Muscle Tone Matters in Healthy Aging:

The Key to Optimal Body Composition and Metabolic Health

Muscle tone is traditionally defined as the residual tension in a relaxed muscle or the passive partial contraction of the muscle during a resting state.1 Physical activity, especially resistance training, is considered an integral part of maintaining muscle tone by stimulating muscle protein synthesis and increasing or maintaining muscle mass. Muscle loss, on the other hand, has been associated with delayed recovery from illness, impaired wound healing, reduced resting metabolic rate, physical disability, poorer quality of life, and higher health care costs.2

Body composition can be measured in multiple ways, but in general, it refers to the amount and distribution of body fat and lean body mass.3 For example, people of the same height, gender, and weight may have different body types and compositions depending on their percentage distribution of fat, bone, water, and muscle. Optimal body composition has been associated with numerous health outcomes and may play an integral role in healthy aging and the risk factors for several diseases, such as cardiovascular disease, diabetes, osteoporosis, and osteoarthritis.4

Muscle Tone Matters

Aging:

Exercise and increasing muscle mass and tone can be an effective way to improve body composition. It has been shown to decrease body fat and increase lean body mass, thus improving overall body composition and promoting a healthy aging process.5 There is support for various exercise modalities that can improve body composition. A randomized control trial (RCT) (n = 89) showed that all types of exercise can improve the body composition of individuals with a sedentary lifestyle; however, strength training programs that develop the entire body may show the greatest impact.6 A meta-analysis comparing the effectiveness of several different types of exercise for overweight and obese individuals found that combined aerobic and resistance training improved body composition and inflammatory status the most.7 Finally, an overview of 12 systematic reviews and 149 controlled trials concluded that exercise favorably impacted body composition by reducing total and visceral fat.8

Metabolic health refers to how well the body generates and processes energy, considering cardiovascular risk, blood sugar regulation, blood pressure, and waist circumference.9 Metabolic health is required for healthy aging. According to a study based on the National Health and Nutrition Examination Survey, the prevalence of metabolic health in American adults is alarmingly low, posing serious individual and public health concerns.9 Muscle tone, maintained through physical activity, can be a powerful regulator of insulin sensitivity and overall metabolic health, with short-term benefits stemming from each training session and long-term adaptations that improve overall body composition.

The skeletal muscle is the largest metabolic tissue in the human body and an important site for glucose metabolism, both during exercise and while at rest.10 It is also regarded as an endocrine and paracrine organ, where muscle-derived proteins, called myokines, facilitate interactions between the muscles and other organs, such as the liver, pancreas, skeletal system, adipose tissues, and the cardiovascular system. During exercise, the muscles are used as a source of fuel for both muscle-stored glycogen and circulating plasma glucose, activating glucose metabolism through insulin-dependent mechanisms and optimizing insulin response and glucose oxidation.11 Routine exercise

further increases skeletal muscle tone, oxidative capacity, and mitochondrial biogenesis, leading to even greater glucose uptake.12 Muscles also influence energy and protein metabolism throughout the body by acting as a reservoir of amino acids, which can be released when needed elsewhere in the body.2 An observation study found that lower muscle mass was a risk factor for metabolic syndrome among people over 65 with normal body mass index.13 Regular exercise and improved muscle tone may improve metabolic health and reduce the risk for metabolic disease, including type 2 diabetes, obesity, metabolic syndrome, and nonalcoholic fatty liver disease.11

What are some nutrients that can support muscle tone and healthy body composition for healthy aging?

Collagen peptides have been shown to support the maintenance of collagen, the primary structural component of connective tissues, accounting for as much as 30% of the body’s total protein.14 Collagen is responsible for muscle flexibility and adaptation and provides structure, support, and strength to muscles, helping them function properly.15

An RCT (n = 53) found that male subjects with sarcopenia who were supplemented with collagen peptides following resistance training achieved improved body composition by increasing lean body mass more than the control group, who completed the same resistance training but without the collagen peptide supplementation.16 Another RCT (n = 77) investigated the effect of collagen peptide supplementation on the body composition and muscle strength of premenopausal women and found that subjects who received the collagen peptides increased lean body mass, hand-grip strength, and leg strength significantly more than the control group.17

Creatine is primarily stored in skeletal muscle and involved in muscular contractions. Both muscle strength and creatine levels naturally decline with age.18 Supplementation has been shown to increase intramuscular creatine concentrations, high-intensity exercise capacity, and lean body mass.19 A metaanalysis showed that older adults who receive creatine supplementation had increased lean body mass and muscle strength during resistance training. 20

Leucine is the essential amino acid that is considered to be the most anabolic.21 For effective muscle stimulation and protein synthesis, the International Society of Sports Nutrition recommends resistance exercise and adequate protein consumption in the range of 1.4 to 2 grams per kilogram of body weight per day (g/kg/d). A higher protein intake of 2.3 to 3.1 g/kg/d may be needed to maximize the retention of lean body mass in resistance-training individuals, where the protein source should contain 700 to 3,000 mg of leucine, in addition to a balanced content of all essential amino acids.22 A systematic review of 16 RCTs found that individuals 65 years or older supplemented with leucine experienced beneficial effects on body weight and lean body mass.23

Other nutrients that may support muscle tone and a healthy body composition include peptides from the fava bean (Vicia faba), whey protein, β-hydroxy βmethylbutyrate, vitamin D, and geranylgeraniol (GG).24–28 As the body ages, a combination of regular exercise, a nutrient-dense whole foods diet, and targeted supplementation, when needed, can support muscle health and overall well-being.

How IgG/C3d Food Sensitivity Screening Can Reduce General Inflammatory Symptoms

Abstract

Objective: To evaluate the effectiveness of an IgG/ C3d-based elimination diet in reducing inflammatory symptoms. Methods: Food sensitivities of 74 participants in Chengdu, China were identified by screening for IgG/C3d against a microarray of food antigens. Participants complied with personalized elimination diets based on the results, and changes to initial reported symptoms were evaluated. Results: Most participants (82.4%) reported a reduction in symptom severity following the elimination diet,

suggesting an association with IgG/C3d mediated food sensitivity and the adverse symptoms studied. Symptoms were categorized by the body system affected; gastrointestinal, dermal, nasal, neurological, musculoskeletal, body weight, insomnia, fatigue, and other. Conclusions: Reductions in inflammatory symptoms were observed after subjects underwent IgG/C3d-based elimination diets, supporting further research into the relevance of IgG/C3d as a sensitive biomarker of food sensitivity.

