DEAR COLLEAGUES,

As we reflect on the past year, we are humbled and exhilarated by the opportunities we’ve had to connect with you, listen to your concerns, advocate on your behalf and on behalf of your patients, and build our Division together.

The Board of Directors, along with a strong cohort of over 50 Family Physician leaders, Division staff leadership and expert consultants, has been visioning a Division, a community and, indeed, a province where Family Physicians are connected and empowered to influence the important decisions that are impacting the way we practice primary care, and the way our patients are experiencing it.

As our Division has grown, so has our need to codify the position of Family Physicians within it. We wanted to ensure that when we say we are, “by and for Family Physicians,” this is not a surface statement, but a principle that is at the core of how we operate both now and into the future.

To this end, we have:

Developed our 2023-2029 Board End Goals and embedded them within all Division programs, projects and initiatives;

Invested in building up Family Physician leaders through leadership training and strong engagement, opening opportunities for FP participation in designing, decision-making and leading all the work we do internally and in collaboration with the HA and others;

Invited you, our colleagues, to connect with FP leaders in your neighbourhoods, at events and beyond; and Held our first annual Physician Wellness Retreat, where we connected as colleagues and considered ways that we can, individually and collectively, create more sustainable practices.

From this foundation, and as the Surrey-North Delta Division of Family Practice grows in both scope and reputation, we are increasing our collective influence as Family Physicians and our ability to lead the way as primary care evolves in BC. All while ensuring we do not lose sight of the goal: better care for our patients and a more supported, sustainable system to work within.

I am writing this message from the privileged position of looking back on a year of unprecedented progress. As an organization, we have truly hit our stride: we have expanded our influence, taking on a larger role in primary care design and decision-making, empowering our members and our FP leaders to be true advocates for their patients, not only at the community level but also regionally and provincially; we have implemented new processes and structures that will serve our growing team and operations into the future; and most importantly, we have undergone a transformative process of empowering our board of directors to truly lead our work, so that everything we do reflects the will of our Family Physician members and will continue to do so through whatever growth and change we encounter in the future.

In one sense, what we have been doing is futureproofing. But we have also been setting ourselves up for success here and now. This success is reflected in:

Establishing and confirming our Board ends (organizational goals aligned with what our members want and need);

Aligning everything we do with the board ends, within the umbrella of Primary Care Networks (PCN) as a vision of an integrated system of primary care, and interconnected with the larger healthcare and wellness system; and

3.

2. The unprecedented collective effort to attach and reattach a total of about 60,000 people in our community.

One of my favourite things about this year’s report is the opportunity it has presented to reflect on the vision board that you – our members – created five years ago. It has been both exciting and inspiring to see how much of that vision is being realized today. Although this has been done with great intention, it is still pleasantly surprising to realize that the results of our hard work are aligned so closely with the vision you hold for primary care in Surrey-North Delta.

As you read about the work we are doing together, I hope that you feel inspired and empowered as leaders in primary care, in our community and beyond. While we have come a long way towards the future of primary care that you have envisioned, we all know we still have a long way to go.

I believe that our common purpose and collective action, guided by your vision and leadership, will carry us forward in creating a better, healthier, more connected primary care experience for Family Physicians and their patients. Thank you for being a part of this work.

THE WELLBEING INDEX: MEASURING SYSTEM HEALTH THROUGH THE LENS OF PHYSICIAN WELLNESS

Championed by physicians in our community including SND Division Co-Chair Dr. Sujatha Nilavar, the Wellbeing Index is an internationally recognized quality metric for the wellbeing of an organization developed by the Mayo Clinic and funded in our region by Fraser Health Authority.

