ABOUT THE CANCER SUPPORT COMMUNITY As the largest professionally led nonprofit network of cancer support worldwide, the Cancer Support Community (CSC), inclusive of its Gilda’s Club Affiliates, is dedicated to ensuring that all people impacted by cancer are empowered by knowledge, strengthened by action and sustained by community. CSC achieves its mission through three areas: direct service delivery, research, and advocacy. The organization includes an international network of Affiliates that offers the highest quality social and emotional support for people impacted by cancer, as well as a community of support available online and over the phone. The Research and Training Institute conducts cutting-edge psychosocial, behavioral and survivorship research. CSC furthers its focus on patient advocacy through its Cancer Policy Institute, informing public policy in Washington, D.C. and across the nation. 1. To increase the capacity of attendees to develop, implement and advocate for effective, evidence-based psychosocial care for people affected by cancer in their communities. 2. To bring together leaders to promote and enhance affiliate collaborations to more effectively address national and local responses to take on cancer together. 3. Speakers Mitch Albom Author/Journalist Mitch Albom is an internationally renowned and best-selling author, journalist, screenwriter, playwright, radio and television broadcaster and musician. His books have collectively sold more than 35 million copies worldwide. Inspired by visits with his dying professor, Morrie Schwartz – who told him “Giving is living,” Mitch began to form charities. He has ten independent efforts, helping those in need. Mitch is a columnist for the Detroit Free Press and has lived in Michigan for more than 20 years.

Karen Bouffard Health Care Writer The Detroit News Karen Bouffard has a special interest covering the health risks faced by children in Detroit and her reporting has been nationally recognized. Karen was a 2013 National Health Journalism Fellow with the University of Southern California’s Annenberg School of Journalism. In December, she partnered with PBS NewsHour on a special report highlighting how emotional trauma and toxic stress contribute to high asthma rates among Detroit children. Karen has been with The Detroit News since 2002, covering a variety of topics from education to human services and state politics. She previously worked in public relations for nearly 20 years. She is a Detroit native and a graduate of Michigan State University.

Tifany Boyles Corporate Social Responsibility Western Union Foundation Tifany Boyles is a philanthropic professional who has worked with organizations such as UNICEF, Kiwanis International, Girl Scouts, Western Union Foundation, and the American Heart Association. She has experience with operations, fundraising,

marketing and partnership development and has a Master’s Degree in Philanthropic Studies through the Lilly School of Philanthropy at Indiana University. She is a frequent speaker for philanthropic organizations and curently lives in Colorado with her family where they enjoy the outdoors and the arts.

Kristin Bull Assistant Managing Editor Crain’s Detroit Business When Kristin first joined Crain’s, it was as a Web editor. Kristin has a rich background as a freelance writer and editor, working across media disciplines and outlets. She brings that broad media savvy to her role as Assistant Managing Editor. Kristin has a unique perspective – a keen and deep appreciation for nonprofit organizations as well as strong knowledge of the business community that is so important to the funding and volunteer support of nonprofits. The landscape is changing – for business and the nonprofits and Kristin can talk about the coverage that matters to the business community.

Joanne Buzaglo, Ph.D. Senior Vice President, Research & Training, Cancer Support Community Joanne Buzaglo, Ph.D. is the Senior Vice President of Research & Training at the Cancer Support Community. She leads strategic initiatives that leverage the research capabilities of CSC’s global cancer patient support network through collaborations with patients, caregivers, advocacy organizations, academics, health economists and leaders in industry. One such initiative is the Cancer Experience Registry, an online vehicle that engages patients and caregivers to inform future priorities in research, innovative programs and health care policy. Dr. Buzaglo is a behavioral health researcher and clinical psychologist with extensive experience in the development and evaluation of theoretically guided interventions designed to help patients cope with the complex challenges associated with cancer. Before arriving at CSC, she was the Deputy Director of the Fox Chase Cancer Center’s NCIfunded Behavioral Research Core Facility, whose mission is to provide consultation and training in behavioral principles relating to research design, program evaluation, and tailored communications. She is a Co-Investigator on a number of PCORI initiatives and has been a Co-Investigator on numerous NCI- and DoD-funded cancer-related studies that focus on enhancing doctor-patient communication, clinical trial participation, patient adherence to medical recommendations and adaptation to cancer survivorship. She received her doctorate at Temple University and completed a post-graduate fellowship in Health Services, Research, & Development at the VA Medical Center in Philadelphia, PA. In addition, she trained at the Center for Cognitive Therapy at the University of Pennsylvania and at Friends Hospital of Philadelphia. Dr. Buzaglo joined the Cancer Support Community in August 2008.

