NEO MED PERFUSION SYSTEM


CristhianSerrano FCV Project

The FCV

The FCV (Fundacion cardiovascular de Colombia) is one of the most important clinics in Colombia, focused in Health services and Medical Product development. Founded in 1986 the FCV has several branches in cities like Bucaramanga, Santa Marta, Manizales and Ibague; Is the only clinic in Colombia who has developed several strategic business units, covering the health business from marketing to telemedicine. One of this units called Bioingenieria is focused on developing and manufacturing medical devices​such as electrocardiographs, vital signs monitors, etc. This project specifically will be developed with the collaboration of the Bioingeniria’s engineering team and the medical staff from the clinics due to an agreement between the FCV and the UIS (industrial university of Santander) from Bucaramanga, Colombia and will be the main project for the Design 8 project at UIS.

The Project • Should be developed in 4 Months • Should be made by an interdisciplinary team of engineers from the FCV and an Industrial design student from UIS. • The budget of the project will cover only the medical staff and the engineering team. • The result of the project will be a concept solution ready for prototype property of the FCV. • This project will be one on the peripheral systems in the new Mobile Intensive Care Unit for neonates which is still in development.

What is a Perfusion Pump? It is an electronic device used to infuse certain drugs to a very precise speed rate in a very precise amount. The way the pump works is basically the same with slight changes depending of the pump’s brand. First, the syringe is manually loaded then mounted in the pump, then the nurse inputs two data values: Speed rate and volume; with this the pump start to work , pushing the syringe and infusing the drugs according to the data input. Most of this pumps have an alarm to alert the medical staff if something is wrong with the process or simply if the process has finished.

What is a Neonate? For the projects purposes a neonate is an infant with just a few hours or days of born with some medical complication, most of the time a premature birth. Due to her/his low corporal mass, drugs should be carefully infused into his body to accomplish this a Perfusion pump is used. The neonate spend most of her/his recovering process inside of an incubator in an intensive care unit, the incubator provides the baby with the appropriate conditions to survive.

Design Process Research During this stage of the project the main concern is to recognize the state of the technology developed by the FCV as well another Perfusion solution developed worldwide by other companies, for that so, several guided visits to the intensive care unit are made, which is the main context of use of the Perfusion system. As part of this process a series of interviews must be made, with doctors and nurses who will form a fundamental part of the entire project team. The final part of the research phase will be a series of sessions with engineers to define the best components for the project.

Creative Process With the information previously gather, the next step will be to explore several topological configurations for the system, in order to find the one who adapts the best to the project needs, this stage will require a series of simple sketches focused in an overall shape rather than in technical details. The concept sketches will be discussed with doctors and engineers in order to chose a concept to work with and evolve during the project.

Prototyping Due to the project’s limited time the FCV focused the project in the developing of a concept solution, although at the end of the project this solution should count with all the blueprints needed to manufacture prototypes. Only small parts would be tested through mock-ups, but just for usability purposes.

Research

The following information is a small sample with the most important data gathered during this process. Needles to say design is not a linear process, so this information wasn’t necessary available when the project began, specially because sometimes the engineers team misinterprets the medical staff needs.

Competitors The B.Braun Solution.

B.Braun has developed a very impressive Perfusion system, with a central control and the possibility of easy transportation, as well as power independency, remote monitoring, alarm control etc. The system allows the integration of infusors and perfusors at the same time while optimizing space.

Other Solutions. Other products offer a much simpler solution with multiple syringes pushed at the same time (not very convenient), others limited to merge two pumps in one single device and other imitate the B.Brown model of stackable pumps but without all the functionality offered by brown.

Our Situation Engineers

Nurses & Doctors

• We have been developing an integrated mobile intensive care unit for neonates (IMICUN), so far it has been applied in some clinics, but the actual product need some big improvements in two main aspects quality and integration.

• The actual product its very erratic, and hard to use for us, we are used to control the Perfusion pump from the pump itself and not from a touchscreen computer with a complete new interface to us.

• We need an entire new system composed by subsystems allowing us to configure every IMICUN, been the Perfusion system an independent element with his own control and energy system. • To make this project we modified a series of Perfusion pumps in order to control the from the touchscreen computer and the FCV already have 200 of this modified pumps, so you have to create a system that suits to this.

• The actual product uses the same screen for the pumps and for the vital signs data which is quiet confusing... Specially in situations of emergency where every second makes a difference, it is life or death. • Constantly we are sending reports to the engineering team, about all the things that need improvements...but we feel ignored by them, every time the show something new to us, we can see the same old mistakes that we put in our reports.

The Context. Characteristics

• Nurses are always in surveillance paying attention to something irregular or alarms from the devices.

• Intensive care Units are very stressful places, but at the same time very quiets at times.

• Space is very important and should be optimized to ease the job and allow as many devices as possible, depending of the situation of every neonate.

