Freeing kidney patients to live their lives More Seven Oaks kidney patients are living active lives as a result of a portable form of dialysis patients can perform on their own at home. The patients are able to continue working, attend school and travel out of the country instead of spending three or more days in a hospital dialysis each week. Healthy kidneys process waste products from the body, but when the kidneys shut down those waste products build up in the bloodstream and need to be eliminated artificially. The conventional treatment is HemoDialysis, in which patients are hooked up to a dialysis machine for several hours, three or more times per week to clean their blood. With Peritoneal Dialysis (PD) doctors surgically insert a catheter into the patient’s abdomen and leave a small stem protruding. Fluids are placed into the patient’s abdominal

cavity to soak up waste products and draw off fluids several times a day. PD is considered a best practice option for patients who still have some kidney function because it allows the patient to maintain their own kidney function for longer and to maintain their lifestyle and mobility. Michael Lefebvre, a 50 year old accountant at Bristol Aerospace says that the option to use PD has allowed him to carry on with his life. The kidney disease he inherited from his mother was getting progressively worse and he was referred to Seven

Oaks for dialysis. In September he started on a method of PD known as the twin bag process in which patients exchange the fluids in their abdomen 4 times per day and which takes about 45 minutes. For him, that meant exchanging the fluids in the morning before work, at lunch, at supper and before bedtime. Because he lives close to his work he was able to come home and replace the fluids while he was eating, instead of having to find a place to do it at work. Now he, too, is on a cycler at night time.

“…patients are able to continue working, attend school and travel out of the country…” Lefebvre says that if his disease progresses he hopes to have the option of Home Hemo-Dialysis rather than in centre dialysis, also because of the freedom to carry on with work and other interests. He’s still able to play golf in the summer and hockey twice a week in the winter with old friends. “We don’t have any refs but we still compete. It’s something I don’t want to give up.” Nephrologist Dr. Sean Armstrong says that over the last two years, the kidney health team Seven Oaks General Hospital has been able to double the number of PD patients from 43 to 84 by performing bedside catheter insertions. He and his team trained at University of British Columbia to learn the bedside technique which is less invasive, doesn’t require an Operating Room or full anesthesia, and allows the dialysis to begin sooner. PD can often be a bridge for younger patients whose kidneys are failing and are waiting for a transplant, Armstrong says. Ryan Hobbs, 22, was a bit shaken in June when he found out he had end stage renal disease. He had been losing his vision and went to a

hospital where he found that his blood pressure was through the roof and that he had likely had kidney disease for several years. He says a form of auto-immune response caused his own body to attack his kidneys. “Just a luck–of-the-draw type thing,” he said.

Swimming is something that he misses because it doesn’t go well with having a catheter and needing to keep the insertion point free of bacteria. “It’s kind of a bummer, but better than being in the hospital for sure.”

Dr. Armstrong presented him with options but 3 days per week in a hospital didn’t seem like much of a choice so Ryan had the PD catheter inserted on August 15. Shortly after he applied to go to University of Manitoba where has been taking three courses and will probably pursue a business major.

Another dialysis patient in her 70’s, Dena Katz, has been on PD for over three years now and is “quite happy with doing it. Maybe I’m just used to it now, but I’m quite comfortable.”

I wasn’t sure before but I thought (now) it’s time to commit to something.” Meanwhile he’s waiting for the transplant which will add about 25 years to his lifespan.

“I started with Hemo-Dialysis but the reason we switched is we were going away in the winter so we asked the doctor if I was a candidate and the answer was yes.” Dr. Armstrong is pleased to see some of his patients having a chance to live a bit, because for many in-site dialysis is the only option currently available. “People who are on (in centre) dialysis say it’s a job (they go to) 3 or 4 days per week.”

“It’s pretty big but definitely bitter sweet. It’s kind of freaky to find out there’s a set number of years you can live (with a transplant) and to be thinking about that number of years at my age, but it also makes me want to live every one of them.” PD does present some limitations. Three days a week he needs to leave for school by 7:30 so that means he has to be in bed early to get 9 hours of overnight dialysis. When he’s with friends in the evening it’s a bit awkward to have to leave to go home to bed at 9:30.

Seven Oaks signs research agreements with Chinese hospitals Innovation in a relatively small community hospital in Winnipeg has led to an international research partnership with two large hospitals in the People’s Republic of China. Seven Oaks Hospital’s Chief Medical Director Dr. Ricardo Lobato de Faria and his Emergency team have been experimenting with the use of Radio Frequency Identification Tags to track and study equipment and staff movements in Emergency. They hope to identify opportunities for efficiency as well as patient safety. Their project caught the interest of someone at a conference from Fudan University in Shanghai which has affiliations with two Chinese Hospitals, Anhui Provincial Hospital in Hefei and Shandong Provincial Hospital in Jinan. Those hospitals invited Dr. Lobato de Faria and Informatrics/Process Improvement Lead Trevor Strome to visit China. According to Dr. Lobato de Faria they found two hospitals that are almost unimaginably large by Canadian standards, with 2800 beds and 5000 beds as compared to Seven Oaks’ 300. Relative to population, however, the two hospitals have similar roles and challenges to Seven Oaks. Anhui Province is located in eastern China south of Beijing and across the basins of the Yangtze River and the

Huai River with a population of over 40 million people. Shandong Province touches Anhui to the north and is on the east coast and closer to Beijing. It has a population over 95 million people and is known as the birthplace of Confucius. That visit last spring was reciprocated with a visit from each of the Chinese Hospitals to Seven Oaks this fall. Both Anhui and Shandong have signed agreements for cooperation as Sister Hospitals with Seven Oaks. The agreements outline four broad areas for cooperation as sister hospitals including: • Co-operation of joint conferences, seminars and workshops

He notes that Shandong has been involved in funding and fostering the development of hospitals in Tanzania. The next step for collaboration between Seven Oaks and its two sisters will be an exchange of individual physicians to visit and work alongside each other. The exchanges will likely involve kidney health and emergency services which are strengths at Seven Oaks.