How IgG/C3d Food Sensitivity Screening Can Reduce General Inflammatory Symptoms

Introduction

Food hypersensitivity constitutes a diverse range of inflammatory symptoms that affect 5-20% of the global population.1 Distinct from Type I IgE mediated food allergy or food intolerance, hypersensitivity is immunoglobulin G (IgG) mediated, and may trigger a range of delayed symptoms, including gastrointestinal discomfort, joint pain, brain fog, and dermatitis. Most patients show delayed sensitivity to at least one of the eight food antigens (milk, eggs, fish, crustaceans, tree nuts, peanuts, wheat, and soybeans), which are responsible for 90% of food sensitivities worldwide.2 In addition, a rise in hidden allergens that may not appear on food labels may increase the difficulty of detecting food-related delayed hypersensitivities and identifying offending foods.3 Food sensitivities may lead to a lower quality of life and require careful personalized dietary and lifestyle management from a healthcare professional.

IgG-mediated food sensitivity is involved in two of the four types of hypersensitivity. Type I hypersensitivity is mediated by Immunoglobulin E (IgE), and is associated with the acute, adverse allergic reaction that results in histamine release and systemic or localized anaphylaxis.4 Type II hypersensitivity is a cytotoxic response mediated by IgG and immunoglobulin-M (IgM) antibody, and Type III hypersensitivity involves IgG or IgM immune complex reactions. Type IV sensitivities are hypersensitivities that are delayed cell-mediated immune responses in which T-cells mount inflammatory responses against endogenous or exogenous antigens.5 There is evidence to support that Type II and III hypersensitivities have significant inflammatory implications due to the activation of the complement system.

While the immunological mechanisms behind food sensitivities are debated, it is thought that the complement system may be one pathway through which certain foods trigger systemic inflammation.6 The complement system is an essential interface between the adaptive and innate immune systems as these systems produce proteins that guide leukocytes to infection sites.7 In the classical pathway, IgG1,3 activates the complement cascade, producing C1q leading to a conformational change that causes autocatalytic enzyme activity. Protein subunits

produce a serine zymogen that cleaves the protein complex C4 to the subunits C4a and C4b. C4b forms a complex that cleaves circulating C2 into C2a and C2b. C2b binds to C4b to form C4b2a (C3 convertase) that then cleaves circulating C3. C3 is then cleaved into the protein subunits C3a and C3b. The former is a peptide mediator in inflammation (anaphylatoxin) for the recruitment of neutrophils and monocytes while the latter covalently opsonizes to the membranes of pathogens. C3b then covalently binds to C3 convertase to form the protease C5 convertase which then cleaves circulating C5. C3b also opsonizes to pathogen surfaces to tag for phagocytosis. In the C5 convertase complex, circulating C5 and is then cleaved into C5a and C5b .8 Like C3a and C2a, C5a is an anaphylatoxin while C5b binds to C9 and forms a membrane attack complex, causing the lysis of pathogens.9 C3b can be cleaved by the plasma protein complement factor 1 (CF1) in concurrence with complement receptor 1 (CR1) and membrane-factor of proteolysis (MCP) into inactive fragments of iC3b.8 Furthermore, CF1 and CR1 cleaves iC3b into C3dg. Serum proteases cleave C3dg in the protein subunit C3d.9 This particular protein subunit binds to antibodies such as IgG and initiates B-cells to attack the bound pathogens.10

Complement receptors CR2 and CD25 coordinate with B-cell antigen receptors (BCR) to respond to antigens bound to C3d, which enhances the immune response when conjugated with an antibody.10,11 When antibody/ C3d complexes bind to pathogen cell surfaces, B-cells are recruited to the binding site and the threshold to produce antibodies is lowered. 12-13 This results in an increase in antibody production from B cells and an enhancement in the IgG-mediated inflammatory response. 4 In this way, measuring IgG/C3d immune complex may be indicative of food-related inflammation. 9

Research also suggests that intestinal permeability and epithelial dysfunction plays a role in mediating food sensitivities, which may manifest in conditions such as Crohn’s disease and irritable bowel syndrome (IBS). Intestinal permeability is a functional attribute of the digestive system allowing for nutrient absorption while maintaining a barrier to prevent potentially harmful lumen products from entering circulation.14 When inconsistent fluctuations of permeability occur for extended periods

of time and there is a decline in essential structural and regulatory protein expression, intestinal permeability occurs. Dietary factors can influence changes in gut microbiota that can negatively influence permeability.15 Delayed food sensitivities (i.e. Types II and III) can initiate a change in microbial species diversity and population proportions in the gut.16 An imbalance within the gut microbiota can compromise tight junction (TJ) proteins, such as occludin, claudin-3,-4,- & -5, as well as regulatory proteins like zonulin, leading to a breakdown of TJ barriers in the digestive lining. 15

Increased intestinal permeability as a result of food-related inflammation can cause ions, proteins, gut microbiota, and other lumen products to translocate into the bloodstream which triggers the formation of IgG/C3d immune complex through the complement pathway. Regulatory proteins, particularly zonulin, play a crucial role in controlling the degree of intracellular structure along digestive epithelia. When zonulin proteins permit excessive transport of intestinal contents, the immune system can generate IgG/ C3d responses that are associated with inflammation .9 Impaired intestinal permeability has links to gastrointestinal conditions such as Crohn’s disease and ulcerative colitis. In the former, reduced expression of occludin, claudin-3,-5, & -8 have been observed while the latter shows a reduced expression in claudin-1 & -4, along with an increased expression of pore-forming claudin-2. 15 It should be noted that less than 10% of patients with genetic susceptibility to intestinal permeability develop the condition, and environmental factors, possibly food antigens absorbed through the gut epithelia, are most likely to contribute. Evidence supports that such dysfunction of paracellular structural proteins could be linked to diseases such as Type 1 diabetes, Celiac disease, and even some cancers.17 Studies suggest that IgG-mediated food sensitivities may be associated with a wide range of chronic inflammatory disorders, including dermatitis and gastrointestinal distress, and may result from sensitivities to the same types of food.18 In addition, chronic inflammatory diseases such as arthritis and multiple sclerosis may be associated with food sensitivity as a result of intestinal permeability. 19 Recent research has found that sleep deprivation may be associated with inflammatory conditions, including inflammatory bowel disease (IBD), gut microbiome dysbiosis, and obesity.20 It is suggested that sleep deprivation that results

from chronic inflammatory conditions may also exacerbate these symptoms.21 In addition, patients with diseases such as rheumatoid arthritis and hepatitis, IgG levels are elevated. Even asymptomatic patients who are positive for IgG antibody against certain foods may be at risk for developing conditions later in life.22

