Centura Center for Rehab

REHAbiLiTATiON HOSPiTALS Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day.

Speech Therapy – Refers to services to assess and treat speech and swallowing disorders. Speech therapy can treat speech, language, cognitive skills or swallowing.

Physical Therapy – Refers to services for the treatment and the rehabilitation process where a physical therapist will often stretch, strengthen, facilitate muscles, challenge balance, test coordination abilities, teach home exercise programs and enhance basic mobility skills.

Occupational Therapy – Refers to services to assess and treat patients to improve their ability to perform tasks in their daily living and working environments. Consider Dental Plans for Savings Everyone Wants To Sell You Dental Insurance; But A Dental Plan Is Better Dental care is the largest out-ofpocket medical expense for many retirees. To save a diseased molar can mean a root canal ($900) and a crown ($1100); totaling $2,000. Only 30% of seniors have any type of dental coverage. But is it worth it to buy dental coverage? Probably not; instead consider a good discount dental plan. A single senior (age 65 or older) cost for a discount dental plan can be as low as $8.95 per month ($107.40 per year). Members of a discount dental plan are given special reduced fees from a network of excellent dentists who will perform all dental procedures

at 20-60% savings. And you do get a tax deduction for your dental care costs. Dental insurance has a low annual limit on benefits – typically $1200. AARP’s basic plan costs $478 per year, which gives you a maximum of $1000 for dental work. Or they have a plan with a $1350 maximum for $664 per year. This is typical of most stand alone dental insurance coverage. Though you usually have twice a year cleanings and exams that do not count against the maximum, are they really “free” when you are paying those types of premiums? Continued on next page

Communication Strategies One of the toughest things to overfor Hearing Loss come when dealing with somebody with hearing loss is the difficulty of communication. Conversations can be frustrating.

Many couples find themselves depressed and isolated. Conversations that were smooth-flowing and easy, become short, curt and just enough to get by. A painful and lonely void can start to grow in these relationships. Don’t give up. Strategies exist to enhance communication with others and prevent this frustration. First and foremost, find out what levels your hearing is at. When people with hearing loss “hear” a word that is totally different then what was said, they are at a disadvantage right from the start. They sometimes fail at understanding the big picture, by missing small parts needed to put the puzzle together. This is also true among people who have normal hearing, yet have trouble with listening and concentrating in specific noisy environments. It is very important to take corrective measures for even minimal hearing losses that may be present. A diagnostic hearing evaluation should be performed by an Audiologist who can provide a hearing solution that is individualized. Next, find your environment baseline. Take a seat in the environment that you are trying to improve, and just listen to the sounds around you. Take note of any distracting sounds

around – like sounds coming from the hallway, or the fan/lights, or the outside noise coming in from the windows. These noises can be really distracting if the person sitting nearby has any listening issues. Make sure ambient noise is minimized, or even removed from the room. When speaking one-on-one with someone, make sure they are looking at your face and that they can repeat back what you have told them. Also, before speaking to the person, try to get their attention by calling their name first, so their subconscious brain can help alert them as well. Walk up to them or have them come to you before giving out important information. Try not to speak from another room. After getting their attention, begin talking. Also, have the person repeat back what you told them to ensure understanding. Written instructions are particularly beneficial. Aural rehabilitation is another therapy that includes speech and lip reading skills to enhance conversation. Finally, don’t give up! Medicare 2016 “In A Nutshell” (Parts A, B & Medigap Plans) Original Medicare is a government health insurance program primarily for people age 65 and older. It has four main parts: A, B, C & D. The chart below is an example of how Parts A & B work together with a traditional Medicare supplement also known as a “Medigap Plan.” Medicare Part A Hospital Insurance

