Coupons de grace by Douglas Winslow Cooper As I learned in French I under Monsieur Pierre Wrinn at Walden High School, the coup de grace is the final, winning stroke in a contest. These days, coupons can serve much the same purpose in building and retaining a customer base. “I don’t buy anything any more without a coupon,” our home health aide told me while clipping coupons on her break. Another staff member chimed in, “Me, too!” I Googled “couponing” and found eight million items, including “extreme couponing,” “true couponing” and “the Krazy Coupon Lady.” Our nursing staff (my wife, Tina Su Cooper, is quadriplegic and ventilator -dependent) regaled me with tales of large savings racked up by “extreme couponers.” They themselves, of course, never take things to extremes: “Moderation in all things…except morality,” as Dwight David Eisenhower was reputed to have said. I pressed the crew for the downside of newspaper coupons: finding and clipping are time-consuming; expiration dates are in tiny print and often too soon; the prices may have been raised to be able to give some people a discount. I, myself, have trouble with scissors, having scored poorly on cut-and-coloraptitude in kindergarten. Fortunately, no glue is needed, but I always worry about what I am losing on the backside of the page from which I am extracting the coupon. Internet coupons overcome some of these negatives.

Clipping coupons online There was a time, back in my youth, when only the elite “clipped coupons,” which was

how they cashed in their quarterly interest payments on their bonds. Today, coupon clipping is almost an indoor sport. Inserts in the Sunday papers are passed around among the clippers, each excising those she wants. Are men doing it, too? Definitely. I’ll be doing it today, but on my computer.

Shall I get body wash? Wrinkle remover? Mud mask? Lip gloss? Which would do me most good? I’ll have to ask my wife. E-Sales Masters, L.L.C., will give me “10% discount up to $500 on fine art plus free shipping.” I may need another Rembrandt, for the foyer. Wait --- I see what I need: “Life Cleaners!” At 10% off, they could start with my office, then upstairs to the bedroom, and the porch needs help, too. Another element of my life that could benefit from cleaning up would be my relationship with my in-laws. Oops, they are “a full-service dry cleaner that operates an automatic kiosk in the ShopRite in Vails Gate.” Would they do a leisure suit?

Some of the other offers include: a free fire extinguisher, discounts on storage, dental work and submarines (the sandwiches, of course). Janco Security would give me a year’s worth of free monitoring, allowing us to rest easy, cut-rate.

Internet coupon offerings effective According to statspotting.com, last year three billion coupons were redeemed in the U.S. In a country of just over 300 million people, that works out to about 10 coupons per person per year. Half of the coupons that were redeemed came from the Sunday newspaper supplements. They note that typically 1% of the coupons offered by such print media are redeemed, but 10% of those offered on the Internet are redeemed, making Internet coupon

offerings particularly effective. The Orange County Chamber of Commerce encourages its members to participate in establishing coupon offers, as well as in taking advantage of the bargains they offer. With something like 1,700 members, there’s room for a lot more coupon offers on the Chamber website -- and the opportunity for a lot more members to reap the rewards. It is well-known by most New Yorkers that for the past several years our state legislators have been trying to tackle the escalating problem of Medicaid spending; developing a variety of solutions to reduce the cost burden on the state. For the most part, their solutions have focused on reducing provider payment rates and decreasing or eliminating services. Medicaid providers have seen their payment rates decrease demonstrably and consistently, year after year, yet have upheld their responsibility to provide quality care to their patients despite the squeeze on their income as costs to provide these services continue to increase while reimbursements decrease.

Medicaid Redesign Team In many of the state’s counties, Medicaid costs account for more than half of the county tax levy and as a whole, New York spends more than twice the national average on Medicaid on a per capita basis. New York spending per Medicaid beneficiary is the second highest in the nation. The magnitude of this financial strain on New York has become so great that in January of this year, Governor Cuomo announced the formation of the New York Medicaid Redesign Team, a task force consisting of 27 individuals charged with reversing overspending and waste in the Medicaid system. This is not the first time a task force in New York has been assigned to take on the challenge of reducing Medicaid spending; two other initiatives were undertaken in the late 1990s and mid 2000s - without success.

