What a great event! It was such a pleasure to see our members and their staff, from across the district, come together to attend the Miami Winter Meeting (MWM). The speakers were simply fantastic! We had a blast with the Mariachis during our lunch break, awards during the cocktail hour, and even dancing at the President’s Reception.

Our profession is no doubt becoming more diverse. Let’s look around. Here in South Florida, we are a real representation of what diversity means. Our patient base also reflects a similar level of diversity.

Dental History Trivia... Paul Benjamin, D.M.D. Grover Cleveland, the only presA ident to serve two non-conpresidential secutive terms (1885-89/ 1893-97; in between cover-up terms, he won the popular vote but lost the Electoral helped by a College), was seeing ominous dark clouds as he cheek retractor began his second term in 1893. The economy was in a nosedive toward what was to be called the “panic of ‘93.” Unemployment was high, banks were closing, and the 53rd Congress could not figure out what to do to correct the economic downward spiral since going off the gold standard with the Sherman Silver Act of 1890. The nation’s confidence, not to mention the world’s, was shaken and into this void President Cleveland planned to exert his influence by bold action starting with getting Congress to repeal the Sherman Act. But, before he could get started solving the nation’s problems, he needed to do something about a “rough spot in his palate” right where he liked to chew his favorite cigars. It just would not go away and was driving him crazy. On June 18, his physician, Major Robert O’Reilly, biopsied it and diagnosed an “epithelioma.” His personal friend and surgeon, Dr. Joseph Bryant was summoned for a second opinion. Bryant evaluated Cleveland and found a “quarter sized, ovoid, ulcerated lesion on the left hard palate.” A second biopsy 1 confirmed it was a verrucous carcinoma. Surgery was the only option, so an immediate date of July 1 was chosen for the operation. The President and his handlers felt if citizens knew about his condition, it could worsen the already brittle economy and could possibly plunge the country toward bankruptcy. The President had two questions that needed to be solved. The first - how to hide the procedure from the press, and second, how to complete the procedure and get back in the public eye quickly. To complicate matters, the President had scheduled a special session of Congress on August 7 to urge the repeal of the Sherman Act, which he felt would be the cornerstone of his second term. Although not the 24/7 news coverage of today, 1893 was the era of yellow journalism and President Cleveland was determined not to be grist for their meal. Their solution to the first question was to have the operation performed on his friend, Commodore Benedict’s, steam yacht Oneida, and then recoup at his summer home, Grey Gables, on Buzzard’s Bay, Massachusetts. No one would suspect anything by the President going out on his friend’s yacht and then taking a little rest and relaxation before the Aug 7 session. So,

a makeshift operating table with all the necessary instruments was placed in the Oneida’s salon. Late at night on May 31, the surgical team assembled on board, headed by Dr. Bryant, with Drs. O’Reilly, Keen, Erdmann, Janeway and Ferdinand Hasbrouck, a New York dentist who was skilled in the use of 2 nitrous oxide. On July 1 , with the Oneida cruising the East River at half speed, Dr. Hasbrouck administered the nitrous oxide and extracted both left maxillary premolars. Cocaine was then injected into Cleveland’s palate and supplemental ether inhalation was used as Dr. Bryant began the initial “partial maxillectomy, including the left upper alveolus from the first bicuspid to just behind the last molar tooth, the hard palate to the midline, and a small portion of the soft palate.” Once the full extent of the lesion was seen, the “remaining left maxilla, exclusive of medial wall and infraorbital plate, was removed in piecemeal fashion, and the cavity was packed with iodoform gauze. Homeostasis had been achieved with pressure, hot 3,4 water, and at one point, galvanocautery.” In Dr. Keen’s 1917 account of the operation, he said the success of keeping this operation secret was the use of a cheek retractor (something we take for granted today) that he discovered in Paris in 1866, allowing the entire procedure to be done intra-orally. One and one half hours later, with the loss of only 6 ounces of blood, the operation was successfully completed. The President stayed on the yacht for four days, then he went to Grey Gables just as planned for a little rest, and to tackle the second question of getting back in the public arena within a month. In order to accomplish this, a secret visit was required by Dr. Kasson Gibson, a New York prosthodontist, who fabricated the first of two vulcanized prostheses to cover the large defect in Cleveland’s mouth. When the President received his second obturator later in October, he wrote this to Dr. Gibson: “My dear Doctor, I hasten to announce that you have scored another dental victory and a greater one than has before attended your manipulation of my corpus. The new plate came last night...I have worn it all day with the utmost ease and comfort...my wife says that my voice and articulation are now much better than they have been for a number of days.” Because of the great successes of the physicians and dentists involved, the President went to Congress as planned on August 7, and Cleveland’s speech was clear, and none were the wiser. Soon afterwards, a rumor started (some believed by none other than Hasbrouck, the dentist on board during the surgery – but give him continued on pg 7

