It is wonderful to be able to celebrate together the outstanding contributions of tonight’s Honorees, Community Care Rx and D&J Transportation, who tirelessly champion our mission. We are also immensely thankful to our dedicated Board of Directors, notably Andrew Zimmern, our esteemed chefs, donors, and all who have made this evening possible.
S:US plays a crucial role in fostering a city where New Yorkers, regardless of circumstance, can lead purposeful lives. The people we serve include survivors of domestic violence, individuals experiencing homelessness, those living in poverty, veterans facing homelessness and other challenges, individuals dealing with mental health and substance use issues, people with intellectual/developmental disabilities, and those living with HIV/AIDS. Since our founding in 1978, S:US has grown to be among New York City’s largest and most-respected non-profit health and social service organizations, serving tens of thousands of New Yorkers in need every year across every borough and on Long Island.
We have set an ambitious goal of leading in the effort to Eradicate Homelessness for New Yorkers. Currently, more than 26,000 households in our city have been living in a shelter for more than a year. Over the next ten years, we’ll be working with partners across the city to reduce this number to fewer than 5,000 people, primarily by opening thousands of new supportive housing units each year. We are building a robust pipeline of projects to help us meet this goal, including purchasing 4 development sites in the past 12 months. That is in addition to the 1,000 units we will open in 2025, with almost 800 already opened.
At S:US, we believe that having food is a human right and that all New Yorkers should have access to fresh, nutritious, and culturally appropriate food. As such, we are committed to reducing food insecurity among New Yorkers through an expansion of our Urban Farms, the creation of new Community Fridges, and the growth of our Food Tote Delivery program, which serves children when they are out of school.

Shaida Gray
This year’s Andrew Zimmern Award winner, Shaida Gray, has endured numerous emotional and physical hardships throughout her life. She came to S:US with a history marked by trauma, including experiences of abuse and neglect which profoundly affected her behavior and self-worth. Despite these challenges, Shaida has persevered with determination, resilience, positivity, generosity, and optimism. Since joining S:US in 2018, she has received support to rebuild her life, and today, Shaida is a vibrant and lovable young woman whose spirit shines brightly.
When the COVID-19 pandemic struck, the S:US Day Habilitation and Residential Team in our Developmental Disabilities Division came together to find ways to keep those they supported engaged. Shaida emerged as a beacon of hope and kindness, deeply aware of the struggles faced by her peers and the community, who were isolated. Among other acts of kindness and generosity, she visited local food pantries to gather supplies for friends who were unable to venture out. As she faces her challenges, Shaida remains a resilient advocate for herself and others. During an S:US virtual meeting focused on Self-Advocacy, Shaida passionately spoke about the importance of community support, inspiring S:US to explore more impactful solutions for food insecurity in the community. Her dedication led to the establishment of Community Fridges—a testament to the belief that individuals with intellectual and developmental disabilities can uplift their communities.
Shaida thrives in the company of others and is always ready to bring joy and laughter to her peers and S:US staff. She loves music as it serves as a calming presence and a source of joy, especially Beyoncé and Ciara. Shaida also has a passion for food, delighting in the flavors of Spanish and Chinese cuisine, and has a talent for baking cornbread and Mac ‘n Cheese. She loves taking part in Beauty Days with hair and nail treatments allowing her to express her vibrant personality. Shaida’s journey is a testament to her unwavering spirit, compassion, and ability to inspire those around her. Through her actions and advocacy, she has not only helped her community endure the hardships of the pandemic but has paved the way for a more supportive and inclusive environment for all. Shaida embodies the belief that even in difficult times, the power of kindness and connection can create lasting change.
Her ability to communicate openly has allowed her to foster friendships and connect with those around her, making her a beloved figure in her community. Always ready to share a smile, she strives to make the world a brighter place.