Bryan K. Clontz, CFP®, CLU, ChFC, CAP, AEP, RICP President Charitable Solutions, LLC Bryan is the co-founder and President of Charitable Solutions, LLC, specializing in non-cash asset receipt and liquidation, gift annuity reinsurance brokerage, gift annuity risk management consulting, emergency assistance funds and life insurance appraisals/audits. He also serves as a Senior Consultant for Ekstrom Alley Clontz & Associates – a practitioner-based community foundation consulting firm in Connecticut, where he has had the pleasure of working with over 250 community foundations. Bryan is the Founder of the Dechomai Foundation, Inc. and the Dechomai Asset Trust - two national donor advised funds focusing on non-cash assets generally, and S-corp transactions specifically. These foundations have received over $400 million since formation. He is also the Founder/President of The Emergency Assistance Foundation, Inc. – a national fund allowing employers to create emergency assistance and disaster relief funds for their employees.

Sara Goldberger, LCSW-R Senior Director, Program Development & Delivery Cancer Support Community Sara Goldberger is the Senior Director of Program Development & Delivery at the Cancer Support Community. She is responsible for the development of new programs and special projects and ensuring quality standards of program delivery across the affiliate network. Sara has worked in the field of psychosocial oncology for the past 19 years, most recently the Director of Program Support at Gilda’s Club Worldwide. Previously, she served as the Program Director of Gilda’s Club Westchester for 6 years. Sara began her social work career at Calvary Hospital, Bronx NY, an acute care hospital for terminally ill cancer patients. She received her MSW from Columbia University in New York where she was the Dean’s Award Scholar in recognition of excellence in both academics and clinical work in the field.

Linda House, RN, BSN, MSM President, Cancer Support Community Linda House has a long-standing history with the Cancer Support Community. First she served as a local, Affiliate volunteer and now, as President of the headquarters office. In her current role, Linda oversees several areas of CSC including Research and Training Institute and business development, marketing and communications and the Cancer Policy Institute. Linda joined CSC after many years working in the

cancer care environment, several of them being direct patient care in the acute oncology and hospice care setting. Immediately prior to joining CSC, Linda served as Executive Director of St. Vincent Cancer Care in Indianapolis where she led a team of individuals to provide care across the cancer continuum. Prior to that, Linda was Senior Director of Advocacy and Professional Relations for Lilly Oncology working with patient and professional organizations on programs and broad issues related to cancer care.

Peter D. Jacobson, JD, MPH Professor Center for Law, Ethic and Health, University of Michigan School of Public Health Peter D. Jacobson, JD, MPH, is Professor of Health Law and Policy, and Director, Center for Law, Ethics, and Health, at the University of Michigan School of Public Health, where he teaches courses on health law, public health law, and public health policy. Jacobson’s recent research has examined health departments’ strategic adaptations, public health entrepreneurship, and the U.S. Surgeon General’s vital mission. Jacobson’s most recent book is Law and the Health System (co-authored—Foundation Press, 2014). Jacobson is Associate Editor for Health Law and Public Health at the Journal of Health Politics, Policy and Law, and is Chair of the Board of Health for Washtenaw County, Michigan. Currently, Jacobson is working on a health law text for health administration students, and is leading a project examining the implementation of health in all policies programs. He is the principal investigator for the Mid-States Region of the Network for Public Health Law.

Vicki Kennedy, LCSW Vice President, Program Development & Delivery, Cancer Support Community As Vice President of Program Development & Delivery for the Cancer Support Community (CSC) global organization in Washington, D.C., Vicki Kennedy, LCSW is responsible for development, delivery and quality assurance of CSC cancer support programs worldwide. An oncology social worker for the past 31 years, Ms. Kennedy has dedicated her career to ensuring that people touched by cancer have awareness of and access to evidence-based psychosocial care. Ms. Kennedy has served as Co-Founder and first Executive Director of the Cancer Support Community (formerly The Wellness Community) Central Indiana; psycho-oncology researcher at the Walther Cancer Institute in Indianapolis; and staff social worker of the Indiana University Cancer Center in bone marrow transplant. Vicki is a past-president of Association of Oncology Social Work (AOSW) and has published numerous articles, co-authored patient education materials, and provided many professional and lay presentations on cancer survivorship.