Florida Dental Association January House of Delegates The Florida Dental Association held its semi-annual House of Delegates (HOD) January 20 and 21 at the Tampa Airport Marriott. This was Dr. Cesar Sabates’ first official HOD session as President of the FDA. Dr. Sabates delivered a powerful message of camaraderie and brotherhood in his address to the delegates. Among the things he talked about was the importance of being proactive in helping to guide the issues pertaining to the dental profession. He included among those, discussions on dental schools, public relations, access to care, membership issues, fluoridation programs, and better communication with organizations outside of the tripartite. During the two day session, the house approved resolutions regarding: - Access to Care White Paper - Hygienist delivery of interim restorative service - Mandating dental exams for children prior to kindergarten - 2012 Legislative priorities - Dentist Supervision of anesthesia administration - Dentists in health access settings - Dental Records retention -The FNDC Schedule for 2012 - Online CE and CE tracking for members only - Audit Committee member terms and responsibilities - Membership Installment payment policy

The Florida Delegation to the ADA represents the FDA at meetings of the American Dental Association House of Delegates. The delegation comprises the ADA’s 17th district. It consists of 21 delegates and 21 alternates and includes many officers of the association, representatives from component dental associations, and at-large members elected by the Florida Dental Association House of Delegates. a break- this was before HIPPA) that something devious had occurred on the Oneida. To squash the rumors, the Secretary of War, Daniel Lammont, came forward and said he was aboard the yacht (which he was) and – you guessed it - he used the old dental excuse, – Cleveland “needed two infected teeth extracted… the President never enjoyed having a dentist work over him. In consequence, he had allowed his dental work to fall so into arrears, that he finally felt compelled to go on the yacht where he could be cool and com-

fortable, and let the dentist make a thorough job of it. This has been done.” The excuse worked, and not until Keen’s 1917 account of the surgery nine years after the President’s death (due to complications from obstruction caused by an intestinal carcinoma) did the public finally learn the truth of what happened on the pleasure cruise along the East River. Oh, and by the way, Cleveland’s deception paid off, because the Sherman Act was ultimately repealed.

SFDDA Elects Dr. Michael D. Eggnatz as FDA Secretary He will become President of the Florida Dental Association in 2017.

At the Annual Business Meeting, which took place this year at the Miami Winter Meeting on January 27, 2012 at Jungle Island, Dr. Michael D. Eggnatz was elected to represent the South Florida District Dental Association (SFDDA) as the Secretary of the Florida Dental Association (FDA). Dr. Eggnatz will join the respected ranks of past FDA presidents from the South Florida District, including Dr. Clifford Marks, Dr. Edmund Parnes, Dr. Alan Friedel, and our current president, Dr. Cesar Sabates, whose term as President will end this coming June at the House of Delegates in Orlando, Florida. The Annual Business Meeting at the Miami Winter Meeting

The SFDDA Annual Business Meeting has been held in many diverse and interesting venues over the years, including most recently, the Alexander Hotel. This year, however, our executive council came up with a new and different idea - to incorporate the meeting into the Miami Winter Meeting on January 27 at Jungle Island.

Over one hundred members of the South Florida District Dental Association gathered at twelve o’clock noon to attend the thirty minute meeting conducted by our president, Dr. Irene Marron. Before the meeting was brought to order, Dr. Beatriz Terry and Dr. Jorge Centurion addressed the membership regarding Dr. Sam

Low’s candidacy for President-Elect of the ADA, and then introduced Dr. Low, who spoke to the members briefly on upcoming issues pertaining to the dental profession. The members also voted and approved our slate of incoming officers consisting of Dr. Jeannette Peña Hall as President Elect, Dr. Marcos Diaz as Vice President, Dr. Elaine DeRoode as Secretary, Dr. Rodrigo Romano as Treasurer, and Dr. Kellie Paxton as Young Member. Dr. Michael Eggnatz was also voted into office as FDA Trustee, along with Dr. Beatriz Terry as FDA Alternate Trustee. Dr. Eggnatz was also voted as Secretary of the Florida Dental Association, and thus taking the place of Dr. Cesar Sabates as our SFDDA designated line officer upon the completion of his term as Immediate Past President at the June, 2013 FDA House of Delegates. We extend our heartfelt congratulations to all of our newly installed officers.

