Preface Clinical documentation occupies a distinctive position in veterinary medicine [1, 2]. It is at once a clinical instrument, a legal record, a communication channel, and an educational substrate [8, 11]. The medical record is the only persistent representation of an encounter that occurred in real time between a veterinarian, a non-verbal patient, and an owner whose recollection of the event will fade and shift over the months and years to follow [3, 13]. Where human medicine benefits from a patient who can recount their own history, veterinary medicine relies entirely on the written word to carry the patient’s story across time and between providers [11, 35].

The chapters that follow approach documentation as a clinical competency rather than an administrative chore [15, 16].

Each module isolates a specific dimension of the discipline — anatomy of the record, adaptive context, clinical timeline, descriptive precision, evidence-based assessment, diagnostic reasoning, resource use, value, teaching, communication, knowledge translation, and the future of practice — and addresses both the underlying logic and the concrete techniques that distinguish exemplary documentation from adequate documentation. Three case studies in the second part of the text demonstrate the integration of these techniques in fully worked clinical scenarios, and a series of appendices in the third part offer the reference material that practitioners reach for in the middle of a busy clinic day [18, 27].

Throughout, the text refers to the consensus framework documents that anchor contemporary small-animal practice: the ACVIM staging systems for myxomatous mitral valve disease [4, 29] and chronic kidney disease; the AAHA diabetes management guidelines [7]; the AAFP feline standards [23]; and the IRIS staging system for chronic kidney disease and proteinuria [28, 39]. Where these frameworks dictate the structure of an assessment or the content of a plan, the text draws the connection explicitly so that practitioners can trace any recommendation back to its evidentiary source [18].

The diagrams that accompany every chapter were generated by Claude, an artificial-intelligence model produced by Anthropic, working under the direction of the authors. They were drawn deliberately in monochrome line art to coexist with the disciplined visual aesthetic of the text, but the analytic content — what each diagram chooses to emphasise, omit, and arrange — derives from the underlying clinical material. The diagrams are intended to function as memory aids, not as standalone clinical references.

We expect this book to be read in two modes: front-to-back as an organised study of clinical documentation, and out-oforder as a reference to be consulted when a specific question arises. The numbered references throughout the text and the appendices in Part III have been designed with the second mode in mind.

A final note. The phrase "if it wasn’t documented, it didn’t happen" is repeated so often in veterinary education that it has lost some of its force. We would prefer to put the proposition more constructively: a well-documented case is one whose author has thought clearly. The structure of the record is a structure of thought, and improving the former is one of the most direct routes to improving the latter.

PART I FOUNDATIONS OF CLINICAL DOCUMENTATION Fourteen modules covering the structure, contexts, language, and reasoning behind exemplary veterinary records

Module 1 — The Power of Precision Documentation Excellence in clinical documentation is one of the highest-leverage skills a veterinary professional can develop, and one of the least frequently taught with rigour. The reason is partly cultural: documentation is often presented as a downstream byproduct of clinical work — a residue left behind after the real medicine is done — rather than as a discipline that itself shapes the quality of clinical thinking. The proposition advanced in this module is the inverse. Precise documentation is the discipline that makes precise medicine possible. The structure of the record imposes a structure on the encounter; the discipline of writing well in real time forces the writer to observe well, to reason well, and to plan well.

In jurisdictions across the United States and Canada, the medical record carries simultaneous legal weight [1, 8]. It is the practitioner’s primary defence against allegations of negligence, the licensing board’s primary instrument for assessing competence, and the only durable evidence that informed consent was obtained [9, 36]. A record produced quickly and superficially in the heat of a busy clinic day will be read carefully and slowly years later, by attorneys, by claims adjusters, and by colleagues who must continue the patient’s care. The asymmetry between the speed of writing and the patience of reading is the central tension that this module addresses.

The PRECISE Documentation Framework The PRECISE framework is a mnemonic for the seven attributes that distinguish exemplary documentation from acceptable documentation [35]. It has been validated across multiple veterinary practice settings — small-animal general practice, emergency and critical-care referral, specialist practice, and academic teaching hospitals — and although individual settings demand different emphases, the seven attributes are universal [15, 35].

P — Patient-Centered Documentation should always focus on the individual patient’s presentation, needs, and response to care. Generic templates become meaningful when they are customised to the specific clinical situation, and they become hollow when they are not. Every entry should answer the question: how does this information impact this patient’s care? When a templated phrase appears in a record without a clinical referent, it dilutes the signal of the document and creates a hostage to fortune. A note that records "lungs clear bilaterally" on a patient who was, in fact, never auscultated is worse than a note that records nothing at all.

R — Relevant A relevant record includes the information that affects clinical decision-making and excludes the information that does not.

The traditional teaching of "pertinent positives and negatives" — recording the findings that support or rule out a differential — is the operational principle. A cardiac patient’s detailed neurological examination may be less relevant than thorough cardiovascular documentation, but the cardiac patient’s mental status, gait, and reflex symmetry are relevant if syncope is on the differential list. Relevance is not parsimony; it is the discipline of choosing what matters for the question at hand.

E — Evidence-Based

Treatment plans and recommendations should reflect current consensus guidelines and published evidence [4-7, 18].

Numerical citations connect clinical decisions to their scientific foundation and create a defence against the claim that a decision was idiosyncratic. Where staging systems exist — the ACVIM stages for MMVD and DCM [4], the IRIS stages for chronic kidney disease [28, 39], the WSAVA body-condition scale [37] — they should be invoked by name and by stage, with the criteria that establish that stage documented in the record. Citing "ACVIM Stage B2" without the LA:Ao and LVIDDN that establish it is a soft assertion; citing "ACVIM Stage B2 (LA:Ao 1.7, LVIDDN 1.85)" is a hard one [4].

C — Complete Documentation must include all elements required for continuity of care, legal defensibility, and billing compliance [1, 35].

Missing elements create vulnerabilities that compound with time. A surgical note that omits the volume of intra-operative blood loss is recoverable in the short term but loses information forever once the staff who witnessed the procedure have left the practice. The minimum data set for each visit type — wellness, sick visit, surgery, emergency, end-of-life — is described in Module 3 [35].

I — Immediate Document findings and decisions at the time they occur. Delayed documentation introduces inaccuracies, creates the appearance of post-hoc rationalisation, and exposes the practitioner to legal challenge [17]. Studies of documentation timing in veterinary teaching hospitals show that records completed more than twenty-four hours after the encounter contain substantially more errors of omission than records completed within the hour [17, 35], and that the differences are concentrated in the very details — specific times, specific dose volumes, specific verbatim client statements — that prove most consequential during litigation [9, 36].

S — Specific Replace vague terms with quantifiable measurements [18]. "A 3 cm firm subcutaneous mass over the left scapula" communicates more than "lump on shoulder." "Grade III/VI left apical systolic murmur" communicates more than "heart murmur present." Replace subjective language with objective measurement wherever possible: temperature in degrees, pulse in beats per minute, respiratory rate in breaths per minute, body weight in kilograms, mass dimensions in centimetres along three axes. The discipline of specificity is a discipline of seeing.

E — Explicit State clinical reasoning explicitly. Document why decisions were made, not only what was done. "Declined radiographs due to financial constraints; discussed risks of empirical treatment, owner accepts" is more defensible than "no rads." The first is a record of an informed conversation between professionals; the second is a fragment that an attorney will frame as an omission. Reasoning in the record is reasoning that survives.

A Documentation Quality Comparison The practical difference between superficial and precise documentation is best appreciated by direct comparison.

Consider a young dog presenting with acute vomiting after probable garbage ingestion.

Inadequate version: "Dog vomiting. Gave fluids and anti-nausea meds. Recheck if not better."

This entry fails on every dimension of PRECISE. It contains no signalment that would frame the differential list. It contains no historical detail that would suggest dietary indiscretion or foreign body. It contains no examination findings, no differential diagnosis, no specific medications with doses, no client communication, and no concrete follow-up timeline. As a clinical document it is uninformative; as a legal document it is indefensible; as a billing document it does not justify the charge that probably accompanied it.

A precision version of the same encounter, written in SOAP format, communicates the case fully: S: 6-year-old FS Golden Retriever, 28 kg, presenting for acute vomiting × 24 hours. Owner reports six episodes of vomiting, initially food then bile-tinged fluid; last vomitus contained a small amount of red blood. Decreased appetite, no diarrhoea. Access to garbage two days ago per owner. Current medications: none. Vaccines current. Diet: Purina Pro Plan.

O: T 102.4°F, HR 110 bpm, RR 24, BP 132/84. BCS 6/9. QAR. MM pink/moist, CRT <2 sec. Dehydration estimated 5% (mild skin tenting, slightly tacky MM). Eyes: NSF. Ears: NSF. LN: NSF. CV: NSF, no murmur. Resp: clear lung sounds bilaterally. GI: mild cranial abdominal discomfort on palpation, no masses, no organomegaly. Normal borborygmi. Rectal: deferred.

A: Acute gastritis/gastroenteritis, likely dietary indiscretion given garbage access. Differential diagnoses: gastrointestinal foreign body (consider if no improvement in 24–48 hours), pancreatitis (breed predisposition), metabolic disease (less likely given duration and signalment).

P: 1) CBC, chemistry, urinalysis to rule out metabolic causes, results pending. 2) Three-view abdominal radiographs — no evidence of obstruction or foreign body. 3) Lactated Ringer’s 500 mL SC for rehydration. 4) Maropitant 28 mg SC (1 mg/kg). 5) NPO 12 hours then bland diet (boiled chicken/rice or veterinary GI diet). 6) Recheck in 24–48 hours, sooner if vomiting worsens, becomes projectile, or lethargy develops. 7) If no improvement in 48 hours: recommend SNAP cPL and consider abdominal ultrasound.

Client communication: Discussed likely diagnosis, differential diagnoses, and treatment plan with owner. Reviewed warning signs requiring immediate recheck. Written discharge instructions provided. Estimate for today’s visit USD 285; paid in full. Owner understands and consents to plan.

The precision version is approximately five times longer than the inadequate version. It is also written in approximately the same total elapsed time, because the discipline of writing in the SOAP structure forces the practitioner to observe and to reason as they write. The relationship between speed and quality of documentation is non-linear; the practitioner who has internalised the structure writes thoroughly and quickly, while the practitioner who has not internalised it writes briefly and slowly, in the sense that the brevity is paid for in subsequent re-work, telephone calls from owners seeking clarification, and occasional regulatory consequences.

Module 2 — Anatomy of the Complete Patient Record A medical record is not a single document but a collection of interconnected components, each serving a specific clinical, legal, and communication function [1, 35]. The professional veterinarian who understands these components and their relationships writes records that read coherently to colleagues, that survive legal scrutiny, and that support continuity of care across the patient’s lifetime [11, 36]. The novice who treats the record as a series of unrelated forms produces documents that are individually competent but collectively incoherent [12, 15].

The diagram on the following page maps the principal components of the complete patient record and the sub-elements that populate each. The structure has the recursive quality common to all good clinical hierarchies: each branch can be expanded to the level of detail appropriate to the encounter, and each element can be traced back to its parent category.

Signalment The signalment establishes patient identity and provides crucial epidemiological context for differential diagnosis development [18, 32]. Age, breed, and sex significantly influence disease probability and should guide initial clinical thinking before any examination findings are recorded [42, 46]. The Doberman Pinscher with exercise intolerance is a different diagnostic problem from the Yorkshire Terrier with the same complaint, even when the examinations are identical, because Doberman Pinscher signals a substantial pre-test probability of dilated cardiomyopathy that the Yorkshire Terrier signal does not [46-48].

A complete signalment in the small-animal context comprises: Species: canine, feline, equine, bovine, avian, exotic, or other.

Breed: specific breed or, for mixed-breed patients, a description of the dominant phenotype. Breed predispositions guide differential diagnosis and the selected reference ranges; "small-breed mixed" and "Doberman" cannot be treated identically when the question is cardiomyopathy [42].

Age: in appropriate units. Days and weeks for neonates, months for juveniles, years for adults. Age in months for a kitten is more clinically meaningful than age in years rounded to zero.

Sex: male or female.

Reproductive status: intact, neutered (M) or spayed (F), or unknown. Reproductive status modifies risk for pyometra, mammary neoplasia, prostatic disease, perineal hernia, and a broad range of behavioural concerns [20].

Body weight: current weight in kilograms, recorded to the nearest 0.1 kg for cats and small dogs and to the nearest 0.5 kg for large dogs. Body weight is essential for accurate medication dosing [27]; recording an estimated weight rather than a measured weight is a deviation that becomes invisible to subsequent providers.

Body condition score: WSAVA 1–9 scale or the older 1–5 scale, with the scale identified explicitly [37]. "BCS 6/9" and "BCS 4/5" describe overweight patients of similar severity but the unmodified "BCS 6" is ambiguous between them.

Identification: microchip number, tattoo, distinctive markings, collar description. The microchip number is the most durable identifier and should be recorded at every visit at which the chip is read [1].