Dr. Joneigh Khaldun, MD, MPH Medical Director Detroit Health Department Dr. Joneigh S. Khaldun, MD, MPH is the Medical Director of the Detroit Health Department, where she leads the citywide clinician engagement strategy and oversees delivery of care in all of the department’s direct service programs. A board-certified practicing urban emergency physician, she has years of local and national experience implementing innovative policies and programs that advance community health. Prior to returning to Detroit, Dr. Khaldun was the Chief Medical Officer and Assistant Commissioner for Clinical Services in the Baltimore City Health Department. A native of Ann Arbor, Michigan, Dr. Khaldun obtained her undergraduate degree from the University of Michigan, her MD from the University of Pennsylvania, and completed her emergency medicine residency at Kings County Hospital in Brooklyn, New York, where she was also a chief resident. Her MPH and fellowship in health policy were completed at George Washington University. She is married and mother to three children.

Brian Lauterbach, CFRE Vice President, Impact & Capacity Building Network for Good Brian Lauterbach, CFRE has served as a professional fundraiser and consultant for more than 100 nonprofit organizations in the United States. In this capacity, he lead the process and strategy redesign of traditional giving that lead to more than 4 million donors giving $300 million to causes most important to them. Craig Lerner Founder, Involve and Board Member, CSC Central Ohio Craig Lerner President of Involve and Involve Media (Columbus, OH & Chicago, IL) has more than 30 years of branding and integrated marketing experience as agency owner, agency director and brand-side marketer. With an early focus on advertising and promotion for pharmaceutical and health care companies, he successfully marketed and promoted products for companies like Procter & Gamble, Pfizer and Eli Lilly. 18

Craig later built a practice with an emphasis on sports and sports marketing where he and his team developed and managed marketing and promotions with many well-known sporting goods brands. In 2005, Craig became vice president of marketing at SPRI Products, a Chicago-based fitness products manufacturer. This is where his trajectory in the wellness and fitness categories sharply accelerated as Craig’s relationships with brands and industry associations grew to new heights. Many leading brands in these categories have leaned on Craig and the Involve team to chart courses toward success through challenging waters. In 2007, Craig became Director of Experiential Marketing for Fahlgren Advertising and managed promotions, sponsorship activation, live events and mobile tours for brands such as NAPA Auto Parts, Cooper Tire, National City Bank (PNC) and 5-Hour Energy. As a primary personal mission, Craig has been deeply involved in the battle against lung cancer. He is proud to have served as a National Board Member with LUNGevity Foundation from 2009 through 2012. He also founded Breathe Deep Columbus, a 5K run and walk to fund lung cancer research, in memory of his father, Frank Lerner. Currently, Craig is a Board Member and incoming Chairman with Cancer Support Community Central Ohio.

Jay Lockaby, MSS, MLSP Senior Vice President, Affiliate Relations & Strategic Growth, Cancer Support Community Jay Lockaby is Senior Vice President, Affiliate Relations and Strategic Growth at CSC HQ. Jay has more than 25 years’ experience in the nonprofit sector, in a variety of roles of increasing responsibility, primarily in management, fundraising and working with boards. He was previously the CEO of the CSC Florida Suncoast Affiliate (formerly TWC-Southwest Florida) for nearly eight years. At the time of his hiring at CSC HQ, CSC FL Suncoast was completing an $8.3 million capital campaign to build the network’s first-ever, Gold LEED Certified Affiliate site, a model facility for the network which opened in October 2010. During Jay’s tenure, CSC Florida Suncoast experienced significant growth both in revenue and the number of people affected by cancer served. Prior to his tenure with TWC/CSC, Jay was a Regional Social Services Director for The Salvation Army, in the Pennsylvania-Delaware region. He is a 1992 graduation of Emory University where he was an NCAA All-American in swimming. He holds Masters degrees in Social Service and Law and Social Policy from Bryn Mawr College in Bryn Mawr, Pennsylvania.

Chris Lockett Executive Vice President, Mission & Delivery, Cancer Support Community Chris has more than 25 years of healthcare experience, the vast majority of it in oncology. He has served in a variety of leadership roles of increasing responsibility in the biotechnology industry. He began as a field sales representative with TAP Pharmaceuticals and progressed quickly into sales management, supporting both public and commercial customers. Chris eventually advanced to lead TAP’s DC office as Director of Government Operations. In that role, he had responsibility for all legislative, health policy, advocacy, alliance development, government contracts and pricing, with one billion in government sales annually. Chris’ team included a government sales organization, state and federal lobbyists, contract lobbyists, health policy and reimbursement policy analysts. In 2006 Chris joined the biotech company Dendreon, where he helped shepherd the FDA approval and launch of the first cancer immunotherapy. While at Dendreon, Chris built and led the company’s patient assistance programs, call center, field payer, provider teams, as well as all government operations. His most recent position prior to CSC was Senior Vice President of Government Relations at HMS, a healthcare IT cost containment company. At HMS he oversaw state and federal teams tasked with facilitating procurements and contracts, including the Medicare Recovery Audit (RAC) program. Chris attended the University of Oklahoma on a ROTC scholarship and received his BA in Political Science. He was a Distinguished Military Graduate and went on to attend an Army-sponsored graduate program at the Gregg Centre for the Study of War & Society at the University of New Brunswick in Canada. Before entering the private sector, Chris served as a platoon leader in the 82nd Airborne Division and as an intelligence officer with the 3rd Ranger Battalion.

