We are focused on the patient’s total well-being, and that’s why we offer various amenities, such as luxurious private suites, fresh and nutritious meals, superb clinical care and aesthetic surroundings — all of which are conducive to enhancing your comfort, as well as your recovery. About Your Stay What to Expect When You Arrive The employees and management team of Salude want to make your stay comfortable and ensure you have a speedy recovery. When you arrive, you will be assisted to your room. A nurse or nursing assistant will visit you in your room within 30 minutes to aid with any immediate needs and provide a general orientation of your room and of the facility. The nursing admission assessment will be completed on your first day of admission. Your medications, based on orders received from the hospital, will be ordered from the pharmacy. If you arrive prior to 4:00 p.m., you will be evaluated by the therapy department that day. If you arrive after 4:00 p.m., your therapy evaluation will take place the next day. Other team members will also visit you within 24 hours of admission. A lunch and dinner tray will be delivered to your room during regular meal times. If you would like a snack before the meal arrives, please let our staff know. Thank you for choosing Salude for your recovery.

Physician Services At Salude, we have two primary care physician groups that will see you if your primary care physician does not make visits to our facility. One of our primary physician groups will be assigned to you and see you after you are admitted to Salude. Our CEO and medical director at Salude is a full-time physician “transitionist� with hospital experience. He helps assure that our care delivery and quality protocols are in place, and also helps our primary care physicians when they may not be in the building. Additionally, we have nurse practitioners in the facility seven days a week. Our nurse practitioners work directly with all of our physicians and are able to see our patients, diagnose their conditions, order testing, order medications and offer other appropriate treatment, all in line and similar to what our physicians are able to provide. We also have a physiatrist/ rehabilitation physician and an infectious diseases physician who serve in consultant roles and will be involved in your care if warranted by our primary care physicians. Finally, at the time of discharge (if your condition permits) you will have the option to have our nurse practitioner team perform home visits.

Social Services We will have Structured Interdisciplinary Bedside Rounds (SIBR) with you and your family representative at the beginning of your stay to discuss your plan of care with the interdisciplinary team. We aim to hold SIBR within 72 hours of admission and will discuss pertinent medical information, therapy goals and your current discharge plan. Additional meetings can be held to review progress as necessary. Please call our main number and ask for social services for assistance. Providing our patients with a safe discharge is important to us so we will discuss with you and your family the plan for your transition from the facility. You and your family will speak with a discharge planner about needed equipment, home or placement issues, upcoming appointments, medication needs and home health services. Please call our main number and ask for social services for discharge planning information.

Transportation We can arrange transportation for you should you need assistance going to a doctor’s appointment. We can schedule a wheelchair or stretcher transport. There is a cost associated with the service that is not covered under Medicare. Please call our main number and ask for your nurse for further details.

Therapy While at Salude, you will be able to coordinate your therapy time with the therapists to maximize your functional outcome. One of our therapists will consult with you to provide more information about the equipment used, what tasks will be done and how to achieve your goals. Patients that arrive after 4 p.m. on the first day will be evaluated the following day by physical therapy and occupational therapy staff. Speech therapy will provide a screening, and, when appropriate, will provide additional evaluation and treatment as necessary. Therapy hours are Monday through Friday 7 a.m. – 7 p.m. with varied hours on weekends and holidays. Physical and occupational therapy is provided seven days a week to allow you to return home as soon as possible. Speech therapy is provided five days a week and will provide evaluation and treatment as needed in the areas of swallowing impairments, voice and language (expressive an receptive communication) and cognitive therapy. Speech therapy works closely with the dietary department in setting up a list of patients who have specific needs for meals with caregivers trained to assist as needed providing appropriate verbal cues for safety with patients


that have swallowing difficulties. All treatments provided are designed following the initial evaluation to meet individualized needs. Once you have been evaluated, our talented therapy team will take your personal goals into consideration, and tailor your treatment program to progress you towards being as independent as possible. Home visits may be recommended by your therapists as you get closer to discharging to address safety issues in your home that will allow for your safe transition. A family member will need to provide the transportation. The therapist can set up a convenient time with your family, assist with your transfers into the car and then follow you in their car to your home. Once at your home, the therapist will assist with transfers out of the car and progress with the further home assessment. Home health services or outpatient services will be recommended depending on your level of progress and needs. Outpatient rehabilitation services are available at Salude and may be initiated immediately following discharge or following home health if you still need therapy. Please communicate with your therapist prior to discharge in order to complete appropriate paperwork and schedule therapy sessions so there will be no lapse in your care.

