you choose to be your agent to ensure that person fully understands your wishes and desires related to medical care and treatment. Your agent should make the decision you would make if you were able to do so. When does my advance medical directive go into effect? If you are so sick and can no longer make medical decisions, your medical team would turn to your health care agent for guidance. It will only be used when you are no longer able to make medical decisions for yourself. What do I do with my advance medical directive after I have it prepared? It is important you tell your agent and loved ones that you have prepared an advance medical directive. Also, make copies of your advance directive to give to your agent, loved ones, caregivers and doctors. I am young and/or I am healthy, do I really need an advance medical directive? It is advisable that all adults have an advance medical directive, no matter their age or health status. You never know when you may become too sick to make decisions related to your medical treatment. When you have an advance medical directive you have the peace of mind that your named agent will be the person to make your medical decisions and that your wishes related to your care and treatment will be followed.

f there is one document in estate planning that generates numerous questions it is the advance medical directive. In an effort to make this document less confusing and a bit less intimidating I have set forth some of the most common questions I have encountered related to this topic. What is an advance medical directive? An advance medical directive is a written document that informs others of your wishes about your health care. It allows you to name a person, referred to as your agent, to make medical decisions for you if you are unable to do so. It expresses your desires and preferences about medical treatment in case you become unable to communicate these instructions during terminal illness or permanent unconsciousness. What is the difference between a living will and an advance medical directive? Today, the terms living will, advance medical directive, healthcare proxy, and healthcare directive are used interchangeably. When living wills were first introduced they were used to help people express their wishes related to artificial life support and other advance medical techniques at end of life. As the use of living wills grew in popularity, so did the health care concerns addressed in the living wills; it grew to include such medical concerns as tube feeding, resuscitation, and organ donation. If I have a power of attorney, do I still need an advance medical directive? Yes, although they are similar, they are also very different. In both documents you name an agent to act on your behalf. However, a power of attorney addresses financial and other legal matter while an advance medical directive addresses medical care and treatment. They are two separate documents. Who should I appoint as my agent under my advance medical directive? It is very important you choose someone you trust; someone you know will follow you wishes. Your advance medical directive is not a list of medical orders but, instead, acts only as guideline. Therefore, it is crucial for you to talk to the person

Can my advance medical directive ever be changed? Yes. As you go through life, oftentimes, your views and opinions related to end of life decisions as well as who should serve as your agent may change. It is important to review your advance medical directive on a regular basis. You can always prepare a new advance medical directive if you have any changes. Just make sure you throw away old copies and provide copies of the new advance medical directive to your agent, loved ones, caregivers and doctors.

Managing Osteoarthritis with Physical Therapy By: Candice Brockel, DPT, Freehold/Howell Physical Therapist In order for our body to move, we have bony connections called joints that are attached by tendons and ligaments. Covering the end of each bone within the joint is a smooth and flexible tissue called cartilage. This tissue provides protection, reduces shock, and decreases friction between the two bones during movement.

What Is Osteoarthritis? Osteoarthritis is a degenerative joint disorder in which there is progressive destruction of the articular cartilage. Due to the loss of the cartilage, the joint space becomes narrow, allowing the bone ends to rub together. This creates pain and may lead to further damage such as bone deformity or bone spur formation.

How Do I Get Osteoarthritis? The cause of primary osteoarthritis is unknown; however, there are factors that may increase your risk for this condition. These include age, gender, past injury/ trauma, obesity/weight gain, leg length discrepancy, and sports or occupations that place repetitive stress on the joint. There are even theories of genetic factors leading to primary osteoarthritis. According to the Centers for Disease Control and Prevention, osteoarthritis is the most common form of arthritis, and is usually more common in women than men over the age of 50.

What Are The Symptoms? Osteoarthritis is characterized by joint stiffness and pain, generally affecting large, weight-bearing joints. Typically, the joint involvement is often asymmetrical. In other words, the complaint of pain is usually worse on one side than the other. Many people report feeling or hearing joint noises such as crunching, crackling, or clicking with movement, not always associated with pain. The stiffness is usually felt in the morning or after prolonged periods of rest, which improves with movement. Other symptoms include increased pain by the end of the day and pain during/after activity, relieved by rest. At the late stages of the disease, you may experience pain even at rest. Limitations in strength and joint range of motion become apparent as well as noticeable joint deformities due to cartilage destruction and joint space narrowing. Inflammation is not a primary characteristic of this disease but may be seen in the late stages due to bone trauma.

How Is It Diagnosed? Osteoarthritis is diagnosed by X-ray results assessed by your physician. Clinical findings of symptoms can also provide a good insight for diagnosis.

How Can Physical Therapy Help? While physical therapy cannot change the damage that has already been done, it can help manage your symptoms to improve activities of daily living. Within our joints there is a substance called synovial fluid, which lubricates and provides nutrients to our cartilage. Because cartilage has no blood supply, when it is damaged there is no way for it to regenerate. However, when we move our joints, synovial fluid is produced. Therefore, it is vital that we get our joints moving to provide extra protection and lubrication to the already damaged area,

which, as a result, decreases pain and improves mobility. When coming to physical therapy, your physical therapist will first perform an evaluation to get the history of your condition and identify limitations or restrictions with your movement through an objective assessment. Since the progression and symptoms of osteoarthritis are different for each person, your therapist will create an individualized exercise program specific for your needs. In general, your therapist will give you exercises in your pain-free range to help promote synovial fluid production. It is also important to decrease the stress/forces placed on your joint by increasing the strength of the surrounding muscles. Depending on the severity of the disease, activity modification and proper instruction on the use of an assistive device may also be needed for pain relief. Along with therapeutic exercises, your therapist can provide hands-on techniques to improve joint function. In addition, certain modalities such as hot packs, cold packs, and electric stimulation can be provided to help ease your pain. You will also be given a customized home exercise program, which, together with your attendance in physical therapy, will become key to improving your symptoms and slowing the disease progression. Candice has completed clinical affiliations in areas of outpatient orthopedic rehab, skilled nursing facility, and school based pediatrics. She has treated a variety of patient populations including post-surgical orthopedics, spinal disorders, arthritis, musculoskeletal injuries, as well as adult and pediatric neurological conditions. Why has this been successful for you? Jim: Through autonomous care, a therapist is able to make clinical decisions based upon their extensive knowledge for what is best for their patient. As a company, we respect each therapist’s background and education. Allowing the therapist to make clinical decisions and assess progress creates better patient outcomes and contributes to their overall experience. OCW: Please explain the importance of treating the emotional part of recovery. Mike: It is essential to address all aspects of recovery and we believe strongly in the continuity of care, meaning pairing every patient with one therapist. This allows the therapist to establish a strong bond with their patients and address any emotional barriers of recovery. Emotional barriers are important because we believe there is a direct correlation between chronic pain and depression. If the mental part is not addressed, recovery will be unsuccessful or short lived. OCW: What are some positive changes you have seen in the field in the last ten years? Mike: There has been great progress in the manual (literally, hands-on) approaches used by physical therapists over the last ten years. Addressing soft tissue

