How Can I Tell If I Have Skin Cancer?

There are three main types of skin cancer: basal cell carcinoma, squamous cell carcinoma, and melanoma. Because each has many different appearances, it is important to know the early warning signs.

ook especially for change of any kind. Do not ignore a suspicious spot simply because it does not hurt. Skin cancers may be painless, but dangerous all the same. If you notice one or more of the warning signs, see a board-certified dermatologist right away, preferably one who specializes in diseases of the skin.

WHAT PATIENTS NEED TO KNOW 1. Everyone needs to perform a self-skin exam once a month.

ACTINIC KERATOSIS

An actinic keratosis (AK) is a crusty, scaly growth caused by damage from exposure to ultraviolet (UV) radiation. An AK is considered a precancer because if left alone, it could develop into a skin cancer. AKs are the product of a lifetime accumulation of UV damage and typically appear on areas most commonly exposed sun, such as the backs of the hands, forearms, face, neck, and scalp.

BASAL CELL CARCINOMA

Basal cell carcinoma (BCC) is the most frequently occurring form of all cancers, with more than 4 million cases of diagnosed in the U.S. each year. BCCs may look like open sores, red patches, pink growths, shiny bumps, or scars. Any skin lesion that doesn’t heal, or bleeds with even gentle trauma, such as drying with a towel, is concerning for a BCC.

2. Melanoma can appear anywhere on the skin, including the palms/soles, under your nails, and places...for most, have never seen the light of day (genitalia). For this reason, it is important to look everywhere. Lock the bathroom door, get out a hand mirror, and check, it could save your life.

3. What to look for: ABCDEs, and an ugly duckling A - Asymmetry: If I draw a line down the middle, do the sides match?

B - Border: Is there a nice crisp border, or does the mole melt into the surrounding skin?

C - Color: Moles should be one color, one shade of brown. Is there a variety of shades or additional colors, such as black, red, white, or blue?

D - Diameter: Any mole larger than 6mm, ie the head of a pencil eraser, has a higher risk of becoming melanoma.

SQUAMOUS CELL CARCINOMA

Squamous cell carcinoma (SCC) is an uncontrolled growth of abnormal cells arising in the epidermis, the skin’s outermost layer. SCCs often look like scaly red patches, crusty bumps, warts, or elevated growths with a central depression. SCCs may appear suddenly and some forms are tender to touch. They can become disfiguring and sometimes deadly if allowed to grow. More than 1 million cases of SCC are diagnosed each year in the U.S., which translates to about 115 cases diagnosed every hour.

Incidence has increased by 200 percent in the past three decades in the U.S., and more than 15,000 Americans die each year from this type of skin cancer.

MELANOMA

Melanoma is a very dangerous form of skin cancer. Melanoma is caused mainly by intense, occasional UV exposure (frequently leading to sunburn), especially in those who are genetically predisposed to the disease. An estimated 178,560 cases of melanoma are diagnosed in the U.S. each year and is fatal for an estimated 9,320 of these patients. Fortunately, if melanoma is recognized and treated early, it is almost always curable.

E - Evolution: Has it changed?

F - The Ugly Duckling Sign: Most people have a “signature mole,” their own personal mole appearance and architecture. Is there a mole that seems different from all the others?

4. Everyone needs a full body skin exam by a board-certified dermatologist once a year, more frequently if you have a family history of skin cancer, a personal history of skin cancer, or live in an area with substantial sunshine.

Virtual Medicine DURING THIS TRYING TIME

I just read about a medical service made available to 33 million Medicare Part B beneficiaries. The company is called, “Doctor on Demand” and it connects people with board certified physicians of their choice over secure live video from the comfort of their home, 24 hours a day, 7 days a week.

his move improves access to convenient healthcare for beneficiaries not just right here in Orange County, but across the United States, so share this news with your loved ones.

