Peripheral Neuropathy is a nerve disorder known to affect millions of people throughout the U.S. It is a complication found in several different medical conditions including Diabetes, Peripheral Vascular Disease, Post Chemo­ Radiation therapies and Metabolic disorders being some of the most common. While we trudge through the last days of winter and look forward to the first day of spring on March 20th, I am reminded of a quote by the late Robin Williams. Balance and Physical Therapy By: Jennifer Vadas, DPT, Manager, All-Care Physical Therapy Center, Jackson Facility Balance plays a key role in the vast majority of typical activities we perform throughout our day. Whether it is reaching for a dish, standing while talking to a neighbor, or swinging a golf club; the importance of maintaining the highest functional level of balance cannot be understated. Balance tends to decline as we age; which puts older adults at an increased risk for falls, and subsequent injury. However, difficulties with varying levels of balance can affect people at all ages. Impairments in balance can often be a secondary effect of many other conditions, including stroke, arthritis, multiple sclerosis, and neuropathy; just to name a few. There are multiple systems of the body that impact balance; they include the visual, somatosensory, vestibular, and musculoskeletal systems. The visual system plays a role in balance by allowing you to see any obstacles in your path, and react accordingly. The brain also receives visual signals to help perceive how the body is oriented in regards to its surroundings. The somatosensory system includes tactile sensation (touch), and proprioception (positional awareness). Tactile sensation in our feet allows us to feel them on the ground below us. This allows us to be aware of any changes in the surface we are walking on. Our proprioceptive system allows us to understand the position of our joints, this is particularly important in our ankles in order to be aware of any postural sway and react accordingly to correct balance. The vestibular system is located in the inner ear; it functions in balance by detecting movements of the head. This allows for clear vision with motion, and maintaining upright positioning. The musculoskeletal system involves our muscles, bones, tendons, and ligaments. It is important to maintain good flexibility and strength in our muscles; as they help to stabilize in balance tasks. If muscles are overly tight, they will be unable to allow for motion required to move correctly, or to compensate if you are losing your balance. Maintaining good muscular strength is important throughout the body. Our postural muscles (i.e. those in the shoulder, back, and abdominals) help to sustain optimal positioning for good balance. Lower extremity strength, from the hips down to the ankles, is important to preserve stability in both static (standing still) and dynamic (moving) tasks. Should one of these systems become compromised, it is important that the remaining function at the highest possible level to overcome that deficit. Physical therapy can be utilized to rehabilitate areas that have been weakened or injured and to increase the functional level of the others. For example, in a patient with visual impairments, physical therapy can be utilized to improve the function of the vestibular system; as well as increase muscular

strength and flexibility. Physical therapy allows you to challenge your balance and perform activities that are difficult, while in a safe setting with professionals who can properly cue you for methods to better perform these activities. Your therapist will perform a comprehensive examination to determine where your particular deficits lie, and will then establish a program focusing on your individual needs. Programs tend to encompass all areas that impact balance; including stretching muscles that are overly tight, strengthening those that are weak, using varied surfaces/directions of movement, performing tasks with eyes closed, and decreasing base of support. There is also a focus on incorporating functional tasks into therapy treatments. A common complaint we hear from people who are coming to therapy for balance is they find they have increased difficulty maintaining their balance while walking in crowds, or in grocery stores when trying to look for objects. Therapy will mimic these scenarios by providing patient with perturbations (similar to being jostled in a crowd), and teaching appropriate reaction strategies. Another common exercise is having a patient walk while both nodding and turning their heads, similar to what we do in a grocery store to look for the box of cereal we want while still moving down the aisle. These are just a few examples of the many activities a patient may perform in therapy as part of a balance training program. It is extremely important to feel safe and confident with functional mobility to uphold a high quality of life. Having feelings of limitations in balance will decrease the possibility of this. Fortunately, there are treatment options available to improve balance and mobility to uphold your personal lifestyle goals. She continued her education at the University of Medicine and Dentistry of New Jersey in Stratford; where she received her Doctorate of Physical Therapy in 2009. Since graduating in 2009, Jen’s primary professional focus has been in the outpatient setting; though she also has experience in acute care and sub acute rehabilitation. Jen’s areas of interest include geriatrics, non-operative and post-surgical rehab, and balance impairments. She has taken continuing education courses in vestibular rehabilitation. WHAT YOU NEED TO KNOW ABOUT SUDDEN HEARING LOSS Imagine you woke up one morning and couldn’t hear out of one ear. Would you know what to do? Would you seek treatment? Unfortunately, for most people the answer to both of these questions is a resounding no. Sudden onset hearing loss is considered a medical emergency, and prompt treatment might just save your hearing. If you experience a sudden hearing loss, you should see an audiologist or ENT physician immediately. A hearing test should be performed immediately as well.

Every year, one out of every 5000 adults experiences sudden onset hearing loss. Also known as sudden sensorineural hearing loss, or SSHL, sudden onset hearing loss occurs all at once or over the course of several days. For most people, the immediate assumption is that they are suffering from allergies, an earwax blockage or sinus infection, so they might decide not to seek treatment.

SSHL doesn’t always manifest itself in the same way. Some people notice when they wake up first thing in the morning that their hearing is different. Others don’t notice a difference until they hold the phone up to that particular ear or try to listen to headphones. In some cases SSHL is preceded by a very noticeable “pop,” which can be quite alarming. Afterward, some patients report a feeling of fullness in the affected ear or a strange feeling on that side of the head, possibly accompanied by tinnitus and dizziness.