Vital Parameters Accurate vital signs establish the baseline physiological status of the patient at the moment of examination and guide emergency triage decisions [18, 38]. Normal ranges vary by species, breed, age, and individual patient, and recording the value without recording the reference frame leaves a colleague guessing. The practitioner who has memorised the normal canine respiratory rate must still record the species against which that range is being applied.

The reference ranges that follow are widely accepted in small- and large-animal practice [18, 20]: Parameter

Canine heart rate is size-dependent: large- and giant-breed dogs commonly run 60–100 bpm at rest, whereas small breeds may sit at 80–140 bpm. A heart rate of 130 bpm in a Great Dane warrants investigation; the same rate in a Yorkshire Terrier is unremarkable. Reference ranges should be cited with their species-and-size context wherever ambiguity exists.

History of Present Illness The history of present illness is the narrative description of the clinical problem from onset to presentation. A wellconstructed HPI takes the reader from the first hint of abnormality through every subsequent change to the moment of

consultation, in chronological order, with specific times, specific descriptions, and the owner’s exact wording where exact wording is available. The HPI is the section in which a verbatim quotation from the owner is most often appropriate, because subtle distinctions in the owner’s description — "vomiting" versus "regurgitation," "limping" versus "non-weightbearing," "tired" versus "weak" — carry diagnostic weight that paraphrasing erases.

Onset — when did the problem first appear, and was the onset acute (over hours) or insidious (over weeks)?

Duration — for how long has the problem been present?

Progression — has the problem worsened, improved, or remained stable since onset?

Frequency and severity — for episodic problems, how often do episodes occur and how severe are they?

Triggers — what circumstances appear to precipitate or worsen the problem?

Alleviating factors — what circumstances appear to relieve the problem?

Associated signs — what other clinical findings have appeared during the same period?

Therapeutic response — what interventions have been attempted, and what was the response?

Past medical history — has the patient experienced this problem or a similar problem before?

The HPI is the principal place in the record where the practitioner can demonstrate that the differential diagnosis was constructed deliberately rather than by association. A history that names the absence of cough in a coughing-breed patient with exercise intolerance is doing diagnostic work even when the answer turns out to be negative.

Physical Examination The physical examination follows a body-systems format. Documentation should reflect the order of the examination — typically general appearance, vital parameters, then systematic examination by region or system — and should record both the abnormal findings and the relevant normal findings (the pertinent negatives). "NSF" (no significant findings) is acceptable shorthand for systems examined and found unremarkable; "not examined" or "deferred" is the correct entry for systems not examined, and is preferable to silence, which is ambiguous.

A thorough physical examination encompasses, at minimum: general appearance and mentation; integument and lymph nodes; head and oral cavity (eyes, ears, nose, mouth); cardiovascular system; respiratory system; gastrointestinal system (abdominal palpation); genitourinary system; musculoskeletal system; neurological system; and a focused examination of any complaint-specific region. The expected duration of a thorough small-animal physical examination is approximately seven to twelve minutes, depending on patient temperament and the depth of the focused component.

Assessment, Diagnostics, and Plan Assessment, diagnostics, and plan together constitute the analytic core of the record. The assessment translates observation into interpretation: it states the working diagnosis, the principal differential diagnoses, the staging or classification (where applicable), and the prognosis. Module 7 develops the structure of evidence-based assessment in detail. Diagnostics document what was performed, what was declined, the results, and the interpretation of those results.

The plan documents the interventions ordered, the client education delivered, and the timeline for follow-up. Each of these sub-sections is treated in its own module of this text; what they have in common is the requirement that the reader of the record can reconstruct the practitioner’s reasoning from the entries provided.

Module 3 — Adaptive Documentation Strategies Clinical documentation must adapt to varying contexts while maintaining core quality standards. The demands of emergency care differ significantly from routine wellness visits, and referral communications require different emphases than primary-care records. The discipline of adaptation is the discipline of varying the surface form while preserving the underlying content: the SOAP framework that organises a wellness visit also organises an emergency, but the relative weight given to subjective and objective sections, the depth of the differential, and the specificity of the plan all change with the context.

Why Detailed Records Matter The medical record performs four interconnected functions, each of which deserves brief development [1, 35].

First, the record ensures continuity of care across providers and across time [11, 12]. In the simplest case this means that when the patient sees a different veterinarian during the practice’s on-call rotation, the on-call veterinarian inherits a complete picture of the patient’s history, current treatments, and outstanding diagnostic questions. In the more complex case it means that the specialist at the referral hospital can read the primary-care record and continue the case rather

than restart it. Continuity depends on completeness and accessibility; a record that is meticulous but not retrievable serves continuity poorly.

Second, the record provides legal protection [8, 9]. The practitioner’s primary defence against an allegation of negligence is the contemporaneous record. The principle that the courts apply, sometimes explicitly and almost always implicitly, is that an undocumented act is presumed not to have occurred [9]. Professional liability insurance carriers report consistently that inadequate medical records are a leading factor in successful malpractice claims, even when appropriate care was in fact provided [36]. Legally defensible records include complete patient identification, detailed examination findings, the diagnostic tests performed and declined, clear assessment and plan, documented client communications, and explicit evidence of informed consent [9, 36].

Third, the record enables evidence-based medicine [18]. Aggregated across cases, well-structured records permit pattern recognition and outcomes analysis at the practice level. The practice that has documented every parvovirus case for ten years has, by accident, generated a longitudinal dataset of clinical interest. The practice that has documented its parvovirus cases inconsistently has generated noise.

Fourth, the record supports billing accuracy and compliance [33, 35]. The connection between clinical documentation and billing is direct: a procedure that was performed but not documented is a procedure that may be billed but cannot be defended if billed; a procedure that was documented but not performed is fraud.

Federal and Regulatory Requirements The American Veterinary Medical Association establishes the baseline standards that serve as the foundation for state regulations and practice standards across the United States [1]. According to AVMA’s Principles of Veterinary Medical Ethics [1], veterinary medical records are an integral part of veterinary care when a veterinarian-client-patient relationship exists, and the maintenance of these records is a professional obligation rather than an option [8].

Beyond the AVMA framework, several federal agencies impose specific documentation requirements that veterinary practices must observe.

Drug Enforcement Administration (controlled substances) The DEA classifies controlled substances into five schedules based on abuse potential and accepted medical use [10].

Schedule II substances carry the most stringent documentation requirements; Schedules III through V carry requirements that are less stringent in form but identical in spirit [10]. The veterinary practice that handles controlled substances must maintain biennial inventories, contemporaneous dispensing logs that record date, patient, drug name and concentration, quantity dispensed, prescribing veterinarian, and DEA registration number, and must reconcile dispensing records against bottle counts at intervals consistent with state regulation [10]. Schedule II ordering requires the DEA Form 222 and meticulous reconciliation against the corresponding receipts. The penalties for non-compliance are not theoretical; loss of DEA registration is a routine consequence of inventory discrepancies that cannot be reconciled [10].

United States Department of Agriculture and APHIS Practices that issue international health certificates, participate in accreditation programmes, or conduct USDA-regulated research are subject to additional documentation requirements that often exceed standard practice documentation.

International health certificates demand documentation that meets both USDA requirements and the specifications of the importing country. Physical examination must be completed within specified timeframes — typically ten days before travel — with detailed findings recorded for all body systems. Vaccination histories require complete documentation including manufacturer names, lot numbers, expiration dates, and routes of administration. Laboratory test results must include test type, collection date, laboratory identification, and official validation. The accredited veterinarian’s USDA accreditation number must be included on all official documents, and the specific date and time requirements for examination and certificate issuance must be observed precisely.

Occupational Safety and Health Administration OSHA requirements apply to the documentation of workplace exposures and occupational injuries that occur in the course of veterinary practice. Animal bites and scratches require documentation of the date and time of injury, the circumstances,

the animal’s vaccination history (especially rabies status), the identification of the animal and owner for follow-up, the medical treatment provided to the employee, and any required follow-up including post-exposure prophylaxis. Zoonotic exposures, needlestick injuries, anaesthetic gas exposures, chemotherapy drug exposures, and radiation exposures all carry their own documentation requirements that the practice manual should address explicitly. These records protect the practice from OSHA citations and protect employees from the consequences of unrecognised occupational disease.

Emergency Documentation Priorities Emergency documentation differs from routine documentation principally in the weight it gives to time. Every entry in the emergency record should carry an explicit time stamp, and the sequence of interventions should be reconstructable from the record alone. The triage assessment should be the first contemporaneous entry, recorded at the moment of presentation; the subsequent interventions should be recorded as they occur, not at the end of the case; and the clinical reasoning that drove each decision should be captured even when — perhaps especially when — it differed from the textbook approach.

A pragmatic emergency-record template includes the following components, each with an explicit time stamp: Triage assessment and category (e.g., Triage 1 — immediate intervention required).

Vital signs at presentation, including mentation and perfusion parameters.

Initial interventions and the time at which each was performed.

Diagnostic results as they become available, with the time the result was received.

Reassessments at intervals appropriate to the clinical state — typically every 15 to 30 minutes during active resuscitation.

Communication with the owner, including the time of each conversation, the substance of the conversation, and the owner’s response.

Disposition: hospitalisation, transfer to a referral facility, discharge, or humane euthanasia.

The discipline of contemporaneous timestamps in emergency records is a discipline of forensic reconstruction. A case that ends well will be questioned only rarely; a case that ends poorly will be reconstructed exhaustively, and the practitioner’s defence will rest on the durable timestamps that anchor the record to the events of the day.

The SOAP Format The Subjective-Objective-Assessment-Plan structure is the universal organisational framework of the modern veterinary record. It distinguishes the patient’s reported experience and the owner’s observations (Subjective) from the practitioner’s direct measurement (Objective) from the interpretation of those data (Assessment) from the resulting interventions (Plan).

The discipline of separating these four elements is what makes a record legible to a colleague reading it for the first time.

Subjective The Subjective section captures the chief complaint, the history of present illness, the past medical history, and any other information whose source is the owner or another non-clinical observer. This is the section in which the owner’s exact words may be quoted, in which behavioural observations are recorded, and in which the social and environmental context of the patient is documented. The Subjective section is unique in the record in that the source of the information is explicitly external to the practitioner; everything in this section is, in principle, reported speech.

Objective The Objective section captures the practitioner’s direct observations: vital signs, physical examination findings, point-ofcare diagnostics performed during the visit, and any laboratory or imaging results received during the encounter. The Objective section is the section in which the practitioner’s eyes, ears, hands, and instruments speak. Findings should be recorded with specificity sufficient that a colleague could perform the same examination and verify the entries.

Assessment The Assessment section is the analytic translation of subjective and objective information into clinical interpretation. It contains the working diagnosis, the principal differential diagnoses ranked by probability, any applicable staging or classification, the prognosis, and the explicit reasoning that connects observation to interpretation. The discipline of writing the assessment in full sentences rather than fragmentary phrases is a discipline of forced reasoning; the practitioner who must construct a coherent paragraph cannot easily skip the inferential steps.

Plan

The Plan section documents the interventions ordered, the client education delivered, and the timeline for follow-up. The Plan should include the rationale for each major intervention, the expected response, the criteria for revisiting the plan if the response is inadequate, and the specific follow-up timeline. A Plan that documents only the order — "Maropitant 1 mg/kg SC" — is incomplete relative to a Plan that documents the order, the rationale, and the follow-up — "Maropitant 1 mg/kg SC for control of vomiting; expect resolution within 4–6 hours; recheck in 24 hours if vomiting persists or worsens."

Referral Documentation Referral documentation deserves particular attention because the referring practitioner and the consulting specialist communicate through the written record almost exclusively. A referral letter should answer three specific questions: what is the principal clinical concern, what has been done to date, and what specifically is being asked of the consulting specialist.

A diffuse "please evaluate as needed" referral wastes the specialist’s time and risks duplicating diagnostics that have already been performed. A focused "please evaluate the cardiac murmur and recommend whether echocardiography is indicated; CBC, chemistry, urinalysis, and three-view thoracic radiographs attached" referral allows the specialist to deploy their expertise to the question at hand.

Module 4 — Crafting Compelling Clinical Timelines A clinical timeline is the narrative spine of a complex case [11, 18]. It records the sequence of events from the first appearance of an abnormality through every subsequent change to the moment of presentation and beyond. The timeline serves three distinct functions: it organises the practitioner’s thinking, it communicates the case to colleagues [12], and it survives as the durable record of the encounter for legal and continuity purposes [8, 35]. Each of these functions imposes specific demands on the form of the timeline.

The timeline organises thinking by forcing the practitioner to commit to specific dates and durations rather than to the vague descriptions ("recently," "for a while," "off and on") that owners frequently offer [3, 13]. The discipline of converting "vomiting for a while" into "vomiting six episodes over the past forty-eight hours" is the discipline of clinical specificity [18].

It is also the discipline that distinguishes a workable history from a description that resists analysis.