Doctor on Demand is the first of it’s kind to expand it’s offering to include Medicare Part B coverage following the CMS expansion of telehealth services in response to the COVID-19 outbreak.

Medicare beneficiaries can access medical care for 90% of the most common conditions treated by both primary care and urgent care facilities such as infections, cold and flu, rashes, and even the diagnosis and testing of COVID-19.

Ongoing care for those with chronic conditions such as asthma, COPD, diabetes, high blood pressure and thyroid issues, as well as prescription 10 August 2020

refills are also an available benefit to Medicare Part B beneficiaries. There are even mental health and psychiatric appointments available.

I quickly created an account for my husband by using his Medicare ID number, email address, birthday, phone number and creating a password for him. In less than a few minutes the options for what kind of medical care he could schedule for were easily listed showing the price for the services. ($0 in this case). I used my computer, but it can also be accessed through a smartphone or tablet by downloading the “Doctor on Demand” app from the App Store or Google play.

During this public health emergency, Doctor on Demand is waiving the copay for Medicare Part B beneficiaries (this could include the $198 annual deductible and the 20% co-insurance), for as long as the government allows. This means that even if you only have just Original Medicare and no Medicare supplement or you’re not enrolled in a Medicare Advantage plan, this service is free to you to use.

Please don’t be daunted by the fact that it’s an online service, it’s really easy. But you do need to have a camera option on your computer to access.

Since cancer patients are particularly vulnerable to infection, protecting them means that safety measures need to start before they arrive at the front door. With limited exceptions,

visitors need to wait for patients outside the building during this crucial period. Visitors are welcome to walk with patients, or bring wheelchairs, to the door, where staff greets them and begins the check-in process.

City of Hope recognizes the importance of involving family in your visits and treatments during this time.

There are several ways visitors can participate in appointments. Patients can conference them in on a video call or a phone call while they’re in the doctor’s office, or the doctor can

come outside the building before or after the appointment to speak with the patient and visitor together.

If you’re a patient, here are some other ways you’ll be protected when you come in for an appointment: As you enter the lobby, the first thing that will happen is a temperature check.

It’s quick and easy. A staff member will hold an infrared thermometer to your forehead to make sure your temperature is under 100 degrees Fahrenheit.

You’ll receive a colored wristband that lets everyone that you’ve been cleared for temperature.

You won’t be surprised to learn that you will also be given a mask. Masks are one of the best ways to shield each other from respiratory droplets, so every patient receives a medical grade mask to wear for the duration of their appointment; you can be certain that our staff also will be wearing masks at all times. If you brought your own mask, you may continue to wear it underneath the mask we provide.

Next stop: the reception desk, for the normal check-in process. You’ll find the waiting area inviting and safe, with signs posted asking that social distancing be maintained, and seating that has been appropriately spaced.

If you have a physician appointment, you’ll wait on the first floor for a medical assistant to take you back for your visit. If you have come for an infusion, an additional temperature check will be performed when you arrive at the second-floor infusion clinic. Everyone in the clinic wears a face mask and either a face shield or goggles, and

gloves are worn by every clinician providing infusion care. In addition, every infusion bay is physically distanced.

To further ensure safety, City of Hope frequently and meticulously sanitizes all areas of the facility, including but not limited to waiting rooms, hard surfaces, and high-traffic and hightouch areas. Hand sanitizing stations are located throughout the building.

Choosing Happiness

without changing the outcome of a situation that led me to feel this anger in the first place.

A dear friend gave me an inspiring book, The Happiness Advantage, by Shawn Achor, a leader in the field of Positive Psychology. In his book, he turns our widely recognized beliefs about happiness upside down. We often believe that when we work hard and find success that leads to happiness. His research shows just the opposite: happiness fuels success, not the other way around.

This led me to dig deeper into the theories and practices of positive psychology. Basically, positive psychology is the science of well-being; it’s the study of the positive aspects of the human experience. Whatsyourgrief.com, a leading website for all things grief-related, had a recent post about coping with grief through PERMA, a model created by one of the founders of the positive psychology movement, Martin Seligman.

