ACZONE® (dapsone) Gel, 7.5% BRIEF SUMMARY—PLEASE SEE THE CLINICAL ACZONE® PACKAGE INSERT FOR FULL PRESCRIBING INFORMATION. INDICATIONS AND USAGE ACZONE® Gel, 7.5% is indicated for the topical treatment of acne vulgaris in patients 12 years and older.

DOSAGE AND ADMINISTRATION For topical use only. Not for oral, ophthalmic, or intravaginal use. After the skin is gently washed and patted dry, apply approximately a pea-sized amount of ACZONE® Gel, 7.5% in a thin layer to the entire face once daily. In addition, a thin layer may be applied to other affected areas once daily. Rub in ACZONE® Gel, 7.5% gently and completely. If there is no improvement after 12 weeks, treatment with ACZONE® Gel, 7.5% should be reassessed.

CONTRAINDICATIONS None.

WARNINGS AND PRECAUTIONS Hematological Effects Methemoglobinemia Cases of methemoglobinemia, with resultant hospitalization, have been reported postmarketing in association with twice-daily dapsone gel, 5% treatment. Patients with glucose‐6‐phosphate dehydrogenase deficiency or congenital or idiopathic methemoglobinemia are more susceptible to drug‐ induced methemoglobinemia. Avoid use of ACZONE® Gel, 7.5% in those patients with congenital or idiopathic methemoglobinemia.

Signs and symptoms of methemoglobinemia may be delayed some hours after exposure. Initial signs and symptoms of methemoglobinemia are characterized by a slate grey cyanosis seen in, e.g., buccal mucous membranes, lips, and nail beds. Advise patients to discontinue ACZONE® Gel, 7.5% and seek immediate medical attention in the event of cyanosis.

Dapsone can cause elevated methemoglobin levels particularly in conjunction with methemoglobin‐ inducing agents (see Drug Interactions).

Hemolysis Oral dapsone treatment has produced dose-related hemolysis and hemolytic anemia. Individuals with glucose-6-phosphate dehydrogenase (G6PD) deficiency are more prone to hemolysis with the use of certain drugs. G6PD deficiency is most prevalent in populations of African, South Asian, Middle Eastern, and Mediterranean ancestry.

In clinical trials, there was no evidence of clinically relevant hemolysis or hemolytic anemia in subjects treated with topical dapsone. Some subjects with G6PD deficiency using dapsone gel, 5% twice daily developed laboratory changes suggestive of hemolysis (see Use in Specific Populations).

Discontinue ACZONE® Gel, 7.5% if signs and symptoms suggestive of hemolytic anemia occur. Avoid use of ACZONE® Gel, 7.5% in patients who are taking oral dapsone or antimalarial medications because of the potential for hemolytic reactions. Combination of ACZONE® Gel, 7.5% with trimethoprim/sulfamethoxazole (TMP/SMX) may increase the likelihood of hemolysis in patients with G6PD deficiency (see Drug Interactions).

Peripheral Neuropathy Peripheral neuropathy (motor loss and muscle weakness) has been reported with oral dapsone treatment. No events of peripheral neuropathy were observed in clinical trials with topical dapsone treatment.

Skin Reactions Skin reactions (toxic epidermal necrolysis, erythema multiforme, morbilliform and scarlatiniform reactions, bullous and exfoliative dermatitis, erythema nodosum, and urticaria) have been reported with oral dapsone treatment. These types of skin reactions were not observed in clinical trials with topical dapsone treatment.

ADVERSE REACTIONS Clinical Studies Experience Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug and may not reflect the rates observed in practice.

A total of 2161 subjects were treated with ACZONE® Gel, 7.5% for 12 weeks in 2 controlled clinical trials. The population ranged in age from 12 to 63 years, was 56% female, and was 58% Caucasian. Adverse drug reactions that were reported in at least 0.9% of subjects treated with ACZONE® Gel, 7.5% appear in the table below.

Experience with Oral Use of Dapsone Although not observed in the clinical trials with topical dapsone, serious adverse reactions have been reported with oral use of dapsone, including agranulocytosis, hemolytic anemia, peripheral neuropathy (motor loss and muscle weakness), and skin reactions (toxic epidermal necrolysis, erythema multiforme, morbilliform and scarlatiniform reactions, bullous and exfoliative dermatitis, erythema nodosum, and urticaria).

Postmarketing Experience The following adverse reactions have been identified during post-approval use of topical dapsone. Because these reactions are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.

Methemoglobinemia has been identified during postmarketing use of topical dapsone (see Warnings and Precautions).

