Live Your Best Life

“Eight Habits that could add up to 24 years to your life.” That headline drew me into a CNN article by Sandee LaMotte recently. I smiled when I read further into the article, “What are these magical healthy habits? Nothing you haven’t heard before.”

They include exercise, eating a healthy diet, reducing stress, sleeping well, and fostering positive social relationships. And then the obvious – don’t smoke, don’t drink too much, and don’t become addicted to opioids.

I smiled again. This sounds like life at Crossroads Senior Living in Delta. Particularly in our Memory Care, (but also for our Assisted Living and Independent residents),Crossroads focuses on daily activities that help our residents stay as nimble as possible. Regularly, we go through stretch exercises, a fun game called noodle ball, gardening, cooking, and painting. It may not be a five-mile run or walk, but we do all we can to keep our residents safely on their feet and moving daily.

Crossroads serves three complete and nutritious meals every day, 7 days a week, plus snacks. We provide housekeeping and spa treatments. Just knowing your meals are prepared and your home is clean, is a huge stress reducer.

Our comfortable apartments foster good sleep and peace of mind knowing there is always someone nearby who can help with any personal activities.

One of the most important things Crossroads and any of the area Assisted Living facilities offer is fostering positive social relationships. It is a common mistake to think mom or dad would rather be at home watching TV. Mom and dad will live longer, healthier lives in the company of people just like them. A senior living facility like Crossroads encourages a social life that often lacks at home. A social life that maintains our loved one’s self-assurance and confidence and a gusto for living.

Crossroads, and CNN, cannot guarantee longer life by making these habits yours. We can, however, promise a healthier, happier life. That is why we encourage you to think seriously about assisted living, particularly if you need memory care. We think it’s the best path to peace of mind living. ~

CARE MANAGEMENT

A care management agency is familiar with various fields of human services – social work, psychology, nursing, gerontology – and trained to assess, plan, coordinate, monitor and provide services for the elderly and their families. Advocacy for older adults is a primary function of the care manager. Agencies are typically either government based human services agencies or are members of national care management associations

Why Would I Consider Trust For My Estate Planning In 2023?

In 2023, and after living through the pandemic, everyone needs an estate plan, it doesn’t matter what your socio-economic circumstances are.

A simple estate plan will typically contain at least three documents. This will generally include a durable medical power of attorney and a durable financial power of attorney. The other document typically found is a way to pass your property upon death such as a will.

However, there are times when a trust might be an option in your estate planning instead of or in addition to a will. So why should you consider a trust? Trusts are useful estate planning tools that can accomplish a variety of goals. They can help avoid probate, minimize taxes, and be used to give property to minor or disabled loved ones. Trusts can be created during a person’s lifetime (Living Trusts) or at the person’s death (Testamentary Trusts). Some different types of Trusts from both categories are discussed below.

Living Trust

A person can transfer their assets to a Living Trust and, as trustee, continue using their assets as they always have.

Tax Planning Trusts

Several different types of Living Trusts provide flexible alternatives for minimizing capital gains and es-

tate taxes, including the Charitable Remainder Trust, Irrevocable Life Insurance Trust, Qualified Personal Residence Trust, Grantor Retained Annuity Trust, and Grantor Retained Unitrust.

Testamentary Trusts

A person can create a Trust under a Will, called a Testamentary Trust, which does not take effect until they are deceased.

Disability Trusts (also known as Special Needs Trusts)

A Disability Trust is a type of Living Trust that allows a disabled person under the age of 65 to use her own assets for her special needs, other than food and shelter, and keep public benefits, such as Medicaid and Supplemental Security Income (SSI).

3 reasons you may need a trust as part of your estate plan

n Are you concerned about protecting a gift from creditors or litigation?

n Does a loved one need nursing home care or Medicaid eligibility?

n Do you want to provide for children, grandchildren or charitable organizations?

The types of assets you own is another important consideration. Here are three typical reasons you might need a trust.

COMMUNITY RESOURCES AND SERVICES E•E COMMUNITY RESOURCES AND SERVICES COLORADO’S WESTERN SLOPE – SUMMER 2023-2024 26

1A

loved one cannot be trusted with a large gift or has special needs

If you have concerns that a child does not have the financial skills to manage a gift or the loved one receives government benefits.

2 You want to transfer complex assets in a thoughtful manner

Trusts can be effective for keeping a vacation home or a closely-held business in the family. For large charitable donations, a trust allows you to leave a vision for how you would like the gift used.

3 Limiting the potential for relationship-damaging fights is important

When you have worked hard and been successful, a trust may be able to limit conflict and the legal fees associated with litigation. ~

Will Do

Many of us procrastinate when it comes to writing a will.

Many of us procrastinate when it comes to writing a will. Perhaps we don’t want to be reminded of our mortality. It may be a fear of the anticipated expense or a loss of control over property. But having a will extinguishes those fears. Want a few other reasons? A will contains your selection of who will receive what, when and how upon your death. You decide who will take care of your minor children. You facilitate the probate process (making it less expensive and quicker). You can disinherit individuals, make gifts or charitable donations and provide for your pets. You decide who administers your

estate to see that everything goes as you have directed. If no will exists, the state will determine how your property is divided.