SSHL differs from other types of hearing loss in a couple of important ways. Aside from its rapid onset, it is often idiopathic, meaning the cause is usually unknown. As a matter of fact, a cause can be identified in only 10 to 15 percent of diagnosed cases. Some possible known causes, however, include: • Tumors (e.g., acoustic neuroma) • Neurologic diseases or disorders (e.g., multiple sclerosis) • Bacterial infections • Head injury or trauma • Inner ear disorders (e.g., Meniere’s) • Autoimmune diseases (e.g., rheumatoid arthritis) • Ototoxic medications • Circulation issues (e.g., vasculitis) SSHL IS DIAGNOSED BY AN AUDIOLOGICAL EVALUATION. The most common treatment for sudden deafness, especially in cases where the cause is unknown, is corticosteroids. Steroids are used to treat many different disorders and usually work by reducing inflammation, decreasing swelling, and helping the body fight illness. Steroids are usually prescribed in pill form or an injection in the eardrum. Time is of the essence when diagnosing and treating sudden hearing loss. About half of people with SSHL will recover some or all of their hearing spontaneously, usually within one to two weeks from onset. Eighty-five percent of those who receive treatment will recover some of their hearing. There are indications that the window for treatment closes two to four weeks after the onset of the hearing loss, meaning the hearing loss will then become permanent and irreversible if medical follow-up is not performed. If you experience sudden onset hearing loss, whatever you do, don’t ignore it in the hopes that it will go away. Basal Cell Carcinoma Basal Cell Carcinoma, also known as BCC, is a type of skin cancer that begins in the basal cells. Appearance

• Oftentimes no symptoms • Sometimes can be itchy or painful • Non-healing lesion • Redness • Loss of color • Small bump resembling a cyst or a pimple • Swollen blood vessels in the skin • Ulcer • Pearly growth • White waxy lump • Scaly patch • On sun-exposed areas, such as the face and neck

• More than three million US cases per year • Caused by sun exposure • Diagnosed and treated by a dermatologist (since BCC sometimes resembles noncancerous skin conditions such as psoriasis or eczema) • In certain cases will need a referral to a surgeon

Prevention

• Seeing dermatologist for regular skin checks • Reporting any new mole or growth • Limiting sun exposure • Using sun screen

Treatment

Treatments include prescription creams, radiation or surgery (Mohs surgery, electrosurgery, cryosurgery, wide local excision) to remove the cancer. -Mohs surgery: Surgical removal of skin cancer, one layer at a time, until only healthy tissue is left.

-Electrosurgery: Using electric current to cut tissue. -Cryosurgery: Using liquid nitrogen to freeze superficial BCC. -Wide local excision: Surgical removal of a small area of cancerous or precancerous tissue, along with a margin of normal tissue. BCCs are easily treated in their early stages. The larger the tumor has grown, however, the more extensive the treatment needed. While BCCs seldom spread to vital organs, they can cause major disfigurement and occasionally result in nerve or muscle injury. Certain rare, aggressive forms can be lethal if not treated promptly. When small skin cancers are removed, the scars are usually cosmetically acceptable. If the tumors are very large, a skin graft or flap may be used to repair the wound in order to achieve the best cosmetic result and facilitate healing.

The Top 5 Issues in Divorce and Special Needs Children I recently helped a divorce attorney prepare for a case in which there is a special needs child. Divorce is not easy and is especially complex when a special needs child is involved. When the parties have a special needs child, it is critical to make sure the parents carefully consider the present and future needs of the child. IMPORTANT ISSUES TO FOCUS ON: 1. Governmental Benefits. If the child is receiving governmental assistance, it is important to involve a lawyer familiar with special needs trusts. The parent and lawyer should anticipate, to the extent possible, the current and short- to mid-term custodial and financial needs of the child so that an appropriate parenting schedule and amount of support can be set in order to avoid going to the Court, which may be costly, to make modifications or to obtain an increase in support. Frequently, children with disabilities are recipients of government benefits such as Medicaid and SSI. These benefits are means tested. In other words, in order to be eligible, there’s only a certain amount of assets that your child can have in his or her name in order to qualify for these benefits. To ensure protection of your child’s benefits, it’s important that the divorce attorney include language for the creation of a special needs trust. Our office frequently helps family lawyers provide the proper language and craft the special needs trust so that the child support is paid directly to the trust instead of to the child, thereby reducing the fear that benefits would be eliminated. 2. The Daytime Needs of the Child. Because of the type of condition of a special needs child, does a parent need to be available during the day to take care of the child? On the other hand, there have been cases where a parent has essentially tried to “exploit” a child’s otherwise minimal disability in order to convince a Court that employment outside of the home was not feasible. In cases involving special