Elements of an Effective Timeline An effective clinical timeline contains the following elements: 1. Specific dates or durations for every event. "Three weeks ago" is acceptable when the date is not known; "vomiting for a while" is not.

2. Brief descriptive labels for each event. "PU/PD noted by owner," "vomiting begins," "anorexia onset," and "obtundation" are appropriate; "got worse" is not.

3. Indication of progression — whether the events represent worsening, improvement, or fluctuation.

4. Notation of any interventions that occurred during the timeline, with the response to each.

5. A clear marker for the moment of presentation, which serves as the temporal anchor for the entire record.

A Worked Timeline Example Consider a feline patient presenting with diabetic ketoacidosis. The clinical timeline that emerges from a careful history might read as follows: Approximately six months prior to presentation: gradual weight loss noted by owner, attributed at the time to summer heat and recent diet change. Increased water intake noted concurrently. Owner did not seek veterinary evaluation.

Three days prior to presentation: onset of vomiting. Six to eight episodes on day one, declining to two to three episodes daily thereafter. No haematemesis. Owner offered bland food without effect.

Two days prior to presentation: complete anorexia. No water intake observed.

Twenty-four hours prior to presentation: progressively obtunded. Owner reports patient barely moving from bed.

Twelve hours prior to presentation: owner notes "acetone smell" near litter box.

At presentation: laterally recumbent, minimal response to stimulation, severe muscle wasting, fruity breath odour, Kussmaul respiratory pattern.

The timeline above tells the diagnostic story before any laboratory data is reported. A reader who has finished the timeline already suspects diabetes mellitus complicated by ketoacidosis, knows that the patient has likely been hyperglycaemic for

weeks to months, and knows that hepatic lipidosis is a substantial concurrent concern. The subsequent laboratory data confirms the timeline rather than substituting for it. This is the diagnostic value of a well-constructed timeline: it converts a nebulous history into a sequence of clinically interpretable events.

When the Timeline is Uncertain Owners often present with imprecise recollections of when events began. The honest response to this uncertainty is to record it. "Owner uncertain of onset; reports the patient ‘has been off for at least a week, possibly longer’" is more useful than a falsely precise "vomiting × 8 days" that the practitioner has invented to fill the gap. Where the owner’s recollection is anchored to a specific external event — a holiday, a house guest, a change in diet or medication — the external event itself becomes the anchor. "Onset reported by owner as ‘after the family returned from holiday on March 14’" is a workable timeline even when the precise date of onset is unknown.

Module 5 — Strategic Data Selection Excellence in documentation is not the same as completeness in documentation. A record that captures every conceivable detail of an encounter is a record in which the clinically meaningful findings are buried in noise. The discipline of strategic data selection is the discipline of choosing what to record and what to omit, with the question always framed as: which findings inform clinical decisions, and which do not?

The framework that organises this discipline in clinical reasoning is the framework of pertinent positives and negatives.

Pertinent positives are findings that are present and that support a particular differential diagnosis. Pertinent negatives are findings that are absent and whose absence helps to rule out a differential. Both deserve documentation; neither is irrelevant. The presence or absence of a finding becomes diagnostically informative only when it has been considered against a hypothesis and recorded.

Pertinent Positives

Pertinent positives include any finding whose presence confirms or supports a differential diagnosis on the active list. They should be documented with the full specificity that distinguishes them from incidental similar findings. "A grade IV/VI left apical systolic murmur" is a pertinent positive in a small-breed dog with cough and exercise intolerance, and the specificity of "left apical" and "systolic" and "grade IV/VI" all carry diagnostic weight: they localise the murmur to the mitral valve region, situate it in the cardiac cycle in a manner consistent with mitral regurgitation, and quantify it in a manner that supports the ACVIM Stage B2 or Stage C threshold.

Pertinent positives also include findings whose presence places a particular differential higher on the list even when they do not confirm it. "Splenomegaly palpable two to three centimetres beyond the costal arch" in a patient with regenerative anaemia is a pertinent positive for immune-mediated haemolytic anaemia, even though splenomegaly itself is nonspecific.

Pertinent Negatives Pertinent negatives are findings that are deliberately absent. The deliberateness is the critical feature. A practitioner who has examined a coughing patient and found no pulmonary crackles has documented a pertinent negative; a practitioner who has not examined for pulmonary crackles has documented nothing. The first is a diagnostic statement; the second is a gap in the record.

The discipline of pertinent negatives is the discipline of documenting what one has looked for. The phrase "no jugular pulsation" in the cardiac examination of a patient with congestive heart failure conveys that the examiner specifically evaluated jugular venous distension and pulsation; the absence of any reference to jugular venous evaluation conveys nothing. The first record will defend itself in subsequent review; the second will be ambiguous.

A Worked Example: ACVIM MMVD Staging Strategic data selection is most explicitly required in the staging of structural heart disease. The ACVIM consensus statement on myxomatous mitral valve disease [4] distinguishes Stage A (at-risk breed without murmur), Stage B1 (murmur without remodelling), Stage B2 (murmur with remodelling), Stage C (clinical heart failure), and Stage D (refractory failure) [4, 30]. Each stage carries specific criteria, and each criterion has a specific documentation requirement.

For Stage A, the relevant data are signalment (predisposed breed) and the absence of a murmur. The documentation should record the breed, the age, and the explicit absence of a murmur on auscultation: "8-year-old MN Cavalier King Charles Spaniel — predisposed breed; no murmur on auscultation" [4].

For Stage B1, the relevant data are the presence of a murmur and the absence of left atrial or left ventricular enlargement.

Documentation should record the murmur (location, timing, grade) and the echocardiographic measurements that establish the absence of remodelling: "Grade III/VI left apical systolic murmur. LA:Ao 1.4 (reference <1.6), LVIDDN 1.5 (reference <1.7) — no remodelling, ACVIM Stage B1" [4].

For Stage B2, the relevant data are the murmur, the echocardiographic measurements of remodelling, and any biomarker data: "Grade IV/VI left apical systolic murmur. LA:Ao 1.7, LVIDDN 1.85 (both above ACVIM B2 thresholds). NT-proBNP 1,420 pmol/L. Resting respiratory rate 22 — ACVIM Stage B2; pimobendan indicated per EPIC trial criteria [29]."

For Stage C, the relevant data are the clinical signs of heart failure, the radiographic findings, the medications, and the response to treatment: "Coughing × 3 weeks. Resting respiratory rate 36 (clinically significant tachypnoea). Thoracic radiographs: cardiomegaly with pulmonary venous distension and perihilar interstitial-to-alveolar pattern consistent with cardiogenic pulmonary oedema. ACVIM Stage C. Initiated furosemide, pimobendan, ACE inhibitor, spironolactone per ACVIM guidelines."

At each stage, the practitioner is selecting the data that establishes the stage, ranking it for inclusion in the record, and omitting data that does not bear on the stage. This is what strategic data selection looks like in practice: not the omission of important findings, but the explicit inclusion of the findings that drive the diagnostic conclusion.

Module 6 — The Art of Clinical Description Clinical description is the discipline of converting sensory observation into communicable information. The eye sees a mass; the description records its size in three dimensions, its consistency, its mobility, its overlying integument, its anatomical location relative to identifiable landmarks, and the time course over which the owner has noted its presence.

The clinical examination of a single mass takes thirty seconds; the description of that examination takes ninety seconds to write and remains intelligible to a colleague who reads it five years later. The asymmetry between examination and description is not a defect of efficiency; it is the source of the description’s value.

Lameness Documentation A lameness description is informative when it identifies the limb, the grade, the gait phase at which the lameness is most apparent, and the localisation of the pain. It is uninformative when it records only that the patient is "limping."

Imprecise: "Dog is limping on back leg."

Precise: "Grade 3/5 left pelvic limb lameness at walk, non-weight-bearing at trot. Onset acute two days ago after jumping from truck bed. Pain localised to stifle with positive cranial drawer test (2+ laxity, firm endpoint) and positive tibial thrust. No crepitus appreciated. Moderate joint effusion palpable. Contralateral stifle: negative drawer, negative thrust, no effusion, full range of motion. Patient resists stifle flexion and extension."

The precise version is approximately seven times longer than the imprecise version, and identifies a working diagnosis (cranial cruciate ligament rupture) without making an explicit diagnostic claim. A colleague who reads the precise version can construct the imaging plan and the orthopaedic referral without reviewing the case in person; a colleague who reads the imprecise version learns nothing.

Mass Documentation A mass description is informative when it specifies the dimensions in three axes, the consistency, the mobility, the location relative to anatomical landmarks, the overlying skin, the time course of growth, and the diagnostic samples obtained.

Imprecise: "Lump on side."

Precise: "2.5 × 2.0 × 1.5 cm firm, well-circumscribed, subcutaneous mass located over the left lateral thorax at approximately the eighth intercostal space, ten centimetres ventral to the dorsal midline. Non-painful on palpation. Freely mobile over the underlying ribs. Overlying skin normal — no ulceration, erythema, or alopecia. First noted by owner approximately three months ago; has grown approximately fifty per cent per owner estimate. Fine-needle aspiration performed: slides air-dried, submitted for cytology. Sample grossly white-tan, moderate cellularity."

The precise mass description anticipates the questions that will be asked about the mass at every subsequent encounter, every imaging review, and every histopathology report. The descriptive framework — dimensions, consistency, mobility, location, overlying skin, time course, diagnostic sample — is generic; it applies equally to subcutaneous masses, oral masses, intra-abdominal masses identified on imaging, and lymph nodes assessed on physical examination.

Quantification Standards Wherever a quantitative measurement can replace a subjective description, it should [18, 37]. The replacements that follow are durable across encounters and across providers:

Size: metric measurements in three dimensions (length × width × height in centimetres). Avoid food comparisons ("pea-sized," "lemon-sized"); they are imprecise and culturally variable.

Time: specific intervals (forty-eight hours, two weeks, three months) rather than vague descriptors (recently, a while ago, off and on).

Frequency: numeric counts per defined time period (six episodes in twenty-four hours; three to four bowel movements per day; one urination event per six hours).

Severity: established scales (lameness 0–5, body condition 1–9, pain composite 0–4, murmur Levine I–VI). Cite the scale by name where the scale is not universal.

Change: percentage or absolute (twenty per cent decrease in food intake; weight gain of 0.5 kg over six weeks).

Location: anatomical landmarks and distances from reference points. "Ten centimetres ventral to the dorsal midline at the eighth intercostal space" is durable; "on the side" is not.

Module 7 — Evidence-Based Assessment Frameworks The Assessment section of the SOAP record represents the analytic core of the medical record. It is the section in which observation is translated into interpretation, in which subjective and objective data are integrated into a diagnostic conclusion, and in which the practitioner’s reasoning becomes accessible to subsequent reviewers. Evidence-based frameworks ensure that assessments reflect current best practices and that the reasoning supporting them is traceable.

Structuring the Assessment A complete, defensible assessment includes the following elements [18, 35]: 1. Problem identification: a clear statement of the clinical problem or problems in specific terms. "Acute nonregenerative anaemia" is more useful than "anaemia"; "left-sided congestive heart failure" is more useful than "heart problem."

2. Working diagnosis: the most likely diagnosis, with the supporting rationale stated. The rationale is the chain of inference from observation to conclusion; it is what distinguishes a defensible diagnosis from a guess.

3. Differential diagnoses: alternative considerations, ranked by probability, with the criteria that would distinguish them.

The differential list is the practitioner’s safety net; an undocumented differential cannot be revisited if the working diagnosis fails to fit.

4. Guideline reference: citation of relevant consensus statements when applicable. ACVIM staging for cardiac and renal disease [4, 5, 6, 24], AAHA guidelines for vaccination [21, 22] and diabetes [7], AAFP feline standards [23], IRIS staging for chronic kidney disease [28, 39].

5. Staging or classification: where established systems exist, the stage should be cited along with the criteria that establish it.

6. Prognosis: expected outcome based on published data when available, expressed in language calibrated to the audience [16].

A Worked Assessment: Congestive Heart Failure with ACVIM Staging Consider a 10-year-old Cavalier King Charles Spaniel presenting with cough, exercise intolerance, and tachypnoea. The complete assessment might read as follows: Assessment: 10-year-old MN Cavalier King Charles Spaniel with left-sided congestive heart failure secondary to myxomatous mitral valve disease. ACVIM Stage C. Supporting evidence: Grade IV/VI left apical systolic murmur. Echocardiography demonstrates LA:Ao 2.1 and LVIDDN 1.95 (both substantially above ACVIM Stage B2 thresholds of 1.6 and 1.7 respectively), consistent with significant left atrial and left ventricular remodelling. NT-proBNP 2,840 pmol/L (markedly elevated). Thoracic radiographs: vertebral heart score 11.8 (reference <10.5), pulmonary venous distension, perihilar interstitial-to-alveolar pattern consistent with cardiogenic pulmonary oedema. Resting respiratory rate 38 breaths/min. Clinical signs (cough, exercise intolerance, tachypnoea) consistent with congestive failure.

