Men and the prostate FROM a young age, women are conditioned to visit their general practitioner on a regular basis. Men, on the other hand, have fewer reasons to visit their doctor regularly and do not cultivate this vital relationship so readily. It is up to you to change that pattern and we can use Prostate Awareness Week to start the process. So as part of Prostate Awareness Week this September, Mr Scott Donnellan, urologist at St John of God Hospital Berwick, shines a spotlight on prostate cancer - the most common cancer in Australian men. Mr Donnellan says that 20,000 men are diagnosed each year with prostate cancer, giving Australian men a one in nine chance of developing prostate cancer in their lifetime. “The risk increases with age and is higher if a father or a brother has been diagnosed,” he said. Early screening is central to the fight against prostate cancer using the Prostate Specific Antigen (PSA) and Digital Rectal Examination (DRE) tests. Since the discovery of the PSA blood test in the early 1990s, prostate cancer can be diagnosed at an earlier stage. This earlier detection and diagnosis makes management of the condition more appropriate. “It is important to appreciate there are many approaches to managing prostate cancer. Each case is considered according to the patient’s age, extent of cancer, aggressiveness of cancer and the desires and expectations of the individual.” The recommendations of The Urological Society of Australia and New Zealand (USANZ) recommends annual PSA testing and DRE in men over 45 years. They also advocate a PSA test at age 40 years in concerned men or men with a positive family history to predict their lifetime risk. A raised PSA level does not necessarily mean that someone has cancer. Mr Donnellan states that the change over time is possibly more important

Urologist Scott Donnellan is urging men to have a prostate check up. to determine the risk and progression of cancer. “It is important to have a PSA test every year to gain the most information.” Symptoms such as waking at night to pass urine, feeling an urge to pass urine, and diminished urinary stream are usually indicative of benign growth. Symptoms related to prostate cancer are a late sign and reflective of more advanced disease. Blood seen in the urine can represent benign or malignant growth anywhere in the urinary tract and must always be investigated.

Mr Donnellan said the best action a man could take was to get himself checked out. The screening tests are simple, painless and may save your life.