DRUG INTERACTIONS No formal drug-drug interaction studies were conducted with ACZONE® Gel, 7.5%.

Trimethoprim-Sulfamethoxazole A drug-drug interaction study evaluated the effect of the use of dapsone gel, 5% in combination with double strength (160 mg/800 mg) trimethoprimsulfamethoxazole (TMP/SMX). During co-administration, systemic levels of TMP and SMX were essentially unchanged; however, levels of dapsone and its metabolites increased in the presence of TMP/SMX. The systemic exposure from ACZONE® Gel, 7.5% is expected to be about 1% of that from the 100 mg oral dose, even when co-administered with TMP/SMX. Topical Benzoyl Peroxide Topical application of dapsone gel followed by benzoyl peroxide in patients with acne vulgaris may result in a temporary local yellow or orange discoloration of the skin and facial hair.

Drug Interactions with Oral Dapsone Certain concomitant medications (such as rifampin, anticonvulsants, St. John’s wort) may increase the formation of dapsone hydroxylamine, a metabolite of dapsone associated with hemolysis. With oral dapsone treatment, folic acid antagonists such as pyrimethamine have been noted to possibly increase the likelihood of hematologic reactions.

Concomitant Use with Drugs That Induce Methemoglobinemia Concomitant use of ACZONE® Gel, 7.5% with drugs that induce methemoglobinemia such as sulfonamides, acetaminophen, acetanilide, aniline dyes, benzocaine, chloroquine, dapsone, naphthalene, nitrates and nitrites, nitrofurantoin, nitroglycerin, nitroprusside, pamaquine, para‐aminosalicylic acid, phenacetin, phenobarbital, phenytoin, primaquine, and quinine may increase the risk for developing methemoglobinemia (see Warnings and Precautions).

USE IN SPECIFIC POPULATIONS Pregnancy Teratogenic Effects: Pregnancy Category C There are no adequate and well controlled studies in pregnant women. ACZONE® Gel, 7.5% should be used during pregnancy only if the potential benefit justifies

the potential risk to the fetus. Dapsone has been shown to have an embryocidal effect in rats and rabbits when administered orally during the period of organogenesis in doses of 75 mg/kg/day and 150 mg/kg/day, respectively (approximately 1400 and 425 times, respectively, the systemic exposure that is associated with the maximum recommended human dose [MRHD] of ACZONE® Gel, 7.5% based on AUC comparisons).

These effects may have been secondary to maternal toxicity.

Nursing Mothers Although systemic absorption of dapsone following topical application of ACZONE® Gel, 7.5%, is minimal relative to oral dapsone administration, it is known that dapsone is excreted in human milk. Because of the potential for oral dapsone to cause adverse reactions in nursing infants, a decision should be made whether to discontinue nursing or to discontinue ACZONE® Gel, 7.5%, taking into account the importance of the drug to the mother.

Pediatric Use Safety and efficacy were evaluated in 1066 subjects aged 12–17 years old treated with ACZONE® Gel, 7.5% in the clinical trials. The safety profile for ACZONE® Gel, 7.5% was similar to the vehicle control group. Safety and effectiveness of ACZONE® Gel, 7.5% have not been established in pediatric patients below the age of 12 years.

Geriatric Use Clinical trials of ACZONE® Gel, 7.5% did not include sufficient number of patients aged 65 and over to determine whether they respond differently from younger subjects.

Glucose-6-phosphate Dehydrogenase (G6PD) Deficiency Individuals with glucose-6-phosphate dehydrogenase (G6PD) deficiency may be more prone to methemoglobinemia and hemolysis (see Warnings and Precautions). ACZONE® Gel, 5% and vehicle were evaluated in a randomized, double-blind, cross-over design clinical study of 64 subjects with G6PD deficiency and acne vulgaris. Subjects were Black (88%), Asian (6%), Hispanic (2%), or of other racial origin (5%). Blood samples were taken at Baseline, Week 2, and Week 12 during both vehicle and ACZONE® Gel, 5% treatment periods. Some of these subjects developed laboratory changes suggestive of hemolysis, but there was no evidence of clinically significant hemolytic anemia in this study (see Warnings and Precautions).

Rosacea News Good news for rosacea sufferers! In addition to many

age-old therapies, several new recently approved treatments are very effective for this chronic condition.

By Michele J. Farber, MD Rosacea can present in several forms.