Differential diagnoses: 1) Cardiogenic pulmonary oedema (working diagnosis, supported by echocardiographic remodelling, NT-proBNP elevation, and characteristic radiographic distribution). 2) Pulmonary hypertension secondary to MMVD (consider if cough disproportionate to oedema severity; would warrant echocardiographic evaluation of pulmonary artery pressures). 3) Bronchial collapse, age- and breed-related (would not produce the radiographic pulmonary venous distension noted). 4) Concurrent infectious pneumonia (less likely given absence of pyrexia, leukocytosis, or focal consolidation; should be reconsidered if treatment response is inadequate).

Staging: ACVIM Stage C — clinical signs of congestive failure with structural heart disease.

Prognosis: median survival from onset of CHF in MMVD is approximately 9–12 months with optimal medical management per ACVIM guidelines [4]. Owner counselled regarding prognosis and quality-of-life monitoring.

Each component of the assessment is supported by specific data; each citation traces back to a guideline or a measurable finding. The assessment is, in this form, simultaneously a clinical conclusion and a defence of that conclusion.

Module 8 — Diagnostic Logic and Decision Science Clinical reasoning is subject to the same cognitive biases that affect all human reasoning [11, 17]. The veterinary practitioner who is unaware of these biases is not insulated from them; they are insulated only from the awareness that prevents them. This module addresses the principal cognitive biases that affect veterinary diagnosis [18, 33] and the formal heuristics that the discipline has developed to manage diagnostic complexity.

Common Cognitive Biases Anchoring Anchoring is the tendency to commit to the first piece of information encountered and to under-adjust subsequent estimates in light of new data. The practitioner who learns from the receptionist that the patient "is here for vomiting" may anchor on gastrointestinal disease and weight subsequent findings against that anchor. The mitigation is the discipline of broad initial differential generation: before committing to a working diagnosis, list five plausible alternatives and identify the data that would support each.

Confirmation Bias Confirmation bias is the tendency to seek information that confirms a working hypothesis and to discount information that contradicts it. The practitioner who has decided that the cough is cardiac will read the radiographs looking for cardiomegaly and may underweight the bronchial pattern that suggests primary respiratory disease. The mitigation is explicit consideration of the data that would falsify the working diagnosis.

Availability Heuristic The availability heuristic is the tendency to overweight diagnoses that come easily to mind, typically because they were encountered recently or because they were memorable. The practitioner who diagnosed insulinoma last week may overestimate insulinoma in this week’s seizing patient. The mitigation is the discipline of base-rate awareness: insulinoma is an uncommon diagnosis, and idiopathic epilepsy is common.

Premature Closure Premature closure is the cognitive equivalent of stopping the search after finding the first answer that fits. The patient with a confirmed urinary tract infection may also have a urolith, a tumour, or a chronic kidney disease that the urinary tract infection masks. The mitigation is the discipline of asking, at each diagnostic checkpoint, what other diagnosis is consistent with the present data.

Diagnostic Momentum Diagnostic momentum is the tendency for a diagnosis applied early in a case to persist through subsequent encounters even when the evidence no longer supports it. The patient labelled "atopic dermatitis" by a colleague last year may receive that label uncritically at this year’s recheck even when the clinical picture has shifted toward a different aetiology. The mitigation is the explicit re-evaluation of working diagnoses at every encounter.

Search-Satisfying Search-satisfying is the tendency to stop searching once one finding has been identified. The radiograph that shows a foreign body in the small intestine still warrants evaluation for free peritoneal gas, mesenteric mass, hepatic abnormalities, and concurrent thoracic abnormalities. The mitigation is the discipline of completing a structured review of every imaging study regardless of when the principal finding is identified.

Occam’s Razor and Hickam’s Dictum Two competing heuristics describe the structure of differential diagnosis in complex patients. Occam’s razor — the principle that, where multiple explanations are possible, the simplest is usually correct — argues for unifying diagnoses.

Hickam’s dictum — that "a patient can have as many diseases as he damn well pleases" — argues that complex patients often have multiple concurrent conditions.

Both heuristics are correct, in different contexts. Occam’s razor is the appropriate frame for the young patient with acute disease, in whom the parsimonious diagnosis is usually the right one: the previously healthy three-year-old dog with vomiting, diarrhoea, and lethargy probably has a single gastrointestinal aetiology rather than three independent ones.

Hickam’s dictum is the appropriate frame for the older patient with chronic disease, in whom the burden of co-morbidity is high: the fourteen-year-old cat with renal insufficiency, hyperthyroidism, hypertension, and atrial enlargement has, by most counts, four diseases rather than one.

The discipline is the discipline of choosing the right frame for the patient. The young patient who appears to have multiple unrelated diseases probably has one disease that explains all of the findings; the older patient who appears to have one disease that explains all of the findings probably has multiple diseases that have been compressed by clinical inattention into a single label.

Module 9 — Leveraging Clinical Resources Contemporary veterinary practice is supported by a large and growing body of consensus statements, guidelines, formularies, and reference texts. The competent practitioner knows the principal documents in their field, knows when to consult each, and documents the consultation in the record. Practice without reference to evidence-based resources is not practice in line with current standards; it is practice as it was conducted before those standards existed.

Essential Clinical Resources The following resources are foundational to small-animal practice in North America. The list is not exhaustive but represents the documents whose recommendations appear most frequently in the medical record:

ACVIM Consensus Statements The American College of Veterinary Internal Medicine publishes consensus statements covering myxomatous mitral valve disease [4], dilated cardiomyopathy [44], hypertension [24, 41, 43], leptospirosis [25], immune-mediated haemolytic anaemia [5, 6], and other conditions. The statements are products of expert panels, are updated periodically, and are open access through the Journal of Veterinary Internal Medicine. They define the staging systems that structure clinical assessment for the conditions they cover [4, 30].

AAHA Guidelines The American Animal Hospital Association publishes guidelines covering vaccination [21, 22], diabetes management [7], dental care, anaesthesia, fluid therapy [38], weight management, and several other topics. The AAHA guidelines are organised around the workflow of the small-animal practice and are designed to be applied directly in the consulting room.

AAFP Feline Practice Guidelines The American Association of Feline Practitioners publishes feline-specific guidelines for vaccination, retroviral testing [23], blood-pressure management, and other species-specific topics [45]. The AAFP guidelines reflect the recognition that feline medicine differs from canine medicine in ways that the AAHA guidelines, written principally with the canine patient in mind, do not always capture.

IRIS CKD Staging The International Renal Interest Society staging system for chronic kidney disease classifies patients by serum creatinine and SDMA, and substages by proteinuria (UPC) and blood pressure [28, 39]. The IRIS system is the operative framework for chronic-kidney-disease management in both dogs and cats [40] and is referenced in essentially every contemporary CKD assessment.

Plumb’s Veterinary Drug Handbook Plumb’s [27] remains the standard formulary reference for small- and large-animal practice. Drug doses, indications, contraindications, and known interactions are summarised in a format optimised for clinical reference. The contemporary practitioner consults Plumb’s — or the equivalent peer-reviewed formulary [19] — for every unfamiliar drug, every unfamiliar indication, and every drug-drug interaction question.

Documenting the Application of Evidence When clinical decisions are made on the basis of consensus guidelines, the record should reflect that fact. The discipline is straightforward and additive: Cite the guideline by name and stage. "ACVIM Stage B2" rather than "early heart disease." "IRIS Stage 3" rather than "moderate kidney disease."

Document the criteria that established the stage. "LA:Ao 1.7, LVIDDN 1.85" for B2. "Creatinine 3.4 mg/dL on two occasions thirty days apart" for IRIS Stage 3."

Document the recommendation that follows from the stage. "Pimobendan 0.25 mg/kg PO q12h initiated per EPIC trial." "Phosphate-restricted diet initiated per IRIS Stage 3 recommendations; calcitriol considered."

Document any deviation from the guideline and the rationale. "ACE inhibitor deferred owing to concurrent dehydration; will reassess after rehydration is established."

The cumulative effect of these citations is a record that is both clinically defensible and educationally valuable. A colleague who reads the record can reconstruct the practitioner’s reasoning, locate the guideline referenced, and either concur with

the conclusion or identify the point at which the reasoning departed from the guideline. This is the discipline of evidencebased practice expressed in documentary form.

Module 10 — Value-Driven Practice Documentation is often perceived as time taken away from clinical care. The opposite is closer to the truth. The practitioner who documents efficiently spends less time on subsequent rework — the chart corrections, the clarifying telephone calls, the post-hoc reconstructions — than the practitioner who documents perfunctorily. The economics of documentation are non-linear and counter-intuitive: a small additional investment at the time of the encounter typically pays back several multiples of itself in the following weeks.

The Economics of Documentation A 2019 study of small-animal practice productivity [33] found that practitioners who documented at the moment of the encounter completed approximately fifteen per cent more cases per shift than practitioners who batched their documentation at the end of the shift. The difference was attributable not to faster typing but to faster clinical thinking: the practitioners who documented contemporaneously had reasoned through each case before moving on to the next and therefore did not return to the previous case to reconcile inconsistencies. The practitioners who batched documentation were performing an additional cognitive task — reconstructing the encounter from memory — that the contemporaneous documenters had eliminated [17, 33].

A second economic dimension concerns the relationship between documentation quality and revenue [35]. Insurance reimbursement, pre-authorisation processes, and the increasingly common practice of pet-insurance audit all depend on the contemporaneous record. A claim that lacks the supporting documentation will be reduced or denied even when the underlying clinical care was appropriate. The practitioner who has not documented the rationale for an MRI will find the MRI claim denied; the same practitioner who has documented "MRI indicated owing to acute non-ambulatory paraparesis with neuroanatomic localisation T3-L3, ruling out IVDD versus FCE versus neoplasia" will find the claim approved.

A third economic dimension concerns risk [9, 36]. Professional liability premiums are not directly tied to documentation quality, but the experience of carriers consistently demonstrates that practices with strong documentation generate substantially fewer paid claims [36]. The AVMA Professional Liability Insurance Trust reports that inadequate documentation is cited in over fifty per cent of successful malpractice claims [36]. A practice that invests in documentation discipline is investing in claim avoidance.

Documentation Efficiency Strategies Several practical strategies improve the speed of documentation without sacrificing quality: Templated structures with mandatory free-text components. The template captures the routine; the free-text fields capture what makes this case different from every other case. A template-only record is generic; a free-text-only record is unstructured; a hybrid record combines the speed of the first with the specificity of the second.

Voice-to-text transcription with subsequent editing. Modern voice-recognition software handles veterinary terminology adequately and produces draft entries faster than typing. The discipline is the discipline of editing the draft within the same encounter, not days later.

Standardised abbreviations from the practice’s approved list. Abbreviations that are universal within the practice — and ideally consistent with broader veterinary convention — speed documentation without sacrificing legibility, provided the practice maintains a current abbreviation reference. The comprehensive abbreviation list in Appendix L of this text is a starting point.

Real-time documentation in the examination room. The practice that has equipped its examination rooms with monitors and keyboards or tablets allows the practitioner to document during the encounter rather than between encounters. The practice that requires the practitioner to return to a workstation after each encounter has built a queue of documentation rework into the workflow.

Structured order sets for common protocols. The practice that has a documented protocol for canine parvovirus, feline DKA, paraparetic dogs, and the other recurrent emergencies has, in effect, pre-documented those cases; the practitioner adds patient-specific findings rather than reconstructing the protocol from first principles.

Module 11 — Teaching and Learning Excellence Documentation is a teachable skill [12, 15, 16]. The novice practitioner does not learn it by reading a textbook; they learn it by writing records, by having those records reviewed by a more experienced colleague, and by writing more records in light of the feedback [11, 17]. The discipline of documentation is the discipline of any clinical skill: it improves through structured practice with explicit feedback. This module addresses the teaching of documentation skills, both by formal mentors to formal trainees and by experienced practitioners to themselves [12].

Teaching Documentation Skills A formal documentation curriculum within an internship, residency, or clinical-mentor programme should include the following components: 1. Calibration. Trainees should compare their records to exemplar records on the same case, identify the differences, and describe the specific elements that distinguish the exemplar.

2. Structured feedback. Mentors should review trainee records against an explicit rubric — the PRECISE framework of Module 1 is one such rubric — and identify the dimensions on which improvement is required.

3. Iteration. The trainee should revise the record in light of the feedback and submit the revision; the mentor should review the revision against the original and confirm the improvement or identify the dimensions still requiring work.

4. Spaced practice. The trainee should encounter the same documentation challenges in different cases over time, with feedback distributed across the cases rather than concentrated in a single review session.

5. Self-assessment. The trainee should develop the capacity to apply the rubric to their own records before submitting them for review, and the mentor should progressively shift from prescriptive feedback to reflective questions ("what would you change about this record?").

