US FEDERAL COURT FINDS IN FAVOR OF ALLERGAN AND ISSUES INJUNCTION AGAINST MERZ Allergan Inc, the maker of Botox®, has won a court order blocking Merz Pharma GmbH from selling its rival cosmetic product, Xeomin®, in the US for up to 10 months, after Allergan claimed its trade secrets were stolen.

The trial related to allegations that a former Allergan sales representatives had copied confidential company data and released it to Merz prior to changing over their employment. At a trial in California, US District Judge Andrew J Guilford, ruled that Merz Pharmaceuticals and Merz Aesthetics violated California’s Uniform Trade Secrets Act and issued an injunction against them. In a written decision, Judge Guilford concluded that Merz misappropriated important trade secrets belonging to Allergan, including “the specific identities and financial details (including sales targets, actual sales amounts, and product volumes over time) of Allergan’s relationships with virtually all of its physician customers in the United States for BOTOX® Cosmetic (onabotulinumtoxinA) and JUVÉDERM® and a large segment of Allergan’s physician customers in the United States for BOTOX® (for therapeutic indications).”The court also found that Merz misappropriated “Allergan’s strategic marketing plans, including its plans to address competition from Merz Aesthetics and Merz Therapeutics. The value of this information is incalculable.”

The court concluded that the injunction, which prohibits Merz from, among other things, selling or soliciting purchases of their product, Xeomin®, in the facial aesthetics market for 10 months, was necessary to serve the public interest. As the court noted, “the right of free competition does not include the right to use the confidential work product of others.” It is not a blanket ban and Merz can still sell Xeomin® to practitioners who request it (unsolicited sales). The injunction may also be lifted earlier subject to certain conditions to be performed by Merz.

David Pyott, Allergan’s Chairman of the Board, president and Chief Executive Officer, said, “We believe that full and fair competition is healthy. It is important to ensure that stand 71 physicians and consumers receive accurate and truthful information and are able to make informed decisions about the products they choose to use. As the leader and innovator in the medical aesthetics category, we remain committed to supporting and growing this market on behalf of physicians and the customers they serve.”

Merz CEO Dr Martin Zügel commented, “We deeply regret that this ruling will affect primarily physicians, patients and our customers in the US. Every physician in the US should be able to offer the best available alternatives for treatment at all times“ The ruling relates only to the United States. Both Xeomin® and Bocouture® continue to be marketed as normal in the UK. In addition to the US ruling, Allergan also prevailed in two separate cases that were filed against Merz in Europe. In Germany, the Hamburg Regional Court ruled that Merz is prohibited from claiming a 1:1 dose equivalent ratio between units of Allergan’s BOTOX® and Merz’s Xeomin®. In Spain, Merz was found to be in breach of the Spanish Pharmaceutical Code for referring to a conversion ratio without, at the same time, making an express warning about the fact that the unit doses are not interchangeable. On a separate matter Merz recently won a PMCPA (Prescription Medicines Code of Practice Authority) ruling here in the UK that found Allergan had breached previous undertakings not to disparage Bocouture®/Xeomin® over unbalanced claims on potency.

ASPS releases 2011 stats The plastic surgery market continues to show significant growth, according to statistics released last month by the American Society of Plastic Surgeons (ASPS). stand 40

rise of 5%. “While the rate of economic recovery in the US is still uncertain, 2011 proved to be a good year for plastic surgery,” said ASPS President Malcolm Z. Roth MD. “Consumer confidence was up, auto sales rose 10%, so it is not surprising that we would also see increased demand for plastic surgery procedures.”

Cosmetic surgical procedures increased 2%, with nearly 1.6 million procedures in 2011. The top five surgical procedures were breast augmentation (307,000 procedures, up 4%); nose reshaping (244,000 procedures, down 3%); liposuction (205,000 procedures, up 1%); eyelid surgery (196,000 procedures, down 6%) and face-lifts (119,000 procedures, up 5%).

Face-lifts are among the top five cosmetic surgical procedures for the first time since 2004, replacing tummy tucks. Interestingly, a procedure popular with men, chin augmentation, nearly doubled in 2011, with 21,000 procedures performed, up 71%. Lip augmentation also rose significantly - up 49%, with more than 25,000 procedures performed.

“We are seeing notable increases in surgical procedures, such as facelifts, that reflect the demands of an ageing boomer population,” said Dr Roth. “However, the overall growth in cosmetic procedures is being primarily driven by a substantial rise in minimally-invasive procedures.”

Cosmetic minimally-invasive procedures increased 6%, with nearly 12.2 million procedures in 2011. The top five minimally-

invasive procedures were botulinum toxin type A (5.7 million procedures, up 5%); soft tissue fillers (1.9 million procedures, up 7%); chemical peels (1.1 million procedures, down 3%); laser hair removal (1.1 million procedures, up 15%) and microdermabrasion (900,000 procedures, up 9%).

Helen Bird reports from the BACD’s Spring Conference at London’s Royal Society of Medicine, where a major change was announced.

Led by its recently elected president, Dr Samantha Gammell, the British Association of Cosmetic Doctors (BACD) met for its Spring 2012 Conference on March 17 at London’s Royal Society of Medicine (RSM).

There was a lot to cram in to the one-day event, including a varied and informative programme of presentations and an especially important BACD members’ meeting, all of which was interspersed with exhibits and networking opportunities.

Conference content After a short welcome address from Dr Gammell, the morning sessions got underway, chaired by Dr Harryono Judodihardjo. First up was a talk on the benefits of combination treatments, in particular re-volumisation with radiofrequency for facial rejuvenation, by Dr John Quinn. Mr Rajiv Grover followed with a compelling presentation on the anatomy of the cheek, tear trough and lid/cheek junction, taking inspiration from Dr Rita Rakus when he advised, “The key to treating the tear trough is to think outside the tear trough.”

Dr Chris Rowland Payne delved into the psychological considerations of cosmetic treatment, giving some interesting tips on spotting signs of psychological issues in potential patients! An eye-opening talk brought the morning session to a close, as Dr Lucy Glancey bravely discussed three cases of major complications following injectables, giving delegates plenty to talk about during the coffee break.

