The Saint Francis Foundation’s mission is to raise funds and provide grants that bolster Saint Francis Memorial Hospital’s ability to deliver exceptional health care services and compassionate care to its patients and communities. Since the Saint Francis Foundation began supporting care for genderdiverse patients in 2019, they have committed nearly $7.4 million. This funding enables high-quality care that ensures access for our trans and gender-diverse community. Over $5.5 million has directly funded state-of-the-art equipment, medical staff training, patient care, and the construction of a dedicated community space within the hospital. Additionally, they’ve stewarded over $965,000 from philanthropic partners to ensure crucial programmatic support, such as patient navigation services. Furthermore, the Foundation has allocated $840,000 over the past five years to support local health care partners, collaborating with organizations like the San Francisco Community Health Clinic, Lyon-Martin Health Services, and Maitri to ensure quality care regardless of financial means.

Saint Francis Memorial Hospital (SFMH) has been an active member of the LGBTQ+ community for decades. Our commitment to serving the LGBTQ+ community continues today as we provide crucial genderaffirming surgeries from expert surgeons to our transgender patients who often face incredible barriers to health care. With the intention of providing the best medical care possible, we created the Gender Institute at Saint Francis Memorial Hospital, a council of transgender health experts and collaborators who continuously assess needs and identify gaps in care.
1966
Dr. Edward Falces performs the first transgender surgery in the United States at Saint Francis Memorial Hospital in San Francisco. Dr. Lesley Hovey, Dr. James Kauth, and Dr. Doug Ousterhout followed in performing surgeries for transgendered patients.

Saint Francis Memorial Hospital hosts San Francisco’s first medical conference on transgender health—marking the public launch of its Gender Institute with transgender only surgeons.

Saint Francis Memorial Hospital nurse Sara Lentz receives a Transgender Certified Care specialization.
Saint Francis Memorial Hospital achieves SRC’s Center of Excellence in Gender Affirmation Surgery Accreditation based on the number of surgeries performed at the hospital. To date, Saint Francis remains the first hospital in the United States and internationally to receive this designation and Dr. Heidi Wittenberg, Medical Director for the Gender Institute has been recognized as a Master Surgeon by the SRC.

A Look Back on 2023
March 2023 Transgender Day of Visibility (TDOV)
The Transgender Day of Visibility event, held on Friday, March 31, was a remarkable and impactful occasion dedicated to celebrating transgender individuals while raising awareness about the discrimination they face and recognizing their valuable contributions to society. The event encouraged participants to show solidarity and proudly display their support for transgender individuals. Transgender pride flag pins were made available at employee entrances and in the cafeterias; this simple gesture not only united individuals in support but also served as a visual representation of their commitment to promoting inclusivity and diversity.

Employees had the opportunity to stop by and learn more about the event’s significance, further strengthening their understanding of the transgender community’s challenges and contributions. A key highlight of the event was the educational panel discussion on empowerment. This hybrid event took place from 12:00 to 1:00 p.m. and was accessible both on-site at Saint Francis Memorial Hospital in the Education room and through a virtual Zoom meeting by scanning a QR code. The panel featured distinguished experts including Dr. Sara Lentz, D. Hilton, Malina Keaton, Dr. Lin Frasier, and Dr. Heidi Wittenberg who shared their insights on vital topics, including gender-affirming care, and empowerment of trans and gender-diverse individuals.
The Transgender Day of Visibility event not only celebrated and empowered transgender individuals but also educated participants about the challenges and achievements within the transgender community. It was a powerful testament to the commitment of our hospital to foster inclusivity, understanding, and support for all. The event served as a beacon of hope, solidarity, and progress, reinforcing the importance of celebrating diversity and promoting equality in our society.
April 2023 “Safe Space for All” and “Be You” Marketing Campaign Launched

In 2023, a marketing campaign was launched to highlight the services offered by the Gender Institute. The campaign and its tagline, “Be You,” celebrates individuality and self-expression while challenging societal norms, aiming to foster confidence and to assist with normalizing the breaking of stereotypes.
The campaign message of Be You signifies that life is already hard. And when you’re not living life as your true self, it can seem impossible. At the Gender Institute, it’s our goal to make you feel whole, so your outside matches that inside feeling. We embrace humankindness and compassion with every interaction, so the whole world can finally meet the real you.
The campaign employed offline and online media channels within San Francisco to raise awareness. Paid digital ads and social media ads run year-round. The campaign incorporates transit shelter and print advertising during Pride Month in June to further highlight the Gender Institute in this crucial awareness period within the LGBTQ+ community.
The Gender Institute also introduced the “Safe Space for All” messaging to ensure that everyone walking through the doors of our hospital knows it is a supportive environment free from judgment for our LGBTQ+ communities.

