Open enrollment is a short period each year when you can make changes to your benefits. This guide will outline all of the various benefits St. Bonaventure University offers, so you can identify which coverages are best for you and your family.

Elections you make during open enrollment will become effective on June 1st, 2025. If you have questions about any of the benefits mentioned in this guide, please don’t hesitate to reach out to the Human Resources Department.

In addition, the College is introducing our new Employee Benefits Advocate, Consiliarium Group.

Consiliarium provides a single point of contact for assistance understanding and navigating St. Bonaventure University’s benefit plans including medical, prescription, dental, vision, and flexible spending plans. Consiliarium will help with locating providers, ordering replacement I.D. cards, estimating out-of-pocket costs and plan coverage, resolving provider billing and insurance claims, and help answer qualifying event questions during open enrollment and year-round support.

Who Is Eligible?

All full time Faculty and Staff are able to enroll in the benefits outlined in this guide, as well as your eligible dependents.

How to Enroll St. Bonaventure University will continue to use the benefits portal for open enrollment, Colleague SelfService. You will login, review, and finalize your 2025 benefit elections. First, verify all your personal information and make any necessary changes.

Once all your information is up to date, it’s time to make your benefit elections.

When to Enroll Open enrollment begins on Tuesday April 22nd, 2025, and runs through Wednesday April 30th, 2025. The benefits you choose during open enrollment will become effective on June 1st, 2025.

How to Make Changes Unless you experience a life-changing qualifying event, you cannot make changes to your benefits until the next open enrollment period. Qualifying events include things like: • • • • • •

Marriage, divorce or legal separation Birth or adoption of a child Change in child’s dependent status Death of a spouse, child or other qualified dependent Change in residence Change in employment status or a change in coverage under another employer-sponsored plan

Dental Insurance In addition to protecting your smile, dental insurance helps pay for dental care and usually includes regular checkups, cleanings, and X-rays. Several studies suggest that oral diseases, such as periodontitis (gum disease), can affect other areas of your body, including your heart. Receiving regular dental care can protect you and your family from the high cost of dental disease and surgery.

Vision Insurance Vision insurance can help you maintain your vision as well as detect various health problems. St. Bonaventure University’s vision insurance entitles you to specific eye care benefits. Our policy covers routine eye exams and other procedures, and provides specified dollar amounts or discounts for the purchase of eyeglasses and contact lenses.

Vision benefits are offered through Guardian. If you seek the services of a provider listed in Guardian’s In-Network Provider directory, your benefits include the following:

Disability Insurance Benefits Plan

2025 Benefit Overview NYS DBL: Coverage begins after four consecutive weeks of service for fulltime eligible employees; after 25 regular work days for part-time eligible employees. This insurance will provide you with 26 work weeks off from work should you become disabled. This benefit will provide 50% of weekly earnings ($170 maximum weekly benefit).

NYS PFL- Full-time eligible employees are covered after 26 weeks of employment, part-time eligible employees are covered after 175 days of employment. This leave provides paid time off to bond with a newly born, adopted, or fostered child, to care for a family member with a serious health condition, or assist loved ones when a family member is deployed abroad on active military duty. This leave provides a 12-week maximum benefit at 67% of weekly wages, up to the New York State Average Weekly Wage. Employees are responsible for the cost of the premium.

60% of your total weekly earnings, up to $1,000 each week. Approved benefits begin as soon as 8 days from date of disability, and paid for up to 26 weeks of your covered disability. As Faculty are covered under the provisions of the Faculty Status & Welfare Handbook, no further coverage is necessary.

60% of monthly earnings, up to $7,500 each month. Approved benefits begin as soon as 180 days from the date of your covered disability.

Accident Accident Insurance provides financial protection by paying a set benefit amount based on the type of injury you experience and the treatment you receive. It covers a wide range of incidents—from everyday injuries to more serious accidents—and applies whether the accident happens off the job.

This insurance can help cover the costs associated with covered accidental injuries and their treatment. Employees may also elect coverage for their spouse and child(ren), provided they choose coverage for themselves.