LUCAS HUNT is the president of HUNT Philanthropy and a celebrated American poet, whose published books include IOWA, HAMPTONS, and NEW YORK. Over the past 12 years, he has helped raise hundreds of millions of dollars for organizations like St. Jude, Make A Wish, American Cancer Society, and The Museum of African American History. Where S:US Serves
S:US has grown to become an integral part of the communities that host our programs and services. We are represented in all five boroughs of New York City and on Long Island.
On any given night, S:US provided housing and shelter to more than 4,500 individuals.
We prevented the eviction of 656 people, issued vouchers to relocate 1,197 families to prevent homelessness, and provided aftercare to 337 individuals.
95% of S:US’ permanent housing residents maintained stable housing.
398 individuals moved from our shelters, residential treatment, and transitional housing into permanent homes of their own.
We engaged more than 700 people in therapeutic horticulture, nutrition programming, and vocational training at our 91 urban farms and gardens.

What We Do
S:US addresses the challenges that prevent individuals and families from leading independent lives in safe, secure homes of their own. We offer emergency, transitional, and permanent housing for people who have lost their homes and may be facing a variety of challengesfrom mental illness to histories of incarceration, unemployment or poverty, from substance use disorders to HIV/AIDS. The stability of a home, in our experience, makes recovery, employment, and health possible.
HOUSING AND HOMELESS SERVICES
Who We Serve
S:US also provides non-residential support services to 4,000+ individuals and families. These offerings include clinical services, day services, job training and employment opportunities, care coordination, and referrals.
Our Housing Footprint
S:US’ housing portfolio consists of 135+ buildings owned, managed or leased in New York City. Of these, 90+ buildings are owned and managed by S:US. Additionally, 1,200+ apartments are leased by S:US to provide scattered-site supportive housing throughout the Bronx, Brooklyn, Manhattan, and Queens.
S:US is the 2nd largest supportive housing developer and service provider in New York State.
S:US is the 3rd largest nonprofit supportive housing developer in New York State.
S:US has developed 18 Low Income Housing Tax Credit (LIHTC) buildings with 964 units of supportive and low-income housing owned and operated by the organization.