Medicaid Planning for Finding a Pathway to Approval Long Term Care Webster defines planning as a map, or a way of doing in pursuit of a desired outcome. That’s why we make plans when it comes to things we value. From the more weighty matters of our education, career, taxes and retirement to the more light topics of vacation, workouts or even yard sales, we invest time and energy to build a pathway to what we want. As we continue to age and look beyond our retirement years we are confronted with yet another challenge and the need for a plan. Many of us have loved ones or will ourselves eventually need long term care (LTC). Whether in need of nursing home, assisted living or in home care, the cost of these services far exceeds what most seniors can carry on their own. For those who cannot afford or qualify for private LTC insurance, Medicaid remains the most common source of funding such services. However, Medicaid approval comes with eligibility requirements that most applicants lack clear understanding of and are not prepared to meet. That is where the value of Medicaid planning begins. Medicaid planning starts with a financial assessment that provides clarity through accurate information which enables better decisions with greater confidence. Planning identifies potential problems with eligibility and works to find solutions

that best match the client needs and priorities. Ultimately, Medicaid planning takes an individual from a point of ineligibility to a position of eligibility before the application is filed. The result is a smoother application process with less stress and worry. Medicaid planning can also identify legitimate means of preserving income and assets. To the surprise of many, Medicaid guidelines enable planning methods that protect income and assets for a spouse, disabled family members or even the applicant themselves. Such preservation can provide a means of supplementing future needs. Remember, proper planning works within the guidelines and never calls one to compromise his/her integrity. So if you or someone you love is in current need of LTC, or you simply want to be better informed of the possibilities in Medicaid planning, seek out the guidance of a qualified professional and begin building your planned “Pathway to Approval”. Overcoming Your Fear PROBATE – AHH!!! Just the of Probate mention of the word sends chills down many American’s backs. But, is it really that bad?

We in Colorado are fortunate that our laws allow us to choose how to proceed into probate (which is the process of winding up the affairs of a decedent by paying creditors and distributing assets and property). The two choices are informal probate or formal probate. Informal probate means that the estate is not court-supervised. Administration of the estate begins by filing a number of probate forms with the District Court, having the Court admit the estate into probate and appointing a personal representative (formally known as an “executor” or “executrix”) to begin the process of paying bills, taxes, selling property and/or distributing the decedent’s assets pursuant to the decedent’s Will or by the intestacy statutes if the person died without a Will. An informal probate can be wrapped up after six months, but most estate matters take up to a year or more. Formal probate, sometimes referred to as supervised administration, occurs rarely, but is sometimes necessary when a dispute exists between those people interested in the decedent’s estate. In this case, the Court conducts hearings periodi-

cally regarding the distribution of the estate and other matters that arise during the administration process. A formal probate can last years and is often very costly. The good news is that 90% of all estates in Colorado are not supervised, thus there are less costs and attorney fees associated with an informal probate. Because Colorado has adopted what is known as

the Uniform Probate Code, probate matters have been greatly simplified, thus, reducing the fear of probate that many of us exhibit today. So, does every estate need to go through the probate process? Not necessarily. For example, if an individual passes away with personal property and bank account assets (not real property) that are valued less than $63,000.00 a small estate affidavit can be used to transfer the property. Integrative Medicine: The Healthcare Health concerns related to the Model of the Future aging population like arthritis, chronic pain, obesity, falls, respiratory diseases, heart diseases, neuropathy and diabetes all require the care of multiple specialty providers. However, having to be seen for these conditions by multiple providers has put many seniors in a difficult position.

Over time, people have slowly become deeply dissatisfied with their healthcare and the healthcare system as a whole. More often than not, it has been due to the fact that they continually feel they are being rushed or the communication between all of their healthcare providers is missing. While the goal of our healthcare system has always been to provide great service that patients require, over the past few years, many healthcare providers have been providing the same care to more people in a lot less time, escalating the percentage of dissatisfaction. When it comes down to your care, everyone knows the importance and the need for your providers to think and work inter professionally. Despite this and everyone’s best intentions,

communication still remains minimal. It has become essential that healthcare providers are able to work together to coordinate treatment, prevent unnecessary testing, and be able to manage chronic conditions while being able to still provide quality care for their patients in the appropriate setting. Impact of Integrative Imagine being able to be treated by multiple healthcare providers in one clinic. Integrative clinics have been able to unite multiple healthcare providers and reduce the overall dissatisfaction by providing more time, attention, better communication, and a wider range of healthcare options all under one roof. Rehabilitation Hospitals, sometimes referred to as Inpatient Rehabilitation Hospitals, provide intensive rehab to patients who are generally medically stable but need help to recover from injury or other medical conditions. They can be found within a hospital or in a free-standing building. Rehabilitation hospitals were created to provide services less costly on a per diem basis than general hospitals but provide a high level of professional therapies such as physical, occupational and speech therapies. Rehabilitation Hospitals must offer a minimum number of physician visits, increased daily rehab hours (3 hours/day, 5 days/week of PT, OT, and/or Speech), and interdisciplinary services including increased RN oversight and nursing hours per day. was diagnosed with Frontotemporal Dementia (FTD). This is due to progressive nerve cell loss in the brain’s frontal lobes; it is the area behind the forehead or just behind the eyes.

