It is with deep gratitude that I share this report, a reflection of the impactful care, compassion, and commitment our team brings to every life we touch.
In our 46th year of caring for life throughout the journey with illness and loss we are proud to highlight some of the ways we do more for our patients and families, for our community partners, and for our team. Meaningful experiences are carefully created by the incredible people who make up our hospice care team and our dedicated volunteers. The dedication, empathy, and willingness to go above and beyond allow us to provide not just care, but comfort; not just services, but presence.
We offer support that extends far beyond Medicare requirements. Navigating progressive serious illness is challenging and our team members and volunteers help to make moments matter by providing more time, more visits, and more personalized attention. Families receive more grief counseling, more guidance, and more heartfelt connection. Our community benefits from healing programs open to all in the community, outreach, education, and access to compassionate care — no matter their circumstances.
The heart of our care lies in the incredible people who provide it. That’s why caring for our team’s wellbeing is not just a priority—it’s a promise. Through continued learning, wellness programs, and a culture rooted in safety, respect, and compassion, we support our team so they can continue caring for families with excellence, empathy, and heart each and every day.
To our community, heath care partners, and donors we thank you for your trust and partnership. Your belief in our mission makes this work possible and honors those who give so much of themselves to carry it out. Our future is bright and with your continued support there is more care, more love, and more connection. We are proud to be the #1 independent, community-based hospice and palliative care provider in Maryland and look forward to a future filled with so much more.
80,000 hours of dedicated attention to patients’ and caregivers’ concerns. Our compassionate and knowledgeable staff are available to answer any questions 24 hours a day, providing peace of mind and immediate support during crises, symptom changes, or emotional distress.
More safety, mobility, and independence.
181,255 days of care with specialized medical equipment in the homes.
Whether it’s a wheelchair, hospital bed, walker, or oxygen support, durable medical equipment gives patients more control over their daily lives and care decisions, prevents complications, and eases caregiving — improving both quality of life and peace of mind.
More dignity.
213,241 days of care with dignity-preserving personal supplies.
Providing patients with incontinence, wound care, and personal care supplies provides patients with dignity and a sense of comfort, control, and respect in their daily lives. These essentials, often overlooked, play a critical role in preserving self-esteem — helping patients feel cared for, dignified, and valued.
More connection.
8,047 volunteer hours of human connection.
Thanks to our robust and rewarding volunteer program, our patients receive more hours of companionship, respite, and meaningful moments — 3,000 more than those required by Medicare.
More for our Patients, Families, and Community
More recognition.
194 Veteran honor salutes.
Veterans deserve and receive special care that recognize their service and sacrifice. Honor salutes validate the veteran’s service and sacrifices, often stirring emotions tied to identity, pride, and belonging. This recognition not only honors their military legacy but also fosters healing, helping veterans feel seen, respected, and valued during a vulnerable chapter of their journey.
More community support.
69% of grief services supported community members beyond our hospice care.
We go beyond Medicare requirements by offering comprehensive grief support not only to the families of our patients but to the broader community. By extending these services to anyone in need, regardless of their connection to our hospice, we reaffirm our commitment to healing and emotional well-being throughout the communities we serve.
More innovation.
Disease Specific Programs • High-Flow Oxygen Therapy • NICHE Training • Clinical Ladder • Telehealth • Virtual Reality • Dementia Support • End-of-Life Doulas • Adventures Through Grief Role-playing • Equine Therapy • Non-Invasive Therapies and Approaches • Simulation Labs • Cutting Edge EHR/EMR • CRISP Exchange * Leadership Ladder • Standardized Nurse Visits • And more.
We are embracing innovation to do more. By investing in these innovative approaches and using data to guide our progress, we are continually striving to raise the standard of care, so that every life we touch is met with skill, presence, purpose, and love.
Everyone was wonderful, particularly at the very end where they were right there when we needed them. Thank you!

I wanted to share my appreciation for Hospice of the Chesapeake. I made a home hospice referral late Wednesday, Hospice of the Chesapeake made contact with family and had DME delivered on Thursday, and we were able to send the patient home for intake Friday evening.
The whole coordination process took about 5 messages in Navihealth, and it went so smoothly I was almost worried it went too well. Thanks for always being such an amazing partner!

More for our Staff
I came to hospice as a burnt-out nurse. I wanted to go back to school to do anything else. I stayed in nursing because I found purpose and peace here. Everything I wanted nursing to be, I found it here. I love being a part of the clinical management team. I can make changes and support a wonderful group of clinicians.

More listening. Meaningful Work · Schedule Flexibility · Workplace Culture
Over the past year, we’ve taken intentional steps to ensure every voice is heard, from anonymous surveys and listening sessions to one-on-one check-ins. This feedback has directly shaped meaningful changes, including the addition of well-being days, enhancements to internal communication, and new professional development opportunities. By responding with action, we’re building a culture where staff feel valued, respected, and empowered —because the better we care for our team, the better we care for our patients and families.