Research has shown that elimination of IgG-specific foods through elimination or rotation diets can mitigate adverse clinical symptoms and reduce food-specific IgG titers leading to improvements in patients’ health.9 IgGbased elimination diets may be therapeutically effective in treating IBS, as well as quality of life, anxiety and depression, and non-gastrointestinal inflammatory conditions, as compared to alternate or sham dietary approaches.23 Compliance with IgG-based elimination diets may significantly reduce GI symptoms in IBS populations.24 Compared to low FODMAP and general healthy eating diets, IgG elimination diet was shown to lead to a significant decrease in symptoms.25 IgG-based elimination diets may also reduce migraine symptoms, as well as abdominal pain in participants with IBS.26,27

Recent work has also shown that most patients suffering from Crohn’s disease show improved symptoms following a food-specific elimination diet.28,29 With regards to atopic dermatitis, IgG has been shown to increase the levels of cutaneous lymphocyte-associated antigen (CLA) that subsequently influences the thymic maturation of certain types of T-cells. Since CLA is known to be a peripheral biomarker of atopic dermatitis, there are possible associations between IgG mediation and certain skin conditions.30 Research has also demonstrated that elimination diets based on IgG-mediated sensitivity has an influence on asthma-like symptoms such as wheezing and rhinitis, as well as rheumatoid arthritis.31,32

Through the gut-brain axis, IgG complexes against food antigens may also play a role in neuroinflammation and associated conditions, including major depression, Parkinson’s disease, Alzheimer’s disease, and epilepsy.33 Dysregulation of metabolic pathways from the central nervous system (like that of the kynurenine pathway for tryptophan metabolism) can lead to the production of neurotoxic metabolites (i.e. quinolinic acid) which plays a part in the progression of neurological

disorders.14 Based on the growing evidence of possible associations with many prevalent and major conditions, IgG-mediated food studies are pertinent in many areas of healthcare research, such as neurology and rheumatology.

Despite existing research, further studies are needed to support a causative relationship between IgG against dietary antigens and food hypersensitivity. Contradictory research suggests that certain food-specific IgG responses can have a protective effect against adverse symptoms. For example, some studies suggest a link with crabspecific IgG and eczema, yet an inverse relationship was shown with tomato-specific IgG and rhinitis.34 Evidence is limited, and studies have challenged the association between IgG and gastrointestinal conditions.35 An increase in research pertaining to IgG-mediated food sensitivity could help answer possible correlations with seemingly unrelated conditions.

It should be made clear that the presence of IgG alone could reflect exposure to dietary foods in both symptomatic and asymptomatic patients, as the production of IgG is likely a typical reaction to the immune system’s frequent encounter of those foods.36 For this reason, IgG/C3d immune complex (IC) may be a more sensitive marker in the identification of delayed food hypersensitivity reactions by measuring activation of the complement system (and thus inflammation), in addition to the adaptive immune response.

Materials & Methods

Subjects This prospective intervention study was carried out by Alanber Health Company, Ltd., in Chengdu, China, between the months of November 2021 and May 2023. All participants resided in Chengdu, China, and were existing patients of the Alanber Health Company clinic. Study participants’ ages ranged from 1-68 years old, and 39% were female.

The criteria for inclusion in the study was the presentation of symptoms prior to screening for IgG/C3d-IC against food antigens and additives, as well as full compliance with the 3-month elimination diet protocol and no consumption of dietary supplements. There was no exclusion criteria based on age, gender, or geographical location. The self-reported initial presenting symptoms were categorized based on the body system affected (Table 1). Participants reported symptoms affecting one or more body systems.

The current study aims to evaluate the therapeutic effectiveness of an elimination diet based on the IgG/C3d immune complex (IC) against 176 food antigens.37 To date, the clinical efficacy of an IgG/C3d-based elimination diet has not been evaluated. IgG/C3d-IC as a biomarker of food sensitivity may provide a more sensitive and targeted approach to identifying offending foods that activate not only the adaptive immune system, but also the innate inflammatory immune response.38 These findings may suggest that the eliminations diets based on IgG/C3d-IC are a more effective method of reducing inflammatory symptoms and have significant implications for personalized clinical dietary interventions.

Gastrointestinal symptoms represented the most frequently observed body system affected, followed by dermal, nasal, and neurological symptoms (Table 2). Less frequently reported symptoms included insomnia, musculoskeletal, general fatigue, and weight maintenance. 17% (13) reported “other” symptoms, including sore throat, itchy eyes, halitosis, arrythmia, coughing, asthma, and anxiety.

455 patients were initially screened for food sensitivities, with 39 presenting few or no symptoms and 255 with no record of symptoms (Figure 1). Out of the 161 participants presenting symptoms, 87 were compliant with the 3-month elimination diet protocol. 12 participants were lost to follow up after the elimination diet phase, leaving a total

of 74 participants for inclusion in the study. Informed consent was obtained by all subjects participating in the study, ensuring that subjects understood the purpose, significance, limitations, and risks of the test and diet management intervention.

Processing of the bloodspot samples included elution in extraction buffer for 2 hours at 2-8°. Samples were added to pre-coated 96-well ELISA plates and allowed to incubate overnight at 2-8°. The unbound sample was washed off using a wash buffer, and HRP-conjugated secondary antibody was added at working dilution. Unbound secondary antibody was washed off and colorimetric TMB substrate was added. An acid solution was added to halt the enzymatic reaction. Plates were read at 450nm.

Sensitivity to specific foods and additives was determined statistically by comparing reactivities between foods within a given sample. Antigens were graded on a scale of 0 (no reactivity) to 4 (maximum reactivity) based on statistical criteria, with 0 indicating no sensitivity, 1+ indicating low sensitivity, 2-3+ indicating moderate sensitivity, and 4+ indicating maximum sensitivity.

