Each day, amazing recoveries take place at our hospital and outpatient locations as our healthcare heroes help people fight to reclaim their lives following the unexpected. Reflecting on fiscal year 2021, we are incredibly proud of all that the hospital, outpatient, and foundation teams have accomplished together.
Thanks to support from our donors, the capital campaign that funded construction of our new hospital exceeded its $50 million goal. This substantial philanthropic investment is already impacting thousands of families as more than 2,100 patients from the Mid-Atlantic region and beyond received exceptional transdisciplinary care that fostered superior outcomes.

Collaborations and community partnerships enable us to further support holistic recoveries. Together with VCU’s C ERSE within the Department of PM&R and the Central Virginia VA Health Care System, we became one of only four centers in the nation to earn dual spinal cord injury and traumatic brain injury model systems status. This means patients and their loved ones can stay closer to home and receive the expert care they need following life-altering injuries and serious neurological conditions.
Our outpatient teams have also produced impressive results by serving more than 8,200 people, many of whom continued their progress with us after being discharged from Sheltering Arms Institute Hospital. The rehabilitation services and technology offered at our outpatient locations complement hospital services to create an unrivaled continuum of care.
These achievements are only a preview of the impact we will make in the coming years. The future holds endless possibilities for rehabilitation patients thanks to our talented team members and generous donors. We have imagined a bold vision: to reinvent rehabilitation for life beyond limits. The robust continuum of services that we offer, from hospital to outpatient to fitness and wellness, fosters this goal by providing support for thousands of individuals and their families as they heal mentally and physically from injury or illness.
Through the generosity of our community, the Sheltering Arms Foundation strengthens our continuum of care by helping align programs, services, equipment, and technology across each stage of the rehabilitation journey to enhance the quality of life for the people we serve. Several initiatives supported by the Foundation in fiscal year 2021 include:
• Purchasing advanced technology such as the EksoNR exoskeleton and electrical simulation cycle to help patients reinforce and build upon gains made in the hospital, outpatient, and exercise-based programs to cultivate a healthier community;
• Adding two VER-CLINIC telehealth carts to Sheltering Arms Institute Hospital for patients to connect with medical specialists and receive consultative services virtually to support their rehabilitation stay without having to leave for appointments;
• Implementing music therapy at Club Rec and Sheltering Arms Institute Hospital thanks to a grant from the Emily and Coleman Hunter Trust and Walter W. Regirer and Maria Teresa Regirer Foundation to help patients express their feelings through songwriting, playing instruments, singing, or swaying to the rhythm; and
• Launching a Club Rec expressive arts program in partnership with the Visual Arts of Center of Richmond through a grant from the David S. Walker, Jr. Foundation (Bank of America) for members to explore a variety of mediums such as painting, sculpture, and collage, which is proven to nurture healing and mental well-being.

As part of our mission, we give back to the community by supporting and partnering with other local nonprofits to extend our patient services. Additionally, we are fortunate to welcome several of our partners to Sheltering Arms Institute Hospital to assist in mentoring patients directly to ensure they have, or are connected with, needed resources before returning to the community.
Organizations seeking a grant from the Sheltering Arms Fund complete an application through the Community Foundation for a greater Richmond. The Community Partnerships Board Committee then determines recipients and presents its recommendations to the Sheltering Arms Corporate Board for final approval.