BACD meetings as it also offered the chance to see live practical demonstrations. These included treatment of the tear trough by Emervel with a cannula (sponsored by Q-Med), a topic that was discussed during the second conference session by Dr Xavier Goodarzian. Dr Goodarzian recommended treating the tear trough in combination with the cheek, since quite often, older patients will see an improvement in the appearance of the tear trough after treatment of the cheek alone, while younger patients benefit simply from a very small amount of filler to the tear trough. Some astounding before and after shots appeared to confirm this and had many audience members gasping in amazement!

A change of topic was brought by Dr David Eccleston, who gave a talk on the use of botulinum toxin to treat migraine and bruxism, focusing firstly on the symptoms of the two conditions, and then the specific points of injection to treat them. While the potential benefits to chronic migraine sufferers are clear, Dr Eccleston stressed that the treatment should only be administered by qualified medical professionals.

To round off the conference sessions, some pragmatic advice was given by Eddie Hooker of Hamilton Fraser on how to avoid insurance claims. Giving a breakdown of the types of claims seen by the company, Mr Hooker said that 21% were related to fat reduction procedures, 20% to laser, 18% to hyaluronic acid dermal fillers, 14% to other treatments such as chemical peels, and 13% related to other dermal fillers. He advised that keeping records in the form of case notes and before/after photos is crucial, pointing out that “if it isn’t written down, it never happened”, and warned against ignoring any complaints or claims.

Exhibits and workshops The end of a diverse and thought-provoking programme marked the start of the lunch break, which allowed delegates to catch up over a bite to eat and browse the exhibition. Exhibitors included Merz Aesthetics, Juvederm, Allergan, Syneron and Health Xchange, which was launching its INTRAcel machine the following day. The lunch break also saw three practical demonstrations take place in the RSM’s seminar suite, which were open to delegates subject to application. Alongside the tear trough treatment workshop by Emervel, a Health Xchange-sponsored demonstration showed the Obagi Blue Peel Radiance treatment, while Dr Raj Acquilla gave a mini master class in ‘3D facial volume replacement, enhancements and beautification with reference to anatomy technique and risk management’ in a workshop sponsored by Allergan.

A new chapter Since the BACD is a close-knit organisation that represents and protects the interests of its members, an update of its activities and future plans is central to these biannual conferences, so the final portion of the day’s proceedings was devoted to a committee update and the all-important members’ meeting and AGM. On this occasion, the meeting was particularly significant as it marked a major change for the BACD and a new direction for the organisation.

A resolution was passed at the meeting that the BACD will now trade under the name the ‘British CollegeofAestheticMedicine’(BCAM).DrSamantha Gammell explained that this decision reflects the start of an exciting new chapter for the association, at a time when it faces a number of challenges but also many opportunities.

The challenges that were addressed during this important meeting included the wide-ranging review of the Lord Howe into the PiP implant situation, the Care Quality Commission registration issues and the VAT question. Other items were raised during the preceding committee update, such as the proposed aesthetic surgery services standard, which has been drafted and is currently open for public comment via the BSI website (www.bsigroup.com). Members were encouraged to provide constructive comments on any clauses they feel to be unclear or omit important information.

Dr Philip Dobson and Dr Paul Myers took to the stage to update members about revalidation and appraisal, respectively. Dr Dobson explained that since aesthetic medicine is an expanding speciality, there is an increasing number of practitioners that are not part of a Designated Body. The BCAM, he added, will be ideally placed to act as a Designated Body to its members when it receives this status, which will allow appraisal and revalidation to take place internally. Dr Myers reinforced the point that both appraisal and revalidation are essential in order to continue practising medicine, while he also urged members to complete the BCAM’s clinical audit, which is now available online (www.cosmeticdoctors.co.uk).

Looking forward While the meeting involved a lot for members to take on board, the day ended on a positive note. The committee was proud to award Dr Mike Comins a Fellowship of the BCAM. Dr Comins was a founding member of the BACD and has been on the committee for 10 years, as president for the past three.

With a new chapter ahead, the BCAM concluded that two of its key requirements in addressing current and future challenges is to be the unquestioned authority in the expanding field of aesthetic medicine, in turn gaining recognition from other organisations and from the public, and to set standards for the practice of aesthetic medicine in terms of ethics and clinical governance.

More than 300 aesthetic practitioners gathered at the Royal College of Physicians in London last month for the ninth annual Q-Med practitioner meeting. The event has become renounced for its high quality educational programme and this year also served as the UK launch for Q-Med’s two new lip products – Restylane Lip Volume and Restylane Lip Refresh.

The meeting was chaired, as always by consultant plastic surgeon and BAAPS president elect, Mr Rajiv Grover, who oversaw proceedings and offered interesting insights on each topic. Dr Toni Phillips introduced the new Restylane for lips products, comparing the need for the different products to the different regional requirements of patients, focussing on how Restylane gives practitioners the tools they need to really tailor their lip treatments.

on the lip anatomy and how you must look beyond the lip to see how it ages and Dr Tom van Eijk gave an insight into his own techniques, including the Lip Tenting technique.

Dermatologist Dr Russell Emerson then led a demonstration session with three models each of which represented a challenging lip case. He used a combination of the two new products to achieve the results that the models required.

Techniques included using both the needle and cannula, leading nicely into the next session, a debate with Dr Stefanie Williams and Dr Hugo Kitchen focusing on the merits of each treatment tool, and their place within the toolkit of modern practitioners. This session also included practical advice

Cosmetic News: Congratulations on your new role at the Association for Facial Aesthetics (AFA). What will be your title and your key areas of responsibility?

Martin Mackenzie: Thank you very much; my title is Chief Executive, CODE-AFA, the part of CODE that specifically serves members who offer facial aesthetic treatments.

CN: Can you tell us how the new board will be structured?

MM: I am still finalising a structure that I feel will create a newly-

The new edition contains news about how the AFA is changing as well as a market overview of key industry suppliers, details of new innovations showcased at the recent IMCAS Conference in Paris and, finally, I talk about a very useful strategic management tool called a ‘PESTEL’ Analysis and explain how it relates to our industry and how it can help your business too.

CN: What are your key objectives for the AFA in your first year in office?

