SCIATICA By: Paul Eberle MSPT, Whiting Senior Physical Therapist

The sciatic nerve is the largest nerve in the body.

It begins in the lower back, travels through the buttock and the back of the thigh, divides into two nerves at the knee and ends at your toes. The term “sciatica” refers to a set of symptoms including pain in the low back, buttock, and legs, which can radiate below the knee to the feet and toes. This radiating pain can be experienced in one or both legs along with numbness, tingling and muscle weakness in the hips, knees and ankles. It is important to understand that the term “sciatica” does not describe the specific causes of the radiating pain, which many individuals from their twenties to their nineties experience. There are two common causes of sciatic pain.

Sciatic pain is most commonly caused by compression of lumbar or sacral nerve roots as they exit the spinal cord.

The most common cause for nerve compression causing sciatica in the younger population is lumbar disc herniation. It has been reported that as high as 90% of all cases of sciatic pain are due to spinal disc herniations which impinge upon the nerve roots where the sciatic nerve originates. The compression or pressing of the nerve roots can cause pain, tingling, muscle weakness, or poor muscle control of the areas supplied by the sciatic nerve. Individuals with lumbar disc herniations, also known as lumbar disc bulges or slipped discs, usually experience increased radiating sciatic pain with forward bending and sitting. Work and daily activities often become limited.

Physical therapy and instruction in patient-specific exercises to reduce discal nerve impingement and to increase spinal flexibility and core strength have consistently been shown to decrease and eliminate sciatic and lumbar pain. The McKenzie approach is a widely used physical therapy regimen. In this approach patients repeatedly perform specific exercises which mechanically reduce the process of disc herniation as discal material (nucleus propulsus) is pressed away from the irritated nerve roots and back toward or into the outer casing of the disc (annulus fibrosus). Treatment then focuses on strengthening postural muscles of the spine and trunk to limit the possibility of symptom recurrence.

The second most common cause of sciatic pain is lumbar spinal stenosis. Most people with lumbar spinal stenosis are over the age of fifty. It is estimated that more than 400,000 people in the United States have leg and/or lower back pain which is attributed to lumbar stenosis. The term stenosis means “choking” and is the result of the spinal cord or nerve roots being compressed within the spinal column. Stenosis is often the result of joint and disc degeneration. Sciatica from lumbar spinal stenosis usually progresses slowly and often becomes more severe with time. The low back pain and radiating buttock or leg pain will often subside for periods but, unfortunately, will often return. In most cases people with lumbar stenosis have pain with standing, walking and transitioning from a sitting to a standing position. This is due to the fact that when the lumbar spine is extended or straight, the nerve roots have less room as they exit the spinal cord and can be compressed. Patients with lumbar stenosis usually

have relief when sitting and often prefer to stand with a slouched posture which reduces nerve compression and pain.

The exercise prescription for patients with lumbar spinal stenosis focuses on increasing flexibility in the spine and legs and maximizing patient walking and daily activity tolerance without worsening pain. Patients are often not able to tolerate exercise in the standing position due to the irritation of tight muscles; therefore focus is initially on loosening up those muscles to decrease symptoms. Once symptom reduction begins, the patient can usually tolerate exercises to strengthen the lower extremities, spine and trunk in positions, notably standing, where they had experienced pain during the day. The goal is to have the patient be able to perform exercises and stretches which allow them to increase their tolerance to walking and standing activities.

The third most common cause of sciatica is piriformis syndrome. In the past it was believed to cause approximately 6% of reported cases of sciatica; however, recent EMG studies have shown that piriformis syndrome may be a more common source of sciatica.

The sciatic nerve runs deep through the buttocks just beneath the piriformis muscle.

In 10 to 15% of individuals, however, it is believed the nerve runs directly through the muscle. These individuals may be at an increased risk of piriformis syndrome. Patients with piriformis syndrome usually feel worse in seated positions, as increased pressure is placed on the sciatic nerve. This is often exacerbated in individuals who sit on their wallets. Common causes are overexercising and prolonged sitting and it appears to be more common in women.

Buttock and sciatic pain resulting from piriformis syndrome respond well to physical therapy when properly diagnosed and treated. Physical therapy treatment begins with a thorough evaluation of the hip joint and spine, followed by specific manual therapy techniques performed by an experienced therapist. The goal of physical therapy treatment is to allow the piriformis muscle to relax and lengthen, relieving the pressure on the sciatic nerve. Modalities including moist heat and ultrasound often assist in allowing the piriformis muscle to relax. Once pain levels are reduced, specific patient exercises and stretches can be taught, allowing patients to control and eliminate their sciatica.

Trusts for the Charitably Minded

You may want to respond in a substantial way to a worthy cause seeking your financial support, but are concerned about how a major gift will affect you and your family in the long run. Deciding upon a beneficiary for your charitable contributions is only the first step. You may be able to do more than you realize.

It’s all a matter of planning, and charitable trusts are tools that can help.

Charitable Remainder Trusts

With a charitable remainder trust, you retain the right to receive income from the trust for life, or for a period of not more than 20 years. Or, you can make another family member the income beneficiary. You give the charity that you have designated the right to receive the assets remaining in the trust once the income payments have been completed. The trust will be either a charitable remainder unitrust or a charitable remainder annuity trust.

The unitrust pays the income beneficiary a fixed percentage of its assets, determined each year. If the trust grows in value, the income payments go up. The annuity trust, in contrast, pays a fixed dollar amount each year regardless of the trust’s investment performance.

Charitable remainder annuity trusts provide greater certainty than unitrusts, but do not protect against inflation. The donor knows exactly how much that the trust will pay each year, unlike unitrust payments that fluctuate

with market conditions. Still, in the long run most of the fluctuations should be upward, creating a growing income flow that helps the donor keep abreast of the rising cost of living.

Tax Advantages of Charitable Remainder Trusts

Because you relinquish the right to change your mind when you set up your trust, you qualify for significant income tax benefits.

Charitable Lead Trusts

In some cases, charitably inclined donors can achieve greater gift-tax economy by creating what you might think of as “family remainder trusts.” Usually, they are called charitable lead trusts.

Generally, the deductible value of a charitable remainder interest ranges from 20% to 50% of the value of the assets that are placed in trust. When you create a charitable lead trust, you are charged with making a taxable gift of only the family “remainder.” Consequently, you use up significantly less of your exemption from federal gift and estate tax. Or, if your previous family gifts already have exceeded your exemption amount, you pay significantly lower tax on the transfer.

The idea behind “planned giving” is that you should look at the ways in which charitable intentions can be linked with steps that create greater financial security for yourself or members of your family.

2. Freedom from capital gains tax. Charitable remainder trusts are often funded with assets, such as securities or real estate, that have grown substantially in value over the years. By donating appreciated property, you may avoid capital gains tax that you would have paid had you sold and reinvested the proceeds yourself.

Frank Discussions About Housing Decisions

Do any of these scenarios sound familiar? Your spouse has had a recent medical incident that has impacted his or her ability to live independently. You have come home to visit your mother for the holidays and realize your mother's home is in disarray, her clothing soiled, the refrigerator nearly empty, and her bills are unpaid. A neighbor has called to tell you your father seems confused and has been getting lost.

Any of these scenario give rise to concern and may trigger the need to consider a safer living environment. The next questions usually include: what do we do? Where will we/our parents go? Do we/our parents try to stay at home with additional assistance? Do we/our parents move in with one of the children? Do we/our parents move into an assisted living facility or a nursing home?

A major life change such as a change in one's living environment deserves careful and thoughtful consideration. The following are some tips that may make the process a little easier.

1. Open Discussion. An open, candid and honest discussion should take place. A family meeting with all interested parties will help determine everyone's views and expectations and will also help to decide the best course of action. This is a stressful time for all concerned but an open discussion can help to alleviate that stress.