through Instrument Assisted Mobilization (IASTM) as well as improving range of motion through joint mobilization is essential to achieving optimal outcomes while reducing pain levels. All of our therapists have been trained in the latest approaches to minimize pain and maximize recovery. OCW: Tell us about some of the benefits of being a regional training center for clinical students. Jim: As we have continued to expand, it has given us the opportunity to train the therapists of tomorrow through clinical education. In doing so, it helps our therapists stay up to date on the latest research and interventions taught in the top programs in the area. We regularly host students from some of the best Physical Therapy programs in the Tri-State area including Stockton University, Drexel University, and Rutgers/UMDNJ. OCW: Tell us about some of the successful therapy used for treating arthritic conditions. Mike: As the baby boomer generation enters into their 60’s and beyond, we have noticed an increase in the diagnosis and incidence of arthritic conditions. In our model of physical therapy, we address restoring joint health through manual techniques and applicable modalities. Many people feel as if arthritic conditions are untreatable; well that is absolutely not true. There are a number of non-invasive treatments that not only reduce, but also alleviate arthritic pain. OCW: What is the one common characteristic that all staff members exhibit throughout the organization? Jim: Our company is filled with people that demonstrate true compassion. It is essential for us to be invested in our patients’ recovery. Additionally, we look for individuals that are always seeking to improve their clinical skills and grow within the organization. Because physical therapy is an ever-changing field, it is essential to keep up with the latest interventions so that we can help more patients. OCW: What would you like All-Care to be known for as a company? Mike: I would like All-Care to be known for providing the best overall physical therapy experience. Skin Cancer: COMMON MISCONCEPTIONS REGULAR SUNSCREEN USE PREVENTS VITAMIN D ABSORPTION

Vitamin D, which is produced in the skin after sun exposure, is known to improve bone health and reduce the risk of certain cancers. Getting an adequate amount of vitamin D is dependent on these three tactics: 1) eating foods that contain vitamin D, 2) taking a vitamin D supplement, and 3) getting a small amount of sun exposure. The fact is that large epidemiologic studies reflecting real life conditions prove clearly that sunscreen use does not cause vitamin D deficiencies. Adequate vitamin D can be obtained safely and cheaply through food and dietary supplements without the risks associated with overexposure to UV radiation. If you are concerned about your vitamin D levels, speak with your doctor. SUNSCREEN IS UNSAFE AND CAUSES SKIN CANCER

Over the years, a few controversies have developed regarding the safety of sunscreen. The safety of sunscreens has been studied in labs and on live subjects by reputable research scientists for many years. The results of these studies provide overwhelming evidence that sunscreens are safe and effective. Jeffrey S. Gosin, MD, FACS Dr. Jeffrey Gosin has dedicated his career to the treatment of individuals with vascular and vein diseases. He is Board Certified in both Surgery and Vascular Surgery. He is a graduate of the University of Pennsylvania and Jefferson Medical College. He completed his General Surgery residency at Robert Wood Johnson University Hospital where he was Chief Surgical Resident. After his residency, he remained at Robert Wood Johnson and completed an accredited fellowship in Vascular Surgery, where he received extensive training in traditional open vascular surgery and minimally invasive procedures, performed medical research, and had numerous papers published in scientific journals. Dr. Gosin has a special interest in minimally invasive techniques and new medical technology and constantly strives to provide the most advanced state-of-the-art therapies to his patients. He was the first surgeon in his region to perform a stent graft repair of an abdominal aortic aneurysm, a treatment that is now the standard for most patients. He is a leader in minimally invasive vascular and vein treatments. Other physicians have travelled to his center to learn these techniques.

Virginia S., Billing Specialist Virginia is our billing specialist. She has been with us since 2002. Prior to joining our group, she worked in the billing department of a large cardiology group outside of Philadelphia. She is available throughout the work day to answer any questions that you may have regarding a bill or health insurance. Karen K., Clinical Manager Karen is our clinical manager and has been with us since 1998. Shore Vascular & Vein Center Maria N., Receptionist Maria, one of our receptionists, has been with Shore Vascular & Vein Center since 2009. She grew up in Atlantic City. After a long career in the casino industry, she changed her focus to the medical field and has been providing excellent assistance to patients since that time. Maria is fluent in both English and Spanish and is able to assist our Spanish-speaking patients in understanding all aspects of their care. When she is not working, Maria enjoys spending time with her two children and four grandchildren. Denise “Dee” T., Nurse Dee is a nurse who has been with Shore Vascular & Vein Center since 2013. She has more than 40 years’ experiences in the medical field, first in New York prior to moving to the Jersey Shore. Dee considers herself “a humanitarian and a patient advocate.” She states, “I believe we have to humble ourselves, and the way you do that is by serving other people!” That commitment is obvious in her kind and compassionate day-to-day interaction with our patients. Joanna A., Medical Assistant Joanna has been a medical assistant with Shore Vascular & Vein Center since 2014. She began her career with our group as an extern while she was still in school. After graduation, she was offered a position in our office and has been with us ever since that time. Joanna enjoys working as a medical assistant and helping other people. Being fluent in both English and Spanish, she finds it particularly rewarding to help our Spanish-speaking patients better understand their condition and treatments. When not working, Joanna enjoys spending time with her family and her two dogs. Joanne H., RN, RVT Joanne has been with our group since 1995 and has had a long and illustrious career in healthcare. Joanne began her career as a critical care nurse. Then in 1985, along with Dr. Stephen Gosin, the founder of our original surgical practice, she helped develop the first noninvasive vascular laboratory in the region. She is a true pioneer and expert in vascular ultrasound. Joanne was the director of the Noninvasive Vascular Laboratory at Shore Medical Center from 1985-

