Oh, My Aching Back… Or Is It My Hip? Written by Fabio Orozco, MD, Board Certified Orthopaedic Surgeon

Aging has become synonymous with pain from time to time. Many of us have had that bad night’s sleep or heavy yardwork that resulted in muscle stiffness and an aching back the next morning. This type of pain usually is relieved with rest, ice, and over-the-counter pain medications. But what if this pain persists and does not improve? Sometimes it may be hard to determine if the pain we are feeling is coming from our back, or our hips, or both. Your doctor can help you distinguish your pain through a physical examination and diagnostic testing. Back and hip pain can become confusing since they are in close proximity to each other. It is also common to have back and hip pain concurrently. Back pain can take the form of radiating pain, or radiculopathy, which is pain that comes off the nerve roots of the spinal column and travels down the neural pathways to the lower extremities, causing numbness, tingling, burning, electrical type of sensations. The most affected nerve that many people are familiar with is the sciatic nerve. When the sciatic nerve is aggravated, pain can be felt deep within the glutes, down the back of the leg and to the soles of the feet. Herniated or bulging discs of the vertebral column are the usual cause of radiculopathy pain. Radiating back pain can cause pain in the hip, but there are ways that can help differentiate the two. Pain in the hip that is related to nerves is usually relieved with walking or moving, it radiates down the leg, and typically does not make simple tasks like tying your shoes challenging. Lying on your back or driving can be quite painful for some. Hip pain that is associated with arthritis of the hip is in the groin. This is where the lower extremities internally and externally rotate from your pelvis. Severe hip osteoarthritis does not improve with walking. Position changes can be difficult, and pain is felt in the groin with rotating the hip joint. People may complain of feeling like their hip is ‘stuck,’ and have difficulty tying their shoes, getting in and out of a car, or crossing their legs. An evaluation of your symptoms begins with a consultation and x-rays. On x-ray your physician can determine if you are experiencing joint space narrowing of the hip joint or compression and curvatures of the lower spine. A physical exam can determine if there is pain with hip joint rotation, pain with walking, position changes, and a description of the subjective findings, such as type of pain you are experiencing, catching, pulling, burning, aching, or numbness. During your consultation you would be asked what exacerbates your pain and what improves it. This would include activity modifications such as weight reduction, use of a

cane or walker, if you have tried pain relievers, exercises, heat and/or ice, and how much your discomfort is affecting your quality of life. After your exam, your physician can conclude if you are suffering from lumbar spine etiology or hip osteoarthritis and determine the best course of treatment. Your treatment for both hip osteoarthritis and lumbar radiculopathy can include further diagnostic testing such as an MRI, physical therapy, home exercises, injections, and medications. However, in severe cases of hip osteoarthritis, surgery may be necessary to help alleviate your symptoms when conservative measures have been exhausted. Severe bone-on-bone hip osteoarthritis is treated surgically with a total hip replacement or arthroplasty. This surgery can be performed using a less invasive direct anterior approach, where your hip is accessed from the front instead of a posterior approach that requires the muscles of the buttocks to be cut. This approach results in a faster recovery, eliminates the need for mobility restrictions, and results in fewer complications and less pain. Hip replacement surgery can be performed at an outpatient surgery center or hospital setting after appropriate medical clearances determine which facility would be best to support a patient’s medical and social needs. If you have pain that has not improved with conservative treatment, it may be time to see your doctor to discuss symptoms and treatment options. Aging does not mean you have to suffer with pain and discomfort. Give us a call today.

Spring Has Sprung

It’s hard to believe that it was just over one year ago that the world as we know it changed forever. Although the physical (and mental) effects of Covid-19 are daunting, we can still find the silver lining. One thing the virus has shown us is that we were all guilty of taking the “little” things for granted. Whether that means the ability to visit grandparents, or sending your kids to school, or attending a concert, we will never look at those things the same way again. After the long months of isolation and quarantining, spring 2021 is a welcome distraction. Between the longer days and mild weather, it’s hard to feel anything but hopeful. In fact, studies show that taking your workout outside can improve your mental and physical well being. Whether you indulge in a post dinner walk or a recreation adult sports league, the warmer temperatures will allow you to enjoy the time outside. Spring is the ideal time of year to enjoy simple pleasures. The flowers are blooming, the sun is shining, and we can enjoy the benefit of extra daylight. “Springing forward” has benefits for all. There is much to look forward too on the horizon. Besides the promise of time spent outdoors, St. Patrick’s Day is just around the corner on March 17th and Easter follows in April. Did you know that the reason we wear green on St. Cosmetic & General Dentistry Patrick’s Day is that it makes you invisible to leprechauns (which like to pinch Linwood Family Dentistry.............................................. 2 anyone they can see)? Breastfeeding: “A True Gift” The topic for this issue is breastfeeding. This is a true gift! It is a gift that a woman has the ability to feed her baby. It is also a gift of love from Mom to her newborn baby. This ability to sustain life is a special thing. During pregnancy, a woman’s body begins to prepare for hypertrophy, meaning they get enlarge and may become darker glands (milk ducts) that are contained in the areola that also Nature prepares a pregnant woman for the birth and feeding of her baby. During labor and following delivery, certain hormones are produced that cause the body properties that help to protect the newborn until his or her own immune system is will become more opaque and white to yellow in color. Skin Heath and Happiness… By Lisa Ledden

As a skin care specialist, I see many patients with a high level of happiness even during these challenging times. I love this specialty because not only do I enjoy my work, but I make a difference in people’s lives. Also patients are generally happy with the outcome of their clinic visit. Studies support that patient happiness is determined by many factors that include access, communication, the quality of medical care, continuity, and demeanor of the provider and staff. Our mission is to hit the mark on all of the above!