This ranges from prominent facial redness, to a predominance of inflammatory pimple bumps, to thickening of skin mainly on the nose, to ocular rosacea with grittiness in the eyes. One of the most plaguing aspects of rosacea is the persistent flushing. The recently approved Mirvaso (brimonidine 0.33%) gel can be extremely effective for this indication.

It works by constricting blood vessels and can be used daily or for special events to combat redness. Soolantra (ivermectin 1%) cream, another new medication, has been shown to be effective for the pimple bumps or inflammatory lesions of rosacea. This cream has antimicrobial properties, and recent studies showed that it was effective for patients who did not clear with topical antibiotic creams.

Therapies

These therapies can be used

alone or in combination with other time-tested rosacea treatments. Rosacea therapies target the increased inflammation, barrier dysfunction, and bacteria that can contribute to rosacea.

Mild to moderate rosacea can be treated with topical creams and washes, such as metronidazole gel/cream, azelaic acid (finacea), sodiumsulfacetamide wash or lotion, pimecrolimus (elidel) cream, and tretinoin cream. Oral treatment can be extremely important for more severe rosacea. This typically includes antibiotics such as doxycycline, and can escalate to isotretinoin (accutane). Oral antibiotics and eye drops can be crucial for decreasing ocular grittiness for those with ocular rosacea.

Take Your Vitamins Certain vitamins are also helpful when patients do not respond to or prefer not to use traditional oral therapies. Nicotinomide and zinc can act as anti-oxidants and have anti-inflammatory effects. These supplements can

be used alone or in combination with other treatments. Newer studies have shown that patients may respond to these treatments even after failing oral antibiotics.

Lasers to the Rescue Laser therapy is the most definitive treatment for the redness and flushing associated with rosacea. There are multiple lasers that can be effective for rosacea, including the pulsed dye laser (PDL), the potassium titanyl phosphate (KTP) laser, neodymium-doped, yttriumaluminium-garnet (Nd:YAG) and the intense pulsed light (IPL) system. Ablative lasers such as the CO2 laser, which help to resurface the skin, can help improve thickening of the nose skin (rhinophyma) associated with rosacea. The newest studies focus on optimizing laser settings and combining lasers for maximum benefit. An in-office consult can help guide the optimal regimen for each person.

Laser therapy is the most definitive treatment for the redness and flushing associated with rosacea.

Top Rosacea Triggers Rosacea can be triggered by several factors including sun exposure, emotional stress, alcohol, spicy foods, caffeine, wind, hot weather, cold weather, heavy exercise, hot showers, and hot drinks among other things. Avoiding known risk factors, to the extent possible, can be an important adjunct to medical therapy. Additionally, gentle skin care as reviewed below can mitigate the effects of many rosacea triggers.

extra gentle skin care to reduce redness and prevent flares.

It is most important to use a non-irritating cleanser, such as Cetaphil or CeraVe, to help lock in moisture. Look for ingredients such as ceramides, hyaluronic acid, and glycerin that can hydrate the skin and avoid alcohol-based products. Next, slather on a non-comedogenic moisturizer, and sunscreen with SPF 30 or above to prevent flares from UV and weather extremes.

Creams, which are thicker than lotions, can be more soothing when the skin is irritated.

Physical sunscreens containing zinc oxide or titanium dioxide also are gentler for broken-

out skin. Lastly, green-tinted moisturizers, such as Eucerin Redness Relief can help camouflage redness under makeup.

Science of Aging Skin What happens to our faces when age and what to do about it

BY MARA C. WEINSTEIN VELEZ, M.D., FAAD Aging is not a choice. There are natural changes that occur with the skin over the decades. The rate at which we age can be due to genetics, physiological, photoaging, pollution, and stress among many of life’s issues. Physiologically, as we age, we tend to lose volume in our face especially in the cheek and temple area.

The bones supporting our eyes become wider and longer (the eye socket) and the length and height of our lower jawline decreases making it less well defined.

Also, the space between the base of the nose and upper lip (cupids bow) widens making the upper lip appear flatter and longer.

Collagen and elastin are responsible for the skin’s elasticity, keeping our skin tight and young looking. As we age, our body stops producing collagen and elastin. Sun exposure, environmental pollutants, stress and illness can introduce free radicals into our skin which only speed up the process of aging causing destruction of collagen and elastin. This results in loose, sagging skin with little elasticity and wrinkles that stay in place even when we are not making natural expressions. This is the difference between static wrinkles and dynamic wrinkles.

Luckily, dermatologists are licensed professionals who are highly educated on the facial aging process, but also have the tools to not only slow the process but

also reverse the signs of aging, to keep you looking like a natural, but better version of yourself. Stick to a maintenance plan and you will be decades ahead!