2009. During that time, she also performed vascular ultrasound tests in our office. She retired from Shore in 2009, but she has remained active in our office vascular lab since that time. Joanne was instrumental in developing the policies, procedures, and technical qualifications for our office-based noninvasive vascular laboratory to achieve and maintain its accreditation through the Intersocietal Accreditation Commission. Joanne is constantly complimented not only on her skills as a vascular technologist, but also on her bedside manner and compassion with our patients. Arunas S., RVT, RPVI Arunas has been our noninvasive vascular ultrasound technologist since 2002. He earned his Doctor of Medicine degree after graduating from the Medical Faculty of Vilnius University in Lithuania in 1988. Arunas had been practicing medicine for nine years in the field of invasive cardiology before moving with his wife and two children to the United States in 1997. Here he started a new career in noninvasive vascular imaging and technology after graduating from the College of Health Professions at Thomas Jefferson University in 2001. Arunas has also worked at the Hospital of the University of Pennsylvania. He is a highly skilled vascular technologist who maintains the standards required of our lab’s IAC Accreditation. Cheryl G., APN Cheryl is our nurse practitioner. She earned her Master’s Degree from New York University in 1997. She joined Shore Vascular & Vein Center in 2014. Prior to joining our practice she had experience in oncology, infectious disease and in cardiology. Cheryl’s primary focus is working with our aesthetic laser patients. While all of the laser procedures are performed by our physician, Dr. Gosin, Cheryl is very knowledgeable in all aspects of laser treatments and works closely with those patients. She assists patients with their pre- and post-procedural care and helps prepare them for their treatment. In her free time, Cheryl enjoys gardening, volunteer work, reading, and spending time with her family. Lori S., Business Manager (not pictured) Lori is our Business Manager. She handles the insurance billing and daily office accounting. She joined Shore Vascular & Vein Center in April 2017 after working many years in Cardiology. Sleeve Gastrectomy 101

WHAT IS A SLEEVE GASTRECTOMY? The Sleeve Gastrectomy reduces the size of the stomach from about 32 ounces (a quart) to approximately 8-10 ounces (the size of a banana). The sleeve restricts the amount of food a person can consume AND has an additional effect - the “chemical/hormonal” effect. The chemical/hormonal effect significantly reduces a person’s appetite for a prolonged period of time after the surgery, anywhere from one to three years. The reduction in appetite supports ongoing weight loss as it is much easier to keep food intake to a minimum when a person doesn’t feel hungry. Bariatric patients who have struggled with their weight and appetite are often amazed by this aspect of the surgery. The reduction in appetite is one of the factors that makes the sleeve a very impactful bariatric option for obesity. IS THE SLEEVE GASTRECTOMY PERMANENT? The Sleeve Gastrectomy is a permanent procedure. Once the portion of your stomach is removed, it cannot be ‘undone’. The gastric sleeve has been around since the 1970s and was originally part of a two-part surgery called the duodenal switch. After years of study and validating the sustained weight loss results, the gastric sleeve was approved as a stand-alone bariatric procedure. The advent of the gastric sleeve, as an insurance-approved procedure for weight loss surgery, took place in early 2010. Dear Friend, I am a dentist, but, like everyone else, my teeth need regular care to preserve them and to make them as attractive as possible. So, just like everyone else, I go to the dentist to receive many of the procedures I deliver to my own patients. When I sat down to write goals for my practice, it occurred to me that the kind of dentistry I want to give to my patients was exactly the kind of dentistry that I wanted to receive. And I can assure you that I would hold my dentist to some pretty high standards. I would want my dentist to know me personally. I wouldn’t want to be a number or “the next patient.” And I wouldn’t want to be taken for granted, even years into the relationship. Maybe I am spoiled, but I get annoyed if I have to do too much waiting. I’d prefer my dentist and staff to be ready for me when I arrive. I’m also a stickler for quality. I would pay a little more if I am receiving top-of-the-line treatment. These teeth are the only ones I get, so I wouldn’t want to choose a dentist because his fees are lowest. I would choose a dentist who could provide me with the highest level of care, which is what I feel I deserve. So what I want as a dental patient is what I hope you want, because the result of this kind of treatment is my true goal: optimal dental health and appearance. IRA Rollovers – Small adjustment, Big impact. When most people think of Individual Retirement Accounts (IRAs), they think of getting to retirement, not getting through retirement. Truth is, for most people, the tasks can be equally difficult. BENEFITS OF AN IRA ROLLOVER If you’re eligible to receive a retirement plan payout because of an upcoming career shift (either a different job or retirement), a rollover to an IRA may help you defer income taxes and avoid early withdrawal penalties. Most importantly – you can continue the tax-deferred growth of your retirement account, meaning more to work with as you age. HOW ROLLOVERS ARE ACCOMPLISHED: The 60-day Rule for IRA Rollovers If you receive your distribution yourself from your company plan, you have 60 days to roll it over to an IRA. If you miss the deadline, the distribution becomes taxable, and if you are under age 59½, you will be subject to a 10% penalty, too. The same 60-day rule applies if you choose to move money from one IRA to another, taking control of the funds in between.

Avoid that problem with a direct transfer. The 60-day rule needn’t be a concern at all if you don’t take your retirement money from your company plan or IRA yourself. You may arrange for a direct or trustee-to-trustee transfer. In both cases, by requesting it, the funds will move from your plan account to an IRA (or from IRA to IRA) without your need to receive the distribution in hand. That means asking your company or current fund manager to send the funds over to the new plan administrator. Bonus of a Direct Transfer! No withholding tax. Your employer must withhold 20% of your distribution for federal income taxes, but this withholding is not necessary when the IRA trustee receives the money directly. INVESTING DURING RETIREMENT Getting through retirement means more than just asset management and tax planning, though that is important to consider. It means accounting for your assets, estimating your expenses, and making a plan so your saving and earning power will last. Conventional wisdom in structuring a portfolio

for retirement sustainability calls for heavy equity exposure during the early years of saving. That gives the portfolio the most growth potential, and the risks inherent in the stock market can be leveled out over time. As retirement approaches, the equity allocation is reduced, and the less volatile fixed-income component of the portfolio is increased. That reduces the chance of a major portfolio loss just before retirement drawdowns begin. When retirement begins, how much can be withdrawn each year without running the risk that the portfolio will be exhausted? That depends upon the behavior of the financial markets and the portfolio’s asset allocation. If a bear market takes hold just as the retirement is starting, the risk of running out of money can be high. On the other hand, in a steady or rising market, the portfolio may generate enough growth to build a buffer in the early years of retirement, protecting it from down markets later on. Investment success requires time, not timing. That is why you should think long-term in your investment strategy and you should not be distracted by passing fluctuations or fads in investment management. Dr. Elisa Velazquez (Dr. Lisa) has had an obsession with teeth for as long as she can remember. It is this passion that drives her practice, Ocean Pediatric Dental Associates. As pediatric dentists, Dr. Velazquez and her talented team of professionals are highly adept at understanding the needs of children and have received specialized training to enable them to work in this environment. OCW: What do you enjoy most about it? Dr. Lisa: I love seeing kids smile and leave our office happy. I get a few hugs a day and it makes my day awesome! I especially love when I can influence a patient and they tell me they want to be a dentist when they grow up. OCW: How do you handle a child that is extremely nervous on a first visit? Dr. Lisa: By talking to them and explaining each step very slowly we show them exactly what needs to be done. We use childfriendly words and try to alleviate their fears of the unknown. Our entire team is highly empathetic to our young patient’s needs and knowledgeable in children’s behavior.