• There are medical equipment everywhere, lots of cables an columns to put pumps and several other devices. • Cleanness is extremely important, everything must stay clean to avoid bacteries and viruses.

• All the devices are constantly in use by nurses and doctors and when something it’s not working properly they need to call technical assistance.

Creative Process

The creative phase of the project starts with a series of sketches with the aim of find a topological solution which integrates the different elements in one conceptual system, therefore details such us the electrical connectivity and independency wont be addressed at this point. This series of sketches representing very general ideas will be presented to the medical and engineering team, in order to receive their impressions, and detect the most appropriate configuration for the Perfusion system.

Concept Refinement After evaluate the concepts and decide which are the most convenient elements in every one, the next step is to make a 3d model who gathers this elements in a single concept. Still not every detail is solved, this pretend to be a more 3 dimensional solution, real measures of the touch screen computer and the perfusors are now taken into account. The result of this process is a vertical configuration of rotative shelves (concept 5) , with a touchscreen computer in one side to control the system. In the main module there are a couple of compartments for the battery system and storage. This system has two extra rotative drawers to add a couple of perfusors in case of emergency. This concepts along with the ten previous concept sketches will be presented to the medical staff and the engineering team, where they will give their professional opinions and feedbacks.

• Maybe to put the perfusors on those rotative drawers will unstable, and send data an electricity to every pup will be a engineering challenge and we don`t have time for that right now.

• We don’t need more storage, and if we needed it wont be a good idea to put it in the Perfusion system it represents a salubrity risk.

• All the ideas are great, and very creative, we prefer those ideas with vertical configuration,it will be the best for the project.

• Doctors and nurses can be very closed minded we have been dealing with this issue since we develop products for them, we try to not too much attention to them. • Let’s take the medical’s staff word, and make much simpler system without working areas and medical supplies storage.

• All you concepts look very nice...but non of them seem to fulfill our real needs, they’re all too complicated to use, and we need a much simpler solution.

• Same for working areas, we still don’t know anything about how the system integrates with a column or the IMICUN, so it will be unstable and can cause accidents. • Its simple we just need three perfusors and the control system, if we need an extra perfusor we will put it directly on the column, focus in ease of use and salubrity.

Redesign Process. The redesign process focuses on much simpler system, enhancing fabrication and functionality, the system is tough to be used mainly on a column, hoping to ad one to the new ICUM. This concept is presented again to the medical staff and the engineering team having the acceptance of both, this idea will be evolved in later phases of the project.

The Final Concept The final concept is more elegant solution and a natural expansion of the past concept, improving details such as the cable handling and the adjust of every perfusor. Transportation and technical support are integrated into the final concept.

Characteristics • By giving inclination to every shelf every Perfusor is locked into its position by gravity, but also this inclination gives a more dynamic aspect to the entire system, every perfusor seems to look upwards to the user. • The battery storage drawer is a space to be used only by the technical support staff, this drawer also covers the wheels at front. • The main column is also use as a conduct for the power an data cables. • The side handles are used to hold the system to mount it or dismount it . • The adaptor is the connection between system structure and the stand, it uses the same principle than the perfusors locking itself by gravity. • The system has an only power cable coming out from the battery drawer. • The top handle is used only to transportation purposes.

The system is controlled by the touchscreen computer on top, the only contact with the syringes is to install them on the pump.

The whole system is transported using the top handle and the wheels, the structure must be inclined to roll properly.

The system is designed to be used only with the modified perfusors property of the FCV, any other pump wont fix properly in the shelves.

The main structure is made of aluminium while the rest of the panels are made of laminated wood with melamine resin to ease the clean up.

Due to the system own weight the union with the adaptor is strong and stable avoiding accidents and even little unstable movements. This union was tested in a series of activities where proved to be the most firm and quick to make. On the other hand the junction between the stand and the adaptor is made by a screw who press holding firmly the stand, a very common solution in this type of stands.


Accessing the USB hub trough the window on the main column, this should be preferably by an expert technician.

Key learning Points • To work with an interdisciplinary team is a challenging task, every team has his own vision of the project and schedule, it is the designer’s responsibility to assemble all this different talents in a way that gives the best result for the users. • It was evident how during the creative part extra functions of the system where added, like medical supply storage or flat working areas. It’s important solve the main issues of the problem before thinking in expand the system • Due to his nature,Medical projects tends to be focused on functionality, but all that functionality should come with usability characteristics that allows the user to take advantage of the system. • Listening but no blindly obey, is key. In a project where every team has a very different approach, is important to take decisions based in what is best for the user and the company. Is this the kind of product we want to be associated with? Is this the best for the user? Are we able to manufacture this? Are we promising something we can’t fulfill?, Are the kind of questions that should guide the direction of a project, independently of a certain a approach in every team. • Adjust to the project’s timeline an schedule is as important as the delivery of quality concept solutions, in this case 4 months should be enough to cover the entire project, otherwise it wouldn’t be possible. • To work on a company is important for many reasons, in this project was very interesting to see how everything has his own procedure, from a request to make a visit or to make a meeting, to approve a budget or have access to medical equipment. At this point the selected concept is taken to be improved in aspects like aesthetics , functionality, costs, etc. Every concept from the previous phase can add something to improve the final concept.