Libby Mullin Policy Consultant, Cancer Support Community Libby serves as the CSC’s policy consultant. Libby brings over 20 years of health policy experience, including a deep understanding of politics and process. She has helped numerous advocacy organizations shape and successfully advance their legislative and regulatory priorities as well as strengthen their strategic alliances and overall presence in Washington. Prior to starting Mullin Strategies in 1999, Libby held several senior positions on Capitol Hill.

Daicia Price, LMSW Professor University of Michigan School of Social Work of Field Instruction Daicia joined the Office of Field Instruction with the School of Social Work as a field faculty in 2016. Prior to becoming a part of the OFI team, Daicia served as Clinical Practice Improvement Specialist in Wayne County with a focus on workforce development. During her time there, she was instrumental in creating a centralized student training program that included collaboration with University of Michigan Detroit Clinical Scholar program. She earned her MSW degree from Eastern Michigan University. Daicia has been a field instructor for Washtenaw Community College, Eastern Michigan University, Wayne State University, Michigan State University and University of Michigan supporting students in their professional development. Daicia has gained clinical and macro practice experience in juvenile justice, community mental health, housing, schools and foster care. She has an interest working to retain healthcare providers in areas experiencing shortages. Daicia is committed to ensuring that new professionals are equipped with the necessary skills to enter the workforce as competent leaders to reach out, raise hope and change society.

Jonathan D. Schick Author GOAL Consulting Group Jonathan believes in the power of effective leaders to unify movements and create community impact. He is a dynamic leadership consultant and nationally known speaker who has founded two successful nonprofits and consulted for hundreds of organizations. As president of GOAL Consulting Group, with offices in New York/ New Jersey, Dallas/Fort Worth and Denver, Jonathan’s clients include theamerican Red Cross, United Way, Major Independent School Districts, and Boys and Girls Clubs. The author of The Nonprofit Secret: Six Principles of Successful Board/CEO Partnerships, Jonathan earned a Masters degree in Educational Administration from Boston College, along with a Bachelors degree in Human Resources Management.