OCW: What attracted you to the field of Pediatric Dentistry?

OCW: Why do some children who stick to a healthy diet still get cavities?

Dr. Lisa: I didn’t see a dentist regularly as a child but was always obsessed with my teeth. For my high school graduation my gift from my parents was to get my teeth cleaned. We did not have dental insurance so this was a big deal. I want this generation of children to have more positive experiences with dental care as part of their overall health. While in college I worked as a dental assistant for an orthodontist and loved the environment. My goal has always been to make a difference for children.

Dr. Lisa: Cavities stem from the bacteria in our mouth and consumes sugars and release acids in the mouth. It can be transmitted from caretakers. The acid breaks down the enamel and dentin of the teeth. The enamel of the teeth can also be affected by a variety of factors including medications and some medical conditions. Unfortunately some people are more susceptible and have higher risk factors. We also explain to families that not all healthy foods are good for your teeth. For example, orange juice is naturally acidic and high in sugar.

OCW: What is the most challenging aspect of your profession? Dr. Lisa: It can be tough to work against the fears that some children (and adults) have of the dentist. Dentistry, in general is sometimes difficult for kids and it is hard to see them be afraid. Our team works hard to help parents overcome their fears so they aren’t transferred to their children.

DENTAL SEDATION FOR CHILDREN Some children require extensive dental care or are very phobic. Sometimes children are unable to sit as a traditional dental patient with just local anesthesia or for x rays and cleanings. S E DAT I O N O P T I O N S : 1) INHALATION SEDATION (IN OFFICE): Inhalation sedation using nitrous oxide can help children handle nervousness, pain, or gag reflex difficulties during dental procedures. Nitrous oxide (N2O), or laughing gas, is simply a gas that you breathe in through your nose. This colorless, sweet-smelling, and non-irritating gas is commonly used in dental offices. It can help your child feel calm, comfortable, and less sensitive to pain. A child must be able to breathe in their nose for the duration of the procedure for this to be effective.

Oral Conscious Sedation is when your child receives a liquid sedative in the office to have his or her treatment completed. This is known as “giggle juice.” The child is drowsy and unaware of the treatment being performed. This is a safe and effective treatment choice for the nervous or fearful child or for very young toddlers. This is all done in accordance to the American Academy of Pediatric Dentistry guidelines (AAPD.org). Often the dental work can be completed in one procedure, depending on the amount of work your child needs.

IV Sedation in the office is recommended for apprehensive children, and children with special needs that would not work well under oral conscious sedation. This is an alternative to Hospital Dentistry or multiple oral sedation visits. A dental anesthesiologist in our office administers the IV and all anesthetic drugs, and all dental work is completed in one visit. This is very safe and effective.

4) HOSPITAL DENTISTRY (at Community Medical Center or Jersey Shore Medical Center) Hospital Dentistry is when your child has ALL dental treatment completed in a hospital as an outpatient. Under general anesthesia with an anesthesiologist, all treatment is completed in one session while the child sleeps. This is a same-day surgery procedure. This procedure is very safe and is ideal for very young children, children with extensive dental needs, special needs children and teens.

Women suffer with the disorder more frequently than men and the initial onset of symptoms occur most often in adolescence and early adulthood but may manifest at any age. Though IBS is considered a benign condition with no risk of cancer, intestinal damage or associated inflammation, it is quite debilitating in many patients. IBS is one of the most common reasons for absence from work in the U.S. Symptoms of IBS vary by the subtype but all commonly include abdominal pain, bloating, pain relieved by defecation, mucus rectal discharge and alteration in bowel habits. What Is The Cause Of IBS? Despite the common frequency of this disorder, there is no defined cause of IBS. Research has failed to detect any specific defect in the gastrointestinal tract that explains the illness. It has been theorized that a motility disorder (nervous system dysfunction that prevents proper muscle function of intestines) is the prime defect but this has yet to be confirmed. Most experts believe that there are likely multiple contributing factors. How Is IBS Diagnosed? Experts in the field have developed clinical criteria to help diagnose IBS. This is based on history a patient gives a doctor during a visit. These criteria can only assist your doctor in making the diagnosis but are not definitive. This means that one must exclude other illnesses that mimic IBS before the diagnosis of IBS is confirmed. Disorders that can mimic or cause similar symptoms to IBS include: infections, Inflammatory Bowel Disease (Crohn’s Disease and Ulcerative Colitis), Celiac Sprue (gluten sensitivity), Lactose Intolerance and Colon Cancer and other abdominal cancers. To exclude these disorders, when signs and symptoms are present, physicians will often perform blood and stool tests as well as place patients on selective diets. Colonoscopy is often necessary to exclude Colon Cancer or Colitis. Only once these more serious conditions have been excluded by patient history and necessary exams can patients be reassured of the diagnosis and treatment recommended. Should I Be Concerned? If your symptoms include bleeding, weight loss, bowel movements that wake you from sleep, oily stools or new symptoms presenting later in life, than it is urgent that you see your doctor right away to have appropriate evaluation as these may be symptoms of much more serious conditions. But even if you do not have these alarm symptoms, you should seek medical advice for treatment of IBS if you suspect you suffer with this disorder. How Is IBS treated? There is no cure for IBS so the goal of therapy is to properly diagnose the disorder and thereafter tailor a treatment to the individual. Doctors will often give advice on diet and prescribe medications depending on the nature of the symptoms.

Pros and Cons of Radiation

As a radiologist, I often receive questions about radiation. Much of the equipment in my field uses radiation to enter my patients’ bodies and create images. We all know that large levels of radiation can be dangerous, but how dangerous is the radiation one receives from an X-ray or CT scan? I am here to answer some of the more common questions that are of concern.

Q: What is my risk of being exposed to radiation? A: Radiation is everywhere and we are exposed to it on a daily basis. It

Otherwise, these patients should receive the absolute lowest levels of radiation while still obtaining the best images possible.

The amount of radiation emitted from CT scanners can be adjusted; however, setting a CT to emit a very low level of radiation produces very poor image quality. Image quality is important for accurate diagnosis of specific diseases and cancer. As a doctor and a radiologist, it is extremely important to me to reduce the radiation exposure to my patients as low as possible while still creating high-quality images for accurate diagnosis. That is why we implement the latest techniques using our new ultra-fast, 64-slice CT scanner, which gives the highest image quality while reducing the radiation dose by up to 60%.