Equipment During your stay, you will be provided with all of your required equipment, such as wheelchairs, walkers, braces and more. If you require any equipment after discharge from the facility, our social worker can make the appropriate arrangements. Please note you may be responsible for these equipment charges and fees.

Laundry Services We provide laundry services at Salude for $25.00 per week. We work seven days a week from 8 a.m. – 4 p.m. If you want us to wash your personal items, we ask that you mark the clothes with your last name and we will have them washed and delivered to you within 24 hours. If your family decides to do your laundry, please bring a hamper and have your family pick up dirty clothes at least two to three times per week. If you have any questions, please call the front desk at extension 3222 to contact a laundry supervisor.

Housekeeping Services We provide housekeeping services every day from 8 a.m. – 7 p.m. We clean the rooms daily, but if you need extra cleaning or have any questions, comments or concerns, please call the front desk at extension 3222 to contact a housekeeping supervisor.

Pain Management When a patient leaves the hospital and comes to Salude, the physician at the hospital sends medication orders with the patient. Once the patient arrives at Salude with their medication orders, the orders are reviewed by a nurse, a physician and a pharmacist. This is a safety process that ensures the accuracy and appropriateness of each medication that a patient receives. Medication cannot be given to a patient until the orders have been through this safety process, so medications may not be available immediately upon the patient’s arrival to Salude. We strive to work through this process as quickly as possible, especially when the patient is in need of pain medication. Controlling the pain experienced by our patients is a high priority at Salude. It is important that patients understand how their pain medication is ordered and what they can do to help minimize their pain. Some patients’ pain medications may be ordered on a routine schedule. This makes it very easy for the pain to be well-controlled with a consistent level of pain medicine being given. This is best when a patient has frequent or constant pain. However, if this type of schedule does not provide enough pain relief, the patient needs to communicate this to the nurse or doctor so the medication can be adjusted. Also, additional treatment may need to be used, such as ice packs or ultrasound, to help the patient reach an acceptable level of comfort. Other patients may have their pain medication ordered “as needed” or “PRN,” which means it is given to the patient when the patient requests it or states they are in pain. This type of medication plan puts much more of the responsibility of controlling the pain in the hands of the patient. This type of plan is best when the patient has occasional pain. If the patient is experiencing frequent or constant pain and finds that this is not adequately controlling their pain, they need to inform the nurse or doctor so the medication can be adjusted. Regardless of how a patient’s pain medication is ordered, it is important for the patient to know that moderate to severe pain does not have to be tolerated. Allowing pain to stay at a moderate to severe level can actually slow down the recovery and rehabilitation process. Before the pain reaches that moderate or severe level, the patient should let the nurse know so pain medication can be given. If the patient’s pain level is severe, and then the patient receives pain medication, it is likely that they may not experience complete pain relief. It is much better to request pain medication when the pain is at a mild to moderate level.

Salude Navigators A staff member will be assigned to periodically check on you throughout the duration of your stay, ensuring that your concerns are addressed, and most of all, to make your experience a near perfect one. Please openly share your experience with your Salude navigator so that all of your needs can be addressed in a timely manner.


What Should You Bring? We want our patients to be as comfortable as possible while at Salude. Therefore, please feel free to bring the following items to the facility. We recommend leaving any valuables at home. It is best to label all items with your name. If you have any questions or concerns, please ask a member of our staff. • Clothing — machine washable, comfortable and can be easily worn and removed • Personal toiletries • Personal appliances, such as an electric razor, a hair straightener, etc. • Personal items, such as pictures, cards or memorabilia

What We Provide Within your room, you will find generic toiletries (hair dryer, toothpaste, mouthwash, razors, deodorant, soap, shampoo, etc.). You may, however, elect to bring personal products of your choice.

