Growing Experience with Fingolimod at Rashid Hospital, Dubai.

Multiple sclerosis (MS) is a chronic disease that afflicts the central nervous system. It is most often characterized by relapsing-remitting course at the onset, followed by secondary progression. Inflammatory tissue damage mediated by auto reactive T and B cells is thought to initiate lesion formation. Variable remyelination and axonal loss result in accumulation of physical and cognitive disability. Many new developments in the field of MS occurred recently. First, it became evident that MS is not only demyelination; degeneration occurs early in the disease and may lead to brain atrophy. Second, it has been shown that MS doesn’t involve the white matter only; the grey matter is affected equally. Third, the introduction of oral MS therapies is proving to be effective. These facts have clinical and therapeutic implications. Most of the MS therapies reduce inflammation, but only Fingolimod reduces brain atrophy. All currently approved diseasemodifying therapies aim to prevent clinical relapses and delay disability.

Until recently, all therapeutic options for RRMS required parenteral administration. Despite excellent long-term safety profiles, patient compliance with first-line drugs—interferon beta (IFNβ) is limited by modest efficacy, frequent injections, and unpleasant side effects. The U.S. Food and Drug Administration (FDA) approval of Fingolimod in September 2010, the first oral disease-modifying therapy, generated much interest among both patients and MS physicians.

The role of Fingolimod in MS treatment continues to evolve. In this review, I will describe our experience and then highlight certain issues to be considered in safe use of Fingolimod for patients with RRMS. In September 2010, the 0.5 mg dose of Fingolimod (Gilenya) was approved in many countries to reduce relapses and delay accumulation of physical disability in relapsing forms of MS. Some regulatory agencies,

such as Health Canada and the European Medicines Agency approved it as second-line therapy. In other countries, like the United States and Switzerland, it was endorsed as first-line therapy. Fingolimod was approved in UAE (Dubai) in December 2010 for RRMS, and it can be used as first line and second-line agent.

The first patient with MS, in the Middle East, started Fingolimod (Gilenya) in December 2010 at Rashid Hospital, Dubai. She was a young Emirati girl with 8 years history of RRMS. She failed first -line treatment with three injectable interferon Beta (IFNβ). She used to get two to three relapses per year; some of these relapses were disabling. She required frequent IVMP courses. Moreover, she developed the known adverse symptoms of these injections including phobias, depression, skin lesions, and because she travels frequently, it was inconvenient for her to take the injections along with her; therefore, she

was eager to start oral therapy. She started Gilenya in December 2010. Since then she had one minor relapse, and now she is working, traveling frequently, and living a normal life.

She demonstrated that Fingolimod was an appropriate option for patients with RRMS with intolerable side effects on IFNβ or an inadequate therapeutic response.

It is also good option for some patients particularly those who are seropositive for JC virus; thus, at risk of developing progressive multifocal Leukoencephalopathy (PML) with Natalizumab.

Although Fingolimod has been approved in the United States as a first-line agent, in the past we tended to recommend IFNβ for the treatment-naive patients, largely because of the lack of experience with the use of Fingolimod, but with the growing experience with the drug, our use of Fingolimod as a first-line agent has increased. This is supported by the post marketing studies which have confirmed good safety and efficacy.

We report our experience with Fingolimod at Rashid Hospital neurology department, which is a tertiary center for the management of Multiple Sclerosis. In the period from December 2010 to July 2013, 93 MS patients were screened, and 85 of them were found eligible to receive Fingolimod prescription. During screening, one patient was found to have brain tumor (Miningioma) and a second patient has sinus

bradycardia of 40 beats per minute, so both were denied Fingolimod. A third patient with type 1 DM was also excluded. A fourth patient with significant liver enzyme elevation was also denied Fingolimod.

Five patients completed screening and were eligible, but they delayed the beginning of the treatment for different personal reasons. 81 out of the 85 patients were prescribed the medication at Rashid Hospital.

