It includes a directory of all of those companies on the International Business Wales Medica mission 2009, as well as a snapshot of some of the most exciting products and services being offered by Welsh companies in this sector. Medica is the largest and leading global exhibition for the medical sector. It runs alongside the leading international fair for medical manufacturing supplies, Compamed. Medica and Compamed together will occupy all 18 halls of the Düsseldorf Exhibition Centre and organisers expect more than 137,000 visitors from over 100 countries. Medica covers the entire range of outpatient and in-patient healthcare, including: electrical medicine/medical devices, laboratory technology/diagnostics, physiotherapy, orthopaedic technology, medical supplies (essential supplies and consumables), information and communication technology, textiles, medical furniture, furnishings, facility management, building services. At the same time, Compamed addresses all aspect of supplies to the medical manufacturing sector: raw materials, components, parts, packaging, services for use in medical device production. International Business Wales (IBW) is the Welsh Assembly Government’s international trade and investment arm, forming part of the Department of Innovation, Enterprise and Networks. It acts as the driving force in establishing

alliances between companies in Wales and their counterparts overseas. At the same time IBW actively seeks out inward investment opportunities for client companies looking to Europe and the UK as a preferred international business location. The Welsh Assembly Government is a democratically elected body, which was established in May 1999. It has devolved powers and responsibilities for developing and implementing policies in Wales on a broad range of issues, including economic development, education, the environment, agriculture, housing, roads and transport. The Welsh Assembly Government works closely with primary legislation and has powers to introduce secondary legislation of its own. MediWales has been active for over seventeen years and is the representative body for the medical sector in Wales. Supported by the Welsh Assembly Government, MediWales is owned by its members and led by directors drawn from senior industrialists and academics in Wales. MediWales generates a unique environment for networking, collaboration and communication between industry, higher education and health service members. This is delivered through an active schedule of seminar events and publications, supported by the MediWales website and maintained via constant communication with members and stakeholders. From this pivotal role, MediWales is able to represent both its members and the wider sector to governmental and international partners.

Paths out of recession by Jess Fisher

Last year’s Review of the Welsh Bioscience sector highlighted an era of mergers and acquisitions for Indigenous Welsh Companies. A year later – and over a year into the global recession – the Review tells a different narrative, one that illustrates the growth in significance for Welsh Bioscience and Medical Technology companies as they attempt to keep the credit-crunch at bay. In March of this year, the Welsh Assembly Government commissioned MediWales – Wales’s Medical Technology and Bioscience forum – to ‘map’ the Welsh Bioscience sector. This project resulted in key findings: there are 322 companies in Wales defined as ‘Bioscience’; the sector employs more than 15,000 people; and contributes more than £1.3 billion to the Welsh economy. Key opinion leaders knew Bioscience in Wales was strong, but these figures substantiate its value, emphasising the sector’s worth as a field for future investment.

The sector’s segmentation by activity provides further insight into Wales’s strengths, but also highlights how international trade shows like Medica are vital to the growth and success of the sector. 63% of companies operating under the umbrella of Bioscience are Medical Technology companies. With this many businesses producing innovative medical devices, in a sector characterised by its global markets, the potential for international trade is huge. Already statistics for the first quarter of 2009 showed that Welsh healthcare exports grew by 9.3%, seeing a rise from £187m to £205m. These indicators suggest that Welsh Medical Technology companies are more than able to trade themselves out of the recession. Adding to this argument, when MediWales Members were asked what their priorities were in this difficult economic climate, surprisingly, they were not funding or investment capital. Though obviously significant, most of our companies said that if MediWales can create opportunities for its members to do business, then sustainability and growth would follow. MediWales has collected evidence indicating that access to clinical expertise and facilities for product evaluations and trials is a critical success factor for its membership. In a survey of the current situation, MediWales requested case studies of companies’ successes in procurement and/or any problems experienced. The resounding reply was that the most significant issue facing our companies is access to clinical expertise throughout the product development process and if this could be improved, then companies could resist difficult financial times. These messages led to the MediWales ‘Clinical Access’ initiative: a working group that has brought together key players and opinion leaders from academia, business and clinical institutions across Wales in order to discuss and implement necessary change. This

vibrant interdisciplinary initiative, supported and observed by the Welsh Assembly Government, has been discussing the following areas: early stage evaluation of ideas; needs assessment; evidence gathering (not always formal clinical trials); product assessment by the UK National Health Service; and procurement. The group is working hard to improve access in all of these areas. Early talks have resulted in progress on establishing the evidence required by procurement and some Welsh companies have succeeded in getting products closer to market, such as DTR Medical, Convatec, and Invacare. Other outputs include the proposed development of a