“The grant awards have given the Foundation for Rehabilitation Equipment and Endowment (F.R.E.E.) stable funding to continue providing our necessary community-based medical equipment reuse program to low-income adults with physical/medical challenges and disabilities. The impact of our partnership includes helping our clients acquire assistive devices such as canes, walkers, wheelchairs, shower chairs, tub benches, bedside commodes, and more that they could not obtain on their own. With our program in partnership with the Sheltering Arms Fund, our clients and their families are able to safely return home after a life-altering medical event with greater functional independence. In addition to giving hope and healing to those in need, the fund’s support plays an instrumental role in our ongoing efforts to advance health equity and build healthier populations across hundreds of communities. Vicki was one of the first patients to utilize the Institute’s newly acquired and first-in-the-U.S. Motek RYSEN, a multi-directional, 3D bodyweight support system that simulates real-life walking conditions.
Vicki Day woke up one morning feeling limp on her left side and began having difficulty speaking. Her husband Steve was concerned and took her to an ER near their home in the Outer Banks where they determined Vicki had an ischemic stroke. The entire left side of her body was paralyzed.
After Vicki transferred to Chesapeake Memorial Hospital and was stable, the medical staff recommended intensive inpatient rehabilitation as the next step. “I told the staff that I wanted to take Vicki to the very best place because the first 90 days are the most important. Every person said, ‘Sheltering Arms Institute is where you want to go,’” recalls Steve.
Vicki had a stomach full of butterflies when she first arrived at the Institute on a Friday evening. She began to question if she had the strength and energy that she needed to fully recover. Vicki said, “My goal was to be able to go back to my home, which is three stories off the ground, and get into it with minimal assistance.”
As part of Vicki’s therapy, her team utilized the Motek RYSEN, an all-directional, 3D bodyweight support system that provides adjustable vertical and horizontal forces to simulate real-life walking conditions and everyday activities. “The technology gives you the jumpstart that you need,” Vicki said. “Being hooked into a harness adds a level of security and takes off some of your weight, so you feel successful faster.”
Upper extremity therapy was also added to Vicki’s schedule, which involves intensive motor retraining through electrical stimulation and weight-bearing exercises.
The Day family was so impressed by the care that Vicki received at Sheltering Arms Institute that they chose to stay in Richmond an additional month for intensive outpatient therapy at Sheltering Arms Hanover Center, which helped prepare the couple to return home.

Fifty-five days after her stroke, Vicki accomplished her goal. With only a light grasp on the railing, she climbed the three flights of stairs leading to her front door on her own.
Every gift, ranging from $5 to $2.5 million, has made exceptional outcomes possible at Sheltering Arms Institute.

Since 1889, Sheltering Arms, its board members, volunteers, team members, and donors have responded to various community healthcare needs. Rooted in that history and determined to shape the future of rehabilitation for generations to come, the Sheltering Arms Board made the bold decision to enter into a joint venture with VCU Health System in 2016 to create a state-of-the-science rehabilitation hospital.
Emboldened by the Sheltering Arms Foundation Board’s commitment of $25 million to make the dream a reality, the Campaign for Convergence, a $50 million philanthropic effort that represented one half of the entire project cost, was launched.
The community was excited by the Pillars of Possibility—clinical excellence, integrated research, teaching excellence, and community—and what the vision would mean to the Richmond region and beyond. Once again, they rallied for Sheltering Arms patients. By the end of 2021, more than 300 donors, including individuals, families, corporations, foundations, and community organizations, had more than matched Sheltering Arms Foundation’s $25 million, closing the Campaign for Convergence with a philanthropic investment of more than $51 million.

When first responders arrived to the scene of a horrible car accident, they thought Jordan Smalls was dead. His tire had popped, the vehicle flipped, and he was ejected. Somehow, Jordan suddenly began to breathe and was rushed to VCU Medical Center.
Upon arriving at Sheltering Arms Institute for traumatic brain injury rehabilitation, Jordan could not walk and had limited function, particularly in his right arm and leg. “It’s really hard to have hope when you’re hurting from a situation you’re
not used to,” Jordan said about the challenges he faced after the accident.
His care team got to work utilizing the Institute’s wide range of advanced technology, including the Armeo Spring in occupational therapy and the EksoNR exoskeleton in locomotor specialty therapy. “After about four or five sessions in the Ekso, Jordan progressed to needing only about 40 percent of the help of the robot to advance the right leg,” said Zach Crump, PT, DPT.
THERAPY FOR EVERY PATIENT
When Jordan’s care team learned that he enjoyed playing basketball, they incorporated the sport into his recreation therapy. His therapist took him outside to enjoy the weather and shoot some hoops.

As the weeks went on, Jordan continued regaining more and more movement and his spirits began to lift. “It’s been the best feeling here. This hospital is the best hospital I’ve ever been to,” Jordan said about his time at the Institute.
Jordan followed up with outpatient therapy at Sheltering Arms Hanover Center and has progressed from using a wheelchair to walking on his own with a cane. He continues to work on his balance and put in extra hours at home. “Don’t ever get in your head that you want to give up. Keep on fighting because every day, you’re going to get better,” said Jordan.
Jordan is still working on regaining full movement. He has faith that he will achieve his goals with the help of Sheltering Arms.