MM: Over the next few weeks I am taking stock of where the association is today to decide how to shape it for the future.

I’ll be focussing first on updating the AFA website – www.the-face. co.uk - and modules and it’s important to keep these updated. Both of these are valuable sources of reference information as well as practical advice for members but they do need a bit of a spring-clean.

Another key area for me is to provide new information that is useful to members such as; industry news, details of product and treatment innovations, interviews with key industry figures as well as reports

CN: How can AFA members get involved in the new look organisation?

MM:To ensure the AFA continues to be relevant to its members and to support the very people we exist to serve, I invite members to contact me. Is there anything you would like to see us do better or differently or perhaps not at all? My own clinic recently used this new CODE Assure system to successfully register with the CQC and it made the whole process not only much easier for us, it also made it make sense!

After graduating from The University of Aberdeen with a Master’s degree in Accountancy in 1992, I spent many years working in the corporate world with well-known travel companies.

There will be more news, more frequently and more practical business and operations advice on running your facial aesthetics business whether you are a dental or medical-based practice.

In 2005, after 13 years of corporate-life, and in my mid-thirties, I ‘took the plunge’ and followed a desire to set up my own business - a facial aesthetics practice along with medical doctor and GP, Dr Xavier Goodarzian. Our clinic, Dr Xavier G Medi-Spa in Southampton has continued to grow over the last seven years and has won a number of awards for customer service. We’ve steadily grown our treatment portfolio and now offer a wide range of facial aesthetics as well as weight-loss treatments and lifestyle therapies.

CN: What do you see as the key industry issues that will affect the AFA moving forward in 2012 and how will you address them?

MM: As the recent ‘PESTEL’ Analysis has demonstrated, available in full for members to view on the AFA website, there are a number of industry-related issues that will affect us or have the potential to affect us in the near future or beyond. Examples of these include; the current Government review of the cosmetic industry following the PIP breast implant scandal, the role of the EU in possible regulation as well as issues relating to CQC compliance and the effects of the struggling economy meaning that businesses – including clinics - having to work harder and smarter to stand still or grow.

We also run a training company - Innomed Training Ltd – established in 2006, that trains doctors, dentists and nurses in facial aesthetics treatments. Innomed Training was voted the best training provider in national industry-awards and, with a focus on independent hands-on training, it continues to achieve high satisfaction scores as demonstrated by a survey of over 450 doctors, dentists and nurses involved in facial aesthetics.

Unlike most associations in our industry, the AFA welcomes practitioners from all three professions as well as practice owners to become members and it offers valuable and practical services to all of these. With my experience and background therefore, I do feel well-placed to speak-up for, serve and support this broad range of industry professionals.

CN: What are your plans for the website? How do you hope to build on this in the coming months?

MM: The website is already rather good but once it is more up-to-date, I’ll be working more on search engine optimisation and of course embracing the host of social media and interactive

opportunities that exist today. Advertising opportunities on the site are also in my sights in the coming months to generate more revenue for the AFA and enable us to grow further.

CN: Do you have any other key messages for AFA members?

MM: I want the AFA to remain relevant to members and become an increasingly valuable resource in helping them to manage and grow their aesthetic businesses. I would also like to see the AFA being a forum for members to share tips, ideas and best practice too.

I believe we are the only association that is not restricted to one profession, and I feel “we are all in it together” so are stronger together. Also, no other association offers so much practical clinic management advice, clinical governance structure and useful information as the AFA and its CODE-affiliated activities.

012 is a particularly patriotic year for us Brits. Not only is London hosting the 2012 Olympics but it is also the Queen’s Diamond Jubilee. But we are not always so patriotic when it comes to what we buy. Polaris Medical Ltd is a UK based manufacturer of laser systems for hair removal, tattoo removal and vascular treatments. From its factory in Bedfordshire, Polaris started making OEM Ruby lasers in the late 90s, initially for tattoo removal, then for hair removal. Ruby lasers for hair removal were quickly made obsolete by the advent of Alexandrite and then Yag systems so today Polaris continues to make Q-Switched Rubies for tattoo removal alongside their Long Pulsed Nd:Yag for hair removal and vascular treatments.

Whether we are animal lovers or not, it is a sad fact of life that animal testing, particularly when it comes to drugs and cosmetics, is still rife.

No one likes to think of animals suffering in science labs, yet the majority of us reap the benefits of such testing, particularly when it comes to drugs and medicines, and, although many people have a conscience about it when it is brought to their attention, the issue is largely ignored and brushed under the carpet because, for many, the pros of animal testing outweigh the cons.

Animal rights activists have long been campaigning for the manufacturers of drugs, like botulinum toxin, to reduce the amount of animal testing they do, especially if there are non-animal alternatives.

One manufacturer that is stepping up to the mark on this issue is Allergan, which announced last month that it had received two positive opinions regarding its fully in vitro, cell-based assay for use in the stability and potency testing of BOTOX® and VISTABEL® (Allergan’s botulinum toxin type A products).

Upon approval, the new assay will be implemented as quickly as possible for the release of product for sale in the relevant countries. Allergan estimates that use of the new assay will reduce the use of animal-based assay testing for BOTOX® and VISTABEL® by 95% or more over the next three years,

as other regulatory agencies around the world approve it. The result of years of research and development commitment, this new assay is the first to be developed and approved for any botulinum toxin currently available worldwide and is specific to Allergan’s botulinum toxin type A products, BOTOX® and VISTABEL®. This approval does not extend to botulinum toxins made by other manufacturers, who will continue to use their animal-based release assays.

The first positive opinion, from Agence Française de Sécurité Sanitaire des Produits de Santé (AFSSAPS), relates to VISTABEL® and paves the way for approval in 29 countries in the European Union. The second positive opinion was granted by the Irish Medicines Board (IMB) for BOTOX® and covers 14 European Union countries involved in the Mutual Recognition Process. The Medicines and Healthcare

products Regulatory Agency (MHRA) has already approved the assay for BOTOX® vials sold in the UK. Michelle Thew, Chief Executive of the British Union for the Abolition of Vivisection (BUAV) said, “We are pleased that Allergan has achieved these significant milestones with the European regulators for the first non-animal based assay for the potency and stability testing of BOTOX® and VISTABEL®. This represents a significant scientific breakthrough in the manufacturing of neurotoxins and is the right and necessary step forward in eliminating the use of animals for release of the product.