The following information is a resume of the information obtained in the phases 1 and 2 of the previous chart. The state of the art was made from the information obtained in E-commerce stores like Amazon or Ebay, where users can share their experiences with the products. The last part is the result of a poll made for this project , surveying 39 people (men and women) with an average age of 29 years old.

State of the Art

Two main trends are found in the market of ironing boards, one trend approach ironing boards as a stand alone product while the other try to mimic into decorative objects or other pieces of furniture. The following are the best products in every trend, selected for obtain the highest points in a comparison chart which evaluate aspects such as aesthetics, durability, ease of use, space consumption, manufacturability, etc.

Wall Mounted Ironing Board This product can be found with some variations including some shelves for storage, a source of light, rotative movement, etc. This solutions cost an average of $300 and its main strengths are the quick use and the use of space. The main negative aspect is the mounting which requires the use of specialized tools.

Revolution 360 This Ironing Board tries to improve the overall user experience, proposing a new way to iron clothes, putting shirts around the table and rotating it, moving the table instead of the garment. The fold system as well as the iron support has been enhance to ease the ironing. This product cost an average of 100 and for that price can be considered a good purchase.


User’s needs The project requirements are for the most part taken from the needs detected in a poll to the users. The main purpose of this poll was to know how the people relate to the activity (ironing), do they love ironing?? Do the hate it?? Why?? If they could change something in the way they usually iron their clothes, what will it be? What king of new features would they like to find in their next ironing board?. It must be said, that this needs are not written in stone, they will be guidelines for the project development, but it’s not a full list of requirements, this needs will help to create the concept as a proposal for an Ironing Board.

Concept rethink

As the previous sketches shown, the initial approach tries to create most of the time a furniture pice which contains the ironing surface, the problem with the previously presented ideas is its complexity and in the near future its costs of fabrication. This ideas were trying to justify the ironing board adding more functions or even camouflaging the actual ironing board into another thing. The project should focus in a way to make the ironing board as simple as possible, providing a solution to the main user needs; but soon it become obvious that the previous user’s bad experiences will be a very important factor to the way the product is perceived, is not enough to enhanced those factors marked by users as important, it is necessary to propose a way to break the user’s bad predisposition and to generate a positive emotion will be the main strategy.

The new concept consists in a simpler wall mounted structure, this structure has a graphic artwork in the back of the ironing surface, this artwork is what the user is going to see when the ironing board is lifted. The surface is able to rotate for a better adaptation to small spaces, notice how the leg is embedded into the surface

In this pictures is shown preliminary view of every component scale, notice the increment in the wide of the ironing surface. Also, inspired in the Revolution 360 model, an accessory in the front part is added, this piece allows the under to iron shoulders more comfortably.

ConceptTest

The concept is refined for this test, the shape of the board is now more narrow at front, this will ease the ironing of pants. Also the artwork is simplified removing the motorbike shown in previous versions; notice how the cat’s snout is now a new component, used to lock the leg of the table in a certain position, this is a solution with functional an aesthetic characteristics. This concept will be presented to a small group (16 people) of users who will give their opinions about the product, the main goal of this is to find out which aspects of the product need some improvement, which are the best; but also to know the potential purchase intention on this product and a tentative price for it.

The different adjustments made to the concept lead now to the manufacturability phase, where materials and fabrication processes are defined, the following is an explosive view showing the products components and describing its materials. Due to its extension the cost analysis wont be included, at the end 150 units should be produced at a price of $50 approximately to be profitable business.

The Final Concept:Miau! The final concept gathers the entire process of this project an its final proposal, the main attempt of the product is to offer more comfortable conditions to the user, increasing stability, a wider space to iron, a better support for the iron and a quick and ease way to use, offering a decorative design that has no other intention than put a smile on the face of those who look or use the final product.

Project Accomplishments

Ease the process. The ironing table can be easily folded/un-

folded to use, also the support can store the iron with space for the clothes hangers.

Wider Surface. By having a wall mounted ironing board, the

need to have a light weight product is not such a relevant factor anymore, this allows the product to have a larger ironing surface, able to contain a shirt that can be ironed much more quickly.

Stability. With a wall mounted ironing board, the possibility of

an accident related with the movements of the board while the ironing is reduced, even to rotate the board is necessary an intentional movement by the user but also is noticeable how the iron support never moves reducing the possibility of accidentally drop the iron.