What We Do
S:US offers a full spectrum of housing, wellness, and supportive services for people with intellectual/developmental disabilities. We are dedicated to helping people live up to their fullest potential by creating opportunities for meaningful relationships, full integration into their community and personal growth.
Our services are tailored to each person’s needs and are offered in integrated settings at our residences and day programs. We provide a comprehensive array of services, including life skills development, employment readiness, job placement, nursing, and other clinical supports. We firmly believe and have made a commitment to providing services that are adequately tailored to meet the needs of each person served. Plans and activities are developed with a person-centered approach, geared towards supporting the exploration of interests with the goal of having people make informed decisions based on their own life experiences.
PROGRAMS & SERVICES
Residential Services
S:US’ residential services provide a range of housing to people with intellectual/developmental disabilities focused on the person’s overall health and safety, as well as on their emotional well-being.
Day Habilitation
S:US’ day habilitation programs facilitate community integration and teach daily living skills.
ADDITIONAL SERVICES
Community Habilitation
This program encourage people with intellectual/developmental disabilities who don’t live in S:US homes to engage in their community by strengthening social skills and aid with independent living activities such as cooking, travel and household upkeep.
Employment
Supported Employment (SEMP) provides support to assist people to obtain and maintain paid competitive jobs in the community.
Self-Direction
This empowers people with intellectual/developmental disabilities and their family to determine, with a great flexibility, the supports and services that best fit their needs.
ABA (Applied Behavior Analysis)
This is used to teach appropriate communication and social skills that decrease the frequency of challenging behaviors that limit opportunities to engage in the community.
IPSIDD (Independent Practitioner Services for Individuals with Intellectual/Developmental Disabilities)
Behavioral Health Medical Services of New York, P.C. is contracted with S:US to deliver skilled behavioral health clinical services including mental health, speech, occupational, and physical therapies to people with intellectual/developmental disabilities within group residences, private homes, at day programs, or other convenient locations.
Grief Counseling
Offered in individual or group settings to people supported and the staff who work with them.
Community Food Security
Programs include Urban Farms, Community Fridges, pantry distributions to people served in our shelters and supportive housing, and wellness programming.
COMMUNITY FOOD SECURITY SERVICES
Setting the Baseline for Food Justice
Over half of the people served by S:US report food insecurity, making food justice work as a critical component for our wholeperson approach to wellness and recovery. With 2023’s release of the S:US white paper, “Uniting on Food Justice: An Innovative Approach from a Health and Human Service Organization,” we acknowledged the complex array of challenges underlying food insecurity in our region, and we defined a role and a pathway forward for S:US in assuring that everyone in our community has physical, social, and economic access to safe and nutritious food that meets dietary needs and food preferences.
IMPROVING FOOD ACCESS
We have one of the largest community fridge networks in NYC. Our five community fridges in Brooklyn and the Bronx are all maintained and regularly stocked by stewards from our Day Habilitation programs.
We are conducting weekly emergency food distributions. Through a partnership with Sharing Excess, we have been distributing hundreds of pounds of fresh, local produce and products to our programs.
We have continued our food tote distribution for families at S:US supportive housing in partnership with GrowNYC.
This seeks to address food security gaps for children and their families during school breaks with food from local BIPOC-owned farms and purveyors. Our Day Habilitation programs coordinated the initiative, including screen printing the tote bags used for distribution, processing, packing, and delivering biweekly vegetables, fruits, and dry goods received throughout the summer!
URBAN FARMS
We continued to expand our urban agriculture work.
We are the largest therapeutic horticulture program in the region, and among the largest urban agriculture programs in NYC.
91 S:US programs participated in Urban Farms for 2024, including new gardens at our Dorothy McGowan supportive housing in Manhattan, and a major expansion of the garden at our Renaissance Shelter in Brooklyn.
Urban Farms hosts ongoing personal development workshops on food and gardening.
From container gardening at Majestic, to vegetable canning at the Brooklyn Clubhouse, to basic cooking skills at Dewitt, S:US’ community continued to grow their skill set through in-person learning activities.

S:US actively targets individuals with behavioral health challenges who are at highest risk for homelessness, unemployment, untreated medical, psychiatric and substance use disorders, and disconnection from family, social and other natural supports.
The individuals we serve are referred to S:US from homeless shelters, shelterbased mental health programs, hospitals, clinics, day programs, and programs serving people involved in the criminal justice system.
PROGRAMS & SERVICES
Care Coordination
TREATMENT SERVICES
provides support services to adults with severe mental illness and/or chronic medical conditions who have had challenges with effectively managing their health, facilitating recovery by preventing/minimizing hospitalizations, enhancing community support and teaching skills.
Assertive Community Treatment (ACT)
delivers comprehensive mental health treatment to individuals, in their homes, shelters, parks and anywhere else they happen to be living, who have a mental illness, that impairs their functioning in the community, engaging them in a sustained relationship that leads to recovery.
Clinics
assists with improving overall participant health by increasing access to mental health and substance use treatment within our community-based health centers as well as integrating psychological rehab and physical health services to round out comprehensive care for high-risk populations.
Crisis Respite Centers
provide a safe, secure, and supported home-like environment for people experiencing an emotional crisis.
Pathway Home
assists members with the transition from a hospital and various other inpatient facilities back to the community.
Residential Substance Use Treatment
treatment services for people experiencing a substance use disorder offering social/psychological counseling, finding permanent housing to people experiencing a substance use disorder.
Harm Reduction Services
provide support to people currently and formerly living with substance use disorders by providing interventions.
Supportive Crisis Stabilization Center (Coming Soon)
will provide voluntary support, assistance, and urgent treatment to individuals with mental health and/or substance use crisis symptoms, provided on-site, 24/7.
Opioid Engagement Services
provides outreach and engagement aimed at providing education and tools for the prevention of relapse/overdose, connections to appropriate levels of care, and facilitating referrals to community-based organizations.
Safe Option Support
teams work with emergency shelters, outreach providers, as well as permanent and supportive housing providers, to transition individuals living on the street into stable housing.
Citywide Addiction Support Network
Peer Teams aid in assessments of needs, referral linkages, and Peer Mentoring aimed to improve the quality of life for individuals in recovery from opioid and stimulant use disorders.
Supplemental Substance Use Prevention and Treatment
provides overdose prevention services, peer services, and connections to outpatient services.
Medication Assisted Treatment
serves to expand and enhance access to medications for opioid use disorder among individuals with opioid use challenges.
Project ENGAGE
An innovative team founded from a collaboration between S:US, Columbia University, and New York Office of Mental Health (OMH), team members are lay staff who are trained and certified community mental health workers to deliver evidence-based interventions. The staff are equipped to provide initial engagement that results in supported connection to care.
Fort Greene Park
S:US partnered with the Fort Greene Park Conservancy to develop and deploy a team of peers to engage and offer services to individuals in and around the park.