IgG/C3dg Antibody Detection against dietary antigens and additives Each study participant was screened for IgG/ C3dg-IC against a panel of 176 different dietary foods and additives using an enzyme-linked immunosorbent assay (ELISA)16. Sample types collected included dried bloodspots only. Briefly, bloodspot samples were collected using a contact-activated lancet to pierce the fingertip. 5 free-falling drops of capillary blood were dropped onto the filter paper of a 503 Bloodspot Card (Eastern Business Forms; Cat #: 10550021). The blood sample was allowed to dry for several hours at room temperature before shipment at ambient temperature.

Elimination Diet Protocol

For each test, a dietician interpreted the test results and issued a 3-month diet management program to each participant. The diet management program consisted of 2 phases: removal and replacement, followed by re-introduction. Initially, subjects were instructed to remove all foods having sensitivities of 2+, 3+, or 4+ from their diet for a minimum of 6 weeks. During this period, alternative sources of nutrients were included in the diet to avoid nutritional imbalance.

The reintroduction phase involved the reintroduction of 1 eliminated food at a time into the diet, followed by one week of observation where participants noted any changes in their initial symptoms. The concurrent introduction of multiple foods was not recommended. If no recurrent symptoms were observed, the specific food may be included in the diet moving forward. If recurrent symptoms were observed, it is assumed that the participant may have a food sensitivity against the reintroduced food, and the implicated food should be entirely removed from the diet. The reintroduction and observation cycles were repeated for each identified offending food until

all foods had been assessed. The diet management program lasted for a minimum of 12 weeks, or total time required to assess all food sensitivities for recurrent symptoms.

Onsite or virtual follow-up visits were conducted for each participant at the conclusion of the diet management program to assess changes in initial presenting symptoms. All changes in symptoms were self-reported and interpreted by the treating dietician. Children’s symptoms were documented and reported to the treating dietician by their caregivers.

Results

Reported Changes in Symptoms Overall, 82.4% of all participants (n = 74) reported an improvement in one or more initial symptoms. Among those initially reporting gastrointestinal symptoms, 41 of the 46 participants (89.1%) experienced a decrease in symptom severity after complying with the elimination diet protocol (Table 3). Participants testified that “symptoms of flatulence, diarrhea, gas, and brain fog improved”, and some experienced “less frequent occurrence of abdominal pain” (translated from Mandarin, Chinese).

Of the 38 participants reporting dermal symptoms, 31 (81.6%) reported a reduction in symptom severity (Table 3). Participants reported improvements in the severity of eczema, acne, pruritis, cheilitis, urticaria, and the general condition of the skin. Of the less frequently reported symptoms including joint pain, insomnia, fatigue, and weight maintenance, 83.3% (5), 75.0% (6), 100.0% (5), and 100.0% (2) reported a decrease in symptom severity, respectively (Table 3).

Discussion

To date, this is the first research to assess how an elimination diet that employs IgG/C3d immune complex (IC) may identify potential delayed hypersensitivities to dietary foods and evaluates an array of inflammatory symptoms, as opposed to a singular condition. Prior studies suggest that IgG-based elimination diets may improve conditions such as migraines and IBS, but this is the first study to evaluate multiple inflammatory symptoms within the same study group using the IgG/C3d-IC biomarker.

General symptoms of inflammation were assessed, and self-reported changes in symptoms demonstrated that the severity of initial symptoms decreased in most patients following the IgG/C3d-based elimination diet. This data is consistent with prior studies, which suggest that IgG based elimination diets may improve symptoms of migraines, IBS, and arthritis. However, our study expands upon prior research by demonstrating that IgG/C3d-IC may serve as a more sensitive and specific biomarker to identify delayed hypersensitivity, as opposed to IgG only. The use of IgG alone against dietary antigens in the identification of food sensitivities is clouded by the potential for elevated serum IgG due to exposure alone.17 For this reason, IgG/ C3d-IC demonstrates a more sensitive marker in the identification of inflammatory foods that may be at the root of inflammatory symptoms.16

These preliminary results support the continued assessment of IgG/C3d-IC as a clinically relevant biomarker for food sensitivity and suggest that elimination diets based on this marker may have clinical utility in reducing inflammatory symptoms. Throughout the sample collection procedure and elimination diet protocol, no adverse reactions were reported, supporting the safe integration of the protocol into clinical practice.

While the results of the current study are promising, they warrant continued evaluation through increasingly rigorous methods. Additionally, the diet protocol was conducted by trained dieticians that facilitated the

implementation and compliance with the elimination diet, which may not mirror “real-world” conditions in which patients do not receive coaching. Future studies should also target geographically diverse populations, which would increase the generalizability of the findings outside of the study population.

Conclusions

In summary, our data provide compelling evidence of the clinical utility of IgG/C3d-IC as a biomarker of delayed

food hypersensitivity. Removal of identified dietary antigens from the diet may improve general symptoms of inflammation, suggesting the clinical utility of IgG/C3d-IC diagnostic testing. Further exploration of IgG/C3d-IC as a clinically useful biomarker is essential to expanding its integration into clinical practice, particularly in the sphere of personalized medicine.

Introduction

Aging is an inevitable process that impacts metabolic health, muscle function, and fat distribution. Traditional weight-centric approaches to health assessment, particularly the reliance on Body Mass Index (BMI), fail to capture the complexities of body composition changes that occur with aging.¹ While BMI can be used for screening and population estimates, its limitations in distinguishing between fat mass and lean body mass make it inadequate for

evaluating an individuals’ metabolic health, physical function, and disease risk.²

As interest in longevity and metabolic resilience grows, Body Composition Analysis (BCA) is emerging as an essential tool in anti-aging medicine. By differentiating between skeletal muscle mass, visceral adipose tissue (VAT), and body fat percentage, BCA allows for more precise risk stratification and treatment

planning.³ Unlike BMI, which does not account for agerelated muscle loss (sarcopenia) or shifts in fat distribution, BCA provides quantitative insights into muscle quality, fat accumulation, and hydration status, enabling clinicians to implement targeted, data-driven interventions.⁴

This article explores the limitations of BMI in aging populations, the clinical importance of body composition metrics for longevity, metabolic health, and functional independence, and the role of BCA in optimizing hormonal interventions, including testosterone replacement therapy (TRT) and hormone replacement therapy (HRT).