Allergan’s cell-based assay now sets the standard for all other botulinum toxin manufacturers and demonstrates that with commitment and perseverance, it is possible to find validated, robust alternative methods of testing medicines while maintaining the

highest level of patient safety. We now call on other companies manufacturing botulinum toxin products, including Ipsen and Merz, to follow Allergan’s ethical lead.”

These approvals follow more than a decade of Allergan research and development and an investment of approximately $65 million directed toward minimising to the greatest degree possible the need for animal-based assays in the manufacture of BOTOX® and VISTABEL®. Until Allergan’s cell-based assay, the mouse LD50 potency assay has been the standard for potency and stability testing for all botulinum toxin products and was required by regulatory agencies around the world for testing final product for release to assure its safety and efficacy. Limitations associated with the mouse LD50 assay include the necessary involvement of animals due to the current structure of the assay, the complexity of the procedures involved, lack of specificity to botulinum toxin type A, as well as variability between assays and laboratories.

Along with the relevant European regulatory agencies, we are committed to securing national approvals and releasing the product using the new assay as rapidly as possible.”

BOTOX® and VISTABEL® are the same product, manufactured in exactly the same way, and therefore the potency and stability of each batch must be determined before release, regardless of whether the batch is intended for medical or aesthetic use. BOTOX® and VISTABEL® are Allergan’s only products that have required use of an animal-based assay for commercial release, as mandated by global regulatory authorities to ensure safe product use in humans.

Currently all pharmaceutical manufacturers are required by the European Medicines Agency (EMA), US Food and Drug Administration (FDA) and by other worldwide health regulatory agencies to protect patients and consumers by assuring product safety and efficacy through animal testing and other methods.

“I applaud Allergan’s long-term dedication to putting research and patient safety before profit in developing a ground-breaking new method for testing BOTOX® and VISTABEL®” said Mr Rajiv Grover, consultant plastic surgeon at the King Edward VII Hospital in London and president elect of the British Association of Aesthetic Plastic Surgeons. “I know that my patients will welcome this initiative, and that they will continue to feel confident about the ethos and efficacy of these products. The time has definitely come for people interested in medical aesthetics to select very carefully what is used to treat them.”

About the BOTOX® and VISTABEL® Cell-Based Potency Assay (CBPA) Over the years, a number of attempts have been made to find ways to replace the animal-based LD50 assay for testing of botulinum neurotoxins but have fallen short for a variety of reasons. Botulinum neurotoxin is one of the most potent naturally occurring substances and has a complex multi-stage mechanism of action. Purified botulinum neurotoxin type A is the active ingredient in BOTOX®/ VISTABEL® and blocks the release of neurotransmitters from nerves, resulting in a localised and temporary reduction in the overacting muscle or gland. The amount of neurotoxin in BOTOX®/VISTABEL® is extremely small; in fact, less than one gram per year is required to supply the entire world.

A suitable replacement potency assay to the LD50 assay must therefore be sensitive enough to measure the complex mode of action of extremely small amount of the neurotoxin used in BOTOX®/VISTABEL® at the cellular level. Established performance criteria against which the new cell-based potency assay needed to be validated included the ability to assess all primary modes of

The new assay does not impact the manufacturing process for BOTOX® or VISTABEL® as the product remains exactly the same. The units will not change either.

It’s important to recognise that the unit doses for botulinum toxins are not interchangeable. The new cell-based potency assay for BOTOX® and VISTABEL® is specific to Allergan’s formulations of botulinum neurotoxin type A. Allergan is discussing how to license the technology to other parties that share its commitment to implementing non-animal alternatives to animal-based assays in the manufacture of their medical products.

We summarise the results of a clinical study on the treatment of striae distensae using fractional ablative radio-frequency and transdermal delivery using ultrasound Authors: Dong-Hye Suh, Sang-jun Lee, Ji-Ho Rhue, Minkyung Shin, Kye-yong Song Background Various lasers have recently been reported as effective treatment modalities for striae distensae. Recently, fractional ablative radio-frequency(RF) and transdermal delivery using ultrasound have been introduced for the purpose of for the improvement of stretch marks.

Objective To evaluate the clinical and ultrastructural changes associated with fractional ablative radio-frequency and transdermal delivery using ultrasound on striae distensae.

Methods A total of 18 subjects with striae distensae were treated with the Legato system (Alma Lasers, Israel), comprising of ablative RF and ultrasound, every two weeks for a total of four sessions. Initially, RF energy perforates the skin with ablative zones of about 100-150µm in width, and 80-120µm in depth, per pass. Thereafter, platelet rich plasma (PRP) was applied. In order to enhance PRP penetration, ultrasound is applied. Clinical photographs were taken before and after each treatment. Subjective satisfaction level was evaluated to prove the efficacy. Biopsies were obtained from 10 subjects, analysed by light microscopy.

Discussion The therapeutic options for striae distensae are numerous and include topical tretinoin, microdermabraision, IPL and lasers. Fractional lasers are also commonly used to treat striae distensa, but more than five treatment sessions are required for good results.

Repetitive treatment can lead to a high rate of hyperpigmentation. The Legato system is a transdermal drug delivery device using fractional RF and ultrasound. We find out about the Ultracontour system for treating localised fat

Body contouring is big business with more and more patients seeking treatments to target localised areas of fat. As the aesthetic market has evolved, the scope for fat reduction treatments has widened and moved away from more traditional surgical procedures, such as liposuction, towards minimally invasive procedures with less downtime. In fact, a recent report by the Millennium Research Group (US Markets for Body Contouring Devices 2012 report) predicted that the demand for fat reduction using non-invasive laser, light and energy-based devices would grown even more dramatically over the next five years.

One treatment that has been making waves is the Ultracontour™ device from Medixsysteme.