Dave Sternberg Founding Partner Loring, Sternberg & Associates Dave Sternberg is a founding partner of Loring, Sternberg & Associates (LSA) and has considerable experience in nonprofit board development and governance work. Dave has been a fundraising professional since obtaining his BA Degree from The Ohio State University. He is a member of the faculty at The Lilly School of Philanthropy at Indiana University’s Fund Raising School, where he teaches Building the Annual Fund, Marketing for Fundraising, Managing the Capital Campaign, Principles and Techniques, Major Gifts; as well as numerous customized training programs nationally and internationally. In 2006 he authored the course Purposeful Boards, Powerful Fundraising in cooperation with BoardSource. Dave is also a Senior Governance Consultant for BoardSource and is the only person in America to be affiliated with both the Lilly School of Philanthropy and BoardSource. From 1991 to 1996 Dave served as a development staff member at both small and large nonprofit organizations in human services and higher education. In 1996, Dave founded Loring, Sternberg and Associates providing fundraising counsel to educational foundations, social service organizations, higher education, membership organizations, children’s groups, and museums. His counsel includes: strategic planning, development audits, on-going fund development planning, feasibility studies, campaign direction, board governance and fundraising training, and annual fund management. Dave authored the second edition of Fearless Fundraising for Nonprofit Boards a publication for BoardSource in Washington, DC, a best seller since its publication. Dave is active in the Association of Fundraising Professionals Indiana Chapter. He served two terms as the Vice President of Programs (1999, 2000) and served as the chapter President 2001. In addition, he served two years as the chair of Indiana Fund Raising Day in 1997 and 1998. Locally in Indianapolis, Dave serves on the Habitat for Humanity of Greater Indianapolis board and is a past president of the Washington Township Schools Foundation. Dave can be seen speaking nationally for the YMCA, Boys and Girls Clubs and at the BoardSource Leadership Forum. Kim Thiboldeaux CEO, Cancer Support Community Kim Thiboldeaux joined The Wellness Community in 2000 as President and CEO. In 2009, the headquarter offices of The Wellness Community and Gilda’s Club joined forces to become the Cancer Support Community, one of the largest providers of social and emotional support worldwide, where Kim has maintained her role as CEO. The combined organization is the largest nonprofit employer of psychosocial oncology mental health professionals in North America, advancing the idea that psychosocial care is as important as medical care in the face of a cancer diagnosis. The Cancer Support Community provides social and emotional support through a network of more than 50 local affiliates, more than 100 satellite locations and online at www. cancersupportcommunity.org. The organization also maintains a Research & Training Institute in Philadelphia and a Cancer Policy Institute in Washington, D.C. Formerly the Director of Patient Relations for Oncology & Transplant at HoffmannLaRoche, Inc., Kim brings to this position a wealth of experience in health care, patient education and national patient advocacy. Kim serves in numerous leadership roles in the cancer and health care communities including: member of the American College of Surgeons Commission on Cancer, CoChair of the Alliance for Quality Psychosocial Care, and Cancer Today magazine National Advisory Board Member. Prior to joining Roche, she served as the Director of Corporate Relations at Whitman-Walker Clinic, a Washington, D.C.-based health clinic that provides comprehensive services to people with HIV and AIDS. Kim is the co-author of Reclaiming Your Life After Diagnosis (2012), and The Total Cancer Wellness Guide (2007), both full length books published by BenBella Books. She is the host of Frankly Speaking About Cancer with the Cancer Support Community, an award-winning Internet talk radio show that airs weekly on VoiceAmerica.com and is aimed at informing and inspiring listeners to live well with cancer. Kim is also a regular contributor to The Huffington Post.

Laura Varon Brown Executive Director & CEO of Gilda’s Club Metro Detroit Before joining Gilda’s Club in 2012, Laura spent 19 years with the Detroit Free Press in various executive-level editing positions. Laura also wrote a column and was a community liaison at the newspaper. At Gilda’s Club, Laura has become a passionate champion for the cancer community. Laura lost her mom to cancer when she in her 20s, and her husband successfully faced cancer when her children were very young. Much of her passion stems from understanding how difficult a path cancer places us

on. Laura’s community relationships and respect for the history of Gilda Radner’s journey and Gilda’s Detroit roots have been a source of support and guidance as Gilda’s Club Metro Detroit grows in reach and purpose.

Michelle Warren, LMSW Women’s Wellness Director Relationship Institute Michelle earned her BA from University of Dayton in psychology. She later earned her MSW from Eastern Michigan University. She completed both of her internships at Gilda’s Club Metro Detroit. Michelle is the Women’s Wellness Director at the Relationship Institute. Michelle has a rich and purposeful history with Gilda’s Club Metro Detroit. Mid-way through Michelle’s MSW program, she was hired as the volunteer coordinator, and eventually transitioned to the Noogieland Program Manager. Post-graduation, Michelle was promoted to the position of Program Director. During her tenure, she was often the guest speaker in the community on various topics including “How to talk to you kids about cancer,” “Living with Cancer: A family member’s perspective,” and of course, informational sessions about her favorite non-profit, Gilda’s Club. Michelle founded a local non-profit cancer coalition committee, collaborated with many other non-profits for events, and organized a grass-roots marathon fundraising team for again….her favorite non-profit, Gilda’s Club. In 2012, Michelle left Gilda’s Club Metro Detroit and joined the Relationship Institute in Royal Oak, a group therapy practice with an emphasis on working with couples. Because of her over a decade years of experience in working with people with cancer, Michelle has been able to specialize her work to women’s issues and chronic illnesses in addition to working with couples.

Brad Zebrack, PhD, MSW, MPH, FAPOS Professor University of Michigan School of Social Work Brad Zebrack’s teaching and research interests are in the area of health, medicine, and quality of life. He is particularly interested in the effects of cancer on the psychosocial growth and development of adolescents and young adults. His funded research also involves evaluations of the implementation of psychosocial support services in medical care settings. Zebrack has clinical social work experience in both pediatric and adult oncology, and has been involved in the development of peer support/advocacy programs for adolescent and young adult cancer survivors. He currently is a member of the University of Michigan Comprehensive Cancer Center’s Socio-Behavioral Program, and involved in development of new curriculum on Inter-Professional Education.