The Limitations of BMI in Aging Populations

For decades, BMI has served as a standard metric for classifying individuals into weight categories based on height and weight. However, BMI does not distinguish between adipose and lean body mass, making it an unreliable measure of metabolic health.⁵ Studies have demonstrated that individuals with similar BMI values may have vastly different body compositions, with varying degrees of muscle mass, visceral fat, and overall metabolic risk.⁶

Furthermore, BMI fails to account for sarcopenia, the age-related loss of skeletal muscle, which contributes to frailty, insulin resistance, and functional decline.⁷ Research suggests that individuals with normal-range BMI but low muscle mass (sarcopenic obesity) face higher mortality risks than those classified as overweight with preserved muscle mass.⁸ Additionally, BMI does not provide insights into fat distribution, an important factor in cardiovascular risk and metabolic disease.⁹ Visceral adipose tissue (VAT), rather than overall fat mass, is more strongly associated with insulin resistance, inflammation, and cardiometabolic disease, yet BMI lacks the specificity to differentiate VAT from subcutaneous fat.¹⁰

Given these limitations, reliance on BMI alone may lead to misclassification of metabolic risk in aging populations, reinforcing the need for BCA in clinical practice.

The Role of Body Composition in Longevity and Metabolic Health

Muscle mass is a strong predictor of longevity and functional independence in older adults. Studies have shown that greater skeletal muscle mass correlates with improved glucose metabolism, reduced inflammatory markers, and lower risk of frailty and disability.¹¹ Conversely, low muscle mass and excessive visceral adiposity are associated with metabolic dysfunction, increased fall risk, and higher mortality.¹²

Visceral fat accumulation, in particular, is a hallmark of metabolic aging. VAT secretes pro-inflammatory cytokines, contributing to chronic low-grade inflammation, insulin resistance, and endothelial dysfunction, which accelerate the aging process.¹³ Research indicates that VAT is a stronger predictor of cardiovascular disease, type 2 diabetes, and allcause mortality than BMI or total fat mass.¹⁴

BCA enables clinicians to assess visceral adipose tissue, offering a more comprehensive picture of metabolic health. By tracking changes in skeletal muscle mass and VAT over time, providers can tailor interventions to preserve muscle mass, optimize metabolic function, and mitigate the risks associated with visceral adiposity.

Preventing Sarcopenia and Functional Decline

Sarcopenia is a primary contributor to frailty and disability in older adults and has been recognized as a major public health concern.¹⁵ The progressive decline in skeletal muscle mass and strength that accompanies aging is associated with higher rates of falls, fractures, loss of mobility, and premature mortality.¹⁶ Moreover, muscle loss has significant metabolic consequences, as skeletal muscle is the primary site of glucose disposal.¹⁷ Declines in muscle mass contribute to impaired insulin sensitivity, increased adiposity, and heightened inflammation, compounding the risk for metabolic syndrome and cardiovascular disease.¹⁸

Weight loss interventions that fail to account for muscle preservation may exacerbate sarcopenia, worsening overall health outcomes.¹⁹ Traditional caloric restriction without concurrent resistance training and adequate protein intake can result in loss of lean mass, decreased strength, and metabolic slowing.²⁰

BCA provides quantitative assessment of skeletal muscle mass, allowing for early detection of muscle deterioration and the implementation of targeted interventions. Clinicians can tailor resistance training protocols, optimize protein intake, and, when necessary, implement hormonal interventions to maintain muscle function and prevent disability.

As BCA provides a quantitative measure of lean mass, it also serves as an essential tool for assessing the impact of hormone-related interventions, particularly testosterone replacement therapy (TRT) and hormone replacement therapy (HRT). A growing body of evidence suggests that muscle loss and fat redistribution seen with aging can be mitigated with hormonal support, further emphasizing the need for precision-driven interventions.

The Utility of Body Composition Analysis in Hormone Management

Testosterone Replacement Therapy (TRT) and Body Composition

Testosterone plays a critical role in maintaining lean body mass, muscle strength, and fat distribution.²¹ With aging, testosterone levels decline, leading to muscle atrophy, increased visceral adiposity, and reduced metabolic efficiency.²² Hypogonadal men are more likely to experience sarcopenia, insulin resistance, and increased VAT accumulation, placing them at higher risk for cardiometabolic diseases.²³

Testosterone replacement therapy (TRT) has been shown to increase lean muscle mass, reduce VAT, and improve insulin sensitivity in hypogonadal men.²⁴ However, response to TRT varies based on baseline body composition, activity level, and concurrent metabolic conditions.²⁵

BCA serves as a valuable tool in monitoring TRT outcomes, allowing clinicians to: assess baseline muscle mass and fat distribution before initiating TRT, track improvements in lean mass and VAT reduction over time, and identify nonresponders and adjust therapy accordingly. Incorporating BCA-guided assessment into TRT protocols ensures more precise, individualized therapy, optimizing both muscular and metabolic benefits.

Hormone Replacement Therapy (HRT) and Fat Distribution

Women throughout the menopause transition frequently experience shifts in fat distribution, characterized by increased VAT and decreased lean mass.²⁶ Estrogen plays a key role in regulating fat metabolism, and its decline during menopause contributes to redistribution of fat toward the abdomen, increasing metabolic risk.²⁷

Hormone replacement therapy (HRT) has been shown to attenuate VAT accumulation and improve metabolic markers in postmenopausal women.²⁸ However, the effects of HRT on muscle mass and overall body composition vary among individuals, necessitating objective tracking.²⁹

BCA provides clinicians with a quantitative measure of HRT-induced changes in body composition, allowing for better assessment of therapy effectiveness and adjustments based on individual response.

Conclusion

As anti-aging medicine focuses on a precision-based approach, reliance on BMI as a singular health metric is becoming increasingly outdated. BCA offers a superior method for evaluating muscle mass, fat distribution, and metabolic health, providing clinicians with the objective data necessary to implement personalized, evidence-based interventions.

Recent discoveries in telomere biology have sparked renewed interest in leveraging telomerase reverse transcriptase (TERT) to address the underlying mechanisms of aging. Telomeres, which cap and protect the ends of chromosomes, shorten with each cell division and are influenced by oxidative stress. Telomerase, particularly its catalytic subunit TERT (also called hTERT in humans), can lengthen

telomeres in certain cell types and has been shown in animal models to reverse some phenotypes of aging. Recent studies highlight that TERT’s “noncanonical” activities—which include mitochondrial protection, regulation of reactive oxygen species (ROS), and epigenetic modulation—may be as critical as its canonical telomere-lengthening function. Introduction

Aging is a multifactorial process characterized by cumulative cellular damage, genomic instability, and a gradual decline in the body’s ability to repair and renew tissues. Among the diverse mechanisms that drive aging, telomere attrition has received significant attention since Leonard Hayflick’s pioneering work in 1961 revealed that human somatic cells possess a finite replicative lifespan.¹

Telomeres are repeated DNA sequences (TTAGGG) that protect chromosome ends. Telomeres shorten progressively with each cell division, eventually triggering cellular senescence or apoptosis.