Ultracontour™ is a non-invasive dual-ultrasound device, which utilises high focalised and UMD transducers to apply energy below the skin’s surface in the subcutaneous tissue to treat fat cells and stimulate lymphatic drainage to increase lymph and blood circulation. Patient group: Three female patients aged between 45 and 57 were selected for the study. Two patients were overweight and one was obese. The patients underwent a Lunar Prodigy Advance analysis in the independent laboratory of Vienna before Ultracontour sessions and immediately after the last session of Ultracontour to ascertain their fat composition in belly, hips and legs, but also their global fat mass BMI. The patients were also measured by tape during the whole protocol and were requested to conduct their normal life style and activity.

Three sessions of Ultracontour were performed on the patients, using the high focalised ultrasound and UMD drainage modes in the same session.

Conclusion: After three sessions of Ultracontour protocol, the two overweight patients showed a global reduction of BMI as well as reduction of android and gynoid fat. The obese patient showed a reduction in android and gynoid fat. The digital body analysis confirmed the previous sonography results of measurable fat tissue reduction in using Ultracontour for localised subcutaneous adiposity in belly, legs and hips.

Traditionally speaking natural and organic products and complementary therapies at large have been scorned for their lack of scientific evidence. The impractical application and often poor quality of natural products has also been a hindrance to the industry. This however is changing rapidly with an increasingly sophisticated array of products and treatments available.

Let’s take for example the subject of skin needling/microneedling. From a Cosmetic industry point of view this is an entirely new discipline. It is commonly associated with aggressive bleeding of the skin, topical retinoids, long recovery times and also a risk of infection.

Skin needling has been performed safely for hundreds and even thousands of years in Asia for cosmetic purposes. Tools such as the seven star needle were used cosmetically for scarring and beauty treatments. This was complemented by traditional techniques such as the jade roller and Gua Sha providing an element of luxury to the treatments. Modern micro-needling can be perfectly adapted into the traditional theories and principles of acupuncture. This provides a fantastic holistic therapy with excellent scientific evidence of effectiveness and high patient compliance.

By combining the ancient principles of acupuncture with modern micro-needling it is possible to treat the skin for scarring. Stretch marks, cellulite, hair loss and anti-ageing using entirely natural techniques. As a cosmetic acupuncturist, it is well known in the industry that using the smallest needles possible achieves the best results. Interestingly research supports the fact that collagen is only inducted up to 0.50.6mm deep even when longer needles are used.

Using holistic micro-needling techniques it is not necessary to bleed the skin, in fact better results are achieved without it. Patient compliance is also higher and there is a huge reduction in the potential side-effects that are created by the use of topical retinoids, which typically result in extremely dry and irritated skin. There is no scientific proof that vitamin A (topical retinoids) enhance the treatments. One of the major studies of micro-needling demonstrated that you could induce up to a 1,000% increase in collagen without the application of any retinoids.

Due to the increase in transdermal absorption that occurs with microneedling, holistically it is best to use ingredients that have been tested over a very long period of time and are known to be safe when taken internally.

Eastern Asian herbs have the additional advantage of also having been combined with acupuncture for hundreds of years, demonstrating their safety and effectiveness. The application of these ancient techniques to modern micro-needling increases a clinicians understanding of skin needling. This allows them to perform far more painless skin needling when taught correctly.

It is also fantastic to integrate into modern micro-needling techniques that compliment skin needling and that were used alongside cosmetic acupuncture in China. These are principally the use of acupressure, Jade Rollers, Facial Gua Sha and topical herbal serums.

In modern times it is all too often the case that society jumps at the latest trend without pausing to adopt and integrate knowledge from our ancestors that have stood the test of time. Holistic micro-needling not only delivers in terms of cosmetic results, it is also much less traumatic, has no down time, high patient compliance and is a relaxing experience.

From the growing number of therapists that are adopting this approach and reporting better results and marketability from the treatments, scepticism about adopting acupuncture techniques into micro-needling will be overtaken by the experience of testing it out and seeing the difference by therapists and their clients. Along with the growing trend for day spas and organic natural anti-ageing treatments, the public is indicating what type of treatments they really want and in such turbulent times, clinicians will undoubtedly follow.

Looking at the rapidly developing and competitive market for laser hair removal, it is difficult for practitioners to know what is best. ‘Painless’ hair removal is the latest thing, but it is impossible to call any hair removal device totally ‘painless’ as pain is subjective and varies between patients. What the new generation of lasers offer is a much more painless option to traditional high peak power laser hair removal.

Fluency and energy delivery

James has an extremely detailed understanding of the energies used in aesthetic devices and the interactions with tissue and skin. He also has a talent for bringing science to life so we can understand some of the basic concepts.

In traditional hair removal the energy is delivered in j/cm2 per pulse so the energy is related to spot size, which affects the penetration depth. With painless hair removal, also known as SHR (Soprano) or Motif (eLase), modes, for example, the energy is delivered in a grid, 10cm x 10cm and the amount of energy delivered in that area is set to heat the hair follicles with a more gentle accumulative heat rather than a single pass. The differences come in how fast we can fill that square with energy. This method is bulk heating and relies on heating the hair root more than the tissue surrounding the follicle.

Pulse Duration Pulse duration relates to the thermal relaxation time of the target (hair) and, in general, larger complex targets need longer pulse durations. For hair removal a millisecond pulse duration range is used. In general in painless hair removal it is a 20-30m second pulse, delivered very quickly, but the handpiece is in constant motion, allowing hairs to gain heat, cool a little, and then get reheated time after time. Treatment time will depend on the system, skin type and total energy required.

Radio-frequency and laser technology

The eLase system adds radio-frequency to the energy mix to speed up the bulk heating process. This is nothing new – the elos™ technology from Syneron Candela is a successful method of achieving this, with years in the market and various peer reviewed studies.

Although RF is non-selective it has two advantages. It non-selectively heats tissue and quickly and effectively gives accumulated heat. RF is more predictable than light with 100% flowing into the tissue. The RF energy can also be used to monitor the impedance (resistance) in skin and allow us to give good feedback about the delivery. RF can be measured in j/cm3 so typically, with Motif, 50j\cm3 is delivered equivalent to 40j/cm2/second. This is on top of the light energy delivered.