Peer Services
Peer-to-peer support helps individuals explore and realize opportunities for community |participation, including employment, and helps them pursue social and recreational opportunities.
Clubhouse & Transitional Employment Program
The Brooklyn Clubhouse offers member-driven, comprehensive, communal, recovery, and rehabilitation services to adults living with mental illness and/or co-occurring substance use. The Clubhouse provides valuable social and vocational opportunities, peer support, and assistance in developing critical life coping and job skills. Peer counselors drive and enhance the recovery- oriented environment supporting Clubhouse member participation in all aspects of its operation.
A Transitional Employment Program also operates out of the Clubhouse with the goal of strengthening members’ work record and skills.
Supported Employment
assists adults living with mental illness to achieve and maintain employment and to support independent living. Staff work collaboratively with participants to individualize a program of work readiness, education, and/or training geared towards developing work ethics and gaining work experience. The program links participants to existing jobs and works with employers to develop new jobs. Employment specialists provide support and job coaching to enhance employee job performance and employer satisfaction. S:US is engaged in expanding our employment services through active partnerships with the business and public sectors.

VETERAN SERVICES
What We Do
S:US offers a full spectrum of housing, employment, and treatment programs for veterans facing a wide range of cooccurring challenges. These high-impact programs directly address the integration needs of service members returning to their pre-deployment communities, and help facilitate veterans’ successful adjustment to civilian life as they recover from the physical and invisible wounds of war.
Through integrated and coordinated care, S:US ensures that all services are veteran-centered and needs-based. A cohesive support system enables successful family and community reintegration for returning veterans, prevents homelessness, and supports career development.
Who We Serve
S:US serves veterans at highest risk for homelessness, unemployment, untreated medical, psychiatric and substance use disorders, and disconnection from family, social and other natural supports.
PROGRAMS & SERVICES
Veterans Employment Program
provides job training, placement, and on-the-job support, emphasizing finding employment solutions for veterans of every service era, recently returning service members, and those interested in training and new careers. Primary client referral sources include local colleges and universities, VA medical centers, the NYC Department of Homeless Services shelter system, veterans’ service organizations, and community-based organizations serving veterans.
Supportive Services for
Veteran Families (SSVF) program is designed to improve very lowincome veteran families’ housing stability through outreach, case management, and other core supports, utilizing a “housing first” approach to assist eligible, homeless veteran households with identifying and transitioning into permanent housing. SSVF is a participant-focused program in which every enrolled household creates individualized Housing Stability Plans that are meant to address their immediate housing crises, set realistic goals for addressing barriers to housing stability, and plan for long-term sustainability.

Since 1995, S:US has operated Knickerbocker, a dedicated residence for formerly homeless veterans with mental health and substance use challenges. Undercliff, another residence dedicated to this population, opened in 2019. S:US also offers support services to veterans who live in our own as well as privately-owned scattered-site permanent housing.