Telomerase is the ribonucleoprotein enzyme complex primarily responsible for maintaining telomere length. In adult humans, telomerase activity is significantly repressed in most somatic tissues. However, it remains active in stem cells, germline cells, and certain highly proliferative tissues. Telomerase consists of two main subunits:

• Telomerase reverse transcriptase (TERT): the enzyme that adds new DNA repeats to the ends of chromosomes.

• Telomerase RNA component (TERC): the RNA blueprint for creating those repeats.

The following section examines how TERT may mitigate a broad range of aging mechanisms, not only by lengthening telomeres but also by modulating epigenetic regulation, cellular metabolism, and mitochondrial function. Evidence from rodent models and in vitro studies suggests that small-molecule telomerase activators can partially restore TERT function in aging tissues. These results pave the way for new therapies designed to extend and improve healthy human lifespans.

Telomeres and the Hayflick Limit

In his seminal work, Leonard Hayflick demonstrated

that human fibroblasts in culture have a finite replicative capacity—generally around 60 population doublings—before entering senescence.¹ Further studies show that telomeres serve as the “molecular clock” behind this limit. Telomeres shorten with each round of cell division, primarily due to:

• End-Replication Problem: DNA polymerase cannot fully replicate the 3’ end of linear chromosomes.

• Oxidative Stress: Telomeric DNA, particularly the guanine-rich G-strand, is highly susceptible to ROS.

• Stem Cell Turnover: Tissues with high proliferative demands (e.g., bone marrow, gut epithelium) experience accelerated telomere shortening if telomerase is insufficiently active.

In humans, telomere length ranges between 8 to 13 kilobases (kb) at birth and gradually decreases with age. Critically short telomeres of around 4 to 5 kb often signal cellular senescence or apoptosis, contributing to tissue dysfunction and association with numerous age-related diseases.²

Telomerase: TERT and TERC

Canonical Function

Telomerase is composed of two main subunits: TERT (telomerase reverse transcriptase) and TERC (telomerase RNA component). During embryonic development, telomerase is highly active, but in most adult somatic cells, TERT is repressed. A partial reactivation of telomerase in adult life can help maintain telomeres in specific tissues, such as hematopoietic stem cells. However, overall telomerase levels in most adult cells remain insufficient to prevent telomere erosion entirely. This insufficiency is further compounded by natural declines in telomerase activity, estimated at around 50% by middle age.

Non-Canonical Functions

Beyond its canonical role in telomere maintenance, TERT has been implicated in an array of “non-canonical” functions, including:

• Mitochondrial Protection and Metabolism:

TERT may translocate to mitochondria, reducing oxidative damage to mitochondrial DNA and improving cellular stress resistance.

• Epigenetic Regulation: TERT can influence DNA methylation patterns by modulating enzymes such as DNMT3b, thereby affecting gene expression and cell fate.

• Stress Response and Senescence: TERT appears capable of repressing or delaying cellular senescence markers like p16.

These additional functions suggest that turning telomerase back on in certain situations could help slow down aging throughout the body, not just by lengthening telomeres.

Lessons from Telomerase Knockout and Reactivation Mouse Models

Telomerase Knockout Models

Laboratory mice naturally have very long telomeres (50–70 kb), which can make them less representative of human aging. Researchers overcame this by creating telomerase knockout (KO) mice lacking TERC.³ After several generations, these KO mice developed significantly shorter telomeres, leading to premature aging phenotypes in tissues with high turnover (e.g., bone marrow, gut epithelium). Additional features included a progressive decrease in median lifespan and exacerbated degenerative pathologies over successive generations.

Reactivation and Age Reversal

In another groundbreaking study, researchers activated TERT in telomerase-deficient mice, showing that telomerase reactivation reversed multiple aging issues— including testicular atrophy, splenic atrophy, and neuronal decline.⁴ Similarly, gene therapy with viral vectors expressing TERT has demonstrated improvements in insulin sensitivity, osteoporosis, and neuromuscular coordination in normal aging mice.⁵ Remarkably, increasing TERT activity in these 1- and 2-year-old mice prolonged their median lifespan by 24 and 13%, respectively.

These findings highlight the possibility that restoring TERT activity to youthful levels may improve or even

reverse specific age-related pathologies in mammals.

TERT and the Hallmarks of Aging

The “Hallmarks of Aging” describes nine interconnected mechanisms—such as genomic instability, stem cell exhaustion, and altered intercellular communication— that collectively drive the aging process.⁶ TERT appears to intersect with many of these hallmarks:

• Genomic Instability: By maintaining telomeres and protecting chromosome ends, TERT reduces the DNA damage response that triggers cell senescence.

• Cellular Senescence: TERT can repress p16 and other senescence markers, mitigating the accumulation of senescent cells.

• Epigenetic Alterations: Through interactions with epigenetic regulators, TERT influences chromatin structure and gene expression, potentially reprogramming cells toward a more youthful state.

• Mitochondrial Dysfunction: TERT’s role in maintaining mitochondrial health extends its protective effects beyond the nucleus.

Ongoing research indicates that modest TERT reactivation can significantly mitigate aging by stabilizing the genome, reducing inflammation, and supporting stem cell function.

Small-Molecule Telomerase Activation

Telomerase Activating Compound (TAC) in Rodents

A screening of more than 650,000 compounds led to the discovery of several Telomerase Activating Compounds (TAC). In mouse models, one such compound demonstrated an approximately 40% increase in telomere length and a 50–90% reduction in the senescence marker p16. Short-term TAC treatment improved hippocampal gene expression related to neurogenesis, while longer-term administration in older mice enhanced cognition and neuromuscular function.7 While these preclinical results are promising, TAC safety and efficacy in humans remain unknown. The compound was administered intraperitoneally, which may limit direct translation to oral or other more patient-

friendly administration routes. Rigorous toxicology studies and clinical trials are necessary to evaluate TAC’s potential in human longevity interventions.