This can be easily avoided with proper planning. By creating your will now, you can always add to the provisions or alter the document as your life evolves. It’s advisable to review your current will every five years to be sure that it’s up to date and still reflective of your future wishes.~

Medicare 2023 “In A Nutshell” (Parts A, B & Medigap Plans)

Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.”

Medicare Part A Hospital Insurance

Generally no cost (If you paid Medicare taxes while working)

$1,600 deductible (per benefit period)

It is possible to pay the $1600 deductible and other daily charges for multiple stays & extended care.

Plan covers: Inpatient hospital care, home health care services, hospice, inpatient skilled nursing & more.

Providers are paid by Medicare assignment.

No coverage outside USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Medicare Part B Medical Insurance

Most people pay monthly $164.90 (Premium could be higher based on income)

$226 annual deductible. (this can increase each year)

80/20 after deductible.

Plan covers: health care providers, ambulance, emergency care, outpatient surgical, wellness & other Medicare approved benefits

Most doctors accept Medicare.

No coverage outside the USA. No out-of-pocket maximum.

Many people add a Medigap plan, Part D or an Advantage Plan (Part C) to fill the “gaps” in the coverage.

Supplemental Policy Plans A - N Medigap

Premium varies by company, age, location & other factors.

Guarantee Issue at initial enrollment (Other special enrollment periods may be avalable)

These plans generally pay the 20% not covered by Part B & other Medicare eligible benefits

When this coordinates with Medicare, many benefits are 100% covered

Can provide benefits outside USA

Plans are available through insurance companies.

Important to add Part D plan to cover prescription drugs & avoid penalties.

Must have Part A & B to enroll.

There is NO additional cost to use the services of a full time insurance professional. Please call us or a broker in your local area for additional assistance.

PALLIATIVE CARE

Specialized medical care for people living with a serious illness. Care is focused on relief from the symptoms and stress of a serious illness. The goal is to improve quality of life for both the patient and the family. Rehabilitation Facilities can only be accessed following a hospital stay as an inpatient in a general hospital setting which has lasted for a certain number of days. Refer to our Skilled Nursing grid on pages 130-131 for an overview of skilled nursing centers and the in-patient/out-patient services they provide.

REHABILITATION HOSPITALS

Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions.  They can be found within a hospital or in a free-standing building.  Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

service is provided for five consecutive days. Care may be provided in a long-term care facility depending on the condition of the individual. Hospice is responsible for financial arrangements with the Long-term care facility, at no cost to the patient or the family.

GIP – General In-Patient Care –

Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period.

CC-Continuous Care – Provided to prevent the individual from going to the hospital when a crisis occurs. Continuous care also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family unable to cope. Continuous care is typically reserved for end of life or other crises that requires a constant presence by a nurse or care team.

HOSPICE CARE DEFINITIONS 98 COLORADO’S WESTERN SLOPE - SUMMER 2023-2024

24/7 ADMISSIONS – Can a patient be admitted to the particular hospice agency 24/7? If a patient qualifies for hospice care, they should be admitted as early as the family requests. Many agencies admit patients to hospice, 24/7, evenings weekends, and holidays.

GRIEF SUPPORT GROUPS – Grief is the normal and universal reaction humans have to the loss of those they love. When your loved one passes, you may go through a period of bereavement and experience grief. Each hospice agency will have a grief and bereavement support program. Many provide grief groups that meet and address issues of grief in a supportive group setting.

INDIGENT PATIENTS – Does the hospice accept non funded or indigent patients? This is usually on a case by case basis and at the determination of the hospice company. Many have a non-profit organization that can help support the care of non-covered patients. Many Hospice agencies utilize social workers to help find resources for indigent families.

as providing an alternative to those who can’t take medication or prefer not to, massage therapy has become essential in hospice care. This is not reimbursed by Medicare; it is an added benefit that select hospice companies have decided to provide for their patients.

LICENSED MASSAGE THERAPY

Massage has become one of the fastest growing trends in therapy for hospice patients. Used as a method for relaxation to alleviate stress, anxiety and pain with a light hands-on approach, as well

MUSIC THERAPY

– Music

Therapy is an established healthcare profession that uses music to address physical, emotional, cognitive, and social needs of individuals. A music therapist is not just a music performer, but actually trained in counseling and music. A music therapist creates goals and objectives for their patients and uses music activities and music experiences (listening, composition, improvisation, or playing/singing a familiar song) in order to achieve those goals. This is not reimbursed by Medicare; it is an added benefit that select hospice companies have decided to provide to their patients.

PAIN PUMPS AVAILABLE – Many patients have their symptoms controlled through oral medications. Some situations require a pain pump where medications are administered through an IV pump. If this is the situation, inquire with the hospice RN case manager to see if pain pumps are available through their insurance.

PCP-OVERSIGHT – Can the hospice patient retain their own primary care

physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria.