The trajectory of competence in documentation, like the trajectory of competence in any clinical skill, follows a familiar curve. The novice produces records that are insufficient in obvious ways; the early-career practitioner produces records that are sufficient but inefficient; the experienced practitioner produces records that are simultaneously thorough and economical. The transitions between these stages are accelerated by feedback and slowed by its absence.

A Self-Assessment Rubric The following rubric, adapted from the PRECISE framework of Module 1, can be applied to one’s own records as a routine self-discipline. The practitioner should be able to answer "yes" to each question for every record produced: Patient-centred: does the record describe this patient specifically, rather than a generic patient?

Relevant: does every entry contribute to clinical decision-making, and would the record stand if the irrelevant entries were removed?

Evidence-based: are the staging systems, guidelines, and dose recommendations cited where applicable?

Complete: does the record contain all elements required for continuity of care, legal defensibility, and billing compliance for this visit type?

Immediate: was the record completed at the time of the encounter or shortly thereafter?

Specific: are vague descriptors replaced by quantitative measurements wherever possible?

Explicit: is the clinical reasoning visible, including the reasoning for declined diagnostics and the rationale for chosen treatments?

A practitioner who applies this rubric routinely to their own records, even briefly, will identify recurring patterns of weakness and address them more rapidly than a practitioner who relies on external review alone.

Module 12 — Clinical Communication Mastery The medical record is the durable artefact of a transaction between three parties — the practitioner, the client, and the patient — whose interests align imperfectly [3, 13, 16]. The practitioner is the steward of the patient’s welfare; the client is the decision-maker and the financial agent; the patient is non-verbal and dependent. The record must serve all three [11, 16]. It is the practitioner’s clinical instrument, the client’s evidence of informed consent, and the patient’s only durable voice across the period of care.

Informed Consent Documentation Informed consent is the legal and ethical foundation of every elective procedure and every significant intervention [9]. The discipline of informed-consent documentation is the discipline of capturing not only the client’s signature but the substance of the conversation that produced it [9, 16]. A signed consent form without a documented conversation is an artefact whose evidentiary weight is limited; a documented conversation with a corresponding signed form is unimpeachable [9].

A complete informed-consent record contains: 1. The procedure or intervention proposed, in language calibrated to the client.

2. The expected outcome and the prognosis with and without the intervention.

3. The principal risks of the intervention, expressed quantitatively where data exist. "Anaesthetic mortality risk approximately 0.05 per cent in healthy young patients, increasing with age and concurrent disease" is more useful than "small risk of complications."

4. The alternatives to the proposed intervention, including the option of no intervention.

5. The estimated cost of the intervention and the financial mechanism (in-clinic payment, pet insurance, financing).

6. The client’s questions and the practitioner’s responses.

7. The client’s explicit consent and the date and time of the conversation.

The discipline of recording the client’s questions is particularly worth emphasising. A client who asks no questions during a consent conversation is a client whose subsequent dissatisfaction will be expressed as "you never told me that this could happen." A client whose questions are documented in the record — "Owner asked specifically about the risk of complications related to anaesthetic depth in older patients; discussed standard monitoring (capnography, oximetry, blood pressure, ECG), recovery monitoring, and advance availability of reversal agents" — has, by their own questions, established the scope of the conversation.

Declined Recommendation Documentation When a client declines a recommended diagnostic or therapeutic intervention, the record must capture the recommendation, the reason for the recommendation, the alternatives offered, the client’s decision, and the practitioner’s discussion of the consequences of the declination. The substance is identical to informed consent in form; the difference is that the documented decision is to decline rather than to proceed.

Recommended abdominal radiographs to evaluate for foreign body or obstruction given the history of dietary indiscretion and the persistence of vomiting beyond twenty-four hours. Owner declined radiographs, citing financial constraints.

Discussed the risk that an obstructive lesion may not respond to symptomatic therapy and may progress to perforation if undiagnosed; discussed the warning signs (worsening vomiting, abdominal distension, lethargy) that would warrant immediate re-evaluation regardless of cost. Owner understands and accepts. Will proceed with empirical antiemetic therapy, fluid support, and a re-examination in 24–48 hours.

The declined-recommendation entry transforms what could be perceived as a deviation from the standard of care into a documented, joint decision in light of the client’s constraints. The practitioner has not departed from the standard; the practitioner has applied the standard, communicated it, and documented the client’s informed choice within it.

Module 13 — Knowledge Translation Skills Veterinary practice requires the practitioner to communicate the same clinical content to audiences whose technical vocabularies differ enormously [3, 13, 16]. The same case may be discussed with a specialist colleague (full medical lexicon), a colleague at the practice (clinical shorthand), a technician executing the orders (clear directives with monitoring criteria), a literate client with a healthcare background (plain medical English), and a lay client without technical background (analogy and cause-and-effect reasoning) [16]. The discipline of knowledge translation is the discipline of preserving the clinical content while modulating the form [11, 13].

Writing for Multiple Audiences Consider the same clinical conclusion — that an older Labrador Retriever has dilated cardiomyopathy with congestive heart failure — translated for five audiences: Specialist consultation: "Doberman-pattern DCM with biventricular CHF, atrial fibrillation, and ventricular ectopy. LVIDDN 2.4, FS 15%, NT-proBNP 3,847. Initiated pimobendan, furosemide, ACEi, diltiazem, spironolactone per ACVIM Stage C protocol. Question for cardiology: appropriate threshold for sotalol or mexiletine in light of 0.6% VPC burden."

Colleague at practice: "7-yr Doberman with classic DCM. Severe LV dilation, AF, frequent VPCs. CHF with pulmonary oedema. Started full quad therapy plus rate control. Will recheck Holter and renal values in two weeks."

Technician: "Frankie is hospitalised on furosemide IV q12h, pimobendan PO q12h, diltiazem PO q8h. Monitor resting respiratory rate every four hours; alert clinician if RR persistently above 30. Watch for vomiting, refusal of food, or worsening lethargy. Discharge tomorrow if RR controlled and BUN/creatinine acceptable."

Literate client: "Frankie has a heart-muscle disease called dilated cardiomyopathy. His heart muscle has weakened over time and the heart can no longer pump efficiently. This has caused fluid to back up into his lungs (heart failure) and has triggered an irregular heart rhythm. We have started medications to remove fluid (furosemide), to help the heart pump

more strongly (pimobendan), and to control the heart rhythm (diltiazem). Most dogs with this condition live three to six months on optimal medical management; some live longer."

Lay client: "Frankie’s heart muscle has gotten thin and stretched, like an old elastic band that has lost its snap. Because of that, the heart isn’t squeezing strongly enough to push the blood through, and some of the blood is backing up into the lungs. That’s why he’s coughing and tiring quickly. The medicines I’m starting today will pull the extra fluid out of his lungs and help his heart pump better. There’s no cure, but with good treatment most dogs do well for several months and many do well for longer."

The clinical content of the five entries is identical. The translation lies in the vocabulary and the use of analogy. A practitioner who can produce all five entries has acquired a competence that is qualitatively different from medical knowledge alone; they have acquired the competence to deliver medical knowledge in the form that the audience can use.

Record Retention and Legal Considerations Veterinary medical records must be retained for periods that vary by jurisdiction. Most North American states and provinces require retention for at least three to seven years from the date of last service, with longer periods specified for paediatric patients (typically the patient’s lifetime plus a defined number of years), for controlled-substance records (federal requirement of two years, often extended by state law), and for radiation-safety records (in some jurisdictions, the lifetime of the practice).

The practical implication is that a record produced today may need to be retrievable a decade from now, by a colleague, a regulator, or an attorney whom the practitioner cannot anticipate. Record-retention policies should be documented at the practice level, applied consistently across all records, and reviewed periodically against the prevailing regulations of the jurisdiction. Electronic records require additional consideration: the format in which records are stored today may not be readable by the software of a decade hence, and the practice that has not addressed format obsolescence may discover that its retained records are inaccessible at the moment they are needed.

Module 14 — Future-Ready Practice The structure of veterinary documentation is changing more rapidly than at any time in the history of the profession [2, 35].

Several technological and regulatory shifts have already begun to reshape the medical record, and the practitioner who is alert to these shifts will adapt to them more gracefully than the practitioner who is not. This closing module of Part I surveys the principal directions of change and identifies the documentation practices that will remain durable across the transitions [11, 35].

Emerging Trends AI-Assisted Documentation Real-time transcription tools and AI-assisted note generation are entering veterinary practice rapidly. The current generation of these tools converts the audio of a consultation into a structured SOAP draft within minutes of the consultation’s end. The practitioner’s task shifts from generating the record from memory to editing a draft generated by the tool. The principal hazard is the loss of the cognitive discipline that producing the record imposes; the principal opportunity is the elimination of the documentation lag that has bedevilled clinical practice since records began.

The discipline that the practitioner must preserve is the discipline of the assessment. The transcription of the encounter captures what was said; it does not capture what the practitioner concluded. The transition from transcription to assessment is the work that the practitioner must continue to do, and the AI-generated draft must be edited explicitly to reflect the practitioner’s clinical judgement rather than the patient’s presentation alone.

Telemedicine Records

The expansion of telemedicine has produced a parallel expansion of regulatory frameworks for telemedicine records. The veterinarian-client-patient relationship that historically required physical examination has been redefined in many jurisdictions to permit virtual establishment under specified conditions. The documentation requirements for telemedicine encounters are typically more demanding than for in-person encounters, because the medium’s limitations must themselves be addressed in the record: the practitioner who concludes a telemedicine encounter without physical examination must document explicitly why physical examination was not feasible, what alternative information sources were used, and the patient’s subsequent disposition.

Cloud-Based Electronic Health Records Cloud-based EHR systems have largely replaced the on-premises servers of the previous generation. The principal documentation implication is portability: the practice that adopts a cloud-based EHR can grant access to specialists, emergency clinics, and the client’s mobile device without the data-export workflows that on-premises systems required.

The principal risk is data sovereignty: the practice must understand where its data resides, who controls it, and what the contingency plan is if the EHR vendor exits the market or is acquired.

Genomic and Biometric Data Breed-specific genomic data and continuous biometric monitoring are entering the veterinary record. Breed-specific genomic panels identify the patient’s risk for the diseases prevalent in the breed; continuous biometric devices (collarmounted heart-rate monitors, activity trackers, temperature sensors) generate longitudinal data of clinical interest. The practical question for the practitioner is which data to incorporate into the record, in what form, and with what frequency.

The discipline that the practitioner must develop is the discipline of selectivity: not all of the available data is clinically meaningful, and the record that incorporates it indiscriminately becomes unreadable.

Standardised Veterinary Terminology SNOMED-CT-Vet and similar standardised vocabularies are slowly entering veterinary informatics. Standardised terminology supports interoperability between EHR systems, supports research that aggregates across practices, and supports the quality-improvement initiatives that depend on consistent labelling of conditions. The practitioner-level implication is modest in the short term — the SNOMED code is generated by the EHR rather than by the practitioner — but the discipline of using consistent terminology will be rewarded as the underlying systems mature.

What Will Remain Durable The technological landscape will continue to change. The disciplines that this text has emphasised — the deliberate construction of the assessment, the explicit reasoning that distinguishes a defensible conclusion from a guess, the disciplined separation of subjective from objective from analytic content, the documentation of the conversation with the client — will remain durable across the changes. The form of the record may evolve from the paper file to the cloud-hosted EHR to the AI-generated draft to whatever follows; the substance, which is the practitioner’s reasoning made visible, will remain the structural backbone of veterinary practice.

Consequences of Inadequate Documentation The closing reflection of Part I addresses what happens when documentation falls short. The hazard is multidimensional: legal, clinical, financial, professional, and personal. The practitioner who does not document well is exposed in each dimension simultaneously, and the exposures compound each other. A legal claim that succeeds will affect the practitioner’s licensure, which will affect their employment, which will affect their finances. A clinical error that recurs because the previous error was not documented will affect the patient, the client’s trust, and ultimately the practice’s reputation. The cost of inadequate documentation is rarely paid in the dimension in which the inadequacy occurred.

Legal and Regulatory Consequences The legal consequences of inadequate documentation are the most consistently described in the veterinary literature [8, 9, 36]. Professional liability claims are decided on the basis of the contemporaneous record, and a record that does not support the standard of care will generally lose at adjudication regardless of whether the underlying care was appropriate [8, 36]. The AVMA-PLIT and other professional liability carriers consistently report that inadequate medical records are a leading factor in successful malpractice claims [36]. Licensing boards apply a similar logic in disciplinary proceedings: the absence of documentation is interpreted as the absence of the corresponding clinical activity [1, 8].

A specific subset of legal consequences arises from the failure to document informed consent [9]. A procedure performed without documented consent is exposed to claims of unauthorised treatment, which carry implications beyond ordinary negligence. The remedy is straightforward in theory and uneven in practice: every elective procedure and every significant intervention should be preceded by a documented consent conversation, and the documentation should reflect the substance of the conversation rather than the bare fact of the signed form [9, 16].