“Making hair removal painless is simply just delivering the energy in a different way...this is possible with most light-based devices” Wavelength The wavelength of the painless systems on the market is 810nm Diode or IPL. Other wavelengths are used for traditional laser hair removal, such as the very effective Alexandrite 755nm, or on darker skin types a longer wavelength such as NdYag 1064nm is used. Diode at 810nm has great safety and efficacy across the board on all skin colours. Making hair removal painless is simply just delivering the energy in a different way and this is possible to do with most light based devices if they have the power. Is it as effective? The answer is ‘yes’’, most people are happy with painless hair removal. It is not as fast as traditional delivery, but it gives a huge advantage to the clinic as a marketing tool and a news story. Some clinics choose to have more than one type of laser wavelength and it is increasingly common to treat with different lasers at different stages during the course of hair removal.

Education The best advice is to attend workshops and learn the science as opposed to being taken in by aggressive marketing strategies. Look in detail at what you are buying; consider the speed of the laser, the ROI and the company you choose as a partner. Newer technologies often have new features, and can be faster and better, but of course they are new to the market.

ABC Lasers has introduced a new high powered Pixel Q-Switched laser to the UK. The fractional non-ablative 1064nm laser is used exclusively with the HarmonyXL platform. Already a hit with dermatologists and laser surgeons in the US, Pixel Q Switched has been featured on the hit TV shows Good Day LA and Dr Oz. The high-powered Pixel Q-Switched 5x5 mm pixel tip is used for encouraging growth of new collagen fibres and non-ablative skin rejuvenation and is part of the Laser360iQ protocol for complete skin rejuvenation. Studies show STYLAGE ® has ‘Je Ne Sais Quoi’ The STYLAGE ® portfolio of products has been shown to have excellent tolerance in the skin in a clinical study. The study undertaken by 10 specialist dermatologists, aesthetic doctors, and plastic surgeons monitored 96 patients treated with STYLAGE ® over a one-year period. Aim: To present the latest findings and future developments in the cosmetic use of botulinum neurotoxin.

Methods: Review of recent literature and new scientific developments.

Results: Botulinum neurotoxin type A preparations onabotulinumtoxinA (BOTOX® Cosmetic/Vistabel®, Allergan Inc.) and abobotulinumtoxinA (Dysport®/Azzalure®/Reloxin®, Ipsen Pharma) have been used for many years and are effective and well tolerated for facial aesthetic procedures. However, advances are continually made in the aesthetics field. New formulations that may exhibit reduced antigenicity are becoming available, such as incobotulinumtoxinA (Xeomin®/Xeomeen® /Bocouture®, Merz Pharma), which is a botulinum neurotoxin type A free from complexing proteins. In addition, lower facial procedures using botulinum toxin combined with fillers are becoming increasingly popular. Injection techniques and patterns are also evolving, with the aim of creating a more natural result and avoiding a ‘frozen’ appearance. Moreover, the diversity of individuals requesting aesthetic procedures is increasing, with growing interest from men and patients with a variety of skin types and colors.

Conclusions: The uses of botulinum toxins for facial aesthetics procedures continue to expand, with new techniques and formulations. The availability of products such as incobotulinumtoxinA may reduce the risk of neutralising antibody development while maintaining the good efficacy and safety of existing formulations.

Abstract Botulinum toxin type A is a 150 kD protein produced

by Clostridium botulinum, which exists in a complex with up to six additional proteins. The ability of botulinum toxin to inhibit acetylcholine release at the neuromuscular junction has been exploited for use in medical conditions characterised by muscle hyperactivity. As such, botulinum toxin is widely recommended by international treatment guidelines for movement disorders and it has a plethora of other clinical and cosmetic indications. The chronic nature of these conditions requires repeated injections of botulinum toxin, usually every few months. Multiple injections can lead to secondary treatment failure in some patients that may be associated with the production of neutralising antibodies directed specifically against the neurotoxin. However, the presence of such antibodies does not always render patients non-responsive. The reported prevalence

of immunoresistance varies greatly, depending on factors such as study design and treated indication.

This review presents what is currently known about the immunogenicity of botulinum toxin and how this impacts upon patient non-response to treatment.

The complexing proteins may act as adjuvants and stimulate the immune response. Their role and that of neutralising and non-neutralising antibodies in the response to botulinum toxin is discussed, together with an assessment of current neutralising antibody measurement techniques. Botulinum toxin preparations with different compositions and excipients have been developed. The major commercially available preparations of botulinum toxin are Botox® (onabotulinumtoxinA; Allergan, Inc., Ireland), Dysport® (abobotulinumtoxinA; Ipsen Ltd, UK), and Xeomin® (incobotulinumtoxinA; botulinum toxin type A [150 kD], free from complexing proteins; NT 201; Merz Pharmaceuticals GmbH, Germany). The new preparations of botulinum toxin aim to minimise the risk of immunoresistance in patients being treated for chronic clinical conditions.

Methods: Fat tissue was harvested from the pre-urinary bladder cavity of four Sprague-Dawley rats and processed using the Coleman technique.

The experiment was performed on the backs of eight BALB/c-nu mice. The injection of free fat grafts was performed on the bilateral side of the back of each mouse. The one side (experimental) was treated with 0.5 ml of a free fat injection combined with 0.5 IU of BoNTA in 0.1 ml of saline. The other side (control) was treated with 0.5 ml of free fat injection combined with 0.1 ml of saline. The mice were killed after 9 weeks, and the injected fat grafts were explanted, after which the weight and volume were measured.

Histologic study was performed with hematoxylin and eosin staining. Statistical analysis of the weight and volume from both sides, the histologic parameters, and cellular integrity was performed.

Conclusion: A difference in the weight, volume, and histologic parameters of the injected fat grafts was observed. The BoNTA-treated side exhibited a significantly higher survival rate than the control side. The histologic examination of the fat grafts also demonstrated that the grade scale of cellular integrity was higher for the BoNTA-treated sides.

Botulinum toxin A significantly reduces the level of fat graft resorption. Therefore, an injected fat graft can be used in conjunction with botulinum toxin A and offers better volumetric improvement.