2. Plan. A plan must be put into place. A decision needs to be made as to where the new home or living environment will be located. All possible residential options need to be evaluated; such as assistance

in the current living situation, or a home closer to family members, or moving in with a family member, or moving into a care facility whether it is a continuing care community, a nursing home or an assisted living facility.

3. Level of Care. The level of care required will often dictate the residential option selected. Consider the level of independence and what activities of daily living, such as bathing and eating, can be done independently. Evaluate the level of supervision or assistance that is needed. Assess the health and physical abilities of the caregiver if that caregiver is to be a family member. Determine the level of medical care needed.

4. Financial Considerations. Needless to say, the economics of affordability is always a consideration. How much money is available to pay for the needed care or available to pay for the new living arrangement must be a factor when making a decision.

Of course, each family's circumstances are unique. However, it is hoped that with open discussions and careful consideration the process of making housing decisions will go more smoothly. As a certified elder law attorney, I have often been called upon to help families through this process.

Varicose Veins: Not Just a Cosmetic Problem

ne of the most common circulatory system problems affecting individuals of all ages is venous insufficiency, also known as “Venous Reflux Disease”.

This occurs when one-way valves in the leg veins begin to fail and become leaky. This causes blood to pool in the legs and can lead to the development of large protruding legs veins, known as varicose veins. It can also cause a variety of painful or uncomfortable symptoms in the affected legs. Some of the most common of these symptoms are leg pain, leg fatigue and heaviness, swelling and skin itching. his article serves as a general overview regarding the Pre Trial Intervention (PTI) program. PTI is a supervisory treatment program that was enacted prior to the New Jersey Criminal Code Title 2C being codified. It can be found in NJSA 2C:43-12. The goal is “it is policy of the State of New Jersey that supervisory treatment should ordinarily be limited to persons who have not previously been convicted of any criminal offense under the laws of New Jersey, or under any criminal law of the United States.” The law has gone through many modifications over the years and is constantly changing.

When a person is charged with a crime, or in some instances an offense (domestic violence disorderly persons offense), that person may be eligible for supervisory treatment in PTI depending on a panoply of variables. These variables include the PTI Director (court personnel) and their willingness to accept the person into the program, the prosecuting agency and their willingness to accept the person into the program, the charge itself, a victim’s input if applicable, and many other considerations. If a person has received a prior PTI, a conditional discharge or a conditional dismissal, he or she is ineligible for this type of supervisory treatment. NJSA 2C:43-12(g)(1). Ordinarily, when you are accepted into the PTI program, you DO NOT have to plead guilty to the charge(s) subject to certain exceptions found in NJSA 2C:43-12(g)(3). The accused should consult a qualified criminal defense attorney immediately and make a timely application so as not to delay or prejudice their rights. The fee for the application is $75 and almost always no statement that is made to court personnel can be disclosed to the prosecuting agency nor can it be used in a subsequent civil or criminal proceeding. The maximum term limit that a person can be supervised in PTI is thirty-six (36) months. If a person does well, and upon approval of the program director, the prosecuting agency and its consent, and the court’s approval, the term may be shortened or even terminated early. If a person violates the terms and conditions of PTI, dismissal from the program can occur. Again, hire a qualified criminal defense attorney to motion to the court for remaining in the program. If the term is completed successfully, the charge(s) are dismissed.

However, the charge and/or arrest will still remain on a person’s record and

they have to wait six (6) months in order to expunge it. NJSA 2C:52-6(c).

Additionally, if a person is rejected from the program, a PTI appeal can be filed arguing that there has been a “patent and gross abuse of discretion” in rejecting the individual.

An example would help illustrate how the process works. On January 1, Mr. X gets arrested at a local department store for shoplifting $650 worth of merchandise. He ultimately gets caught on surveillance video and the local police arrest him and charge him with third degree shoplifting, a crime with a maximum punishment up to 3-5 years in state prison with various fines and assessments. He is released on a summons (he has no prior record, lives locally, has no other pending charges and was born in the United States). He hires a qualified criminal defense attorney and makes a timely application for the PTI program. The program director agrees to let him in and the prosecuting agency is at first hesitant due to the department store and its concern of recent spikes in shoplifting, but nonetheless acquiesces.

Mr. X comes to court on April 1 on an accusation (waives his right to have this matter presented to a grand jury) and the court admits him into the program for a period of 12 months with conditions, such as making restitution to the department store and keeping away from that department store. Mr. X finishes the program in the requisite time frame and the indictable (felony) charge is dismissed.

Mr. X then waits until October 1 (6 months after) and moves to expunge his arrest, with which he is successful, and thus avoids traditional prosecution and a criminal record. As a famous proverb tell us: “Everyone makes mistakes in life, but that doesn’t mean they have to pay for them the rest of their life.

Sometimes good people make bad choices. It doesn’t mean they’re bad … it means they’re human!”

February is National Children’s Dental Month and we celebrate all winter long!

To teach kids about dental health, we visit over 2,500 students in Ocean County. Our team of pediatric dental health specialists visits with Girl Scouts, Boy Dr. Elisa Scouts, preschools, nursery schools, elementary schools Velazquez and Head Start to give them a fun and educational lesson on caring for their teeth. The teachers and the children enjoy our interactive sessions and the goodie bags with toothbrushes, educational materials and stickers. In February, we also host our annual Give Kids A Smile Day! The first Friday of February each year is a national event and participating offices provide FREE dental care. Each year we strive to treat 100 kids for FREE! Many families with NO dental insurance or with state plans are so appreciative of the opportunity to receive quality FREE dental care! Our efforts to educate children and their families are to help prevent cavities and promote proper oral health. Dental caries are caused by a

breakdown of the tooth enamel. This breakdown is the result of bacteria on teeth that break down foods and produce acid that destroys tooth structure and results in tooth decay.

Although dental caries are largely preventable, they remain the most common chronic disease of children aged 6 to 11 years and adolescents aged 12 to 19 years. Tooth decay is four times more common than asthma according to the CDC – Centers for Disease Control & Prevention.

5 Considerations in Choosing a Special Needs Trustee Written By Christine L. Matus, Esq.

Choosing the right person to serve as trustee of a special needs trust is one of the most important and difficult issues in creating the trust. A trustee typically manages the day-to-day operations of the trust, often making distributions to the trust’s beneficiary, investing the trust’s assets, and paying the trust’s bills – all while maintaining the beneficiary’s eligibility for public benefits programs.

The law isn’t very strict about who may serve as trustee, as long as the person is over 18 years of age and is capable of managing his or her own affairs. A trustee can be the child’s parent or other relative, a trusted friend, or a professional such as a lawyer, accountant, trust company, bank or private professional fiduciary.

Here are five considerations to help in the choice of who should serve.

1. FAMILIARITY WITH PUBLIC BENEFIT PROGRAMS. To ensure that your beneficiary’s eligibility is never compromised, a trustee’s knowledge of public benefit programs is crucial. Many government benefits like Medicaid, Supplemental Security Income (SSI) and Section 8 housing have very complicated and contradictory rules governing special needs trusts.

2. DOES THE TRUSTEE HAVE TIME TO DO THE JOB? Serving as the trustee of an active special needs trust can seem like a full-time job. Depending on a beneficiary’s needs, the trustee could spend a good deal of time paying bills, monitoring government benefits, helping to secure housing, paying for medical care and

serving as a link between the beneficiary and a variety of service providers. If a trustee finds that she can’t perform all of these tasks when needed, or if she is sacrificing her family life or other professional obligations in order to work as a trustee, then it may be time to look for a professional trustee.

3. CONSIDER A PROFESSIONAL TRUSTEE. This could be an attorney, accountant, trust company, investment firm, bank or private professional fiduciary.

A professional allows you to take advantage of that individual’s or institution’s experience with public benefits, investments, money management and tax planning.