August 2023 Pride Month Celebrations
Our participation in LGBTQ+ Pride Month was an essential aspect of our program because it exemplified our dedication to inclusivity, our unwavering support for the LGBTQ+ community, and our commitment to creating an accepting and educational work environment. It allowed us to celebrate diversity and unity, build community partnerships, and further our mission of making a positive impact in the lives of all individuals we serve.

By actively engaging in Pride Month, we underscore our dedication to fostering an inclusive environment for our entire community, encompassing all staff members, patients, and their families. Pride Month serves as a platform for educational opportunities that benefit not only our staff but also our program participants. Through initiatives such as HEI training sessions, reflective pauses, and information booths, we strive to promote awareness, understanding, and empathy.

Participating in the 20th Annual Trans March Tabeling event during the San Francisco Community Health Center’s resource fair has allowed us to cultivate meaningful partnerships and broaden our impact. These partnerships play a crucial role in expanding our reach, fostering collaboration with various organizations, and pooling resources for the greater good. Our organization is deeply rooted in the value of inclusion, which serves as a driving force in our mission. These educational experiences are invaluable for personal growth and contribute significantly to the process of community building. We are committed to creating a supportive and inclusive space that celebrates diversity and ensures the well-being of all individuals within our community.





August 2023 First Annual Gen I Symposium

The success of the first annual Gen I Symposium represents a significant milestone in addressing the pressing health care needs of individuals across diverse gender identities. In a world where the importance of gender-affirming care is increasingly acknowledged, this symposium serves as a beacon of hope. It united experts from various medical fields to share their knowledge and best practices, fostering awareness and understanding among health care providers and the community at large. The symposium’s impact resonates with the core mission of the Gender Institute, aimed at delivering compassionate, high-quality, and affordable health care services to transgender and gender-diverse individuals and their families.
The symposium also succeeded in achieving its goal of diverse attendance, with participants ranging from local hospitals, community partners, independent practices, surgeon offices, and professional organizations such as WPATH, making it a collaborative effort that stands to revolutionize gender-related health care. We want to sincerely thank Alyssa Charles, Gender Institute Business Manager; Thao Allen, Director of Business Development; and Dr. Heidi Wittenberg, Gender Institute Medical Director for hosting an extravagant and engaging event. November 2023 Transgender Day of Remembrance (TDOR)
Transgender Day of Remembrance is an annual observance held on November 20th to honor the memory of transgender and gender diverse individuals who have lost their lives as a result of anti-transgender violence. It serves as a day to raise awareness about the discrimination, violence, and hardships that transgender and gender diverse people often face and to remember those who have tragically lost their lives due to these prejudices.
The staff at Saint Francis Memorial Hospital participated by holding a candlelight vigil, a dedicated memorial, and a lunch and learn to commemorate the lives of transgender individuals who have been victims of hate crimes, bigotry, or violence. These events also aim to educate the public about the challenges faced by the transgender community and advocate for greater acceptance, understanding, and support for transgender people.

The Transgender Day of Remembrance was first established in 1999 to memorialize Rita Hester, a transgender woman who was murdered in Allston, Massachusetts, in 1998. Over the years, TDOR has grown into an international observance, drawing attention to the ongoing struggles for transgender rights and safety around the world. It serves as a poignant reminder of the work that still needs to be done to combat transphobia and promote inclusivity and equality for transgender and gender-diverse individuals.
Surgery Excellence