Three patients started the first dose in Europe though it was prescribed at Rashid Hospital. Eight patients were treatment-naïve patients. All patients were included in the analysis. Fingolimod was most frequently used in relapsing remitting MS as second-line and was prescribed as a first-line agent in eight cases. First dose observation (FDO) was uneventful in all the patients. Adverse events during FDO were self-limited and included transient symptomatic bradycardia (n = 1), chest tightness (n = 1) and hypertension (n = 1).

The patient who developed symptomatic self-limited ventricular nodal ectopics and bradycardia was referred to a cardiologist, FDO was extended to 8 hours, no treatment was given, ECG normalized, and she recovered. She was discharged home and was called the next day for a follow-up; the ECG proved to be normal. Fingolimod was well-tolerated during FDO, and adverse events were self-limited. Most Fingolimod patients were satisfied with their FDO experience. Satisfaction with Fingolimod was high and observed higher acceptance among treatmentexperienced compared to treatment-naïve patients.

Adverse effects leading to discontinuation by more than one patient included leucopenia/lymphopenia (3), macular edema (1), and liver enzyme elevation (1). Three patients discontinued treatment because they became pregnant while on treatment, one of them resumed Fingolimod after delivery of normal twin babies and two are still pregnant 7 and 3 months respectively. Interestingly, one patient gave birth to healthy twins, and another was also pregnant with twins. The adverse effects we noticed were similar to those described in clinical trials, but the discontinuation rate was lower. Lymphopenia and increased liver enzymes were the most common laboratory abnormalities. Fingolimod was continued in 78 patients.

The clinical trials that led to the approval of Fingolimod demonstrated benefits in relapses, disability progression, magnetic resonance imaging (MRI) activity, and brain volume loss in treatment-naïve and previously treated patients with relapsing-remitting MS. Additional clinical trial and post marketing data suggest that Fingolimod is a safe, effective, and well-tolerated treatment option when patients are selected and monitored appropriately.

Multiple Sclerosis National Awareness Campaign 2013 This campaign, which has been organized throughout the year 2013 by the Multiple Sclerosis Society of Friends in United Arab Emirates in collaboration with Novartis Pharmaceutical Company, aims to raise awareness on Multiple Sclerosis among all segments of society in addition to educating and enlightening the Multiple Sclerosis patients and their families.

The National Campaign Activities: C: Annual Calendar

With the participation of group of Multiple Sclerosis patients, an annual calendar for 2013 was designed.

In this calendar, each patient represents a specific month, where he would express his thoughts through short paragraphs or positive words that can plant hope in fellow patients’ hearts.

MS Nurse Committee

On February 19, 2013 a committee of specialized Multiple Sclerosis and neurology nurses, from all over the country, was formed. The purpose of this committee is to conduct workshops every trimester in order to enhance health education among the medical staff that are in charge of Multiple Sclerosis patients.

MS-Patients 3rd Forum

This forum was held in Dubai in the presence of Sheikh Mohamad Bin Khalifa Al Nahyan, the members of Multiple Sclerosis association as well as MS patients and their families. The agenda included awareness lectures as well as working groups on natural therapy and living with Multiple Sclerosis. Patients were given the chance to share their experiences with MS.

Dear MS Nurses, On May 31, 2013, I have had the pleasure to attend the First UAE MS Nurses’ Working Group. The event, which took place in Dubai, UAE, was held under the theme, “Touching Lives with a Caring Heart.”

I have found the workshop to be quite enriching, extremely enlightening, and very well-organized. During the event, Dr. Toufik Al Ssadi gave an inspiring speech on MS and EDSS. In addition, two specialized MS nurses presented a lot of valuable information and case studies to the fellow and attending nurses, who are credited with 3 CME HRS accredited by MOH. I am delighted to share with you that during this meeting, I was voted by all the attendees as the MS Nurse for the semester extending from January to June 2013.

Here, I would like to take this opportunity to thank all the nurses for their kind expression of appreciation. Your hard work and inspiration to the whole team members are highly commendable.