What can you do to have happy, healthy skin? 1. Bathe in warm, not hot, water. Use mild cleansers (for example, Dove unscented cleanser). 2. Moisturize following the shower while the skin is still somewhat wet. 3. Block harmful UV waves. Use sunscreen, avoid intense sun exposure, and wear protective clothing. 4. Reduce stress. 5. Do not use tanning beds or sunlamps. 6. Get plenty of sleep. 7. Schedule annual skin examinations.

Colorectal Cancer: Are You Aware? What’s The Big Hype? Nina Paonessa, DO, FACOS

Nina Paonessa, DO, FACOS, an established colorectal surgeon practicing in Ocean and Monmouth Counties, answers questions about colorectal cancer, one of the leading cancers in both men and women.

How common is colorectal cancer and who is at risk? Colorectal cancer is the second most common cancer in the United States among both men and women. It affects 140,000 people annually and causes 50,000 deaths. The average person has a one in 20 chance of developing colorectal cancer in his/her lifetime. This risk is increased if there is a family history of colorectal polyps or cancer, and is even higher still in those patients with a history of breast, uterine or ovarian cancer, and patients with a history of extensive inflammatory bowel disease such as ulcerative colitis or Crohn’s colitis. Additionally, a person’s risk for developing colorectal cancer increases significantly with age.

What causes colorectal cancer?

How is colorectal cancer treated? Nearly all cases of colorectal cancer require surgery. Radiation and chemotherapy are sometimes required in addition to surgery. If the cancer is diagnosed and treated in the earliest stages, approximately 80-90% of patients are restored to their normal health. The cure rate decreases to 50% or less when the cancer is diagnosed in the later/advanced stages. Less than 5% of all colorectal cancer patients require a colostomy or “bag,” thanks to modern technology. Additionally, modern technology has made possible minimally invasive colorectal surgery using laparoscopic or robotic surgery techniques. Both laparoscopic and robotic colorectal surgery are types of minimally invasive surgery in which small incisions are made to perform major surgery. This allows for shorter hospital stays, decreased incisional pain and earlier return to work and other activities.

Can colorectal cancer be prevented? Colorectal cancer is one of the few cancers that is preventable. Obtaining a screening colonoscopy at regular intervals is the single best way to prevent colorectal cancer. Your colorectal surgeon or specialist is able to review the guidelines for screening colonoscopy based upon your age, medical history and family history. Although not definitively proven, studies published in the literature mention additional factors that may lower your risk of developing colorectal cancer and include eating a high-fiber, low-fat diet, maintaining a healthy weight, and engaging in regular, vigorous exercise.

It is generally agreed that nearly all colon and rectal cancer begins in benign polyps, called adenomas. These pre-malignant polyps, or growths, occur in the wall of the colon and/or rectum and may eventually increase in size and become cancer, or medically speaking, carcinoma. This is known as the “adenomacarcinoma sequence.” Several factors have been mentioned in the literature that may increase a person’s risk for developing colorectal cancer, including age, a diet high in fat and cholesterol, an inactive/sedentary lifestyle, obesity, smoking and environmental exposures.

What are the symptoms of colorectal cancer? Unfortunately, polyps and early cancers do not produce any physical signs or symptoms. Therefore, symptoms usually indicate late or advanced disease. The most common symptoms of colorectal cancer are rectal bleeding and changes in bowel habits such as constipation or diarrhea. These symptoms are common in many other disorders of the gastrointestinal tract, so it is very important to have a thorough examination should you experience them. NOT ALL RECTAL BLEEDING IS DUE TO HEMORRHOIDS. It is very important for patients to remember this and seek evaluation if they are experiencing rectal bleeding. Other symptoms such as abdominal pain and weight loss are usually late symptoms and quite possibly indicate advanced/extensive disease.

How is colorectal cancer diagnosed? Evaluation of colorectal cancer is first begun with a complete history and physical exam, including a digital rectal exam and possibly testing the stool for blood. A colonoscopy is the most definitive test used to diagnose colorectal cancer. A colonoscopy is performed by a colorectal surgeon or other specialist in order to get a direct visualization of the inside of the colon and rectum. A colonoscopy is a safe, painless, outpatient procedure performed under a light sedation. Based upon the findings on colonoscopy, your colorectal surgeon or other specialist may recommend other testing such as blood work, a CT scan of the abdomen and pelvis, pelvic MRI and/or ultrasound.

