WELCOME We are proud that Radiant Imaging is strongly committed to quality patient safety, lifelong learning, and continual process improvement. Radiant Imaging was formed to bring our radiology practices together under a comprehensive, multi-specialty, quality driven structure to better serve our referring physicians, hospitals, and healthcare services partners of Southern California. As the healthcare industry undergoes fundamental changes and a movement towards transparency begins, we have decided to take a proactive approach to be at the forefront of patient safety and quality. This quality report offers a high level overview of Radiant Imaging’s 2017 performance. The report highlights a subset of our quality activities, improvement initiatives, and provides general insight into our practice. Under the Radiant umbrella, both radiology groups will continue to provide the same quality of service, patient care, and commitment that we have exemplified in the last five decades. Our goal is to be the trusted and all-inclusive diagnostic resource for patients, physicians and healthcare allies. In line with this goal, Radiant Imaging is proud to be part of Strategic Radiology, a national group of radiology practices that advance the profession of Radiology by improving patient care and quality of service. As an active member of Strategic Radiology (SR), a national consortium of radiology groups across the United States, Radiant Imaging joined the group to collaborate in an improvement model in which data and best practices are shared, clinical practice information is interchanged, and certain practice expenses are consolidated. Through our participation with Strategic Radiology, our goal is to improve value through the development of higher quality and more cost efficient medical imaging. We understand many of our healthcare partners have a choice of where to send their patients for medical imaging and exam interpretations. TECHNOLOGY

Our radiologists and staff recognize the need to keep all of our technology at the cutting edge of radiology. Radiant Imaging has 3T & 1.5T MRI machines with the latest software updates and coils, numerous 64-slice, and 160-slice multidetector CT scanners, and top of the line mammographic equipment. Our nuclear medicine department routinely uses PET/CT for improved attenuation correction and precise localization of lesions. These technological improvements are part of our daily practice because they improve images and deliver a better product to the patient, despite the fact they often require significantly more technical expertise to manage and to interpret.

The field of imaging has changed significantly over the years; a direct correlation to this change is the introduction of new technology and regulatory requirements. Radiant Imaging has been able to stay current with advancements in imaging. Radiant Imaging is working towards a paperless and fully electronic workflow, allowing for swift and seamless communication across all locations and to outside referring providers. We focus on technological enhancements that help us practice better medicine and optimize customer service.

RADIANT

ONLINE SYSTEMS We have developed our on-line presence to provide a vital electronic form of communication to our patients and referring physicians. Through websites and social media outlets, we are able to provide information to educate our viewers about different exams and increase awareness to prompt patients to undergo screenings as a preventative measure, ultimately encouraging patients to action and potentially speed time to diagnosis.

DOCTORS • PACS is a web based system to review and retrieve images and reports for not only our radiologists and staff, but for our referring physician offices anytime and anywhere. • Radiant Imaging has the capability to interface with certain EMR systems for reports and images.

PATIENTS

• Full field digital mammography which is quicker for the patient and more detailed for the radiologist, resulting in an enhanced interpretation of the image of the breast.

• Our Patient Portal allows patients to view their own reports after four business days. This useful and interactive tool also allows patients to contact Radiant Imaging staff directly in a secure, HIPAA compliant environment. • Request appointments on-line.

Image Wisely®. American College of Radiology (ACR) national initiative to reduce radiation dose in all modalities that use ionizing radiation.

Radiation Dose Optimization

Image Gently®. ACR driven initiative to take extra steps to ensure appropriate dose modifications for pediatric patients, an identified sub-set of patients that are more likely than others to be impacted by medical imaging radiation.

SAFETY There is a national emphasis on medical radiation exposure and a national move towards reducing patient radiation dose. Radiant Imaging has been an early adopter of best practices and we continue to optimize radiation dose through testing new software, providing feedback on radiation exposure, and participating in radiation optimization campaigns.

Dose Monitoring Programs®. Radiant Imaging equipment has been updated in order to minimize patient exposure during imaging examinations. Radiant Imaging has installed Dose Monitor™ through Strategic Radiology™ in order to track patient exposure by machine, exam protocol, site of service and technologist. All data will be aggregated on a national basis to identify opportunities for improvement.

We participate in: Standardized Protocols and Benchmarking with Strategic Radiology™. With our collaborative efforts in Strategic Radiology™, the radiation dose optimization sub-committee shares and reviews CT protocols from participating practices to develop standards and best practices. Focus On

Patient Satisfaction Radiant Imaging strives to set and meet high expectations in patient experience and service satisfaction and we measure our progress via a Patient Satisfaction Survey, an on-line survey administered through Strategic Radiology. Every day patients that have received care and services at our centers are asked to complete an on-line survey. The management team receives all compliments as well as any comments that require follow-up. We utilize the on-line survey tool as well as the comments to identify opportunities to improve the level of care and service provided. Benchmarking with Strategic Radiology™ enables us to identify and adapt best practices from other leading imaging facilities around the country.