Q: Why is radiation dangerous? A: When the cells of our body are exposed to

radiation, it can create something called free radicals. Free radicals damage the DNA of that cell. Most of the damage is repaired by the cell, but if the damage is severe, two things could happen: the cell can die or the cell’s DNA is repaired incorrectly. If the cell dies, a new cell is created; however, if many cells die and the damage is severe, it could take a long time to recover. If the cell’s DNA is repaired incorrectly, that cell has the potential to become cancerous.

In summary, radiation is all around us in low doses. Most medical imaging exposes us to low levels of radiation, which does not significantly increase our risk of developing cancer in our lifetime. As adults grow older, radiation exposure becomes less and less dangerous. And there are some who believe that occasional exposure to very low levels of radiation can be beneficial. If You Have Been Told You Are Not A Candidate For Dental Implants:

SEE DR. BANKS!

Often, patients are told that they are not candidates for dental implants because they “don’t have enough bone” or their “sinus is too low.” Both conditions are correctable problems in almost all patients, according to Dr. Banks. When the gum ridge where teeth are missing is too thin to place a dental implant, bone grafting can be performed to correct the problem. What that means is that a bone replacement product, or even the patient’s own bone, can be placed into the deficient area, stabilized, and allowed to heal. Following healing, dental implants can be placed into that area and then restored with porcelain teeth. This procedure is called “ridge augmentation” and can be accomplished in both the top and bottom jaws. When teeth are missing, the sinus that lies above the top jaw will expand downward into the ridge and cause bone loss in the gum ridge. This is a correctable problem as well, according to Dr. Banks. “A Sub-Antral Sinus Augmentation – commonly called a ‘Sinus Lift Procedure’ – can be performed, sometimes even at the time of implant placement. This procedure increases the amount of bone in the area where we would like to place a dental implant. Again, a bone replacement product or even the patient’s own bone can be used.” A common misconception is that the bone is placed into the sinus. In reality, the sinus membrane is actually lifted up (hence to name “sinus lift”) back to where it should be, and the bone graft material is placed under the sinus membrane, restoring the gum ridge and the sinus contour together in one procedure. Dr. Banks incorporates state-of-the-art diagnostic procedures, such as 3D cone beam CT scanning of the face and jaws, to evaluate her patients for bone grafting procedures. “That way, I can accurately diagnose the magnitude of the bone loss in the area where we would like to place implants and the quality of the patient’s bone ridge in that area, and pre-plan the surgery to ensure the best possible outcome.” She also prepares and uses advanced biologic materials into grafting procedures such as L-PRF (Leukocyte-Platelet Rich Fibrin) which are made right in the office at the time of surgery. And finally, there is a preconceived notion that these procedures are painful procedures. “There are multiple anesthesia options for the surgical part of the bone grafting process,” she says. “We offer intravenous general anesthesia and sedation, nitrous oxide and local anesthesia” so that all her patients can be treated comfortably. “Of course, I’m making it sound easy but these procedures are technically demanding and take years of practice to gain the proper expertise. You only want your bone grafting procedures performed by a well-trained, ABOMS-certified oral and maxillofacial surgeon.”

Sometimes Good People Find Themselves In The Criminal Justice System-Use An Expungement To Help You

s we stand here in 2017, finances become ever so important in an extremely competitive job market. We are in a technologically advanced society where people can learn about you and your background with the stroke of a computer key. Depending on the agency that is conducting the criminal background check, whether private or public, a wealth of information about you can be yielded. Having a conviction in New Jersey for a crime, offense or otherwise can possibly jeopardize securing a job if you have been involved with the state criminal justice system. Not only can a conviction affect your ability to secure a livelihood, it may need to be disclosed and can affect college applications, graduate school applications, professional licensing requirements, relationships, court proceedings, military entrance, certain jobs requiring a background clearance, housing and insurance. The law regarding expungements is highly technical and therefore you need an experienced criminal attorney to obtain an expungement. An expungement is legally defined as “the extraction and isolation of all records on file within any court, detention or correctional facility, law enforcement or criminal justice agency concerning a person’s detection, apprehension, arrest, detention, trial or disposition of an offense within the criminal justice system.” N.J.S. 2C:52-1a. Expungements apply to juveniles and adults. Our legislature has recognized the role of an expungement. Life experiences and observation suggest that a multitude of reasons exist why a person has landed in the criminal justice system – marital issues, health problems, mental health issues, drug dependency, financial failure, death of loved ones or simply a product of circumstance. Recognizing these unfortunate yet real problems, New Jersey criminal law states in pertinent part that regarding expungements “the primary objective of providing relief to the reformed offender who has led a life of rectitude and disassociated himself with unlawful activity.” N.J.S. 2C:52-32. Not all crimes and offenses are subject to an expungement. For example, motor vehicle offenses for which a person was arrested or convicted are generally not subject to an expungement. See N.J.S. 2C:52-28. A person seeking an expungement application should know that New Jersey requires certain time passage before one is even eligible to apply. In some cases, depending on the facts and circumstances of the individual, it may be a waiting period of ten (10) years, five (5) years, three (3) years, six (6) months or immediately.

Particularly significant are expungements applications that can be filed immediately. The Governor signed a fairly new law to be effective April 18, 2016. See N.J.S. 2C:52-6a et seq. However, many experienced attorneys and persons involved in the state criminal justice system are not aware of it and how it can help their clients. This new law allows for an expungement application to be filed immediately if a person’s charges were dismissed or resulted in an acquittal, with certain exceptions. To illustrate, we recently helped an individual in Essex County. We were successfully able to get his indictable charge downgraded to a simple assault. Thereafter, we got the simple assault charge dismissed. I prepared all of the necessary paperwork and provided it to the judge to transfer over for expungement purposes. There was no waiting period and this process helps clients who are looking for immediate filing and an expeditious expungement of same. The Administrative Office of the Courts (AOC) has provided a criminal directive and legal form to be utilized in these instances. The law on expungements, like many laws, can and often does change from moment to moment. Likewise, the judicial interpretations dictate the roadmap in this technical area. Depending on what you were convicted of, the time frame, what your criminal records are before and after, and a list of other crucial factors determines if you will be afforded an opportunity for a clean slate. When it comes to your life and your livelihood, you need an experienced attorney who will tend to your best interests. A Passion for the Law: Meet Attorney Ehsan F. seeing it on the news and social media from various trials, arrests, and day-to-day crimes being committed. OCW: What is the most rewarding aspect of your profession? Ehsan: When a client seeks the advice of an attorney, it is usually for a problem. However, when in need of a criminal defense attorney it is more serious because a person’s life and liberty are in jeopardy. My role is to provide sound advice and the best possible outcome based on the current law and facts of their case. While this is challenging, it is also highly rewarding. People land themselves in the state criminal justice system for a myriad of reasons resulting from drug and alcohol issues, mental health issues, marital problems, financial troubles, and most often lapses in judgment. Fortunately, in New Jersey there are options we can help them with. Drug offenders can partake in drug court rehabilitation if eligible. The one time mistake making individual may be afforded a diversion program of Pre Trial Intervention (PTI) and we are seeing a rise in an effort to assist individuals with mental health issues. For lower level offenses, jail time can potentially be removed in lieu of probation with strict conditions such as anger management, community service, and or a host of other things deemed necessary by the court. Additionally, if a person has already been convicted and has led a law-abiding life, we can provide expungement of a criminal record services if eligible. he short answer is NEVER. It is important to understand some basic facts about having a “fresh start.” “Normalcy” is defined as being within certain limits that define the range of normal functioning. How we each come to define those limits over time is a whole other story.