Human-Applicable Approaches

Researchers are focusing on controlled, short-term telomerase activation. Although some plant extracts and synthetic compounds show promise in laboratory tests, extensive, well-designed clinical trials are still needed to verify their safety and effectiveness in humans.

TA-65: A Proven Commercially Available Telomerase Activation Product

TA-65 is a patented, all-natural, plant-based compound that can help maintain or rebuild telomeres that diminish as people get older. It has been shown to transiently activate telomerase and is formulated as a 99% pure extract.

• In telomeric repeat amplification protocol (TRAP) assays, TA-65 produces about a 3-fold increase in telomerase activity at 0.1 mcMol, making it a moderate telomerase activator in neonatal foreskin keratinocytes and fetal lung fibroblasts.

• Serum levels in humans reach the middle concentration range after a single dose, suggesting it could be relevant for clinical applications.

• TA-65 was one of the earliest telomeraseactivating molecules to undergo extensive study and is often considered the “gold standard” in the field. Over 12 published studies, including double-blind, placebo-controlled trials, have investigated TA-65’s impact on immune function, metabolic health, and other markers of aging.

Key Double-Blind, PlaceboControlled TA-65 Studies

• Immune Senescence in Healthy Adults

• A randomized, double-blind, placebo-controlled clinical trial with 500 healthy volunteers demonstrated that daily intake of TA-65 significantly decreased senescent T cells (CD8+CD28-) compared to placebo. Subjects

taking TA-65 saw meaningful improvements in immune markers with no product-related toxicity or serious adverse events.8,9

• TACTIC Trial in Post-Myocardial Infarction (MI) Patients

• The TACTIC trial (phase IIa, single-center, parallel-group) enrolled 90 participants aged 65+ who had experienced an MI within the previous six months. Participants received 8 mg doses of TA-65 or placebo twice daily for 12 months.

• Key findings included:

• Significant increases in total lymphocyte counts, particularly CD3+, CD4+, CD8+ T-lymphocytes, B-lymphocytes, and NK cells.

• A 62% reduction in the inflammatory marker hsCRP compared to placebo.

• Fewer post-MI complications (e.g., chest or joint pain) and a 30% lower incidence of adverse events in the TA-65 group.

• These results suggest that telomerase activation and improved mitochondrial function may combat immunosenescence and chronic inflammation after MI, potentially aiding in recovery and long-term cardiovascular health.9

Current Clinical Implications and Future Directions

Although telomerase-activating therapies in humans remain in early stages, partially restoring TERT to youthful levels is a compelling concept. Future clinical goals include:

• Reducing Cellular Senescence: By lengthening critically short telomeres, TERT reactivation may decrease the burden of senescent cells linked to chronic inflammation (inflammaging).

• Enhancing Tissue Regeneration: Key tissues, including the hematopoietic system, skin, and nervous system, may benefit from mild, targeted increases in telomerase.

• Improving Metabolic and Mitochondrial Health: TERT’s non-canonical roles in mitochondrial function may help improve insulin sensitivity and reduce oxidative damage.

With TA-65 already on the market, individuals seeking a telomerase-activating approach have an option supported by multiple studies. Nonetheless, continued research in this area will help confirm optimal dosing and which populations might benefit the most—particularly among those recovering from conditions like myocardial infarction.

Conclusion

Telomere biology is once again drawing major attention in anti-aging medicine, with TERT now seen as central to maintaining or renewing healthy cell function. Evidence from animal models demonstrates that moderate

The Rise of Social Media Medicine

In this updated edition, Dr. Schwartz addresses a troubling new development: the explosion of health information and misinformation on social media.

“Since the first publication of Don’t Let Your Doctor Kill You, much has changed in the world,” she writes. “COVID, AI, and politics have brought about a huge shift in the way the public is involved in healthcare. Unfortunately, not much has improved the way doctors treat patients, and sadly an even more troublesome trend has emerged: the rise of biohackers, influencers, and longevity gurus who have free reign on social media.”

The aftermath of COVID has created an entirely new challenge for patients. The pandemic not only exposed the fragility of our healthcare system but unleashed a tsunami of self-proclaimed experts offering untested solutions. From “wellness influencers” with millions of followers to celebrity doctors who rarely practice medicine anymore, these new authorities often prioritize engagement and supplement sales over evidence-based advice.

Dr. Schwartz warns that while traditional doctors may

not know best, “a social media influencer probably knows even less.” She explores how both systems— traditional medicine and influencer culture—share disturbing similarities: exploiting fear, prioritizing profit over people, and making promises they can’t keep.

The GLP-1 weight loss drug saga illustrates this problem. While patients struggle with legitimate health conditions, pharmaceutical giants have seen their stock prices soar selling thousand-dollar medications. When shortages made these drugs unavailable, patients found affordable alternatives through compounding pharmacies—only to have the FDA abruptly declare the shortage “over” and crack down on these options.

This isn’t about patient safety; it’s about protecting pharmaceutical profits. These companies push unit dose packages specifically designed to put compounding pharmacies out of business. Their onesize-fits-all dosing is typically too high and lacks the personalization that compounding provides.

The Insurance Trap

Perhaps nowhere is the system’s dysfunction more evident than in the insurance industry. What began as a way to protect patients has morphed into a labyrinthine system that often blocks care while extracting maximum profit.

Dr. Schwartz shares the story of Lauren, whose comprehensive insurance coverage became a ticket to an endless carousel of specialists, tests, and procedures. After complaining of fatigue, Lauren entered a medical gauntlet that included blood tests, CT scans, MRIs, a bone marrow biopsy, and consultations with over twenty specialists—yet no one addressed her original complaint or coordinated her care.

“Once an insurance company is willing to pay for a system run amok,” Dr. Schwartz writes, “you are at risk of being broken down into millions of tiny little pieces, each with individual labels, without a clear treatment plan and absolutely no perspective or interest in improving your quality of life. It’s like donating your body to science while you’re still alive.”

She also reveals how the “game of documentation” has corrupted medical care. Doctors have learned to manipulate diagnosis codes to extract maximum payment from insurers, turning patients into collections of billable conditions rather than people needing help.

Victoria Reggio shares another heartbreaking example: her sister Emily faithfully paid premiums on a long-term care insurance policy for twenty years, only to have the insurer drag its feet when she needed coverage during her terminal illness. The company finally approved her claim—three days after she died. Emily had paid more than $100,000 in premiums but never received a penny of benefit.