4. HOW COMFORTABLE ARE YOU GIVING TRUST CONTROL TO AN OUTSIDER? For those who are uncomfortable with the idea of an outsider

managing a loved one’s trust, it is possible to appoint a family member and an independent trustee as cotrustees. By doing so, you can rest assured that there is a person who is familiar with the beneficiary and has her best interests at heart and that the public benefit programs; requirements are being met. Another option is to simultaneously appoint a trust “protector,” who has the powers to review accounts and to hire and fire trustees, and a trust “advisor,” who instructs the trustee on the beneficiary’s needs.

5. IS A POOLED TRUST AN OPTION? A pooled trust, which is administered by a non-profit corporation, may be a good option for some families.

Such trusts pool the resources of many beneficiaries, and those resources are managed by a non-profit association.

Pooling trust resources can reduce administrative fees, increase the total funds available for investment, and permit access to better investment opportunities.

Because a pooled trust accepts contributions from many beneficiaries, the trust is able to make more stable investments and provide additional management services that a conventional special needs trust might not be able to afford.

The Risks of Indoor Tanning Exposure to UV radiation increases the risk of developing skin cancer, according to the National Cancer Institute. Melanoma, the deadliest form of skin cancer, is linked to sunburns. The risk of melanoma of the skin increased by 75 percent when tanning bed use started before age 35.

Sunlamps and tanning beds promise consumers a beautiful bronzed skin, vitamin D, and good mood year-round, but in reality the ultraviolet (UV) radiation from these devices has serious health consequences.

A tan is the skin’s reaction to exposure to UV rays when skin produces more melanin, a pigment that darkens the skin. Over time, this damage might cause wrinkly skin and permanent brown or red pigmentation. This leathery look may not show up until many years after a tan or sunburn.

Two types of UV radiation, utilized by sunlamps and tanning beds, are UV-B and UV-A rays. UV-B rays penetrate the top layers of skin and are most responsible for sunburns. UV-A rays penetrate to the deeper layers of the skin and are often associated with allergic reactions from UV exposure that present like itchy red bumps. Most importantly, both UV-B and UV-A rays damage the skin and can lead to skin cancer. Exposure to UV radiation can cause irreversible damage to the eyes. Ocular malignant melanoma is deadly.

In July 2009, the International Agency for Research on Cancer (IARC), part of the World Health Organization, concluded that tanning devices that emit UV radiation are more dangerous than previously thought. IARC moved these devices into the highest cancer risk category: “carcinogenic to humans.” Previously, it had categorized the devices as “probably carcinogenic to humans.”

In addition to the serious risk of skin cancer, tanning can also cause immune suppression, impairing proper functioning of the body’s immune system and the skin’s natural defenses, leading to infections and skin cancer.

Your Body – What Matters Most

It is a unique combination of physical and interpersonal skills that make an orthopedic physician good at what he does. So many of the procedures require delicate, precise movements, and a steady hand. If surgery is necessary, it can involve plenty of stressful situations and doctors must be able to adapt to changes as they arise. For over 50 years the staff and doctors at Advocare Stafford Orthopedics have been serving the Southern Ocean County community in a myriad of ways. Known for caring for patients of all ages from infants, to the elderly, and treating countless conditions from diseases of the bone, to diabetic wounds. Orthopedics involves the treatment of injuries and diseases affecting the hands, feet, arms, legs, neck and back. Additionally, a large part of the practice is the treatment of broken bones and sports medicine. From total hip replacements to knee replacements the driving philosophy behind their care has always remained the same: try non-surgical forms of treatment first, and surgery only if other measures fail.

Getting to Know the Physicians Samuel E. Epstein, D.O. OCW: What attracted you to the field of Orthopedic Medicine?

Dr. Epstein: My father was a family physician and every summer he would take me to his office while he treated his patients. I knew at a very young age that I too wanted to become a doctor and help people. In high school and college I wrestled and played football. Needless to say, I had sports injuries that required treatment. Orthopedic medicine was very intriguing to me and I wanted to be able to take care of these musculosketal injuries both operatively and non-operatively.

OCW: What are the most common conditions you treat on a daily basis?

Dr. Epstein: We see a great deal of hip, knee, ankle, foot, shoulder, elbow, and back and neck pain due to injury or arthritis. Conditions such as carpal tunnel and trigger finger are also starting to increase..

OCW: What sets advocare/Staffford Orthopedics apart from other facilities like it?

Dr. Epstein: Our office has been treating patients in Southern Ocean County for over 50 years and we are known for treating the whole patient, considering their individual needs. In most cases, we opt for conservative treatment first. Only after this fails do we recommend surgery. We understand most of our patients are in pain and usually fit them into our schedule within one day, even if it means extended hours. Our office staff is very “user-friendly” and competent.

OCW: Tell about your educational background.

Dr. Epstein: I received my medical degree at the Philadelphia College of Osteopathic Medicine and did my internship and orthopedic residency at Detroit Osteopathic Hospital. I also did some additional training at the University of Pennsylvania in sports medicine and spine surgery.

OCW: Are you a part of any medical associations?

Dr. Epstein: I am a three time former chairman of the department of surgery at Southern Ocean Medical Center, and am a member of the American Osteopathic Association and the American Osteopathic Academy of Orthopedics.

OCW: Can you tell us about some of the more cutting edge options you offer your patients?

Dr. Epstein: So many of our patients are faced with joint, muscle or tendon pain that has not been resolved with medication, physical therapy, or cortisone injections. They may be contemplating surgery to alleviate their pain; or they may have resigned themselves to live with it. However, there are other options. One of those is Platelet Rich Plasma (PRP) injections. Many high profile athletes go to Europe to have these injections done.

Essentially, your blood is made up of plasma (liquid part of the blood) and cells, white blood cells and platelets. The platelets help the blood to clot when you cut yourself. They also contain a high concentration of healing factors, which conveniently allow the cut to heal. To make PRP we draw your blood and spin it down in a centrifuge. When the blood is spun down, it separates into a layer of red blood cells, white blood cells and platelets. We draw off the platelet rich layer of the blood plasma (PRP) as this portion has a much higher concentration of healing factors. The PRP is then injected directly into the damaged tendon, muscle or joint. These PRP injections help the body to heal tendons and muscles damaged by injury or overuse and help to sooth the pain in arthritic joints.

Although these injections are not yet FDA approved, I provide them to my patients. The reason they are not yet approved is because the FDA process of approval is very expensive.

Unlike pharmaceutical drugs, PRP cannot be sold for profit since it’s obtained from your own blood. Therefore, there is no financial incentive for companies to go through the expense of getting FDA approval. Unfortunately, without FDA approval, most insurance will not cover the expense of obtaining the PRP. However, many of our patients consider the expense worth it, especially if it can alleviate their pain or help them to avoid surgery.

OCW: How do you stay current with the latest advances made in your field?

Dr. Epstein: Being a doctor is a life long learning process. I am constantly reading journals, attending conferences, and listening to webinars to stay current. I am also involved in hospital committee’s in which I use “evidence Based Medicine” and “Best Practices” to develop the highest quality treatment protocols for my patients. However, it is critical to remember that there is no substitute for experience and we all learn by doing.

I have been doing Orthopedic Surgery for 27 years and have had the opportunity to really hone my craft.

Eric S. Baskin, DPM FACFAS OCW: What attracted you to the field of Podiatric Medicine?

Dr. Baskin: In college I worked in an army hospital where I shadowed different physicians from all fields of medicine. What really impressed me about podiatry is the fact that patients come into the office with pain and leave pain-free after a few minutes of treatment. There are so few areas of medicine that can claim this.

OCW: What are the most common conditions you treat on a daily basis?