needs children, it is imperative to accurately assess the true needs of the child. 3. Child Support. In New Jersey, there are special considerations allowed to enter when calculating child support, including the disability of the child. Be sure to have an attorney who is able to gauge the needs of the child beyond the basic calculations and formulas that are used for child support. 4. Life insurance. Typically, in a divorce, the payer of child support is required to obtain life insurance to guarantee child support in case the payer dies. When there is a child with special needs, it’s important to take precautions that any life insurance proceeds should not go to the child directly but instead to the special needs trust. 5. Education. Oftentimes, the child may receive special education services. If so, the custody order must clearly outline each parent’s role in the educational decision-making process. Unless the custody agreement grants one parent sole legal custody or revokes a parent’s educational decision-making rights, it is assumed that both parents will have the right to attend IEP meetings, the right to consent to services in educational placement, and the right to request mediation and due process hearings. There are many other considerations to contemplate in these cases. Conventional And Complementary Medicine: The Perfect Partnership For Your Pet Conventional medicine aims to narrow down a health issue to a discrete problem, and then find a treatment to control the resultant symptom. Like giving antibiotics for an infection, or giving insulin to a diabetic, it’s an excellent launch pad for getting back to a state of health. However, because conventional medicine is inherently reductionist in nature, it often fails to address the big picture. onna Kolomeyevskaya, MD, is a fellowship-trained gynecologic oncologist specializing in surgical and medical therapy of women with gynecologic cancers, as well as precancerous disease of the female genital tract and complex benign conditions. A member of Barnabas Health Medical Group, Dr. Kolomeyevskaya treats patients at both Monmouth Medical Center in Long Branch and Community Medical Center in Toms River. As a third-generation physician, Dr. Kolomeyevskaya has a passion for medicine. Fascinated by the complexity of women’s health care issues, she has dedicated herself to a career in gynecologic oncology. After graduating magna cum laude from Boston University, she earned her medical degree at Albany Medical College in New York. Dr. Kolomeyevskaya completed a residency in obstetrics and gynecology at Baylor College of Medicine in Houston, Texas, followed by a gynecologic oncology fellowship at Roswell Park Cancer Institute in Buffalo, New York. Early in her career, she distinguished herself in research and has authored and co-authored numerous scientific

HPV and Cancer: What you need to know HPV (human papillomavirus) is the most common sexually transmitted infection, affecting most sexually active people at some point in their life. The infection is asymptomatic, and one can get infected and re-infected many times throughout one’s life. There are many strains of the HPV virus and it is the “high-risk” forms of HPV that can cause cancers of the cervix, vagina, vulva, oropharynx, anus, penis, and scrotum. Whereas “low-risk” strains of HPV cause genital warts. “We’re not used to thinking of cancer as a sexually transmitted disease,” says Dr. Kolomeyevskaya. “But in the case of cancers caused by HPV, that’s what it is.” The good news is we are better at screening patients: women get co-tested for HPV at the time of their Pap smear. Then, any abnormalities are closely followed and treated, if necessary. Getting vaccinated against HPV before the onset of sexual activity reduces the likelihood of contracting it or developing high-grade precancerous changes or cancer. Dr. Kolomeyevskaya emphasizes safe-sex practices to lessen the chances of transmission of other sexually transmitted diseases (STDs) that also potentiate HPV infection. The body can clear the infection on its own – especially in those who are young and healthy – but people can easily become re-infected, and partners can pass it back and forth.

Early Diagnosis, Prevention The most important step women can take to protect themselves against HPV-related cancers is to have regular, routine visits with their gynecologist. Routine examinations and Pap smears conducted by gynecologists with HPV co-testing have resulted in a dramatic decrease in the incidence of cervical cancers in the United States. Women should be aware of the “contributing factors” that raise their risk of developing HPVrelated cancers. These include smoking, infection with chlamydia or genital herpes, multiple partners (or partners who have had multiple partners, or partners with cervical dysplasia), early onset of sexual activity, etc. People with a suppressed immune system, such as those with

HIV or organ transplant patients, are at greater risk of HPV infections. HPV vaccines were developed to prevent chronic HPV infection. One of the HPV vaccines approved by the Food and Drug Administration and commonly administered now is called Gardasil 9. The vaccine is recommended for boys and girls at age 11 or 12 – before they become sexually active. Young people can receive the vaccine up to age 26, although it’s less effective once a person has been exposed to HPV. “Parents should talk to their child’s pediatrician about getting the HPV vaccine early – well before the onset of sexual activity,” says Dr. Kolomeyevskaya. “Through early prevention, the younger generation has the opportunity to avoid the types of cancers that my colleagues and I treat every day.” It is important to understand that condoms do not fully protect against STDs, including HPV, and adolescents should also be aware that transmission is also possible during oral sex.

Treatment There is no treatment that eradicates HPV, and the highest regression rates are seen in teenagers and young adults with competent immune system. Patients can be treated for HPV- induced cancers and pre-cancerous conditions of the genital tract and oropharynx, as well as genital warts. In the rapidly advancing field of medicine, it is important for patients to receive state-of-the-art care. “Do not hesitate to ask questions, and discuss with your physician current standards of care and guidelines,” Dr. Kolomeyevskaya advises.

Doctor Omar Tamimi joined the practice in 1992. He is Board Certified in Gastroenterology. He is a graduate of Rutgers University and University of St. George’s Medical School. His medical training was completed at Seton Hall University, St. Joseph’s hospital, Patterson, NJ. Since completing his medical training he has performed over 25,000 endoscopic procedures and has vast experience in all aspects of the practice of Gastroenterology. Doctor Tamimi had been a member of the executive committee at Kimball Medical Center since 2003 and served as the Chief of the Medical Staff from 2007-2009. Dr Tamimi is currently Medical Director of the Endoscopy Center of Ocean County. Doctor Tamimi and his wife Patty have three children with whom they are very involved. Doctor Tamimi enjoys outdoor sports, vacationing and spending time with his family and is a big fan of Rutgers sports teams and Bruce Springsteen.

