Comments/observations:

• Human Factors is the most selected hazard this month and to date

• Most selected sub cat. Is Other followed by Competence levels

• Other :

- The identification of tripping hazards, obstructions and potential dropped objects due to poor housekeeping

- The reporting of defective plant, tools and equipment / maintenance requirements

- The reporting of interventions during unwanted behaviours – mainly actions on stairs, not holding hand rail or on phone etc

• Competence Levels:

- The identification of tripping hazards, obstructions and potential dropped objects due to poor housekeeping

- Individuals not wearing the correct PPE or not wearing / storing it correctly

- The reporting of interventions during unwanted behaviours – mainly actions on stairs, not holding hand rail or on phone etc

• Unsafe observations mainly around:

- Identification of PDO’s (due to poor housekeeping) & Unsecured objects

- Identification of Maintenance Requirements (includes floors / surfaces & doors as well as PDO’s due to corrosion)

- Housekeeping during and after work scopes - routing, storage and placement of equipment, tools and waste - the potential to slip, trip or fall.