Dr. Baskin: I see a great deal of patients dealing with pain from gait abnormalities to deformities such as flat feet, fractures, and sprains. I also see dermatological diseases, nerve related diseases, and diabetic related conditions of the foot and ankle. In addition, one of my specialties is pediatric abnormalities such as toe walkers. As a podiatrist, I wear many hats including that of plastic surgeon, orthopedist, dermatologist, and neurologist of the foot and ankle.

OCW: What sets your practice apart from others like it?

Dr. Baskin:: It really is all about our commitment to patient satisfaction. We strive to be the “Nordstrom’s” of orthopedics, and foot and ankle related treatment. By providing our patients with the best possible service, quality of care, and highest code of ethics, we make this happen on a daily basis. We realize when people come to our office they are trying to avoid surgery and we strive to keep them out of the operating room. This is accomplished by providing the most comprehensive and conservative treatments in modern medicine.

If those treatments have been exhausted and surgery is required, we utilize the latest technology and techniques. Our primary goal is to get people back to their busy lives as soon as possible.

OCW: How do you stay current with new advances made in your field?

Dr. Baskin: I attend continuing education conferences yearly, as well as train residents and students when they do rotations in our office. I also love when I am asked to lecture at surgical conferences. My most recent lecture was in June where I completed my residency. It was really nice to be back with other alumni..

OCW: What can older women do to prevent diabetic related problems to the foot and ankle?

Dr. Baskin: Good blood sugar control will prevent many of the diabetic related complications associated with the foot and ankle. Control of glucose levels through lifestyle and medical guidance can prevent loss of sensation, peripheral vascular disease, and foot deformities. Appropriate shoe gear and treatment of conditions such as flatfeet, high arched feet, bunions, and hammertoes are also important in preventing complications. I am always happy to educate our patients on various shoe options, conservative and surgical treatments, and foot care to keep them healthy and active.

Judy Kerlin, APN: I see patients with a variety of orthopedic conditions, including but not limited to joint pain from underlying arthritis, as well as those dealing with back and neck pain. Additionally, I treat acute injuries involving bones, muscles, and tendons.

OCW: Are there any success stories that stick out in your mind?

Judy Kerlin, APN: There have been many success stories in the 21 years that I have cared for patients with orthopedic problems. It is always uplifting when the patient follows-up after treatment and they are better.

OCW: What is the most rewarding part about your job?

Judy Kerlin, APN: The most rewarding part of my job is watching patients return to their usual activities and have the ability able to enjoy life again.

OCW: What do you enjoy doing in your time off?

Judy Kerlin, APN: I enjoy spending time with my family.

OCW: How do you stay current with new advances made in your field?

Judy Kerlin, APN: There are different requirements to maintain my license and certification. I obtain continuing medical education online, with medical journals, and at seminars.

Thyroid Disorders and Surgery What is a thyroid disorder?

Diseases of the thyroid gland are very common, affecting millions of Americans. The most common thyroid problems are: • An overactive gland, called hyperthyroidism (e.g., Graves disease, toxic adenoma or toxic nodular goiter) • An underactive gland, called hypothyroidism (e.g., Hashimotos thyroiditis) • Thyroid enlargement due to overactivity (as in Graves disease) or from under-activity (as in hypothyroidism).

Patients with a family history of thyroid cancer or who had radiation therapy to the head or neck as children for acne, adenoids, or other reasons are more prone to developing thyroid malignancy.

surgical treatment is recommended. Nodules that are suspicious or cancerous on biopsy require surgical removal. Large nodules are often removed for a number of reasons. Benign large thyroid nodules may cause pressure symptoms in the throat or cause difficulty swallowing. Thyroid nodules that produce excess thyroid hormone may also need to be removed. Lastly, some thyroid nodules are large enough that standard diagnostic approaches prevent accurate assessment of the possibility of cancer and, therefore, need to be surgically removed.

Thyroid surgery is an operation to remove part or all of the thyroid gland. It is performed in the hospital and general anesthesia is usually required. Typically, the operation removes the lobe of the thyroid gland containing the suspicious nodule or lump. A frozen section (immediate microscopic reading) may be used to determine if the rest of the thyroid gland should be removed during the same surgery. Based on the result of the frozen section, the decision will be made in the operating room if removal of any remaining thyroid tissue is necessary. Dr. Peters will discuss these options with you preoperatively.

As an alternative, Dr. Peters may choose to remove only one lobe and wait for the final pathology report before deciding if the remaining lobe needs to be removed. If a malignancy is identified in this way, he may recommend that the remaining lobe of the thyroid be removed at a second procedure.

What happens after thyroid surgery?

The surgery usually takes 2-3 hours, and most patients are watched carefully in the hospital overnight. The pain is often minimal, and most patients take four to seven days off from work.

Endometriosis and Infertility WHAT IS ENDOMETRIOSIS?

Endometriosis is a condition where tissue normally located in the uterus is abnormally found elsewhere in the abdomen. It affects women primarily from the late teenage years to the time of menopause. Classically, endometriosis is responsible for much of the chronic pelvic pain symptoms in women of reproductive age, and a common cause of infertility. Although the exact cause is unknown, evidence supports a genetic predisposition to developing endometriosis.

WHAT ARE THE SYMPTOMS?

Women with endometriosis often experience painful periods and pelvic pain varying from a mild annoyance to severe debilitating pain. It is estimated that endometriosis may be present in up to a third of reproductive age women with pelvic pain, and up to forty percent of women presenting with infertility.

HOW IS ENDOMETRIOSIS DIAGNOSED?

A detailed medical history, physical exam, and pelvic ultrasound are common components in the workup of a patient with suspected endometriosis. Medical history may be consistent with painful menstrual periods, including pain with intercourse and pain when having a bowel movement or voiding. Ovarian cysts can be found on ultrasound that may or may not be symptomatic. A definitive diagnosis is based on surgical exploration of the abdomen, with visual evidence of endometriosis.

WHAT CAUSES ENDOMETRIOSIS?

Although the exact cause of endometriosis is unknown, this benign disease is thought to arise from a combination of menstrual fluid that backflows from the uterus into the fallopian tubes and enters the abdomen. Another likely factor is cellular changes that create endometriosis implants in the abdomen, or that an altered immune system response allows these endometriosis cells to successfully survive and grow outside of the uterine cavity.

HOW DOES ENDOMETRIOSIS AFFECT MY CHANCES OF FERTILITY?

Women with endometriosis have a strong association with infertility. It is estimated that between 20 and 40 percent of infertile women have endometriosis, and that the extent of infertility is directly associated with the severity of disease.

WHAT ARE MY TREATMENT OPTIONS?

Treatment of endometriosis depends on the clinical symptoms, which include pelvic pain and/or infertility.

Medical treatment includes the use of anti-inflammatory medications or oral contraceptives in those women with mild symptoms not desiring pregnancy. Medications that decrease the production of estrogen in the body are also being utilized to treat this disease. Although medical therapies address the symptoms, they do not provide a measurable benefit regarding fertility. Therefore, on a case-by-case basis, surgical treatment may be beneficial in restoring ovarian and fallopian tube anatomy, and to remove endometriosis implants. Fertility treatments may often be required to achieve pregnancy.

Endometriosis can affect many facets of a woman’s health. It is important that patients be educated about the implications of this condition, and that proper evaluation and care is implemented by their provider.

Hepatitis C and the Baby Boomer

More than 75% of patients infected with Hepatitis C were born between 1945 and 1965 and are part of the Baby Boomer generation. This has prompted the Centers for Disease Control and Prevention to recommend routine testing for Hepatitis C virus for all Baby Boomers. Hepatitis C is one of the leading causes of cirrhosis, primary liver cancer, and the need for liver transplantation. Unfortunately it often progresses to severe organ damage before any symptoms are present, making it a silent killer.

What is Hepatitis C and how do I get it?

Hepatitis C is a viral infection that is transmitted by blood. The virus reproduces in the cells of the liver, leading to chronic liver inflammation.