The Hansen Foundation Written by Lisa Whitley, PMP – Executive Director There is no shortage of stories when it comes to the challenges of a person in the throes of addiction. While hitting rock bottom often serves as a familiar turning point, the story is far from over, and the road to recovery remains a difficult one. Physical struggles are compounded by emotional ones, and without the right resources and support systems in place, the chance for a full recovery often remains out of reach. In this, the first of a six-part series, we will share the valuable work of The Hansen Foundation (a non-profit organization dedicated to providing exceptional care and support for those recovering from addiction), and the commitment to our mission: To Transform Humanity, One Person at a Time, One Day at a Time. The Hansen Foundation Residences, Serenity Estates and Serenity Houses, provide an affordable, long-term, home-like atmosphere and necessary recovery support. Your Painful Heel Could Mean A Visit To The Podiatrist

Pain on the bottom of your foot, whether sharp or dull, that lingers for days or weeks deserves a visit to the podiatrist. Heel pain can interfere with your ability to do the activities you love and become chronic, making simple tasks such as standing or walking uncomfortable or impossible. At Go Feet, the podiatry team can evaluate your heel pain and offer relief. They can also help determine if the heel pain is a symptom of a structural problem of the feet or ankles. Don’t just tolerate heel pain – in most cases, heel pain can be treated with conservative measures. A visit to the podiatrist may help you resolve it before it becomes chronic and debilitating.

Symptoms Of Heel Pain

You should see a podiatrist if you have a sharp pain at the bottom of your heel. It might be most intense when you first wake up in the morning or when you stand up from a chair after a period of inactivity. The pain feels like a knife stabbing your heel at first, then becomes a dull ache over the course of the day. If you’re active – as in running or playing

sports – the pain may fade during your activity, only to return with a vengeance later in the day or the next morning.

Causes Of Heel Pain

A common cause of heel pain is plantar fasciitis, a strain of the ligament that supports your arch. Plantar fasciitis often develops due to overuse, such as lots of running or wearing shoes that are unsupportive or too tight. Flipflops, high heels, and worn-out tennis shoes are often to blame. A heel spur, which is a growth of bone on the front part of the heel bone, is another common cause of heel pain. A spur forms when the plantar tendon pulls away from its attachment to the bone and a calcified spur forms. Causes include wearing worn-out or poorly fitting shoes, carrying excess weight, running on hard surfaces, or gait abnormalities. Heel pain can also develop due to warts or cuts or cracks that just don’t heal.

Help From A Podiatrist

At Go Feet, the podiatrists can evaluate your heel pain to find the cause. If you have an underlying problem such as a flat foot or a bunion, heel pain may just be a secondary symptom caused by the irregular gait these structural problems cause. A podiatrist can treat bunions or flat feet, and your heel pain disappears along with the primary structural issue. If you have plantar fasciitis, going for a long time without treatment only prolongs your pain – it takes longer to heal the longer you allow it to fester. The Go Feet podiatrists can advise you how to amend your activity temporarily to reduce symptoms and let your heels rest. They can also help you find shoes that fit properly, so you don’t aggravate heel pain. In some cases, you might benefit from orthotics or other custom inserts. Podiatrists can also remove warts or help heal abrasions, blisters, cracks, and sores that aren’t healing because of a compromised immune system. Heel pain rarely requires surgery. Call the podiatry team at Go Feet if you’re experiencing heel pain to get the issue resolved as quickly as possible.

Don’t go another day.

The Magnificent “Wordologist” Amanda Gorman

Theguardian.com

As I sit down to write the next feature, I am reflective over the past year, January 6, 2021 and most recently the inauguration of our nation’s President. Regardless of your political views, the inaugural poem read by Amanda Gorman was an incredibly moving piece. As I logged into my computer at work to watch the inauguration, I was just in time to view the 22-year-old Harvard graduate’s presentation of hope and peace to the world. The ability to put into words the current climate of the world with such elegance and power leaves a person, well, speechless.

I was so enamored by Amanda Gorman I had to learn more about her. I watched her speech about 10 times and then as many YouTube interviews I could find. Amanda has a twin sister and another sibling. Amanda was raised by a single mother in California. Amanda was interested in writing at a young age due to her limitations with her speech. Amanda’s writing became her voice. Amanda began performing her written work in the second grade. Amanda’s interest in literature continued to grow, and it has been reported that a speech by Nobel Prize laureate Malala Yousafzai inspired Amanda to become a youth delegate for the United Nations in 2013. By 2014, Amanda received the award of Youth Poet Laureate of Los Angeles. Amanda used her words to advocate for social change and the underserved. In the same year, Amanda “founded” “One Pen One Page,” which is known to be a non-profit organization that “encourages youth advocacy, leadership development and poetry workshops.” All of this greatness before college! By 2015 she was published. At the young age of 19, Amanda was awarded the United States first female African American National Youth Poet Laureate award. Amanda credits former First Lady Michelle Obama for her work in the national award. First Lady Jill Biden was moved by Amanda’s writings after observing Amanda recite her famous poem “In this Place” at the Library of Congress in Washington, DC. Amanda has contagious energy and is incredibly positive. I particularly enjoyed her presentation of a poem that was created from her experience during an audition for The Lion King on Broadway. Amanda Gorman described real-life interactions with her mother and sister that are Poets.org. hilarious, horrifying and a true portrayal of family life. Amanda reflected feeling “broken” at not getting her big break for a part in The Lion King and yet she ends up being selected to write the inaugural poem with few restrictions. Amanda very openly shares that she grew up with a speech impediment and, even when she was a finalist for the inauguration presentation, had her doubts of being selected to write and deliver “The Hill We Climb.” A poem that was started conservatively over the course of a few weeks and then, after the event of January 6, 2021, Amanda seized the opportunity to be the voice of peace. Amanda Gorman has already made an incredibly significant impact on the world and specifically young girls. Amanda has overcome obstacles that have included a Latimes.com