For many of the people I work with, a “normal” day starts at 8 am, when they are arisen by a cacophony of debt collectors ringing their house phone and/or cell phone, harassing them for money they simply do not have. A normal day may include a similar call made to their workplace, or in some cases a neighbor or relative. A normal day usually ends with a trip to the mailbox, where various forms of threats against one’s home, wages and belongings await. Serves 4

1 sheet puff pastry, thawed and rolled out into a 14in square. 4 boneless chicken breasts, cooked 1/3 cup herbed soft cheese 1/4 cup fresh parsley , chopped 1 large egg Optional ingredients-chopped spinach, diced roasted red peppers, additional cheese, etc.

Preheat the oven to 400 degress. Roll out puff pastry, cut into four equal sized pieces. Place 1 1/2 tablesoons of herbed cheese in the center of each piece. Top herbed cheese with a chicken breast and then chopped parsley Next, fold the corners of the pastry over the chicken breast. Place chicken pastry seam side down on a baking sheet covered in parchement. Lightly brush the top with a whisked egg. Bake for 20-25minutes or until crust is golden brown.

Serves 6

What is an Anal Abscess? An anal abscess is an infected cavity filled with pus found near the anus or rectum. What is an anal fistula? An anal fistula is frequently the result of a previous or current anal abscess, occurring in up to 50% of patients with abscesses. Inside the anus are small glands. Sometimes, these glands become clogged and become infected. This leads to an anal abscess. The anal fistula is the tract or tunnel that forms between the clogged gland inside the anal canal and the infected skin/abscess outside of the anus. A fistula can present with or without an abscess. Other causes of anal fistula are: Crohn’s disease, radiation, trauma and malignancy/cancer.

How does a person develop an anal abscess or fistula? Most anal abscesses are the result of an acute infection in the glands inside of the anus. Occasionally, bacteria, fecal material or foreign matter can clog the anal gland and create a condition for an abscess to form. Once an abscess is drained, in approximately 50% of cases, a tunnel (fistula) will persist under the skin connecting the infected gland to the outside skin where the abscess was located. When this occurs, patients typically experience some type of persistent drainage from the site of where the abscess had been in the perianal skin.

What are the signs or symptoms of an abscess or fistula? A patient with an abscess may have pain, redness or swelling in the area around the anus. Other common symptoms are fevers, chills, fatigue and generally not feeling well. Those patients who develop an anal fistula frequently complain of perianal irritation, moisture in the perianal area and/or persistent, episodic drainage in the perianal area. A common complaint of patients with anal fistula is the sensation that they cannot keep the perianal clean or that they feel “wet” in the perianal area.

Are antibiotics necessary to treat anal abscess or anal fistula? Antibiotics, alone, are NOT a wise treatment for anal abscess or fistula. Studies have shown that for uncomplicated abscesses, the addition of antibiotics to surgical drainage does not improve healing time or reduce the potential for recurrences. There are, however, certain circumstances in which antibiotics may be added to surgical drainage. These would include those patients with extensive cellulitis, patients with cardiac valvular disease, and those patients with altered or suppressed immune systems. This determination is made by your surgeon after taking a careful, detailed medical history and performing a physical examination.

What is the treatment for anal fistula? Surgery is almost always necessary to treat an anal fistula. This may require a single stage or multi-staged procedure. Colorectal surgeons are specialists who are trained to treat anal fistula and are well versed in the different types of surgeries to treat anal fistula. The surgery involves connecting the external opening (site of drainage on the skin) to the internal opening (the clogged gland). There are several different types of surgical procedures to accomplish this and a colorectal surgeon would determine which procedure is best suited to your condition.

Are there any tests performed to diagnose an abscess or fistula? A majority of anal abscesses/anal fistulas are diagnosed by history and physical examination alone. Occasionally, additional studies such as CT scan or MRI can assist the surgeon with the diagnosis of deeper abscesses or to help identify a deeper or complex fistula tract/tunnel.

How is an anal abscess treated? The treatment of anal abscess is surgical drainage. Once the overlying and surrounding skin has been numbed with local anesthetic agents, an incision is made in the skin over the abscess to drain the pus. Chemotherapy Before Breast Cancer Surgery or Not? Sumy H. Chang, MD, FACS, Provides Perspective Traditionally, breast cancer patients underwent surgery to get a mass removed, then the doctors decided whether or not chemotherapy was needed. But now, in many cases, women with large or aggressive cancer undergo chemotherapy prior to surgery. Why the change? Sumy H. Chang, MD, FACS, a fellowship trained, board-certified breast surgical oncologist, says there are a number of factors to consider when making treatment decisions, specifically, whether chemotherapy before surgery is recommended and why it’s beneficial. Dr. Chang is a provider with Barnabas Health Medical Group (BHMG), and is affiliated with Community and Monmouth Medical Centers, RWJBarnabas Health (RWJBH) facilities. According to a 2015 Yale University study published by the Journal of the American College of Surgeons, researchers found that women with larger breast tumors who had chemotherapy were more likely to have a lumpectomy than a mastectomy. Furthermore, the study states that the proportion of women

being treated with chemotherapy before surgery steadily increased from 13.9% of women in 2006 to 20.5% of women in 2011.

• Age of patient – younger women typically have more aggressive breast cancer while older women tend to have less aggressive cancer

“As part of the overall treatment program, patients must meet with a medical oncologist prior to undergoing surgery to review options and discuss the risks of chemotherapy,” says Dr. Chang. “Generally speaking, if there’s a large tumor, we would opt to shrink it through chemotherapy treatment prior to surgery to perform a lumpectomy rather than a mastectomy. If a patient’s lymph nodes are involved, we work to shrink the tumor so we can minimize the lymph nodes we need to remove.”

