination of both. The Healthcare commission will police whether standards are met.

Anyone who reprocesses instruments in a unit that is not accredited for another person to use outside their hospital will be transferring the same legal responsibilities for the device to themselves and is FULLY RESPoNSIBLE for its safety and effectiveness, not the manufacturer who has produced and labelled the item Anyone who reprocesses or reuses a reprocessed single use instrument may be committing an offense or contravening the national

guidance on single use devices.

The consumer protection act 1987 will hold a person liable if a single use item is re-used against the manufacturer’s recommendations. (mHRA DB:2006(04)oct 2006 Device bulletin Single –use medical Devices: Implications and consequences of Reuse). Anyone re-using or reprocessing a single use device is FULLY RESPoNSIBLE. When using disposable medical products, we need to be aware of our carbon footprint. When you look at the cost of disposing the single use device compared to re-processing the re-usable device, this is pretty much balanced. Taking into consideration the energy used for the washers and autoclaves and transportation, then when looking at single-use instruments the cost of incineration of clinical waste, it is kind of equal.

You must aim to make your practice more environmentally friendly and the disposal of medical instruments is a particularly challenging area. The choice of whether to sterilise surgical equipment or use disposable surgical kits has environmental and cost implications from both angles. But all instruments used must comply with the medical Devices Directive.

Vistabel® (botulinum toxin type A) Abbreviated Prescribing Information Presentation: Botulinum toxin type A (from clostridium botulinum), 50 Allergan Units/vial.

Temporary improvement in the appearance of moderate to severe vertical lines between the eyebrows seen at frown, in adults <65 years, when the severity of these lines has an important psychological impact for the patient. Dosage and Administration: See Summary of Product Characteristics for full information. Do not inject into blood vessels. Doses of botulinum toxin are not interchangeable between products. Not recommended for patients <18 or >65 years.

Use for one patient treatment only during a single session. Reconstitute vial with 1.25ml of 0.9% preservative free sodium chloride for injection (4U/0.1ml). The recommended injection volume per muscle site is 0.1ml (4U). Five injection sites: 2 in each corrugator muscle and 1 in the procerus muscle: Total dose 20U. Contra-indications: Known hypersensitivity to any constituent. Myasthenia gravis, Eaton Lambert syndrome. Infection at proposed injection sites. Pregnancy or lactation. Warnings/Precautions: Use for one patient treatment only during a single session. Relevant anatomy and changes due to prior surgical procedures must be understood prior to administration. Product contains less than 1mmol sodium (23mg) per dose. Do not exceed recommended dosages and frequency of administration. Epinephrine (adrenaline) or any other anti-anaphylactic measures should be available. Very rare reports of adverse reactions possibly related to spread of toxin distant from site of injection. Therapeutic doses may cause exaggerated muscle weakness. Caution in patients with history of dysphagia and aspiration. Patients or caregivers should seek immediate medical care if swallowing, speech or respiratory disorders arise. Too frequent or excessive dosing can result in risk of antibody formation, which may lead to treatment failure. Caution in the presence of inflammation at the proposed injection site(s) or when excessive muscle weakness or atrophy is present. Caution when used in patients with amyotrophic lateral sclerosis or with peripheral neuromuscular disorders. Effects of administering different botulinum toxin stereotypes simultaneously, or within several months of each other, is unknown and may cause exacerbation of

excessive neuromuscular weakness. Interactions: Theoretically, the effect may be potentiated by aminoglycoside antibiotics or other drugs that interfere with neuromuscular transmission.

Adverse Effects: See Summary of Product Characteristics for full information on side effects. Based on controlled clinical trial data, the proportion of patients that would be expected to experience an adverse reaction after treatment is 23.5% (placebo: 19.2%). These adverse reactions may be related to treatment, injection technique or both. In general, reactions occur within the first few days following injection and are transient and of mild to moderate severity. Pain/burning/stinging, oedema and/or bruising may be associated with the injection.

Uncommon: Infection. Psychiatric disorders. Uncommon: Anxiety. Nervous system disorders. Common: Headache. Uncommon: Paresthesia, dizziness. Eye disorders.

Common: Eyelid ptosis. Uncommon: Blepharitis, eye pain, visual disturbance. Gastrointestinal disorders. Uncommon: Nausea, oral dryness. Skin and subcutaneous tissue disorders.

Common: Erythema, Uncommon: Skin tightness, oedema (face, eyelid, periorbital), photosensitivity reaction, pruritus, dry skin. Musculoskeletal and connective tissue disorders. Common: Localised muscle weakness, Uncommon: Muscle twitching. General disorders and administration site conditions. Common: Face pain, Uncommon: Flu syndrome, asthenia, fever. The following have been reported rarely for glabellar lines and other indications: Rash, urticaria, pruritus, erythema multiforme, psoriasiform eruption, anaphylactic reaction (angiodema, bronchospasm), alopecia, madarosis, tinnitus and hypoacousia. Adverse reactions possibly related to spread of toxin distant from injection site have been reported very rarely (muscle weakness, dysphagia, or aspiration pneumonia which can be fatal).