Over the course of decades and without many symptoms, this process leads to a scarred and damaged liver. Risk factors for transmission include drug use,

tattoos, and blood transfusions prior to widespread blood screening practices of the 1990s. Patients with HIV are at an increased risk, as well as those in the healthcare profession, especially after needle sticks from an infected patient. Still others have become infected without an identifiable source.

How do I know if I have the infection?

A simple blood test called a Hepatitis C antibody test can detect if you have been exposed to the virus. If this is positive, then further tests are needed to see if active virus is circulating in your blood. Once your infection is confirmed by blood testing, imaging of the liver with an ultrasound and additional blood testing to further understand the extent of the infection will be required.

How is Hepatitis C treated?

In the past the treatment of Hepatitis C often required up to a year of therapy with a drug called interferon combined with oral medications. Unfortunately this treatment was not very successful and the drugs themselves caused a lot of side effects. Currently there have been major advances in the successful treatment of Hepatitis C. The treatment for some types of Hepatitis C can now be as simple as one pill a day for eight weeks, with a cure rate over 90%. The treatment of Hepatitis C is rapidly evolving, with many new drugs and regimens being developed.

How can I take charge of my risk of Hepatitis C?

The most important component to managing Hepatitis C is to know whether or not you’re infected. The first place to ask about this is with your primary care provider, who can easily order the needed lab testing. Once a positive test is found, the next step is to speak with a gastroenterologist and to set a course for a successful treatment. Meet the Doctors of Gastroenterologists of Ocean County Omar Tamimi, MD

Doctor Omar Tamimi joined our practice in 1992. He is Board Certified in Gastroenterology. He is a graduate of Rutgers University and University of St. George’s Medical School. His medical training was completed at Seton Hall University, St. Joseph’s hospital, Patterson, NJ. Since completing his medical training he has performed over 25,000 endoscopic procedures and has vast experience in all aspects of the practice of Gastroenterology. Doctor Tamimi had been a member of the executive committee at Kimball Medical Center since 2003 and served as the Chief of the Medical Staff from 2007-2009. Dr. Tamimi is currently Medical Director of the Endoscopy Center of Ocean County. Doctor Tamimi and his wife Patty have three children with whom they are very involved. Doctor Tamimi enjoys outdoor sports, coaching Toms River Little League, vacationing and spending time with his family and is a big fan of Rutgers sports teams and Bruce Springsteen.

She received extensive training in all aspects of Gastroenterology and has performed over 20,000 procedures.

She has been chosen as one of “America’s Top Physicians” annually since 2007. Doctor Collier is currently Medical Director of the Endoscopy Center of Toms River . q Chicken Breasts With Grilled Pineapple and Tomatillo Salsa Grilled chicken topped with a delicious sweet and spicy combination of savory onions, sweet pineapple and rich tomatillo salsa (Serves 4)

2 Tsp olive oil ½ large sweet onion, cut into thick rings 8 fresh pineapple spears 1 ½ c tomatillo salsa (store bought, or make your own!)

2 Tsp cilantro, fresh, chopped 1 Tsp lime juice 4-6 oz chicken breasts, boneless, skinless (or your personal preference)

Valentine Marshmallow Love Bugs - A Fun Unique Craft For Kids You will need:

• spaghetti • marshmallows • wax paper • lollipop sticks • rolling pin • pink sprinkles • sugar • mini heart cookie cutter • styrofoam block • mini gumdrops • shortening • large gumdrops • knife • heart confetti sprinkles • toothpick • black edible marker • light corn syrup

1. Slide two marshmallows (or three) on a lollipop stick. Fill a deep bowl or cup with water. Dip the marshmallows in and remove quickly, dabbing excess water on a paper towel. Hold the wet marshmallows over a bowl or plate and sprinkle generously to coat with the sprinkles. Rest the stick of marshmallows on a piece of wax paper.

2. Use the marker to add two black dots to two white heart sprinkles. Use a toothpick to dab dots of corn syrup on the back of the eyes to adhere to the marshmallows. Add a red heart sprinkle to the lower marshmallow.

3. Cut two mini gum drops in half horizontally with a sharp knife. Use a small piece of dried spaghetti, about 1-inch long, to skewer the top half of the gum drop and attach to the top of the bug’s head.

4. Roll a large gumdrop with a rolling pin on a surface dusted generously with granulated sugar. Roll about 1/4 inch thick. Cut out two mini hearts. Skewer the hearts with another small piece of dried spaghetti and attach to the bug’s body for wings. Prop in a Sytrofoam block to fully dry.

Rebuilding Credit After a Bankruptcy: What You Need To Know

O

ne of the most common questions I hear from clients is, “How will this affect my credit?” It’s a common question because it’s a good question, with many subparts. Oftentimes, clients fear the credit ramifications of filing a bankruptcy. The perception is that the filing of a bankruptcy is an “end” rather than a “new beginning.” I try to relay the latter, as it is the intent of the bankruptcy courts to provide the honest debtor a fresh start. This fresh start includes the ability to rebuild one’s credit over time.

Let’s address the various subparts that pertain to the overall theme of rebuilding one’s credit: 1. How long will it take for me to rebuild my credit?

Generally speaking, it takes people approximately two years to rebuild their

credit after the filing of a bankruptcy petition. What I try and emphasize to clients is that, although your credit will take an immediate hit, the bottom line is that you are being proactive in addressing and resolving the situation. This is a far better approach than languishing with poor credit for a number of years and getting nowhere. In some cases, it is best to take the initial credit hit, wipe the slate clean, and start to rebuild.

2. How do I rebuild my credit?

The best way to rebuild credit is by using credit. Many bankruptcy clients are surprised to find the number of credit opportunities they will be inundated with after filing for bankruptcy. The reason for this is that creditors know that a bankruptcy debtor is ineligible to file bankruptcy again for another eight years, generally.

Therefore, the creditors wish for debtors to resume the practice of relying on credit as soon as possible. I recommend a conservative approach to most clients, whereby they look to obtain a secured credit card (where the client essentially fronts the amount of credit offered) and use it for their monthly groceries or other regular expenses. Then, pay it off every month. Using credit helps to rebuild credit.

3. How long will a bankruptcy be on my credit report?

Make no mistake about it: a bankruptcy will be on your credit report for a period of ten years. However, as stated above, it will take you about two years to rebuild your credit. In the interim, the cash that will be freed up from not having to pay exorbitant minimum fees and the like will help most debtors operate their households while simultaneously rebuilding their credit.

4. Will I ever be able to obtain a mortgage again?

Absolutely. Most mortgage companies will not consider applicants who

have filed until they are 2-3 years post-bankruptcy. However, this period of time is also essential for clients to re-evaluate the course of their life/lifestyle. While rebuilding one’s credit post-bankruptcy, it is important that folks take advantage of the clean slate they have been provided and set a strategy that makes the most sense going forward. Therefore, that 2- to 3-year period of rebuilding is a welcome and needed respite.

I hope that the foregoing proved helpful in relaying the message that bankruptcy is not an end. Rather, it is a new beginning – an opportunity to start again and empower oneself to set your own fiscal course and rebuild over time.

VBAC Candidacy

“We typically see a 70-80 percent success rate when it’s determined that women are an appropriate candidate and go through a VBAC,” says Katie.

VBACs should only be performed in hospitals with the staffing and availability to perform an emergency cesarean, should it be needed. Anesthesiologists and neonatologists should be present at the hospital as well. During a VBAC, the patient is monitored closely for any signs of fetal distress or uterine rupture.

VBAC Benefits A vaginal birth shortens the recovery period. It’s typically a two–night hospital stay with a 6-week recovery period while a cesarean requires a 3-4 night hospital stay and a total recovery period of about 8 weeks.