Key Learning Points • Keep it simple. It is important to enhance the main functions of a product before to try to add new ones, new functions wont make disappear the already existing issues within a product, this just will add an inconvenient complexity that increase the things to improve. • Design should address prejudice. Understanding the historical relationship between the user and the previous products of the same type is vital, sometimes functional improvements are not enough to make a product accepted, in this case a sort of reconciliation between the user and the product was needed, the product should find a way to gain an opportunity to be seen with more unprejudiced eyes, in this case humor and art were the main strategy to face this issue. • User’s opinions are important, but it is equally important that they are not in charge of the project, their opinions or suggestions will always need and interpretation from the project makers, it is not about give the users all they ask, it is about study their relationship with the product and find the real variables which affects this relationship to have real design arguments.

Curtiembres del Valle Curtiembres del valle is a company from Santander, its main activity is focused on leather tannery, being the company a family business now in its third generation. The tannery is located at The San Jose valley a small town 3 hours from Bucaramanga where the main office and company’s warehouse is located. The company focused its production in vegetable leather and all its process are eco friendly what has made the company in one of the most important tanneries of Santander. This project will be made as graduation project in a collaboration between the Industrial University of Santander and Cuertiembres del Valle ltd. The company will provide all the source materials and most important, the necessary knowledge about the material to make a right application of this.

What is Vegetable Leather? Is the leather tanned using vegetable components instead of mineral ones (chrome), the vegetable leather is characterized for being less pollutant in its fabrication process and having a great old texture on the final product. The vegetable leather uses a component called Tanin, which may be found in several barks, like chestnut, oak, quebracho among others. About the material characteristics , it behaves in a very similar way than the chrome tanned leather, but with some differences , vegetable leather may discolor if is over exposed to water or sometimes it may shrink, also the variety of colors can be limited; in the other hand vegetable leather is known for having a much better aging and flexibility than chrome tanned ones.

The following information is a resume of the information obtained during the first phase of the project, which include a survey to 14 veterinaries of Bucaramanga, a survey to 31 pet owners in order to find the most relevant factors for this project, also a study about the different kinds of products in the market analyzing its strengths and weaknesses to find new product opportunities.

Concept Development

In this phase, the creative development will be different from the other projects, it will begin with a mind map as a sort of brainstorming (this mind map wont be included, is too big) to create general ideas; later, the sketch part will be made using reference images, this technique will speed up the process in order to have more time for prototypes. After the referenced sketches, every concept will be modeled in 3d to refine its adaptation and check the overall look. This concepts will be filtered and later prototyped to start doing adaptability tests in dogs.

After analyzing every concept in a comparative matrix with the strongest market competitors, having into account the user’s needs, three collar models ( 9-1011) and two harness models ( 1-and the result of merge 4-5) are selected to keep in development and go to the next phase.

Concept tests results. The prototypes were shown to the six most important veterinaries in the city to catch their opinions and suggestions, the following is a summary of the data gathered.

In order to have the right measures for every size, several mock-ups are made for every product, this ones will be tested in several dogs with different proportions in order to find a set of sizes that adjust to many dogs as possible, this mock-ups will be the last step before define the final concept for every product. Notice how the harnesses models were the only ones to be modeled, the measures for the collar will be extracted from the results of the harnesses.

The package development will be focused in create a solution that allows the buyer see the textures in the product, see the size and know the model contained in it. The visual concept express the fashion sense in this products and the big importance of materials textures in them. In the other hand the brand will be called “Endal�, name of a very famous dog, known for his work as a service dog, Dogs Today named him dog of the millennium, and his image will represent the bound between a dog and his owner, a relationship determined by loyalty, friendship and companionship.

Project Accomplishments

Style. The final line of products, mix leather and fabrics tex-

tures to create a more versatile look, allowing the manufacturer change textures according to the fashion trend; this also allows most of the products to be suitable for a male or female pet.

Ergonomics. The entire line of products cover this aspect,

every product has his own way to take care of the pet, some more than others, giving to the owner the possibility of a traditional collar, like the tie collar, or a suspenders harness which is a more focused solution.

Manufacture. All the products were manufactured by the

local labor, optimizing processes and material, cost could be successfully reduced, having at the end a very competitive product.


Key Learning Points • Prototyping is crucial to this kind of projects, every mock-up provides useful information to the project, the time invested in prototypes will be one of success factors in the future. • Manufacturing limitations are real, to know this limitations will give the designer the ability to focus in what can be done, but is important not to sacrifice creativity to early in the process, in order to find solutions changes will be made, this is the nature of every product development. • Is important to accept that many dogs, or breeds will be unable to use this products, due to his anatomical proportions or even habits, to know this fact will allow the designer to focus in those dogs or breeds which can use the product. • This is the kind of project where a project management approach is very useful, due to the large amount of activities to make and the short time available to accomplish them.