Becoming an Empowered Patient

The brilliance of Dr. Schwartz’s book lies not just in identifying problems but in providing solutions. She offers practical guidance for transforming yourself from a “perfect patient” into an “empowered patient” who can navigate both traditional medicine and newer health information sources.

Both medical arrogance and influencer overconfidence can be dangerous, she warns. The path to optimal health often lies somewhere in between—taking what’s valuable from both conventional and alternative approaches while avoiding the pitfalls of each.

Both health care professionals and consumers interest in the quest for lengthening healthspan and performance health is escalating, the race against the disease of aging and its effects on metabolic balance is a constant pursuit to create countermeasures to preserve vitality. With aging, we often focus on the visible signs: wrinkles, graying hair, body contours and changes in posture. However, a less noticeable but more insidious process occurs known as “inflammaging.” This is the chronic, low-grade inflammation process that contributes significantly to the aging process and the onset of age-related diseases.

Inflammaging is not merely a symptom, but a process characterized by the chronic upregulation of the immune response, leading to a steady state of inflammation signaling resulting in damage tissues, organs and eventually the mitochondrial level. This process is often silent until significant damage occurs, manifesting as various diseases such as arthritis, osteoporosis, diabetes, heart disease, and neurodegenerative diseases.

Inflammaging is driven by metaflammation (metabolic inflammation). Metaflammation is triggered by metabolic changes associated with a

multitude of factors such as skewed diets, genetics, nutrient status, insufficient physical or excessive activity, persistent stress or in many cases environmental burden or infection. As the damage accelerates from subsequent inflammatory cytokine, neurochemical and hormone dysregulation, the evolution of dyslipidemia, muscle loss due to reduced IGF-1 production, alterations in iron and ferritin regulation, insulin resistance, bone loss, decrease in neurogenesis and the most critical damage occurs as mitochondrial renewal and mitochondrial energy production is compromised due to alterations in NAD/NADH ratios.

The Roots of Inflammaging and Metaflammation

The source metaflammation and inflammaging have multiple root causes most of the time. With some causality having greater urgency. It involves the innate immune system becoming increasingly dysregulated as we age or because hormonal disruption, lifestyle choices, environmental intoxication, and other factors accelerate the process. You can trigger this cascade prematurely and accelerate your biological age. Factors such as cellular debris, accumulation of senescent cells, and the production of reactive oxygen species contribute to this chronic inflammatory state. The immune system, designed to eliminate pathogens and resolve inflammation, begins to falter and, paradoxically, promotes inflammation resulting in damage to cells, organs and tissue.

Detecting and Measuring Inflammation

Understanding whether you are on the path to inflammaging or metaflammation involves recognizing their markers. Chronic inflammation can often be detected through biomarkers in the blood, such as C-reactive protein (CRP), interleukin-6 (IL-6), and tumor necrosis factor-alpha (TNF-alpha). Elevated levels of these markers can indicate ongoing inflammation and suggest an increased risk of agerelated conditions. Other metabolic markers such as mean platelet volume, glucose, insulin, galectin 3, adiponectin advanced lipid tests including Lpa, LP-PLAII2, oxLDL, ApoB and urine pH can paint a more complete picture of all the metabolic processes that may need to be addressed in order to return the body to homeostasis. One of the most important things to understand that it is not how

many therapies to apply, it is understanding the sequence of when to apply advanced protocols, whether it is peptides, senescent therapies, detoxification or mitochondrial targets. Continually pushing biochemical pathways through stimulation or inhibition can lead to negative feedback loops that could result in more cellular damage.

Strategies to Combat Inflammaging and Metaflammation

1. Dietary Adjustments: Adopting a diet rich in antiinflammatory foods is crucial. Testing for food allergies and sensitivities can be helpful and in some cases essential. Focus on a Mediterraneanstyle diet that includes plenty vegetables, modest fruit, nuts, whole grains as tolerated or needed based on activity level, and lean grass fed or wild caught protein sources. The core foundation to longevity is maintaining gut integrity. A dysregulated gut microbiome can lead to increased production of lipopolysaccharides (LPS). When the microbiome becomes stressed and have enhanced die off, LPS levels can increase. Combined with increased intestinal permeability (often referred to as “leaky gut”), more LPS enters the bloodstream attaching to tissues and crosses the blood brain barrier activating microglial cells resulting in neuroinflammation.

2. Regular Physical Activity: Exercise not only helps in managing weight but also in reducing metaflammation. Moderate, regular physical activity can lower the levels of pro-inflammatory cytokines and increase the production of anti-inflammatory cytokines. In addition, exercise stimulates the glymphatic system to help clear by-products of brain metabolism. The harder you train the more you need to focus on recovery. Over training can be just as damaging as not exercising.

3. Adequate Sleep and Stress Management: sleep disturbances and high stress are significant contributors to inflammation. Insomnia is considered a disorder of hyperarousal, the brain takes on too much sympathetic stress and cannot shift to deep wave sleep preventing circadian regulation of the suprachiasmatic nucleus, release of growth hormone and disrupts insulin response. Ensuring sufficient sleep and managing

stress through techniques such as meditation, yoga, or even 4 minutes of box breathing 2-3 times a day helps to reset the vagus nerve and restore parasympathetic tone. Supporting nutrients such as glycine, inositol, magnesium, theanine, relora and melatonin can have a signficnat impact on sleep hygiene

4. Supplements: There are so many supplements, peptides and bioregulators that can help correct metaflammation and inflammaging. The key to the success of these nutrients is learning what to take and how to know it is working. Synapsin for cognition, BPC 157, KPV and curcumin for supporting inflammation or the repair of tissues, adaptogens, aged garlic extract, S-acetyl glutathione, the list goes on and on. One of the biggest mistakes I have seen in my 40 years of precision health and biohacking is that people will take too many compounds without a specific goal or a desired result.

5. Pharmacological Interventions: Pharmacological treatments may be necessary to manage chronic inflammation and they do play a role in longevity health. This is where compounding pharmacy and other services shine. Whether it is for peptides, methylene blue, bio-identical hormones or rapamycin, at times these interventions can accelerate results and reverse meta inflammatory processes.

Inflammaging and metaflammation are not just buzzwords but real phenomena that affect health resilency, healthspan and longevity. By understanding these processes and integrating practices into daily life, the march toward the disease of aging can be minimized. As I often say, managing inflammation is not just about extending life— it’s about enhancing the quality of every year we gain.