Doctor Edgar Bigornia is Board Certified in Gastroenterology. He is a graduate of Brandeis University and attended Medical School at SUNY Health Science Center at Brooklyn (Downstate) in New York. Dr. Mirchandani has presented half a dozen abstracts at the national gastroenterology meetings, and his work has won two awards at the American College of Gastroenterology. He is the author of a chapter published in “Curbside Consultation of the Pancreas.” Dr. Mirchandani and his wife, neurologist Lakshmi Nadiminti, D.O., are the parents of two young children. Dr. Mirchandani enjoys spending time with his family, listening to music and is an avid sports fan.

What Is Adult Day Care?

The need for alternative care for the aging is essential. 70 million more elderly will be present by 2030 and they will need services for themselves as well as respite for those caring for them. Caregivers are receiving increased attention as a group whose needs are finally being identified. Consumer demand is increasing for flexible and

affordable community-based care as options to institutional care. Adult Day Care is the ideal choice for all inclusive care and affordability during the day. Adult Day Care Centers provide a structured, comprehensive program in a protective setting. Members go in the morning and return home at the end of the day. Besides providing a therapeutic environment for the member in need, Adult Day Care Centers offer family relief from the burden of care giving. It allows those caregivers who work to continue to care for their loved one and provides a much-needed break for ‡ the nonworking caregiver. Financing for such services was fragmented, but has improved with many state and federal programs started to supplement the cost. “Why Did I Get Cancer, Doc?”

This is a question I hear at least once a day. Whether it is Mark, who was diagnosed with leukemia at age 45, or Marge, who was diagnosed with colon cancer at age 80, or Mary, who was diagnosed with breast cancer in her thirties, they all have the same question, ultimately: “Why me? Why did I get cancer? How long did I have it? What could I have done to prevent it?” The answer is not simple. Scientists have been researching for decades to find the answer to this question. In fact, there is no one simple explanation to why one person developed cancer, while another person in the same environment and genetic background may not develop it in his or her lifetime. But scientists have found links between certain things and the development of cancer. It is known that cancer is caused by changes in one’s DNA. DNA is a component of the nuclei in the cells. When the DNA is damaged somehow, it makes the cells multiply in abundant numbers. Usually, abnormal cells that are made in the body are destroyed by the body’s own defense mechanism, a process called ‘apoptosis’ or programmed cell death. When there is a defect in apoptosis, cells accumulate and become tumors. The damage to the DNA can be caused by several factors. There are some changes or mutations that are inherited and are present from birth, which can later make a person susceptible for cancer. Only a small percentage of cancers develop this way. In the majority of cases, certain environmental factors play a major role. Some factors that play a role in the development of cancer are: • Lifestyle factors (nutrition, tobacco use, physical activity, etc.) • Naturally occurring exposures (ultraviolet light, radon gas, infectious agents, etc.) • Medical treatments (chemotherapy, radiation, immune system-suppressing drugs, etc.) • Workplace exposures • Household exposures • Pollution Substances and exposures that can lead to cancer are called carcinogens. Some carcinogens do not affect DNA directly, but lead to cancer in other

ways. For example, they may cause cells to divide at a faster than normal rate, which could increase the chances that DNA changes will occur. Carcinogens do not cause cancer in every case, all the time. Substances labeled as carcinogens may have different levels of cancer-causing potential. Some may cause cancer only after prolonged, high levels of exposure. And for any particular person, the risk of developing cancer depends on many factors, including how they are exposed to a carcinogen, the length and intensity of the exposure, and the person’s genetic makeup. There are several substances that are proven to be carcinogens. Still others are felt to be involved with a high risk for cancer, but their role is not substantially proven. The International Agency for Research on Cancer (IARC) is a part of the World Health Organization (WHO) that works on identifying causes of cancer. IARC classifies potential carcinogens into five different groups, depending on the level of evidence that links each to the cancer. Some of the known Group 1 carcinogens are: alcohol, afflatoxins, benzene, coal tar, hepatitis C virus, radioactive materials, tobacco smoking, etc. A detailed list of the carcinogens is available on the American Cancer Society’s website, cancer.org. Even though exposure to carcinogens increases the chance of developing cancer, a variety of other factors also play a role. While most of it is beyond our control, there are some things which a person can do to reduce their chance of getting cancer. 1. Practice a healthy lifestyle and good dietary habits. Eat more fruits and vegetables and lower the amount of fatty foods. 2. Exercise daily. 3. Avoid obesity. 4. Quit smoking. 5. Avoid excessive alcohol use. 6. Avoid prolonged exposure to sunlight. Use protective sunscreen. 7. Get regular medical check-ups for screening. This includes screening mammogram, PAP smear, colonoscopy, chest CT in heavy smokers, and dermatology evaluation for screening against skin cancer.

Meet The Doctors Of University Urology Associates of New Jersey Mukaram Gazi, MD, FACS Dr. Mukaram Gazi, the founding member of UUANJ, graduated from the University of Miami School of Medicine and completed his post-graduate training in urology at UMDNJ Robert Wood Johnson Medical School (now known as Rutgers RWJ Medical School) in New Brunswick, NJ. A friend of my parents’, Linda, came to me with a problem. Lately, she has been dizzy with partial loss of vision in her right eye. When her doctor examined her, he heard an abnormal sound in her neck (called a “bruit”). His concern was she had carotid artery stenosis and was at risk to have a stroke. Her doctor recommended a carotid ultrasound. Upon hearing this, Linda was very worried and wanted to know more.