speech impediment, poverty, single-parent households, and the pressures of an Ivy League education. In an interview with James Corden (January 21, 2021), Amanda shared the events that happen after an inauguration. As any young adult would do after getting a good grade, scoring the winning goal or making their bed, Amanda’s reaction and mindset was “maybe she could get a new iPhone.” A young individual so talented, it is quite humbling to see that yes, she is real! Amanda is very self-aware as she described herself as “a skinny black girl, descendant from slaves and raised by a single black mother can dream of becoming a president.” On inauguration day, Amanda Gorman stood before the nation poised, strong, articulate, and delivered a message that will continue to resonate with us for decades. Amanda Gorman has made it known to America that Instagram.com she has a dream to be on the ballot in the 2036 Presidential election. Amanda is not stopping at the inauguration. Amanda recently debuted a children’s book entitled Change Sings. The book is about how a young girl has the power to change through music. Amanda shared in an interview with James Corden that she wanted to write a book in which young girls are activists and not observers in the story. Additionally, it has been reported that Amanda was offered a modeling contract with IMG Models, and by the time of this publication she will have performed before Super Bowl 2021 to honor three pandemic heroes. There is no doubt that Amanda will continue to shine bright through her powerful prose. The historic events of the world over the last year have dimmed the lights for many. I appreciated Amanda’s description of how we have “braved the belly of the beast.” A message quite fitting on many levels in each of our lives. In “The Hill We Climb,” Amanda brings back to focus that we are amazingly resilient wherein she reminds us that “even as we hurt we hope” and we will “rebuild, reconcile and recover.” Amanda gave us more than a simple poem on inauguration day; she gave us inspiration to live each day to its fullest. I encourage everyone to get Messynessychic.com out there and live Happy!

State Mandated Vaccines Several years ago, the State of New Jersey issues a regulation that all eleven-year-old has to be protected against two bacteria, pertussis and Neisseria Meningitidis. Pertussis causes whopping cough. Neisseria Meningitidis causes lethal meningitis. These are diseases you want protection against. The Adacel vaccine protects against pertussis, diphtheria and tetanus. The Menactra vaccine protects against Neisseria Meningitidis. • Neisseria Meningitidis is a relatively rare bacteria, but, if someone does contact the infection, she/he is very sick. Despite the best medical care possible, the fatality rate is ten percent. And even if one survives the infection, he/she has a 30 percent chance of being crippled for life. We’re talking loss of an eye, an arm, or hearing-that sort of crippled. • Pertussis is a much more common infection than Neisseria. And to add to the problem, human beings are the only known host for pertussis. The only way to protect yourself is to be vaccinated. I recommend all adults get the Adacel vaccine. • Whooping cough is an obnoxious beastie. If you contract it, you do not acquire permanent immunity. You have about ten years of protection, and then you can get it again. Four of our families have come down with pertussis in the past few years. The members of each family were coughing for two months. The Chinese call pertussis the cough of one hundred days. People break ribs this way. And it is imperative that new parents get the Adacel vaccine. The incidence of pertussis has increased from 3,000 cases annually in the 1970s to 25,000 cases in the 2010s. And the fatality rate has almost tripled. Those succumbing to pertussis are babies under six months of age. The babies are getting pertussis from their parents and caregivers. Other Important Vaccines • Gardasil. Another vaccine that is important for adolescents to get is Gardasil. This vaccine protects against cancers caused by the Human Papilomavirus. Originally, the vaccine was created to protect women from cervical cancer. Further research has demonstrated that the Human Papilomavirus causes other cancers in both men and women. The virus causes penile cancer in men, and mouth, tongue, and anal cancer in men and women. There are over 33,000 new cases of Human Pipilomarivus cancers every year. • Flu Shot. Influenza is an annual problem. As of today, we do not have any information the severity of this year’s flu. Last year’s flu season was mild. The good news is that the pediatric fatality rate from the flu has been dropping. Evryone six months of age and over should get a flu shot, as long as the individual is not allergic to the vaccine. It is particularly important for pregnant moms to get the shot. This will protect the baby because the mom’s antibiotics will cross the placenta and protect the baby. After birth, babies under six months of age cannot get a flu shot. And no, the injectable influenza vaccine cannot give you the flu. I urge all parents to give their children the protection those vaccines provide, and the long life the vaccines help ensure.

