• This application is required for any family wishing to receive a K-8 tuition scholarship.
• The application must be submitted with enrollment to complete the process.
• Only one scholarship may be applied to a student’s tuition.
• Scholarship funds are not given directly to families; but rather, are deducted from school tuition.
• If a family fails to meet qualifications at any point during the school year, the scholarship may be revoked.

Assumption Scholarship
To be eligible for this scholarship, a parent and their child/ren must be baptized Catholic, be registered as active parishioners, participate meaningfully at Assumption Catholic Church, and be in good financial standing from the previous academic year. Mass attendance and participation will be monitored regularly through the parish envelope system.
Saint Augustine Scholarship
To be eligible for this scholarship, a parent and their child/ren must be baptized Catholic, be registered as active parishioners of a Catholic Church in the Diocese of St. Augustine for three months and be in good financial standing from the previous academic year. *Letter from Pastor verifying faithful Mass attendance must be attached to this form to be considered eligible for this scholarship.

Step Up for Students (FTC, FES-EO, FES-UA) Scholarships
Families who are awarded these state-funded scholarships must provide a copy of their award letter from the scholarship granting organization to the school office before it will be reflected in the FACTS Tuition Management System. Please read, check off, and sign below.
I assume financial responsibility and agree to submit tuition payments via Facts for any amount not covered by the state scholarship.
I agree to approve quarterly payments with Step Up for Students in a timely manner.
I understand that if I use state funds for purposes other than paying tuition, I am personally responsible for making tuition payments to Assumption Catholic School.