Net Promoter

A key indicator of excellent patient experience is the Net Promoter Score (NP) score. This score is a widely recognized metric that reflects the likelihood of a customer to recommend a service provider to a friend or family member. Our survey questions are on a standard 5 point scale with 5 being very good and 1 being very poor. Patients who give us a score of 5 on the likelihood to recommend question are Radiant Imaging’s loyal enthusiasts or promoters. Those who give us scores of 1, 2, or 3 are detractors. The NP is calculated by simply subtracting the percent of patients who are detractors from the percent who are promoters.

PROVIDING RIGHT TEST, RIGHT PATIENT, RIGHT TIME: The Center for Medicare & Medicaid Services (CMS) wants to ensure that the right test is given to the right patient at the right time and Radiant Imaging could not agree more. As part of their Hospital Outpatient Quality Reporting Program, CMS has created several Outpatient Imaging Efficiency (OIE) measures aimed to promote high-quality, efficient care with a careful eye to reduce unnecessary exposure to contrast material and/or ionizing radiation. Monitoring Key Mammography Indicators Radiant Imaging has complied with and exceeded the requirements of the Mammography Quality Standards Act (MQSA), as regulated by the Food and Drug Administration (FDA), since the final regulations became effective in 1994. In addition to adhering to MQSA standards, Radiant Imaging voluntarily obtained accreditation as an American College of Radiology Breast Center of Excellence. The Jim and Eleanor Randall Breast Center is accredited by the National Accreditation Program for Breast Centers (NAPBC), which means they are held to the highest standards of care for patients with diseases of the breast. Furthermore, our radiologists regularly review key patient outcome metrics associated with providing the highest level of breast cancer care. Cancer Detection Rate: The cancer detection rate is comprised of the number of cancers correctly identified by the screening mammogram interpretation.

RADIOLOGIST PERFORMANCE RADIOLOGIST 360 REVIEW

Radiant Imaging has established through its quality and safety committees processes and policies to ensure that our physicians understand and emulate our values and work toward achieving our goals. In addition, the committee has instituted a 360° review program. Input is gathered from radiology colleagues, referring clinicians, technologists, other radiology staff, and physician leadership.

CONDUCTING PEER REVIEW The American College of Radiology developed a web-based peer review program called RADPEER™ whereby, during the interpretation of current images, a radiologist will review a prior set of images and score the prior interpretation on its accuracy using a standardized scale. All results are monitored by the Radiant Quality Committee.

Subspecialty Care Radiant Imaging offers an extensive range of radiological services, for the convenience of the patients and providers to ensure the delivery of the best quality of care. The variety we offer allows patients to maximize their chances of diagnosis before needing a biopsy or a surgical intervention and it also means that our physicians are using the latest tools for diagnosis, which may accelerate the delivery of care. Protocol Management Radiant Imaging takes each examination as a unique case. Our radiologists and technologist have developed protocols for all modalities we use. The protocols take in consideration patient’s age, body size, renal function, and clinical indications. The protocols have been developed through iteration, in order to reduce radiation dose, improve quality, decrease the number of images and scan time, and to minimize the need for downstream testing. Subspecialty Focus Radiant Imaging’s protocols, interpretations, education, and communications are all reflective of the subspecialty knowledge and expertise of our Radiologists. We have subspecialties in cardiac imaging, body imaging, breast imaging and intervention, emergency radiology, interventional radiology, interventional neuroradiology, molecular radiology (including nuclear medicine and PET/CT), musculoskeletal imaging, pediatric imaging, neuroradiology, ultrasonography, and vascular surgery. This level of subspecialization has allowed us to build relationships and collaborate with specialized physicians (i.e. orthopedists, cardiologists, neurosurgeons, etc) to ensure the best diagnosis and treatment for patients. Our subspecialties have allowed our peers to put their trust in our reads and look to us for guidance on radiological management of cases, which in turn results to less downstream testing or unnecessary biopsies. OUR WORK This report provides a high-level overview of Radiant Imaging’s commitment to offer the best in patient care and the best in customer service. We meet this commitment by investing in human resources with expertise in Quality and Process Improvement, providing them with the technology infrastructure to do their job, and by constantly cultivating a culture that embraces continuous quality improvement. By actively seeking external collaboration with other national leaders in the realm of quality as it relates to medical imaging and intervention, we are able to refine and accelerate our internal efforts. The results that we are achieving, when benchmarked against national “best practices�, are a source of pride in our organization. However, by its very nature, a quality improvement program is a never ending process. While we have come a long way along the quality journey, and are actually helping to lead the way nationally in collaboration with our Strategic Radiology partners, there is still much work to be done. We have audacious goals and there are many challenges ahead of us. We view these challenges as opportunities to improve our organization, and in doing so, improve the care we provide to our patients, service we provide to our referring physicians, and the value we bring to our partner institutions and community.