Clinical and Patient-Care Consequences The clinical consequences of inadequate documentation are subtler than the legal consequences but more frequent [11, 18]. A record that does not capture the prior history exposes the patient to repeated diagnostic work, missed trends, and discontinuities at the moment of greatest need (the after-hours encounter, the on-call colleague, the emergency referral). A patient with chronic kidney disease whose creatinine has been documented at every visit can be staged accurately [28, 39]; a patient whose creatinine has been documented sporadically cannot.

Medication errors are a particular hazard of inadequate documentation [27, 33]. The patient whose drug allergies are not in the prominent header of the record will eventually receive the drug to which they are allergic; the patient whose previous adverse reactions are buried in a free-text field will eventually receive the drug that produced the reaction [33]. The remedy is the structured allergy and adverse-reaction list at the front of every record, updated at every visit at which a new exposure occurs.

Financial and Business Consequences Inadequate documentation has direct financial consequences for the practice [13, 35]. Insurance and pet-insurance claims that lack the supporting documentation are reduced or denied. Diagnostic and therapeutic services that were performed but not documented cannot be billed defensibly. Audits — internal, external, or regulatory — that identify documentation gaps may trigger refunds, fines, or, in the extreme case, the termination of insurance contracts.

A subtler financial consequence is the cost of staff time spent reconstructing inadequate records [33]. The technician who must telephone the owner to confirm a dose that was not recorded, the receptionist who must assemble a referral packet from incomplete records, and the practice manager who must respond to a regulatory inquiry are all spending time that better documentation would have saved.

Professional and Reputational Consequences The professional consequences of inadequate documentation accrue more slowly than the legal or financial consequences but are no less real. Colleagues who have inherited cases from a practitioner whose records are unreadable will recommend that practitioner less frequently. Specialists who have received uninformative referrals will eventually direct their reciprocal referrals elsewhere. The practitioner whose name is associated with consistent

documentation discipline acquires a reputation that supports their career; the practitioner whose name is associated with the opposite acquires a reputation that constrains their career.

The personal dimension of these consequences is often overlooked. The practitioner who has been named in a malpractice claim, who has faced licensing-board scrutiny, or who has been confronted with a documentation deficiency in the middle of an otherwise routine workday experiences the events as personal failure rather than as predictable consequence. The investment in documentation discipline is, viewed from this angle, an investment in personal welfare. A practitioner whose records routinely defend themselves does not experience the documentation review as a threat.

Part I closes with the proposition that opened it: precise documentation is the discipline that makes precise medicine possible. The fourteen modules of Part I have addressed the structure, the contexts, the language, the reasoning, and the institutional setting of the discipline. The case studies of Part II demonstrate the integration of the discipline in fully worked clinical scenarios. The appendices of Part III provide the reference material that the discipline draws on day to day. Read together, the three parts of this text constitute the substrate of competent contemporary veterinary documentation.

PART II CLINICAL CASE STUDIES Three fully worked cases illustrating the integration of documentation discipline with clinical reasoning

Case Study 1 — Dilated Cardiomyopathy in a Doberman Pinscher The first case study integrates the documentation disciplines developed in Part I with the clinical reasoning required for a complex cardiac presentation. The patient is a seven-year-old neutered male Doberman Pinscher referred for evaluation of progressive exercise intolerance and cough. The case is selected because it illustrates the use of the ACVIM staging system, the integration of multimodal diagnostic data, and the disciplined communication of a guarded prognosis.

Signalment and Presentation Patient: "Frankie," 7-year-old MN Doberman Pinscher, 42 kg, BCS 5/9.

Chief complaint: Exercise intolerance and cough × 3 weeks.

History of Present Illness Progressive exercise intolerance over three weeks. The patient previously jogged five kilometres daily with the owner without fatigue; now fatigues after 0.5 kilometres and refuses to continue. A soft, non-productive cough developed approximately one week ago, worse at night and when lying down. No change in appetite (eating well). No vomiting, diarrhoea, syncope, or collapse episodes. Owner has not noted any abdominal distension, although recognises in retrospect that he had attributed mild abdominal fullness to weight gain.

Past medical history: no previous cardiac history. Annual examinations have been unremarkable, and the patient has never been screened for occult cardiomyopathy despite the breed risk. Heartworm prevention is current (ProHeart, last administered six weeks prior to presentation). Diet: a major-brand large-breed adult maintenance formula. No grain-free or boutique diet exposure. The owner reports no prior anaesthetic events and no surgical history.

Physical Examination General: quiet but alert and responsive. BCS 5/9. Mild temporal and epaxial muscle wasting noted, consistent with subacute systemic disease.

Vital parameters: T 101.2°F, HR 160 bpm (irregularly irregular), RR 36 breaths/min.

Cardiovascular: irregularly irregular rhythm with palpable pulse deficits (approximately twenty per cent of expected pulses absent at the femoral artery). Grade II/VI left apical systolic murmur. Weak, rapid femoral pulses. Jugular distension present without overt pulsation. Jugular vein evaluated specifically and the absence of jugular pulsation documented as a pertinent negative.

Respiratory: mildly increased bronchovesicular sounds bilaterally. Soft end-inspiratory crackles caudodorsally in both hemithoraces, more prominent on the left. No wheezes. Tracheal pinch test elicits a soft cough, not a paroxysm.

Abdominal: hepatomegaly palpable two to three centimetres beyond the costal arch. A fluid wave is questionable; the practitioner suspects small-volume ascites but cannot confirm by palpation alone. No discrete masses. No splenomegaly.

Peripheral: no peripheral oedema. Femoral pulses described above. Capillary refill 1.5 seconds. Mucous membranes pink, slightly muddy.

Diagnostic Workup A diagnostic plan was constructed to evaluate the cardiac, respiratory, and renal status of the patient and to establish the ACVIM stage of the underlying cardiomyopathy.

Electrocardiography

Six-lead ECG performed. Rhythm: atrial fibrillation, ventricular response 160 bpm. Forty-seven ventricular premature complexes documented in a five-minute recording, most unifocal but several multifocal. QRS width within normal limits. No ST-segment changes.

Thoracic Radiography Three-view thoracic radiographs (right lateral, left lateral, ventrodorsal). Generalised cardiomegaly with vertebral heart score 13.2 vertebrae (reference <10.5 for the breed). Left atrial enlargement evidenced by dorsal deviation of the trachea and splitting of the mainstem bronchi on the lateral view. Pulmonary venous distension. Perihilar interstitial-to-alveolar pattern consistent with cardiogenic pulmonary oedema. Small-volume pleural effusion bilaterally.

Echocardiography Echocardiography performed by the referring practitioner with subsequent over-read by a board-certified cardiologist.

Severely dilated left ventricle (LVIDDN 2.4; reference <1.73 for Dobermans per ACVIM consensus). Severely dilated endsystolic dimension (LVIDSN 1.9). Markedly reduced systolic function (fractional shortening 15%; reference >25%). Left atrial enlargement (LA:Ao 1.9). The mitral valve leaflets are structurally normal; the regurgitation observed is functional, secondary to annular dilation. E-point septal separation 18 mm (increased). Sphericity index increased.

Laboratory NT-proBNP 3,847 pmol/L (markedly elevated; reference <900). Cardiac troponin I 0.28 ng/mL (mildly elevated). CBC within normal limits. Serum chemistry: BUN 32 mg/dL, creatinine 1.4 mg/dL — mild azotaemia, likely prerenal in the context of CHF. Electrolytes within normal limits. Urinalysis: USG 1.038, no proteinuria, no haematuria, no glucosuria.

Twenty-Four-Hour Holter Monitor Atrial fibrillation throughout the recording. Mean ventricular rate 142 bpm (range 98–198). Eight hundred and forty-seven VPCs total, representing a ventricular ectopy burden of 0.6%. Twelve couplets and two triplets documented. No sustained ventricular tachycardia.

PART II CLINICAL CASE STUDIES Three fully worked cases illustrating the integration of documentation discipline with clinical reasoning

Assessment

Figure C1.1 — Pathophysiology of dilated cardiomyopathy in the Doberman, with the cascade leading to congestive heart failure

Dilated cardiomyopathy with atrial fibrillation and biventricular congestive heart failure. ACVIM Stage C [4, 30, 44].

The presentation is classic for Doberman DCM, a disease that affects approximately fifty per cent of the breed in some populations [46-48] and that has a substantial sub-clinical phase preceding overt failure. The combination of severe systolic dysfunction (FS 15%), severe chamber dilation (LVIDDN 2.4), atrial fibrillation, and clinical signs of congestion (cough, crackles, pulmonary venous distension and oedema on radiographs) confirms advanced disease [44, 46]. The presence of atrial fibrillation worsens prognosis and requires rate control to prevent further decompensation [48]. The ventricular ectopy burden of 0.6%, while not at the threshold typically considered for antiarrhythmic therapy, increases the risk of sudden cardiac death and warrants close monitoring [48].

Differential diagnoses considered and ranked: 1) Dilated cardiomyopathy with biventricular CHF (working diagnosis, supported by echocardiographic systolic dysfunction, chamber dilation, NT-proBNP elevation, AF, and characteristic radiographic findings); 2) Pulmonary hypertension secondary to cardiomyopathy (would warrant Doppler estimation of pulmonary artery pressures; not pursued at this consultation but flagged for re-evaluation); 3) Concurrent infectious or aspiration pneumonia (less likely given absence of pyrexia and leukocytosis; would reconsider if treatment response inadequate); 4) Diet-associated DCM (less likely given absence of grain-free or boutique diet exposure; will measure taurine and carnitine to rule out reversible component).

Treatment Plan per ACVIM Stage C Guidelines Acute management [4, 27]: Furosemide 2 mg/kg IV bolus, then 2 mg/kg PO q12h.

Oxygen supplementation as needed for tachypnoea above 40.

Cage rest during stabilisation.

Chronic management: Pimobendan 0.25 mg/kg PO q12h (positive inotrope; standard of care per ACVIM guidelines for Stage C MMVD and DCM).

Furosemide 2 mg/kg PO q12h, titrated to lowest effective dose monitoring resting respiratory rate.

Enalapril 0.5 mg/kg PO q12h, initiated after renal values stabilise post-rehydration.

Diltiazem 1.5–2 mg/kg PO q8h for rate control (target resting heart rate 120–140 bpm).

Spironolactone 1 mg/kg PO q12h (aldosterone antagonist).

Consider sotalol or mexiletine if VPC burden increases or sustained VT develops.

Dietary: Sodium-restricted veterinary cardiac diet.

Taurine 500 mg PO q12h pending plasma taurine and whole-blood taurine results; carnitine supplementation considered.

Monitoring: Owner to monitor resting respiratory rate at home; target <30 breaths/min; alert clinician at >30 sustained.

Recheck in 5–7 days for assessment of treatment response, renal values, and electrolytes.

Repeat Holter monitoring in 2–4 weeks to assess rate control and arrhythmia burden.

Repeat echocardiogram in 3 months.

Client Communication Discussed the diagnosis, prognosis, and treatment plan with the owner over a forty-minute consultation. Explained that Doberman DCM is a genetic disease affecting approximately half of the breed in some populations, and that the disease has a substantial silent phase preceding the onset of clinical signs. With optimal medical management, the median survival from the onset of CHF is approximately three to six months, although individual variation is considerable and some patients live materially longer. The presence of atrial fibrillation and ventricular arrhythmia increases the risk of sudden cardiac death despite treatment.

The owner asked specifically whether ICD or other device-based therapies are available; explained that these therapies are not standard in veterinary practice and are not currently offered at most institutions. The owner asked about the possibility of cardiac transplantation; explained that veterinary cardiac transplantation is investigational and not available in clinical practice. The owner asked about quality-of-life monitoring; provided written guidance on resting respiratory rate monitoring, exercise tolerance assessment, and the warning signs that warrant immediate re-evaluation.

The owner understands the prognosis and elects aggressive medical management. Written discharge instructions provided. Emergency contact information for after-hours and weekend care discussed. Estimate for hospitalisation, diagnostics, and initial pharmacotherapy: USD 3,200; payment plan established.

Prognosis Guarded to poor [46-48]. The median survival for Doberman DCM with congestive heart failure is approximately three to six months despite optimal treatment [46]. The presence of atrial fibrillation and ventricular ectopy further worsens prognosis [48], and the patient remains at significant risk of sudden death from arrhythmia or acute decompensation. The owner has been counselled accordingly.

Case Study 2 — Diabetic Ketoacidosis in a Feline Patient The second case study illustrates the documentation of an emergency presentation, the integration of point-of-care diagnostics with formal laboratory data, and the structured execution of an evidence-based treatment protocol over multiple phases of stabilisation.

Signalment and Presentation Patient: "Mochi," 11-year-old MN Domestic Shorthair, 4.2 kg (owner reports 6.8 kg six months ago), BCS 3/9.

Chief complaint: Vomiting, lethargy, and decreased appetite × 3 days.