Rhonda started noticing some behavior changes in her mother. “She started getting up in the middle of the night and getting dressed. She would set the table for the entire family. She was doing things out of nature financially,” Rhonda said. “She moved in with us in January of 2009 but we didn’t get the diagnosis of Frontotemporal Dementia until May. Finally—everything we had been experiencing—that just pulled it all together,” said Rhonda, with a look that can only be described as bitter sweet. They finally had the answers to the why? but now what? Caregivers are so often put in this role or a position of a role reversal. Ruth was referred to palliative care and eventually found herself on hospice care. As a full-time caregiver, Rhonda was able to utilize hospice’s respite program. “This allowed me some down-time. I could go sit at the park and read a book; go to my own doctor appointments; attend group meetings for care-

givers; it just allowed me to remove myself from the environment and recharge my batteries,” said Rhonda about her experience with the respite program. Rhonda shared the realistic views behind closed doors and what it’s really like to care for an ailing adult. “It’s like taking care of an adult infant. Every two hours I would take her to the bathroom; I would shower her; I dressed her; I would change her wet and soiled pants; I would feed her.” Rhonda also said she was never fully rested, only sleeping lightly at night because she always had one ear on her mother. As an educator in the community, to physicians, people are often surprised to learn of the many additional programs hospice offers and how, sometimes one program can collaborate with another. Until or unless someone is in the position of being a caregiver, people don’t realize the effects, physically and emotionally.

The focus is on comfort rather than treatment. HOUR MINIMUM – The least number of hours a Non-Medical Home Care Agency will be available for hire. Medicare certified Home Health Agencies are not on a specified time frame. INTRAVENOUS THERAPY (IV) – Refers to any medical treatments/ medications that need to be introduced into the body via a patient’s veins. Numerous IV Therapies can be

provided in a patient’s home under supervision of a MD. Some of these include: inter-infective/ antibiotic therapies, hydration therapy, pain management, chemotherapy and central line care and maintenance. MED-ALERT – Company offers a pendent or home alert system that notifies staff that user is in distress and needs immediate assistance. NON-MEDICAL HOME CARE – Assistance with Activities of Daily Living (ADL’s), homemaking, companionship and other non-medical in home care. These services are available for hire after, or in conjunction with Medicare certified services. Private payment, Medicaid and Veterans Benefits are a few of the ways personal care can be paid for.

A Client’s Home – What does it mean to be home? What a Privilege! Is it a location? Is it the cozy feeling when you walk in to a warm house on a cold day? Is it who you are with? Or is it a combination of many factors?

As proponents of home care, it is imperative that we recognize the aspects of what people define as home and their personal priorities in their dwellings. I was recently in the home of a client of mine, and her husband verbalized how the caregiver who was helping his wife liked to clean the kitchen floor right before she left in the afternoon, but that he and his wife liked their house fully clean first thing in the morning in case they had unexpected company that day. Home care today encompasses many things—from cleaning a client’s home the way they like, to skilled care in the home that would have taken place in a hospital not that long ago. Studies have shown and many of us have firsthand experience of the benefits of recovering, healing or just maintaining ourselves in a supportive home environment.