PERMA stands for Positive emotion, Engagement, Relationships, Meaning, and Accomplishments; the five core elements he believes help us work towards a life of fulfillment, happiness and meaning.

How do we find fulfillment, happiness and meaning in a world that is so broken right now? If Achor’s theory proves true, my happiness can fuel my worldview. Achor proposes an intriguing challenge. He encourages, “Spend two minutes a day scanning the world for three new things you’re grateful for. And do that for 21 days.

The reason why that’s powerful is you’re training your brain to scan the world in a new pattern, you’re scanning for positives instead of threats.

It’s the fast way of teaching optimism.”

This brings me back to my childhood and the Magic 8 Ball that I love so much. There were 20 possible responses to my burning questions about life; 10 positive, 5 negative, and 5 indifferent. The game was interestingly designed to give me the positive answer I was hoping for

more times than not, leaving me feeling good and wanting to ask more questions.

So how do we put these positive psychology elements into practice in our own lives? Reframe your thoughts.

Challenge your fears and your anger.

Engage with your social network.

Find ways to connect instead of shutting yourself off from others.

Celebrate small accomplishments.

Recognizing even the smallest victories boosts our confidence and leverages it toward larger successes.

Accept that some things are not within your power to change. Let go of that control and focus on what you can do.

y the time my friend’s grandmother passed away at 92, she had fallen six times. Two of those falls resulted in broken bones - once, her hip. Horrifying, to find oneself in that position. The Hidden Risks and Costs of Selling a Home in Uncertain Times As the residential real estate industry is forced to re-think how it operates during the global pandemic, many homeowners are wondering how to “safely” approach the home selling process in this uncertain time.

S

enior Home Purchase Program (SHPP) offers a unique service which addresses the hidden risks and costs which homeowners now face with the traditional home selling process.

“Selling a home, even in “normal times” can be overwhelming, especially for seniors who have been in their homes for years and feel emotional about moving; and now there is the added concern of health risks during the sales process,” says Rob Perkins, SHPP co-founder.

Rob and his sister Corinne Ross had this experience with grandparents who had lived in their homes for decades and needed to move. “It took a small village of relatives to manage their home sales,” remembers Perkins.

After a decade in residential real estate investing, Perkins and Ross created the Senior Home Purchase Program (SHPP) in 2015, a niche service offering seniors a transparent and simple solution to home selling.

SHPP purchases homes directly in as-is condition, alleviating the stress, risk, and costs associated with a traditional home sale.

With SHPP, there are no agents, no home preparation, no showings, and no costs to the seller. Homes are sold “as-is” with clutter, unwanted fur-

Next, SHPP copiously reviews the local market in comparison to the home. “We then meet and present our absolute best offer based on our careful evaluation.

We are transparent in illustrating our analysis and how it differs from the traditional offer. With no costs or fees to the seller, our SHPP offer is very compelling,” he explains.

It is safe to say that I know nothing about pipes. I know a little about drywall, and some about framing; my husband has his general contractor’s license so I’ve picked up a few nuggets of knowledge over the years - albeit in conversation, not in practice.

There are a bunch of things I can now catch you up on: leaks, sprinkler lines, lead testing. But truly, all you need to know is this: In many cases, ePIPE can have your pipes restored the same day without displacing you from your home.

It isn’t always easy to trace the beginnings of a business, especially a lucrative one. Many times you have to wend back through a great many partnerships, over all the handshakes and deals made, past the initial boom to the original idea.

S

ometimes though, there’s a crystallized history. An evident path. Such were the beginnings for Marco Scognamiglio. First generation college graduate from Arizona State University with a degree in Business Communication and Management, the Dana Point native followed in his parents’ footsteps into the local restaurant business. Marco currently operates five Southern California Subway franchises - in San Clemente, Mission Viejo, Laguna Hills and two in Dana Point.