PET THERAPY – There’s nothing like a visit from a four-legged creature to brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. For some patients, contact with these gentle animals evokes memories of pets in their lives, past or present. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare, it is an added benefit that select hospice companies have decided to provide for their patients.

VA CONTRACT – If the patient is not a Medicare recipient and is covered by the VA, the VA will make payments directly to their contracted agencies. If a patient is a Veteran, but is a Medicare Recipient, they may choose any Medicare approved hospice agency for their care. ~

Ways to Pay for Assisted Living

Making a move to assisted living can come with many questions especially regarding finances. Pricing can vary by community depending on factors such as amenities and levels of care. Knowing your loved ones’ monthly income and available assets is valuable information to know beforehand.

Many assisted living communities require private pay. Private pay means funds come from “private” sources like checking or savings accounts, retirement accounts, stocks, pensions, social security income or from selling a home.

Other options to pay for assisted living housing and care include longterm care insurance, veteran’s aid, or life insurance.

Long-term care insurance can assist in paying for a percentage of the monthly fees depending upon the policy coverage. This type of insurance pays a pre-determined amount per day if certain care requirements are met. Care requirements could include needing help bathing, assistance with medications, or a dementia diagnosis. The policy may also have a waiting period before the plan will reimburse, which can vary from 30 days to a few months.

Veteran’s aid, also known as Aid and Attendance benefit, is available for qualified veterans and surviving spouses. The veteran must meet medical and financial eligibility qualifications. This benefit is for

individuals who need help in performing daily activities like bathing, dressing and feeding. Details on these criteria can be best explained by an expert at the local VA office from a Veterans Benefit Advisor.

Life insurance is a potential option if your policy allows you to cash out the value to pay for long-term care. Some insurance companies will do a buy-back on the policy at a percentage of its value. Or it may be possible to sell your policy to a third-party company as a “life settlement.” According to Forbes, the seller typically gets more than the cash surrender value of the policy, but less than the amount of the death benefit. It is always best to consult with a financial advisor before considering these options.

Some assisted living communities accept the Medicaid waiver and typically have a limited number of apartments to lease under that payor source. Not every community accepts the Medicaid waiver. Medicaid benefits vary from state to state and are a combination of state and federal funds. Specific functional and financial criteria must be met for individuals to be approved for Medicaid and it involves an application process. ~

PUBLIC HOUSING (PH) – Affordable apartments for low-income families, the elderly and persons with disabilities.

REHABILITATION AND THERAPY –

Many skilled nursing facilities offer restorative and rehabilitative physical and occupational therapy, and speech-language pathology services. Occupational therapy services in this setting are generally reimbursed by Medicare Part A for short-term, acute care clients; Medicare Part B for long-term residents who require skilled services and have medically necessary need for rehab; and often by private insurance, Medicaid, or private pay for younger clients.

The Short on Rehab

What is Post-Acute Rehabilitation? It is an inpatient center that provides therapies and/or Skilled Nursing.

Adults go there to recover from illness, injury, surgery, or other medical conditions following treatment in an Acute Care Hospital. Post-Acute Rehabilitation is a term used interchangeably with Short Term Rehab, Transitional Care Unit and Skilled Nursing Rehab Facility (SNF). These facilities offer many similar services, such as: Physical Therapy, Occupational Therapy, Speech Therapy and Skilled Nursing Services. All centers have a Physician directing care and all must follow the same rules and regulations governed by Medicare. Not all Post-Acute Centers accept Medicaid and/or private insurance.

An individual’s Medicare benefits begin after a qualifying Acute Hospital stay if skilled services (Therapy and/or Nursing) are needed in order to regain physical health to go home safely. The first 20 days of PostAcute Rehab is paid for by Medicare without a co-payment. Starting on day 21 there is a co-payment, and, depending on the individual’s circumstance, the co-payment is that person’s responsibility whether it be paid by the individual themselves, by a secondary insurance, or by Medicaid. It is important to remember that not all Post-Acute Rehabilitations Centers are licensed to take Medicaid. Keeping this in mind, one would want to consider whether aftercare will require only a few days

or, in some cases, a month or more in Rehab. (In extreme circumstances, Medicare stays can be as long as 100 days if the individual continues to qualify for skilled services.)

Questions to ask:

n Tour the facilities and take a family member and/or trusted friend with you. Look at the room, dining area, community rooms, and Therapy Facilities.

n What insurances does the facility accept, and will the facility bill your private insurance?

n Does the facility accept Medicaid? Very important if you may need extended Rehab.

n Does the facility offer Physical, Occupational, and Speech Therapy? Do they have specialized equipment or services?

n Ask about dining services and whether the facility is able to meet your individualized preferences. Ask to view a menu and you may even want to arrange to eat a meal.

n What is the facilities Medicare quality (CMS) star rating?

n Can the facility provide 1st medication dosing on site?

n Do they bill secondary insurance?

n What does the facility do to provide an individual plan of care that meets your discharge goals? ~