VBAC also allows the patient to avoid a major abdominal surgery as well potential risks associated with the procedure, including hemorrhage, blood clots and infection.

“Many of my patients choose VBAC because they want to have the experience of delivering a baby,” says Katie.

Not all women are good candidates for VBAC. Reasons may include: history of extensive uterine surgery, a breech baby, placenta previa, twins or any other contraindication for vaginal birth.

When making recommendations for or against VBAC, Katie and her colleagues take a careful look at all possible factors including delivery challenges, dilation issues and more.

“Even if a woman had trouble dilating, VBAC is not automatically ruled out.

Instead, we investigate the reason for lack of dilation. Was it because the baby was in distress? How far did the woman get into dilation?” The bottom line is that women need to make choices that work best for them and their baby. It’s important for women to know that VBAC is an option and to arm themselves with information and work with a midwifery/ obstetrical team that is open to VBAC, should they choose that route,” says Katie.

Fecal Incontinence: Dealing with an Embarrassing Condition What is fecal incontinence?

Fecal incontinence, or bowel incontinence, is the inability to control bowel movements, causing the involuntary passage of stool. Symptoms of this often embarrassing disorder range from occasionally leaking small amounts of stool and passing gas, to completely losing control of bowel movements. Classified as a pelvic floor disorder (PFD), fecal incontinence occurs when muscles in the pelvic area cannot support the organs in the pelvis.

These organs include the bladder, uterus (women), prostate (men), and rectum.

What causes fecal incontinence and PFD?

The brain controls the muscles of the pelvic floor through the nerves. Medical conditions or injuries that impact the health of nerves (such as diabetes, Parkinson’s disease, stroke, back surgery, spinal stenosis, or childbirth) can result in weakness or injury of the pelvic floor muscle, resulting in fecal incontinence and PFD. Those with PFD are unable to control the muscles in the pelvic floor to have a bowel movement.

People with PFD contract these muscles rather than relax them, and because of this, they either can’t have a bowel movement or they have an incomplete one. PFD also includes urinary incontinence and pelvic organ prolapse. In fact, many people suffering with fecal incontinence also experience urinary incontinence. Urinary incontinence is a loss of bladder control due to bladder muscles that are too weak or too strong. Symptoms range from mild leaking to uncontrollable wetting, and become more common with age. Pelvic organ prolapse is a condition in which the uterus, bladder and bowel may “drop” onto the vagina and cause a bulge through the vaginal canal. Also common is a disorder in which the rectum becomes “stretched out” and slips out of the anus. This is called rectal prolapse. It is estimated that nearly one-third of all U.S. women are affected by one of these types of pelvic floor disorders in their lifetime.

In most cases, surgery options are minimally invasive, offering less pain, less scarring and shorter recovery time. She always approaches the problem with conservative measures prior to exploring surgical options, if and when necessary.

How is fecal incontinence diagnosed?

Physicians will begin the exam by asking about symptoms, taking a careful history and performing a visual inspection of the anus and perineum. A physical examination will determine the patient’s ability to control the pelvic floor muscles.

The physician will check for nerve damage, muscle spasms or muscle weakness.

Additionally, the physician may request or perform any number of medical tests available, including blood work, X-rays and ultrasounds, to uncover the cause of fecal incontinence.

Who is at risk for developing fecal incontinence?

Although men and women are at risk of developing fecal incontinence and PFD, pelvic floor disorders occur more frequently in women than men. Nearly one in 10 women older than the age of 40 has fecal incontinence, possibly as a complication of childbirth. Other risk factors include being over the age of 55, overweight, postmenopausal, having a past pelvic surgery, nerve damage or a connective tissue disorder.

How is fecal incontinence treated?

Fecal incontinence can be treated in a number of non-invasive ways.

Treatment usually combines self-care, medicines, physical therapy and home exercise.

Treatment depends on the cause of the problem you have and what best fits your lifestyle. Dietary changes often reduce the occurrence of fecal incontinence. As stool consistency is affected by what you eat and drink, your doctor may recommend diet modification and adjusting nutritional supplements taken. If nerve or muscle damage is the cause of the fecal incontinence, the physician may recommend exercises and similar therapies to improve function and strength. These exercises include bowel training and biofeedback. Biofeedback is a technique taught by specially trained physiotherapists that can increase anal muscle strength, allowing patients to learn to relax or contract pelvic floor muscles. Medication sometimes helps patients with fecal incontinence. When appropriate, these may be prescribed by your physician.

CW: Tell us about the main services you provide.

CW: What is the single most important aspect of your business that sets it apart from others like it?

Bob: We provide very fast turnaround for most projects by our courteous and knowledgeable staff. We take a “can do” attitude

to every single request. It really comes down to good old fashion customer service.

CW: How did you become involved in printing services?

Bob: About 16 years ago we relocated into a larger facility on Fire Rd. in Egg Harbor Township. Being as we were located on a major highway and surrounded by commercial and retail businesses, we decided to create a new division focused on retail and commercial printing. Already in the digital copier equipment business, it seemed like a natural diversification. I felt that we would be able to leverage our expertise in the equipment business to be successful in the commercial printing business.

CW: How does it feel to have your children follow in your footsteps? Was it a natural progression for them?

Bob: It’s extremely rewarding. I think many parents would love to have their children follow in their footsteps especially if they own a business. It presents excellent opportunities and challenges for everyone involved. I feel very fortunate to have both of our sons working in the business.

CW: Tell us about the vehicle wrapping services you offer.

Bob: We have provided vehicle wrapping and lettering for over four years with over 100 projects completed. They are mostly advertising graphics along with partial wraps and a few full color changes. This is an extremely labor intensive and technical procedure. We provide a complete solution from concept and design, through production and installation. We also provide vinyl lettering and magnetic signs.

Limiting Exposure to Opioids For Our Patients The opioid epidemic has reached epic proportions in New Jersey.

I don’t think there is a citizen of New Jersey who doesn’t personally know, or know of, someone who has died, or overdosed, or is struggling with addiction to opioids. Due to the stigma attached to addiction, the issue of using opioids intra-operatively, or prescribing opioids for pain relief, is the “elephant in the room” that people don’t want to address: not only in our society, but also in my practice.

Most practitioners my age were trained in the 1990s. The philosophy of pain management was such that a patient’s level of pain was considered, and measured, as if it were the “fifth vital sign” in addition to height, weight, blood pressure, and pulse rate. The ethical approach to pain management at the time was to address our patient’s pain generously, with the goal of completely eliminating pain. Hospital accreditation processes relied heavily on whether patient pain levels were measured and treating to zero levels of pain. This may have led to unwarranted opioid prescribing, as if opioids were the only means to manage pain. With the high numbers of opioid prescriptions written, who knows how many of these prescriptions were actually necessary and how many were used for unintended purposes.

Addiction is a complex disease that disrupts regions of the brain involved in motivation, knowledge, judgement, and memory. Addiction is devastating to the addicted person, their families, and their loved ones. Addiction is unpredictable. The reason why only some people can safely take prescription pain medications without becoming addicted and others cannot is not known at this time. The introduction of a patient to opioids, especially the opioid-naïve patient, for legitimate pain control carries a real risk of addiction. Many people have become opioid addicted following prescription of narcotics for a legitimate reason; for example, a surgical procedure. When the prescription runs out, the addicted person seeks other means, such as illegal pills or even heroin.

Let’s face it. Surgery, especially oral and facial surgery, can be painful.

The conditions I treat, such as infections, broken bones, painful teeth, jaw, and soft tissue problems, can be painful. So what can we do to eliminate opioids, or at the very least, minimize opioids for pain management before, during, and after surgery in my office?

