Tippecanoe Valley sports physical clinic

July 13-14

Woodlawn Hospital will be hosting a two day sports physical clinic at Tippecanoe Valley Middle School. This is an easy way for students to be eligible for summer practices.

The first day runs from 10 a.m. to noon Thursday, July 13. The second day of the clinic runs from 3-5 p.m. Friday, July 14.

To receive a physical you must be a student at Tippecanoe Valley schools.

All students need a legal guardian present. The legal guardian must fill out and sign their child’s Indiana sports physical form along with consent to treat form prior to the physicals.

Students with incomplete documents will not be evaluated.

There is a $5 charge for the physical.