I hope we will continue this good work and incessant progress in caring for MS patients.

Living with MS

A Patient testimonial

I have been living in Dubai for more than 6 years now.

However, I had been assigned to work on a project in Abu Dhabi for the third time. Therefore, I have to drive for many hours every day in order to get to work and back. I have got a busy and hectic daily schedule, while I have Multiple Sclerosis. I live alone since my family is in the Philippines, where my wonderful wife is taking care of my two daughters.

I was diagnosed with MS in September 2012. I have experienced a number of relapses before that date, but doctors back then made different assumptions.

When I was working in Karachi, Pakistan in 2006, I suddenly experienced a blurred vision in one eye.

The doctors examined me and said it is only optic neuritis, and I was treated with steroids. Few weeks later my vision got better. In that same year, I went back to my home country as the situation in Pakistan got unstable, and I was scared to stay there. In a few months, the same eye got blurred again until it went totally blind. My eyes were examined again, and once again I just given steroids and my vision improved in due time. I began to surf the Internet searching for a solution to my dilemma.

In my search, I suspected that I have MS, but I was not totally concerned with the issue as I thought it does not fit my profile even though most symptoms seemed to match my case. However, I excluded the idea believing that MS would not afflict me since I am an Asian, and I was not living in a cold environment.

In 2007, I arrived in Dubai looking for a greener pasture.

Things were going well at my end until I woke up one day in 2010 and was terrified to see in the mirror that I became cross eyed. I applied for a leave from my work and rushed to a neurologist. I was scanned by an MRI, but it did not reveal anything. I was given steroids again, and my vision became normal again.

Then in 2011, the right side of my face suddenly got paralyzed. I could not even eat properly, and I was so depressed. I was then diagnosed with Bell’s palsy.

Then in 2012, I felt that something was seriously wrong with me. I could not balance myself, and there was a ringing tone in my ears, which was very annoying. The doctor requested another MRI, and this time white lesions appeared in my brain, indicating it I have MS. Since January 2013, I have had no relapses. I feel completely normal and I lead a regular life. I still have a hectic job with a heavy workload. Lately, I had my annual vacation, where I travelled home and went swimming with my family. I even had a zip line adventure with my daughter.

If you have MS, seek help but live your life. Live normally as if nothing has happened to you. Just do not give up, and make yourself and your family happy!

‫في النصف األيمن لوجهي‪ .‬لم يعد بامكاني حتى تناول الطعام‪،‬‬ ‫وأصبت باحباط شديد‪ .‬تم تشخيص الحالة على أنها شلل بيل‬ ‫(‪ .) Bell’s Palsy‬ثم في ‪ ،2012‬شعرت أن شيئًا سيئًا أصابني‪ .‬لم‬ ‫استطع التوازن وكان هناك رنين مزعج في أذني‪ .‬طلب الطبيب‬ ‫مني مجددًا أن أخضع لمسح التصوير بالرنين المغناطيسي‪،‬‬ ‫وهنا ظهرت اآلفات البيضاء في دماغي‪ ،‬مشير ًة أني مصاب بداء‬ ‫التصلب اللويحي‪.‬‬ ‫ومنذ العام ‪ ،2013‬لم اصب بأي انتكاسات‪ .‬أشعر أنني طبيعي‬ ‫وأعيش حياة عادية‪ .‬وال تزال مهنتي مضنية ومزدحمة باألعمال‪.‬‬ ‫مؤخرًا‪ ،‬أخذت اجازتي وذهبت إلى دياري حيث قمت بالسباحة مع‬ ‫عائلتي‪ .‬حتى أني قمت بمغامرة االنزالق على الحبال المعلقة (‪zip‬‬ ‫‪ )line‬مع أبنتي‪.‬‬ ‫إذا كنت مصابًا بداء التصلب اللويحي‪ ،‬إبحث عن المساعدة واحيا‬ ‫حياتك‪ .‬إستمر في العيش كأن شيئًا لم يحدث لك‪ .‬ال تستسلم‪،‬‬ ‫وأجعل نفسك وعائلتك سعداء‪.‬‬