For starters, we have the ability to provide sedation and general anesthesia services here while a patient is being treated for a surgical problem. I have the ability to also utilize local anesthesia to numb the surgical site. The combination of intravenous or inhalation anesthesia with local anesthesia is very effective in almost all patients. Therapeutic medications to minimize and post-operative swelling and inflammation are administered as part of our opioid-avoidance anesthesia technique. Long-lasting local anesthetics provide post-operative pain relief in the immediate phase. I have found it is generally not necessary to administer narcotics in the general anesthesia technique utilized here in my office.

Next, I have found that non-narcotic medications are effective in most patients for pain before and after surgery. Using a nonsteroidal anti-inflammatory medication such as ibuprofen in combination with acetaminophen is just as effective and, in some cases, more effective than narcotic medications. If it is absolutely necessary for a patient to take a prescription narcotic for postsurgical pain, in compliance with New Jersey law P.L. 2017, c. 28, which imposes restrictions on how opioid medications may be prescribed, no more than a five-day supply of the lowest effective dose available of an immediate-release opioid drug is prescribed for any patient.

Finally, every effort is made to keep our opioid-naïve patients just that – naïve to opioids. Taking opioids out of the equation altogether requires a commitment involving the surgeon, the patient, and the family. Realistic expectations regarding the surgical and anesthesia experience, levels of discomfort to be expected, and the duration of the recuperative phase must be clearly understood by all. Taking all this into account, it is possible to have an opioid-free procedure.

Hormonal Therapy as Part of Breast Cancer Treatment: Should Treatment be Extended?

Sumy H. Chang, MD, FACS, Provides Perspective Hormonal therapy is common practice for women battling estrogen receptor sensitive (ER-positive) breast cancer – which accounts for about 80 percent of all breast cancers. It’s used to reduce the risk of the cancer returning, and is typically the last medicine in the treatment process after surgery, chemotherapy and/ or radiation. However, a recent analysis indicates that some patients may reduce cancer recurrence by taking the hormonal therapy for a longer time period.

Here, Sumy H. Chang, MD, FACS, a fellowship trained, board-certified breast surgical oncologist, weighs in on hormonal therapy as part of breast cancer treatment. Dr. Chang is a provider with Barnabas Health Medical Group (BHMG), and is affiliated with Community and Monmouth Medical Centers, RWJBarnabas Health (RWJBH) facilities.

Dr. Chang says that studies like this are important as it helps some women from experiencing breast cancer treatment a second time.

the treatment plan for estrogen-receptor (ER)-positive breast cancer. However, for patients who tolerate hormonal therapy and have prolonged life expectancy we’re starting the conversation of extending the hormonal therapy for 10 years – which is in line with findings from the recent study.”

The analysis, published in The New England Journal of Medicine, was conducted by researchers from the Early Breast Cancer Trialists’ Collaborative Group. For this study, researchers took data from 88 clinical trials, including 62,923 women with ER-positive breast cancer.

Dr. Chang says most of her patients have minimal side effects from hormonal therapy, but others may experience joint pain that affects their quality of life. The study points out that decisions about extending therapy beyond the initial 5 years without any recurrence need to balance additional benefits against additional side effects.

Dr. Chang notes there are several factors to keep in mind to determine the length of hormonal therapy, including: How responsive was the tumor to estrogen;

if the patient is tolerating medicine, other medical conditions and how large the tumor was in terms of stage.

Risk of recurrence is based on stage and the nature of the tumor. Overall, the survival rate for breast cancer is high and recurrence rate is low, especially for estrogen and progesterone sensitive tumors.

“Not every woman wants to have hormonal therapy. For some, it’s the negative side effects such as joint pain and weight gain, cost and for others, it’s a daily reminder of their breast cancer diagnosis,” says Dr. Chang.

“And after 5 years, many patients look forward to stopping this medicine because it may be a mindset they are free and clear of cancer.”

The additional length of therapy – from 5 to 10 years – may be significant financial burden to some, including seniors who may be on a fixed income.

The benefits of continuing hormonal therapy may outweigh not taking it. The bottom line: hormonal therapy lowers recurrence of cancer coming back and it is a discussion worth having with your medical oncologist.

Frida Kahlo In the fall my daughter had been talking to me about a book she was reading for her Spanish class.

Every day we talked about the story. My daughter encouraged me to “look up” this woman she was reading about as I would really love her. As I was preparing for this feature of the new year, I was not sure the timing would be such that I could write about the infamous female that my daughter had been reading about and who had appreciated her story. I acquiesced to her request and as I began my research, I was truly amazed by such an extraordinary individual.

Born with the name of Magdalena Carmen Frieda Kahlo y Calderon (a/k/a Frida Kahlo) on July 6, 1907 to a German father and a Mexican Indian mother in Mexico City, Frida was one of seven daughters. At the tender age of six years old, she contracted polio, which left her with a deformed leg. After recovering from her illness, at the age of 14 Frida enrolled in a school in Mexico for medicine. Frida had aspirations of becoming a physician as a young child. Her education was short-lived after Frida was impaled by a metal rod during a bus accident. Frida suffered injuries to her spine, pelvis, collarbone and ribs. After the accident, Frida would spend two months in the hospital in a full body cast recovering from her injuries and then another month at home. Frida was known to have endured intense pain. As a means of coping with her pain, Frida’s father encouraged her to paint. Frida began painting self-portraits and had said, “I paint myself because I am often alone and I am the subject I know best.” Frida was known to be an extremely prolific painter and painted the true reality of her pain and her life.

Frida was known to embrace her Mexican culture and painted very colorful pictures of herself and also included symbols of her heritage. In a few of her paintings she would incorporate a monkey into the painting, which for Frida was a symbol of protection. Frida’s artwork was discovered by a famous Mexican painter by the name of Diego Rivera.

Diego Rivera admired Frida’s artwork and encouraged to her to continue to pursue a career in art.

Frida and Diego began a love affair that would result in a marriage that was not supported by Frida’s parents due to the significant age difference between them. The marriage blossomed into an extremely tumultuous relationship. Both Frida and Diego engaged in relationships outside of the marriage and they even divorced once and then reunited. However, it is reported that they both reportedly deeply cared for one another. Frida

was known to have had a temperament similar to a man back in the 1900s. She was “loose” with her words, had no problem using profanity and also was known to smoke and drink alcohol, which was not considered very “lady-like” for this historical time period.

Throughout Frida’s life she continued to struggle with her health and had to undergo a serious of operations as a result of her injuries. She endured amputations to her toes and leg. Frida lived in constant pain both emotionally and physically. This was a time where there were no medications to ease the pain. However, Frida continued to paint pictures that had meaning in her life of significant time periods.

Frida’s artwork was discovered by Andre Benton. He had an interest in hosting an art exhibit to feature Frida’s artwork in France. Frida’s health was deteriorating and she was hospitalized; however, she would not allow this to deter her ability to attend her debut of her exhibit. Frida had her hospital bed transported to the gallery and set up so she could attend her exhibition! It is reported that the Louvre had purchased one of Frida’s paintings. This was the first piece of artwork purchased from a 20th-century Mexican artist, let alone a female.

Frida Kahlo passed away on July 13, 1954 at home.

Frida’s cause of death is unknown as there are reports that reflect her passing was as a result of depression and other reports reflect it was as a result of medical complications.

As the feature comes to a close, I encourage everyone to reflect on Frida’s life and her ability to overcome many traumatic and difficult moments in her life. “I never painted dreams. I painted my own reality” was another quote by Frida. It is obvious that Frida’s artwork was a true expression of her reality and how she was able to process her life, albeit good or bad. As we begin the new year filled with new beginnings, I encourage everyone to remember Frida and allow her history to motivate you to keep moving forward in 2018!

The U.S. Department of Health and Human Services’ Office of Women’s Health is coordinating a week-long observance celebrated nationwide as Women’s Health Awareness Week. The aim is to empower women to improve their physical and mental health.

THERE ARE A FEW IMPORTANT STEPS WOMEN MUST TAKE IN THE ROAD TO HEALTH: • Get active.