Clients can be around their own belongings, furnishings, family members, and pets which promotes a sense of security and safety and can decrease the stress on the client and family. With less stress comes increased health and wellness as well as a sense of control of their situation. Perhaps their diagnoses is out of their control, but if they are in their own environment, they potentially could feel better in general about where they are in life. As home care workers, let’s be mindful of where we are and who we are with when we step over the threshold into another person’s home, and treat it and them as we would like to be treated. Hospice Care Definitions HOSPICE Hospices provide individuals facing terminal illness with care at home or in a healthcare facility. The goal is to assist patients to live the last stage of their lives with dignity. The focus is on comfort rather than treatment. Services Provided: RN - REGISTERED NURSE – Assesses patient’s physical needs, develops and coordinates the plan of care, ensures symptom control and management, provides patient/family teaching as needed SW - SOCIAL WORKER – Assesses patient and family emotional and social needs, develops a plan of care, provides counseling, referrals and support to meet identified needs CHAPLAIN – Assesses patient and family spiritual needs, develops a plan of care, provides counseling, assists with memorial preparations. HOME HEALTH AIDE – Provides direct personal care to patient, emotional support to patient and family, reports identified needs to RN. VOLUNTEER – Provides respite and needed non-medical services for patients and their families, offers support at time of death and during bereavement. Levels of Care: RO-ROUTINE CARE – Occurs when regularly scheduled visits are adequate in maintaining the individual’s symptom management. Care is

GIP – General In-Patient Care – Provided in a skilled nursing facility when the symptoms cannot be managed elsewhere. The skilled nursing facility must have 24-hour R.N. coverage as deemed by Medicare guidelines. General in-patient care is usually short in duration. This level of care is usually due to symptoms which are out of control and more intensive nursing services are required. If an individual is at a care facility, room and board are covered by hospice during this period. CC-CONTINUOUS CARE – Provided to prevent the individual from going to the hospital when a crisis occurs. It also is given when more help is needed to assist the individual and family in coping with the crisis. This level of care is usually provided at the individual’s residence no matter where that may be. The criteria for continuous care may include: Severe pain or symptoms which are out of control, psychotic episodes, breakdown of caregiver, imminent death and family

What is Palliative Care? continued pain related, other symptoms such as labored breathing, nausea, anxiety or bleeding can be the presenting problem. The goal is to provide comfort, support and practical assistance in finding needed services for those with chronic illnesses, frail health or post-surgical problems that are not yet end-of-life issues. The most common conditions seen in palliative care are cancer, COPD and congestive heart failure. Hospice always includes palliative care, but palliative care does not always include hospice, because the patient does not have to be near death to receive palliative services. Palliative care teams emerged first in hospitals, but more recently, services are being provided in private homes, clinics and long term care facilities in an effort to provide comfort while keeping people out of emergency rooms and hospital beds. The palliative care team usually includes a doctor, nurse and social worker, although in recent years, “Navigators” have been added, to help patients navigate the maze of healthcare services in the community. Unfortunately, palliative care is not well supported by private insurance or government programs such as Medicare and Medicaid.

Because of the lack of reimbursement, most hospices can’t afford to provide non-hospice palliative care services, so many patients are left to live with their symptoms until they reach hospice-eligible status meaning six or less months to live. Medicare is beginning to embrace the inherent value of palliative care, recognizing the need for services during the gap that exists between hospitalization and hospice. Older patients – in particular – leave the hospital and become depressed, confused or ineffective in managing their own medications, only to escalate back to the hospital, or on to hospice. The frail elderly and those with chronic health conditions should contact their family doctor for a referral to a palliative care provider. A determination of eligibility can be provided quickly, and the care team works in conjunction with the patient’s personal physician to assure a coordinated approach to symptom management. whatever the prognosis. It is appropriate at any age and at any stage in a serious illness and can be provided along with curative treatment. PCP-OVERSIGHT – Can the hospice patient retain their own primary care physician in addition to the hospice medical director? All hospice organizations require a medical director, a physician to oversee their care. Many times the hospice medical director will work in conjunction with the patient’s own physician. The patient, family and hospice team can determine which physicians will oversee the care and certify the hospice eligibility criteria. PET THERAPY – There’s nothing like a visit from a four-legged creature to