Spring Cleaning

“Have nothing in your home that you do not know to be useful or believe to be beautiful.” — William Morris The winter is finally over and with spring comes new fragrances and pollens outside. To fully embrace the new season, some decide to clean their homes by moving everything and doing a full clean and assessment of what should be kept or thrown out. We collect treasures over our lifetimes that represent memories and letting go of those treasures can be hard. An article from the Journal of Gerontology: Series B, titled “The Material Convoy after 50” discusses how people divest themselves of possessions as they age. David J. Ekerdt and Lindsey A. Baker through analysis of data from a national panel conclude that “After age 50, people are progressively less likely to divest themselves of belongings. After age 70, about 30% of persons say that they have done nothing in the past year to clean out, give away, or donate things, and over 80% have sold nothing.” Although the authors suggest that with a larger set of possessions, “their continued keeping can be a predicament for oneself and others,” they explore many reasons why less is divested as we age. The reasons they bring up that we find compelling are: 1. The longer we live, the more possessions we accumulate that signify our identity and “The ongoing collection of belongings can secure continuity of the self in the face of aging and vulnerability.” 2. As we age, we feel less up to the task of assessing value and divesting where there isn’t any: “a major reason for less possession management is the rising

risk of poor health at older ages that can limit the capacity to carry out the cognitive, physical, social, and emotional tasks of divestment.” 3. We’ve divested already: “Most obviously, people may have already divested (perhaps by having moved) and so are content with their collection of belongings.” At Garden State Trust Company, we deal with inter-generational wealth transfer and help people make the transition easier for their beneficiaries. Here are some questions to consider when you’re deciding if something should be kept for a beneficiary: In regard to pragmatic value: Have I used this possession in the last five years? Would any of my beneficiaries use this possession, or do they have something already that replicates it’s use they prefer? As an independent trust company, Garden State Trust Company has served as executor to administer and settle many, many estates. Without question, one of the most difficult tasks in an estate is the distribution of non-financial possessions one has accumulated during one’s lifetime, especially when you have multiple beneficiaries with no direction from the deceased as to who is to receive what! We have witnessed relationships between children irreparably damaged because of a fight over a ring, a picture or some other item that they thought would belong to them someday. It’s truly a shame that this happens; however, it happens more than you think. The next time you meet with your estate planning attorney to discuss your Will, make sure the distribution of your “things” is discussed. Spring cleaning may solve some of this problem. Ira J. Brower

Our client lost consciousness and was helicoptered to the hospital, where she was diagnosed with a broken leg and a facial laceration that ultimately scarred. Our client was a passenger in a vehicle that was turning left at an intersection. Another car ran a red light, causing our client to sustain a severe comminuted fracture of her upper arm, requiring surgery with a significant keloid scar. Our client was hit from behind at an intersection. She had had neck surgery just one year prior. When our client came to us, she was really concerned her neck was reinjured. Ultimately her neck was fine, but her shoulder had a tear that required surgery.

Healing Your Relationship With Food Stress levels have exploded over the past year. People are contending with their own personal and family problems, further complicated by the pandemic, job concerns, political and social issues, and on and on. For many, this layer upon layer of stress and worry has created or exacerbated unhealthy eating habits. It’s not unusual to hear, “I’ve gained 15 lbs. since the start of all this.” Maybe you’ve noticed yourself visiting the fridge or kitchen cabinets more often – hunting for food as a way to soothe your worries or calm your fears – as you escape for a few blissful moments into a dish of creamy ice cream, or a piece of gooey pizza, or a bunch of chocolate peanut butter cups. After a while, you may find yourself reaching for food simply because it’s become an automatic behavior. Whether you’ve gained weight or stayed the same, if you’ve gotten stuck in unhealthy eating patterns, your health and well-being can suffer. When you use food as a way to deal with emotions, you’re creating an unhealthy relationship with food – and embedding troublesome habits. But you are capable of changing that. To begin healing your relationship with food, it’s important to remind yourself that food is for fuel. Yes, you should enjoy and savor it – but always remember that eating is a way to provide nutrients so that your body and mind can function optimally. Consider the following questions. For any “yes” response, consider more appropriate, commonsense ways to handle those feelings and situations – and start making the changes you need to make: • Do you use food as a way to deal with unpleasant feelings (i.e., stress, anger, sadness)? • Do you use food as a way to reward yourself? • Do you use food as a substitute for companionship and love? • Do you use food to procrastinate? • Do you use food as a way to handle boredom or entertain yourself?

Hypnosis For Women ❀ ❀ ❀ ❀

Weight Control Smoking Cessation Relaxation/Comfort Fear Release

Imagine yourself responding more effectively. Notice the details. Mental rehearsal can help you do this. Once you start changing your mindset, amazing things can happen. If you find that you need help with this, using hypnosis and coaching can transport you from knowing what steps to take, to actually taking those steps. This approach is based on neuroscience – the study of how the mind works. Research in neuroscience has taught us that the brain is constantly rewiring itself, which means you are always capable of changing. By accessing your subconscious mind, negative patterns are much easier to neutralize, and positive patterns can be embedded. An important side benefit of this approach is that it bolsters your ability to relax. Hypnosis is an efficient, holistic, and empowering modality. It works wonderfully for people who are motivated to change, believe that with the right tools and techniques they are capable of changing, and understand they need to be a partner in the process. (Virtual and in-person sessions available. COVID precautions are in place.) For a Free Phone Consultation, call me at 732270-0080. Integrating hypnotic techniques and coaching paves the way for successful outcomes and boosts your sense of well-being.