Dr. Brian Gurinsky, a Fortress Dentist. I live in Denver, Colorado. God’s Country. I skied here once in college and that was it, I wanted to live near the mountains. Living here is so conducive to being active and staying fit. I enjoy skiing, basketball, tennis, running and triathlons to help me decompress and stay on top of it all. The Birth of the Alan E. Friedel Service and Leadership Award by Dr. Richard Mufson and Dr. Michael D. Eggnatz How does an organization like ours, or a group of friends and colleagues within its rank and file, begin to adequately honor and thank a fellow member who has, for the better part of his professional life, devoted a seemingly endless amount of time, energy, outstanding leadership and service to the profession nearest and dearest to his and our hearts? How can one possibly communicate a sufficient level of gratitude and praise to someone who has been an exemplary leader as president of his local South Broward affiliate, our South Florida District and Florida Dental Associations, who has served as member and/or chairman of numerous committees and councils at our state and national level (FDA Association Affairs, Alternate Council on Dental Health, Financial Advisory Committee, the Committee to Study FDA Governance, Integration of Affiliates, Grass Roots Representative Washington, DC, Council on Dental Care, Council on Membership, Council on Ethics Bylaws and Judicial Affairs (CEBJA), ADA Reference Committees on Budget and Business Matters, on Workforce, Fifth District Reporting Chairman on Budget and Business Matters, Committee to Study the Office of ADA Treasurer, ADA Council on Dental Benefit Programs, Sub-Committee on the Code on Dental Procedures and Nomenclature, Code Revision Committee, Committee on Elder Care, ADA BOT Electronic Health Record (EHR) Work Group, Chairman of Vocabulary Advisory to EHR Work Group, Chairman of the ADA BOT Committee to Study the Delta Dental Settlement Agreement), who has also chaired national campaigns for two Florida dentists (Robert Ferris for ADA Second Vice President and Don Cadle for ADA President-Elect), and has sat on a committee for a third dentist from Georgia (Howard T. Jones for ADA President-Elect), and who is currently serving as Speaker of the House of the FDA House of Delegates? 16

Giving that person a special award might be the first thing that comes to mind. Great idea. But wait a minute. What if he has already received any and all possible awards the organization has to give to honor his dedication and achievements? How about the “Dentist of the Year” award?” It’s already been given and received. How about arguably the most prestigious of awards bestowed upon the deserving few by our parent organization, the Florida Dental Association - the “Leon Schwartz Lifetime Achievement Award?” This one has also already been given and received. So the question still remains. How can one possibly begin to top this level of honor and recognition? At some point during the past year or two, it became apparent to Dr. Alan E. Friedel that he had – at least for now - reached a given point in his journey through so many levels and positions within organized dentistry, that he felt he had given all of himself he has to give. As current Speaker of the FDA House of Delegates, Alan has let it be known that the upcoming June session of the House at the Gaylord Palms Resort and Convention Center in Orlando, coinciding with the conclusion of his term as Speaker, would also mark the end of his final position or office he would hold within organized dentistry. He has also stated that he would not disappear entirely but rather, would remain in a more limited role in some form, on the sidelines if you will, as an available source of information, guidance, and as a mentor, if asked, and as needed. As much as others who know Alan would not like to think his ascension to yet other heights of office within organized dentistry would be put “on hold” for now, and not abandoned for good, it has nevertheless created a pause for thought in searching for yet an ever more unique and special way – if one were to exist – to honor

South Florida District Dental Association’s 2012 Outstanding Member Dentist of the Year By Cesar Sabates, D.D.S. Dr. Paul Benjamin received the SFDDA Outstanding Member Dentist Award at the Miami Winter Meeting During the Awards Ceremony held on Friday evening. Dr. Paul L. Benjamin is an individual that is most deserving of this award. His integrity, charisma and genuine interest in the advancement of the dental profession has earned him the respect of his fellow colleagues. He has been a voice for the underserved in our community and has upheld the mission of the South Florida District Dental Association with solemn dignity with his dedication of countless hours to philanthropy as a member of Community Smiles for over thirty years. Dr. Benjamin is a Fellow of the American College of Dentists and the Pierre Fauchard Academy. He is also a Fellow and Master of the Academy of General Dentistry. He has been an active member of the American Dental Association, Florida Dental Association (FDA), South Florida District Dental Association (SFDDA), Miami Dade Dental Society, Community Smiles and several study clubs. He is one of six general dentists board members of the Practice Based Research Networks.

Dr. Benjamin has served as a trustee to the FDA, Program Chairman and General Chairman of our Miami Winter Meeting, President of the Greater Miami Dental Society, President of the University of Florida College of Dentistry Alumni Association and President of the Academy One Hundred. He has been a Clinical Assistant Professor in the University of Florida College of Dentistry in Hialeah Center since 1997. He has also been a Voluntary Instructor of Surgery in the University of Miami College of Medicine, Department of Oral Maxillofacial Surgery, and has lectured in general dentistry to the Dade County Dental Research Clinic, South Florida District Dental Association, Greater Miami Dental Society, Florida Dental Assocation and for a number of other dental study clubs. For many years, he taught the SFDDA Radiology Certification Course for Dental Assistants.

2012 Miami Winter Meeting Highlights! Spectacular, Spectacular! That is all that can be said about the 2012 Miami Winter Meeting. What a wonderful meeting it was. From the welcome Mimosa to the President’s Reception and everything in between, the South Florida District Dental Association delivered in true South Florida style.

The afternoon lunch hour was punctuated by the lively arrival of the “Fiesta Grande,” a full ensemble of traditionally dressed mariachis. The troubadours entered the expo, trumpets blaring, announcing their arrival, and then breaking into song as they walked toward the center of the hall. Attendees were soon shouting requests and singing along as the musicians serenaded the crowd.