brighten the day of a terminally ill patient. Pet therapy teams visit with patients wherever they reside – at home or in professional care settings. Studies have shown many benefits of pet companionship, ranging from reduced blood pressure and anxiety to relief from boredom, pain and loneliness. This service is not reimbursed by Medicare. Is Independent Living Four Questions to Ask Yourself. Right for Me? When you’re an older adult, any housing change can seem like you’re losing your independence. However, as the name suggests, independent living is more about making your life easier than giving up your independence. Sometimes, acknowledging your limitations—that you’re unable to manage the upkeep of your existing home, for example—and accepting some assistance now may help you maintain your regular independent routine for longer. 1. How easy is it for me to maintain my current home? Maintaining a home may be a longstanding source of pride for you, but it can also become a burden as you age. Perhaps your home has a large yard which requires constant maintenance, or maybe it’s becoming more difficult to clean those extra rooms that are rarely used. If your home is difficult to access, such as on a steep hill or up several flights of stairs, it may be harder for you to leave your home as often you’d like, leading to more isolation. Or increased crime may mean that your neighborhood is now too dangerous to walk around safely. Some of these challenges may be partially remedied by hiring outside help, remodeling parts of your home, or by other family members lending assistance. However, if you’d like a place that does not require a lot of maintenance and upkeep, independent living may give you more free-

dom and flexibility in the long run. 2. Is it difficult for me to connect to friends and family? The more isolated you are, the greater your risk for depression and other mental health problems. You may have a difficult time getting out of the house, perhaps due to trouble driving or increased mobility issues. Or your friends and neighbors may be busy with other work or family commitments, or the neighborhood may not be easy to get around. While the phone and Internet can help, nothing can take the place of face-to-face human connection. Independent living facilities can give you a built-in social network of peers, while many also provide structured activities such as sports, arts, or field trips. 3. How easy is it for me to get around? You may live in an area where you have to drive to attend social activities, visit friends, and shop. If you find yourself less comfortable with driving, you may find yourself relying more and more on public transportation or family and friends to get around. It may be harder to visit others, go to activities you enjoy, or keep doctors’ appointments. As well as onsite amenities, many independent living or retirement communities also offer convenient transportation options to outside activities.

Are You Noticing You go to visit mom or dad (or the Signs? both) at their home, as you have done countless times before.

The changes may be so subtle or come on so slowly you may not immediately notice the signs, but today something you see causes you to have concerns. Perhaps your parent or loved one is not engaging in activities or hobbies which once brought such pleasure. You could see a pile of unopened bills on the counter. You may even notice that the quality or amount of food in the house is not up to par anymore, or that mom or dad is increasingly disheveled and confused. These may be indications that mom or dad need help. It is not uncommon for an adult child to serve as a caregiver to a parent. Sometimes this responsibility grows beyond that caregiver’s abilities or the time they can dedicate to that care. As a caregiver, if you have begun to wonder if the commitment is exceeding you, look to yourself as a guide. Is the time required to properly care for your loved one interfering with your job, your personal relationships, and perhaps even your health? Many experience caregiver’s guilt, but one must rise above the guilt and start to consider alternatives. It is ok to

seek out help! Many times the decision to move a family member to Assisted Living has been born of a crisis or medical emergency. However, if you begin to consider options before crisis strikes, an informed decision made with plenty of time to act can make a world of difference. There are senior care advisors out there who specialize in assisting families find the best care for their love one when living at home is no longer a viable option. There are many considerations to take into account during this time, and an advisor can help navigate you through the process. Also, there are local government resources in the community whose primary focus is to assist seniors and their families get the assistance they need. The best thing that you can for your loved one is to equip yourself with the support and knowledge to make the best decision possible, and start to look for that help as soon as you begin to notice the signs.

SKILLED NURSING FACILITY – Facilities provide 24/7 care for residents and offer full assistance with ADL’s. Daily rates typically include a furnished bedroom and a private or shared bath, meals, activities, transportation, phone service, cable television and all personal/nursing care. Payor sources usually accepted are Medicare, Medicaid, VA, some insurance plans and private sources. TRACH CARE - Refers to care administered to patients who have undergone a tracheotomy.

A tracheotomy is a surgical procedure in which a cut or opening is made in the windpipe (trachea). The surgeon inserts a tube into the opening to bypass an obstruction, allow air to get to the lungs, or remove secretions. VENTILATOR - Moves breatheable air into and out of the lungs, to provide the mechanism of breathing for a patient who is physically unable to breathe, or is breathing insufficiently. WANDERGUARD - Offered at some facilities. A resident with memory loss or confusion wears a small discrete pendant. Each external door in the care home is fitted with a radio detector which detects when a pendant moves near an exit. The WanderGuard will sound an audible alarm, alerting the staff of a patient wandering.