History of Present Illness Progressive weight loss over six months despite reportedly increased appetite during the same period. The owner recalls the patient eating "constantly" and drinking more water than usual for "a few months" but attributed these observations to summer heat and a recent change in diet. No previous diagnosis of diabetes mellitus has been recorded. The patient has not been seen for veterinary care in eighteen months.

Acute deterioration began three days prior to presentation with vomiting (six to eight episodes on the first day, declining to two to three episodes daily thereafter). The patient stopped eating completely two days prior to presentation. Owner notes progressively increased lethargy — the patient is now barely moving from his bed. The owner describes the patient’s urine as possibly darker than usual and reports an "acetone smell" near the litter box noted yesterday.

Environment: indoor only, single-cat household. Vaccines current. Diet: dry kibble offered free-choice (a major-brand maintenance formula). No known toxin exposure (no antifreeze, no lily exposure, no over-the-counter analgesics). No new medications. No travel.

Physical Examination General: obtunded, laterally recumbent, minimal response to stimulation. Severe muscle wasting with prominent spinous processes and pelvic landmarks. BCS 3/9.

Vital parameters: T 99.1°F (hypothermia), HR 200 bpm, RR 40 breaths/min with increased effort. Hydration estimated at ten per cent — skin tenting persists for more than three seconds, eyes are sunken, mucous membranes are dry and tacky.

Breath: a strong fruity acetone odour is noted on close approach.

Cardiovascular: tachycardia with regular rhythm. Weak femoral pulses. CRT three seconds. Mucous membranes pale.

Respiratory: increased rate and depth consistent with Kussmaul-pattern breathing, attributable to compensatory hyperventilation in response to severe metabolic acidosis. Lung sounds clear bilaterally. No coughing, no nasal discharge.

Abdominal: hepatomegaly palpable. Small urinary bladder. No pain on palpation. No discrete masses.

Neurological: obtunded, weak. Plantigrade stance noted in the hind limbs when the patient is supported in standing — a finding consistent with diabetic polyneuropathy.

Emergency Diagnostics Point-of-Care Glucometer: greater than 600 mg/dL (exceeds the meter’s upper limit). Urine dipstick: glucose 4+, ketones 3+. Packed cell volume 52% (haemoconcentration consistent with severe dehydration). Total protein 9.2 g/dL.

Venous Blood Gas

pH 7.08 (severe acidaemia; reference 7.35–7.45). pCO2 18 mmHg (compensatory respiratory alkalosis). Bicarbonate 8 mEq/L (severe metabolic acidosis; reference 18–24). Base excess −20 mEq/L. Anion gap markedly elevated (calculated >30 mEq/L).

Electrolytes Sodium 138 mEq/L (within reference but corrected sodium for the magnitude of hyperglycaemia is likely materially elevated). Potassium 4.8 mEq/L (within reference but anticipated to fall with insulin therapy and rehydration; potassium supplementation must be initiated proactively). Chloride 98 mEq/L. Ionised calcium 1.1 mmol/L (low normal).

Serum Chemistry BUN 68 mg/dL (elevated; differential of prerenal azotaemia versus early intrinsic renal disease pending response to rehydration). Creatinine 3.8 mg/dL (elevated). Phosphorus 8.2 mg/dL (elevated). ALT 245 U/L (elevated, consistent with hepatic lipidosis in this severely catabolic patient). ALP 189 U/L (elevated). Total bilirubin 1.2 mg/dL (mildly elevated). Betahydroxybutyrate 6.8 mmol/L (severely elevated; reference <0.6).

Urinalysis USG 1.045 (the urine is concentrated despite the polyuric presentation, an apparent paradox explained by the osmotic effect of glucosuria). Glucose 4+. Ketones 3+. Protein 1+. Sediment: 0–2 WBC/hpf, 0–1 RBC/hpf, no bacteria, no crystals.

Culture pending.

Assessment

Figure C2.1 — Pathophysiology of diabetic ketoacidosis

Diabetic ketoacidosis with severe metabolic derangement [7, 49].

Diagnosis is supported by marked hyperglycaemia (>600 mg/dL); ketonaemia (β-hydroxybutyrate 6.8 mmol/L); ketonuria (3+); severe metabolic acidosis (pH 7.08, HCO3 8 mEq/L); and a clinical presentation consistent with the diagnosis (chronic weight loss with PU/PD, acute vomiting, anorexia, obtundation).

Concurrent problems: severe dehydration estimated at ten per cent; prerenal azotaemia (BUN 68, creatinine 3.8) requiring reassessment after rehydration; probable concurrent hepatic lipidosis (elevated ALT/ALP, weight loss, anorexia); diabetic polyneuropathy (plantigrade stance); and probable urinary tract infection (the diabetic patient is at increased risk; pending culture).

This is a life-threatening emergency. The combined severity of the dehydration, the metabolic acidosis, and the electrolyte derangement that will follow insulin therapy place the patient in a category requiring intensive care for the first 24–48 hours. The owner has been advised that survival to discharge is approximately seventy per cent with appropriate intensive care; this figure is consistent with published outcomes for feline DKA.

Treatment Protocol per AAHA Diabetes Guidelines Phase 1 — Initial Stabilisation, Hours 0–6 [7, 27]: IV catheter placed (22-gauge cephalic).

0.9% NaCl: initial bolus 10 mL/kg over thirty minutes, reassess perfusion, then continue at twice maintenance rate (approximately 18 mL/hour for this patient).

Potassium supplementation: 20 mEq KCl per litre of fluids, anticipating the fall in serum potassium that will accompany insulin therapy and rehydration. Reassess potassium every four hours and adjust the supplementation

accordingly.

Begin regular insulin CRI at hour two (allowing initial volume restoration to occur first): 1.1 U/kg/day delivered as 0.05 U/kg/hour.

Monitor blood glucose every one to two hours initially. Target a decrease of 50–75 mg/dL per hour; rapid drops increase the risk of cerebral oedema.

Add dextrose to fluids when blood glucose falls below 250 mg/dL (2.5% initially, increase to 5% if needed) to allow continuation of insulin therapy without inducing hypoglycaemia.

Phosphorus supplementation if serum phosphorus falls below 1.5 mg/dL. Phase 2 — Continued Management, Hours 6–24: Continue IV fluids; reassess rate based on hydration, urine output, and ongoing losses.

Continue insulin CRI; adjust based on glucose response.

Monitor electrolytes every four to six hours initially.

Begin antiemetics if vomiting continues: maropitant 1 mg/kg IV q24h.

Begin enteral nutrition as soon as vomiting is controlled — critical to prevent hepatic lipidosis. A nasogastric or oesophagostomy feeding tube should be considered within the first twenty-four hours if the patient does not eat voluntarily.

Phase 3 — Transition, Hours 24–72: When the patient is eating, blood glucose is stable, and ketones are negative, transition from CRI to subcutaneous insulin.

Glargine (Lantus) 1–2 U/cat BID initially.

Continue monitoring blood glucose every four to six hours during the transition.

Discharge planning includes owner education in subcutaneous injection, glucose monitoring, dietary management, and the recognition of hypoglycaemia.

Client Communication Discussed diagnosis, severity, and the treatment plan with the owner over a forty-five-minute consultation. Explained that diabetic ketoacidosis is a life-threatening emergency, that survival to discharge is approximately seventy per cent with appropriate intensive care, and that treatment will require two to five days of hospitalisation. The estimate for the initial 48– 72 hours of care is USD 2,500–3,500.

Long-term management will require twice-daily insulin injections, a dietary change to a high-protein low-carbohydrate diet, and regular monitoring including periodic blood-glucose curves and fructosamine measurement. Diabetic remission is possible in 25–30% of cats with appropriate management, particularly with early aggressive control and an appropriate diet.

The owner asked about the role of dietary therapy alone; explained that dietary therapy is supportive but not sufficient in the established diabetic, and that insulin is required for control. The owner asked about the cost of long-term management; provided a written estimate covering insulin, glucometer and supplies, dietary therapy, and the recommended schedule of recheck appointments. The owner understands the severity, accepts the treatment plan, and provides written authorisation for intensive care including the placement of a feeding tube if required.

Prognosis Guarded for short-term survival, with approximately seventy per cent survival to discharge with appropriate treatment [49].

If the patient survives the initial crisis, prognosis for long-term management is fair to good. Remission is possible in twenty-five to thirty per cent of appropriately managed feline diabetics [50]. Hepatic lipidosis is a particular concern given the anorexia, the weight loss, and the elevated hepatic enzymes; early nutritional support is essential [49].

Case Study 3 — Immune-Mediated Haemolytic Anaemia The third case study illustrates the documentation of a complex haematological emergency, the integration of laboratory and imaging findings with the clinical presentation, and the structured discussion of treatment options with substantial morbidity and a guarded prognosis.

Signalment and Presentation Patient: "Daisy," 5-year-old FS Cocker Spaniel, 11 kg, BCS 6/9.

Chief complaint: Lethargy, weakness, and decreased appetite × 4 days.

History of Present Illness Progressive lethargy developing over four days. The owner initially noticed that the patient was "not herself" — less interested in walks, sleeping more. Appetite decreased to approximately fifty per cent of normal. Pale gums noted by the owner the day before presentation. Possible darker urine noted but not confirmed. No vomiting, diarrhoea, or coughing. No bleeding from any orifice.

No known toxin exposure: no access to onions, garlic, zinc-containing objects, or pennies. No recent medications — not on any chronic therapies. No recent vaccinations (last vaccinations approximately fourteen months ago). No recent surgery or transfusions. No travel history. No exposure to ticks or fleas of clinical concern.

Past medical history: unremarkable. No previous episodes of similar illness. Heartworm and ectoparasite prevention current.

Physical Examination General: lethargic, weak when standing, prefers to lie down. BCS 6/9.

Vital parameters: T 103.4°F (mild pyrexia), HR 160 bpm (tachycardia), RR 44 breaths/min, BP 142/88 mmHg. Mucous membranes: markedly pale (white). Sclerae icteric (yellow). The icterus was confirmed against natural light, not against the consulting-room fluorescent lighting in which it was first appreciated.

Cardiovascular: tachycardia with regular rhythm. A grade II/VI systolic left basilar haemic murmur is auscultated, attributable to the anaemia. Strong but rapid femoral pulses. CRT 1.5 seconds.

Respiratory: tachypnoea with clear lung sounds.

Abdominal: splenomegaly palpable two to three centimetres beyond the costal arch. The liver may be mildly enlarged but cannot be confirmed by palpation alone. No abdominal pain.

Lymph nodes: within normal limits.

Integument: no petechiae, ecchymoses, or evidence of bleeding.

Diagnostic Workup Complete Blood Count Packed cell volume 14% (severely anaemic; reference 35–55%). RBC 2.1 × 10^12/L. Haemoglobin 4.8 g/dL. Reticulocyte count 285,000/μL (markedly regenerative; reference <60,000). MCV 78 fL (macrocytic, consistent with a vigorous regenerative response). WBC 18,500/μL (mild leukocytosis). Neutrophils 14,200/μL. Platelets 165,000/μL (low normal).

Blood Smear Review

Spherocytes 3+ — pathognomonic for immune-mediated red cell destruction in the dog. Polychromasia 3+ (regenerative response). Anisocytosis 3+. nRBCs 2 per 100 WBCs. Howell-Jolly bodies occasional. No evidence of blood parasites.

WBC morphology normal.

Coombs and Slide Agglutination Slide agglutination test positive (microscopic agglutination confirmed in saline-diluted preparation, ruling out rouleaux as the cause of the apparent agglutination). Direct Coombs test positive (IgG, IgM components both detected).

Serum Chemistry Total bilirubin 4.2 mg/dL (markedly elevated; reference <0.4). Indirect bilirubin predominant, consistent with haemolysis.

ALT 145 U/L (mildly elevated). ALP 220 U/L (mildly elevated). BUN 28 mg/dL (within reference). Creatinine 0.9 mg/dL (within reference). Albumin 3.1 g/dL (low-normal). Globulins 4.2 g/dL.

Urinalysis USG 1.022. Bilirubin 3+ (consistent with haemolysis). Haemoglobinuria positive. No glucosuria, no proteinuria, no significant sediment.

Imaging Three-view thoracic radiographs: no evidence of pulmonary thromboembolism (a recognised complication of IMHA), no cardiomegaly, no pulmonary infiltrates. Abdominal ultrasonography: mild splenomegaly without focal lesions; mild hepatomegaly with diffuse hyperechoic parenchyma; gallbladder unremarkable; no abdominal masses; no lymphadenopathy.

Infectious Disease and Tick-Borne Screen SNAP 4Dx Plus negative for heartworm antigen, Ehrlichia, Anaplasma, and Lyme. Babesia PCR negative.

Assessment

Figure C3.1 — Mechanism of immune-mediated haemolytic anaemia

Primary (idiopathic) immune-mediated haemolytic anaemia [5, 6, 51].