• Visit a health care professional regularly for check-ups and preventive screenings.

• Eat healthy.

• Pay attention to mental health, which includes getting enough sleep and managing stress.

• Avoid unhealthy behavior.

One of the major diseases affecting women is cancer, which kills more than a quarter million women every year in the U.S. But that number could be reduced through early diagnosis and appropriate treatment.

Some cancers affect both men and women while others are unique to women. They include breast cancer[note that breast cancer is NOT unique to women, men get it too] (the most common in the U.S.) as well as cancer of the ovaries, cervix, uterus and external genitalia. That is the group we will focus on here.

Cervical Cancer Cervical cancer is the third most common cancer among women worldwide. More than 85% occur in developing countries, but it is also among the top ten causes of cancer deaths among women in the U.S. Over 95% of cases are linked to a sexually transmitted virus called HPV (human papilloma virus).

Risk factors include early and promiscuous sexual activity, smoking, prolonged use of birth control pills, STDs, and immunosuppression. In its early stages, this disease can be asymptomatic. Symptoms in later stages include bloody vaginal discharge, pain, and abnormal vaginal bleeding (in between regular menstrual cycles, post-coital, and post-menopausal).

Screening and Early Detection The good news is that screening and early detection can prevent deaths from cervical cancer. Both cervical cancer and pre-cancerous lesions can be detected by doing PAP smears. Women should start routine PAP tests at 21 years of age. Between the ages of 21 and 29, they should have the test every two years; between 30 and 65, every five years; and over 65, they may discontinue screening if they have had: • Three negative pap tests in a row; • Regular pap tests; • No abnormal pap tests in 10 years.

If a total hysterectomy is performed for benign reasons, there’s no need for PAP testing afterwards; if done for cancer, or if cervix is left behind, screening should be continued.

HPV Vaccination Another good piece of news is that there are two very effective vaccines – Gardasil and Cervarix – that will prevent those cases of cervical cancer that are linked to HPV. The vaccine is given to females between the ages of 9 and 26 years, and to males between 9 and 21. It’s given in three doses, ideally before the person has become sexually active. Please note: vaccinated women still need screening! Side effects include mild redness, tenderness and swelling at the injection site, as well as an increased risk of blood clots, but no long-term side effects have been reported.

When Is It Time For Assisted Living?

No one wants to move from their home into assisted living. However, in some cases, it is the best option to keep elderly or aging parents safe and healthy. To determine if it’s time for assisted living, or if your elderly parent can safely remain at home, take a good look at the present housing situation, health status and medical needs. Ask yourself these questions.

SIGNS THAT MAY INDICATE IT’S TIME FOR ASSISTED LIVING: • Is your parent telling you that he is eating, but you’re seeing food go bad in the refrigerator?

• Is your parent falling? To determine the answer, is your parent covering up bruises she doesn’t want you to see?

Cough And Cold Season Is Here—Are You Ready?

Believe it or not, cough and cold season is upon us. Our nights are becoming very cold and children have been back in school for about six months now. This is the season everyone dreads. It seems we see people with the sniffles everywhere—at work, at the mall and even at home. Now is the time to take some steps that will reduce your risk of catching a cold and stock up on things that will help with the symptoms when the inevitable happens.

You can reduce your risk of catching a cold or flu.

Here are some helpful hints:

WASH YOUR HANDS FREQUENTLY: This will help stop the spread of germs. Everything you touch potentially has cold germs on it— doorknobs, handrails, pens, PIN pads, etc. Also, keep hand sanitizer easily accessible—keep it in your car and your desk at work.

REDUCE STRESS: Whenever a big day is coming up, or a deadline you have been rushing to meet, you are more likely to become sick. Relax, your health is more important. Also, try to get at least eight hours of sleep per night.

EAT WELL: Certain foods combat colds. Try to include as much of the following as possible in your diet: broccoli, cauliflower, garlic, onions, citrus fruits, and whole grains. Some evidence also exists supporting using vitamin C and Echinacea to boost your immune system.

DRINK PLENTY OF WATER: Drinking plenty of water helps flush germs out of your body. Water also helps keep your mucous membranes moist, allowing them to trap the virus and dispose of it.

BREATHE FRESH AIR: Try to breathe as much fresh air as possible.

Recycled, contaminated air can make you sick! Get outside as much as possible. Cold air will not give you a cold!

For the most part, everyone catches some degree of a cold at least once a year. Once you feel the symptoms coming on, then what? For starters, continue the above tips. Also, cover your mouth and nose when you sneeze or cough (no one else wants your germs). And most importantly— CONTINUE TO WASH YOUR HANDS FREQUENTLY.

Will You Speak Up For A Child In Need?

Abused and neglected children need you to be their voice There are children living in our community that have no one to speak up for them. They are the over 700 children residing in Ocean County’s foster care system. When parents are unable, unwilling or unfit to care for a child, the child is removed from the home. The time they spend in foster care is often confusing, scary and, unfortunately, leaves them feeling isolated and alone. This is where a CASA volunteer comes in. CASA volunteers listen and get to know the child by talking with everyone in that child's life.

Every foster child deserves a CASA volunteer. Volunteers will join a group of caring individuals and helpful staff as we provide a voice for these children in need.

As a CASA volunteer, you will form a special relationship with your assigned child. CASA volunteers interact with the child, family members, social workers, healthcare providers, teachers and other relevant individuals to become a knowledgeable advocate for the child’s needs. The CASA then makes recommendations to the Judge on the child’s behalf and their best interest.

Colleen Kahl, Founder, Enspirited Projects Brick Native, Colleen Kahl earned her BFA in Acting from Montclair State. Upon graduation, she began working as an actress, administrator and producer, working on Broadway concerts, off-broadway musicals, producing projects, administrative opportunities and theatre education experiences.

After working in NYC, Colleen began her business, Enspirited Projects. Enspirited Projects is a creative programming and engagement company that partners with schools, municipalities and non-profits. Enspirited creates and implements a variety of programs (drama, fashion, self-esteem, anti-drug, superhero, princess, puppetry, special needs programs and more) for youth across the state.

Colleen’s professional goal of improving the creative thinking and self-esteem of local young people is what introduced her to CASA of Ocean County.

Upon hearing about the overwhelming needs of youth in the foster system, Colleen trained as an advocate and immediately set-up a philanthropic partnership between CASA and her new emerging project, The Creativity Studio of Enspirited Projects.

She started the practice about 20 years ago with a handful of patients in a small office she shared with her husband, a periodontist in Toms River. Imani Rosario, MD, University Urology Associates of New Jersey Dr. Rosario earned her medical degree from the Georgetown University School of Medicine and completed her post-graduate training in urology at Rutgers New Jersey Medical School. After completing her training, Dr. Rosario was appointed Assistant Professor of Surgery in the Division of Urology at Rutgers New Jersey Medical School.

In this position, she was responsible for training and teaching medical students, general surgeons and urologists in training. Dr. Rosario is Board Certified by the American Board of Urology. She joined UUANJ in 2016.

Rebecca Kolas, Certified Lice Technician and Owner, Lice Lifters of Ocean County

Rebecca Kolas is both a Certified Lice Technician and Owner of Lice Lifters of Ocean County in Toms River, which opened in August 2016 and serves Ocean, Monmouth and Atlantic Counties as the premier lice removal company. Since that time she has opened a second location, Lice Lifters Central Jersey in Hamilton which serves Mercer, Middlesex, and Burlington Counties.

Rebecca was introduced to the Lice Lifters franchise when her two youngest children contracted lice. Trying over the counter and prescription medication, she was unable to conquer lice, which have become resistant to the harmful pesticides, chemicals and toxins contained in those products. Rebecca was then introduced to Lice Lifters as a customer and found that it was the only treatment that was successful, as well as safe and natural.