£1.3 billion to the Welsh economy ‘clinical access route map’: this will not only layout clear pathways for healthcare companies to follow across the five identified stages, but it will detail who to communicate with in Wales and how to proceed. The fact that this working group was able to materialise at all highlights another of Wales’s strengths. Key opinion leaders in the Welsh Assembly Government, clinicians (including consultants, procurement nurses and clinical researchers), clinical academics, and industry are able to meet and discuss important issues that affect the sector. The ease with which this occurred could be down to good communication networks, however it is also likely to be a result of a devolved health service and a relatively small nation, which makes high level access across interest groups that much easier. Whatever the reason, meetings like this show that Wales is able to communicate with little bureaucracy and create working partnerships harder to achieve in the wider UK. As a result, Wales has a unique opportunity

to create successful clinical pathways and systems of its own if it takes advantage of the benefits size and devolution brings. A re-occurring annual Welsh highlight is the BioWales conference, tradeshow and brokerage event – one of the UK’s largest Bioscience conferences and bio-partnering events. Now in its eighth year, BioWales has become a landmark event, attracting delegates from throughout the UK and Europe. The success of Bioscience relies on successful connections and collaborations between its institutions. BioWales promotes and provides a platform for vital networking year after year, offering continuity and the right environment - a major factor in its popularity. Next year the conference takes place between 17-18 March, at the Vale Hotel near Cardiff. The bio-partnering event specifically encourages international collaborations between the UK and Europe through its one-to-one meetings. The people interested in brokerage meetings are linked up by the Enterprise Europe Network (EEN), which brings together those seeking and those offering technology opportunities as well as supporting access to European-wide research funding frameworks. The activity from BioWales feeds into EEN activities that the Welsh Assembly Government’s Department for Economy and Transport facilitates throughout the year, which often leads to

322 Bioscience companies international partnerships and inward investment. Like the Welsh Bioscience sector, the event has grown substantially since inception: last year it attracted over 400 delegates with more than 40 exhibitors. Next year’s conference is themed around regenerative medicine; stem-

cell industrialisation; point-of-care diagnostics; cancer genetics; and translational research, reflecting the current academic and industrial excellence in both Wales and UK-wide.

15,000 employees Eminent speakers from both industry and academia will describe some of the recent developments in these areas, helping to map future trends in the Bioscience landscape. Underpinning the success of strong networks and collaboration, Wales has a vibrant manufacturing base; a large number of innovative medical device companies; world-renowned academic institutions; and academic and clinical centres of excellence. These strengths, combined with the effects of the sector mapping project, the clinical access initiative and a strong programme of regular events, has made Wales and the sector an exciting and vibrant environment to work in. Confident and self aware of its own strengths, there is an understanding of how things currently stand, which helps to set goals and targets for the future. Bioscience’s successes have been highlighted, but the sector is also honest about where it needs to improve. The deep-routed networks and willingness by key players to implement change and be involved show that the sector - and indeed Wales - is proactive, not just reactive to problems. As Wales takes advantage of its skills and opportunities, there will be a multiple pay-off: enormous potential for improved healthcare, reduced costs, economic benefits, and increased inward investment. The unique and sustained identity of Wales as a small, but clever devolved nation, places the country in a position to push Bioscience as Wales’s strongest sector.

Oncomorph Analysis, a spin out company from Aberystwyth University, was founded in 2008 to develop the use of existing MRI data with new image analysis software to improve the diagnosis and staging of prostate cancer. The company is developing a suite of software tools, including annotation and segmentation functions, for both external and internal tissue identification and a 3D registration tool, to yield tissue volumetric data. The core technology that enables Oncomorph Analysis to interrogate digital images is a shape/texture-based segmentation tool that can differentiate between the patterns of a tissue through the analysis of a small number of the total pixels that make up the larger image area. This reduces the amount of data that needs to be ‘crunched’ and speeds up the whole process of analysis. Objectives at Medica: The aim of going to Medica is to establish links with existing image technology businesses to broker joint venture opportunities to accelerate the route to market.