Any physician in Surrey-North Delta can register to anonymously answer a 2-minute questionnaire. The Wellbeing Index provides some important data:

Metrics inform physicians and stakeholders on challenges to the well-being of care providers Physicians can self-monitor your personal well-being and access resources

The PES Team supports R3W initiatives through data tracking and processes; connectionconnecting physicians with peer mentors; supporting them with audits and sudden practice closures; and supporting wellness events such as the Physician Wellness Retreat. Because the PES team engages meaningfully with FPs early on in their journey with the Division, members can begin on a leadership pathway sooner than in the past. This alignment has resulted in roughly 15 new members becoming involved in working groups

The more physicians in our community sign up, the more robust and accurate the data, and the more leverage physicians have to collectively advocate for provider wellness as the foundation of a strong, sustainable health care system.

PATHWAYS AND PATHWAYS REFERRAL TRACKER

Pathways is an online specialist referral tool for physicians and their office staff to enhance and improve the process of referring patients to specialists and clinics. The aim of Pathways is to facilitate quality referrals by providing a comprehensive database of specialists and clinics across the province, allowing users to quickly sort through and filter specialist and clinic information to find a provider that best meets a patient’s needs.

Pathways consists of 4 core elements: Medical Pathways (physician-facing), Public Medical Directory, Community Services Directory, and the Referral Tracker (RT).

MOA NETWORK

Acknowledging the vital role an MOA plays in the patient journey, the aim of the MOA Network is to empower our MOAs through the provision of both MOA education and MOA wellness opportunities as they relate to proactive primary care and the careful cultivation of professional relationships that encourage positive communication and support.

We believe that supporting MOAs is a critical component for building effective and sustainable primary care networks in the Surrey and North Delta community and strive to uphold this vision.

WRAPPING UP THE REFERRAL TRACKER PROJECT: THE FINAL COUNTDOWN

At the end of June 2023, the Referral Tracker rollout in Surrey-North Delta officially ended. The project closed and the transition from the implementation phase to the operations phase began.

To mark this goal post, the Referral Tracker team in SND ran “The Final Countdown” campaign between April and June 2023. Notable achievements of this successful campaign include:

Achieving the campaign goal of 100 referrals sent in a single month, A combined total of 370 referrals sent up to March 31, 2024 66 new FPs onboarded, and 16 new SPs onboarded during the campaign.

As part of the our ongoing Referral Tracker work, we have successfully implemented Referral Tracker as the sole referral tool for SND’s PCN resources, enabling transparent, timely two-way communication between referring FPs and NPs and PCN team members.

LONG-TERM CARE INITIATIVE

SUPPORTING PHYSICIAN CONNECTIONS WITH AN FP WHATSAPP GROUP

The long-term care initiative receives funding to support over 1500 LTC beds across 13 facilities in our community. This initiative is guided by the direction of two physician committees: the LTCI All-Physicians Committee, which is open to all SND physicians currently providing long-term care in Surrey-North Delta, as well as the LTCI Steering Committee, which guides the decisions around provision of long-term care in our community.

Through the funding of the Family Practice Services Committee (FPSC), the aim of the LTCI is to design and implement local solutions that deliver dedicated FP MRP services for residents in long-term care homes.

With a team-based approach, the LTCI supports all physician members in a spirit of collegiality and collaboration.

The LTCI was selected for presentations at the 2024 JCC pre-forum and Quality Forum, highlighting the importance of the supportive, collaborative and team-based approach being taken. This approach supports the professional and personal wellness of participating FPs.

At the start of the COVID–19 pandemic, some of the challenges experienced by physicians included isolation and a general loss of connection. We addressed this challenge in part by creating neighbourhood-specific WhatsApp groups for Family Physician members.

Since their inception, the WhatsApp groups have grown to include 240 physicians across 3 geographical regions within Surrey-North Delta, enabling members to connect, share information, and provide feedback to Division staff.

In 2024, the groups were merged into a single WhatsApp “Community” to promote greater interconnectedness and provide a larger platform for members to ask for and offer support.

The success of our FP Members WhatsApp groups has shown what we can do when everyone comes together to address different challenges and potential solutions at the practice level, the neighbourhood level and the system level.

Using the WhatsApp community and other digital tools, the SND Division will continue to foster an environment where different groups come together in conversations that inspire and empower physicians to enact positive change.