Abstract A variety of non-invasive techniques have been utilised for the enhancement of cutaneous changes seen with photo-ageing. Such methods include chemical peels, microdermabrasion, ablative and non-ablative lasers, and various rejuvenating light sources. However, the most widely used minimally invasive cosmetic procedures for the correction of undesired rhytides and enhance facial features through contouring and volumisation are injections with botulinum toxin and soft tissue fillers. Their extensive long-term safety and relative ease of procedural techniques have led to high satisfaction levels worldwide. Nonetheless, proper training of the fundamentals in injection technique, the choice of the appropriate candidate, and knowledge of potential adverse events are imperative to ensure an excellent and safe outcome.

Goal setting Richard Crawford Small on the key to success when goal setting

At RCS Towers Friday is a sacred day, as it is the beginning of the week. Yep, you heard me; we start our working week with two days off so hands up if you want to work for RCS. On this most sacred of days we begin with a practise that has served me well for the last 10 years and I can say will transform your life and business if used well. Indeed clever people, you have guessed it – goal setting.

We use goal setting routinely to help in all of our planning phases, as goals are simply objectives; things I will achieve for the customer, for myself, for my colleagues and for my family.

So what is goal setting?

A true professional in the medical aesthetics industry, Richard has enjoyed a career that has seen service in The Royal Navy to Medical Device Sales and Corporate Account Management with Allergan. Richard started RCSConsulting in January 2012 and has successfully use his “Think-Plan-Do” methodology in driving greater success for clients including Allergan, Schuco International, The Hospital Group, Transform and Harley Aesthetics. Richard is also an accomplished speaker and presents regularly at industry conferences and meetings.

A quick Google search and the business world will quickly tell you that the best useable method for goal setting is SMART; • Specific • Measureable • Achievable • Realistic • Timebound This is a worthy method, but it doesn’t work for me because it sets too many limitations. We dream big as children, and nothing is impossible. So why as adults do we believe that ‘realistic and achievable’ are the limits of our capabilities?

The corporate world should simply say what it means, “In corporations we like you to set goals, but don’t be too lofty, we don’t want you to get ideas above your station”.

Call me an eternal optimist and unrealistic dreamer, but I saw through this and very soon afterwards discovered the definition of goal setting still used by us at RCS; “A committed decision to reach a predetermined specific goal, combined with a burning desire, followed by immediate, massive action repeated consistently for as long as it takes until your goal is reached“.

The author of this quote is one of the most successful people in his generation, an athlete, an actor, a businessman, a politician, and multi-

millionaire. I’ll tell you who it is at the end, but let’s look again at the key phrases used in this definition; • Committed Decision • Specific Goal • Burning Desire • Immediate MASSIVE Action • Repeated Consistently • For as long as it takes They are powerful words when you think about it, and actually quite obvious as every day we decide to do something, we set an objective, we act and repeat until it is done.

So if this is the case, why do we fail? Why do we procrastinate? Why do some people achieve great success and others do not? Why are there so many self-help books?

All men are NOT created equal, and the ability to set, and achieve goals is what separates us.

Why is goal setting so effective?

Goal setting focuses the subconscious mind on the task in hand and enables us to apply our considerable resourcefulness to achieving it. The old adage of winners never quit and quitters never win underpins our philosophy of success.

What is the difference between success and failure?

A Committed Decision Do you CHOOSE to fail because you DECIDE TO STOP trying to succeed? I believe this is the case, it took me four years to get RCS Consulting off the ground, and it began on this scrap of paper in January 2008 after an impromtu goal setting session in a car park in Crystal Palace.

and if your enthusiasm to achieve it needs to be mustered then review its importance. Which would you rather have, infectious enthusiasm or a polite shrug?

What’s the point of working hard to learn how to dance if you hate dancing? You’ll quit and feel like you have failed, when all you did was choose the wrong goal. I want to climb Everest, but I don’t have a burning desire to do it – my friend Tarka L’Herpiniere did, and he has now summited three times! In fact he should probably be writing this.

Take IMMEDIATE and MASSIVE ACTION I love this bit, Immediate and Massive action, not tomorrow but NOW. Medical aesthetics is packed full of business owner operators and its great. The simple fact that you are reading this means that the streak of entrepreneurship runs deep in you and you know how to get things done. But sometimes the burst of enthusiasm does wane and we need a little kick. I just think “I must move this rock now and it must move a long way”; often this is in the “Think and Plan” stage of our planning process and gets us a good head start.

I had had enough of the life of a sales representative and was listening to an audiobook of “Tony Robbins – Awaken the Giant Within”; I remember driving Mrs RCS crazy with my constant regurgitation of “a decision is not a decision unless you act”, which she felt was pretty obvious and other highly clichéd soundbites.

But that’s the real point isn’t it? It doesn’t matter if it is a cliché or not, or you know you should, unless you COMMIT to the decision and act they’re still only words. Maybe handwriting lessons would have helped, but cut a guy some slack - I was writing on the steering wheel. It wasn’t moving.

A Specific Goal So let’s assume that you have made a Committed Decision to your first specific goal, which should actually be to goal set, grab a nice paper and pen, and write down the following headings; • Career • Family • Adventure • Contribution Now write down 10 SPECIFIC GOALS under each heading, and let your mind soar, in this life we are constrained only by ourselves so really go for it. I write down Orbit the Earth and Climb Everest under adventure every goal setting day.

Next thing is to RANK YOUR GOALS in order of importance to you on a scale of 1 to 10. In the example from 2008 I wrote that an MBA was less important to me than the respect of the industry and my customers, I still don’t have an MBA because it is no longer relevant, but respect always is.

Burning Desire Everyone has to do things that bore the pants off them, while I cast my mind back to the last instalment of the Twilight Series and immediately my motivation to do anything other than scream is lost. However, your new goal MUST be worth the effort, this MUST become a magnificent obsession

Repeated Consistently Now, this is the real challenge isn’t? Going to the Gym, dieting, learning a language or instrument this is the phase of goal setting that you WILL fall down on, because we are creatures of habit, and our default habits is to make excuses to not do things.

If you’ve been not doing things for most of your life it’s going to be a tough habit to break! Apparently it takes 30 days for habit forming activities to embed themselves and probably five minutes to break them, and often we take on too much in one go. Change your life and habits one at a time, and train yourself to succeed and achieve your goals.