The Welsh Wound Network is a new collaborative organisation dedicated to improving the clinical care of people with wounds in Wales. ZooBiotic Ltd is based in Bridgend, South Wales and manufactures and markets larvae products for the treatment of sloughy and necrotic wounds. All products are produced in accordance with current European 'Good Manufacturing Practice' (GMP) requirements in our state-of-the-art aseptic facility. 'Free range' LarvE® and our BioFOAM® Dressings are Zoobiotic Ltd's core products. BioFOAM® dressings consist of larvae that are enclosed in net pouches which contain pieces of hydrophilic polyurethane foam. The BioFOAM® Dressings can be left on a wound for up to 5 days. Each dressing is supplied in a plastic oyster as well as a paper/polythene bag which acts as a microbial barrier and is permeable to air. The 'free range' LarvE® are applied directly to the wound and seek out areas of slough or necrotic tissue. They are concealed in a net dressing or similar. 'Free range' LarvE® can be left on a wound for up to 3 days. LarvE are supplied in a sterile container with a lid that is permeable to air and acts as a microbial barrier.

Boots Centre for Innovation Physicool, a unique stretchy reusable bandage with an interesting story, has been launched by the Boots Centre for Innovation. Available on sale through the Boots website, this is yet another product to be launched by the Centre, a not-for-profit organisation that seeks out innovative health and beauty products at any stage – from concept to shelf ready – for Alliance Boots pharmacy-led stores. When Physicool is applied to a physical strain or injury the temperature of the affected area drops significantly, about 10C15C. It can be used straight out of the pack, without prior need to freeze or introduce ice as with other solutions. The cooling and compression provide instant treatment for inflammation and bruising of muscles, tendons and ligaments; by rapidly reducing swelling and pain, the bandage helps to speed up the healing process. The cooling effect lasts approximately 2-3 hours; even after the bandage has been removed it works by drawing heat out through rapid evaporation, whereas other products work by conducting cold in. The Physicool range comprises four products: three bandage sizes which address different body areas, and a recharge solution allowing further use from exhausted bandages - which can be washed as necessary.

After discussions on how they could capitalise on the media attention, the Centre were able to bring forward the launch on www.boots.com to coincide with the TV show in August 2009.

BBC’s Dragons’ Den are currently discussing with Kay about featuring Physicool in a follow-up programme that tracks the dragon’s investment and involvement in the company. Boots and the Boots Centre for Innovation may well be featured in this opportunity. Aside from the Dragons’ Den, Physicool is also supported by famous cricket player Andrew Flintoff. Andrew’s been using the product for pain relief and to accelerate recovery from persistent injury problems for some time. Physicool is in talks with Andrew’s agent for future endorsement, making additional media attention very likely.

DTR DTR Medical - the rapidly growing specialist in sterile single-use surgical instruments - is seeing a significant rise in enquiries from many parts of Europe, Middle East and beyond, underlining the growing need for high quality disposable equipment. DTR Medical believe there are several factors influencing the increase in sales for sterile single-use equipment. Firstly, sterile disposables reduce the risk of cross-

contamination. Complications like surgical site infections and extended hospital stays are minimised, which lower costs and reduce potential litigation. Secondly, disposables rule out reprocessing – the need to clean reusable instruments. Reprocessing is a difficult and timely process, often leading to delayed patient throughput and raised costs from high manual handling rates. Another positive with single-use disposables is that they don’t need constant validation and repair, which is expensive and can disrupt theatre procedures if reworking is delayed. Lastly, with reusable equipment there is the issue of replacement capital: busy units demand multiple sets to cope with high patient numbers. This is even more expensive where reprocessing is done away from the clinical location, making funding often much more than anticipated.

GX Systems These cumulative cost-saving and patient benefits are being noticed far and wide. During a recent international ENT symposium in London - the 27th Politzer Society leading surgeons from across Europe expressed interest in using DTR Medical’s single-use instruments in their own clinical practice. Some delegates were so committed to these

potential benefits, that they placed orders during the meeting. Single-use surgical instruments are not designed to replace all re-usable items, but are most effective when used selectively to resolve a combination of the above factors. Managers of sterilisation services are acutely aware of these issues and welcome the support disposable products bring them. For instance, validating the sterility of long lengths of tubing is often problematic: DTR Medical eradicates this problem by supplying its range of pre-cut lengths of sterile silicone tubing, which are now being sold in markets as far afield as New Zealand.