Q: What is the carotid artery? What is stenosis? A: We have two carotid arteries located on each side of our neck. It is the pulse we feel when we lightly press on the side of our neck. The carotid arteries carry blood from the heart to the brain. Over time, a waxy substance called plaque can build up in the carotid arteries. Plaque is made up of fat, cholesterol, and blood cells. This plaque can harden and start to calcify. Plaque building up can narrow the inside of the vessel decreasing the amount of blood going to the brain. This is called stenosis and can lead to a stroke. Another fear is that a piece of plaque will become loose and travel with the blood to the brain. This can also lead to a stroke. Q: What causes this plaque build up? A: The mechanism of plaque build up is complex. However factors that accelerate plaque build up and stenosis include high blood pressure, high cholesterol and fat in the blood, smoking, and diabetes Q: What are the symptoms of carotid stenosis? A: Sometimes there are no symptoms and the condition is instead detected during a physical exam. Sometimes carotid stenosis can cause a person to have dizziness or vertigo. Sudden partial loss of vision can be from plaque dislodging from the carotid artery wall and traveling to occlude the blood supply of the eye. In severe cases, a patient can present with small, temporary strokes called transient ischemic attacks (TIA). The danger of TIAs is that they often lead to much larger, potentially debilitating strokes. Q: What is a carotid ultrasound? A: An ultrasound is a machine that uses sound waves to see inside the body. A probe is placed on the body.

Sound waves are transmitted and received by the probe and a computer creates pictures of the body. This works much like sonar on a submarine. Ultrasound is extremely safe, and uses no radiation. The test is inexpensive and it can detect blood flow in real time. The ultrasound can detect plaque and the degree of narrowing inside the blood vessel. It can also detect direction and speed of the blood flow. Treatment can be implemented to decrease the amount of plaque and restore blood flow. Q: How is this test performed? A: All jewelry around the neck is removed. It is recommended for our patients to wear open neck shirts. The patient lies on a table, facing upwards. The ultrasound technologist places the probe on both carotid arteries and takes pictures. A radiologist interprets these images. Q: Are there any other tests that can detect stenosis? A: Sometimes the blood vessels are very tortuous which limits detailed evaluation. Also, the ultrasound cannot see the blood vessels when it enters the skull on its way to the brain. In such cases, a CT scan or MRI can be performed to image the carotid arteries. Q: If I have stenosis, what can I do? A: If the amount of plaque is mild and not causing significant stenosis, medications can be used to reduce the amount of cholesterol and fat in the blood as well as to control blood sugar. Exercise is highly encouraged. Smoking cessation is a must! If the stenosis is critical, a vascular surgeon maybe needed to clear the blocked arteries.

Think about what happens when you continue to resort to your usual pattern. o Going out for a walk. o Taking a hot bath or shower. o Journaling your thoughts. o Having a soothing cup of tea. o Listening to music you enjoy. o Doing some stretches or yoga poses. o Knitting, painting, playing a musical instrument. • Make it easy to take those actions. For example,

keep a journal around, keep your workout clothes / sneakers in plain sight, have a variety of herbal teas available.

• Close your eyes and mentally rehearse using your improved comfort strategies. Notice yourself

taking the positive actions you plan to take. Mental rehearsal is a powerful strategy for making change. Notice the details. • Take steps to lower your overall stress level so that you’re better able to remain on an even keel and can handle life’s bumps; for example:

o Learn to do centering breaths (a.k.a., deep belly breaths or diaphragmatic breaths) o Pay attention to your negative self-talk and challenge it. o Get daily exercise – at least 30 minutes a day. o Practice mindfulness / meditation techniques.

Facts About The Causes Of Varicose Veins Could you be suffering from vein disease without knowing it? One out of three Americans over 45 has some form of vein disease--problems in the network of veins that carry blood back to the heart. Symptoms may seem harmless at first but can develop into serious problems if left untreated. Fortunately, early detection can prevent life threatening consequences. What Causes Varicose Veins? Vein disease occurs when veins have trouble carrying blood from your body back to your heart. The main cause is heredity. You are at higher than average risk if your mother or grandmother -- or even someone on your father’s side -- suffers from serious vein problems. And, due to pregnancy, women are more often affected than men. How does it start? v The first sign of vein disease is often tired, heavy feeling legs. This is a clear indication that the return flow of blood from the legs to the heart is impaired. If these early complaints are not taken seriously, it’s not long before the discomfort may be accompanied by swelling of ankles and lower legs--a sign that the blood has become congested in the leg veins. You may find that the problem is more pronounced after a day of prolonged sitting or standing. That’s because the leg veins damage easily and may become enlarged by the pressure of the pooled area. As the vein walls become weaker, they may start to leak fluid into the surrounding tissues, and the swelling increases. When you lie down for a while the swelling may go down. But, chances are, it will return the next day. Are there serious consequences? Yes. In addition to leg pain and unsightly varicose veins, untreated vein disease could lead to thrombosis, or blood clots. Thrombosis occurs when a

blood clot forms due to chronic venous blood congestion. If the clot travels from the leg to the lungs, a life-threatening pulmonary embolism could occur. Untreated vein disease can also lead to discoloration of the skin on your lower legs and possible leg ulcers, which are very hard to heal. Are there any treatments? Yes. Early detection and consultation with your doctor are the first steps to better leg health. Under your doctor’s care, you can explore treatment alternatives such as prescription compression stockings, which help support weakened veins and may enable you to lead a pain-free, active life. It is important, however, to maintain a doctor-prescribed program of wearing medical stockings, or your condition could worsen over time.