Guardian offers two comprehensive plan options—a Low Plan (Value Plan) and a High Plan (Advantage Plan)—so you can choose the coverage level that best fits your needs.

How Do I Enroll?

You must enroll through our benefits portal, Colleague Self-Service, during open enrollment.

Employee Assistance Program (EAP) St. Bonaventure University provides confidential referral services to benefit eligible Faculty, Staff and their eligible dependents through Guardian.

Service provided by the program: counseling services, child/elder care resources, legal and financial consultations, concierge services among other resources.

Participants’ right to privacy is fully protected by law and the College's EAP policy. Employees may contact the EAP at 800-386-7055, or the Office of Human Resources for related literature.

Flexible Spending Accounts What Is a Health Care FSA?

Paying for health care can be stressful. That's why St. Bonaventure University offers an employer-sponsored flexible spending account (FSA). A health care FSA lets you use pre-tax dollars for certain IRS-approved medical care expenses not covered by your insurance plan. For example, cash that you now spend on deductibles, copayments or other out-of-pocket medical expenses can instead be placed in the health care FSA on a pre-tax basis.

For 2025, the FSA maximum contribution limit is $3,300.

What Are the Benefits of a Health Care FSA? There are a variety of different benefits of using a health care FSA. It saves you money. Allows you to put aside money tax-free that can be used for qualified medical expenses. It is flexible. You can use your FSA funds at any time, even if it’s the beginning of the plan year.

What Is a Dependent Care FSA?

Similar to health FSAs, dependent care FSAs allow you to contribute pre-tax dollars to qualified dependent care.

The maximum amount you may contribute each year is $5,000 (or $2,500 if married and filing separately).

Health Savings Account What Is a HSA? HSA monies can be used for qualified medical expenses not covered by health insurance. The HSA is an Employeeowned bank account in which you can deposit pre-tax money to save for future medical expenses or pay for services prior to meeting your deductible. You can only contribute to an HSA account if you elect one of St. Bonaventure’s High Deductible Health Plans. $1,000 Catch-Up is only for those 55 years and older.

What Are the Benefits of a HSA?

A Health Savings Account (HSA) is a powerful tool for managing healthcare costs and saving for the future. It offers triple tax advantages: contributions are tax-deductible, growth is tax-free, and withdrawals for qualified medical expenses are also tax-free. Telemedicine – Virtual Care What is Virtual Care?

With Virtual Care, you can access a doctor from your home, office or on the go – 24 / 7 / 365. Through Highmark’s virtual care benefit, doctors can visit with you either by phone or secure video to help treat many non-emergency medical conditions. Highmark’s virtual care provides access to 24/7 urgent care, mental health care, virtual physical care to help you overcome back, joint, or muscle pain at home, and more.

Make Preventive Care A Priority Preventive Care Services are fully covered, even with a high-deductible health plan. Take advantage of preventive care available to you and your family to ensure you identify concerns that can be treated early.

HOW CAN I ACCESS MY FSA? THE BENIVERSAL MASTERCARD The Beniversal Prepaid Mastercard can be used at qualified merchants providing medical products and services (doctors, dentists, medical labs, hospitals, medical supply stores, vision centers, certain drug stores, and retail merchants). A list of These merchants is available at our website.

The card also allows for contactless payments through Apple Pay®, Samsung Pay® and Google Pay®.

When using your card, always save your itemized receipts. With an FSA, the IRS requires BRI to verify that 100% of transactions are for eligible expenses. Since some qualified merchants also offer services/items that are not eligible, we may contact you requesting documentation to verify a transaction. Instructions will be provided in the event of a request.

LOG IN TO YOUR BRIWEB ACCOUNT BRIWEB is your secure participant login for managing your accounts with BRI. BRIWEB allows you to view balance and transaction information, submit claims, download plan documents and more. To get started, go to BenefitResource.com.

Click the Employees tab and select “BRIWEB LOGIN” Select “register an account.” You will need to assign yourself a personal login ID and password, and you will need the company code and member ID provided to you by your employer available during registration.

Once logged in, a dashboard will provide a quick snapshot of your account(s). To manage your FSA, navigate to the “Medical FSA” tab.
