IMPORTANT INFORMATION

Access to the Hospice’s ser vices is usually by referral from your GP, hospital team, or another health or social care professional, however, we can also take self referrals via our Access Team.

Our admission criteria help us to make sure that the people who need our ser vices most can access the ser vices they need when they need them. Even if you are referred to us for one specific ser vice, we might of fer you something else that bet ter suits your needs.

We don’t provide long term care and many people come into in patient care for a shor t time and then their care is transferred back to their GPs and other more appropriate ser vices.

Our aim is to help you to live your life as independently as possible, for as long as you can and be where you want to be. We discharge people from our in patient unit when their symptoms have stabilised and people are ready to move on or move home We also discharge people from our communit y ser vices when they no longer need our help and suppor t.

When you are discharged, we ’ll make sure you have the help and suppor t you need. We’ll involve the people helping you plan your discharge so we can be sure you have ever y thing you need once you leave our care, whether you ’ re going home, to a care home, or to an NHS complex care unit.

We hold ourselves to the highest standards and are scrupulous in protecting you from contracting preventable infections while you are with us. If you have an infection, we ’ll care for you and tr y to minimise the risks to you and those around you, including our work force

We ask that:

→ if you or someone at home is feeling unwell, especially if you ’ ve been vomiting or had diarrhoea, please don’t visit the hospice

→ if you ’ re unsure about whether to visit or not, please telephone and speak to the nurse in charge

→ you use the alcohol gel provided, before and af ter visiting, and thoroughly wash your hands, par ticularly af ter going to the toilet

→ you please don’t touch dressings, infusions or other equipment

If you have any questions or worries please don’t hesitate to speak to a member of the team. Please also look out for fur ther information on infection control which can be found

We use an integrated electronic patient record system alongside NHS Lothian to ensure that we can provide the best possible care for you. For more information, please see the policy on our website. You can request access to your medical records under the Data Protection Act (1998) and General Data Protection Regulations (2018). Our Medical Director is responsible for patient confidentialit y and data protection and will happily talk to you about

YOUR PRIVACY

We are committed to maintaining the confidentiality of ever yone accessing our ser vices. With your consent , we will share information with other members of your health and social care team to ensure your care needs are met .

We’ll also share your information with your family and those important to you when you’re admitted to our services, if you’re happy for us to do that. If there are people in your family or in your health and social care team you don’t want us to share information

We’re fully commit ted to ensuring you are fully involved in all decisions about you care and suppor t. If you lose the capacit y to make decisions for yourself, we will make sure you are not treated any less well and will take account

→ who you are as a whole person, what ’ s impor tant to you and how you have lived your life

→ your past and present wishes about your treatment, providing you with information you can understand to help you par ticipate in decisions

→ the views of your named person, carer, guardian or welfare at torney

→ the range of options available for treatment and what will provide you with the maximum benefit

One of the things we encourage people to do is to think about what the future might hold, and begin to plan. That might be planning for treatment, making suitable living arrangements, talking to your family and children, or put ting your af fairs in order.

None of us find it par ticularly easy to talk about ourselves or someone we love becoming more poorly.

Having suppor t to think through our options, and talk about our choices at an early stage means that if something happens and you ’ re unable to tell us what you want, your family and the people caring for you have a good idea about what you ’ d want them to do.

People of ten want to plan where they will die - at home, in a care home, or with us. While we make ever y ef for t to ensure that things happen as you have planned, sometimes it is not possible. We’ll always be open and honest with you because we ’ re commit ted to suppor ting you to live your waywhatever that means for you.

People give in many ways. Some volunteer their time and exper tise and we’re proud to have nearly 700 volunteers involved in a wide range of roles. They work in almost ever y area of our work including chaplaincy, on the wards, in the shops, in governance and development, complementar y therapies, in the Iona Café, taking care of our gardens and much more. For ever y member of staf f, there are three volunteers who help make sure we deliver the best possible care and suppor t.

OUR SUPPORTERS

We deliver ever y aspect of our care outlined in this director y absolutely free of charge to all patients and their families.

We’re an independent charit y. Our independence is one of our strengths but it also means we have to raise around 75% of all the money we spend through fundraising and suppor t from our local communit y and corporate sponsors. We’re incredibly grateful.

Lots of people suppor t us through our charit y shops across Edinburgh and the Lothians. All our shops accept donations of goods.

Some people choose to leave a gif t to St Columba’s Hospice Care in their Will. These, like all the gif ts we received, are vital to the continued deliver y of our ser vices.

Other people get involved with us through fundraising

Others donate to us in memor y of a loved one, take on sponsorship challenges, organise events, and take par t in our events such as our annual ar t exhibition and Light up a Life appeal.

A huge thank you to ever yone who suppor ts us. We can only care because they do.