Remember the final part - for as long as it takes. It took me four years to achieve the goals I wrote in Crystal Palace, and as Tarks will tell you, climbing Everest isn’t something you can just decide to do on a Tuesday afternoon and squeeze in after the gym, (well you can, you’ll just die a slow painful death, which is kind of the ultimate failure).

So, who is the genius who set your humble RCS on his way to independence, and becomes the strange focus on a Friday morning?

Albert Einstein? Vince Lombardi? Richard Branson?

Nope.

Arnold Schwarzenegger, unbelievably.

Arnie arrived in the USA from Austria and simply wrote his goals for the year on index cards. He looked at it every day, and made a conscious effort to achieve them. By the time he was 30 he was a Millionaire, long before his Hollywood movie days.

So take your time, focus on what you want to achieve, be sure you want it and enjoy the ride.

If Arnold can do it, why can’t you?

You may also want to add the word ‘mobile’ to your description and title and create a specific mobile sitemap as this will help Google recognise that the pages are specifically made for a mobile browser.

Ensure it is quick Put yourself in your target audience’s shoes and think about what their needs are why are they using your mobile site? What do they want? Chances are they want immediate results/ information, so strip out any unnecessary frills that will cause the page to load so slowly that the user will lose interest and go elsewhere. After all, most people are using a 3G connection when using their mobile out and about and so data speeds are much, much slower than using Wi-Fi. Make it easy to navigate Consider the screen sizes that will be used when people view your mobile site and ensure that the layout is simple and easy to use without complex menu structures or vital information that requires lots of scrolling and zooming to get to.

Provide only essential information If large bodies of information are necessary, think of how to split this out into more navigation points or pages. Break up the content so that consumers can click to the part of the data that is important to them at that moment.

Make it easy to use Make it clear for the user what action function or button has been selected. Highlight buttons in a way that is easy to see by use of a different colour or a clear box outline of the button.

Text entry on mobile devices is harder than on a desktop and must be thoughtfully laid out for mobiles to enable users to fill out information easily.

Take advantage of mobile tricks Mobile users expect to be able to call a business at the touch of a button or make an enquiry through their preferred email client. Take advantage of mobile technology by ensuring phone numbers are coded in such a way that allow one click calling. Forms can often be awkward to use, so enable visitors to enquire via email, again with a single click.

stand 27 BOTOX® (botulinum toxin type A) Abbreviated Prescribing Information Presentation: Botulinum toxin type A (from clostridium botulinum), 50 or 100 or 200 Allergan Units/vial. Indications: Temporary improvement in the appearance of moderate to severe vertical lines between the eyebrows seen at frown (glabellar lines), in adults <65 years, when the severity of these lines has an important psychological impact for the patient. Dosage and Administration: See Summary of Product Characteristics for full information. Do not inject into blood vessels. Doses of botulinum toxin are not interchangeable between products. Not recommended for patients <18 or >65 years. Use for one patient treatment only during a single session. Reconstitute vial with 1.25ml of 0.9% preservative free sodium chloride for injection (4U/0.1ml). The recommended injection volume per muscle site is 0.1ml (4U). Five injection sites: 2 in each corrugator muscle and 1 in the procerus muscle: total dose 20U. Contraindications: Known hypersensitivity to any constituent. Infection at proposed injection site(s). Warnings/Precautions: Relevant anatomy and changes due to prior surgical procedures must be understood prior to administration. Do not exceed recommended dosages and frequency of administration. Adrenaline and other anti-anaphylactic measures should be available. Reports of side effects related to spread of toxin distant from injection site, sometimes resulting in death. Therapeutic doses may cause exaggerated muscle weakness.

Caution in patients with underlying neurological disorder and history of dysphagia and aspiration.

Patients should seek medical help if swallowing, speech or respiratory disorders arise. Clinical fluctuations may occur during repeated use. Too frequent or excessive dosing can lead to antibody formation and treatment resistance. The previously sedentary patient should resume activities gradually. Caution in the presence of inflammation at injection site(s) or when excessive weakness/ atrophy is present in target muscle. Caution when used for treatment of patients with peripheral motor neuropathic disease. Use with extreme caution and close supervision in patients with defective neuromuscular transmission (myasthenia gravis, Eaton Lambert Syndrome). Contains human serum albumin. Procedure related injury could occur. Interactions No interaction studies have been performed. No interactions of clinical significance have been reported. Theoretically, the effect may be potentiated by aminoglycoside antibiotics or other drugs that interfere with neuromuscular transmission. Effects of administering different botulinum toxin stereotypes simultaneously, or within several months of each other, is unknown and may cause exacerbation of excessive neuromuscular weakness. Pregnancy: BOTOX® should not be used during preganancy unless clearly necessary.

Based on controlled clinical trial data, the proportion of patients that would be expected to experience an adverse reaction after treatment is 23.5% (placebo: 19.2%). In general, reactions occur within the first few days following injection and are transient. Pain/burning/stinging, oedema and/or bruising may be associated with the

Gastrointestinal disorders. Uncommon: Nausea, oral dryness. Skin and subcutaneous tissue disorders. Common: Erythema, Uncommon: Skin tightness, oedema (face, eyelid, periorbital), photosensitivity reaction, pruritus, dry skin. Musculoskeletal and connective tissue disorders.

Common: Localised muscle weakness, Uncommon: Muscle twitching. General disorders and administration site conditions. Common: Face pain, Uncommon: Flu syndrome, asthenia, fever.

The following other adverse events have been reported since the drug has been marketed: dysarthria; abdominal pain; vision blurred; pyrexia; focal facial paralysis; hypoaesthesia; malaise; myalgia; pruritus; hyperhidrosis; diarrhoea; anorexia; hypoacusis; tinnitus; radiculopathy; syncope; myasthenia gravis; erythema multiforme; dermatitis psoriasiform; vomiting and brachial plexopathy; rash; psoriasiform eruption; anaphylactic reaction (angiodema, bronchospasm); alopecia and madarosis.

Adverse reactions possibly related to spread of toxin distant from injection site have been reported very rarely (exaggerated muscle weakness, dysphagia, or aspiration pneumonia which can be fatal).