In the spring of 2021, Marilyn West began having difficulties with her right foot due to a series of complications related to diabetes and COVID-19. She ended up in St. Mary’s Hospital with sepsis and was preparing for a partial amputation. “Because of the skills of my healthcare team and the grace of God, I only lost one toe and that’s what led me to Sheltering Arms Institute,” Marilyn said.
When Marilyn first arrived at the Institute, she was unable to use her right foot, relied heavily on her left leg, and had difficulty getting around. “I was in really bad shape,” she recalls. “I just thought, ‘Gosh, I’m never going to get better. If I’m going to be like this all my life, what is it going to be?’”
To work on Marilyn’s mobility and independence, Brandon Smith, PT, DPT, said, “Technique-wise, we constantly worked on standing tolerance in the parallel bars and how to transfer while keeping weight off of her affected side.”
After learning about Marilyn’s love for classical and jazz music as well as tennis, her team played the genres to make her feel more comfortable and pass
the time during her challenging therapy sessions. They also went outside and incorporated a racquet into therapy. Reflecting on these special touches Marilyn said, “That was so positive and had such a great impact on me.”
During her stay, Marilyn’s husband was also a patient at Sheltering Arms Institute. The team planned a brunch in the café for the couple over the Easter holiday. Marilyn describes other “game changers,” including a custom shoe, incorporating family into discharge planning, and a special stationery delivery. “It’s those kinds of amenities that I think really help to distinguish Sheltering Arms Institute from other facilities,” Marilyn said. “You feel like you’re in a home away from home.”
Today, Marilyn is back on her feet and has resumed nearly all of her daily activities, describing herself as “back to wholeness.”

In fall of 2021, the Virginia Consortium for Spinal Cord Injury Care, a partnership among Virginia Commonwealth University’s (VCU) Center for Rehabilitation Science and Engineering (CERSE), the Central Virginia VA Health Care System (CVHCS), and Sheltering Arms Institute earned federal designation as one of only 14 Spinal Cord Injury (SCI) Model Systems Centers in the U.S.
This prestigious designation recognizes the Consortium’s outstanding integrated system of healthcare from hospital treatment to community-based services as well as its innovative interdisciplinary research program to improve the lives of Virginians who have sustained a SCI.
The award includes a $2.2 million grant over five years funded by the National Institute on Disability, Independent Living, and Rehabilitation Research.
We are proud to be one of only four centers in the nation to earn dual spinal cord injury (SCI) and traumatic brain injury (TBI) model systems status.
We are humbled to receive this award. It represents many years of hard work by our collaborative research team and the outstanding clinical care delivered by the Central Virginia VA Health Care System and Sheltering Arms Institute. The co-principal investigators on the grant are Ashraf S. Gorgey, MPT, PhD, FACSM, FACRM, VCU CERSE professor and director of SCI research at the CVHCS, and Zina Trost, PhD, associate professor and CERSE integrative rehabilitation research scientist. The medical director of the grant is Timothy Lavis, MD, chief of spinal cord injuries and disorders and senior SCI specialist for Sheltering Arms Institute.
The centerpiece of the Consortium’s research is an innovative and bold study to improve arm and hand function in individuals with tetraplegia. The research study proposes to use noninvasive electrical stimulation to improve both arm and hand muscle mass and nerve function, a novel approach that has not
previously been tried. The second proposed study urgently tackles the difficult topic of identifying specific sources of health inequities following SCI. This study aims to identify practical and implementable solutions that can improve rehabilitation access and health outcomes for people from rural areas as well as racial and ethnic minority backgrounds.
“As a community partner and former Sheltering Arms patient, I appreciate being actively involved in building this research program,” said Richard Bagby, executive director of the United Spinal Association of Virginia and chair of the Virginia SCI Model Systems Advisory Board.
“This research is innovative and can potentially make a real difference in people’s lives.”
Just hearing the name rehab, I thought it was going to be miserable, but I came here, and it brought my spirits up knowing how fun they are and how much they’re willing to work with me and help me achieve my goals.