‫‪/ 6‬ممرضي التصلب المتعدد‬

‫اعزائي ممرضي التصلب المتعدد‪،‬‬ ‫لقد كان من دواعي سروري المشاركة بأول ورشة عمل لممرضي التصلب المتعدد‬ ‫في دولة اإلمارات العربية المتحدة‪ُ .‬أقيم هذا الحدث تحت شعار «لمسة من القلب‬ ‫تح ّيي األمل» في مدينة دبي‪ .‬كانت هذه الورشة غنية بالمعلومات الق ّيمة‪ ،‬كما كان‬ ‫تنظيمها ممتازًا‪ .‬خالل اللقاء ألقى الدكتور توفيق الصعيدي كلمة ملهمة عن مرض‬ ‫التصلب المتعدد وتوسعّ نطاق حالة اإلعاقة‪ .‬كما قدم ممرضان متخصصان الكثير‬ ‫من المعلومات المهمة ودراسات حول حاالت عديدة‪ ،‬للزمالء الحاضرين الحائزين على‬ ‫رخصة التدريب في مركزالتعليم الطبي المستمرالخاضع إلشراف وزارة الصحة في اإلمارات‪.‬‬ ‫يسرني أن أعلمكم أنه خالل هذا اللقاء تم إنتخابي ممرضة التصلب المتعدد للفصل‬ ‫المتدد من كانون الثاني(يناير) حتى حزيران (يونيو) ‪ 2013‬من قبل كافة الحاضرين‪.‬‬ ‫هنا أود أن أنتهز هذه الفرصة ألشكر جميع الممرضين على هذه المبادرة الطيبة‪.‬‬ ‫إن عملكم الدؤوب وإلهامكم ألعضاء الفريق جديران بالثناء‪ .‬آمل أن نتابع هذا العمل‬ ‫ونواصل التقدم في اطار رعاية مرضى التصلب المتعدد‪.‬‬ ‫شكرا لكم على تفانيكم‪.‬‬ ‫مع أطيب التحيات‪،‬‬ ‫تغريد فتافاتح‬

‫الحملة الوطنية للتعربف بمرض‬ ‫التصلب اللويحي‬ ‫‪2013‬‬ ‫هذه الحملة التي تنظمها جمعية أصدقاء مرضى التصلب‬ ‫اللويحي اإلماراتية خالل سنة ‪ 2013‬وذلك بالتعاون مع شركة نوفارتز‬ ‫لألدوية‪ ،‬والتي تهدف لزيادة الوعي بمرض التصلب اللويحي بين‬ ‫جميع شرائح المجتمع في الدولة وكذلك التثقيف الصحي لمرضى‬ ‫التصلب اللويحي وذويهم‪.‬‬

‫من فعاليات الحملة الوطنية‪:‬‬ ‫التقويم السنوي ‪:2013‬‬ ‫تم عمل تقويم سنوي لسنة ‪ 2013‬بمشاركة نخبة من مرضى‬ ‫التصلب اللويحي في اإلمارات‪ ،‬بحيث يتسنى لكل مريض أن يمثل‬ ‫شهر من أشهر السنة وذلك بكتابة فقرة يعبر فيها عن خواطره‬ ‫أو الكلمات االيجابية التي تزرع األمل في قلوب المرضى‪.‬‬ ‫لجنة ممرضي التصلب اللويحي‪:‬‬ ‫تم تشكيل لجنة في ‪ 19‬فبراير ‪ 2013‬مكونه من عدد من الممرضين‬ ‫المتخصصين في قسم األعصاب من كافة أنحاء الدولة‪ ،‬وتهدف‬ ‫اللجنة إلى إقامة ورشات عمل تدريبية كل ‪ 3‬أشهر وذلك لدعم‬ ‫التثقيف الصحي للطاقم الطبي المسؤول عن مرضى التصلب‬ ‫اللويحي‪.‬‬ ‫المنتدى الثالث لمرضى التصلب اللويحي‪:‬‬ ‫والذي تم تنظيمه في دبي ‪-12‬ابريل‪ ،2013-‬حضر المنتدى الشيخ‬ ‫محمد بن خليفة آل نهيان و أعضاء جمعية التصلب اللويحي‬ ‫و المرضى و ذويهم‪ ،‬إحتوى جدول المنتدى على محاضرات توعوية‬ ‫للمرضى كما تم عمل ورشة عمل عن العالج الطبيعي و التعايش‬ ‫مع المرض‪ ،‬وإتاحة الفرصة لعدد من المرضى للمشاركة بتجاربهم‬ ‫مع المرض‪.‬‬