His ties to the restaurant industry are deeply rooted and familial. His father, Enzo, owns the popular Brio Tuscany Grille in Dana Point and his mother, Jill, operated several Subway franchises herself so between his education and literally growing up in the industry, Marco was well poised to embark on his own entrepreneurial endeavors with the goal to “get involved and give back” to the community in which he lives and works.

“My first job was cleaning dishes and calamari in the back of my father’s Italian restaurant in Dana Point,” says Marco.

“I saw the community he created in that space, and wanted nothing more than to build upon that legacy to create my own success and community relationships someday.”

A board member of the Dana Point Chamber of Commerce, and a member of the Laguna Hills Chamber of Com-

merce, Marco is committed to serving those around him, especially now, when times are so uncertain for many due to COVID-19.

“My goal has always been to grow my organization so I can feed and provide for my beautiful community, while also giving my team members better opportunities for their own personal and professional growth,” says Marco. “Right now, there is no greater need than enmeshing a true sense of wellbeing for my team and our customers in everything we do.”

Marco has taken many steps to pivot his business during the pandemic, including taking the opportunity to provide for local heroes, starting “Subs for Heroes” - a program that has to date donated over 200 meals to local hospitals.

“Frontline workers are doing everything they can to ensure the health and safety of all of our loved ones right now,” he says. “It is the absolute least we can do.”

serve handmade, thin, crispy, flaky crust pizza. I had “Make Your Own Pizza” with prosciutto and arugula, complete with a soft flurry of mozzarella. You know, the kind of pie you sip wine with, reflecting on the old country while a hot steam of freshness rises poetically around you. Or, as others put it: one heck of a tasty pizza.

For heartier appetites, I recommend the Filetto Toscano (USDA Premium choice filet mignon medallions pan roasted and served with a sweet red onion, shallots, Chianti reduction and melted fontina cheese, and favorites like the Rigatoni Casa Brio characterized by grilled chicken breast, caramelized onions, roasted garlic, baby spinach, cremini mushrooms, sweet baby peas and fresh basil topped with fresh buffalo mozzarella.

have to admit - I’m pretty particular when it comes to deciding where to go for dinner on date night, out for a family meal or simply stepping out to enjoy a glass of fine wine and live entertainment after work. Lucky for me, I live in Dana Point and am only a stone’s throw from a popular eatery that dishes up all this and delicious Italian cuisine to boot: Brio Tuscany Grill.

This place isn’t the kind of establishment that has huge, bedazzled lights pointing to it with acrobatic sign holders out front. This restaurant respects itself in a quaint location at 24050 Camino Del Avion in Dana Point. If you didn’t know about it, you might miss it driving by. But after you read this, I guarantee you’ll make it a point to seek it out.

For starters, the Antipasto Italiano Platter arrives with an array of flavorful favorites - Prosciutto di Parma, artisan salame, fresh buffalo mozzarella, Tuscan olives, grilled artichoke hearts, roasted bell peppers and toasted ciabatta bread. Yum! A seafood aficionado? Try the Mare Platter which is made for sharing: Buttermilk battered calamari fritti, seared Ahi tuna and blue shrimp scampi.

he world has changed over the past few months. I’m sure, if you’re like me and most of my mom friends, you’ve already had to adjust, reschedule or even cancel some of your family’s muchanticipated travel plans during these unprecedented times of shelterin-place, and safer-at-home.

Before we know it school will be back in session and our family didn’t want to let the entire summer fly by without some semblance of a “vacation.” We had our reservations about traveling 42 August 2020

a beeline for family fun at the Balboa Bay Resort in nearby Newport Beach.