Diagnosis is supported by severe regenerative anaemia (PCV 14%, reticulocytosis 285,000/μL); spherocytosis 3+ (pathognomonic in the dog) [5]; positive slide agglutination and Coombs testing [5]; haemolytic biochemistry pattern (markedly elevated indirect bilirubin, bilirubinuria, haemoglobinuria); and the absence of identifiable triggers. The negative tick-borne screen, the absence of recent vaccinations or medications, and the absence of any neoplastic mass on imaging support the classification as primary rather than secondary IMHA [5, 51].

Concurrent considerations: the patient is at significant risk for pulmonary thromboembolism, the leading cause of mortality in IMHA [6, 51]. Hypercoagulability is anticipated and prophylactic anticoagulation is indicated [6]. The patient is also at risk for transfusion reaction; type-and-cross matching is essential before any blood-product administration.

Differential diagnoses considered and ranked: 1) Primary (idiopathic) IMHA (working diagnosis, supported by criteria above); 2) Secondary IMHA from occult neoplasia (ruled out as far as imaging permits, but cannot be entirely excluded; would warrant re-evaluation if the patient relapses despite immunosuppression); 3) Tick-borne disease causing haemolysis (ruled out by negative serology and PCR); 4) Drug- or vaccine-induced IMHA (no temporal relationship identified); 5) Microangiopathic haemolytic anaemia (no schistocytes on smear, no evidence of DIC).

Treatment Plan Immunosuppression [6, 27, 51]: Prednisone 2 mg/kg PO q24h, with the recognition that doses of 2 mg/kg per day are at the upper end of the conventional dosing range and that some patients require higher doses for initial control.

Azathioprine 2 mg/kg PO q24h × 14 days, then q48h, as a steroid-sparing second-line agent.

Consideration of cyclosporine 5 mg/kg PO q12h if response to prednisone and azathioprine is inadequate at 7–10 days.

Anticoagulation: Clopidogrel 2 mg/kg PO q24h for thromboprophylaxis throughout the immunosuppressive course.

Consider the addition of low-molecular-weight heparin (enoxaparin 1 mg/kg SC q12h) in this patient given the severity of the anaemia and the breed predisposition.

Supportive care: Type-and-cross-matched packed red cell transfusion if PCV falls below 15% with continued clinical deterioration, or below 12% regardless of clinical state.

Gastroprotection: omeprazole 1 mg/kg PO q24h during high-dose prednisone therapy.

Monitor for the well-recognised complications of immunosuppression: opportunistic infection, gastrointestinal ulceration, and the metabolic effects of glucocorticoid therapy.

Monitoring: Daily PCV, total solids, and clinical assessment during hospitalisation.

Recheck CBC weekly for the first month, then biweekly to monthly as the patient stabilises.

Liver enzymes monitored monthly during azathioprine therapy.

Slow taper of prednisone over four to six months, contingent on sustained haematological response.

Client Communication Discussed the diagnosis, prognosis, and treatment plan with the owner over a fifty-minute consultation. Explained that immune-mediated haemolytic anaemia is a serious disease in which the patient’s immune system mistakenly destroys her own red blood cells. The disease has a mortality rate of approximately twenty-five to fifty per cent in the first month even with appropriate treatment, attributable principally to pulmonary thromboembolism, infection during immunosuppression, and refractory disease. Survivors typically require months of immunosuppressive therapy with the corresponding side effects, and a subset experiences relapse despite treatment.

The owner asked specifically about the cost of treatment; provided a written estimate of approximately USD 4,500–6,500 for the first three months including hospitalisation, transfusions if required, medications, and recheck appointments. The owner asked about the prognosis for cure versus the probability of long-term management; explained that the goal of treatment is sustained remission and that some patients can be tapered off immunosuppression, but that lifelong therapy is required in some cases. The owner asked about the side effects of the medications; reviewed the principal side effects of prednisone (polyuria, polydipsia, polyphagia, panting, weight gain, immunosuppression, gastrointestinal effects), azathioprine (bone marrow suppression, hepatotoxicity, gastrointestinal effects), and cyclosporine (gastrointestinal effects, gingival hyperplasia, opportunistic infection, expense).

The owner understands the severity, accepts the treatment plan, and consents to hospitalisation, transfusion if required, and the prescribed pharmacological regimen.

Prognosis Guarded [5, 6, 51]. Mortality from IMHA in the dog is approximately twenty-five to fifty per cent within the first thirty days of diagnosis despite appropriate treatment [51]. The principal cause of death is pulmonary thromboembolism [6, 51].

Survivors face a prolonged course of immunosuppression with the attendant side effects and the risk of relapse. Longterm remission is achievable in a substantial proportion of survivors, although lifelong therapy may be required in some cases.

Appendix A — IRIS CKD Staging Guidelines The International Renal Interest Society (IRIS) staging system [28, 39] is the consensus framework for chronic kidney disease in dogs and cats [40]. The system stages by serum creatinine and SDMA, and substages by proteinuria (UPC) and blood pressure [28, 41]. The staging conducted at presentation should be repeated as the disease progresses; a single stage assignment captures the disease at one moment in time, not the trajectory of the disease over time.

Glossary of Key Terms A working glossary of the principal technical terms used throughout this text. Definitions are pitched at the level of a smallanimal practitioner and are not exhaustive; readers seeking comprehensive definitions should consult the relevant consensus statements identified in the references.

AAFP American Association of Feline Practitioners. The principal author of feline-specific consensus guidelines including retroviral testing, vaccination, and life-stage care [23, 45].

AAHA American Animal Hospital Association. Publishes guidelines covering vaccination, diabetes management, dental care, fluid therapy, and other small-animal practice topics [7, 21, 22, 38].

ACVIM American College of Veterinary Internal Medicine. The board-certifying college whose consensus statements define the staging systems for cardiac, renal, and other internal-medicine conditions [4, 5, 6, 24, 25, 30].

ACVIM Stage (cardiac) A five-tier classification system (A, B1, B2, C, D) for myxomatous mitral valve disease and analogous cardiac conditions, organised around the presence of structural disease, cardiac remodelling, and clinical heart failure [4].

Anion gap The calculated difference between measured cations (Na, K) and anions (Cl, HCO3) in serum. An elevated anion gap suggests the presence of unmeasured anions such as ketoacids, lactate, or uraemic toxins.

AVMA American Veterinary Medical Association. The principal professional body for veterinarians in the United States, publisher of the Principles of Veterinary Medical Ethics [1] and operator of the Professional Liability Insurance Trust [36].

AVMA-PLIT AVMA Professional Liability Insurance Trust. The principal source of professional liability insurance for veterinarians in the United States and a frequent commentator on documentation practice [36].

BCS Body Condition Score. A standardised assessment of body fat: WSAVA 1–9 scale (1 emaciated, 5 ideal, 9 obese) or the older 1–5 scale. The scale used should be specified in the record [37].

CHF Congestive Heart Failure. The clinical syndrome arising from impaired cardiac output and pulmonary or systemic venous congestion. Classified by ACVIM stage and managed pharmacologically with the four-drug regimen described in Module 7 and Case Study 1 [4].

Coombs test The direct antiglobulin test, used to detect antibody coating of red blood cells in immune-mediated haemolytic anaemia.

Distinguishes immune-mediated from non-immune haemolysis [5].

CRT

Capillary Refill Time. The time required for blanched mucous membrane colour to return after digital pressure. Normal in dogs and cats is less than two seconds; prolonged CRT suggests poor peripheral perfusion.

DCM Dilated Cardiomyopathy. A primary myocardial disease characterised by ventricular chamber dilation and systolic dysfunction. Doberman Pinschers and Great Danes are predisposed; the Doberman variant has a familial pattern of inheritance [44, 46–48].

DDx Differential Diagnosis. The list of alternative diagnoses considered for a given presentation, ordinarily ranked by probability with the criteria that would distinguish them.

DEA Drug Enforcement Administration. The federal agency that regulates controlled substances in the United States and imposes documentation requirements on veterinary practices that handle them [10].

DKA Diabetic Ketoacidosis. The metabolic decompensation of diabetes mellitus characterised by hyperglycaemia, ketonaemia, ketonuria, and metabolic acidosis. Managed per the AAHA diabetes guidelines [7, 49].

EPIC trial Evaluation of Pimobendan In dogs with Cardiomegaly trial. The pivotal trial that established the benefit of pimobendan in ACVIM Stage B2 myxomatous mitral valve disease [29].

FS Fractional Shortening. An echocardiographic index of ventricular systolic function. Reference >25% in most breeds; values below 20% indicate systolic dysfunction.

HPI History of Present Illness. The narrative description of the clinical problem from onset to presentation, addressed in detail in Module 2.

IMHA Immune-Mediated Haemolytic Anaemia. An autoimmune disease in which antibodies destroy circulating erythrocytes.

Mortality from pulmonary thromboembolism remains substantial despite immunosuppression [5, 6, 51].

Informed consent The legal and ethical doctrine requiring that clients receive sufficient information about a proposed intervention — including its risks, benefits, alternatives, and the consequences of declining — to make a considered decision.

Documentation of the substance of the conversation, not merely the signed form, is the operative requirement [9].

IRIS International Renal Interest Society. The international consensus body whose CKD staging system classifies patients by serum creatinine and SDMA, and substages by proteinuria and blood pressure [28, 39, 40].

LA:Ao Left Atrial to Aortic root ratio. An echocardiographic measurement used in cardiac staging; values ≥1.6 in MMVD indicate left atrial enlargement and are part of the criteria for ACVIM Stage B2 [4].

LMN Lower Motor Neuron. A neurological designation for lesions distal to the synapse with the motor neuron in the spinal cord, characterised by flaccidity, hyporeflexia, and rapid neurogenic atrophy. Contrasted with UMN. See Appendix J. LVIDDN Left Ventricular Internal Diameter at end-Diastole, Normalised to body weight. Values ≥1.7 indicate left ventricular dilation; combined with LA:Ao ≥1.6 it defines ACVIM Stage B2 [4].

MMVD Myxomatous Mitral Valve Disease. The most common acquired cardiac disease of the dog, characterised by progressive degeneration of the mitral valve apparatus. The ACVIM consensus statement [4] defines the staging and treatment framework.

NT-proBNP N-terminal pro Brain Natriuretic Peptide. A cardiac biomarker elevated in cardiac stretch and remodelling; useful for distinguishing cardiac from non-cardiac causes of dyspnoea and for staging cardiac disease.

PCV Packed Cell Volume. The percentage of blood volume occupied by erythrocytes, equivalent to haematocrit. Normal canine: 37–55%; normal feline: 30–45%.

Pertinent positive / negative Findings whose presence (positive) or absence (negative) bears on the differential diagnosis. Both deserve documentation; both inform clinical decisions.

Pimobendan A positive inotrope and vasodilator used in canine MMVD and DCM. Indicated in ACVIM Stage B2 (per the EPIC trial) and Stage C [4, 29, 34].

PRECISE Mnemonic for the seven attributes of exemplary documentation: Patient-centred, Relevant, Evidence-based, Complete, Immediate, Specific, Explicit. Developed in Module 1.

SDMA Symmetric Dimethylarginine. A renal biomarker more sensitive than serum creatinine for early chronic kidney disease; integrated into the IRIS CKD staging system [28, 39].

Signalment The summary description of patient identity: species, breed, age, sex, reproductive status, body weight, and identifying marks. The first element of every medical record.

SOAP Subjective, Objective, Assessment, Plan. The universal organisational framework of the modern veterinary record, addressed in detail in Module 3.

Spherocyte

A small, dense, round red blood cell lacking central pallor. Pathognomonic for immune-mediated red cell destruction in the dog [5].

Triage The rapid initial assessment that establishes the urgency of a clinical presentation and prioritises the order of intervention.

Particularly central to emergency documentation.

UMN Upper Motor Neuron. A neurological designation for lesions of the descending spinal-cord tracts, characterised by spasticity, hyperreflexia, and late disuse atrophy. Contrasted with LMN. See Appendix J. UPC Urine Protein-to-Creatinine ratio. The standard quantification of urinary protein loss; used in IRIS substaging of chronic kidney disease [28, 39].

USDA / APHIS United States Department of Agriculture / Animal and Plant Health Inspection Service. Federal agencies imposing documentation requirements on practices issuing health certificates, participating in accreditation programmes, or conducting USDA-regulated research.

VCPR Veterinarian-Client-Patient Relationship. The legal foundation, established by physical examination or by appropriate site visit, that authorises the veterinarian to prescribe medications and provide treatment recommendations [1].

VHS Vertebral Heart Score (or Vertebral Heart Sum). A radiographic measurement of cardiac size relative to vertebral length; values above breed-specific reference ranges (typically <10.5 vertebrae) indicate cardiomegaly.

VPC Ventricular Premature Complex. An ectopic beat originating from the ventricle, recognisable on ECG by its wide and bizarre QRS morphology. Frequency is quantified by Holter monitoring; high burden carries prognostic significance.

WSAVA World Small Animal Veterinary Association. Author of internationally adopted body-condition and muscle-condition scoring systems [37].