Anhui for instance is developing a new Emergency Department and quite interested in process improvements and the use of technology at Seven Oaks. Dr. Lobato de Faria attributes the interest in research partnerships to a general tend in China to “open up to the world and be more involved.”

eager to support innovation at Seven Oaks Seven Oaks General Hospital Foundation has taken its support of the hospital to a new level with another hugely successful gala dinner that sold out and raised over $100,000. The event at Fort Garry Hotel attracted 400 prominent Winnipeg supporters of Seven Oaks including business owners, executives, philanthropists and physicians. The funds from this year’s gala dinner will go to the Access to Care campaign to build an innovative medical clinic, a bright beautiful atrium on the front of the hospital along with a pharmacy, gift shop and new food services for the comfort of patients and visitors. One Sweet Affair has become an annual event that includes a competition between Winnipeg’s finest pastry chefs. The dessert creations are judged by celebrities and then auctioned off to support Seven Oaks Hospital. The fun and food is just part of the story of growing interest in a serious cause according to Seven Oaks General Foundation Executive Director Sunny Chohan.

“We’re gratified to see so many business and community leaders from all over Winnipeg and even the province of Manitoba come together to support the work of the hospital. The word is out that interesting and innovative approaches to healthcare are happening at Seven Oaks, and the community is eager to get on board,” she said. For instance, this year’s event featured an opportunity to showcase the innovation in kidney health at Seven Oaks, which has become a hub for kidney health in Manitoba, as one of the largest centres for dialysis, but also preventive approaches and research. Nephrologist Dr. Paul Komenda, Seven Oaks’ new Research Director outlined ambitious plans to further the research and innovation agenda at the hospital. He highlighted efforts to test process improvements, to connect patients with exercise and education approaches to manage chronic disease and to deliver high quality care in all programs. One Sweet Affair Gala is already scheduled for next year on Thursday, October 18th, 2012. The dinner is part of an overall philanthropy program at Seven Oaks. For instance the Foundation recently supported the purchase of new furniture for patients and visitors through its ongoing fundraising including a grateful patients program. Access to Care Update Construction on the Access to Care project at Seven Oaks Hospital is well underway and the trusses for the roof of the new atrium are scheduled to be in place this month. Work to renovate the interior of the hospital for a new clinic, pharmacy and food services area is also well underway and on pace for completion in the spring. Fundraising for the $1.2 million community commitment portion of the $7.6 million development is picking up momentum with large donations by the Radiology Group of Winnipeg, the Nephrology group at Seven Oaks Hospital and now a $50,000 contribution from the Winnipeg Foundation. healthy lifestyle change Wellness Institute at Seven Oaks General Hospital is launching a new program to make it easier for people with health challenges to find support for healthy living.

What makes a Top Employer? Seven Oaks Hospital has once again been recognized as a Top Employer in Manitoba.

FREEway to Wellness is, as the name suggests, a free program that provides the basics on how to get physically active, eat well and make positive lifestyle changes no matter where you’re starting point is.

It’s the fourth time Seven Oaks has been named for the honour by Maclean’s Magazine. Seven Oaks was named a Top 100 employer three times previously and has also received a national award for Best Employer of 50 plus Canadians.

The program, offered monthly, will guide participants towards support for their individual goals, whether that’s other education programs at the Wellness Institute, an individual program at home, or other education and activity supports elsewhere in the community.

Seven Oaks supports health, wellness and work life balance for healthcare professionals – a deliberate strategy that contributes to higher quality care for patients. That support includes exercise, weight loss and smoking cessation programs and fast access to physiotherapy and chiropractic care for injuries on or off the job.

Executive Director Casie Nishi says Wellness Institute (WI) is offering the FREEway to Wellness program because many people living with a chronic condition, or just needing to make changes to their health don’t know what their options are and aren’t sure where to start or where to go. “We are a medical fitness facility and we want to make sure we reach people with challenges like diabetes or high blood pressure who need some guidance.” The first sessions are scheduled for January, February and March. (See Program Guide page 6). WI offers other free programs such as diabetes education and a six-week Get Better Together program for anyone living with a chronic disease. That program is available at more than a dozen locations in Winnipeg.

Jordan Forbes, Director of Human Resources says that “If we can help heal an employee’s injury, we can reduce the amount of time they have to be away from work.” Some of the unique supports for staff are less tangibly about health and wellness, such as an employee council, bursaries for higher education and management training, and a peer-led reward and recognition program, while others target health and work life balance directly, such as healthy food options and an on-site farmers’ market. “Prospective employees see that we’re fostering innovation and change and welcoming their ideas and involvement, not just filling positions,” Forbes said.

“Purchasing a membership to our gym isn’t the only way to get help,” Nishi said. “The gym programs are something we do well and are proud of, but not everyone in the world needs to come to 1075 Leila to be healthy.”