Colorectal Cancer: Are You Aware? What’s The Big Hype? Nina Paonessa, DO, FACOS, an established colorectal surgeon practicing in Ocean and Monmouth Counties, answers questions about colorectal cancer, one of the leading cancers in both men and women.

sedation. Based upon the findings on colonoscopy, your colorectal surgeon or other specialist may recommend other testing such as blood work, a CT scan of the abdomen and pelvis, pelvic MRI and/or ultrasound.

How common is colorectal cancer and who is at risk? Colorectal cancer is the second most common cancer in the United States among both men and women. It affects 140,000 people annually and causes 50,000 deaths. The average person has a one in 20 chance of developing colorectal cancer in his/her lifetime. This risk is increased if there is a family history of colorectal polyps or cancer, and is even higher still in those patients with a history of breast, uterine or ovarian cancer, and patients with a history of extensive inflammatory bowel disease such as ulcerative colitis or Crohn’s colitis. Additionally, a person’s risk for developing colorectal cancer increases significantly with age.

How is colorectal cancer treated? Nearly all cases of colorectal cancer require surgery. Radiation and chemotherapy are sometimes required in addition to surgery. If the cancer is diagnosed and treated in the earliest stages, approximately 80-90% of patients are restored to their normal health. The cure rate decreases to 50% or less when the cancer is diagnosed in the later/advanced stages. Less than 5% of all colorectal cancer patients require a colostomy or “bag,” thanks to modern technology. Dr. Paonessa prides herself in being able to maintain this standard of care in her own practice. Not only is she able to minimize the number of colostomies/”bags” created for her patients, but she performs a majority of her colorectal cancer surgeries using laparoscopic surgery. Laparoscopic colorectal surgery is a minimally invasive surgery in which small incisions are made to perform major surgery. This allows for shorter hospital stays, decreased incisional pain and earlier return to work and other activities.

What causes colorectal cancer? It is generally agreed that nearly all colon and rectal cancer begins in benign polyps, called adenomas. These pre-malignant polyps, or growths, occur in the wall of the colon and/or rectum and may eventually increase in size and become cancer, or medically speaking, carcinoma. This is known as the “adenomacarcinoma sequence.” Several factors have been mentioned in the literature that may increase a person’s risk for developing colorectal cancer and include: age, a diet high in fat and cholesterol, an inactive/sedentary lifestyle, obesity, smoking and environmental exposures. What are the symptoms of colorectal cancer? Unfortunately, polyps and early cancers do not produce any physical signs or symptoms. Therefore, symptoms usually indicate late or advanced disease. The most common symptoms of colorectal cancer are rectal bleeding and changes in bowel habits such as constipation or diarrhea. These symptoms are common in many other disorders of the gastrointestinal tract so it is very important to have a thorough examination should you experience them. NOT ALL RECTAL BLEEDING IS DUE TO HEMORRHOIDS. It is very important for patients to remember this and seek evaluation if they are experiencing rectal bleeding. Other symptoms such as abdominal pain and weight loss are usually late symptoms and quite possibly indicate advanced/ extensive disease. How is colorectal cancer diagnosed? Evaluation of colorectal cancer is first begun with a complete history and physical exam, including a digital rectal exam and possibly testing the stool for blood. A colonoscopy is the most definitive test used to diagnose colorectal cancer. A colonoscopy is performed by a colorectal surgeon or other specialist in order to get a direct visualization of the inside of the colon and rectum. A colonoscopy is a safe, painless, outpatient procedure performed under a light Board-certified in general surgery and proctology and a fellow of the American College of Osteopathic Surgeons, Dr. Paonessa specializes in colon and rectal surgery. She believes in patient-centered care and preserving each patient’s independence, well being and dignity.

Can colorectal cancer be prevented? Colorectal cancer is one of the few cancers that is preventable. Obtaining a screening colonoscopy at regular intervals is the single best way to prevent colorectal cancer. Your colorectal surgeon or specialist is able to review the guidelines for screening colonoscopy based upon your age, medical history and family history. Although not definitively proven, studies published in the literature mention additional factors that may lower your risk of developing colorectal cancer and include: eating a high-fiber, low-fat diet, maintaining a healthy weight, and engaging in regular, vigorous exercise.

When Does Forgetting Become An Issue? Is it memory loss or am I just being forgetful? Did I just lose my keys or will I never remember where I put them? Is it OK that I can’t remember how to do basic chores because I’m “getting older”? The first answer brings good news because you are asking these important questions, not someone on your behalf, and, even more important:

When your memory has significantly declined, is poor compared to performance typical for your age but DOES NOT interfere with independent, day-to-day functions, you can be described as having Mild Cognitive Impairment or MCI. Cardiac Breakthrough Slipping Away At The Jersey Shore… The Jersey Shore has suffered some great losses due to Super Storm Sandy and Nor’easter’s and now, another great loss seems inevitable. Whether it be the ears or the tail, that first bite of the Chocolate Bunny is simply the best but have you ever thought about how that bunny came to be? Each year shortly after Valentine’s Day we begin the Easter Bunny production at Chocolate Carousel. Our bunnies are not mass produced, but rather made by hand the old-fashioned way. It’s a daunting process because there are so many styles to make; large and small, sitting or standing, hollow and solid. Of course each is made in milk chocolate, dark chocolate and white chocolate. About midway through the season I start dreaming about Easter Bunnies and that’s the moment I realize it’s really Easter again. Nonetheless, the making of the chocolate Easter bunny is a very significant part of our year at Chocolate Carousel. Our bunnies are part of our family and each has its own name and personality. We name our bunnies after members of our staff, but only after they have worked with us for two consecutive Easters. At that point the staff member earns a bunny, picks a style to carry their name and so a new bunny is created. The fact is that the life of a chocolate Easter bunny is not an easy one. They are born from a vat of carefully tempered chocolate which has been slowly melted and stirred until it is a smooth creamy consistency. Then the chocolate is carefully poured into rows and rows of shiny plastic molds that are hand clipped together to form a three dimensional form. Once the molds are clipped shut they must be slowly rotated by hand until the chocolate evenly covers all the mold’s surfaces.