Dining Information Each day you will have the option of choosing our Chef’s Daily Special or one of our alternate menu items along with your choice from our assortment of drink options. Daily specials can be viewed on channel 56 or a written copy can be obtained from the nursing staff. Choose an entrée, side dish, beverage and dessert for lunch and dinner. For breakfast, you can choose the Chef’s Daily Special, the breakfast bistro, which is only an extra protein item, and/or the breakfast bread as well as two beverages. If you need assistance, please let your nurse know, or call extension 3227 or 3228. When finished filling out your choices, please pick which location you choose to dine: your room or our dining room on the second floor.

Grab-N-Go Café Located on the second floor next to our dining room is our Grab-N-Go Café. Here you and your visitors can conveniently buy snacks and drinks. At this time we only accept credit cards or debit cards with a credit card logo, with the exception of American Express.

Visitors and the Salude Dining Experience The dining services staff at Salude would like your family and guests to be able to enjoy our delicious meals. The menu is located on channel 56 and is also available in the front lobby. For your convenience, please fill out a Guest Tray Ticket and pay in advance at the Grab-n-Go café only. At this time we only accept credit cards or debit cards with a credit card logo, with the exception of American Express. Should you prefer something else, the Grab-N-Go Café is available for light fare.

Call Buttons

To page a nurse, please use your white call button. This cord with an attached button is found clipped to your bed. When pressed, a nurse or CNA will be notified to assist you. The red button can be used to turn on your overhead light.

Bed Adjustment To adjust your bed, use the attached white remote that says “INVACARE.” Press the appropriate arrows related to what section of the bed you would like to adjust.

Operating Your Television All patient rooms are equipped with a flat screen TV and channels provided by DirecTV. Use the gray TV remote control to turn on your TV and perform any other TV functions. When you first turn on the TV with the red power button, you will see a screen that lists “Movies, TV and Weather.” Select which category you want with the arrows and enter button located in the center of the remote. At any time you can push the yellow “Menu” button on your remote to get back to this screen. • Power on/Turn off: red power button on the top left of remote

Wellness Program At Salude, we believe that recovery is based on multiple dimensions — social, occupational, emotional, physical, intellectual, environmental and spiritual — that’s why we offer the EnerG® program created by Aegis Therapies® to support the factors that impact patient health on a daily basis. The EnerG program offers fun, interactive wellness activities and active participation classes designed to keep our patients motivated and moving while increasing their ability to be independent longer. The wellness coordinator will provide you with a current calendar of programs and discuss with you your interests and goals. Here are some brief descriptions of programs:

Wellness University Patients experience multiple educational opportunities, including guest speakers, study groups and mini courses on subjects, such as nutrition for seniors, cognitive enhancement, heart healthy choices, exercise benefits, accessing programs available in your community and many more. We encourage patients to let us know what they want to learn.

Nutrition for Seniors A series of eighteen classes, each with weekly “homework,” that can get seniors on the path to healthy eating and a healthy lifestyle. Classes are also beneficial as a one time healthy eating learning experience.

Cognitive Enhancement Classes Mounting research shows that mental stimulation and play is critical to a person’s mental health. You can participate in a group setting or individually if you prefer. The classes can be treated as a series of ten weekly or bi-weekly, and are also beneficial as a one time learning experience.


Chair Yoga Gentle, seated yoga exercises designed for seniors. The classes include posture instruction, breathing techniques, upper body asanas and stretches, followed by a brief hydration and cool down, then lower body stretches and education about relaxation and mindfulness.

Wii Wellness Interactive video sports, such as bowling, tennis and darts in a fun social setting.

Cyber CafĂŠ Located in our Multipurpose Room on the second floor, our cyber cafĂŠ offers classes on Internet and social networking, word processing programs, games and other computer-related interests. The instructor can also help you to look up anything on the internet that may interest you. Our Multipurpose Room is full of interesting things besides our computers, such as games and art supplies.