Your late 20s is a good time to consider preventative Botox, at this age it’s likely only needed twice a year. The earlier you start Botox, the less you will need it over time as your muscles will learn not to frown all the time. If you already have brown spots and enlarged pores, consider quarterly peels (Glycolic acid, Vi Peel or Jessner’s) or microdermabrasion or a light laser treatment, such as Clear + Brilliant or a laser toning treatment with the Laser Genesis, both have minimal downtime and are considered lunchtime procedures for maintenance.

• Consider serums with growth factors and peptides to stimulate collagen production regularly

At the Dermatologist in Your 30s • Now is the time to add regular use of neurotoxin (Botox, Dysport, or Xeomin) every 4-6 months. It’s also a good time to check in with your dermatologist about fillers, especially to the cheeks to lift and soften the lines starting around your nasolabial folds.

• Start to address texture irregularities of the skin surface from left behind acne scars. Consider Fraxel 1550 monthly until they are softened (it may require 4-6 treatments) with the added bonus of boosting collagen and elastin production with a non ablative resurfacing laser.

• Consider microneedling for maintenance quarterly. It has minimal downtime and is typically less expensive than a laser treatment. Microneedling creates microscopic wounds in the dermis, which stimulates a wound healing and repair response that encourages neocollagenesis and neovascularization (new collagen production and new blood vessels to nourish the repair).

• Address those brown spots! Your age is easily detected by the amount of brown spots you have on the skin (hands, neck, chest, arms). Get rid of them to give the surface of the skin a more even skin tone. Studies have shown that by evening out the color of the skin, you can take 6-12 years off! Consider IPL or Fraxel 1927 for easy, minimal downtime procedures, requiring 1-2 treatments to see significant reduction.

In Your 40s Now is when your skin starts to look tired and begins to lose its bounce. Skin also feels rougher, drier, and bruises more easily. You probably have more hyperpigmentation and deeper expression lines.

• Use an eye cream with peptides and growth factors • Gentle cleansing • Antioxidant serums • Exfoliate weekly • Drink 8 glasses of water daily to stay hydrated!

At the Dermatologist in Your 40s Skin tightening! Ulthera or Thermage help to tighten the skin with either ultrasound technology or radiofrequency. Think of these procedures as a way to rebuild the foundation of the facial structure.

Once you have a solid foundation, you can continue to build from the bottom up to help create a better, more youthful version of your 40 year old self. These procedures tend to be on the pricier side but the results are permanent and worth it. The changes are subtle and noticeable. The best part is that there is no downtime. Repeat every 18-24 months for lasting results. After all — we keep on aging so it’s a constant battle against time.

In Your 50s If you have read this far, the 50s+ is a time to really get into a maintenance regimen and keep it! The ideal 50s+ antiaging regimen looks something like this:

At Home Skincare in Your 50s In the AM: • Cleanser or cream with LHA, glycolic acid • Antioxidant serum • Eye cream • Moisturizer with SPF In the PM: • Peptide/growth factor serum • Eye cream • Retinoid • Moisturizing cream if your skin is dry

At the Dermatologist in Your 50s At Home Skincare in Your 40s • Use a rich moisture cream at all times containing lipids and ceramides to restore moisture barrier, which is lost with age

Keep it simple! there are only 4-5 musts for your maintenance plan, everything else can be considered fine tuning.

There are so many new lasers and new technology on the horizon but if you

start with these simple procedures and do them regularly, you are guaranteed to always be up to date with anything else that is developed. You and your dermatologist can work together to decide what else you may need.

Neurotoxin every 4 months (on the average) to treat crow’s feet, glabella and forehead. This is your workhorse and you must continue to maintain it!

Regular neurotoxin injections act as a good “check in” with the dermatologist 3 times a year to make sure you are on the right track with your regimen and to check in to see if there is anything new on the horizon—there are always new and updated treatments in the works!

Fillers for the cheeks, under eyes/ tear troughs, lips, temples, chin area and hands every 6-9 months. You may not need this often, check in with your dermatologist during the Botox visit.

Skin tightening (face, neck, chest) every 18-24 months.

Skin resurfacing (face, neck, chest) to treat texture, tone, and pore size with a non ablative fractionated laser (such as Fraxel 1550). Initially 4-6 sessions spaced 4-6 weeks apart to make the most dramatic difference with minimal downtime. Then continue 2 times a year.

Or try IPL initially 1-3 sessions every 4-6 weeks ideally (in the fall or winter) then once a year.