To help hospital efficiency, single-use disposable instruments must be of a consistent high quality, but also be readily available at all times. DTR Medical takes particular care to ensure every instrument meets specification and it uses a wide range of manufacturing processes to ensure these can be reproduced repeatedly.

In order to keep up with the need of its users, the company is always improving and re-evaluating its systems. DTR Medical recently changed its method of manufacturing a series of medical instruments – the Frazier Suction Handle Range – in order to allow for greater flexibility and efficiency. The range, used across most surgical specialities, now includes two and four tip fenestrations. This modified manufacturing process has enabled the potential for more lines of products. A similar approach has been used to produce a range of sterile single-use Tibbs Arterial Cannula, which are used in many vascular procedures. Growing international demand means DTR Medical must plan ahead, and this is creating new partnerships with Welsh Universities to deliver major new lines, including important developments in gynaecological surgery. The Dynamic Gastric Model (DGM) is a state-of-the-art simulation of human gastric processing. A truly ‘dynamic’ in vitro system, it replicates the biological functions of the human stomach. The GX Design team took an existing prototype and through collaboration with their client transformed the artificial stomach into an exciting exploratory tool for the medical industry. The brief given to GX Design Engineers by their client PBL began at the client’s Norwich based “Model Gut” laboratory at the Institute of Food Research (IFR). Here, the in-use prototype was introduced to functional upgrades, which resulted in the improved - and now commercially attractive - second-generation DGM. The DGM provides predictive simulations of the key conditions and processes which take place in the human stomach. Real foods and meals are processed in real time, providing digested samples for further analysis with full data readout via complex computer software control. Previous to GX Design Engineer’s input, the DGM consisted of several independent systems in order to facilitate full operability. GX Design Engineers optimised the design by combining multiple systems and introducing new design elements. The upgrades enabled their client to take the newly branded DGM to market, primarily aiming at the food and pharmaceutical industries. The process begins in the newly designed Charge Station. Acid and enzyme fluids are stored for dispensing in a stable, temperature controlled environment. Here, fluids can be quickly and easily refilled

before being accurately pumped to the Processing Station via a ‘pulse free’ pumping system. The Processing Station is based upon a three-stage modular design which replicates the physical mixing of food and gastric secretions that occur in the human stomach. The initial stage takes place in a flexible funnel into which food is entered and mixed with acids and enzymes. As a result, specialist materials and finishes were selected to chemically resist the corrosive nature of stomach acid. This proved technically challenging when incorporating transparency into the design in order to facilitate viewing of food materials being processed. Food is then fed into the second stage of the process, which simulates the digestive activities of the lower part of the human stomach (antrum). Located in the central column of the Processing Station, food is subject to simulated physiological shear from a completely redesigned valve system. This novel design improves functionality through increased flow channel diameter and reduced dead volumes, resulting in increased reliability and achieves closer characteristics to the human stomach it is modelled upon.

In the third stage food material then leaves the Model via a new valve designed to give minimal sample loss, into a sample container now accessible without process interruption. This facilitates discrete sampling of the digesta output. Simulated digesta can then be analysed and taken on to an additional stage simulating duodenal digestion. GX Design Engineers have implemented new ideas and developments spawned from a dynamic working relationship with their client. “GX Design Engineers have taken a basic prototype and skilfully woven new features and improvements into the design in a relatively short timescale. We are really excited to see the new DGM in all its working glory”, said Dr Martin Stocks, Business Development Manager of PBL and the Model Gut unit.

The Safeceps™ are a modern-day plastic adaptation of the traditional obstetric forceps. The product encompasses a number of patented technologies which allow for the categorical measurement of traction and compressive force exerted upon the fetal head during instrumental delivery. The Safeceps™ are used in conjunction with a monitoring software system (called the SafeGuard™) which visualises and records the measurement data in real-time, changing a subjective operation to a systematic and evidenced based procedure. Creation of a proof-of-concept unit required the application of state-ofthe-art prototyping and design technology. Challenges included encapsulating an electronically embedded three-dimensional metal component within a thin biocompatible elastomeric. A number of units had to be produced in days