‫وضعها لتوأمين بصحة جيدة‪ ،‬فيما ال تزال السيدتان اآلخريان حامالن بالشهرين السابع والثالث‬ ‫ِ‬ ‫على التوالي‪ .‬من المثير لإلهتمام أن احدى المريضات أنجبت توأمين معافين‪ ،‬وهناك مريضة‬ ‫اخرى حامل بتوأمين‪ .‬الحظنا أن التأثيرات السلبية التي ظهرت خالل المعالجة شبيهة بتلك‬ ‫التي ُوصفت خالل التجارب العيادية‪ ،‬ولكن معدل اإلنقطاع عن العالج كان أقل ضألة‪ .‬اللمفاويات‬ ‫وأنزيمات الكبد المتزيادة كانت من أكثر الشوائب المخبرية شيوعًا‪ .‬استمر ‪ 78‬مريض بتلقى‬ ‫عالج فينغلومود ( ‪.)Fingolimod‬‬ ‫أثبتت التجارب العيادية التي أدت إلى اقرار فينغلومود (‪ )Fingolimod‬فعاليته في تقليل االنتكاسات‪،‬‬ ‫العجز التقدمي‪ ،‬المسح التصويري بالرنين المغناطيسي‪ ،‬والنقص في حجم الدماغ عند المرضى‬ ‫الذين تلقوا العالج للمرة األولى أو الذين تعرضوا النتكاسات متكررة‪ .‬المزيد من التجارب العيادية‬ ‫فعال‪ ،‬جيد التحمل عند‬ ‫والبيانات التسويقية تشير بأن فينغلومود (‪ )Fingolimod‬هو عالج أمن‪ّ ،‬‬ ‫المرضى الذين تم انتقأهم ومراقبتهم بشكل مناسب‪.‬‬