“Spring Is Here!” Pets

Springtime is one of the busiest times at the Funny Farm. It’s also known as kitten season. On average, over 120 kittens and cats get adopted from the Funny Farm. Health & Wellness At The “Heart” Of Yoga Practice Is Inquiry

29

Heidi Farber For human beings to transform in positive ways, there must be selfreflection. The Yoga Sutras of Patanjali move us into the realm of inquiry. For the yoga student, it offers us a vocabulary to begin to see into our patterns of emotional and physiological holding. The sutras are threads that help us unravel our entanglements. Let this be your guide to self-knowledge. The core of who you are affects your relationships, perceptions and actions. The Yoga Sutras provide a structure that allows us to navigate through the twists and turns of life. Patanjali has become the source for yoga. His sutras are accepted as yoga philosophy. It begins with the Sanskrit word Atha. Now is the inquiry of yoga; now is the teaching of yoga. Cultivate a beginner’s mind. Let your heart set your intention. Then the journey is the reward and life unfolds naturally. See newness in the now. Commit to the process in every moment. The 8 Folded Path begins with Yama and Niyama. This is an ethical and moral practice. Yama is means regulation, control or restraint. Each one is a guideline for behaving in a benevolent manner toward others from moment to moment. Yama and Niyama teach us to be skillful, wise and compassionate. Ahimsa is the very first Yama and it guides our actions toward the benefit of others. It brings

us toward equanimity and insight. You can bring about peace internally and you will generate a powerful external effect. Refrain from causing distress in thought, word or deed. Respect and cherish all life. We have the power to neutralize unwholesome thoughts. Mindfulness calls us to be fully present as if totally intimate with all things. Compassion is the wish-fulfilling jewel. Practicing loving kindness opens the heart. Love is an act of surrender and the key to understanding all life’s mysteries. Ahimsa is the key to inner and outer happiness and harmony. Let your pure inner light of awareness shine. Begin where you are.

Do Artificial Sweeteners Help with Weight Loss? Written By Barbara Greenling, DNP, MSN, APN-C

Do you drink diet soda, versus regular, thinking you’re doing your body a favor? Are you under the impression that artificial sweeteners are a healthier alternative, especially for weight loss? You are not alone. Recent statistics show that close to a third of Americans say they use artificial sweeteners daily. Typical examples include aspartame (Equal, NutraSweet), sucralose (Splenda), and stevia (Truvia, Pure Via). They’re all available in packets and are also added to soda, yogurt, and other foods.

But do these sugar substitutes actually help you lose weight? New research suggests they do not. In fact, these zero-calorie additives may have the opposite effect — and possibly even increase the risk of high blood pressure, diabetes, and heart disease. Researchers reviewed 37 studies that included a total of more than 400,000 people. But only seven were “gold standard” randomized controlled trials, which compared people who used artificial sweeteners with a control (placebo) for six to 24 months. Those studies found no clear proof that the fake sugars helped overweight people shed pounds. Perhaps a different approach to dropping the pounds is in order. Instead of focusing on replacing sweetness in your food and drinks, maybe re-training your palette to less – sweet alternatives would be best in the long run. For example, unsweetened iced tea is far more thirst-quenching and refreshing than the sweetened version. You may actually find you prefer it if given the chance! Start to experiment by leaving your comfort zone a bit and trying new ways to limit and enjoy your sweets. This is especially important if you are overweight so as to prevent bigger problems down the line. If, however, you’ve already been diagnosed with type 2 diabetes and you’re overweight, you should get started as soon as possible on a weight loss plan. It

is important to work with a medical professional or registered dietitian to help you figure out a plan that will work for you—a healthy meal plan, physical activity, and realistic goals will help you reach a healthy weight.

There are many advantages to losing weight (and not just diabetes-related ones):

• Boost your energy level • Lower your cholesterol levels (especially important for people with type 2 diabetes) • Protect your heart (also important for people with diabetes, since heart-related complications are very common) • Make it easier to control your blood glucose level As you may already know, losing weight can be a challenge, but don’t let that stop you. Do whatever you need to in order to stay motivated. It is the amount of calories we eat that contributes to weight gain. Make small changes. Learn portion sizes and reduce the amount of snacks in your day to reduce the total amount of calories you consume each day. Find cookbooks with healthier recipes using low-fat options. A Final Note on Weight Loss For some people, losing weight doesn’t help them have better control of their blood glucose levels, and that’s all right. They may need to use medications or insulin to keep their blood glucose level in the normal range, but they should also still eat healthy foods and increase physical activity. Everyone should strive for a healthy weight. Losing weight can help lower your body’s insulin resistance, but if it doesn’t help you achieve better blood glucose control, that doesn’t mean you’ve failed—or that you should give up. Losing weight and then maintaining a healthy weight are healthy choices for life—whether you have type 2 diabetes or not.

Psychiatry

31

So What’s Next? By Charles E. Meusburger, MD, FASAM The purpose of this article is not intended to be political commentary, but rather a psychiatric/psychological and addiction dialogue about human minds under duress and the thinking that causes it.