These newly conceived bunnies are then tapped, jiggled, shaken, spun around and tilted to remove all air bubbles and to evenly disburse the chocolate before they are placed into the refrigerator to set. Once in the chill they need to be spun about every 30 seconds to ensure that the chocolate sets evenly creating a perfectly hollow bunny. Once the bunnies are carefully removed from their molds they are born. Upon their birth, they are trimmed of excess chocolate and each receives a set of sugar eyes and a bow carefully hand placed by a decorator. They are then placed into shiny clear bags tied with ribbon and ready to be presented to our customers for their Easter baskets. The bunny’s fate is sealed. They are too good to resist and will surely be enjoyed on Easter Sunday. So stop in and meet our bunnies, whether it be Henry, our most popular style sitting proudly on the shelf or Margie, our largest bunny tipping the scales at over 8 lbs, our bunnies are always fresh, delicious and worthy of their reputation as the most important element of every Easter basket.

March Is Red Cross Month The American Red Cross sets aside the month of March as Red Cross Month, a time to celebrate the every day heroes who help their neighbors when they need it most. These heroes are the Red Cross volunteers, blood donors and financial contributors who make the work of the Red Cross possible. On Saturday, March 18, the Red Cross will recognize the incredible impact three local honorees have made in the community during its annual Red, White and Bling gala. Honorees include the Bonfiglio family of Tommy’s Tavern and Tap and Allyson Gilbert, both of whom are recipients of the Clara Barton Community Humanitarian Award, and HealthSouth Rehabilitation Hospitals of Tinton Falls and Toms River, recipient of the Clara Barton Corporate Humanitarian Award.

Penelope Rochelle, DVM, CVA, CVMST, Thrive Veterinary Wellness Center Dr. Rochelle graduated from Ohio State University in 2006. She was a local equine ambulatory veterinarian for four years, and then worked at a mixed animal practice in Maine for two years. During that time, she decided to take the acupuncture course at the Chi Institute, thinking she would only use the principles to treat back pain in horses. However, this course opened her mind in new ways of healing, and left her wanting more. She earned her certification in spinal manipulation from the Healing Oasis in 2013 and has taken advanced courses in functional neurology since that time. She is passionate about her patients and their wellness. Dr. Rochelle grew up in Monmouth Beach. She lives in Red Bank with her husband, daughter and newborn son, along with their two rescue dogs. Nina Paonessa, D.O, FACOS, Paonessa Colon & Rectal Surgery P.C. Dr. Paonessa was born and raised in Albany, NY. She was raised to value a strong work ethic and to take pride in her work. Two principles she grew up on were “if you are going to do something, do it right or don’t do it at all” and “do unto others…” This is how she lives her life and how she runs her practice. Dr. Paonessa knew from a very young age that she wanted to become a surgeon. She volunteered as a candy striper in high school and then as an Emergency Medical Technician in college. She treats all dermatologic issues, including malignant and benign conditions. She became interested in this specialty when her daughter was diagnosed with vitiligo when she was only seven years old. The White House In Oakhurst Presents

For The Love Of Tea, An Exhibition Of Tea And Art

By bringing together the talent of several up and coming artists, The White House in Oakhurst plans an exhibit to celebrate art, love, passion, and tea.

For the Love of Tea, opened on February 15 at Gallery 45 at The White House in Oakhurst, is a special exhibit showcasing the artistic passions and talents of Ruby Silvious, Mil Wexler Kobrinski, and Kirsten Kristensen. Inspired by the tea bag paintings of Ruby Silvious, the exhibit features an array of her work alongside a collection of ceramic tea pots and sets by New Jersey native Mil Wexler Kobrinski. Ruby Silvious, a mixed media artist and graphic designer from Hudson Valley, New York, recently published a book titled 363 Days of Tea. Following her journey to create art with one reused bag of tea every day, the book chronicles her life mug by mug, and day by day. After experiencing Ruby’s artwork firsthand at the Atlantic Gallery in Chelsea, White House owner Kirsten Kristensen felt moved to showcase Ruby’s work. Kirsten believed Ruby’s passion, and the underlying environmental theme of repurposing used tea bags as art, would be a perfect fit for The White House. Exhibiting with Ruby will be local artist Mil Wexler Kobrinski. A passionate philanthropist, Mil has been a fixture of the Monmouth County art community and served as a mentor to many local artists. Influenced by her studies of history, anthropology, and philosophy, Mil’s ceramic tea pots and tea sets are all handmade and display a wide variety of styles and artistic technique. Though owner Kirsten Kristensen’s art takes the form of fresh expertly brewed tea, on display during the exhibit will be her collection of antique and historic tea tins. Like the many unique and special teas available at The White House, For the Love of Tea promises to be the perfect blend of culture, community, and history. And, of course, tea.