‫المتعدد المتك ّرر‬ ‫عالجي مناسب للمرضى المصابين بالتصلب‬ ‫ّ‬ ‫ّ‬ ‫االنتكاس والكمون والغير قادرين على تقبل األعراض الجانبية‬ ‫لعالج اإلنترفيرون بيتا‪.‬‬ ‫كما أن مادة (الناتليذامب المضادة‪ )Natalizumab -‬تجعله خيار‬ ‫جيد خاصة للمرضى المعرضين لالصابة بفيروس ‪ ،JC‬الذين هم‬ ‫عرضة لإلصابة باعتالل بيضاء الدماغ المتعدد البؤر التقدمية‬ ‫(‪ .)PML‬ورغم أنه تم اقرار فينغلومود (‪ )Fingolimod‬في الواليات‬ ‫المتحدة كعالج بديل أولي‪ ،‬بقي توجهنا‪ ،‬في الماضي‪ ،‬إلى‬ ‫المعالجة باستخدام اإلنترفيرون بيتا مع المرضى الذين لم تسبق‬ ‫معالجتهم بمضادات الفيروسات الرجعية‪ .‬هذا يعود لقلة خبرتنا‬ ‫في استعمال فينغلومود (‪ .)Fingolimod‬ولكن مع ازدياد خبرتنا‬ ‫مع هذا العقار‪ ،‬ازداد استخدامنا لفينغلومود (‪ )Fingolimod‬كعالج‬ ‫أولي‪ .‬وخيارنا العالجي هذا مدعوم من قبل الدراسات التسويقية‬ ‫الالحقة التي أثبتت صحته وفعاليته‪.‬‬ ‫نفيدكم بخبرتنا مع فينغلومود (‪ )Fingolimod‬في قسم األمراض‬ ‫العصبية في مستشفى الرشيد في دبي‪ ،‬مركز التعليم العالي‬ ‫الدارة داء التصلب اللويحي‪ .‬ففي الفترة الممتدة من ديسمبر ‪2010‬‬ ‫حتى يوليو ‪ ، 2013‬خضع ‪ 93‬مريضًا للفحص‪ ،‬وتبين أن ‪ 85‬منهم‬ ‫مؤهل لتقبل عالج فينغلومود (‪ .)Fingolimod‬خالل الفحص تبين‬ ‫أن أحد المرضى مصاب بورم في الدماغ (‪ )Miningioma‬ومريض‬ ‫آخر مصاب ببطء القلب الجيبي (بمعدل ‪ 40‬طلقة في الدقيقة)‪،‬‬ ‫لذا منعا من تلقي عالج فينغلومود (‪ .)Fingolimod‬وهناك مريض‬ ‫ثالث مصاب بالسكري‪ -‬النمط األول فاستثني من العالج‪ ،‬ومريض‬ ‫رابع مصاب بانزيم تضخم الكبد فاستبعد أيضًا من استخدام‬ ‫فينغلومود (‪ . )Fingolimod‬وهناك خمس مرضى وجدوا قابلين‬ ‫للعالج إال أنهم اختاروا تأخير البدء به السباب شخصية‪ 81 .‬من‬

‫أصل ‪ 85‬مريضًا وصف لهم العالج في مستشفى الرشيد‪ ،‬ولكن‬ ‫ثالثة منهم أختار بدء العالج في أوروبا رغم أن الوصفة تمت في‬ ‫مستشفى الرشيد‪.‬‬ ‫ثمانية مرضى لم تسبق معالجتهم بمضادات الفيروسات‬ ‫الرجعية سابقًا‪ .‬تم شمل جميع المرضى في الدراسة‪ .‬في‬ ‫معظم الحاالت‪ ،‬استخدم فينغلومود (‪ )Fingolimod‬كعالج بديل‬ ‫في معالجة االنتكاسات المتكررة وكعالج أولي في ثمان حاالت‪.‬‬ ‫لم تحدث أي تأثيرات تثير اإلهتمام عند تلقي الجرعة األولى‪.‬‬ ‫فاألحداث السلبية عند تلقي الجرعة األولى كانت محدودة إذ‬ ‫تضمنت أعراض عابرة كبطء بطراقات القلب (مريض واحد)‪،‬‬ ‫ضيق الصدر (مريض واحد)‪ ،‬إرتفاع ضغط الدم (مريض واحد)‪.‬‬ ‫أحيلت المريضة التي تعرضت إلى مشاكل في طرقات القلب‬ ‫إلى طبيب مختص‪ ،‬عندها أمتدت فترة تلقي الجرعة إلى ثمان‬ ‫ساعات‪ ،‬لم تعطى المريضة خاللها أي عالج آخر‪ ،‬كما أعيدت‬ ‫كهرباء القلب إلى طبيعتها السابقة‪ ،‬واستعادت المريضة‬ ‫رجت المريضة من المستشفى وطلب منها‬ ‫عافيتها‪ .‬آخيرًا خُ ّ‬ ‫الحضور في اليوم التالي‪ ،‬حيث ثبت أن كهرباء القلب لديها‬ ‫طبيعية‪ .‬تبين خالل تلقي الجرعة األولى أن فينغلومود‬ ‫(‪ )Fingolimod‬دواء جيد التحمل‪ ،‬حيث بقيت العوامل السلبية‬ ‫محدودة‪ .‬تقبل فينغلومود (‪ )Fingolimod‬كان عاليا لدى المرضى‬ ‫الذين خضعوا لعالجات سابقة مقارنة مع الذين لم يخضعوا ألي‬ ‫عالجات مسبقة‪ .‬هناك بعض األعراض السلبية التي أدت إنقطاع‬ ‫بعض المرضى عن العالج منها نقص الكريات البيض ‪ /‬اللمفاويات‬ ‫(ثالثة أشخاص)‪ ،‬وذمة البقعة الصفراء (مريض واحد)‪ ،‬وانزيم‬ ‫تضخم الكبد (مريض واحد)‪ .‬ثالث سيدات توقفن عن تلقى العالج‬ ‫عندما علمن أنهن حوامل‪ .‬أحداهن عاودت العالج بعد‬