After a year of scathing, unpredictable and deadly diseases, half biological and the other half behavioral, most of us want to go on a long vacation elsewhere— anywhere but here! Oh, and don’t forget to include a little predictable pampering if possible too. It feels like it’s all been earned and a bit overdue. Powerlessness is the main source of the power of trauma to leave its mark and to be re-experienced and perpetuated in one’s mind for decades after the actual trauma occurred. The entire planet Earth has been vicariously and very personally traumatized by the ravages, images, losses and fears from a yearlong global pandemic. The vector for this biological onslaught appears to always be just one step ahead of all the ingenious and devoted scientists, medical and psychiatric/addictions professionals and all those in leadership positions; at least the ones who have demonstrable skills to actually lead. This tsunami seems to have an insatiable and voracious appetite to eliminate as many homosapiens as it can afflict, one way or another. To date, there have been 2.13 million deaths worldwide with 99.3 million cases confirmed globally and rising aggressively every day. The United States has almost double the number of fatalities of its closest competitor, Brazil. The residual deficits for those who contract the illness and survive it are extensive. It may affect any organ system and can be very seriously debilitating, lingering for long and/or indefinite periods. As if this constellation of pathological interlopers wasn’t enough, it’s been secretly mutating in different global locations to more contagious and possibly more virulent strains than the original version. Those “new and improved” invisible assassins travel and are already in the United States. The vital question currently unanswered is: ‘Will our present vaccine defense work well enough against these newer mutants?’ Stay tuned and stay hopeful. Our lives, behaviors, workplaces, childcare, socializing, and humanness have all been severely altered and remain—again—uncertain as to when, what, who and how anything or anyone will be returning to a prior state of “life as we know it.” For all the complaining

about it we did prior, most of us long for a renewed opportunity to bask in the mundane and predictable. The entire country—frankly, the world—was also vicariously and some personally traumatized by the insurrection against our Democracy and the United States government that took place at the U.S. Capitol on January 6, 2021 by Americans against America because of widely perceived altered realities, one’s perception is one’s reality. The powers of consistent indoctrination quickly followed by intimidation strikes again in history to manipulate the masses and create hysteria, chaos and focused assaults on law and order for a select few who seek absolute power absolutely, at all costs. This was just ‘Insurrection American Style.’ It has all been ‘played’ before, many times over the centuries, but we say ‘that could never happen here.’ Obviously—yes, it can and it did and will continue until there are serious, decisive and definitive measures taken on several levels to restore not just order, but freedom. Disenfranchised people make for easy fodder for cultivation into something aggressive and extreme. It’s like emotions on steroids—sometimes literally, without cognitive oversight and internal “veto power” over hyperbole, vitriol (better known as propaganda) and predictably predictable violence; gratuitous mostly, can occur anywhere at any time; especially when the co-contributing factors all line up well for mindlessness being forged into fanaticism based on biased beliefs morphed into incorrect facts, better known as lies. Propaganda envelops the frightened, insecure, disenfranchised and disgruntled; it gives them purpose, a voice, a cause and a drug. Propaganda fuels preexisting fears and inadequacies to coopt reason, trust and free will. It insidiously inserts and substitutes false control. It fuels all that is primitive in the masses, including selfabsorption and righteous indignation. This then creates localized venues to be hated and scapegoated; to channel all the stoked-up rage and manipulated madness that serve the goals of their masters with mindless devotion, disregard, and absent insight into what’s true and good. Abuse comes in many forms and all shapes and sizes. So the core elements for ‘A Perfect Storm’ have been averted—barely. However, we are not done yet. The key question is, will we learn or will we deny and condemn ourselves to repetition and a renewed form of slavery? And then it’s “game on” blindly. All abuse cases have this mind control and manipulation at their core in order to secure what the abuser wants—power and control (the ultimate highs) over the person or, in this case, over the country. In cases and studies from history all over the world, this has occurred repeatedly. The individual person

caught up in incestual abuse over long periods of time always has an element of ‘it’s the abused person’s fault’ that the abuse is occurring—they are to blame; they want it, seek it, and even enjoy it, creating the icon of severe states of confusion and distorted norms. This allows the perpetrator to do exactly what they want—continue to abuse. The victim doesn’t know how to resist full dominance for the real high: power and control. Indoctrinate and intimidate and one can get anyone, singularly or entire countries, to do literally anything—even here. The scariest consequences of mass manipulation is when, inevitably, the created mass monster no longer even obeys its master(s). Once there has been a taste of this power, the addiction begins and will continue. The power addicts have zero insight into their disease and will dismiss anyone or anything who disagrees or interferes with this now ‘turned on’ insatiable obsession to grab it all at all costs. These persons rationalize the irrational and justify the means for their exaggerated ends, and have no conscience, compunction, empathy or remorse. Remember, sociopathy knows no limits. The ‘7 Deadly Sins’ form a partnership with the ‘4 Horsemen of the Apocalypse.’ So “what’s next?” Doom or gloom on parade and head for the hills with the K rations and newly purchased weapons to hide and defend? I think not. That would be surrendering to all that is simply wrong. Genuine hope based on reality checks and rekindled trust in us to exercise good judgment, positive influences on each other, on each day, and on ourselves and each other using the reaffirmation of truth, strength in stalwart rejection of selfishness and bigotry, greater tolerance and inclusion, celebration of thoughtfulness and kindness, reinforcing the value of every individual and the ability of the human mind to think, analyze correctly and seek better, healthier solutions as a force of one or many for goodness, growth, health and freedom. Humility and gratitude are far more inspirational and beneficent than hate, dishonesty and violence. Do not succumb to the viruses, neither the biological ones nor the behavioral ones—be smart and safe. Want and ignorance only yield arrogance and loss. Rise above the hyperbolic, easily led crowds and their propagandabased emotions and actions. Be a genuine resource for yourself and as many as you can all around you to remember your gifts, prioritize well, utilize existing wisdom and seek counsel throughout the perilous journey. You need not do this all alone; but we all need to do these things and we can—we must.