The resort features newly appointed guest rooms and suites, fresh, modern, contemporary furnishings and amenity-rich luxury including a full-service spa complete with nine

treatment rooms and a premier blow-dry bar for moms who want a little extra pampering. The 24-hour fitness center and array of fun and energizing outdoor endeavors are designed to keep every member of the pack happily occupied. From the perfect pool day (think lounge chairs, daybeds, private cabanas and Pool Bar for drinks and lunch) to onproperty recreation rentals including electric Duffy boats, kayaks, paddle boards and bikes, our family was able to create our own customized experience that combined the best of both - relaxation and recreation.

Upon arrival, we stocked up on snacks and beverages at Blend Cafe and boarded a Duffy boat for a meandering self-guided tour of the picturesque waterways of Newport Harbor. Dinner at the resort’s seafood-focused waterfront restaurant A&O Kitchen+Bar showcased expertly crafted cocktails, local brews and shareable plates for even the most discriminating palate (by that I mean there is something on the menu for every member of our family, including finicky eaters).

I opted for a flavorful tuna poke, my husband went with a grilled-toperfection signature burger made from grass-fed beef, caramelized

onion and white cheddar on brioche and the kids both cleaned their plates of fish and chips before diving into a sweet treat of peanut butter s’mores for dessert.

Grab a gourmet coffee at Blend Cafe to start your day, paddle board the afternoon away, and come back home for a relaxing waterfront dinner at A&O Kitchen+Bar (casual indoor and outdoor seating, including fire pits and live weekend entertainment). The Resort is implementing safety measures, and offering a flexible cancellation policy (stays through September 30th can be cancelled up to 24 hours in advance), in accordance with CDC

guidelines, to ensure its guests are brought the highest level of service, no matter the changing landscape.

The Benefits of Diabetic Socks

he primary purpose of diabetic socks is to keep your feet dry, decrease your risk for foot injury and avoid preventing or slowing blood circulation. The difference between diabetic socks and regular cotton socks is that they are usually are made of materials that wick away moisture, are fitted, padded and are non-binding.

Because people with diabetes are at higher risk of foot injuries and infection due to damage to their circulatory and nervous

systems caused by high blood sugar levels, they would benefit from wearing diabetic socks.

Nerve damage, or neuropathy, decreases sensation and increases risk of injury, particularly on the bottom of the feet and can also cause people with diabetes to be unaware of an injury and delay treatment.

Circulatory problems also make it harder for wounds to heal.

Ralph Waldo Emerson famously said “who you are speaks so loudly I cannot hear what you’re saying.” Keeping this quote in mind has served me well over the years, particularly in choosing friends … and when seeking out the services of professionals. In my opinion, integrity isn’t a word heard often enough in the business world. Competitive, certainly.

By Donald A. Hunsberger, Esq. Decades ago we published a piece warning parents sending their kids to college for the first time to provide their children with more than what freshmen need to pack for their lives at school.

O

ur article described materials the undergrads should leave at home with their parents, as well as take away to the university: every student who is over eighteen should (MUST), leave their parents with an Advance Health Care Directive, a Durable Power of Attorney for Property and Finances, and a very strongly drafted HIPAA form.

We gave out reams of these articles to parents and students, but over time, for no particular reason, the distribution flow dried up. We stopped circulating the article, except to our own clients at the workplace whom we knew had children going off to school.

Sadly, one fine young man of our acquaintance left to study on the East Coast, well equipped for his academics, but without any of the paperwork for his parents when he got sick. He went across the country, leaving his family here in Orange County without the tools to talk to doctors, hospitals, nurses, or any other health care provider when, as it tragically came to pass, he had a heart attack away from home.

The result was too painful to fully relate. Just know that even after this wonderful fellow had passed on, his

family was not able retrieve their son’s medical records to learn what precisely had taken their cherished child.

Before he died, Mom and Dad could not discuss matters with the healthcare providers, who were simply following the law as provided by our Congress.

We do not want this to ever happen again. Today, in the midst of a global pandemic, it is more important than ever to have signed HIPAA authorization forms for each of your offspring so that their physicians may discuss their healthcare needs with you.