‫خبرة متزايدة مع عالج فينغلومود‬ ‫(‪ )Fingolimod‬في مستشفى الرشيد‪ ،‬دبي‬ ‫الدكتور جهاد إنشاسي‬ ‫التصلب اللويحي أو تصلب األنسجة المتعدد هو مرض التهابي‬ ‫تقدمي يصيب الجهاز العصبي المركزي (أي الدماغ والعمود‬ ‫ّ‬ ‫الفقري واألعصاب القحفية)‪ .‬يتميز هذا المرض غالبًا بالتص ّلب‬ ‫المتعدد المتك ّرر االنتكاس والكمون في البداية‪ ،‬وقد يتحول إلى‬ ‫ّ‬ ‫تقدمي ثانوي في مرحلة الحقة‪ .‬إن تلف األنسجة‬ ‫متعدد‬ ‫تص ّلب‬ ‫ّ‬ ‫ّ‬ ‫الملتهبة ناجم عن رد الفعل التلقائي للخاليا (ت) و(ب) التي يعتقد‬ ‫أنها تؤدي إلى تشكل الضرر‪ .‬فعودة الميالنين المتقلبة والنقص‬ ‫في المحور العصبي يتسببان بتراكم العجز الجسدي والدماغي‪.‬‬ ‫مؤخرًا برزت العديد من التطورات في حقل التصلب اللويحي‪.‬‬ ‫أو الً ‪ ،‬لقد بات معروفًا أن تصلب األنسجة المتعدد ال يقتصر‬ ‫فقط على فقدان الميالنين اذ يحصل التلف في المراحل األولية‬ ‫للمرض وقد يؤدي إلى ضمور الدماغ‪ .‬ثانيًا‪ ،‬لقد ثبت أن التصلب‬ ‫اللويحي ال ينحصر فقط بالمادة البيضاء للدماغ بل يطال أيضًا‬ ‫المادة الرمادية‪ .‬ثالثًا‪ ،‬إن العالج الفموي (أي عن طريق الفم) قد‬ ‫برهن فعاليته‪ .‬ولهذه الوقائع إنعكاسات عديدة‪ .‬بامكان معظم‬ ‫العالجات تقليل اإللتهاب‪ ،‬ولكن وحده فينغلومود (‪)Fingolimod‬‬ ‫قادر على تقليص الضمور الدماغي‪ .‬كافة العالجات المعدلة‬ ‫للمرض تسعى إلى منع اإلنتكاسات وتأخير اإلعاقة‪.‬‬ ‫حتى األن‪ ،‬كل الخيارات العالجية لالنتكاس التدريجي تستدعي‬ ‫الحقن‪ .‬ورغم إمتياز وسالمة العالج‪ ،‬ال زال تك ّيف المرضى مع‬ ‫أدوية العالج األولى‪ -‬مثل اإلنترفيرون بيتا‪ -‬مقيدًا بالفعالية‬ ‫المتواضعة‪ ،‬الحقن المتكرر‪ ،‬واألعراض الجانبية المزعجة‪ .‬ولكن‬ ‫فموي‪ ،‬من قبل إدارة الغذاء‬ ‫إقرار فينغلومود‪ ،‬كأول عالج معدل‬ ‫ّ‬ ‫ولد الكثير من اإلهتمام‬ ‫والدواء األمريكية (‪ )FDA‬في سبتبمر ‪ّ 2010‬‬ ‫حد سواء‪ .‬إن دور فينغلومود‬ ‫لدى المرضى واألطباء على‬ ‫ّ‬ ‫(‪ )Fingolimod‬في عالج التصلب اللويحي في تطور مستمر‪.‬‬ ‫سأتناول في هذا المقال الخبرة في العالج بواسطة فينغلومود‬ ‫(‪ ،)Fingolimod‬مضيئًا على بعض النواحي في معالجة التص ّلب‬ ‫المتعدد المتك ّرر االنتكاس والكمون‪.‬‬ ‫ّ‬ ‫في سبتمبر ‪ ،2010‬أقر عالج فينغلومود (‪)Gilenya-Fingolimod‬‬ ‫للحد من‬ ‫بمعدل ‪ 0.5‬ميليجرام للجرعة‪ ،‬في العديد من الدول‬ ‫ّ‬ ‫االنتكاسات وتأخير تراكمات العجز الجسدي لشكل االنتكاس‬ ‫في التصلب اللويحي‪ .‬لقد أقرت بعض الهيئات التنظيمية مثل‬ ‫وزارة الصحة الكندية ووكالة األدوية األوروبية العالج بواسطة‬ ‫فينغلومود (‪ )Fingolimod‬في المرحلة الثانية للمعالجة‪ ،‬بينما‬ ‫أقرته بعض الدول كالواليات المتحدة وسيوسرا كعالج أولي‪ .‬أقر‬ ‫فينغلومود (‪ )Fingolimod‬في دبي في ديسمبر ‪ 2010‬لمعالجة‬ ‫المتعدد المتك ّرر االنتكاس والكمون في المرحلتين‬ ‫التص ّلب‬ ‫ّ‬ ‫األولى والثانية‪.‬‬