Factors that determine chemotherapy treatment decisions include: • HER2 status – a sign of aggressive behavior • Tumor size – typically larger than 2 cm • Grade of cancer • Lymph node involvement – the higher the number of positive nodes, the more beneficial chemotherapy may be

Dr. Chang says another reason to choose chemotherapy is if a patient is not an operative candidate – meaning the tumor is attached to the muscle, chest wall or ribs. The surgery would be extremely aggressive, so shrinking the tumor will likely lead to a less complicated surgery. “While chemotherapy is beneficial for many patients, not everyone is a candidate for chemotherapy and there are a host of side effects,” continues Dr. Chang. “Since chemotherapy affects the heart function, we need to run tests to be sure the patient can withstand the treatment.” The bottom line: we examine pathology and take an individualized approach to best meet each patient’s individualized needs. Our goal is to tailor the patient’s treatment according to the nature of the cancer and help them resume their full lives.

Fast forward to today where life expectancy has reached the late 80s to early 90s, but the age of menopause has remained relatively the same. The CDC advises using condoms and other barriers consistently and correctly, getting tested and waiting for results before having intercourse, and communicating openly and honestly with your partner, as a few key ways to reduce the risk of infection.

Did you know that lung cancer is the leading cause of cancer deaths in both men and women in the U.S.? Approximately 200,000 new cases are diagnosed each year and 27 percent of all cancer deaths are from lung cancer.

What causes lung cancer?

SMOKING. Smoking is the number one risk factor for lung cancer. Even

though non-smokers can also get lung cancer, the risk is much higher in those who smoke. The more you smoke, the higher your risk. The likelihood of getting lung cancer decreases after you quit smoking. Tobacco smoke damages cells in the lungs, causing the cells to grow abnormally. Regular exposure to smoke from someone else’s cigarettes, cigars or pipes (called second-hand smoking) can increase a person’s risk of lung cancer, even if they are non-smokers. EXPOSURE TO ASBESTOS. Certain jobs such as shipbuilding, asbestos mining, insulation or automotive brake repair may cause exposure to asbestos. Asbestos fibers can irritate the lung and cause cancer. When combined with smoking, the risk of developing lung cancer is much higher. RADON. Radon is an invisible, odorless gas released by soil and some rocks. Exposure to higher levels of radon can cause cancer, especially lung cancer. WHAT ARE THE TYPES OF LUNG CANCER?

There are two different types of lung cancer: small cell and non-small cell. This difference is based on the type of cancer cells. Most lung cancers are non-small cell. Among the non-small cell lung cancers, there are several subtypes: adenocarcinoma, squamous cell and large cell. WHAT ARE THE SYMPTOMS AND SIGNS?

Symptoms and signs include coughing, hemoptysis (coughing up blood), shortness of breath, chest pain, fatigue and weight loss. Most often, the symptoms do not manifest until the cancer is advanced. Sometimes lung cancer can be detected as a shadow in the lung on an X-ray done for other reasons. WHAT IS THE DIAGNOSIS?

Once your doctor suspects that you have lung cancer, you will be sent for CT scans and in some cases, a PET scan. A PET scan can detect cancer more accurately than other tests. The diagnosis is confirmed by taking a biopsy of the suspicious lesions seen on the X-ray or CT scans. The scans also help to see if the cancer has spread to any other organs, a process called metastasis. ARE THERE DIFFERENT STAGES?

Staging is different for small cell and non-small cell lung cancers. Small cell lung cancer has only two stages: limited stage (confined within the lungs) or extensive (spread outside the lungs). Non-small cell lung cancer has four stages: stages I to IV. Stage I is a small tumor that is confined to the lung, that has not spread to the lymph glands. Larger tumors and those that are spread to the lymph glands come under stages II and III. When the cancer has spread to other organs, it is stage IV. WHAT ARE THE TREATMENT OPTIONS?

Treatment is based on the stage and the type of lung cancer. Small cell lung cancer is mainly treated with chemotherapy. In limited stage small cell lung cancer, radiation can be combined with chemotherapy. Early stage non-small cell lung cancer is best treated by surgical resection. An experienced surgeon removes the tumor, surrounding normal lung tissue and the lymph glands around it. Even after removing the tumor, cancer cells can remain and cause recurrent cancer, either in the lung itself or in other organs. Chemotherapy, radiation or both are used after surgery to reduce the chance of recurrent cancer. Advanced stage lung cancer is treated with chemotherapy. The types of chemotherapy drugs used are based on the type of lung cancer, age of the patient, other medical illnesses, etc. There is ongoing research to improve the outcome in patients with lung cancer. for the treatment of various cancers. These targeted agents attack the cancer cells specifically, sparing other cells. Targeted agents cause less severe side effects than chemotherapy. A few different targeted agents are found to be beneficial in lung cancer. Tarceva (erlotinib) is a drug that works in certain types of lung cancers carrying a mutation in a receptor called EGFR. A new drug called crizotinib was approved recently for the treatment of lung cancer that carries a gene called ALK fusion oncogene. No two tumors are alike! Even among lung cancers, there are different distinguishing features. Your oncologist chooses the type of treatment based on a number of factors, including the cell type, stage, different biological markers on the cancer cells and your other medical problems. WHAT YOU SHOULD KNOW:

It’s That Time Of Year Again…Should You Get A Flu Shot?

What You Need To Know

The flu is a respiratory infection that’s caused by many different viruses. Up to one in five people get the flu in the U.S. each year.1 It comes on suddenly, causes worse symptoms than the cold and can be serious for some people. As you know, last year, a new strain of flu was thrown into the mix. Called swine flu at first, then H1N1, it caused symptoms similar to regular flu, such as fever, cough, aches, chills and fatigue.2 Did you get a flu shot last year? If you’re someone who’s always thought flu shots are just for your grandparents, think again. For the 2017-18 flu season, the Centers for Disease Control and Prevention (CDC) now recommends that almost everyone get a flu shot. That is, everyone six months and older. These changes will simplify guidelines that have been confusing to many people. You should know that the new flu vaccine will include the H1N1 strain. Any shortage in vaccine supplies this season may slightly alter a plan to vaccinate everyone at once, however. In that case, the CDC may prioritize vaccines as it has in the past. It would first target people who

are at higher risk of developing complications from the flu. This includes people who are 65 and older and pregnant women. Also, people with certain chronic medical conditions, such as asthma or heart disease, would take priority. In addition, it is also important for those coming into close contact with these people to get vaccinated early.3 Now, if you’re pregnant, you may wonder if it is safe to get a flu shot. The short answer is, yes. However, this is not true if you have a severe allergy to eggs. That’s because the ingredients for flu shots are grown inside eggs. Also, you should not get a vaccine if you have had a severe reaction to a flu shot in the past. If you’re pregnant, request the flu shot, not the nasal spray vaccine. It is made from an inactivated virus. This makes it safe for you and your baby during pregnancy. Avoid trying to get pregnant for four weeks after a vaccination with a nasal spray vaccine.4 So, when should you get a flu vaccine? Now is a good time to start thinking about it. Flu season runs from November to April. Most cases hit between the