March Is National Women’s History Month Submitted by Rita King, MSW, LCSW t is the most wonderful time of the year! March is National Women’s History Month and the theme for 2017 is “Honoring Trailblazing Women in Labor and Business.” This year, 13 amazing women are being recognized through the National Women’s History Project. Each and every woman has contributed to the greater good of women in business and has worked hard to increase opportunities for women in the work force. The honorees begin with Rebecca Anderson, Community and Economic Development Organizer. Rebecca is known for being the founding director of the “Hand Made in America” organization. This organization was comprised of what was known as a craft-focused economy. Barbara Hackman Franklin, Former Secretary of Commerce. Barabara has an extensive background in politics. Barbara’s service began in 1971 as an assistant to President Richard Nixon. She would go on to serve four additional Presidents in this position. Barbara is well known for her work in recruiting women for “high-level” government jobs. Alexis Herman, Former Secretary of Labor. Alexis began her work in the social service field. After Jimmy Carter became President of the United States,

Alexis was appointed as the youngest director of the Women’s Bureau. It is reported that Alexis was also the first African American female to serve as an assistant to the President. Alexis was also noted as being the first African American Secretary of Labor and this was out of men and women. Lilly Ledbetter, Equal Pay Activist. Lilly was the key plaintiff in the landmark case, Ledbetter v. Goodyear Tire & Rubber Co. This was a discrimination case in which Lilly had been paid significantly less than her male co-workers in the same position. Lilly prevailed in the Superior Court; however, lost on appeal to the Supreme Court. President Barack Obama signed the Lilly Ledbetter Fair Pay Act into law on January 29, 2009. Lilly is retired from her position with Goodyear. However, she continues to advocate on behalf of women in the workplace. Kate Mullany, Organized First All-Female Labor Union. In 1864 Kate organized the first female Collar Laundry Union. Kate also organized a strike for an increase in wages and safer working conditions, where, for five days workers walked off the job and demanded more benefits. The strike was a success and the women ware provided an increase in their salaries. Lucy Gonzalez Parsons, Labor Organizer and Socialist Leader. Lucy was known as a labor organizer, radical socialist and anarchist. Lucy was the founder of the Industrial Workers of the World. A unique

quality about Lucy is that she was Native American, African American and Mexican. Lucy was known for her activism against discrimination in the workforce as well as fighting for the rights of people of color, the homeless and women. Lucy was even arrested on more than one occasion for her distribution of anarchist materials. Barbara “Dusty” Roads, Flight Attendant’s Union Leader. Barbara was a former flight attendant who fought against discrimination in the workforce on behalf of age limits with flight attendants. Believe it or not a woman could “age out” at 32 back in the 1950s. Thanks to the trailblazing efforts of Barbara that is no longer the case. Andra Rush, Founder/CEO, Rush Group. Andra operates Dakkota Integrated Systems, Detroit Manufacturing Systems and Rush Trucking Corporation. These companies are known to be one of the largest Native American companies. Andra started a trucking company with a loan from her parents and her savings. There was a van and two pick-up trucks. Today, there are over 1,100 trucks on the road delivering goods across the United States.

2017 Theme Is Honoring Trailblazing Women In Labor And Business Nina Vaca, CEO and Chairman of Pinnacle Group. The Pinnacle Group is an IT company that started off small and grew significantly under Nina’s leadership. Nina, a Latino American is one of the few to have the honor of serving as a director on a publicly traded company. Nina is also known for her advocacy for women and girls in the STEM field. Maggie Lena Walker, Businesswoman and Community Banking Leader. Maggie is known as the first African American woman in the United States to charter a bank. During Maggie’s youth, she was a member of an organization that focused on humanitarian causes. Yvonne Walker, President, Service Employees International Union Local 1000. Yvonne was a former Marine. Working as a legal secretary Yvonne became involved in the union and later became the first African American woman to serve as president of the Service Employees International Union Local 1000. Yvonne has 95,000 California workers with whom she has gained the trust and respect to advocate on their behalf as a result of her ability to problem solve and work toward the benefits of the workers in the union.

Addie L. Wyatt, Labor Union Leader and Civil Rights Activist. Addie is one of eight children. Addie was married at the young age of 16. When she applied for a job as a typist, she faced discrimination and was offered a job in the canning department. Addie rebounded and later became the first African American woman elected to office in Local 56 of the United Packinghouse Workers of America. Addie participated in the infamous March on Washington with Martin Luther King, Jr. as well as the March from Selma to Montgomery, Alabama. Addie was also appointed by none other than the great Eleanor Roosevelt to the United States Commission on the Status of Women and was also a founding member of the National Organization for Women. Norma Yeager, First Woman Stockbroker to be Permitted on the Floor. Norma was a stockbroker and fought to end the discrimination against women on the trading floor. Norma who began a traditional life as a wife and “homemaker,” enrolled in the prestigious Hornblower & Weeks, Inc. stockbroker training program and became the first female to graduate the program. Norma took down the wall that did not

permit women on the same stock trading floor as men. She later owned and operated her own brokerage firm for almost 20 years before selling it. Norma has written a book about her life entitled Breaking Down the Walls and also has a movie deal. Each and every one of these women featured exhibited a tremendous amount of courage in the face of adversity. They each have amazing histories and many have been inducted into various “Halls of Fame.” They have displayed resilience and strength in their passion to keep moving forward in labor and business. Rita King, MSW, LCSW is a full-time mental health therapist and advocate of women’s history. During her undergraduate studies in psychology, Rita developed a strong passion for women’s history after completing classes in political science. As an activist, Rita has raised awareness of the importance of women’s history through lectures, history lessons in elementary and middle schools and creating women’s history displays at local libraries. Rita also utilizes her knowledge of women’s history to empower women and young girls in her current role as a mental health therapist.