‫لقد بدأت أول مريضة في الشرق األوسط بتلقي عالج فينغلومود‬ ‫(‪ )Gilenya-Fingolimod‬بديسمبر ‪ 2010‬في مستشفى الرشيدي في‬ ‫المتعدد المتك ّرر‬ ‫دبي‪ .‬للمريضة‪ ،‬وهي فتاة إمارتية‪ ،‬تاريخ مع التص ّلب‬ ‫ّ‬ ‫االنتكاس والكمون عمره ثمان سنوات‪ .‬لم تستطع هذه المريضة‬ ‫تقبل العالج األولي أي بواسطة حقن اإلنترفيرون بيتا ثالث مرات‬ ‫اسبوعيًا‪ ،‬مما جعلها عرضة لالنتكاسات التي كانت تتكرر من‬ ‫مرتين إلى ثالث مرات سنويًا‪ .‬بعض هذه االنتكاسات كان مسببًا‬ ‫للعجز‪ ،‬لذا استلزمت عالج الميثيل الوريد (‪ .)IV-MP‬عالوة على‬ ‫ذلك‪ ،‬لقد ظهرت لديها األعراض الجانبية المعروفة والناجمة عن‬ ‫استعمال هذه الحقن‪ ،‬كالهلع والكآبة واآلفات الجلدية‪ .‬وبما أن‬ ‫المريضة كثيرة التنقل والسفر‪ ،‬كان من الصعب عليها مثابرة‬ ‫العالج عن طريق الحقن‪ ،‬لذا كانت حريصة أن تبدأ عالجًا فمويًا‪.‬‬ ‫فاستهلت بتناول فينغلومود (‪ )Gilenya‬في ديسمبر ‪ .2010‬ومذ‬ ‫ذلك الحين لم تتعرض إال إلى انتكاسة واحدة طفيفة‪ ،‬وقد عاودت‬ ‫العمل‪ ،‬والسفر من حين إلى أخر‪ ،‬والعيش بطريقة طبيعية‪.‬‬ ‫لقد برهنت هذه المريضة أن فينغلومود (‪ )Fingolimod‬هو خيار‬