end of December and early March. You can probably get the vaccine starting in September. It’s best to do it as early as possible so you can head off any infections “at the pass.” Getting the flu shot early gives your body time to build up immunity. This means it will protect you better against the flu.5 If you haven’t gotten the vaccine by December or so, or if it hasn’t been available for some reason, still go ahead and get it when you can. Remember that the nasal mist vaccine is only for healthy, non-pregnant people who are between 2 and 49 years old.5 If you do develop cough and cold symptoms, stop in to see us. Karen is the director of midwifery at Ocean Gynecological and Obstetrical Associates in Toms River and Lakewood, N.J., which is part of Barnabas Health Medical Group, the state’s largest hospital-based multispecialty medical group, and affiliated with Community Medical Center, all within the full network of services offered by RWJBarnabas Health. In her work, Karen empowers women to find a birth experience of their choosing by educating them on the options and expectancies of delivery. She helps her patients remain safe and healthy during the entirety of their pregnancy and through delivery by uncovering and addressing fears and providing a firm, available voice of support. Prior to joining Ocean Gynecological & Obstetrical Associates, Karen worked at Columbia Presbyterian Medical Center in New York City for nearly a decade. She received a Bachelor of Science degree in nursing from William Patterson University in Wayne, N.J., and her master of science in nursing, with a specialty in nurse midwifery, from Columbia University. She is certified by the American Midwifery Certification Board. Karen also teaches prenatal yoga. Our mission is to provide a safe haven and caring environment for vulnerable youth to enable them to unlock their potential and learn skills that will empower them to build healthier lives, relationships and futures.

BIKE RIDE 2017

TEEN PROGRAM

Ocean’s Harbor House held its 21st annual “Ride For New Beginnings” bike ride from June 23-25, 2017. The three-day ride raised over $75,000 to support programs for the homeless youth Ocean’s Harbor House serves. Close to 40 riders pedaled an average of 75 miles a day. The riders started in Toms River, pedaled through the Sourlands Mountain Area in Princeton and completed the ride at the Transitional Living Program house in Toms River where they were greeted with a celebratory barbecue for riders and their friends and families. Ocean’s Harbor House is the ONLY youth shelter of its kind serving homeless, abused and neglected youth, ages 10-21, in all of Ocean and Monmouth Counties. The annual ride has enabled Harbor House to expand its facilities and enhance its programs. The ride has raised over $2 million since its inception! The 2018 Ride is scheduled for July 11-15, 2018 starting in Cape May.

Close to 100 community partners attended an extremely thought-provoking and informative presentation by Dr. Kenneth Ginsburg, nationally recognized author of Reaching Teens: Strength-Based Communication Strategies to Build Resilience and Support Healthy Adolescent Development. The area of focus for Dr. Ginsburg’s seminar was trauma informed care; adolescent brain development, grief and loss, and self-care/mindfulness for youth workers. Attendees were mesmerized by Dr. Ginsburg’s insightful presentation about youth development and the audience was rapt with attention throughout the almost 4-hour program. The seminar was provided free for Harbor House staff and our community partners, who included DCP&P, OPC, PCANJ, Anchor House and Collier House. The event was hosted by Ocean’s Harbor House and was supported by a Justice and Mental Health Collaboration Program grant from the US Department of Justice to the Ocean County Department of Human Services.

Covert Operations: Women in the Military In October of this year, I was a participant in a training alongside law enforcement. There was a mix of police officers, corrections officers and Port Authority officers, all from various departments in New Jersey. Hidden amongst the group were two veterans who served active duty in the military and then entered in the law enforcement sector upon their discharge. One of the veterans was a female who modestly identified herself as being a veteran. Fascinated by this officer’s modesty, I began contemplating how many other women have dutifully served our country and yet have remained “under the radar” without recognition for their commitment for their service.

the line of duty. By World War II, the number of females who enlisted increased dramatically to 350,000. The Japanese also captured 67 women who became POWs for three years. By 1942, American women were stationed in England, France, Australia, New Guinea and the Philippines. The military branches had created reserves for females. Specifically, the Navy created Women Accepted for Volunteers Emergency Service. The Marines created Semper Paratus and the Marine Corps Women’s Reserve. The Air Force created Women Air Force Service Pilots. In 1991, 40,000 women had served in the Gulf War. The Gulf War was the first time that men and women were deployed together to engage in active warfare. Although women were permitted to serve in the war, they were limited to certain positions, and “by assignment” they were kept from combat zones. It would not be until 2013 that females would be approved to serve in combat zones. There are two females who received the Silver Star Award. The first was Leigh Ann Hester for her “exceptional valor” in close-quarter combat as a result of her leadership in the Iraq War. The second was Monica Lin Brown for her heroic effort to protect wounded soldiers in her platoon by risking her own life and running into the line of fire to save their lives. It is reported that she placed her body over her fellow soldiers to protect them from further harm. As noted above, women were not permitted to serve in combat zones. It would not be until 2015 that an order would be signed permitting women to serve. As part of the training, women were required to complete Army Ranger School. In 2015, Captain Kristen Griest and First Lieutenant Shaye Haver were the first females out of 96 students to graduate from Army Ranger School, which was also the first year the school accepted females. Currently there are only two women who have held the rank of general. The first was General Ann E. Dunwoody, who retired from active duty in 2012. The second is General Janet C. Wolfenberger (USAF). There are many amazing females who have served our country and have remained “under the radar.” These women appear modest and yet have had a significant impact on our military units. Through their perseverance they have leveled the battlefields to stand side by side with their male counterparts. The motto of the military has evolved to include women to “be all they can be.”

As I began researching for this feature, histiry has reflected that women in the military traditionally were assigned to “auxiliary” roles. However, women served in the military dating back to the Revolutionary War, the Civil War and the Mexican Wars. It was reported that women would disguise themselves as men and enlist into all branches of the military. In 1782 Deborah Sanson Gannett from Plymouth, Massachusetts, enlisted in the Army under the name of her deceased brother. She was injured in the line of duty two times and was so concerned her identity would be revealed, she even tended to her own surgical needs. She, alongside of her male counterparts, fought together in the Revolutionary War for 17 months. Deborah was compensated 22 years after her service and was granted a pension from the United States Army. Following in the footsteps of Deborah, Margaret Corbin was known to have entered the battlegrounds with her husband, albeit under cover as a man! Margaret became famous when her husband was killed and she continued to load and fire the cannon against the British. Elizabeth Newcume boldly joined the military in 1847. Elizabeth spent 10 months on active duty disguised as a man. After it was discovered that she was female, she was immediately discharged.