Don’t suffer alone ...

If You Have Been Told You Are Not A Candidate For Dental Implants:

Put Your Best Face Forward SEE DR. BANKS! Dr. Kathy A. Banks is Ocean County’s most experienced Oral and

Often, patients are told that they are not candidates for dental implants because they Maxillofacial Surgeon injectable facial “don’t have enough bone” or their “sinus in is toodelivering low.”

fillers and rejuvenation products.

Both conditions are correctable problems in almost all patients, according to Dr. Banks. When the gum ridge where teeth are missing is too thin to place a dental implant, bone grafting can be performed to correct the problem. What that means is that a bone replacement product, or even the patient’s own bone, can be placed into the deficient area, stabilized, and allowed to heal. Following healing, dental implants can be placed into that area and then restored with porcelain teeth. This procedure is called “ridge augmentation” and can be accomplished in both the top and bottom jaws. When teeth are missing, the sinus that lies above the top jaw will expand downward into the ridge and cause bone loss in the gum ridge. This is a correctable problem as well, according to Dr. Banks. “A Sub-Antral Sinus Augmentation – commonly called a ‘Sinus Lift Procedure’ – can be performed, sometimes even at the time of implant placement. This procedure increases the amount of bone in the area where we would like to place a dental implant. Again, a bone replacement product or even the patient’s own bone can be used.” A common misconception is that the bone is placed into the sinus. In reality, the sinus membrane is actually lifted up (hence to name “sinus lift”) back to where it should be, and the bone graft material is placed under the sinus membrane, restoring the gum ridge and the sinus contour together in one procedure. Dr. Banks incorporates state-of-the-art diagnostic procedures, such as 3D cone beam CT scanning of the face and jaws, to evaluate her patients for bone grafting procedures. “That way, I can accurately diagnose the magnitude of the bone loss in the area where we would like to place implants and the quality of the patient’s bone ridge in that area, and pre-plan the surgery to ensure the best possible outcome.” She also prepares and uses advanced biologic into grafting procedures such as L-PRF Ultra XC plus® to reduce the appearance of the nasolabial Dr. Banks’ experience with Botox® began in thematerials mid 1990’s while working injections of Juvederm with patients with various temporomandibular joint problems. “In those days, folds (marionette (Leukocyte-Platelet Rich Fibrin) which are made right in the office at the time of surgery. lines) around the mouth and nose are the most common services

Forgiveness:

How It Can Help Us Live Longer And Healthier Lives A Buddha teaching states, “Holding on to anger is like grasping a hot coal with the intent of throwing it at someone else; you are the one who gets burned.” When someone hurts us, we can hold onto anger, resentment and thoughts of revenge; or we can forgive. We can let go of the burning hot coal and, instead, embrace peace, hope, gratitude and joy.

the pain and moving on. And most of the time, we are the only ones suffering; the offender is fine and living their life. • Know your role. It is seldom the case that one person is 100 percent innocent. Therefore, it is important to know what we could have done to prevent what happened. This can help us move beyond feeling like a victim. • Learn from the past, live in the present, believe in the future. It is no longer happening, except in our minds. When we start thinking about the past, acknowledge it, and return to the present.

Dr. Nina’s What You Need To Know about forgiveness

• Welcome peace. Whether it is meditation, therapy, prayer, deep breathing, or exercising, these actions help clear our minds and allow peace to enter.

Reduced Stress and Anxiety

• Empathize and feel compassion. Trying to see things from the offender’s point of view can help us understand the situation better and feel compassion for them. We are not excusing their actions, or condoning them, but we are allowing ourselves to be happy and move on. Let love for them and, more importantly, ourselves, grow in our hearts.

and how it can help us live longer and healthier lives:

Anger releases the same hormones — adrenaline and cortisol — that are seen during a “fight or flight” situation. While having our muscles tense, our blood pressure increase, and our pores sweat is a natural, primal response to predators, if this reaction is relived over and over again, it can cause anxiety and depression. Not surprisingly, studies have also shown that letting go of a grudge lowers the risk of alcohol and substance abuse.

Better Heart Health Finding forgiveness in our heart will benefit our heart. Studies have shown that forgiving someone decreases our blood pressure and heart rate, and subsequently decreases the amount of work that our heart must perform. In fact, chronic stress makes platelets “sticky.” This means they are more likely to clot or bunch together and block off blood flow to our heart or brain, which can result in a heart attack or stroke, respectively.

A grudge functions much like cancer; if we don’t remove it with clear margins, it will grow and may be the death of us. And while we may be in the process of forgiving and letting go of the negativity remember: “Never wrestle with pigs. You both get dirty and the pig likes it.” — George Bernard Shaw

Reduced Pain Take two pills, forgive the unforgivable, and call me in the morning. Studies have shown that forgiveness can decrease the level of pain in people who suffer from chronic back pain. While the mechanism for this is not yet clear, this opens up alternative avenues for treatment. It could lift the weight off of our aching back. The problem is that “Everyone says forgiveness is a lovely idea until they have something to forgive”—CS Lewis. Additionally, there is no manual to help us maneuver through it. Experts in the field have made some helpful recommendations: • Commit to let it go. Forgiveness is an active process, not something that will just happen.