Seventeen-year-old Roanoke native Jordan Gillespie was enjoying summer vacation with his family when he noticed a bump under his arm. His doctor determined that the bump was from a spider bite and sent him home with antibiotics. About a week later, he began experiencing back pain that progressively became more severe. “I had to get carried out of my house on a stretcher because I was paralyzed at that point,” Jordan recalls.
Further tests revealed that the spider bite — from a brown recluse— caused a Methicillin-resistant Staphylococcus aureus (MRSA) infection in Jordan’s blood stream that attacked his spinal cord. He underwent spinal surgery at VCU Medical Center and was diagnosed with a T3 spinal cord injury, meaning he had no feeling from the ribs down. “The doctor told me I had a 20 percent chance to ever walk again. At that point, I put my head down,” Jordan said remembering the moment when he received the devastating news.
The Gillespie family decided on Sheltering Arms Institute for Jordan’s next step after conducting research on the best facilities, programs, and technologies for spinal cord injury recovery.
During his first few days at the Institute, Jordan began to stand, take steps, and practice home care tasks such as transferring, showering, and getting dressed on his own. His therapy team utilized myriad state-of-the-art technologies throughout his five-week stay, including the EksoNR robotic exoskeleton as well as Hocoma’s Andago and Lokomat. “I used to run a lot, and they actually put a soccer ball in front of my feet, and I ended up kicking it,” Jordan said with a smile about his customized therapy.
In just a few weeks, Jordan progressed significantly. Upon discharge, he continued outpatient therapy at home and returned to school. He’s gearing up for his senior year of high school and hopes his story inspires others who may be going through a difficult time: “Just keep smiling and keep going. Keep your head high.”
Katherine Lynch was suffering from debilitating pain due to scoliosis. Instead of waiting for an appointment to see her doctor, Katherine was able to begin treatment at Sheltering Arms Hanover in a matter of days. “My therapist got back with me right away when I called. I was very happy to begin treatment and education for dealing with scoliosis and improving my posture,” Katherine recalls.
Historically, patients were required to first visit their physician and obtain a referral before seeking physical therapy services. Occupational and speech therapy services were not limited in this way, yet patients also typically pursued referrals first. However, a change in Virginia law allows patients to see a physical therapist directly. In August 2021, Sheltering Arms launched direct access allowing patients to seek services from all of our therapists without a physician’s referral first.
Making direct access a viable option for the community was a group effort that involved nearly every department within Sheltering Arms and required new workflows for many teams. Christina Withers, PT, DPT, PCS, manager of the
Sheltering Arms Hanover Outpatient Center, was tasked with leading an interdepartmental work group to make direct access a reality.
This access to therapy proved even more critical during the COVID pandemic era as more individuals put their health concerns on hold.
The key thing about direct access is that you can get to therapy, get your impairments addressed, and recover faster.

Furthermore, physician offices have been overwhelmed with people infected with COVID-19, resulting in delays in other needed healthcare services since March 2020.
“Under these circumstances, our ability to offer access to physical rehabilitation services without the need to first see a physician has helped reduce delays in accessing needed
healthcare services,” said Dianne Jewell, PT, DPT, PhD, FAPTA, president and CEO of Sheltering Arms.
Barbara Bishop, MS, ANP-BC, MSCF, CNRN, a valued referral partner, says having skilled, educated, and certified therapists evaluate patients and begin intervention is invaluable for both the patient and the healthcare system when community demands far exceed available resources. “Direct access empowers the patient to take control and responsibility over their own health, promoting wellness in the patient and the community,” said Barbara.

New Pool Component Improves Outcomes for Patients with Neurological Conditions
Exercise Physiologist Hope Adams began noticing many patients with chronic pain utilizing the therapeutic pool at Sheltering Arms’ Bon Air Center and thought to herself, “This could really benefit our population with neurological conditions.”
She embarked on a research journey to explore the benefits of aquatic exercise combined with land-based therapy for individuals with neurological conditions. “When people with neurological conditions are in the pool, their tone and spasticity decrease so they’re able to get more muscle firing, which translates to walking,” explains Hope.
Hope and one of her clients began working together in the pool and on land and saw dramatic results and thus, the NeuroFit Aquatics program was born. From there, she investigated additional equipment needed to scale the program and how it would fit into the continuum of care. “We were able to acquire and repair equipment through donor funds to make the program more accessible for our clients,” Hope said.
Eight patients have enrolled in the program so far, including Shawn Smith, who suffered an ischemic stroke that led her to hospital rehabilitation at Sheltering Arms Institute. While planning her hospital discharge, her care team told her about the Sheltering Arms NeuroFit Aquatics program.

NeuroFit Aquatics also benefits people with spinal cord injuries, traumatic brain injuries, and cerebral palsy. The water allows Hope to perform spinal traction on patients for full decompression resulting in decreased pain, improved posture, and lengthened muscles.
In Shawn’s case, her balance and core stability have increased and she has greater flexion and extension of her knee. She no longer needs a wheelchair and is walking with a cane with little to no assistance.
Currently, NeuroFit Aquatic appointments are available each Monday. We plan to increase the number of appointments to support more access to this sought-after program.
Patients who participate in NeuroFit Aquatics coupled with land-based exercises have increased gait length and speed, endurance, and the ability to perform more strength-based exercises.