Surgery Leader Jaimie Weber was chosen as a member of the Association of periOperative Registered Nurses (AORN) Taskforce: Care of the Transgender Patient. This task force was created to ensure that all patients, regardless of gender identity, receive compassionate nursing care. The workgroup will create a position statement on behalf of AORN, toolkit for caregivers, as well as other clinical practice guidelines. This work serves to ensure gender inclusivity and equity for all patients receiving care in the perioperative setting. This information will be shared at the annual AORN global surgical conference and expo, March 2024. SFMH’s was also recognized in an article in AORN’s Outpatient Surgery Magazine for their innovative aqua wristbands to help avoid misgendering.
At Dignity Health Saint Francis Memorial Hospital, inclusive language is the centerpiece of a renowned gender care program that ensures every patient who walks through its doors receives the best possible care.
SFMH does this by adhering to this strategy:
• Ask for patient’s name and pronouns every time.
• Document this information in the EMR.
• Offer an aqua band to all patients that makes their name/pronouns visible to providers.
• Use the patient’s chosen name and pronoun (even if it doesn’t match the ID). The tone of this strategy is set from the start.
“We don’t use ‘preferred pronouns.’ We ask, ‘What are your pronouns?’” says Ms. Weber. “It’s no different than when there’s a Robert who likes to go by Bob. Across the board, it’s helpful to find language that’s geared toward creating an inclusive program that extends to all our patients and visitors.”

Gender Champions
The nurses on the 10th floor at our hospital have emerged as true pioneers in championing inclusivity in transgender health care. Their groundbreaking initiative to spearhead 100% pronoun documentation in the Electronic Health Record (EHR) and introduce a pronoun board is a shining example of their dedication to providing the highest standard of care. By ensuring that the diverse gender identities of patients are accurately and respectfully recorded in the EHR, they have created an environment where every individual feels seen and heard. This bold and forward-thinking action not only reflects their commitment to inclusion but also sets a remarkable standard for the entire health care community to follow, making them the true trailblazers of progress in transgender health care.

Education

Throughout the year, the Gender Institute has been steadfast in its commitment to delivering impactful educational teachings, reaching diverse communities and organizations. From collaborating with the Northern Nevada Give Out Campaign to hosting the Gen I Symposium, the Institute has engaged in a range of initiatives aimed at fostering a deeper understanding of genderrelated issues. We hosted a productive lunch and learn session with Santa Cruz providers, providing valuable insights and fostering dialogue. Furthermore, our involvement in the SFUSD Adolescent Day treatment training underscores our dedication to reaching educational institutions, ensuring that educators are equipped with the necessary knowledge to create inclusive environments. The Institute extended its influence by participating in events organized by ACNL, CASC, and TransYOUniting Pride, contributing to a broader discourse on gender equity and empowerment. Through these diverse engagements, the Gender Institute has made significant strides in promoting awareness, understanding, and action surrounding crucial gender-related topics. Furthermore, we can’t deliver world-class care without a team of educated, culturally-responsive providers, so we’ve embedded a foundational course of gender-affirming care within our New Employee Orientation. All clinical staff attend our foundational training which identifies specific barriers faced by trans and gender-diverse patients, instructs staff on how to assess and document names and pronouns (and how to respond in the event of misgendering), and offers best practices in gender-affirming care. Each new provider leaves this training with suggested scripting and a tip sheet to assist with documentation, as well as contact information so that they can continue to consult with Gender Institute staff as questions and challenges arise. Our gender care module is also included in annual competencies and regularly updated, so that our entire team is kept current and engaged in best practices.

With unwavering dedication and a deep understanding of the unique challenges faced by this community, our Gender Navigator takes a proactive approach in ensuring that every patient’s journey is marked by respect, affirmation, and exceptional care.
The Gender Navigator begins their crucial work during preoperative preparations, reaching out to patients to ensure that their preferred names and pronouns are thoughtfully and accurately documented in their medical records. This is more than just paperwork; it’s the first step in creating an environment where patients can be their authentic selves, ensuring they feel seen and respected throughout their care journey. The Gender Navigator personally welcomes patients, offering reassurance and empathy that can alleviate pre-surgery apprehension. This is a testament to our commitment to patient-centered care and is just one of the many ways we go the extra mile for those we serve.

“There are lots of things that limit you, but you also have to think how you’re limiting yourself. I decided to learn French while recovering because it was the hardest thing I could think to do. I didn’t think I could, but I am - just like my surgery. It’s very much like every other part of my transition in that I question ‘why didn’t I do this sooner?’ It’s not perfect, but my life feels so much better. I feel more and more like myself. Every step in my transition has improved my life.”