In February, the Medicaid Redesign Team issued a total of 79 proposals aimed at reducing spending; their work was touted as “historic progress for New York State.” Consistent with solutions implemented in past years, the majority of these proposals focused on reducing payment rates to providers, but did not address the mounting growth of individuals enrolled in the system – an issue I contend is the root cause of the rise in Medicaid spending.

Medicaid enrollment has increased In the year 2000, 14.4% of New York’s population or 2.7 million individuals were Medicaid enrollees. Medicaid expenditures exceeded $23.6 billion. At the time, 8.4 million New Yorkers were employed, contributing income taxes to fund the system. Over the course of the next 10 years, the growth rate of Medicaid enrollment and associated costs significantly outpaced the slight 1.4% employment growth in New York. The number of Medicaid enrollees increased by 73% to 4.7 million individuals or 24% of New York’s total population. Medicaid spending in 2010 rose to a total of $49.4 billion, a 109% increase since the year 2000. The over-arching issue of focus our legislators, state committees or task forces should be tackling when developing solutions to address the Medicaid spending crisis is changing Medicaid eligibility criteria and decreasing total enrollment. Regardless of the number of provider rate cuts implemented or services eliminated, if the overall number of individuals approved to receive Medicaid services continues to rise at rates exceeding population and employment growth, the state will remain financially crippled for years to come.

Charlie, who is President of RAL Supply Group in Middletown, has been a member of the Board of Directors since 1996 and I’m really looking forward to the subtle yet powerful style of leadership he will bring to this organization. My deepest thanks go to outgoing Chairman Donna Johnson, who dedicated two years of her life to doing everything in her power to make the Chamber the outstanding organization it is today. Health notes Several residents of the Elant at Goshen Nursing Care Center have taken their usual fundraising activities to another level, deciding to pay it forward. A portion of the funds raised, which usually go toward resident activities, this year are being donated to organizations in the community, including Food Bank of the Hudson Valley, People for People Fund and Inspire. Residents of Elant at Newburgh recently participated in the nursing home’s second Olympic games. U.S. Rep. Nan Hayworth recently visited with residents and toured Elant at Fishkill. Energy efficient lighting at 60% off For a limited time, Chamber members can take advantage of Central Hudson’s Commercial Lighting Program. Participants will receive a lighting assessment and retrofit with high-efficiency lamps and fixtures, paying only 40 percent of the total cost. The average payback is approximately 14 months, and the cost can be financed interest-free over that period. Central Hudson pays the balance of the new lighting installation. Chamber members who have benefited from the program this year include Vanacore, DeBenedictus, DiGovanni & Weddell, L.L.P. and Mount Saint Mary College. The program is open to commercial and

non-residential customers with an average monthly electric demand of up to 350 kilowatts. To enroll, commercial customers should call (845) 486-5221 on weekdays. “Since the Commercial Lighting Program was first offered in November 2010, more than 1,490 commercial and governmental projects have been completed, realizing in total an estimated annual savings of more than 34 million kilowatt-hours in electrical usage, or enough energy to power 4,300 homes for one year,” said James P. Laurito, President of Central Hudson. “Participating small commercial customers have realized an average savings of 23 percent on their electric bills, and mid-size customers an av-

erage of 11 percent,” he added. “Together, these savings will reduce electric bills in total by an estimated $3.25 million per year, and avoid 17,000 tons of greenhouse gas emissions into the environment.” Building owners should be aware of a mandate by the U.S. Department of Energy to phase out production of older T12 fluorescent lamps and ballasts beginning July 2012. “This is an ideal opportunity to replace these older fixtures and lamps at a substantial cost reduction, and realize energy savings year after year,” said Laurito.
